Recent Trends In Racial Disparities And Cardiovascular Disease Burden In Patients With Polysubstance Abuse Among Hospitalizations In The United States

Abstract:BACKGROUND The global burden of cardiovascular disease has increased over the last few years. This increase maybe attributed to an increase in polysubstance-abuse in recent years. HYPOTHESIS Individuals with polysubstance abuse are at high risk for adverse cardiovascular in-hospital outcomes. METHODS We conducted our analysis on discharge data from the Nationwide Inpatient Sample (NIS) from 2016 to 2020. By using International Classification of Disease 10th edition-Clinical Modification codes, we obtained data from patients aged ≥18 years with a diagnosis of polysubstance abuse. The primary outcome of interest was in-hospital mortality. The secondary outcomes were in-hospital cardiovascular events (heart failure, myocardial infarction, acute stroke, acute pulmonary embolism, and arrhythmias). RESULTS We studied a total of 168,834 hospitalizations among patients with polysubstance abuse. The annual number of hospitalizations remained similar (31,881 - 31,981) from 2016 to 2020. The highest number of hospitalizations was seen among whites (65.96%), followed by blacks (19.42%). The commonest age groups involved were 25 to 34 years (25%). When stratified by sex, white females were hospitalized more frequently compared to white males (71.58% vs 62.50%) (p<0.0001), whereas more black males were hospitalized compared to their female counterparts (21.38% vs 16.26%) (p<0.0001). Overall, in-hospital mortality did not change significantly during the study period (18.97%-24.53%) (P=0.6629). In-hospital mortality was highest in whites (69.78%) compared to blacks (16.74 %) (see table 1). Median length of hospital stay remained the same (3 days) and average cost of hospitalization per patient (p<0.0001) increased over the study period from $22,339.00 in 2016 to $29,099.50(p<0.0001) in 2020. Major cardiovascular events among patients hospitalized for polysubstance abuse showed an increase in the annual number of events (see table 2). CONCLUSION Although the total number of admissions of patients with polysubstance use has remained similar in recent years, recent trends indicate that the healthcare costs and proportion of patients with cardiovascular events among polysubstance users are rising.
