Abstract:Objective To analyze the clinical features of infective endocarditis(IE) and help in early diagnosis and treatment.Method A retrospective analysis was performed on the clinical profiles of 51 cases comfirmed as IE from 20009.1.1 to 2010.10.31.The clinical profiles included underlying cardiac diseases,blood culture results,echocardiographic manifestations as well as clinical outcomes.Result Of the 51 patients,14(27.45%) had no underlying cardiac diseases,exceeding those(23.50%) who had previous rheumatic valvular disease.The predominat causative organism was Staphylococci(46.88%,15/32) and the second most prevalent pathogen was Streptococcus(28.12%,9/32).Echocardiographic evaluation showed a high positive rate(80.39%).21 patients underwent surgical treatments.5 cases of death occurred in the 51 cases.Conclusion The clinical characteristics of IE has greatly changed.The management of IE requires multidisciplinary cooperation.Clinicians and surgeons shall cooperate to manage the disease.

Abstract:The epidemiology, clinical presentation, and treatment of infective endocarditis (IE) has significantly changed over the past few years in developed countries. However, relevant data from developing countries are different and remain scarce. The objective of this study was to evaluate the clinical presentations, treatment and outcomes of IE patients in a tertiary hospital in East China over an 8-year period. This was a retrospective observational study of consecutive cases of definite or possible IE as per the modified Duke criteria between January 2008 and December 2015. A total of 135 definite and 39 probable IE cases were identified. The mean age was 47.8 ± 15.7 years, with a male preponderance (1.9: 1). Degenerative valve disease accounted for 30.5% cases of IE, followed by congenital heart disease (29.9%) and rheumatic heart disease (14.9%). Native cardiac valves were present in 93.7% of the IE patients. Echocardiography and blood culture were performed in all patients, of whom 55.2% were found to have large vegetations (≥10 mm) and the positive rate of blood culture was 60.3%. Streptococcus remained the chief causative agent that was identified in 61.9% of culture-positive patients. Glycopeptide antibacterials and cephalosporins were the most frequently used antimicrobial drugs for IE therapy. Seventy-six (43.7%) of the IE patients were surgically treated. The mortality rate during hospital stay was 10.9%. Our data reflected clinical and microbiological profile, and treatment of IE in a tertiary hospital located in the East China.

Abstract:Objective To investigate the clinical features,diagnostic examinations,treatment and clinical outcomes of patients with definite infective endocarditis (IE).Methods This retrospective study included 28 adults with IE who were admitted to Sir Run Run Shaw Hospital from January 1,2009 to June 30,2011.All the IE cases met the modified Duke criteria and the data were reviewed on clinical presentations,microbiological profiles,echocardiographic findings,in-hospital course and clinical outcomes.Results The mean age was (50.9± 17.4) years with a male preponderance.All patients had native valve IE.Mitral valve prolapse was the most common underlying heart disease.Fever was presented as the primary manifestation.Blood culture results were positive only in 36.4％ of the 11 cases.Streptococcus viridans represented the leading causative agent.Echocardiography was positive in 96.4％ of the cases.46.4％ of the cases underwent surgery for IE.Eighteen cases had at least one major complication.The in hospital mortality was 3.6％.Conclusions Typical clinical presentations of IE are rare.Comparing with previous studies,there are no changing trends in the microbiology.Advanced antibiotics are overused during the treatment,which is awaiting improvements.
Epidemiology of Infective Endocarditis in Rural Upstate New York, 2011 - 2016

Abstract:BACKGROUND: The epidemiology of infective endocarditis (IE) depends on a number of host factors whose prevalence can vary globally. The usual patient population affected by IE is sicker and older, often with many comorbid conditions. The risk is growing in younger populations due to the emerging epidemic of intravenous (IV) drug use. We have performed a temporal trend analysis of various factors of IE in the rural counties covering a major part of central Upstate New York.METHODS: We performed a retrospective analysis of electronic medical records of patients who were admitted in a tertiary care hospital in rural Upstate New York and diagnosed with IE from January 1, 2011 to December 31, 2016. Forty-five patients were identified with definite IE and nine with possible IE.RESULTS: Total incidence of IE was 3.5 cases per 100,000 person years in the total population and 4.4 if we consider total population ≥ 18 years in the denominator. A significant (P = 0.022) increase in incidence of IE from 2011 to 2016 was seen by univariate analysis. Incidence was higher in males (P = 0.029) and for those aged 65 or older (P = 0.0003). IV drug use among cases is noted to be more prevalent in 2015 and 2016 compared to previous years.CONCLUSION: In this study of patients in a rural region of New York, an increase in the incidence of IE was seen over the study period with changes in patient characteristics and etiology over this time. We speculate that an increase in IV drug use could be a leading factor in the recent and future increased incidence of IE in the area.

Abstract:Introduction: How the incidence of infective endocarditis (IE) changed in various demographic subgroups of the United States along with the worsening opioid epidemic over the last decade is unknown. Methods: This retrospective cohort study used data from the 2005-2018 State Inpatient Databases of Florida and New York. Cases of incident IE were combined with census data to compute age, sex- and race-specific incidence. Joinpoint regression was used to quantify the annualized percentage change (APC) in incidence over time. Results: Of 98,221 incident IE admissions, 70.0% were Non-Hispanic White (NHW) and 60.6% were ≥ 65 years old (yo). The average annual age and sex-standardized incidence of IE in cases/100,000 population was 19.2 (95%CI 18.7-19.6) but this varied by age, sex, and race. Incidence was ≈20% higher in men (20.9 [95%CI 20.2-21.7]) compared to women (17.5 [95%CI 16.8-18.2]) and increased with age in both sexes. Incidence was higher in Black: 22.5 (95%CI 21.0 - 23.9) vs NHW: 20.0 (95%CI 19.3 - 20.6), Hispanic: 13.2 (95%CI 12.3 -14.1) and Asian/Pacific Islander patients: 5.9 (95%CI 4.7 - 7.1). The age and sex-standardized incidence did not change over time (APC 0.5%, p=0.646). However, incidence increased in women 18-44 (APC 11.0%, p<0.001), men 18-44 (APC 7.3, p 2-fold in all age/sex groups over time but the pace of increase was faster in NHW women and men 18-44 yo compared to other races of similar demography (p-for-time interaction <0.001). Conclusions: Over the last decade, among residents of Florida and New York, incidence of IE increased in several demographic groups, but the most prominent rise was among young NHW patients, especially young NHW women.

Abstract:Background: Infective endocarditis (IE) is the most serious cardiovascular infection in adults. The AHA guidelines in 2007 narrowed the indications for antibiotic prophylaxis for IE. Yet, little is known about recent IE trends among older adults (age≥65 years), who are a growing at-risk population in the United States. Methods: We used the Inpatient Medicare standard analytic files to identify 100% of fee-for-service (FFS) patients aged ≥65 with a primary or secondary diagnosis of IE from 1999 to 2010. We built a denominator file from Medicare administrative data to report IE hospitalizations per 100,000 beneficiary-years. Thirty-day and 1-year mortality rates were determined using the vital status files from the Centers for Medicare & Medicaid Services. Results: Annually, over 20,000 FFS Medicare beneficiaries were hospitalized with IE. IE incidence was 84 per 100,000 patient-years in 1999, rose gradually to 103 per 100,000 patient years in 2005, and has had a slight decline thereafter (Table 1). From 1999 to 2007, 30-day and 1-year mortality rates remained high, ~15% and ~ 35% respectively. Mortality rates during 2008 were at 16% and 37% respectively; however, risk-adjusted analysis did not show a consistent rise in mortality rates from 2008 onwards. Conclusion: Although endocarditis is frequently cited as a rare condition with stable incidence, our findings signify the high burden of IE in older adults. We did not observe a major rise in IE hospitalizations or mortality rates after publication of AHA guidelines in 2007. Our observational study supports the safety of these recommendations.

Abstract:Obtaining informed consent in psychiatry clinical research involving subjects with diminished mental abilities and impaired consent capacity has been a challenge for researchers, posing many ethical concerns and procedural hurdles due to participants’ cognitive deficits and impaired ability to judge reality. Regulations seem inadequate and provide limited guidance, not sufficient to address all the ethical issues inherent in different situations related to obtaining consent from decisionally impaired persons. Researchers are struggling to find a balance between risk-benefit ratio, research advancement, and autonomy of study subjects. Inspired to improve the consent process in psychiatry clinical research, many studies have been conducted focusing on various informed consent-related ethical concerns, with the aim of developing appropriate strategies and optimizing the informed consent procedure in psychiatry clinical research, overcoming the ethical concerns. This article critically reviews the various ethical issues and consent challenges, their underlying reasons, and investigates the appropriate strategies and practices needed to be adopted while obtaining informed consent from subjects with impaired consent capacity, participating in psychiatry clinical research.
Patient characteristics, treatment patterns, and prognosis in drug-use-associated infective endocarditis in Denmark from 1999-2018

Abstract:Background While the proportion of drug-use-associated infective endocarditis (DU-IE) has been increasing during the opioid crisis in the United States, it is unknown whether this is seen in Denmark, where several preventive means have been implemented. We aimed to assess the temporal proportion of DU-IE and examine the rate of IE recurrence and mortality. Methods This nationwide cohort study identified all patients with first-time infective endocarditis in 1999-2018. Drug use was defined using ICD-8/10 codes or prescription filling of medication for opioid use disorder. Long-term mortality was examined with a Kaplan-Meier estimator and a multivariate Cox model. The recurrence of IE was examined with the Aalen-Johansen method and a multivariate cause-specific hazard model. Results We included 8,843 patients with IE: 407 with DU-IE (60.7% male, median age 43.8 years) and 8,436 with non-DU-IE (65.8% male, median age 71.5 years). The proportion of DU-IE decreased from 5.9% to 3.8% during our study period. The one-year cumulative incidence of all-cause mortality was 16.9% (CI 12.9%-20.8%) for patients with DU-IE and 17.3% (CI 16.4%-18.2%) for patients with non-DU-IE. Drug use was associated with higher one-year mortality (adjusted HR 1.64 (CI 1.23-2.21)). The one-year cumulative incidence of IE recurrence was 12.8% (CI 9.3%-16.3%) in patients with DU-IE and 4.3% (CI 3.8%-4.8%) in patients with non-DU-IE. Drug use was associated with a higher one-year recurrence of IE (adjusted HR 3.39 (CI 2.35-4.88)). Conclusion In Denmark, the proportion of patients with DU-IE fell by one-third from 1999-2018. DU-IE was associated with higher mortality and recurrence rates than non-DU-IE.

Abstract:Background Infective endocarditis continues to be a significant concern and may be undergoing an epidemiological transition. Methods Were studied 1804 consecutive episodes of infective endocarditis between 1978 and 2022. The mean age was 48 (±19), and 1162 (64%) patients were male. Temporal trends in demographic data, comorbidities, predisposing conditions, microorganisms, complications and in-hospital death have been studied over the decades (1978-1988, 1989-1999, 2000-2010 and 2011-2022). The outcomes and clinical characteristics were modeled using nonlinear cubic spline functions. Findings Valve surgery was performed in 50% of the patients and overall in-hospital mortality was 30%. From the first to the fourth decade studied, the average age of patients increased from 29 to 57 years (p<0.001), with significant declines in the occurrence of rheumatic valvular heart disease (15% to 6%; p<0.001) and streptococcal infections (46% to 33%; p<0.001). Healthcare-associated infections have increased (9% to 21%; p<0.001), as have prosthetic valve endocarditis (26% to 53%; p<0.001), coagulase-negative staphylococcal infections (4% to 11%; p<0.001), and related-complications (heart failure, embolic events, and perivalvular abscess; p<0.001). These changes were associated with a decline in adjusted in-hospital mortality from 34% to 25% (p=0.019). Interpretation In the 44 years studied, there was an increase in the mean age of patients, healthcare-related, prosthetic valve, coagulase-negative staphylococci/MRSA infections, and related complications. Notably, these epidemiological changes were associated with a decline in the adjusted in-hospital mortality.

Abstract:Several studies have demonstrated a shift in the spectrum of infective endocarditis (IE) in the developed world. We aimed to investigate whether demographic and microbiologic characteristics of IE have changed in India.A retrospective analysis of patients with in north India between 2010 to 2020.The clinical and laboratory profiles of 199 IE admitted to an academic hospital patients who met the modified Duke criteria for definite IE were analysed.The mean age was 34 years, and 84% were males. The main predisposing conditions were injection drug use (IDU) (n=71, 35.7%), congenital heart disease (n=46, 21.6%), rheumatic heart disease (n=25, 12.5%), and prosthetic device (n=19, 9.5%). 17.1% of patients developed IE without identified predispositions. Among 64.3% culturepositive cases, the most prevalent causative pathogens were Staphylococcus aureus(46.1%), viridans streptococci (7.0%), enterococci (6.0%), coagulase-negative staphylococci (5.5%), gram negative bacilli (5.5%), polymicrobial (5.5%), and Candida (1.0%). The tricuspid (30.3%), mitral (25.6%), and aortic (21.6%) valves were the most common sites of infection, and 60.3% had large vegetations (>10 mm). Systemic embolization occurred in 55.3% of patients at presentation. Cardiac surgery was required for 13.1%. In-hospital mortality was 17.1% and was associated with prosthetic devices (p-value, 0.001), baseline leucocytosis (p-value, 0.036) or acute kidney injury (p-value, 0.001), and a microbial etiology of gram negative bacilli or enterococci (p-value, 0.005).IDU is now the most important predisposition for IE in India, and S. aureus has become the leading cause of native valve endocarditis with or without IDU.
Trends in the incidence and mortality of infective endocarditis in high-income countries between 1990 and 2019

Abstract:Background: Infective endocarditis (IE) remains a life-threatening disease with high morbidity and mortality. Objectives: To describe temporal trends in IE incidence, mortality and survival over the last 30 years. Methods: Nineteen high-income countries (the 'EU 15+') were included. Age-standardised and sex-stratified incidence rates (ASIRs) and mortality rates (ASMRs) for IE were extracted from the Global Burden of Disease (GBD) database between 1990 and 2019, and mortality to incidence ratios (ASMIRs) were calculated. Trends were analysed using Joinpoint regression analysis. Results: ASIRs were higher in males than females and increased in both sexes in all countries between 1990 and 2019. A recent steep rise in ASIRs was noted in several countries including the UK, the USA and Germany. ASMRs increased for both sexes in all countries except Finland and Austria. The largest increase in ASMR was observed in females in Italy (+246%). ASMIRs were generally higher in females compared to males, with large increases in ASMIRs (indicating worsening survival) at the end of the 20th century, but more recent stabilisation or decline across the study cohort. Conclusions: While the incidence and mortality of IE have increased over the last 30 years, recent data suggest that these trends have plateaued or reversed in most countries studied. However, a recent surge in incidence in several countries (including the USA and UK) is of concern, while unfavourable outcomes in females also merit attention. More encouragingly, this analysis provides the first indication of improving IE survival at population level, supporting recent advances in diagnosis and treatment.
The Evolving Epidemiology of Infective Endocarditis: An Observational Study from Western India.

Abstract:Background: Over the last few decades, the epidemiology and the management of infective endocarditis (IE) is undergoing significant evolution and advancements respectively. Many studies from the developed world and some from India have brought this to light. Objective: To study demography, risk factors, microbiological spectrum, clinical profile, management, and outcome of IE in a tertiary care setup in western India. Materials and Methods: This is a retrospective observational study of 90 patients diagnosed with definite as well as possible IE and admitted to the intensive care units (ICU) and wards in our institute from April 2015 to May 2022. Demographics, signs and symptoms, laboratory results, echocardiography, radiological findings, treatment, and outcomes were studied. Results: 73 (81.1%) were definite and 17 (18.9%) were possible cases of IE. 93.3% cases were community-acquired, 6.7% were hospital acquired. Mean age of the patients was 43.3 + or - 18.8 years. Male: female ratio was 4.3:1. 62.2% had an underlying cardiac illness, with congenital heart disease (CHD) being the most common (18.9%) followed by rheumatic heart disease (RHD) (16.5%). Fever, weight loss, dyspnea, and cough were the common presenting symptoms. 69 (76.7 %) patients had native valve IE, 13 (14.4%) had prosthetic valve IE, 6 (6.7%) had vegetations on the endothelium without involvement of the valves, 2 (2.2%) patients had vegetations on the prosthetic material used for repair of congenital heart defects. About 48% of patients had a history of prior receipt of antibiotics. Blood cultures were positive in 48 (53.3%) patients. Most common valve infected was the mitral valve (MV) and the most common organisms isolated were the viridans group of streptococci (VGS), with penicillin (Pn) MIC <0.5 mcg/ml of all but one isolate. Transesophageal echocardiography (TEE), done in 17 patients, improved the pick-up of abnormal echo findings from 83.3% to 91.1%. Ceftriaxone was the most common antibiotic used. Surgery was done in 22 (24.4%) patients. 67 (74.4%) patients were cured, 14 (15.6%) patients died and 9 (10%) patients were lost to follow-up. Conclusion: The increasing age of patients, absence of any underlying cardiac ailment, the importance of TEE in patients with high suspicion of IE, and the trend towards early surgery were the important aspects highlighted with respect to the changes happening in the field of IE. Almost all the strains of VGS, the most common isolated organisms, retained their sensitivity to Pn and ceftriaxone, making the treatment possible with once-a-day dosing of ceftriaxone monotherapy, highlighting the utilization of outpatient antibiotic therapy (OPAT).
Infective endocarditis presentations during the COVID-19 pandemic: Have they paid an untold toll?

Abstract:Background: COVID-19 caused restrictions and re-allocation of medical resources among all healthcare services. During the peak of the pandemic, several unrelated–yet critical–conditions had silently taken their toll. Infective endocarditis (IE), owing to its non-specific clinical presentation, may have been largely mislabeled as COVID-19 in a number of cases. Results: This retrospective observational study reviewed all IE presentations at an IE unit in a university hospital during the peak of COVID-19. Patient characteristics, courses, and outcomes were compared with historical controls from our IE database published before the COVID era. We identified 30 IE cases [Group A] during the COVID-19 peak in our region (June 2021 to June 2022), with a 25% decrease compared to the usual annual rate. This is in contrast to the expected surge during the pandemic. Compared with group B (398 published IE cases from our database), group A had significantly longer symptoms-to-presentation intervals (60 [31–92] vs. 28 [14–72] days, p = 0.01). Male sex dominated both groups, but group A had significantly less pre-existing structural heart disease. Despite the more liberal use of empirical antibiotics in the COVID-era, group-A had lower rates of culture-negative IE. Compared to group B, group A demonstrated a better response to medical therapy, fewer arterial embolizations, fewer indications for surgery, and fewer overall complications, except for increased acute kidney injury. This can be explained by the abundant use of non-steroidal anti-inflammatory drugs. The data analysis strongly suggests that there might have been a natural selection or selection bias of IE patients with favorable profiles to survive the pandemic to the appropriate diagnosis. Conclusions: The diagnosis of IE and commencing the appropriate workup were significantly undermined during the COVID-19 pandemic. The inexplicable decline in IE referral rate and the favorable outcomes witnessed during the pandemic strongly suggest a referral bias and natural selection of those who survived the pandemic to the appropriate IE diagnosis.

Abstract:The epidemiology of infective endocarditis (IE) is evolving, driven by an aging population and increased use of endovascular devices, and is associated with significant morbidity and mortality. This study aims to evaluate changes in microbiological trends and clinical outcomes in patients with IE over the past decade in Switzerland. This nationwide cohort study analyzed in-hospital claims data from patients hospitalized with IE in Switzerland between 2012 and 2021. We assessed incidence rates per 100,000 hospitalizations, categorizing them by common pathogens. The outcomes included in-hospital and 6-month mortality, admission to the intensive care unit (ICU), and length of hospital stay (LOS). Among 15,255 hospitalizations with IE, the annual number of cases increased from 1,361 in 2012 to 1,636 in 2021. The most frequently diagnosed pathogens were Staphylococcus aureus (increasing from 19.8% to 30.0%, p trend <0.01) and Streptococcus species (from 17.6% to 24.4%, p trend <0.01). Infective endocarditis caused by Staphylococcus aureus was associated with the highest in-hospital (19.9%) and 6-month mortality (30.3%), and ICU admission rates (44.2%), with no relevant changes over time. The longest LOS was observed in hospitalizations with IE caused by Enterococcus species (mean 23.9 days) and Staphylococcus aureus (23.8 days). This nationwide cohort study showed an increase in IE hospitalizations from 2012 to 2021, primarily due to Staphylococcus aureus and Streptococcus species. Predominantly, Staphylococcus aureus was associated with adverse outcomes that remained consistently high over time compared to other or unidentified pathogens.

Abstract:Abstract Background Staphylococcus has replaced streptococcus as the most common cause of infective endocarditis (IE) in developed health care systems. The trend in developing countries is less clear. Aim To examine the epidemiological trends of infective endocarditis in a developing nation. Methods Single-centre, retrospective study of patients admitted with IE to a tertiary hospital in Malaysia over a 12-year period. Results The analysis included 182 patients (n = 153 Duke’s definite IE, n = 29 possible IE). The mean age was 51 years. Rheumatic heart disease was present in 42%, while 7.6% were immunocompromised. IE affected native valves in 171 (94%) cases. Health-care associated IE (HCAIE) was recorded in 68 (37.4%). IE admission rates increased from 25/100,000 admissions (2012) to 59/100,000 admissions (2017). At least one major complication on admission was detected in 59 (32.4%) patients. Left-sided IE was more common than right-sided IE [n = 159 (87.4%) vs. n = 18 (9.9%)]. Pathogens identified by blood culture were staphylococcus group [n = 58 (40.8%)], streptococcus group [n = 51 (35.9%)] and Enterococcus species [n = 13 (9.2%)]. staphylococcus infection was highest in the HCAIE group. In-hospital death occurred in 65 (35.7%) patients. In-hospital surgery was performed for 36 (19.8%) patients. At least one complication was documented in 163 (85.7%). Conclusion Staphylococcus is the new etiologic champion, reflecting the transition of the healthcare system. Streptococcus is still an important culprit organism. The incidence rate of IE appears to be increasing. The rate of patients with underlying rheumatic heart disease is still high.

Abstract:Background Understanding the epidemiology of infective endocarditis (IE) is crucial in the context of an aging population, increased comorbidities, and the heightened occurrence of healthcare-associated cases. Comprehensive data on causative agents, affected valves, and associated mortality are essential for current clinical practice. Methods This systematic review was conducted according to the PRISMA guidelines. A random effects model meta-analysis was conducted. Eligible studies reporting outcomes on IE were identified through a search of the PubMed/Medline database from 2010 to 2021. Results One hundred-thirty-three studies with 132,584 patients from six continents were included in the analysis. The most common causative agents of IE were Staphylococci species in 36% of cases, followed by Streptococci species in 26% of cases and Enterococci species in 10% of cases. Of the studies providing further speciation, the predominant species was Staphylococcus aureus with an incidence of 29%, followed by Viridans Streptococcus with an incidence of 12%. We found an in-hospital/30-day mortality of 17% (CI: 16%-19%) from 109 studies worldwide. In the subgroup analysis, the highest mortality rates were observed in Latin America 33% (CI: 28%-38%) and Africa 25% (CI: 21%-30%), while the lowest mortality rates were reported in Oceania 13% (CI: 8%-19%), followed by North America 14% (CI: 11%-18%), Asia 15% (CI: 12%-18%), and Europe 17% (CI: 16%-19%). The aortic valve was affected in 46% of cases, followed by the mitral valve in 43%. Tricuspid valve IE and multivalvular IE were identified in 7% and 14% of the cases, respectively. Conclusion Our study highlights the shift in epidemiological profile of IE over the last decade with S. aureus identified as the most common causative microorganism of IE in almost one third of cases. IE remains a highly morbid and fatal condition with high mortality rates even in tertiary and high-expertise centers worldwide. While left-sided IE is by far more common, right-sided IE and multivalvular IE are not rare entities and should be considered early in the differential of patients presenting with suspected IE to decrease the risk of future complications.Global burden of infective endocarditis.

Abstract:Abstract Background To explore the trends in epidemiology and the risk factors related to the prognosis of infective endocarditis in a tertiary hospital over the past ten years. Methods A retrospective cohort study was performed. A total of 407 consecutive patients who were admitted with infective endocarditis were included. The clinical characteristics and the risk factors related to the prognosis of infective endocarditis during this period were analyzed. Results A total of 407 patients with infective endocarditis were included, the average age was 48 ± 16 years old with an increasing trend and in-hospital mortality rate was 10.6% and one-year mortality rate was 11.3%. Among patients with underlying heart disease, congenital heart disease was the most common (25.8%), followed by rheumatic heart disease (17.0%) which showed a decreased trend during this period (P < 0.001). There were 222(54.5%) patients with positive blood cultures results and Streptococci (24.6%) was the main pathogens with an increasing trend. There were 403 patients (99%) with surgical indications, but only 235 patients (57.7%) received surgical treatment. Hemodialysis (P = 0.041, OR = 4.697, 95% CI 1.068–20.665), pulmonary hypertension (P = 0.001, OR = 5.308, 95% CI 2.034–13.852), Pitt score ≥ 4 (P < 0.001, OR = 28.594, 95% CI 5.561–148.173) and vegetation length>30 mm (P = 0.011, OR = 13.754, 95% CI 1.832–103.250) were independent risk factors for in-hospital mortality. Conclusions There were no significant changes in the overall incidence of infective endocarditis, but the clinical features of infective endocarditis had slightly changed during the past ten years. Streptococci infective endocarditis was still the predominant. Patients with hemodialysis, pulmonary hypertension, Pitt score ≥ 4 and vegetation length>30 mm had an worse in-hospital outcome.
