Abstract:‘R apid, bloody, and blinding’ are the words that Beaumont & Hollows (1972) used to classify the rare form of severe bilateral rapidly progressive proliferative diabetic retinopathy in young adults, which was later designated florid diabetic retinopathy by Kohner & Oakley (1975). Blindness was unevitable. Today, prognosis for vision in this particular group of patients is more favourable but depends entirely on early diagnosis, timely laser coagulation, and use of vitreo-retinal surgery, and yet vision can still be lost even if those measures are implemented. The purpose of this article is to describe the characteristics of florid retinopathy and to compare that condition with the much more common slowly developing type of proliferative retinopathy usually occurring in middleaged type 1 diabetic subjects. The two forms of the disease differ not only with regard to the rate of progression, but also with respect to the clinical features that are evident prior to the onset of proliferative retinopathy. It is my hope that this article can shed some light on two important issues: how to define patients who are at risk of rapid deterioration to a state of severe diabetic retinopathy that is likely to develop into florid retinopathy, and how to use our present knowledge to recognize and treat those individuals in time. It is highly possible that the current treatment paradigm will be modified in the future. Indeed, as more biochemical pathways are identified, novel therapeutic options may become available that will enable successful medical intervention at the early stages of such severe diabetic retinopathy. Comparison of the progression of florid retinopathy and slowly developing retinopathy in diabetic subjects is best achieved by considering the three representative cases described in the following. Case 1 is a 24-year-old woman who has had type 1 diabetes since she was 13 years old. She had long neglected her disease, and metabolic control was generally very poor, and during adolescence she was hospitalized for ketoacidosis several times. Also, her HbA1c values varied between 12% and 14% (reference interval 3.5–5%). As a result of substantial efforts on behalf of the diabetologist as well as the patient herself, this patient’s HbA1c levels decreased from 12.1% to 8.3% during a 3-month period. However, thereafter she stopped taking the prescribed laboratory tests for more than a year, and, when she resumed the testing 17 months after interruption, her HbA1c was 14.1%. This patient underwent annual fundus photography within our screening programme for early detection of sight-threatening diabetic retinopathy. The degree of her diabetic retinopathy had been classified as mild for a few years. However, between the last two visits for photographic screening, there had been a clear change from mild to moderate disease with haemorrhages and cotton wool spots in both eyes (Fig. 1A) and that deterioration coincided with the transiently improved metabolic control. The angiogram revealed dilated vessels (Fig. 1B) and diffuse leakage in the macular region (Fig. 1C) and in the nasal field, as well as small areas of nonperfusion (Fig. 1D). Because of the progression of the retinopathy in combination with poor metabolic control, along with the patient’s previous history of neglecting the disease, both eyes were treated with focal ⁄grid photocoagulation in the macular region followed by panretinal photocoagulation. Despite the laser treatment, there was a rapid progression to severe proliferative retinopathy and vitreoretinal traction within 6 months (Fig. 1E), which necessitated bilateral vitreo-retinal surgery. After the laser and surgical treatment (Fig. 1F), visual acuity stabilized at 0.8 in both eyes. Case 2 is a 29-year-old man who has had diabetes since he was 13 years old. He was poorly regulated over the years, and his HbA1c was around 10– 12%, apart from a shorter period when levels went from 12.3% to 6.4% in 2 months and back to 10.4% after another 3 months. Progression from mild to moderate retinopathy with microaneurysms, haemorrhages, and cotton wool spots (most pronounced in the left eye; Fig. 2A) occurred between two regular photographic screening controls performed 1 year apart, and this change coincided with the improved metabolic control. As in case 1, the angiogram revealed irregularly dilated vessels and diffuse leakage in the macular region (Fig. 2B), although the vessel abnormalities and ischemia were more pronounced in the mid-periphery (Fig. 2C). Abstract:Purpose: To present the performance of a multi-point plastic scintillation detector (mPSD) as a tool in vivo dosimetry in brachytherapy. Methods: A previously optimized three-point sensor system was used for in vivo HDR brachytherapy measurements (using the scintillators BCF-60, BCF-12, and BCF-10). The light detection system of the mPSD consisted of compactly assembled photomultiplier tubes (PMTs) and dichroic mirrors and filters to achieve a highly sensitive scintillation light collection. The PMT signals were recorded using a NI-DAQ board at a rate of 100 kHz. Dose measurements covering a range of 0.5 to 10 cm from the 192Ir source were carried out according to TG-43 U1 recommendations in order to: (1) characterize the system's response in terms of angular dependence; (2) obtain the relative contribution of positioning and measurement uncertainties to the total system uncertainty; (3) assess the system's temporal resolution; and (4) track the source position in real time. Results: The positioning uncertainty dominated close to the source, whereas the measurement uncertainty dominated at larger distances. A maximum measurement uncertainty of 17 % was observed for the BCF-60 scintillator at 10 cm from the source. The average best compromise between positioning and measurement uncertainties were reached at 17.4 mm. The detector further exhibited no angular dependence. The system provided an average location with a standard deviation under 1.7 mm. The maximum observed differences between measured and expected source location was 1.82 mm. Dose deviations remained below 5% in all the explored measurement conditions. With regard to dwell time measurement accuracy, the maximum deviation observed at all distances was 0.56 s. Conclusions: The performance of the system demonstrated that it could be used for real-time dose, position and dwell time measurements during HDR brachytherapy.

Abstract:Brachytherapy is a radiotherapy procedure performed with radioactive sources implanted into the patient's body, close to the area affected by cancer. This is a reference procedure for the treatment of prostate and gynecologic cancer due to the reduction of the dose released close to organs at risk (e.g., rectum, bladder, colon). For this reason, real-time dose verification and source localisation are essential for an optimal treatment plan. The ORIGIN collaboration aims to achieve this goal through a 16-fibre sensor system, designed to house a small volume of scintillating material in a transparent fibre tip to enable point-like measurements. The selected scintillating materials feature a decay time of about 500 μs and the signal associated with the primary γ-ray interaction results in the emission of a sequence of single photons distributed over time. Therefore, the dosimeter requires a detector with single-photon sensitivity and a system designed to provide dose measurements by photon counting. Uniformity of fibre response, system stability and reproducibility of measurements are key features of the dosimeter. The characterisation of the 16-channel dosimeter system equipped with thermo-electrically cooled Silicon Photomultipliers, carried out in the laboratory using an X-ray cabinet, is discussed and the results are compared with an earlier version equipped with SiPMs operated at room temperature.

Automated multi-parameter high-dose-rate brachytherapy quality assurance via radioluminescence imaging

Abstract:Abstract The purpose of this study is to leverage radioluminescence imaging for the development of an automated high-dose-rate (HDR) brachytherapy quality assurance (QA) system that enables simultaneous measurements of dwell position, dwell time, wire velocity, and relative source strength in a single test. The system consists of a radioluminescence phosphor sheet (a mixture of Gd 2 O 2 S:Tb and PDMS) positioned atop a HDR needle applicator, a complementary metal-oxide-semiconductor digital camera used to capture the emitted radioluminescence signals from the scintillator sheet, and an in-house graphical user interface for signal processing. The signal processing was used to extract source intensity, location, and elapsed time, yielding the final measurements on dwell position, dwell time, and wire velocity. The source strength relative to the well chamber calibration (in unit of Air-Kerma strength, S k ) is measured by establishing a calibration curve that correlates S k with the detector response. Validation experiments are performed using three customized treatment plans. With these plans, the dwell position and dwell time are verified for a range of 110.0 cm–117.5 cm and 2 s–16 s, respectively, and the linear correlation with S k is demonstrated for the source strength varying between 28 348 U (cGy cm 2 h −1 ) and 41 906 U. The wire velocity, i.e. the speed of the radioactive source averaged over the distance in between dwell positions, is calculated for various distances ranging from 5 mm to 50 mm. Results show that the mean deviations of the measured dwell position and dwell time are 0.1 mm (range from 0 to 0.2 mm) and 32.5 ms (range from 0 to 60.0 ms) with respect to the planned values, respectively, and the system response is highly linear with S k ( R 2 = 0.998). Moreover, the measured wire velocities are comparable to previously reported values. Benefitting from the compact hardware design and image processing algorithms, the system provides a practical, reliable, and comprehensive solution for HDR QA.

Abstract:PURPOSE To use quantities measurable during in vivo dosimetry to build unique channel identifiers, that enable detection of brachytherapy errors. MATERIALS AND METHODS Treatment plan of 360 patients with prostate cancer who underwent high-dose-rate brachytherapy (range, 16–25 catheters; mean, 17) were used. A single point virtual dosimeter was placed at multiple positions within the treatment geometry, and the source-dosimeter distance and dwell time were determined for each dwell position in each catheter. These values were compared across all catheters, dwell position by dwell position, simulating a treatment delivery. A catheter was considered uniquely identified if, for a given dwell position, no other catheters had the same measured values. The minimum number of dwell positions needed to identify a specific catheter and the optimal dosimeter location uniquely were determined. The radial (r) and vertical (z) dimensions of the source-dosimeter distance were also examined for their utility in discriminating catheters. RESULTS Using a virtual dosimeter with no uncertainties, all catheters were identified in 359 of the 360 cases with 9 dwell position measurements. When only the dwell time were measured, all catheters were uniquely identified after 1 dwell position. With a 2-mm spatial accuracy (r,z), all catheters were identified in 94% of the plans. Simultaneous measurement of source-dosimeter distance and dwell time ensured full catheter identification in all plans ranging from 2 to 6 dwell positions. The number of dwell positions needed to uniquely identify all catheters was lower when the distance from the implant center was higher. CONCLUSIONS The most efficient fingerprinting approach involved combining source-dosimeter distance (i.e., source tracking) and dwell time. The further the dosimeter is placed from the center of the implant the better it can uniquely identify catheters.

Characterization of a novel time-resolved, real-time scintillation dosimetry system for ultra-high dose rate radiation therapy applications

Abstract:Background: Scintillation dosimetry has promising qualities for ultra-high dose rate (UHDR) radiotherapy (RT), but no system has shown compatibility with mean dose rates ($\bar{DR}$) above 100 Gy/s and doses per pulse ($D_p$) exceeding 1.5 Gy typical of UHDR (FLASH)-RT. The aim of this study was to characterize a novel scintillator dosimetry system with the potential of accommodating UHDRs. Methods: A thorough dosimetric characterization of the system was performed on an UHDR electron beamline. The system's response as a function of dose, $\bar{DR}$, $D_p$, and the pulse dose rate ${DR}_p$ was investigated, together with the system's dose sensitivity (signal per unit dose) as a function of dose history. The capabilities of the system for time-resolved dosimetric readout were also evaluated. Results: Within a tolerance of $\pm$3% the system exhibited dose linearity and was independent of $\bar{DR}$ and $D_p$ within the tested ranges of 1.8-1341 Gy/s and 0.005-7.68 Gy, respectively. A 6% reduction in the signal per unit dose was observed as ${DR}_p$ was increased from 8.9e4-1.8e6 Gy/s. Additionally, the dose delivered per integration window of the continuously sampling photodetector had to remain between 0.028 and 11.64 Gy to preserve a stable signal response per unit dose. The system accurately measured $D_p$ of individual pulses delivered at up to 120 Hz. The day-to-day variation of the signal per unit dose at a reference setup varied by up to $\pm$13% but remained consistent (<$\pm$2%) within each day of measurements and showed no signal loss as a function of dose history. Conclusions: With daily calibrations and ${DR}_p$ specific correction factors, the system reliably provides real-time, millisecond-resolved dosimetric measurements of pulsed conventional and UHDR beams from typical electron linacs, marking an important advancement in UHDR dosimetry.

Abstract:Brachytherapy has an excellent clinical outcome for different treatment sites. However,in vivotreatment verification is not performed in the majority of hospitals due to the lack of proper monitoring systems. This study investigates the use of an imaging panel (IP) and the photons emitted by a high dose rate (HDR)192Ir source to track source motion and obtain some information related to the patient anatomy. The feasibility of this approach was studied by monitoring the treatment delivery to a 3D printed phantom that mimicks a prostate patient. A 3D printed phantom was designed with a template for needle insertion, a cavity ('rectum') to insert an ultrasound probe, and lateral cavities used to place tissue-equivalent materials. CT images were acquired to create HDR192Ir treatment plans with a range of dwell times, interdwell distances and needle arrangements. Treatment delivery was verified with an IP placed at several positions around the phantom using radiopaque markers on the outer surface to register acquired IP images with the planning CT. All dwell positions were identified using acquisition times ≤0.11 s (frame rates ≥ 9 fps). Interdwell distances and dwell positions (in relation to the IP) were verified with accuracy better than 0.1 cm. Radiopaque markers were visible in the acquired images and could be used for registration with CT images. Uncertainties for image registration (IP and planning CT) between 0.1 and 0.4 cm. The IP is sensitive to tissue-mimicking insert composition and showed phantom boundaries that could be used to improve treatment verification. The IP provided sufficient time and spatial resolution for real-time source tracking and allows for the registration of the planning CT and IP images. The results obtained in this study indicate that several treatment errors could be detected including swapped catheters, incorrect dwell times and dwell positions.
Seven-probe scintillator dosimeter for treatment verification in HDR-brachytherapy

Abstract:Purpose: We introduce a novel concept of a compact multiprobe scintillator detector and demonstrate its applicability in HDR-brachytherapy. Our fabricated seven-probe system is sufficiently narrow to be inserted in a brachytherapy needle or in a catheter. Methods: Our multiprobe detection system results from the parallel implementation of a miniaturized scintillator detector at the end of a bundle of seven fibers. The resulting system, which is narrower than 320 microns, is tested with a MicroSelectron 9.1 Ci Ir-192 HDR afterloader, in a water phantom. The detection signals from all seven probes are simultaneously read with an sCMOS camera (at a rate of 0.06 s). The camera is coupled to a chromatic filter to cancel Cerenkov signal induced within the fibers upon exposure. By implementing an aperiodic array of six scintillating cells along the bundle axis (one probe is kept bare to assess the stem effect), we first determine the range of inter-probe spacings leading to optimal source tracking accuracy. Then, three different source tracking algorithms involving sequentially or simultaneously all the scintillating probes are tested and compared. In each case, dwell positions are assessed from dose measurements and compared to the treatment plan. Dwell time is also determined and compared to the treatment plan. Results: The optimum inter-probe spacing for an accurate source tracking ranges from 15 mm to 35 mm. The optimum detection algorithm consists of adding the readout signals from all detector probes. In that case, the error to the planned dwell positions is of 0.01+/-0.14 mm and 0.02+/-0.29 mm at spacings between the source and detector axes of 5.5 and 40 mm, respectively. Using this approach, the average deviations to the expected dwell time are of -0.006+/-0.009 s and -0.008+/-0.058 s, at spacings between source and probe axes of 5.5 mm and 20 mm, respectively.

Abstract:Surface brachytherapy (BT) lacks standard quality assurance (QA) protocols. Commercially available treatment planning systems (TPSs) are based on a dose calculation formalism that assumes the patient is made of water, resulting in potential deviations between planned and delivered doses. Here, a method for treatment plan verification for skin surface BT is reported. Chips of thermoluminescent dosimeters (TLDs) were used for dose point measurements. High-dose-rate treatments were simulated and delivered through a custom-flap applicator provided with four fixed catheters to guide the Iridium-192 (Ir-192) source by way of a remote afterloading system. A flat water-equivalent phantom was used to simulate patient skin. Elekta TPS Oncentra Brachy was used for planning. TLDs were calibrated to Ir-192 through an indirect method of linear interpolation between calibration factors (CFs) measured for 250 kV X-rays, Cesium-137, and Cobalt-60. Subsequently, plans were designed and delivered to test the reproducibility of the irradiation set-up and to make comparisons between planned and delivered dose. The obtained CF for Ir-192 was (4.96 ± 0.25) μC/Gy. Deviations between measured and TPS calculated doses for multi-catheter treatment configuration ranged from −8.4% to 13.3% with an average of 0.6%. TLDs could be included in clinical practice for QA in skin BT with a customized flap applicator.

Time-gated scintillator imaging for real-time optical surface dosimetry in total skin electron therapy

Abstract:The purpose of this study was to measure surface dose by remote time-gated imaging of plastic scintillators. A novel technique for time-gated, intensified camera imaging of scintillator emission was demonstrated, and key parameters influencing the signal were analyzed, including distance, angle and thickness. A set of scintillator samples was calibrated by using thermo-luminescence detector response as reference. Examples of use in total skin electron therapy are described. The data showed excellent room light rejection (signal-to-noise ratio of scintillation SNR ≈ 470), ideal scintillation dose response linearity, and 2% dose rate error. Individual sample scintillation response varied by 7% due to sample preparation. Inverse square distance dependence correction and lens throughput error (8% per meter) correction were needed. At scintillator-to-source angle and observation angle <50°, the radiant energy fluence error was smaller than 1%. The achieved standard error of the scintillator cumulative dose measurement compared to the TLD dose was 5%. The results from this proof-of-concept study documented the first use of small scintillator targets for remote surface dosimetry in ambient room lighting. The measured dose accuracy renders our method to be comparable to thermo-luminescent detector dosimetry, with the ultimate realization of accuracy likely to be better than shown here. Once optimized, this approach to remote dosimetry may substantially reduce the time and effort required for surface dosimetry.

Technical Note: A Novel Electromagnetic-Tracked Scintillation Dosimeter for Accurate In Vivo Dosimetry in HDR Brachytherapy

Abstract:Since large dose are administrated to the patient in brachytherapy treatment, ensuring the right dose is delivered is highly critical. This study presents a first step in solving the standing issue of accurately knowing the dosimeter position at all time during in vivo dosimetry. In this work, an energy independent dosimeter, namely plastic scintillation detector, is coupled to an electromagnetic (EM) sensor having sub-mm positional accuracy for real-time tracking of the dosimeter position. However, adding an EM sensor adds materials in the path to the scintillator and thus could potentially perturb the dose <a class="link-external link-http" href="http://measurements.To" rel="external noopener nofollow">this http URL</a> confirm the perturbation presence, 4 different sensors were placed in front of the scintillator so the radiation does not arrive to it directly. Variation of the distance between the sensor and the scintillator was used to quantify the effect on the signal at 0° and 90°.To test the dependence for each sensor, the signal measurement were taken from 0° to 90° with 10$°$ increment. The 5PCBDOF sensor showed an increased signal of almost $20{\%}$ with increasing beam angle. Sensor 5DOF, 5DOFthin and 6DOFshowed no significant angle <a class="link-external link-http" href="http://dependance.The" rel="external noopener nofollow">this http URL</a> 6DOF and 5DOFthin sensor's cable revealed no extra signal attenuation. The latter gives a smaller overall attenuation. Therefore, the 5DOFthin is chosen to be part of the novel dosimeter construction. It has a jitter error of $\pm$ 0.06 mm and a reproductibility of$ {\pm}$ 0.008 mm. In the optimal operating range, the average positional uncertainty is less than 0.2 mm. Average angle errors are at most of 1.1$°$. It is feasible to integrate an EM tracking sensor to an energy independent plastic scintillation dosimeter with minimal impact to the collected signal as well as sufficient positional accuracy to keep dose uncertainty below 5${\%}$.

Abstract:Ninety-five percent of primary hyperparathyroidism is caused by either a solitary hyperfunctioning adenoma or chief cell hyperplasia. While there is a great deal of variation in the location of the parathyroid glands, 90-95% of all abnormal parathyroid tissue will be identified at initial exploration regardless of the preoperative localization procedure. Adenomas not identified at initial exploration are likely to be ectopic. Reexploration is a difficult and time-consuming procedure. The challenge, therefore, is to develop a reliable, noninvasive test that will locate aberrant parathyroid tissue preoperatively with the goal of simplifying the surgical procedure and reducing the incidence of reexploration. A radiopharmaceutical which localizes selectively in parathyroid tissue would be ideal. Currently, no single radiopharmaceutical is adequate. Recently, dual isotope subtraction scintigraphy using T1-201 chloride and Tc-99m pertechnetate has yielded promising results. The authors have investigated this procedure in 42 patients, 25 of whom to date have had surgical exploration. Of the 25 adenomas or focal hyperplasia sites found at surgery, 23 (92%) were correctly located preoperatively. There were two false-negative sites and five false-positive sites. The purpose of this paper is not only to describe the methods used and results obtained but also to discuss the merits and limitations of the technique in comparison with other imaging modalities so that its role, in today's climate of cost containment, may better be defined.
Cherenkov imaging combined with scintillation dosimetry provides real-time positional and dose monitoring for radiotherapy patients with cardiac implanted electronic devices

Abstract:Background and purpose: Cardiac implanted electronic devices (CIED) require dose monitoring during each fraction of radiotherapy, which can be time consuming and may have delayed read-out times. This study explores the potential of Cherenkov imaging combined with scintillation dosimetry as an alternative verification system. Methods and materials: Time-gated, complementary metal-oxide-semiconductor (iCMOS) cameras were used to collect video images of anthropomorphic phantoms and patients undergoing radiation treatment near chest wall cardiac devices. Scintillator discs and optically stimulated luminescence dosimeters (OSLDs) were used for dose measurement. Accuracy of spatial delivery was assessed by overlaying predicted surface dose outlines derived from the treatment planning system (TPS) with the Cherenkov images. Dose measurements from OSLDs and scintillators were compared. Results: In phantom studies, Cherenkov images visibly indicated when dose was delivered to the CIED as compared to non-overlapping dose deliveries. Comparison with dose overlays revealed congruence at the planned position and non-congruence when the phantom was shifted from the initial position. Absolute doses derived from scintillator discs aligned well with the OSLD measurements and TPS predictions for three different positions, measuring within 10 % for in-field positions and within 5 % for out-of-field positions. For two patients with CIEDs imaged over 18 fractions, Cherenkov imaging confirmed positional accuracy for all fractions, and dose measured by scintillator discs deviated by <0.015 Gy from the OSLD measurements. Conclusions: Cherenkov imaging combined with scintillation dosimetry presents an alternative methodology for CIED monitoring with the added benefit of instantly detecting deviations, enabling timely corrective actions or proper patient triage.

Abstract:Objective: High-dose-rate (HDR) brachytherapy lacks routinely available treatment verification methods. Real-time tracking of the radiation source during HDR brachytherapy can enhance treatment verification capabilities. Recent developments in source tracking allow for measurement of dwell times and source positions with high accuracy. However, more clinically relevant information, such as dose discrepancies, is still needed. To address this, a real-time dose calculation implementation was developed to provide more relevant information from source tracking data. A proof-of-principle of the developed tool was shown using source tracking data obtained from a 3D-printed anthropomorphic phantom. Approach: Software was developed to calculate dose-volume-histograms (DVH) and clinical dose metrics from experimental HDR prostate treatment source tracking data, measured in a realistic pelvic phantom. Uncertainty estimation was performed using repeat measurements to assess the inherent dose measuring uncertainty of the in vivo dosimetry (IVD) system. Using a novel approach, the measurement uncertainty can be incorporated in the dose calculation, and used for evaluation of cumulative dose and clinical dose-volume metrics after every dwell position, enabling real-time treatment verification. Main results: The dose calculated from source tracking measurements aligned with the generated uncertainty bands, validating the approach. Simulated shifts of 3mm in 5/17 needles in a single plan caused DVH deviations beyond the uncertainty bands, indicating errors occurred during treatment. Clinical dose-volume metrics could be monitored in a time-resolved approach, enabling early detection of treatment plan deviations and prediction of their impact on the final dose that will be delivered in real-time. Significance: Integrating dose calculation with source tracking enhances the clinical relevance of IVD methods. Phantom measurements show that the developed tool aids in tracking treatment progress, detecting errors in real-time and post-treatment evaluation. In addition, it could be used to define patient-specific action limits and error thresholds, while taking the uncertainty of the measurement system into consideration.

Abstract:\emph{Objective}. Optical fibre dosimeters offer great promise for real-time in vivo dose measurement in radiation-based treatment modalities such as radiotherapy and brachytherapy. This is attributed to their many useful qualities such as high spatial resolution and sensitivity. However, there are several requirements that an optical fibre dosimeter must meet to be acceptable for dose measurement in a specified treatment modality. In this work, the dosimetric performance of a novel optical fibre dosimeter for use in external beam radiotherapy is presented. \emph{Approach}. The dosimeter was characterised for photon beam energies between 6-15 MV using a Varian TrueBeam Linac at dose rates between 100-2400 MU/min and assessed based on its repeatability, dose dependence, dose rate dependence, energy dependence and dose-per-pulse dependence. \emph{Main Results}. The results demonstrated excellent short-term repeatability of 0.3 , good linearity in response (R 0.9997), and minor dose rate dependence between 0.53-2.49 for all beam qualities investigated. As the scintillator of the optical fibre dosimeter is non-water equivalent, Monte-Carlo-TOPAS simulations were used to calculate the absorbed dose energy dependence. A dose-per-pulse dependence was also investigated and compared with dosimetry measurements made with an ionisation chamber and simulated from the treatment planning system. An over-response of 20 was found at the lowest investigated dose-per-pulse, and an under-response of 34 was found at the highest investigated dose-per-pulse. This is believed to be due to an intrinsic energy dependence making this type of optical fibre dosimeter unsuitable for external beam radiotherapy dosimetry. \emph{Significance}. The optical fibre dosimeter evaluated in this work was primarily designed for high-dose-rate brachytherapy whereas this study includes the first measurements made in external beam radiotherapy and highlights the challenges of transferability of the dosimeter to a different radiation source.

Abstract:Background: Sophisticated modern radiation therapy treatments require comprehensive validation in 3D. Purpose: Investigation and characterization of a novel 3D dosimetry system consisting of ClearView radiochromic gel dosimeters (commercially available from Modus Inc) and an in-house telecentric optical CT scanner DLOS (the Duke Large Field of View Optical-CT Scanner). Methods: Spectrophotometry measurements were made on small volumes of ClearView gel irradiated with 6X photon doses up to 40 Gy to determine linearity and temporal stability of dose response. Clinical evaluation of Clearview/DLOS system was conducted in two phases. Phase one involved simple photon and electron benchmark irradiations, delivered to 15 and 10 cm diameter dosimeters, at various energies and doses. Phase 2 investigated application to the verification of two single isocenter multi-target (SIMT) stereotactic radiosurgery (SRS) deliveries. These were patient treatments for two and five brain lesions, respectively, and delivered to 15 cm diameter dosimeters. SIMT treatments were delivered by Varian TrueBeam 6X with doses of 40 Gy. For dose read-out, dosimeters were optically scanned in the DLOS both pre- and post- irradiation (within 24 h). 3D reconstructions (1 mm3 resolution) of the change in linear-optical- attenuation (proportional to dose) was obtained using in-house software and 3D Slicer. Measured and predicted (Eclipse TPS) doses were compared through percent depth-dose (PDD), cross plane and in-plane profiles, and relative 3D gamma analysis (performed at a range of 7%/4 mm down to 2%/2 mm). Regions of known artifacts were excluded from analysis (jar base, neck, and wall). The SIMT SRS deliveries were additionally compared to SciMoca, an independent Monte Carlo second check software. Results: Linearity of dose response was confirmed with R2 ≥ 0.9986 at both 520 and 630 nm wavelengths and at three post-irradiation time points: 21 h, 6 and 10 days. Dose profiles of all benchmark irradiations, in both 15 and 10 cm dosimeters, show good agreement in useable areas of the gel compared to Eclipse dose calculations, with root mean square errors (RMSE) ≤ 0.0054, and R2 ≥ 0.9808. Gamma pass rates for the 15 cm dosimeter benchmark irradiations were ≥ 94% at 2%/2 mm (central axis), ≥ 90% at 3%/3 mm (left lateral), ≥ 90% at 2%/2 mm (electron), and ≥ 94% at 3%/2 mm (stacking field). Similar high passing rates were observed for benchmark irradiations to the smaller 10 cm diameter dosimeters. Very high Gamma pass rates were found for SIMT SRS deliveries, with 99.82% and 97.80% at 3%/2 mm, for the two and five target plans, respectively. Conclusion: This work presents the first investigation of ClearView dosimeters in combination with a telecentric optical-CT scanner (DLOS). Simple benchmark irradiations demonstrate ClearView/DLOS can accurately recreate and measure relative 3D dose within non-artifact regions (i.e., > 1 cm away from walls). Application to SIMT SRS deliveries demonstrated the viability of the system as a means for comprehensive 3D verification of complex treatment deliveries as well as confirming the treatment planning system dose distribution. The results indicate that DLOS/ClearView system is a highly effective 3D verification tool for SIMT irradiations and can be applied with 3%/2 mm gamma criteria where passing rates of > 95% are to be expected.

Abstract:Objective: The aim of this work is to investigate the dose rate dependence of thermoluminescence and optically stimulated luminescence detectors (TLDs and OSLDs) in a wide uniform ultra-high dose rate electron beam and demonstrate the potential use of TLDs and OSLDs to correct the ion recombination in air-filled ionization chambers. This study avoids previously reported complications related to the field size and homogeneity. Approach: Two types of OSLDs (BeO and Al 2 O 3 :C) and three types of TLDs (LiF:Mg,Ti, LiF:Mg,Cu,P, CaF 2 :Tm) were irradiated simultaneously in a uniform 16 MeV electron beam generated by a clinically decommissioned C-Arm LINAC, modified to deliver dose per pulses between 8.3 × 10 −4 Gy and 1.255 Gy, corresponding to instantaneous dose rates between 2 × 10 2 Gy s −1 and 3 × 10 5 Gy s −1 . A prototype ultra-thin parallel plate ionization chamber was employed as reference detector. Main results: Reproducible results were achieved both at conventional (standard deviation of the data < 2%) and at the highest dose per pulse (standard deviation of the data < 4%). No trend in the dose rate response of the TLDs and OSLDs was observed in the investigated dose per pulse range. The Al 2 O 3 :C OSLD was found to be the most precise detector, with a standard deviation of the data < 2% at all investigated dose rates and dose levels. Significance: The dose rate independence of the investigated TLDs and OSLDs make them good candidates for dosimetry at ultra-high dose rates, at least up to 3 × 10 5 Gy s −1 . A dose rate independent method to measure the dose per pulse is proposed, which can be applied to characterize ultra-high dose rate electron beams and correct for ion recombination in ionization chambers.

Abstract:Source localisation and real-time dose verification are at the forefront of medical research in brachytherapy, an oncological radiotherapy procedure based on radioactive sources implanted in the patient body. The ORIGIN project aims to respond to this medical community's need by targeting the development of a multi-point dose mapping system based on fibre sensors integrating a small volume of scintillating material into the tip and interfaced with silicon photomultipliers operated in counting mode. In this paper, a novel method for the selection of the optimal silicon photomultipliers to be used is presented, as well as a laboratory characterisation based on dosimetric figures of merit. More specifically, a technique exploiting the optical cross-talk to maintain the detector linearity in high-rate conditions is demonstrated. Lastly, it is shown that the ORIGIN system complies with the TG43-U1 protocol in high and low dose rate pre-clinical trials with actual brachytherapy sources, an essential requirement for assessing the proposed system as a dosimeter and comparing the performance of the system prototype against the ORIGIN project specifications.

Abstract:Purpose: This study investigates scintillation dosimetry coupled with Cherenkov imaging for in vivo dose monitoring during whole breast radiation therapy (WBRT). Given recent observations of excess dose to the contralateral breast (CB), in vivo dosimetry (IVD) could help ensure accurate dose delivery and decrease risks of secondary cancer. This work presents a rapid, streamlined alternative to traditional IVD, providing direct visualization of measurement location relative to the treatment field on the patient. Methods and materials: Ten WBRT patients consented under an institutional review board-approved protocol were monitored with scintillation dosimetry and always-on Cherenkov imaging, on both their treated and CB for 1 to 3 fractions. Scintillator dosimeters, small plastic discs 1 mm thick and 15 mm in diameter, were calibrated against optically stimulated luminescent dosimeters (OSLDs) to generate an integral output-to-dose conversion, where integral output is measured in postprocessing through a custom fitting algorithm. The discs have been extensively characterized in a previous study for various treatment conditions including beam energy and treatment geometry. Results: A total of 44 dosimetry measurements were evaluated, including 22 treated breast and 22 CB measurements. After integral output-to-dose calibration, in vivo scintillator dosimeters exhibited high linearity (R2 = 0.99) with paired OSLD readings across all patients. The difference between scintillation and OSLD dose measurements averaged 2.8% of the prescribed dose, or an absolute dose difference of approximately 7 cGy. Conclusions: Integration of scintillation dosimetry with Cherenkov imaging offers an accurate, rapid alternative for in vivo dose verification in WBRT, circumventing the limitations of conventional point dosimeters. The additional benefit of visualizing measurement locations relative to the treatment field provides users an enhanced understanding of results and allows for detection of high dose gradients. Future work will explore the applicability of this technique across a broader range of radiation therapy treatments, aiming to streamline IVD practices.
