Abstract:In recent years, endoscopic imaging has become the major diagnostic approach for early cancer. It is difficult to accurately obtain the information of lesions and components content by using conventional white light imaging (C-WLI). Multispectral imaging techniques such as narrowband imaging are widely used in endoscopic clinical because of their specificity on the surface structure of digestive tract mucosa. However, there is still a lack of multispectral techniques for tissue components specificity. Tissue components such as lipid, hemoglobin are closely related to the generation and development of tumors, but it is difficult to observe the characteristics of lipid and hemoglobin by conventional white light imaging. Therefore, we studied the multispectral endoscopic imaging technique for the analysis of digestive tract mucosal components. Based on the reflectance spectrum characteristics of these tissue components, we determined their specific wavelength, and comprehensively considered the absorption, scattering, oxygenation and others in order to determine the most appropriate optical band in the wavelength range of 365-660nm. Through experimental verification, we choose different narrow-band wavelength combinations for lipids and hemoglobin oxygen saturation respectively to achieve multispectral imaging. In order to simulate the optical properties of digestive tract surface, we made tissue optical phantom. At the same time, we tested and optimized the imaging system and algorithm by the experiments in vitro, and obtained the optimal multispectral image of tissue components, then realize the quantitative detection combined with the content analysis algorithm. Our pilots show that the multispectral imaging system can improve the contrast of endoscopy image, enhance the detail information, achieve high precision detection of tissue components content, and control the error within 10%.

Abstract:BACKGROUND/AIMS: Endoscopic mucosal resection (EMR) has been an effective method to assess patients with esophageal nodules arising in Barrett's esophagus (BE).Optical Coherence Tomography (OCT) enables real-time, three-dimensional (3D) imaging of tissue microstructure, with resolution approaching histology and can be used to evaluate pathological features both In Vivo and within EMR specimens.In comparison, the latter is difficult to assess on confocal endoscopy which depends on In Vivo fluorescein to achieve optimal images .METHODS: A prototype OCT imaging system was developed in collaboration with the LightLab imaging Inc., providing images with an axial resolution of ~5um in tissue and ~15um along the transverse dimension.The OCT system has an axial scanning rate of ~59kHz and the system sensitivity is 103dB.The in-house developed fiberoptic imaging probe (2.5mm in diameter) can pass through the working channel of the endoscope.A 3D-OCT dataset covering an imaging area of 20 mm x 8 mm x 2 mm can be acquired in 20s.Standard EMR was performed on 12 patients using Duette (Cook Medical, Inc.).Twentyfour EMR specimens, including 9 benign, 6 BE, 2 BE with low-grade dysplasia, 1 BE with high grade dysplasia, and 6 early adenocarcinomas, were subsequently imaged with 3D-OCT and sent for standard H&E histology.Imaging features identified in the OCT images were then compared with matching histology.RESULTS: Cross-sectional and en face images could be reconstructed from the 3D-OCT dataset.Cross-sectional OCT image showed features matching with the corresponding histology.BE glands were observed at regions over the lamina propria layer while both the lamina propria layer and the muscular mucosa were preserved in 87% of OCT imaging dataset.En face OCT highlighted the distribution of the glandular structures in Barrett's esophagus and allowed quantification of BE gland density.In lesions with intramucosal and invasive adenocarcinoma, the muscular mucosa layer was less distinct or lost in the OCT images.This may be due to the destruction of tissue architecture or the thickened mucosa from the tumor.No lamina propria layer is identified in the datasets of 6 early adenocarcinomas.In addition, the shape and the distribution of the glandular structures were irregular compared to those of the BE glands.These distinctive patterns associated with adenocarcinoma were observed in both cross-sectional and en face OCT images.CONCLUSIONS: We demonstrated 3D-OCT images of EMR specimens of human esophagus ranging from benign nodules, BE with LGD, BE with HGD to adenocarcinoma.These studies comparing 3D-OCT images with histology are promising in improving In Vivo evaluation of esophageal nodules in the future.
A miniaturized, tethered, spectrally-encoded confocal endomicroscopy capsule

Abstract:Background and objective: The tethered spectrally-encoded confocal endomicroscopy (SECM) capsule is an imaging device that once swallowed by an unsedated patient can visualize cellular morphologic changes associated with gastrointestinal (GI) tract diseases in vivo. Recently, we demonstrated a tethered SECM capsule for counting esophageal eosinophils in patients with eosinophilic esophagitis (EoE) in vivo. Yet, the current tethered SECM capsule is far too long to be widely utilized for imaging pediatric patients, who constitute a major portion of the EoE patient population. In this paper, we present a new tethered SECM capsule that is 33% shorter, has an easier and repeatable fabrication process, and produces images with reduced speckle noise. Materials and methods: The smaller SECM capsule utilized a miniature condenser to increase the fiber numerical aperture and reduce the capsule length. A custom 3D-printed holder was developed to enable easy and repeatable device fabrication. A dual-clad fiber (DCF) was used to reduce speckle noise. Results: The fabricated SECM capsule (length = 20 mm; diameter = 7 mm) had a similar size and shape to a pediatric dietary supplement pill. The new capsule achieved optical sectioning thickness of 13.2 μm with a small performance variation between devices of 1.7 μm. Confocal images of human esophagus obtained in vivo showed the capability of this new device to clearly resolve microstructural epithelial details with reduced speckle noise. Conclusions: We expect that the smaller size and better image performance of this new SECM capsule will greatly facilitate the clinical adoption of this technology in pediatric patients and will enable more accurate assessment of EoE-suspected tissues. Assessment of Barrett's Esophagus and Dysplasia with Ultrahigh-Speed Volumetric En Face and Cross-Sectional Optical Coherence Tomography

Abstract:Background This study aimed to evaluate the use of ultrahigh-speed volumetric en face and cross-sectional optical coherence tomography (OCT) with micromotor catheters for the in vivo assessment of Barrett’s esophagus and dysplasia. Methods 74 OCT datasets with correlated biopsy/endoscopic mucosal resection histology (49 nondysplastic Barrett’s esophagus [NDBE], 25 neoplasia) were obtained from 14 patients with Barrett’s esophagus and a history of dysplasia and 30 with NDBE. The associations between irregular mucosal patterns on en face OCT, absence of mucosal layering, surface signal > subsurface, and > 5 atypical glands on cross-sectional OCT vs. histology and treatment history were assessed by three blinded readers. Results Atypical glands under irregular mucosal patterns occurred in 75 % of neoplasia (96 % of treatment-naïve neoplasia) vs. 30 % of NDBE datasets (43 % of short- and 18 % of long-segment NDBE). Mucosal layering was absent in 35 % of neoplasia and 50 % of NDBE datasets, and surface signal > subsurface occurred in 29 % of neoplasia and 30 % of NDBE datasets. Conclusions Atypical glands under irregular mucosal patterns are strongly associated with neoplasia, suggesting potential markers for dysplasia and a role in pathogenesis.

Abstract:Current endoscopy techniques have difficulties to provide both high resolution and large imaging depth, which significantly hinders the early diagnosis of gastric cancer. Here, we developed a label-free, large-depth, three-dimensional (3D) chromatic reflectance confocal endomicroscopy. In order to solve the problem of insufficient imaging depth of traditional chromatic confocal microscopy, a customized miniature objective lens both with large chromatic focal shift and correction for spherical aberration was used to focus light of different wavelengths at different depths of the sample simultaneously, and a fiber bundle containing 50000 single-mode cores was used to collect the confocal reflectance signal. To acquire detailed information along the axial direction at a faster speed, a high-speed multi-pixel spectrometer was used to realize simultaneous detection of multi-depth signals. Specifically, we have built up a label-free fiber-optic 3D chromatic reflectance confocal endomicroscopy, with 2.3 µm lateral resolution, imaging depth of 570 µm in 3D phantom and 220 µm in tissue, and 1.5 Hz 3D volumetric frame rate. We have demonstrated that the fiber-optic 3D chromatic confocal endomicroscopy can be used to image human gastric tissues ex vivo, and provide important morphological information for diagnosis without labeling. These results show the great potential of the fiber-optic 3D chromatic confocal endomicroscopy for gastric cancer diagnosis.

Abstract:criteria to differentiate squamous mucosa from gastric cardia and BE, a training set composed of 19 images was compiled with features distinguishing these three types of mucosa.The 10 readers reviewed the training set and subsequently interpreted images in a test set of 45 different images as squamous mucosa (n=15), gastric cardia (n=15), or BE (n=15).All readers had no formal training prior to viewing the training set.Four readers had experience with OFDI images prior to the exam.Interobserver agreement for differentiating the three tissue types and BE vs. non-BE mucosa was determined using multi-rater Fleiss's κ value.The images were later randomized and four readers repeated the test three weeks later to assess intraobserver reliability.Results: For all 10 readers, the mean time to completion of the test was 25.1±18.4minutes or 33.5±11.2seconds/image.There was excellent agreement between all 10 readers for OFDI diagnosis of BE vs. non-BE (κ= 0.811, p<0.0001) and for OFDI diagnosis of BE vs. gastric cardia vs. squamous mucosa (κ= 0.866, p<0.0001).Interobserver agreement for those with prior OFDI experience was excellent (κ =0.872, p<0.001).For those with limited OFDI exposure, interobserver variability between the six readers was also good (κ= 0.765, p<0.001).The 4 readers who repeated the test showed excellent intraobserver agreement of diagnosing BE vs. non-BE (median κ= 0.975, IQR: 0.94, 1.0).Heterogeneous backscatter and irregular surface morphology were the two features most commonly associated with the diagnosis of BE.Conclusions: In this study we have demonstrated that trained readers have a high interobserver agreement for using OFDI to discriminate BE, squamous mucosa, and gastric cardia and BE from non-BE tissues.Future studies will assess real time accuracy of OFDI during endoscopy to detect BE and grade dysplasia.

Abstract:BACKGROUND/AIMS: Three-dimensional endomicroscopy using optical coherence tomography (3D-OCT) provides cross sectional and volumetric images of tissue microstructure without requiring extrinsic contrast agents. We recently described buried Barrett's esophagus (BE) glands and ability to predict the efficacy of radio frequency ablation (RFA) using this uniquely suited imaging modality. In this study, we demonstrate a processing method that provides enhanced contrast to 3D-OCT images by analysing the variations in the intensity profile of the structural images. The processed 3D-OCT images improve the identification and classification of subsurface tissue architectures. METHODS: The study was performed at the VA Boston Healthcare System (VABHS) with the study protocol approved by the VABHS, Harvard Medical School and M.I.T. The analysis method calculates the normalized gradient of the local intensity profile of 3D-OCT images and overlays the processing result onto the original intensity image in a Hue-Saturation-Value (HSV) color space. Images acquired from upper esophagus of normal and BE patients undergoing endoscopic surveillance are analysed with the new processing method. RESULTS: 3D-OCT intensity images (Fig. 1(A, C), and the overlay images derived from the intensity analysis (Fig. 1(B, D)) from endoscopically normal-appearing (A and B) and Barrett's (C and D) esophagus demonstrate the ability of the new analysis method to enhance the contrast of the 3D-OCT images. Consistent with characteristic features of normal esophagus, intensity image in Fig. 1(A) shows squamous epithelium (SE) and lamina propria/muscularis mucosa (LP/MM) layers. These two layers are better delineated in the enhanced HSV image shown in Fig. 1(B), where the SE layer appears with the tones blue and the LP/MM layers appears with the tones of green and red. Application of this method for 3D-OCT images taken from the BE patient showed disruption of this layered architecture (Fig. 1(D)). Moreover, images taken from BE patients immediately after RFA showed enhanced contrast for the residual burned tissue and the underlying subsurface structures, showing that the enhanced contrast might also be useful in assessing RFA treatment response and efficacy. CONCLUSIONS: This study demonstrates that by analysing the variations of the local intensity profile it is possible to enhance the contrast of conventional 3D-OCT images without the use of extrinsic contrast agents. The application of this method should aid the identification of fine subsurface structures and possibly improve the diagnostic capability of the 3D-OCT. Abstract:Endoscopic mucosal ablative therapies are becoming accepted as a treatment for Barrett's esophagus (BE) with high-grade dysplasia. Radiofrequency ablation (RFA) has been shown to be effective in achieving broad and superficial BE ablation and replacement with normal-appearing squamous epithelium. Endoscopic optical coherence tomography (EOCT) is an imaging technique that uniquely allows visualization of sub-surface structures. The new three-dimensional EOCT (3D-EOCT) provides both cross-sectional and en face images of examined structures. 3D-EOCT may represent a novel method for post-ablation BE surveillance by revealing different glandular structures underneath the post-ablative surface epithelium.

Abstract:Angiogenesis is associated with the neoplastic progression of Barrett's esophagus (BE). A statistically significant increase in microvasculature density has been observed in non-dysplastic BE (NDBE), BE with low grade dysplasia (LGD) and high grade dysplasia (HGD). Narrow banding imaging (NBI) and magnification NBI have been used to detect dysplasia. However, mag NBI has limited field of view and only visualizes the surface vascular pattern. Ultrahigh speed endoscopic optical coherence tomography (OCT) technology with precision micromotor scanning probes can acquire dense volumetric data enabling depth resolved en face OCT to visualize surface mucosal patterns. En face OCT angiography (OCTA) can visualize subsurface microvasculature without requiring exogenous contrast. In this study, we investigate microvascular features in NDBE and dysplasia with OCTA.

Abstract:(1) Background: Barrett’s esophagus (BE) is a complication of chronic gastroesophageal reflux disease and is a precursor to esophageal adenocarcinoma. The clinical implication of subsurface glandular structures of Barrett’s esophagus is not well understood. Optical coherence tomography (OCT), also known as volumetric laser endomicroscopy (VLE), can assess subsurface glandular structures, which appear as subsurface hyposcattering structures (SHSs). The aim of this study is to develop a computer-aided algorithm and apply it to investigate the characteristics of SHSs in BE using clinical VLE data; (2) Methods: SHSs were identified with an initial detection followed by machine learning. Comprehensive SHS characteristics including the number, volume, depth, size and shape were quantified. Clinical VLE datasets collected from 35 patients with a history of dysplasia undergoing BE surveillance were analyzed to study the general SHS distribution and characteristics in BE. A subset of radiofrequency ablation (RFA) patient data were further analyzed to investigate the pre-RFA SHS characteristics and post-RFA treatment response; (3) Results: SHSs in the BE region were significantly shallower, more vertical, less eccentric, and more regular, as compared with squamous SHSs. SHSs in the BE region which became neosquamous epithelium after RFA were shallower than those in the regions that remained BE. Pre-ablation squamous SHSs with higher eccentricity correlated strongly with larger reduction of post-ablation BE length for less elderly patients; (4) Conclusions: The computer algorithm is potentially a valuable tool for studying the roles of SHSs in BE.
Endoscopic Histological Assessment of Colonic Polyps by Using Elastic Scattering Spectroscopy

Abstract:BACKGROUND:Elastic-scattering spectroscopy (ESS) can assess in vivo and in real-time the scattering and absorption properties of tissue related to underlying pathologies.OBJECTIVES:To evaluate the potential of ESS for differentiating neoplastic from non-neoplastic polyps during colonoscopy.DESIGN:Pilot study, retrospective data analysis.SETTING:Academic practice.PATIENTS:A total of 83 patients undergoing screening/surveillance colonoscopy.INTERVENTIONS:ESS spectra of 218 polyps (133 non-neoplastic, 85 neoplastic) were acquired during colonoscopy. Spectral data were correlated with the classification of biopsy samples by 3 GI pathologists. High-dimensional methods were used to design diagnostic algorithms.MAIN OUTCOME MEASUREMENTS:Diagnostic performance of ESS.RESULTS:Analysis of spectra from polyps of all sizes (N = 218) resulted in a sensitivity of 91.5%, specificity of 92.2%, and accuracy of 91.9% with a high-confidence rate of 90.4%. Restricting analysis to polyps smaller than 1 cm (n = 179) resulted in a sensitivity of 87.0%, specificity of 92.1%, and accuracy of 90.6% with a high-confidence rate of 89.3%. Analysis of polyps 5 mm or smaller (n = 157) resulted in a sensitivity of 86.8%, specificity of 91.2%, and accuracy of 90.1% with a high-confidence rate of 89.8%.LIMITATIONS:Sample size, retrospective validation used to obtain performance estimates.CONCLUSION:Results indicate that ESS permits accurate, real-time classification of polyps as neoplastic or non-neoplastic. ESS is a simple, low cost, clinically robust method with minimal impact on procedure flow, especially when integrated into standard endoscopic biopsy tools. Performance on polyps 5 mm or smaller indicates that ESS may, in theory, achieve Preservation and Incorporation of Valuable Endoscopic Innovations performance thresholds. ESS may one day prove to be a useful tool used in endoscopic screening and surveillance of colorectal cancer.
Novel Spectroscopically-Guided Forceps for Polyp Identification

Abstract:Purpose: To validate a new medical device designed to co-register Elastic Scattering Spectroscopy (ESS) readings with tissue biopsy for in vivo detection of tissue dysplasia during lower gastrointestinal (GI) endoscopy. Methods: Patients were enrolled from an extant pool referred for lower GI endoscopy. Fiberoptic bundles were integrated between the jaws of specialized biopsy forceps. The fiberoptic bundles were attached to a light source and spectrometer controlled by a laptop computer using custom ESS software. The integrated device was used whenever endoscopic tissue sampling was indicated according to current standards of care. The integrated device measures the elastic scattering spectra from tissue exposed to pulsed broadband white light from a xenon lamp. This modified forceps device was then able to biopsy the exact tissue measured by ESS. The tissue was then submitted for standard histopathological diagnosis as well as for an independent secondary review to confirm histology. The optical biopsies were then correlated to the physical biopsies. The spectra obtained were classified with support vector machines (SVMs) using features extracted by performing principle component analysis (PCAs). Results: Preliminary results are reported from the first 14 patients. A total of 21 biopsies of colonic polyps (13 adenoma and 8 hyperplastic) were analyzed. Signal processing thus far has yielded a sensitivity of 84.62% and specificity of 75% for adenomatous vs. hyperplastic polyps. Conclusions: The spectroscopically guided forceps are able to co-register optical and physical biopsies. Preliminary analysis is optimistic for the device's ability to accurately and reliably differentiate dysplastic from non-dysplastic polyps of the colorectum. The study is ongoing and more data is required to fully evaluate the device. Use of the device could help endoscopists target biopsies thus increasing the yield of tissue biopsy. If applied to current colon cancer screening recommendations, the device and system could decrease risks and costs of biopsy as well as save procedure time.
Targeting c-MET for Endoscopic Detection of Dysplastic Lesions Within Barrett's Esophagus Using EMI-137 Fluorescence Imaging

Abstract:Purpose: Esophageal cancer (EC) carries a poor prognosis with 5-year overall survival of less than 20%. Barrett's esophagus (BE) increases the risk of esophageal adenocarcinoma (EAC). The aim of this study was to investigate the ability of EMI-137, a mesenchymal-epithelial transition factor (c-MET)-targeting optical imaging tracer, to detect dysplasia in BE. Experimental Design: c-MET expression in human esophageal tissue was investigated using Gene Expression Omnibus (GEO) datasets, tissue microarrays and BE biopsies. EMI-137 was tested in a dual xenograft mouse model bearing OE33 (c-MET high expression) and FLO-1 (c-MET low expression) tumors. Fluorescence molecular endoscopy (FME) was performed in a mouse model of Barrett's-like metaplasia and dysplasia (L2-IL1β). Tumors and organs-of-interest were evaluated through ex vivo fluorescence imaging. Results:MET mRNA expression analyses and c-MET immunostaining confirmed upregulation of c-MET in BE and EAC compared to normal epithelium. There was strong accumulation of EMI-137 in OE33 xenografts 3 h post injection decreasing by more than 50% on co-injection of a 10-fold molar excess of unlabeled EMI-137. The target-to background ratio (TBR) at 3 h p.i. for OE33 and FLO-1 tumors was 10.08 and 1.42, respectively. FME of L2-IL1β mice showed uptake of EMI-137 in dysplastic lesions within BE with a TBR of 1.9 in vivo, and greater than 2 in ex vivo fluorescence imaging. Conclusions: EMI-137 accumulates in dysplastic lesions within BE and in c-MET positive EAC. EMI-137 imaging has potential as a screening and surveillance tool for patients with BE and as a means to detecting dysplasia and EAC.

Abstract:Optical coherence microscopy (OCM) combines confocal microscopy and optical coherence tomography (OCT) to improve imaging depth and contrast, enabling cellular imaging in human tissues. We aim to investigate OCM for ex vivo imaging of upper and lower gastrointestinal tract tissues, to establish correlations between OCM imaging and histology, and to provide a baseline for future endoscopic studies. Co-registered OCM and OCT imaging were performed on fresh surgical specimens and endoscopic biopsy specimens, and images were correlated with histology. Imaging was performed at 1.06-microm wavelength with <2-microm transverse and <4-microm axial resolution for OCM, and at 14-microm transverse and <3-microm axial resolution for OCT. Multiple sites on 75 tissue samples from 39 patients were imaged. OCM enabled cellular imaging of specimens from the upper and lower gastrointestinal tracts over a smaller field of view compared to OCT. Squamous cells and their nuclei, goblet cells in Barrett's esophagus, gastric pits and colonic crypts, and fine structures in adenocarcinomas were visualized. OCT provided complementary information through assessment of tissue architectural features over a larger field of view. OCM may provide a complementary imaging modality to standard OCT approaches for endoscopic microscopy.

Abstract:Esophageal and gastric cancers are common diseases with high morbidity and mortality; thus, early detection and treatment are beneficial to improve prognosis. Confocal laser endomicroscopy (CLE) is a novel imaging technique that permits the histological analysis of tissues during endoscopy. CLE has been shown to uniquely affect the diagnosis of early upper gastrointestinal cancers. Relevant literature was searched using PubMed and Google Scholar databases. Despite inherent flaws, CLE can reduce tissue damage and improve diagnostic accuracy to a certain extent. CLE in combination with other imaging methods can help enhance the detection rate and avoid unnecessary biopsies in the management of esophageal or gastric cancer and precancerous lesions. CLE is of great significance in the diagnosis and surveillance of early cancers of the upper gastrointestinal tract. Further technical innovations and the standardisation of CLE will make it more responsive to the needs of routine clinical applications.

New Instant Digital Pathology for EUS/EBUS Samples: The Last Advance in Bedside Diagnostics for Lung Carcinoma

Abstract:Background Ex vivo fluorescence laser scanning microscopes (FCMs) allow digital tissue imaging directly from fresh, unfixed specimens without the need for conventional histological slide preparation. To date, no data have been reported on the use of FCMs in the endoscopy suite for immediate evaluation of endoscopic ultrasound (EUS)/endobronchial ultrasound (EBUS) fine needle aspiration/biopsy (FNA-B) specimens of lung lesions and/or mediastinal lymph nodes. Objectives The aim of this study was to evaluate the performance of the FCM Vivascope 2500 (Vivascope, Munich, Germany) in providing real-time adequacy assessment and diagnostic information on the digital images of fresh unprocessed EUS/EBUS FNA-B specimens and to compare it with the corresponding final histological sections of formalin-fixed and paraffin-embedded cell blocks. Methods and Results Thirty-two patients (50% male; 71 ± 8 years old) were enrolled between May 2023 and June 2024. In 28/32 (87.5%) patients, samples were defined as adequate at Vivascope evaluation, and in 20/28 (71.4%) patients, samples were classified as malignant. At final cytohistological evaluation, 87.5% of specimens were defined as adequate and 20/28 (71.4%) were diagnosed as malignant. There was perfect agreement between the Vivascope assessment of adequacy and the final cytohistological assessment on the same specimen (k Cohen 1). From a diagnostic point of view, perfect agreement was found between the two techniques in the identification of malignant neoplasms (k Cohen 1). Conclusion The use of FCM could provide rapid information on both the adequacy and malignancy of the sample obtained during EBUS tissue acquisition (EBUS-TA), with minimal or no preparation and without destroying or losing the tissue. This technique allows for obtaining representative material in EBUS/EUS for lung cancer staging and is expected to change the turnaround time from biopsy sampling to diagnostic characterization of the tumor, ultimately improving patient management both at diagnosis and at restaging in follow up.

Abstract:Esophageal squamous cell carcinoma (ESCC) features atypical clinical manifestations and a low 5-year survival rate (< 5% in many developing countries where most of the disease occurs). Precise ESCC detection and grading toward timely and effective intervention are therefore crucial. In this study, we propose a multidimensional, slicing-free, and label-free histopathological evaluation method based on multispectral multiphoton fluorescence lifetime imaging microscopy (MM-FLIM) for precise ESCC identification. To assess the feasibility of this method, comparative imaging on fresh human biopsy specimens of different ESCC grades is performed. By constructing fluorescence spectrum- and lifetime-coded images, ESCC-induced morphological variations are unveiled. Further quantification of cell metabolism and stromal fibers reveals potential indicators for ESCC detection and grading. The specific identification of keratin pearls provides additional support for the early detection of ESCC. These findings demonstrate the viability of using MM-FLIM and the series of derived indicators for histopathological evaluation of ESCC. As there is an increasing interest in developing multiphoton endoscopes and multiphoton FLIM systems for clinical use, the proposed method would probably allow noninvasive, label-free, and multidimensional histological detection and grading of ESCC in the future.
Narrow-Band Imaging and White-Light Endoscopy with Optical Magnification in the Diagnosis of Dysplasia in Barrett'S Esophagus: Results of the Asia-Pacific Barrett'S Consortium

Abstract:Objective: The advent and utility of new endoscopic imaging modalities for predicting the histology of Barrett's esophagus (BE) in real time with high accuracy appear promising and could potentially obviate the need to perform random biopsies where guidelines are poorly adhered to. We embarked on evaluating the performance characteristics of white-light endoscopy with magnification (WLE-z), narrow-band imaging with magnification (NBI-z) and a combination of both modalities.Design: This was a prospective online study with 28 endoscopists from 11 countries (Asia-Pacific region) participating as assessors. In total, 35 patients with BE were assessed using 150 slides from WLE-z and NBI-z randomly arranged using a simple classification with corresponding histology. The overall Accuracy (Acc), Sensitivity (Sn), Specificity (Sp), Positive Predictive Value (PPV), and Negative Predictive Value (NPV) of WLE-z, NBI-z and a combination of both were calculated.Results: The overall Acc for WLE-z and NBI-z images was 87.1% and 88.7 %, respectively. When images from the two modalities were placed side by side, the Acc increased to 90.3 %. The Sn, Sp, PPV, and NPV of WLE-z were 48%, 92%, 45%, and 93% while with NBI-z, these improved to 89%, 89 %, 56%, and 98%, respectively. When both imaging modalities were viewed together, they improved further to 93%, 90%, 61%, and 99%.Conclusion: The high NPV (99 %) when both WLE-z and NBI-z were used simultaneously indicates that areas with regular appearance that are diagnosed with confidence can effectively be left alone and not biopsied when performed at a skilled resourced center. This approach could potentially lead to a paradigm shift of how patients with BE are assessed.

Abstract:Endoscopic imaging technologies such as confocal laser endomicroscopy (CLE)1 and narrowband imaging (NBI)2 have been used to investigate vascular changes as hallmarks of early cancer in the GI tract. However, the limited frame rate and field of view make CLE imaging sensitive to motion artifacts, whereas NBI has limited resolution and visualizes only the surface vascular pattern. Endoscopic optical coherence tomography (OCT) enables high speed volumetric imaging of subsurface features at near-microscopic resolution3,4, and can image microvasculature without exogenous contrast agents5 such as fluorescein, which obliterates the image in areas of bleeding, or after biopsies and resections. OCT has been demonstrated for visualizing microvasculature in small animal models6 and larger vasculature in swine3, however, the speed, resolution, and stability of previous systems were not sufficient for 3D visualization of microvasculature in endoscopic clinical applications5. Here, we present an ultrahigh speed endoscopic OCT technology that achieves >10 times faster imaging speed than commercial systems and high frame-to-frame stability, enabling OCT angiography in the human GI tract. Endoscopic OCT angiography of normal esophagus, non-dysplastic Barrett’s esophagus (BE) and normal recto-anal junction are demonstrated.
