Abstract:Objective: The advent and utility of new endoscopic imaging modalities for predicting the histology of Barrett's esophagus (BE) in real time with high accuracy appear promising and could potentially obviate the need to perform random biopsies where guidelines are poorly adhered to. We embarked on evaluating the performance characteristics of white-light endoscopy with magnification (WLE-z), narrow-band imaging with magnification (NBI-z) and a combination of both modalities.Design: This was a prospective online study with 28 endoscopists from 11 countries (Asia-Pacific region) participating as assessors. In total, 35 patients with BE were assessed using 150 slides from WLE-z and NBI-z randomly arranged using a simple classification with corresponding histology. The overall Accuracy (Acc), Sensitivity (Sn), Specificity (Sp), Positive Predictive Value (PPV), and Negative Predictive Value (NPV) of WLE-z, NBI-z and a combination of both were calculated.Results: The overall Acc for WLE-z and NBI-z images was 87.1% and 88.7 %, respectively. When images from the two modalities were placed side by side, the Acc increased to 90.3 %. The Sn, Sp, PPV, and NPV of WLE-z were 48%, 92%, 45%, and 93% while with NBI-z, these improved to 89%, 89 %, 56%, and 98%, respectively. When both imaging modalities were viewed together, they improved further to 93%, 90%, 61%, and 99%.Conclusion: The high NPV (99 %) when both WLE-z and NBI-z were used simultaneously indicates that areas with regular appearance that are diagnosed with confidence can effectively be left alone and not biopsied when performed at a skilled resourced center. This approach could potentially lead to a paradigm shift of how patients with BE are assessed.
Incremental yield of dysplasia detection in Barrett's esophagus using volumetric laser endomicroscopy with and without laser marking compared with a standardized random biopsy protocol

Abstract:Background and aims: Volumetric laser endomicroscopy (VLE) is a new wide-field advanced imaging technology for Barrett's esophagus (BE). No data exist on incremental yield of dysplasia detection. Our aim is to report the incremental yield of dysplasia detection in BE using VLE. Methods: This is a retrospective study from a prospectively maintained database from 2011 to 2017 comparing the dysplasia yield of 4 different surveillance strategies in an academic BE tertiary care referral center. The groups were (1) random biopsies (RB), (2) Seattle protocol random biopsies (SP), (3) VLE without laser marking (VLE), and (4) VLE with laser marking (VLEL). Results: A total of 448 consecutive patients (79 RB, 95 SP, 168 VLE, and 106 VLEL) met the inclusion criteria. After adjusting for visible lesions, the total dysplasia yield was 5.7%, 19.6%, 24.8%, and 33.7%, respectively. When compared with just the SP group, the VLEL group had statistically higher rates of overall dysplasia yield (19.6% vs 33.7%, P = .03; odds ratio, 2.1, P = .03). Both the VLEL and VLE groups had statistically significant differences in neoplasia (high-grade dysplasia and intramucosal cancer) detection compared with the SP group (14% vs 1%, P = .001 and 11% vs 1%, P = .003). Conclusion: A surveillance strategy involving VLEL led to a statistically significant higher yield of dysplasia and neoplasia detection compared with a standard random biopsy protocol. These results support the use of VLEL for surveillance in BE in academic centers.

Abstract:Background and Aims Endoscopic Barrett's esophagus (BE) and esophageal adenocarcinoma (EAC) detection is invasive and expensive. Non-endoscopic BE/EAC detection tools are guideline-endorsed alternatives. We previously described a 5-methylated DNA marker (MDM) panel assayed on encapsulated sponge cell collection device (CCD) specimens. We aimed to train a new algorithm using a 3-MDM panel and test its performance in an independent cohort. Methods Algorithm training and test samples were from two prospective multicenter cohorts. Participants: All BE cases had esophageal intestinal metaplasia (with or without dysplasia/EAC); controls had no endoscopic evidence of BE. The CCD procedure was followed by endoscopy. From CCD cell lysates, DNA was extracted, bisulfite treated, and MDMs were blindly assayed. The algorithm was set and locked using cross-validated logistic regression (training set) and its performance was assessed in an independent test set. Results Training (N=352) and test (N=125) set clinical characteristics were comparable. The final panel included 3 MDMs (NDRG4, VAV3, ZNF682). Overall sensitivity was 82% (95% CI 68-94%) at 90% (79-98%) specificity and 88% (78-94%) sensitivity at 84% (70-93%) specificity in training and test sets, respectively. Sensitivity was 90% and 68% for all long and short-segment BE respectively. Sensitivity for BE with high grade dysplasia and EAC was 100% in training and test sets. Overall sensitivity for nondysplastic BE was 82%. Areas under the receiver operating characteristic curves for BE detection were 0.92 and 0.94 in the training and test sets, respectively. Conclusions A locked 3-MDM panel algorithm for BE/EAC detection using a non-endoscopic CCD demonstrated excellent sensitivity for high risk BE cases in independent validation samples. Abstract:Abstract Background Confocal laser endomicroscopy (CLE) has advantages in detecting gastric neoplastic lesions, meanwhile it requires strict patient cooperation. Sedation could improve patient cooperation and quality of endoscopy. However, sedation is still not very popular in some resource-limited countries and regions. The purpose of this study was to compare propofol-based sedated versus un-sedated CLE in the value of diagnosing early gastric cancer (EGC) and precancerous lesions. Methods A retrospective, cohort, single center study of 226 patients who underwent CLE between January 1, 2015 and December 31, 2017 was performed. Patients enrolled were allocated into the propofol-based sedated group (n = 126) and the un-sedated group (n = 100). The comparison of validity and reliability of CLE for identifying EGC and precancerous lesions between the two groups was performed through analyzing CLE diagnosis and pathological diagnosis. Reporting followed the STROBE guidelines. Results The area under receiver operating characteristic curve (AUROC) of diagnosing EGC in the sedated group was 0.97 (95 % CI: 0.95 to 0.99), which was higher than that in the un-sedated group (0.88 (95 % CI: 0.80 to 0.97), P = 0.0407). CLE with sedation performed better than without sedation in diagnosing intraepithelial neoplasia and intestinal metaplasia (P = 0.0008 and P = 0.0001, respectively). For patients considered as high-grade intraepithelial neoplasia or EGC by endoscopists, they would not get biopsy during CLE but receive endoscopic submucosal dissection (ESD) subsequently, and the misdiagnosis rate of CLE was 0 % in the sedated group and 27.59 % (95 % CI: 10.30–44.91 %) in the un-sedated group (P = 0.006). Conclusions Propofol based sedation was associated with improved diagnostic value of CLE for detecting EGC as well as precancerous lesions (intraepithelial neoplasia OR intestinal metaplasia).
Efficacy of probe-based confocal laser endomicroscopy for surveillance in ulcerative colitis endomicroscopy for ulcerative colitis surveillance

Abstract:We read with interest the study by van den Broek and colleagues published in the recent issue of Endoscopy 1. The authors examined whether the use of probe-based confocal laser endomicroscopy (pCLE) may increase the efficiency of surveillance of patients with ulcerative colitis. In the study, 48 visible lesions and 87 random areas in 22 patients with ulcerative colitis were evaluated by real-time narrow-band imaging (NBI) and high-definition endoscopy (HDE). Fluorescein-enhanced pCLE was performed prior to biopsy. The outcome measures were the feasibility and the diagnostic accuracy of pCLE. They showed that the sensitivity, specificity, and accuracy of pCLE were 65 %, 82 %, and 81 %, whereas these figures were 100 %, 89%, and 92 % for real-time endoscopic diagnosis with NBI and HDE. From these results, the authors concluded that pCLE for ulcerative colitis surveillance is feasible and has reasonable diagnostic accuracy. We agree with the authors that taking random biopsies is time-consuming and of doubtful efficiency. We have previously shown that pit pattern diagnosis might be useful in improving the efficiency of surveillance for ulcerative colitis 2. Considering that pCLE is, in fact, in vivo histology and has shown high agreement with true histopathology, it may be possible that pCLE substitutes random biopsies and obviates targeted biopsies. However, in the van den Broek study, there are major issues that need to be discussed before their conclusion can be drawn. The first point concerns the management of lesions that are indefinite for dysplasia. Van den Broek used the Vienna criteria for histopathological classification 3. The Vienna criteria were established to develop common worldwide terminology for gastrointestinal epithelial neoplasia, including esophageal, gastric, and colorectal lesions. Neoplastic lesions are categorized as Category 3, Category 4 or Category 5. The van den Broek study used these categories to classify biopsied lesions. This classification is used for neoplastic lesions in general; however, it is well known that histopathological classification of specimens in ulcerative colitis needs special attention because histopathological diagnosis is sometimes very difficult due to the inflammatory changes. This makes it difficult to make a distinction between low-grade dysplasia and non-neoplastic lesions with inflammation. This is why Riddell et al. divided the classification of “indefinite for dysplasia” into three different categories – probably negative, unknown, and probably positive, each of which needs a different follow-up strategy 4. Riddell reported that patients with “unknown” or “probably positive for dysplasia” need short-interval follow-up 4. Therefore, it is important not to miss lesions with “indefinite for dysplasia (unknown or probably positive)” during surveillance. As the histological distinction between low-grade dysplasia and indefinite for dysplasia is sometimes difficult, it seems that pCLE can hardly distinguish between these two lesion types. However, in the van den Broek study, lesions that were “indefinite for dysplasia” were excluded because the study focused only on neoplastic lesions. It would be of interest to determine whether or not pCLE can identify lesions classified as “indefinite for dysplasia.” The next point is the outcome measure of feasibility. Van den Broek et al. expressed the feasibility of pCLE as pCLE imaging time required, percentage of imaging time with clear pCLE histology, and pCLE video quality as rated by two endoscopists. However, the pCLE imaging time required may not reflect the total time of examination directly and we believe that the most important issue is the total time needed to complete the surveillance colonoscopy in each individual. To compare the efficacy of surveillance between target biopsy and random biopsy, a nationwide randomized controlled study is ongoing in Japan 5. In this trial, the total time of the examination is included as the outcome measure. We believe this prospective trial will clarify the difference between target biopsy and random biopsy in terms of total time needed for the examination. We believe that the van den Broek study, which was undertaken by a leading group in the world in this field, has shown very important findings. However, to evaluate the efficacy of pCLE, we believe that the above issues need to be discussed further.

Abstract:Case report A 40-year-old male transplant recipient with six siblings comes from a remarkable family with Alport's syndrome. The first brother is unaffected, his sister is deaf and has had five eye operations, and he and his other four brothers are all deaf and developed end-stage renal failure in their early 20s. At the age of 14 he found it increasingly difficult to see and field a cricket ball, and then noticed he was getting progressively short-sighted in bright light. He was first seen by an ophthalmologist at the age of 21, a year after he started haemodialysis. He was initially thought to have keratoconus (a degenerative condition leading to conical distortion of the cornea), but sub-sequently slit-lamp examination confirmed that he had anterior lenticonus. At the age of 26 he had a cadaver transplant. His vision could not be corrected satisfact-orily with spectacles and he developed cataracts during steroid treatment. Three years after his transplant he underwent bilateral cataract surgery with lens implantation, which restored his vision.

Abstract:Esophageal squamous cell carcinoma (ESCC) is one of the most common histological types of esophageal cancers. It can seriously affect public health, particularly in Eastern Asia. Early diagnosis and effective therapy of ESCC can significantly help improve patient prognoses. The visualization of intrapapillary capillary loops (IPCLs) under magnification endoscopy (ME) can greatly support the identification of ESCC occurrences by endoscopists. This paper proposes an artificial-intelligence-assisted endoscopic diagnosis approach using deep learning for localizing and identifying IPCLs to diagnose early-stage ESCC. An improved Faster region-based convolutional network (R-CNN) with a polarized self-attention (PSA)-HRNetV2p backbone was employed to automatically detect IPCLs in ME images. In our study, 2887 ME with blue laser imaging (ME-BLI) images of 246 patients and 493 ME with narrow-band imaging (ME-NBI) images of 81 patients were collected from multiple hospitals and used to train and test our detection model. The ME-NBI images were used as the external testing set to verify the generalizability of the model. The experimental evaluation revealed that the proposed method achieved a recall of 79.25%, precision of 75.54%, F1-score of 0.764 and mean average precision (mAP) of 74.95%. Our method outperformed other existing approaches in our evaluation. It can effectively improve the accuracy of ESCC detection and provide a useful adjunct to the assessment of early-stage ESCC for endoscopists.
New method for real-time visualization and quantitative characterization of early colorectal cancer in endoscopy: a pilot study

Abstract:Background and study aims Endoscopic optical diagnosis is crucial to the therapeutic strategy for early gastrointestinal cancer. It accurately (> 85 %) predicts pT category based on microsurface (SP) and vascular patterns (VP). However, interobserver variability is a major problem. We have visualized and digitalized the graded irregularities based on bioinformatically enhanced quantitative endoscopic image analysis (BEE) of high-definition white-light images. Methods In a pilot study of 26 large colorectal lesions (LCLs, mean diameter 39 mm), we retrospectively compared BEE variables with corresponding histopathology of the resected LCLs. Results We included 10 adenomas with low-grade intraepithelial neoplasia (LGIN), nine with high-grade intraepithelial neoplasia (HGIN) and early adenocarcinoma (EAC), and seven deeply submucosal invasive carcinomas. Quantified density (d) and nonuniformity (C U ) of vascular and surface structures correlated with histology (r s d VP: –0.77, r s C U VP: 0.13, r s d SP: –0.76, and r s C U SP: 0.45, respectively). A computed BEE score showed a sensitivity and specificity of 90 % and 100 % in the group with LGINs, 89 % and 41 % in the group with HGINs and EACs, and 100 % and 95 % in the group with deeply invasive carcinoma, respectively. Conclusions In this pilot study, BEE showed promise as a tool for endoscopic characterization of LCLs during routine endoscopy. Prospective clinical studies are needed. Abstract:Volumetric laser endomicroscopy (VLE) uses optical coherence tomography (OCT) for real-time, microscopic cross-sectional imaging. A US-based multi-center registry was constructed to prospectively collect data on patients undergoing upper endoscopy during which a VLE scan was performed. The objective of this registry was to determine usage patterns of VLE in clinical practice and to estimate quantitative and qualitative performance metrics as they are applied to Barrett's esophagus (BE) management. All procedures utilized the NvisionVLE Imaging System (NinePoint Medical, Bedford, MA) which was used by investigators to identify the tissue types present, along with focal areas of concern. Following the VLE procedure, investigators were asked to answer six key questions regarding how VLE impacted each case. Statistical analyses including neoplasia diagnostic yield improvement using VLE was performed. One thousand patients were enrolled across 18 US trial sites from August 2014 through April 2016. In patients with previously diagnosed or suspected BE (894/1000), investigators used VLE and identified areas of concern not seen on white light endoscopy (WLE) in 59% of the procedures. VLE imaging also guided tissue acquisition and treatment in 71% and 54% of procedures, respectively. VLE as an adjunct modality improved the neoplasia diagnostic yield by 55% beyond the standard of care practice. In patients with no prior history of therapy, and without visual findings from other technologies, VLE-guided tissue acquisition increased neoplasia detection over random biopsies by 700%. Registry investigators reported that VLE improved the BE management process when used as an adjunct tissue acquisition and treatment guidance tool. The ability of VLE to image large segments of the esophagus with microscopic cross-sectional detail may provide additional benefits including higher yield biopsies and more efficient tissue acquisition. Abstract:BACKGROUND/AIMS: Endoscopic mucosal resection (EMR) has been an effective method to assess patients with esophageal nodules arising in Barrett's esophagus (BE).Optical Coherence Tomography (OCT) enables real-time, three-dimensional (3D) imaging of tissue microstructure, with resolution approaching histology and can be used to evaluate pathological features both In Vivo and within EMR specimens.In comparison, the latter is difficult to assess on confocal endoscopy which depends on In Vivo fluorescein to achieve optimal images .METHODS: A prototype OCT imaging system was developed in collaboration with the LightLab imaging Inc., providing images with an axial resolution of ~5um in tissue and ~15um along the transverse dimension.The OCT system has an axial scanning rate of ~59kHz and the system sensitivity is 103dB.The in-house developed fiberoptic imaging probe (2.5mm in diameter) can pass through the working channel of the endoscope.A 3D-OCT dataset covering an imaging area of 20 mm x 8 mm x 2 mm can be acquired in 20s.Standard EMR was performed on 12 patients using Duette (Cook Medical, Inc.).Twentyfour EMR specimens, including 9 benign, 6 BE, 2 BE with low-grade dysplasia, 1 BE with high grade dysplasia, and 6 early adenocarcinomas, were subsequently imaged with 3D-OCT and sent for standard H&E histology.Imaging features identified in the OCT images were then compared with matching histology.RESULTS: Cross-sectional and en face images could be reconstructed from the 3D-OCT dataset.Cross-sectional OCT image showed features matching with the corresponding histology.BE glands were observed at regions over the lamina propria layer while both the lamina propria layer and the muscular mucosa were preserved in 87% of OCT imaging dataset.En face OCT highlighted the distribution of the glandular structures in Barrett's esophagus and allowed quantification of BE gland density.In lesions with intramucosal and invasive adenocarcinoma, the muscular mucosa layer was less distinct or lost in the OCT images.This may be due to the destruction of tissue architecture or the thickened mucosa from the tumor.No lamina propria layer is identified in the datasets of 6 early adenocarcinomas.In addition, the shape and the distribution of the glandular structures were irregular compared to those of the BE glands.These distinctive patterns associated with adenocarcinoma were observed in both cross-sectional and en face OCT images.CONCLUSIONS: We demonstrated 3D-OCT images of EMR specimens of human esophagus ranging from benign nodules, BE with LGD, BE with HGD to adenocarcinoma.These studies comparing 3D-OCT images with histology are promising in improving In Vivo evaluation of esophageal nodules in the future.

Abstract:Purpose: Esophageal cancer (EC) carries a poor prognosis with 5-year overall survival of less than 20%. Barrett's esophagus (BE) increases the risk of esophageal adenocarcinoma (EAC). The aim of this study was to investigate the ability of EMI-137, a mesenchymal-epithelial transition factor (c-MET)-targeting optical imaging tracer, to detect dysplasia in BE. Experimental Design: c-MET expression in human esophageal tissue was investigated using Gene Expression Omnibus (GEO) datasets, tissue microarrays and BE biopsies. EMI-137 was tested in a dual xenograft mouse model bearing OE33 (c-MET high expression) and FLO-1 (c-MET low expression) tumors. Fluorescence molecular endoscopy (FME) was performed in a mouse model of Barrett's-like metaplasia and dysplasia (L2-IL1β). Tumors and organs-of-interest were evaluated through ex vivo fluorescence imaging. Results:MET mRNA expression analyses and c-MET immunostaining confirmed upregulation of c-MET in BE and EAC compared to normal epithelium. There was strong accumulation of EMI-137 in OE33 xenografts 3 h post injection decreasing by more than 50% on co-injection of a 10-fold molar excess of unlabeled EMI-137. The target-to background ratio (TBR) at 3 h p.i. for OE33 and FLO-1 tumors was 10.08 and 1.42, respectively. FME of L2-IL1β mice showed uptake of EMI-137 in dysplastic lesions within BE with a TBR of 1.9 in vivo, and greater than 2 in ex vivo fluorescence imaging. Conclusions: EMI-137 accumulates in dysplastic lesions within BE and in c-MET positive EAC. EMI-137 imaging has potential as a screening and surveillance tool for patients with BE and as a means to detecting dysplasia and EAC.

Abstract:Objectives/HypothesisProbe-based confocal laser endomicroscopy (pCLE) is a novel technique allowing real-time evaluation of the histological features of tissues in vivo at the cellular level. This study aimed to evaluate the feasibility of using pCLE in the diagnosis of nasopharyngeal carcinoma (NPC). Study DesignFeasibility study. MethodsIn this study, the pCLE images of the lesions, as well as the surrounding or contralateral normal mucosa of the lesions, were acquired in vivo from each patient after intravenous injecting of 2.5mL fluorescein. Biopsy specimens were collected at the imaged sites followed by a histopathological diagnosis by the pathologists, which was used as the gold standard. The pCLE images were compared to histopathological diagnosis of visualized sites by using sensitivity, specificity, negative predictive value (NPV), and positive predictive value (PPV). ResultsDiagnoses based on pCLE images correlated well with the gold standard diagnoses based on tissue histology. The overall sensitivity, specificity, PPV, and NPV for diagnosis of carcinoma versus nondysplasia were 93.8% (67.7%-99.7%), 90.5% (68.2%-98.3%), 88.2% (62.3%-97.9%), and 95.0% (73.1%-99.7%), respectively, and the four indices for pCLE diagnosis of dysplasia versus nondysplasia were 60.0% (17.0%-92.7%), 80.9% (57.4%-93.7%), 42.9% (11.8%-79.8%), and 89.5% (65.5%-98.2%), respectively. The overall sensitivity for diagnosis of carcinoma versus dysplasia was 93.8% (67.7%-99.7%), specificity was 40% (7.3%-83.0%), PPV was 83.3% (57.7%-95.6%), and the NPV was 66.7% (12.5%-98.2%). Abstract:BACKGROUND: Preliminary data suggest that an encapsulated balloon (EsoCheck TM ), coupled with a two methylated DNA biomarker panel (EsoGuard TM ), detects Barrett's esophagus (BE) and esophageal adenocarcinoma (EAC) with high accuracy. The initial assay required sample freezing upon collection. AIM: Assess a next-generation EsoCheck sampling device and EsoGuard assay in a much-enlarged multicenter study clinically enhanced by utilizing a CLIA-compliant assay and samples maintained at room temperature. METHODS: Cases with nondysplastic BE (NDBE), dysplastic BE (indefinite=IND, low grade dysplasia = LGD, high grade dysplasia = HGD), EAC, junctional adenocarcinoma (JAC), plus endoscopy controls without esophageal intestinal metaplasia, were prospectively enrolled. Medical assistants at six institutions delivered the encapsulated balloon per orally, with inflation in the stomach. The inflated balloon sampled the distal 5 cm of the esophagus, then was deflated and retracted into the capsule, preventing sample contamination. EsoGuard bisulfite sequencing assayed levels of methylated Vimentin (mVIM) and methylated Cyclin A1 (mCCNA1). RESULTS: A total of 243 evaluable patients – 88 cases (median age 68, 78% men, 92% white) and 155 controls (median age 57, 41% men, 88% white) – underwent adequate EsoCheck sampling. Mean procedural time was approximately 3 minutes. Cases included 31 NDBE, 16 IND/LGD, 23 HGD, and 18 EAC/JAC. Thirty-seven (53%) non-dysplastic and dysplastic BE cases were short segment BE (SSBE; < 3 cm). Overall sensitivity was 85% (95% CI= 0.78-0.93), and specificity was 85% (95% CI=0.79-0.90). Sensitivity for NDBE was 84%. EsoCheck/EsoGuard detected 100% of cancers (n=18). CONCLUSION: EsoCheck/EsoGuard demonstrated high sensitivity and specificity in detecting BE and BE-related neoplasia.

Abstract:Background This study aimed to evaluate the use of ultrahigh-speed volumetric en face and cross-sectional optical coherence tomography (OCT) with micromotor catheters for the in vivo assessment of Barrett’s esophagus and dysplasia. Methods 74 OCT datasets with correlated biopsy/endoscopic mucosal resection histology (49 nondysplastic Barrett’s esophagus [NDBE], 25 neoplasia) were obtained from 14 patients with Barrett’s esophagus and a history of dysplasia and 30 with NDBE. The associations between irregular mucosal patterns on en face OCT, absence of mucosal layering, surface signal > subsurface, and > 5 atypical glands on cross-sectional OCT vs. histology and treatment history were assessed by three blinded readers. Results Atypical glands under irregular mucosal patterns occurred in 75 % of neoplasia (96 % of treatment-naïve neoplasia) vs. 30 % of NDBE datasets (43 % of short- and 18 % of long-segment NDBE). Mucosal layering was absent in 35 % of neoplasia and 50 % of NDBE datasets, and surface signal > subsurface occurred in 29 % of neoplasia and 30 % of NDBE datasets. Conclusions Atypical glands under irregular mucosal patterns are strongly associated with neoplasia, suggesting potential markers for dysplasia and a role in pathogenesis.

Abstract:endoscopist's discretion.Material and Methods: UGIE reports of all patients were reviewed and those with CLE selected.All lower esophageal and cardia biopsies as well as CLE biopsies were reviewed for SIM.BE was diagnosed if specialized intestinal metaplasia (SIM) was present within the CLE.Results: Of 727 patients (mean age: 38+/-15 years, 57% female), 74 (10.2%) had short segment CLE (≤2cm, SCLE) &/or CLE islands, & 3 (0.4%) had long segment CLE (> 2cm, LCLE).Among patients with SCLE/island, 34 had biopsies from CLE with the following findings: SIM in 10 (29.4%), gastric metaplasia in 3 (8.8%),cardia type mucosa in 11 (32.4%),normal esophagus in 3, reflux esophagitis in 4 and inadequate biopsy in 3.One of the patients with LCLE had SIM and the other two had cardia type mucosa.Overall BE was seen in 1.5% of the studied population.SIM was seen in 14.8% of all CLEs biopsied and in 1.9% of protocol biopsies of patients without CLE &/or esophageal ulcer (Lower esophagus: 1/586; Z-line: 3/389; cardia: 8/592).Those with CLE had less severe symptoms (CSS: 70.3 vs. 84.2;P: 0.02) and were more likely to harbor hiatal hernia (72.4% vs. 57.9%;P: 0.01).However, there was no difference in age, gender, endoscopic erosions, BMI, or size of hiatal hernia.Among patients with CLE, those with BE had higher grades (B, C, D) of LA classification (45.5% vs. 4.3%; P: 0.003) and a trend for less severe symptoms (CSS: 36.1 vs. 59.7;P: 0.08).None of the assessed factors could characterize patients with CLE having SIM on biopsy.Conclusions: According to our data, at least 1.5% of Iranian GERD patients coming to medical attention have BE & tend to have less severe symptoms.This is much lower than western reports.Whether this is due to the nature of the GERD, genetic predisposition, or other cause remains to be elucidated.Understanding the cause can cast light on pathogenesis of BE.

Abstract:INTRODUCTION AND OBJECTIVES: Confocal laser endomicroscopy (CLE) enables dynamic imaging of the cellular structures below the mucosal surface and holds promise in providing real time optical diagnosis and grading of urothelial carcinoma. We presented our initial experience using CLE for the diagnosis of urothelial carcinoma of bladder. METHODS: 21 patients scheduled to undergo transurethral resection of bladder tumors were recruited. One urologist who had already participated in a computer-based training course performed all the CLEs. After standard white light cystoscopy (WLC), fluorescein was administered intravesically as a contrast dye. A 2.6 mm probe based CLE (Cellvizio, Mauna Kea Technologies) was passed through a 26Fr resectoscope to image normal and abnormal appearing areas. The images were collected with 488 nm excitation at 8 to 12 frames per second. The endomicroscopic images were compared with standard hematoxylin and eosin analysis of transurethral resection of bladder tumor specimens. RESULTS: Of the 21 recruited patients, 1 had no tumor but inflammation, 2 had carcinoma in situ ( CIS), 7 had low grade tumors, 10 had high grade tumors and 1 had a bladder liomyoma. The overall diagnostic accuracy is 80.9% (17/21) combining CLE+ WLC. 1 inflammation was regarded as CIS, 1 low grade tumor was regarded as high grade and 2 high grade tumors were regarded as low grade. CONCLUSIONS: CLE is an adoptable technology for novice CLE observers after a training session. CLE demonstrated clear differences between normal mucosa and malignant lesions. However, real-time differentiation between low and high grade tumors, inflammation and cancer, can still be challenging. Figure. No caption available. Table. No title available. Abstract:Essential oils have traditionally been used for decades to alleviate the symptoms of various mental problems. In terms of anxiolytic-like properties, lavender oil is probably the most commonly used and best-studied essential oil. Although there is compositional variance among the oils extracted from different origins, there have been few studies performed to date to investigate how these differences affect the expression of anxiolytic-like activity. This paper discusses the interspecies differences and contributions of the main constituents in the expression of anxiolytic-like effects upon inhalation, as assessed in mice by the elevated-plus maze test, by comparing the essential oils from six different species of Lavandula. The results showed qualitative and quantitative variations in the tested oils, which results in significant differences in their anxiolytic-like activities. Our findings also suggest that linalyl acetate (LA) works synergistically with linalool (LO) and that the presence of both LA and LO is essential for the whole oil to work as an inhaled anti-anxiety agent.

Abstract:Esophageal and gastric cancers are common diseases with high morbidity and mortality; thus, early detection and treatment are beneficial to improve prognosis. Confocal laser endomicroscopy (CLE) is a novel imaging technique that permits the histological analysis of tissues during endoscopy. CLE has been shown to uniquely affect the diagnosis of early upper gastrointestinal cancers. Relevant literature was searched using PubMed and Google Scholar databases. Despite inherent flaws, CLE can reduce tissue damage and improve diagnostic accuracy to a certain extent. CLE in combination with other imaging methods can help enhance the detection rate and avoid unnecessary biopsies in the management of esophageal or gastric cancer and precancerous lesions. CLE is of great significance in the diagnosis and surveillance of early cancers of the upper gastrointestinal tract. Further technical innovations and the standardisation of CLE will make it more responsive to the needs of routine clinical applications.
