Minimally Invasive Percutaneous Plate Osteosynthesis Combined with Locking Compression Plate for the Treatment of Distal Tibial Fractures

Abstract:Objective To evaluate the short-term outcomes of minimally invasive percutaneous plate osteosynthesis(MIPPO) combined with locking compression plate(LCP) in the treatment of distal tibial fractures.Methods From June 2004 to March 2006,16 patients with distal tibial fractures were treated using MIPPO combined with LCP in our department.According to the AO classification,there were 7 cases of type 43A1,5 type 43A3,2 type 43B1 and 2 type 43C3.After reduction of tibial fracture with three methods,LCP was inserted and fixed with locking screw.All the cases were followed up periodically after the operation.Results All the cases were followed up for 5 to 20 months(mean,11.5 months).Primary wound healing was achieved in 16 patients.No complications,such as non-union,abnormal union and breakage of the plate,occurred in the cases.Callus formation was seen on X-rays at 7.6 weeks(4 to 12 weeks) after the surgery,when the patients began part weight-bearing exercises.The bone union was detected at 16 weeks(8 to 20 weeks),when the patients began full weight bearing.3 patients felt uncomfortable at the distal tibia around the implant.According to the AOFAS Ankle-Hindfoot Scale,14 cases(87.5%) were excellent and 2(12.5%) were good.Conclusions MIPPO combined with LCP technique is an effective treatment for the fractures of the distal tibia.This minimally invasive method can achieve stable fixation,promote bone healing,and permit early functional rehabilitation.
Extended Anterolateral Approach for Thetreatment of Tibial Plateau Fractures with Posterolateral Bone Fragment

Abstract:Objective To investigate the differences and clinical effects of extended anterolateral approach in the treatment of simple/complex tibial plateau fractures with posterolateral bone fragment.Methods From January 2013 to December 2015,forty-six patients with posterolateral tibial plateau fractures treated in our hospital were included in our research.The posterolateral plateau fractures were reduced and fixed by the extended anterolateral approach.According to Schatzker fracture classification,patients with type Ⅱ fractures were treated as simple group and patients with type Ⅴ and type Ⅵ fractures were treated as complex group.According to tibial plateau three column classification,patients were divided into single column group,double column group and three column group.The operative time,bleeding volume,fracture healing time,postoperative Rasmussen score,knee mobility,reduction and fixation of fractures and postoperative complications were compared among the groups.Results All 46 patients were followed up for 23-45 months,with an average of 31.9 months.There were 24 cases in simple fracture group and 22 cases in complex fracture group.The mean operation time of simple group was 124±33.8 min,and that of complex group was 175±65.5 min,with significant difference (t=2.302,P=0.025);the mean bleeding volume of simple group was 118±93.5 ml,and that of complex group was 190± 149.4 ml,with significant difference (t=1.905,P=0.028).12 cases were treated with arthroscopy.Bone union was achieved in all patients at the last follow-up,and daily activities were not limited.The average healing time was 4.58 months in simple group and 5.54 months in complex group.The excellent rate of fracture reduction was 83.3％ in simple group and 27.3％ in complex group (x2=14.679,P=0.000).Posterolateral bone fragment was not completely fixed in 4 cases (16.7％) in simple group and 8 cases (36.4％) in complex group.The average Rasmussen function score was 26.8±2.1 and the range of motion of knee joint was 100°-120° (average 115.5°±6.2°) in simple group;The average Rasmussen function score was 23.5±3.4 points and the range of motion of knee joint was 95°-115° (average 106.3°±7.4°) in complex group,neither with no significant differences.According to the three-column fracture classification system,there were 6 cases in the single-column group,24 cases in the double-column group and 16 cases in the three-column group.There were significant differences in the operative time (F=5.039,P=0.000),bleeding volume (F=5.215,P=0.000) and the excellent rate of fracture reduction (x2=7.003,P=0.030) between these three groups.But there was no significant difference in the time of fracture healing,un-fixation rate of posterolateral bone fragment,knee joint Rasmussen score and knee jointactivity.The excellent rate of fracture reduction was 83.3％ in arthroscopy assisted cases and 47.1％ in arthroscopy un-assisted cases respectively (x2=4.749,P=0.002).Postoperative wound infection occurred in 1 patient.Conclusion Extended anterolateral approach could provide good reduction and fixation of tibial plateau fractures with posterolateral bone fragment with satisfactory clinical outcomes obtained in both simple and complex fractures.The operation was relatively simple and posterolateral bone fracture could get better reduction and fixation in simple tibial plateau fractures,and arthroscopy treatment was helpful to improve fracture reduction.

Abstract:Tibial plateau fractures (TFP) are serious injuries of the knee and difficult to treat.In the past decades, many kinds of fixation methods have been proposed in the literature. From non-operative management to current staged and combined management with provisional external fixators, before conventional angular stable platingshave been suggested for complex TPF. The conventional techniques have been widely accepted, but many patients developed complications and unsatisfactory outcomes. Restricted internal approaches likeMinimally invasive plate osteosynthesis(MIPO), Less Invasive Stabilization System (LISS) and further less invasive techniqueslike fine-wire apparatuses, arthroscopically assisted fixation, tibialtuberoplasty and tibial nailing for selected cases have been developed.Most authors agree that functional outcomes of complex TPF depend mainly on the quality of anatomical and stable reduction of the intraarticular and posterior fragments while the type of fracture may be deemed as an independent predictor. Minimally invasive techniques have been proven to provide fewer complications, better functional outcomes, and early weight-bearing than the conventional open surgical techniques.
Arthroscopy-assisted operative management of tibial plateau fractures

Abstract:This retrospective review evaluated the results of arthroscopy-assisted surgery for tibial plateau fractures in 45 patients with closed tibial plateau fractures. The fracture involved articular depression in 27 patients in whom lifting and bone grafting with autogenous corticocancellous iliac bone graft was required. In 23 patients there were also meniscal lesions, which were treated by partial resection in 16 and repaired in 7. Internal fixation was performed using screws in 36 knees and plate in 10 knees. Radiological results were evaluated according to the Resnic-Niwoyama criteria; mean follow-up was 36 months (range 14-72). There was no intraoperative complication in the series, but postoperatively there were one infection and one loss of correction. Results were satisfactory in 89% of cases, according the Rasmussen criteria. Arthroscopy is thus an excellent and minimally invasive method for assessment and treatment of tibial plateau fractures. The advantages are complete and anatomical reduction in the fractured articular surface and evaluation of other concomitant intra-articular pathology and entails only little additional morbidity, especially compared to arthrotomy.

Minimally Invasive Plate Osteosynthesis for Distal Tibial Fractures

Abstract:Objective To report the efficacy of treatment of distal tibial fractures with minimally invasive plate osteosynthesis (MIPO). Methods From March 2006 to January 2010, 22 cases of distal tibial fractures were treated with close reduction and the MIPO technique. They were 8 men and 14 women,with an average age of 45. 3 years (range, 24 to 75 years). The fracture healing was observed through regular follow-up radiography. Functional recovery of affected lower extremity was evaluated according to Johner-Wruhs scoring system. Results All the patients were followed up for an average of 14 months (range,6 to 33 months). All the fractures were healed after an average time of 18. 6 weeks (range, 15 to 24 weeks). Soft tissue problem occurred in only one case. According to Johner-Wruhs scoring, 12 cases were excellent, 9 good and one fair. The total excellent to good rate was 95. 5%. Conclusion In treatment of distal tibial fractures, MIPO technique can improve fracture healing and functional recovery through decreasing postoperative complications.

Abstract:Objective:To explore the epidemiological characteristics, clinical characteristics, treatment strategies and clinical results of non-dislocated hyperextension tibial plateau fracture.Methods:A total of 25 cases of non-dislocated hyperextension tibial plateau fracture patients were collected, including 12 males and 13 females, aged 27-79 years with an average age of 51±15 years. Causes of injury: 18 cases of traffic accident, 2 cases of heavy injury, 5 cases of falling injury. Schatzker classification of tibial plateau fracture: 4 cases of type II, 5 cases of type IV, 13 cases of type V, 3 cases of type VI. Three columns theoretical classification: 4 cases of simple lateral column, 5 cases of simple medial column, 7 cases of medial column+lateral columns, 9 cases of three columns. Preoperative tibial plateau posterior slope angle was -10-0 degrees, average-5.2 degrees. Preoperative MRI showed 5 cases of medial collateral ligament (MCL) injury, 3 cases of posterolateral complex (PLC) complex injury, 2 cases of PLC+ posterior cruciate ligament (PCL) injury and 10 cases of menisci injury. The change of tibial plateau posterior slope angle was more than 10 degrees in patients with ligament injury, 5 patients had a tibial plateau posterior slope angle change more than 15 degrees, 5 patients had a tibial plateau posterior slope angle change more than 10 degrees, and the patients with a tibial plateau posterior slope angle change less than 10 degrees had no ligament injury; 6 patients with simple lateral column or medial column fracture had a ligament injury, 2 patients with medial column+lateral column fracture had a ligament injury, and 2 patients with three column fracture had a ligament injury.Results:All patients were followed up for 12-24 months, with an average of 16.4 months. The operation time was 124±33 min (65-180 min), and the bleeding volume was 106±48 ml (20-200 ml). All patients were treated by open reduction and internal fixation. According to the characteristics of fracture, targeted reduction of articular surface and restoration of lower limb force line were carried out. The plate was placed on the anterior part of tibial plateau to fix the fracture fragment. Evaluation of postoperative fracture reduction: 20 cases were anatomic reduction, 5 cases were good reduction (between 2-5 mm articular surface collapse), and the excellent rate of fracture reduction was 100%. Internal fixation: 4 cases were treated with simple lateral plate, 2 cases with simple medial plate, 15 cases with medial+ lateral plate, 1 case with medial plate+ lateral anchor suture, 1 case with medial plate+ posterior screw, 1 case with lateral plate+ fibular screw, 1 case with medial plate + lateral plate+ lateral anchor suture. The meniscus was repaired in 8 patients, including 5 medial and 3 laterals. The fracture healing time was 3-6 months (mean 3.3 months). The postoperative knee Rasmussen score was 24.9±3.5 (18-29), and the postoperative knee joint mobility was 118°±9° (90°-130°). Superficial infection occurred in 2 patients.Conclusion:The main imaging characteristic of "non-dislocated hyperextension tibial plateau fracture" is the change of tibial plateau posterior slope angle. The injury of single anteromedial column/anterolateral column fracture is easy to combine with the "diagonal" injury, and when the tibial plateau posterior slope angle changes more than 10 degrees, it is easy to be associated with peripheral ligament injury. By using open reduction and internal fixation, restoring the joint articular surface and lower limb force line, repairing the soft tissue structure and ligament, and reconstructing the stability of knee joint, we can achieve satisfactory results.

Abstract:A new class of imidazo[2,1‐b]thiazole chalcone derivatives were synthesized and evaluated for their anticancer activity. These chalcone derivatives show promising activity, with log GI50 values ranging from −7.51 to −4.00. The detailed biological aspects of these derivatives toward the MCF‐7 cell line were studied. Interestingly, these chalcone derivatives induced G0/G1‐phase cell‐cycle arrest, down‐regulation of G1‐phase cell‐cycle regulatory proteins such as cyclin D1 and cyclin E1, and up‐regulation of CDK4. Moreover, these compounds elicit the characteristic features of apoptosis such as enhancement in the levels of p53, p21, and p27, suppression of NF‐κB, and up‐regulation of caspase‐9. One of these chalcone derivatives, 3 d, is potentially well suited for detailed biological studies, either alone or in combination with existing therapies.
Minimally invasive percutaneous plate osteosynthesis with locking TCP versus dynamic condylar screw in the internal fixation of comminuted fracture of the distal femur

Abstract:BACKGROUND:There are many therapies for comminuted fracture of the distal femur,but no uniform standard for clinicians is considered to choose appropriate therapy.OBJECTIVE:To compare the effects of minimally invasive percutaneous plate osteosynthesis(MIPPO) with locking TCP versus dynamic condylar screw(DCS) in the internal fixation of comminuted fracture of the distal femur.METHODS:Thirty-seven patients with comminuted fracture of the distal femur were randomized into two groups:MIPPO with locking TCP group(n=19) and DCS group(n=18).General conditions,damages,healing time,complications and function recovery were compared and analyzed between the two groups.RESULTS AND CONCLUSION:The patients undergoing MIPPO with locking TCP could walk earlier,recovered better and had a shorter healing time(P < 0.05).According to Karlstorm Knee Functional Evaluation Standard,the excellent and good rate in the MIPPO with locking TCP was significantly higher than that in the DCS group(P < 0.05).Locking TCP combined with MIPPO technology was better than DCS in treating comminuted fracture of the distal femur.

The plate was placed on the anterior part of tibial plateau. Evaluation of postoperative fracture reduction was as follows: 20 cases reached anatomic reduction, 5 cases reached good reduction (between 2 and 5 mm articular surface collapse), and the excellent rate of fracture reduction was 100%. The fracture healing time was 3–6 months (average 3.3 months). The postoperative knee Rasmussen score was 18–29 (average 24.9), and the postoperative knee joint mobility was 90–130° (average 118°). Two patients suffered superficial infection. Conclusions The main imaging characteristic of “non-dislocated hyperextension tibial plateau fracture” is the change of tibial plateau posterior slope angle. The injury of single anteromedial column/anterolateral column fracture is easy to combine with “diagonal” injury, and when the tPSA changes more than 10°, it is easy to be combined with ligament injury. By reducing the joint articular surface and lower limb force line, repairing the soft tissue structure, and reconstructing the knee joint stability, we can get satisfactory results. Reinforced bar-ring Hybrid external fixator combined with limited internal fixation for complex fractures of tibial plateau

Abstract:Objective To introduce the experience and analyze the clinical outcome of applica- tion of reinforced bar-ring hybrid external fixator combined with limited internal fixation for complex frac- turcs of tibia plateau in patients.Methods From Aug.2002 to Feb.2005,21 patients with complex fractures of tibial plateau were treated with reinforced bar-ring hybrid external fixatar combined with lim- ited internal fixation.According to Schatzker classification,there were 13 patients with typeⅤfracture and 8 with typeⅥfracture.Voluntary and non-weight beating knee joint motion to partially weight bear- ing was taken one week after operation.Progressive loading was taken continuously until there was clinical evidence of fracture stability and then full weight bearing was done.Results The mean follow-up peri- od was 15.9 months (10-24 months).All the fractures got united and the mean union time was 4.2 months(3-8 months).All the injured knees could extend to less than 5 degrees and flex to more than 90 degrees after rehabilitation.Evaluated by Merchant score system for knee joint,81% of patients achieved excellent healing (7 cases) and good healing (10 cases).Conclusion Operation by using reinforced bar-ring hybrid external fixator combined with limited internal fixation can reduce complications and avoid stiffness of the knee joint.It is an effective method for complex fractures of tibial plateau.
Analysis of Curative Effect of Complicated Tibial Plateau Fractures with Open Reduction and Internal Fixation with Bilateral Plates

Abstract:Objective To analyze the results of treatment of complicated tibial plateau fractures with open reduction and internal fixation with bilateral plates.Methods From 1997 to 2004,63 patients with complicated tibial platform fractures underwent internal fixation with bilateral plates and bone grafting.Fixation with bilateral plates was performed during operation with small plates crews placed medially and the elongated ones inserted laterally after the underlying separation.The minimal skin incision for the fixation of the screws would decrease the length of skin flap and stripping surface.Meanwhile,sufficient bone grafting during operation could effectively support reduction of articular surface.Functional exercise of extension and flexion of knee joint was performed early postoperatively. Results The inversion and eversion of knee joint occurred in no patients.A slight re-collapse of tibial plateau occurred postoperatively only in 5 patients (8%).The infection of wound and necrosis of skin flap occurred in 3 patients.According to Lysholm score standard, the excellent and good rate was 90.4%. Conclusion It is a simple and effective method to treat complicated tibial platform fractures with bilateral plates.It will improve the effect and reduce complications significantly to perform sufficient bone grafting intraoperatively,and make the drainage unobstructed and do the early functional exercises postoperatively.

Abstract:The fractures of tibial plateau fractures are a type of intraarticular fracture, representing nearly about 1% of all fractures. [ 1 ] These fractures pose considerable surgical challenges because they require precise anatomical reduction and secure fixation to allow for early rehabilitation and improved functional results. The major therapeutic aims for intraarticular fractures are always to reconstruct the articular surface and ensure stable fixation to allow for early mobility. The treatment of tibial plateau fractures adheres to two fundamental principles. First, it involves achieving an anatomical reduction of the joint surface and reconstructing the mechanical axis of the limb. Second, it entails restoring the stability of the knee joint. [ 2 ] Schatzker type I to III fractures are relatively low-energy injuries, sustained in the lateral tibia plateau, whereas type IV to VI are high-energy trauma injuries. [ 3-5 ] Displaced tibial plateau fractures have conventionally been managed using a surgical technique known as open reduction internal fixation (ORIF). [ 6 , 7 ] Various approaches and techniques have been developed to address tibial plateau fractures, including the anterolateral and posteromedial inverted L-shaped approaches. However, it is important to note that ORIF, despite its effectiveness in managing these fractures, has been associated with several reported complications. These complications may include infections, hematoma formation, surgical wound dehiscence, and knee stiffness. [ 8 ] The role of arthroscopy in the management of tibial plateau fractures has evolved, and more recently, arthroscopically assisted internal fixation (ARIF) is considered a viable choice among available surgical techniques. This technique was initially developed in the 1980s by Reiner, McGlynn, and Jennings and has demonstrated its effectiveness, particularly in the treatment of Schatzker types 1, 2, and 3 fractures when compared to traditional ORIF. For Schatzker types 4, 5, and 6 fractures, arthroscopy appears to enhance the quality of reduction and eliminate the need for extensive arthrotomy. [ 9-13 ] The main advantage of this approach is its minimally invasive nature during surgery, which avoids damage to the articular cartilage, menisci, and other delicate knee structures. The combination of arthroscopy and percutaneous fixation brings several benefits, including enhanced fracture reduction, optimal visualization for joint reconstruction, and the ability to address associated injuries simultaneously.

The study finds the mediating effect of CRM processes between CRM resources and customer retention programs of CRM which themselves have direct effect on customer retention. Also, the suitable research methodology for doing this study will be explained.

Abstract:Purpose The present study was designed to investigate the precise procedure and effectiveness of percutaneous minimally invasive fixation assisted by TiRobot in managing AO/OTA type 41B2 tibial plateau fracture to provide an alternative solution for clinical application. Methods In total, 10 participants with AO/OTA type 41B2 tibial plateau fractures diagnosed by preoperative imaging examinations were enrolled in this study between May 2019 and May 2022. They were 5 males and 5 females, with an average age of 45.6 ± 11.3 years old (range 27–62 years old). All of them had closed fractures, including 6 cases with anterior cruciate ligament (ACL) tibial insertion avulsion fractures, 1 case with medial collateral ligament (MCL) tear, and 4 cases with a lateral meniscus tear. From injury through surgery, the entire time frame was 4.0 ± 1.5 days (range, 2–7 days). Following indirect percutaneous reduction assisted by TiRobot, the Jail method was used to treat all patients with minimally invasive internal fixation. Patients with ligament or meniscus injurieswere treated with arthroscopic surgery in one stage. The standardized functional exercise was performed postoperatively. The knee function was measured using the Hospital for Special Surgery (HSS) score, and the fracture reduction was assessed through the Rasmussen radiology score. Results All patients were followed up for 12.7 ± 6.8 months (6–24 months).The fracture healing time was 11.8 ± 0.8 weeks (10–13 weeks), and the X-rays revealed satisfactory fracture reduction.The knee joint's Rasmussen score was 17.8 ± 0.4 (in the range of 17–18) a year after the procedure, with 8 patients receiving outstanding ratings and 2 cases receiving satisfactory scores. The HSS score was 93.8 ± 2.3 (range, 89 to 96), of which 10 cases were excellent. The motion range of the kneewas 138.7°±2.7° (range, −5° to 0° to 135°). No adverse effects or serious complications, such as internal fixation failure, postoperative infection, popliteal vascular injury, and common peroneal nerve injury, were observed during the last follow-up visit. Conclusions The intelligent assistance and accurate guidance of TiRobot can simplify and standardize procedures of percutaneous minimally invasive fixation in theSchatzker type III tibial plateau fracture treatment. This technique increases the precision of indirect percutaneous reduction and screw fixation while minimizing bone grafting.

Abstract:Rheumatoid arthritis is a systemic disease which locally involves the joint synovial membrane. The alterations of soft tissue, cartilage, and bone are secondary to the pathologic changes occurring within this very dynamic structure (Table I). This communication will describe the most pertinent radiologic findings in the earliest phases which may assist the clinician in making the diagnosis. It is imperative that the radiologist recognize these early changes so that proper therapeutic measures may be instituted promptly and effectively. The soft-tissue alterations must be studied carefully since they may be the first findings apparent and in some instances may be demonstrated radiographically before physical signs are evident. Abstract:Tibial plateau fractures are quite common among lower limb fractures. Several fracture classifications exist including Schatzker classification, in which tibial plateau fractures are divided into six types where each increasing numerical category indicates increasing severity of the injury and worsening prognosis. Arthroscopic-assisted techniques using a lateral or medial metaphyseal window have shown results comparable to open internal fixation methods with multiple advantages. We present a case of a medically and surgically free 40-year-old lady who presented to our emergency department complaining of left knee pain following a fall from the stairs. Clinically there was significant swelling and tenderness over the lateral aspect of the left proximal tibia, radiographs showed a Schatzker type III tibial plateau fracture, confirmed by computed tomography (CT). A combined intraoperative arthroscopic- and fluoroscopic-guided reduction of the articular depression through a lateral cortical window was achieved and the fracture was fixed using a minimally invasive fixation technique. The postoperative course was uneventful. The patient had recovered full range of motion and the wounds were barely visible. One-year X-ray showed healed fracture without any evidence of displacement or subsidence.

Abstract:PurposeComplex bicondylar tibial plateau fracture (TPF) has always been a tricky problem for surgeons. We created a novel external device used intraoperatively consisting of Kirschner wires, and combined with minimally invasive plate oseoynthesis (MIPO) technique to treat complex bicondylar TPFs, and the clinical effect and feasibility were further evaluated.MethodsFrom March 2016 to February 2021, 49 cases (29 males and 20 females) were identified as bicondylar TPF, the mean age 47.2 (27–69). All patients adopted the device and MIPO technique. A series of score, complications, and radiographs in the follow-up period, from three months, six months, one year, and two years and the last follow-up, were recorded, from visual analogue score (VAS), hospital for special surgery (HSS), and Short-Form 36 (SF-36), containing physical (PCS) and mental (MCS), and Rasmussen score.ResultsForty-seven patients showed good functional recovery. No patients were lost, mean follow-up time was 28.17 ± 2.81 (24.2–35.4) months. Operation time was 89.80 ± 13.46 (58–110) min. At the last follow-up, VAS was 1.3 ± 0.92 (0–4), HHS was 93.10 ± 2.63 (89–99), PCS was 49.20 ± 7.40 (38–65), and MCS was 50.08 ± 4.77 (43–62). Complications were as follows: cutaneous necrosis (3, 6%), asymptomatic arthritis (3, 6%), symptomatic arthritis (1, 2%), and deep venous thrombosis (1, 2%). Mean fracture healing time was 11.82 ± 1.5 (10–15.4) weeks. All patients got recovery without extra surgery and removed the implants at 12.85 ± 0.76 (11.2–15.4) months.ConclusionTemporary traction device of bilateral external fixator combined with MIPO technique was simple and convenient, with a smaller soft-tissue damage, an easier operational approach, and its worth being promoted.

Abstract:Objective: To explore the clinical efficacy of double-plate fixation for the treatment of old tibial plateau fractures with Schatzker type IV through anterior midline and posteromedial approaches. Methods: From July 2013 to July 2015, 15 patients with old tibial plateau fractures were treated with internal fixation using locking reconstructive plate for the posteromedial fragment and anatomical locking plate for anteromedial fragment through antero midline and posteromedial approaches. There were 9 males and 6 females, with an average age of 49.2 years old (ranged, 21 to 61 years old). Eight patients had injured in the left side and 7 in the right side. According to Schatzker classification, all patients were type IV. The mean interval from injury to operation was 26.5 days (ranged, 21 to 65 days). The main clinical symptoms before operation were knee joint swelling, pain, deformity and limitation of motion. The X-ray and CT confirmed the fracture type. The indexes such as tibial plateau tibial shaft angle (TPA), femoral tibial angle (FTA) and posterior slope angle (PSA) were compared between immediate postoperation and final follow-up using postoperative X-ray film. The knee functions were evaluated using the HSS (Hospital for Special Surgery) knee score system. Results: Two patients had incision complications which healed by correct treatment, 1 patient had traumatic arthritis. All patients were followed up for mean 16.6 months (ranged, 13 to 24 months). No infections, deep venous thrombosis, implant loosening and breakage, fragment displacement, plateau surface collapse and bone nonunion found. The bone union time ranged from 3 to 8 months (mean 6.07 months) after operation. The average immediate postoperative value of TPA, FTA and PSA were(86.81±1.67)°, (168.00±3.29)° and(10.20±1.47)° respectively; and(86.47±1.67)°, (168.53±3.03)° and (10.54±1.21)° respectively at the final follow-up evaluation, showing no statistical differences(P>0.05). According to the HSS score system, 26.33±3.86 in pain, 20.00±1.79 in function, 16.00±1.55 in range of motion, 8.67±0.94 in muscle strength, 8.53±1.67 in flexion deformity, 9.33±0.94 in joint stability, and the total mean score was 88.86±8.92. The outcomes were excellent in 10 cases, good in 4, and fair in 1. Conclusions: Double-plate fixation via combined anterior midline and posteromedial approaches is an ideal surgical method for old tibial plateau fractures with Schatzker IV type, showing satisfactory exposure, reliable reduction and fixation, and benefiting for early functional exercise. The short-term clinical results was satisfactory.
