Abstract:The purpose of this study is to give an exact formulation of optimization of volumetric-modulated arc therapy (VMAT) with sliding-window delivery, and to investigate the plan quality effects of decreasing the number of sliding-window sweeps made on the 360-degree arc for a faster VMAT treatment. In light of the exact formulation, we interpret an algorithm previously suggested in the literature as a heuristic method for solving this optimization problem. By first making a generalization, we suggest a modification of this algorithm for better handling of plans with fewer sweeps. In a numerical study involving one prostate and one lung case, plans with varying treatment times and number of sweeps are generated. It is observed that, as the treatment time restrictions become tighter, fewer sweeps may lead to better plan quality. Performance of the original and the modified version of the algorithm is evaluated in parallel. Applying the modified version results in better objective function values and less dose discrepancies between optimized and accurate dose, and the advantages are pronounced with decreasing number of sweeps.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is how to maintain or improve the plan quality while shortening the treatment time in volumetric modulated arc therapy (VMAT) with the sliding - window delivery method. Specifically, the author focuses on the impact of reducing the number of sliding - window sweeps over a 360 - degree arc on the treatment plan quality. By reducing the number of sweeps, faster VMAT treatment can be achieved, but the quality of the plan may be sacrificed. Therefore, the research aims to explore the trade - off between treatment time and plan quality and proposes an improved algorithm to better handle plans with fewer sweeps, thereby minimizing the negative impact on plan quality while ensuring treatment efficiency. ### Main research contents 1. **Exact formulation of the optimization problem**: The paper first gives the exact mathematical formulation of the sliding - window VMAT optimization problem, which involves expressing the accurate dose as an explicit function of the sliding - leaf trajectory while considering the rotation of the gantry. This exact formulation is a non - smooth optimization problem. Although directly solving this problem is not within the scope of this research, it provides a basis for subsequent algorithm development. 2. **Explanation and improvement of existing algorithms**: Based on the above - mentioned exact formulation, the author re - interprets the algorithm proposed by Papp and Unkelbach as a heuristic method to solve this optimization problem. Then, the author proposes an improvement to this algorithm, especially when dealing with plans with fewer sweeps. The improved algorithm can better handle these situations, thereby improving the quality of the plan. 3. **Numerical study**: By generating plans with different treatment times and numbers of sweeps, the impact of reducing the number of sweeps on the plan quality is studied. The research results show that in some cases, even if the number of sweeps is reduced, good plan quality can be obtained, especially when the treatment time limit is tight. ### Key findings - **Trade - off between plan quality and treatment time**: As the treatment time limit increases, reducing the number of sweeps can sometimes improve the plan quality instead. This indicates that in some cases, more efficient treatment can be achieved by reducing the number of sweeps without significantly sacrificing the plan quality. - **Performance of the improved algorithm**: The improved algorithm performs better when dealing with plans with fewer sweeps, especially in reducing dose differences. This indicates that the improved algorithm has better potential in practical applications. ### Conclusion By exactly formulating the optimization problem of sliding - window VMAT and proposing an improved algorithm, the paper provides new ideas for maintaining or improving the plan quality while shortening the treatment time. The research results show that even when the treatment time limit is tight, efficient and high - quality treatment plans can be achieved by reducing the number of sweeps. This finding is of great significance for clinical applications, especially in cases where rapid treatment is required.
