Abstract:In this paper the author discusses two categories of patients which differ in terms of the impact they have in the countertransference. On the one hand, there are patients who create an empty space in the analyst's mind. The response they provoke is a kind of depressive feeling that remains after they leave. The patient may bring dreams and associations, but they do not reverberate in the analyst's mind. The experience is of dryness, a dearth of memory, which may--at times--leave the analyst with a sense of exclusion from the patient's internal world. At the other extreme, there are patients who fill the consulting room. They do that with their words, dreams and associations but also with their emotions and their actions. The experience is that the analyst is over-included in the patient's world. They have dreams that directly refer to the analyst and the analyst feels consistently involved in the patient's analysis. The pathway through which the analyst can understand both these types of patients is via the countertransference or, to put it another way, the analyst's passion. In 'Analysis terminable and interminable' Freud suggested that the bedrock of any analysis is the repudiation of femininity. The author believes this statement may be viewed as lying at the crossroads of the discussion about the limits of the theoretical and clinical psychoanalytic formulations which she refers to. In the examples presented the author relates the repudiation of femininity in its connections to the gaps implicit in psychoanalytic understanding.

Abstract:An attempt is made to encircle time and the times psychoanalytically. They are understood as the result of the interplay of different psychic systems: Timelessness of the Ucs system (psychic reality), actual time in the Pcpt-Cs (perceptual reality), and vectorial-linear time in the Cs/Pcs systems (reality principle). Time shows itself in the moment of presence, but it can only show itself if there is a temporal antecedent. At the same time, time and space are intertwined, so that the past is initially the place where something happened. However, the interplay of the mental systems with time and space can only develop in the object relationship. A short clinical example of an autistoid perversion illustrates this dynamic.

Abstract:This paper employs a rhetorical form designed to clarify and sharpen the focus of the very special stance required--which must be painstakingly learned under careful supervision--in order to effectively tune in to communications coming from the unconscious of the patient. This is the hardest task that must be mastered to become truly empathic and sensitive in dyadic relationships, a unique expertise that marks the psychiatrist as a genuine specialist in medical practice. Regardless of theoretical orientation, neither the form or content of any therapeutic intervention can be appropriate unless it is empathically based. Clinical vignettes illustrate the lack of such empathy, and readings are suggested that enhance our approach to learning this skill, borrowing especially from Kohut and Bion. The great importance of the often ignored "background" of the patient's communication is emphasized, and is illustrated from the field of music in the work of John Cage and Anton Webern. The congruence between this clinical psychiatric problem and the main thrust of Continental philosophy, which attempts to put man back in touch with himself, is described. Suggestions are offered to supervisors how to develop these skills in the novice. Finally, a discussion is presented of the effect on the professional and personal life of the therapist who has not developed these skills, emphasizing the dangers of "burn-out" in therapists and the implicit philosophy of life in a money-oriented practice of psychotherapy. The dangers of not attending to such matters even during residency training are pointed out, in an attempt to raise the consciousness level of the therapist to the extreme importance of background practices both in the patient and the therapist.
PRODUCT PROPERTIES : A NEW APPLICATION OF COMPOSITE MATERIALS

Abstract:A new class of physical properties of composite materials is that of "product properties" in which the phases or submaterials of the composite are selected in such a way that an effect in one of the phases or submaterials leads to a second effect in the other phase. A typical example is the magneto-electric effect in a composite material with one magnetostrictive and one piezoelectric phase: a magnetic field induces a distortion of the magnetostrictive phase, which in turn distorts the piezoelectric phase in which art electric field is generated. The composite as a whole can be considered macroscopically as a new, homogeneous material with a magneto-electric effect not exhibited by any of the composing phases on their own. The coupling, in this case, is of the mechanical kind. The entire class of product properties can be searched systematically for interesting properties by a kind of matrix scanning procedure. Typical examples will be given in the present paper.

Abstract:Implicitly or explicitly, time dominates the psychoanalytic situation. The precision, consistency of duration, and regularity of analytic sessions enhance the patient's ego boundaries, counteracting the regressive effects of timelessness induced by free association. The extended overall duration of psychoanalysis and the high frequency of sessions favor the development of transference neurosis. The interpretation of the transference in the here and now of the analytic situation illuminates the past, and as a result, the patient's self-image and that of the world become better integrated. The sense of time in the analytic situation for both patient and analyst varies along with the vicissitudes of transference and countertransference.

Generalized granuloma annulare associated with gastrointestinal stromal tumour: case report and review of clinical features and management

Abstract:The paraneoplastic variant of granuloma annulare (GA) is a rare cutaneous manifestation of underlying malignancy that is most commonly associated with systemic lymphoma. We report an interesting case of a patient with gastrointestinal stornal tumour (GIST) of the stomach presenting with extensive generalized GA. GIST was diagnosed 2 months after the diagnosis of GA. Resolution of the GA was seen 1 month after surgical excision of GIST. The close correlation of the clinical courses of these two rare diseases suggests that their coexistence was more than a coincidental finding. This case highlights the importance of excluding paraneoplastic GA, especially in cases where the skin manifestations are extensive and resistant to treatment.
Rethinking the concepts of the unconscious and analytic time

Abstract:The author proposes ways of rethinking the concepts of the unconscious and time in the analytic setting, including the very existence of the unconscious. Freud (1915) stated that the success psychoanalytic thinking has in making inferences about the patient's unconscious makes the existence of the unconscious "incontrovertible." The author submits that this success does not establish the existence of the unconscious; rather, the inferences we think we make about the unconscious are inferences about consciousness itself - the totality of our experiences of thinking, feeling, sensing, observing, and communicating with ourselves. The author then offers thoughts about a second analytic concept, the experience of time in the analytic setting. He conceives of there being two inseparable sorts of experiences of analytic time that stand in a dynamic relationship with one another: diachronic time (clock time) and synchronic time (dream time). In diachronic time, time is sequential; one thing leads to another. In synchronic time, all time is contained in the present. In analysis, childhood trauma is experienced for the first time (in synchronic time) in the co-created subjectivity of patient and analyst.

Abstract:The contemplation, containing, and linking that circulate within the analyst’s private space are positioned as key to states of psychic equivalence (the melding of psychic reality and material reality) and to sustained states of unknowing, which are held to be necessary for analytic work and fantastic spontaneity. These modes of practice are considered as they relate to rituals that promote the analyst’s self care. An account of a psychotherapy with a 5-year-old electively mute girl is offered to illustrate the work undertaken in the analyst’s private space, as he seeks to build and sustain potential space and the possibilities borne through play.

Abstract:Journal of the American Psychoanalytic Association, Ahead of Print. This paper engages a Winnicottian formulation of the analytic field to highlight the often unspoken, implicit erotic dimensions of analytic work. Material from an analysis with a very inhibited, emotionally constricted man shows both patient and analyst encountering difficulty in "locating" each other within the analytic field. Paradoxically, a felt sense of connection was also palpable, and possibly/impossibly erotic. Movement in the treatment required the development of what the analyst came to think of as an "erotic sonar"—a "sounding" in the erotic body of each participant that could indicate the place of creative connection. A sensing in the body for the "feel" of the analytic dyad became a kinetic reading of emotional closeness or distance, as well as indicating the subtle, and shifting, tones of the relationship. Emphasis is placed on bodily experience in a subjective registering of the erotic, both as sexuality and as a more general experience of passion, vitality, and creativity.

Abstract:This paper follows our previous one, where we described a psychoanalytic conception of language, thought, and internalization that is informed by the thinking of Lev Vygotsky. Here, several aspects of the analytic process which allow for the understanding of ineffable experiences in the analysand's history and the analytic situation are investigated: specifically, primal repression, metaphor, and the role of speech in free association. It is suggested that Freud's notion of primal repression be revived and redefined as one aspect of the descriptive unconscious. Some implications of primal repression for transference and resistance are explored. The metaphoric in its broad sense is examined as one example of how early dynamic experiences embedded in the process of language acquisition can be reached within the clinical situation. It is proposed that an understanding of free association is enhanced by awareness of distinctions between inner, egocentric, and social speech. The basic rule can be interpreted as an invitation for the analysand to use inner speech in collaboration with the analyst as best he or she can. Further, the aliveness and degree of superficiality of the analysis can be seen as a function of the analyst's ability to appreciate the properties of inner speech and foster the conditions in the analysis that allow for its unfolding.

Abstract:Time is an important source of containment vis-à-vis the pressure of affects and the nondimensional immensity of mental space experienced by difficult patients. A more articulated spatiotemporal integration can be facilitated by the analyst's musical "reverie" during intense emotional exchanges in analytic sessions. This reverie can be visual, olfactory, kinaesthetic, etc., no less than auditory or musical. Music is indeed connected with both the concrete world of bodily sensations and the symbolic expressions of culture, and may be an important transitional phenomenon in analytic communication on both unconscious and conscious levels. Two clinical cases are presented in which the patient's awareness of the passage of time, associated with the analyst's internal musical experiences, made it possible in one case to reduce intense panic attacks and, in the other, to overcome the patient's rigid obsessive defenses, giving him access to fluid and unforeseen emotions. In these two instances of working through, the perception of time helped establish confidence in the creative contribution of the "unheard melodies" (Keats) of affects to the functioning of thought.

Psychoanalytic Dialogues

Abstract:This paper offers a reflection and an open, experiential reading of one of Winnicott's most fascinating case studies. I try to show how the question of time in the psychoanalytical context emerges not only in relation to a deviation from the traditional setting but also concerns an axis inherent to some key therapeutic issues, such as the meaning of time and the temporal as a quality of holding in regression to dependency; or as a component in the transference experience or in the therapist's holding in mind of the patient's potential, through holding the future. The high frequency of meetings in psychoanalysis facilitates qualitative change, allowing patients and therapists to be in touch with deep, primitive strata of the psyche. Extending time in psychoanalytic treatment, therefore, whether as an ad hoc response to the patient's needs or as an extension of the setting, is tantamount to enabling a multi-layered experience of time across the spectrum of its manifestations. Attention to the temporal component in therapy and the experience of time in the transference is a valuable resource in our work. This paper contributes to the attempt to elaborate this dimension of psychoanalytic listening.

Abstract:The experience of time has been studied psychoanalytically largely through the conceptual frameworks of ego psychology and classical analysis. The implications of self psychology and Lacanian theory for understanding time experience have been relatively little explored. These implications would develop, on the one hand, from Kohut's investigations of fragmentation phenomena and, on the other, from Lacan's concept of the ego and his use of the short session. But time experience cannot be understood entirely in terms of individual histories and the psychoanalytic situation. A preoccupation with time is central to Western culture and is traceable to the social context—thirteenth-century monasticism—in which the clock was first invented. Time enters the analytic session especially in the handling of beginnings and ends, reflecting divergent notions of the purpose of treatment. The standing of time-frame rules in the different systems defined by ego psychology, self psychology, and Lacanian theory are examined together with their underlying assumptions about the nature of reality. These metaphysical issues—no longer academic but made concretely meaningful in each session—go beyond illuminating the concept of time. Our response to them will determine both the direction of each patient's treatment and the course of psychoanalysis itself.

Abstract:A deep understanding of the subjective experience of time in traumatized individuals may require a comprehensive framework that takes into account both psychoanalytic and phenomenological contributions. Referring to the retroactive interpretation of past experiences, the concept of Nachträglichkeit is critical to analyze how trauma can be signified in the form of the après-coup , in which the original traces of traumatic experiences are signified only at a later time. Trauma alters the temporal sequence of past, present, and future, thus leaving the psyche in a time-shifted dimension, where the shadow of the past extends over the present, and the unbearable present hinders growth and development. A clinical vignette is presented to illustrate how trauma can disrupt the temporal nature of subjective experience by reshaping the meaning of psychic events. Ultimately, trauma treatment aims at inscribing the person's experience into a unified and coherent self-narrative.
An Analyst's Hallucination as a Manifestation of Osmotic Communication

Abstract:This article conveys the psychoanalyst's hallucinatory experience during a session with a patient who experienced premature birth trauma. Engaging with the patient's primal fears of disappearance and confusion with the object through hallucinosis initiated the analyst's engagement with her own trauma. The concept of osmotic communication within the patient-analyst relationship is viewed as central to description and understanding of the primal dialogue of two unconscious minds. The filtration of psychic content from the patient's to the analyst's unconscious is facilitated by the semipermeable membrane of analyst's receptivity. As a recipient, the analyst embodies and processes the patient's unmentalized experiences in a transformative manner.

Abstract:Individual canine synovial villi were used to establish short-term synovial organ cultures. These villi incorporated 3H-glucosamine into highly-polymerized 3H-hyaluronic acid (3H-HA), which was the only 3H-glycosaminoglycan identified in the culture medium. Some 3H-HA, and larger amounts of other 3H-glycosaminoglycans, were recovered from cultured tissues. Culture medium 3H-HA content was proportional to the surface area of cultured villi. Organ cultures of nonvillous synovium were compared with villi; nonvillous cultures synthesized less 3H-HA per mm2 of their synovial intimal surface than villi. These cultures complement cell culture techniques for in vitro studies of synovial lining cell function.
