Abstract:Objective: To analyse the associations between the intake of fish and marine mammals and risk factors for cardiovascular disease, ie lipid profile, fasting blood glucose, blood pressure and obesity, in a population whose average consumption of n-3 fatty acids is high compared with Western countries.Design: Information was obtained from a population survey in Greenland: interview data, clinical data and fasting blood samples were obtained from a random sample of Inuit from three towns and four villages.Subjects: Two-hundred and fifty-nine adult Inuit (74% of the sample).Results: Marine diet was positively associated with serum high-density lipoprotein (HDL) and blood glucose and inversely with very-low-density lipoprotein (VLDL) and triglyceride. Association with low-density lipoprotein (LDL), diastolic and systolic blood pressure, waist–hip ratio and body mass index were inconsistent and not statistically significant. The pattern was similar within groups with low, medium and high consumption of marine food.Conclusions: There are statistically significant associations between the consumption of marine food and certain lipid fractions in the blood also in this population with a very high average intake of marine food. The observation that blood glucose is positively associated with marine diet in a population survey is new and should be repeated. Impacts of decline harvest of country food on nutrient intake among Inuit in Arctic Canada: impact of climate change and possible adaptation plan

Abstract:BACKGROUND: The pervasive food insecurity and the diet transition away from local, nutrient-rich country foods present a public health challenge among Inuit living in the Canadian Arctic. While environmental factors such as climate change decreased the accessibility and availability of many country food species, new species were introduced into regions where they were previously unavailable. An adaptation such as turning to alternate country food species can be a viable solution to substitute for the nutrients provided by the declined food species. The objective of this study was to estimate the impact on nutrient intake using hypothetical scenarios that current commonly harvested country foods were reduced by 50%, and were replaced with alternate or new species.METHODS: Data collected during the 2007-2008 Inuit Health Survey from 36 Canadian Arctic communities spanning Nunavut, the Inuvialuit Settlement Region and Nunatsiavut were used.RESULTS: A 50% decline in consumption of fish, whale, ringed seals and birds (the food that was reported to be in decline) resulted in a significant decrease in essential nutrient intake. Possible substitute foods were identified but some nutrients such as zinc and especially vitamin D were most often found lacking in the alternative diet.CONCLUSIONS: If the alternative species are not available or feasible, more expensive and less nutritionally dense store-bought foods may be sought. Given the superior quality of country foods and their association with food security, and Inuit cultural health and personal identity, developing skills and awareness for adaptation, promoting regional sharing networks, forming a co-management agency and continuing nutritional monitoring may potentially preserve the nutritional integrity of Inuit diet, and in turn their health and cultural survival.

Abstract:The COSI hyperspectral imaging system, suitable for small RPAS, is able to produce high resolution hyperspectral data products. By extensive inflight testing, we have identified the main challenges for achieving reliable high quality results. Based on these insights, we propose a refined radiometric calibration strategy. It uses a set of three reference targets, two grey and one colored target, which are to be measured inflight. We present on-ground measurements of the targets with COSI, as in flight measurements, demonstrating the merits of the approach are still ongoing.
Transitions of Food Groups and Nutrients in the Northeast of China: Aa 3-Year-interval's Follow-Up Study

Abstract:Objective: To explore the dietary patterns and changes in population of northeastern Chinese over time. Design: A longitudinal, descriptive, comparative, population-based study. Setting: Heilongjiang, China. Study participants: 1468 and 1418 participants aged 0-95 who had complete records on the three 24-hour dietary recalls in 1997 and 2000 surveys, respectively.Results: Differences in food consumption and nutrient intake existed in groups divided by gender, residence and household income. The staple food was cereals which offered 53.4 - 70.1% of the total energy in diet of any populations, and the next was fat and protein with 19.2 - 34.3% and 10.4 - 12.0%, respectively. Fat and protein devoted more proportion of energy than carbohydrate did in diet of the participants in developed and urban areas, and with higher income. Conclusion: Certain increases in energy from protein, alongside a significant rise in energy from fat and a visible decline in energy from carbohydrate, were documented as main features in dietary patterns of Northeastern Chinese during the study period.
Reproducibility and validity of a 45 item food-frequency questionnaire for inuit in Greenland

Abstract:Since 1993, dietary assessment has been carried out in Greenland as part of recurrent population health surveys. In preparation for the next survey in 2024, 91 participants from the survey in 2018 were selected for a validation study of the Food Frequency Questionnaire (FFQ). The 91 participants were reinterviewed 38–50 months after the first FFQ and invited to complete a food diary. As part of the 2018 survey, blood was analysed for mercury. The food diary was completed by 65 participants. The agreement between the two FFQ interviews was very good for macronutrients and fatty acids ( p > 0.20), whereas the calculated intake of mercury was 22% higher in the second FFQ ( p = 0.04) due to a higher intake of whale meat and muktuk (whale skin). The agreement between the second FFQ and the food diary was good for local food, imported meat and cakes/sweets/snacks but fruit and vegetables, dairy products, beverages and added sugar were significantly underreported in the food diary. Food items not included in the FFQ were identified from the food diaries. The correlation between the intake of marine mammals and blood mercury was moderate (Spearman's rho = 0.41–0.50; p < 0.0001). The results will inspire future dietary studies in the circumpolar North.

Can Dietary Fat Intake Modify Genetic Associations with Blood Lipid Level? Observations from a Swedish Cohort Study

Abstract:Humans have for a long time contemplated the link between diet and disease. As early as 400 BC, Hippocrates, commonly referred to as the father of western medicine, stated, “let food be thy medicine and let medicine be thy food”. Today, a poor diet is still considered one of the major risk factors for common complex diseases such as cardiovascular disease (CVD). CVD refers to a class of diseases that involves heart and blood vessels. According to WHO, CVD is one of the leading causes of morbidity and mortality in the industrialized world [8]. The cause of CVD is multifactorial, of which dyslipidemia plays a central role. Dyslipidemia is a state of elevated low density lipoprotein cholesterol (LDL-C), triglycerides, and/or reduced high density lipoprotein cholesterol (HDL-C). In the past, the treatment strategy of dyslipidemia has involved pharmacological intervention (e.g. statins or fibrates) to lower serum LDL-C and triglycerides. However, it is not until recently that the importance of diet and lifestyle are emphasized in clinical treatment and prevention of dyslipidemia. The classic ‘diet-heart’ hypothesis postulates that high intake of foods rich in fat raise the level of circulating cholesterol and triglycerides in the blood, thereby contributing to the risk of heart disease. However, recent research efforts have shown this hypothesis to be overly simplistic, where total fat intake as percentage of total energy intake is not as important as initially believed, whereas fat quality seems to be much more important. Studies have shown that replacement of saturated fat with mono- or poly- unsaturated fat appears to be beneficial for health whereas there seems to be no clear beneficial effect if saturated fat is replaced with (refined) carbohydrates. Like many other complex diseases, dyslipidemia and CVD are products of a genetic predisposition and environmental risk factors. Large-scale population genetics studies have identified 164 common genetic variants (single nucleotide polymorphisms [SNPs]) associated with lipid levels. It is plausible that environmental factors, including diet, physical activity, alcohol consumption and smoking, may interact with genetic variants to modify circulating lipid levels in the context of dyslipidemia. Studying gene-environment interactions may help explore novel mechanisms of disease, and inform future personalized nutrition/medicine based on genetic make-up. A new research field combining genetics, nutrition and biochemistry emerged in 2000, known as nutrigenetics, to study inter-individual variation in response to diet. If dyslipidemia can be effectively intervened upon through lifestyle, a large proportion of CVD cases could be delayed or even avoided. Furthermore, targeted diet and lifestyle interventions can protect against drug overconsumption, abuse or adverse side effects. Last but not least, compared to the massive costs related to disease diagnosis and treatment, the use of lifestyle interventions to control and prevent disease may prove to be a much more economical and effective way to improve public health interventions targeting populations susceptible to CVD and related traits. The Nunavik Inuit Health Survey comprises a small cohort of adult Inuits (N=553), in which interactions between 35 SNPs (in 20 genes) and components of dietary fat intake on plasma lipid levels were investigated]. 14 interactions with total fat intake, and 13 interactions with saturated fat intake were observed on blood lipid traits (total cholesterol, LDL-C, HDL-C and triglycerides), with an additive effect observed when looking at the combined genetic burden. It is meaningful to replicate the same analysis within a population of European ancestry to verify the reliability of the findings by Rudkowska et al., thus furthering our understanding on the biological mechanism of dyslipidemia.
Persistent organic pollutant exposures among Greenlandic adults in relation to lifestyle and diet: New data from the ACCEPT cohort

Abstract:High concentrations of persistent organic pollutants (POPs) in blood of the Greenlandic population are well known. The exposure is mainly through traditional food intake, including marine mammals and seabirds. The present study aimed to follow up on POP concentrations (organochlorine pesticides, polychlorinated biphenyls, per- and polyfluoroalkyl substances, and halogenated flame retardants (HFRs)) and relations to lifestyle and diet of the mothers included in the Greenlandic ACCEPT cohort (3-5 years after inclusion in 2013-15) and to include the children's fathers. This new data collection in 2019-20 included blood samples for measurement of POP concentrations and lifestyle and food frequency questionnaires from 101 mothers and 76 fathers aged 24-55 years living in Nuuk, Sisimiut, and Ilulissat, Greenland. The mothers' intra-individual median percentage decrease in POP concentrations from inclusion to this follow-up (3-5 years later) was 16-58%, except for mirex (0% change). Median concentrations of POPs were 1.4-4.6 times higher in fathers than in mothers. The POPs differed by residential town with generally higher concentrations in Ilulissat compared to Sisimiut and Nuuk. We report, for the first time, novel HFRs in human samples from Greenland. However, concentrations were low and only dechlorane plus (with its anti-isomer) was detected in >50% of the samples. Most POPs correlated positively with age and n-3/n-6 fatty acid ratio. The lipophilic POPs correlated positively with the percentage of life lived in Greenland, whereas few POPs correlated positively with BMI, income (personal and household), education, and alcohol intake. The POPs generally associated positively with the intake of marine mammals, seabirds, and dried fish, while few POPs associated positively with Greenlandic fish intake. In contrast, POPs generally associated negatively with imported meat products intake. The study findings may be of interest for future dietary recommendations in Greenland. We discuss the potential explanations for the findings and suggestions for future research.
Unique patterns of dietary adequacy in three cultures of Canadian Arctic indigenous peoples

Abstract:Abstract Background Information is needed on dietary adequacy of Arctic indigenous populations in Canada. Extensive work has been completed on composition of Arctic food and food use, and dietary reference intakes are available. Objective To complete the first comprehensive dietary adequacy assessment of three populations of adult Arctic indigenous people. Setting and subjects Dietary assessment interviews were conducted with randomly selected indigenous adults during two seasons in 44 representative communities of Yukon First Nations ( n = 797), Dene/Métis, ( n = 1007) and Inuit ( n = 1525). Methods Twenty-four-hour recalls were used to derive adjusted distributions of usual nutrient intakes in four age/gender groups for assessment of dietary adequacy for carbohydrate, dietary fibre, protein, n –3 fatty acids, n –6 fatty acids, calcium, copper, iron, magnesium, manganese, phosphorus, selenium, zinc, vitamin A, riboflavin, folate, vitamin B 6 , vitamin C, vitamin D and vitamin E. Results Nutrients with high prevalence of adequacy for most age/gender groups in all three cultures were protein, carbohydrate, n –3 fatty acids, iron, copper, zinc, manganese, selenium, riboflavin and vitamin B 6 ; some individuals exceeded the upper intake level for iron, zinc, selenium, vitamin A and vitamin D. Estimated average requirement nutrients of concern for adequacy were magnesium, folate, vitamin A, vitamin C and vitamin E; however, a few age/gender groups were exceptions. Prevalence of inadequacy for AI nutrients which may be undesirably high were fibre, n –6 fatty acids and calcium. Vitamin D was more adequate in Inuit women and men than for Yukon First Nations or Dene/Métis. Conclusions Unique patterns of dietary adequacy exist among Arctic indigenous peoples. Local wildlife food sources and market food sources should be maximised for their nutrient contributions to Arctic diets.

Traditional food consumption is associated with higher nutrient intakes in Inuit children attending childcare centres in Nunavik

Abstract:Objectives: To describe traditional food (TF) consumption and to evaluate its impact on nutrient intakes of preschool Inuit children from Nunavik. Design: A cross-sectional study. Methods: Dietary intakes of children were assessed with a single 24-hour recall (n=217). TF consumption at home and at the childcare centres was compared. Differences in children's nutrient intakes when consuming or not consuming at least 1 TF item were examined using ANCOVA. Results: A total of 245 children attending childcare centres in 10 communities of Nunavik were recruited between 2006 and 2010. The children's mean age was 25.0±9.6 months (11-54 months). Thirty-six percent of children had consumed at least 1 TF item on the day of the recall. TF contributed to 2.6% of total energy intake. Caribou and Arctic char were the most reported TF species. Land animals and fish/shellfish were the main contributors to energy intake from TF (38 and 33%, respectively). In spite of a low TF intake, children who consumed TF had significantly (p<0.05) higher intakes of protein, omega-3 fatty acids, iron, phosphorus, zinc, copper, selenium, niacin, pantothenic acid, riboflavin, and vitamin B12, and lower intakes of energy and carbohydrate compared with non-consumers. There was no significant difference in any of the socio-economic variables between children who consumed TF and those who did not. Conclusion: Although TF was not eaten much, it contributed significantly to the nutrient intakes of children. Consumption of TF should be encouraged as it provides many nutritional, economic, and sociocultural benefits.

Abstract:The article is the first in the series of four that present the results of a study on environmental contaminants in coastal Chukotka, conducted in the context of a multi-disciplinary investigation of indigenous foodways in the region of the Bering Strait. We provide an overview of the contemporary foodways in our study region and present the results of the survey on the consumption of locally harvested foods, carried out in 2016 in the Chukotkan communities of Enmelen, Nunligran, and Sireniki. The present results are evaluated in comparison to those of the analyses carried out in 2001–2002 in the village of Uelen, located further north. Where appropriate, we also draw comparative insight from the Alaskan side of the Bering Strait. The article sets the stage for the analyses of legacy persistent organic pollutants (POPs) and metals to which the residents become exposed through diet and other practices embedded in the local foodways, and for the discussion of the Recommended Food Daily Intake Limits (RFDILs) of the food that has been sampled and analyzed in the current study.
Association of food insecurity with dietary patterns and expenditure on food, alcohol and tobacco amongst indigenous Inuit in Greenland: results from a population health survey

Abstract:Abstract Background Amongst the indigenous Greenlandic Inuit, the experience of food insecurity has been attributed to a lack of money to buy enough food of sufficient quality to sustain a family, although a preference for alcohol and tobacco over food has also been cited. The purpose of the article was to compare dietary patterns and expenditure on food, alcoholic beverages and tobacco between survey participants who reported food insecurity and those who did not. Methods A countrywide cross-sectional health survey was carried out among 1886 adult Greenlandic Inuit in 2018. Diet was estimated by a food frequency questionnaire. Food insecurity status was based on the household hunger scale. Analyses were carried out by univariate general linear models adjusted for age, sex and social position. Results Nine percent of the participants reported food insecurity. Food insecurity was higher among younger participants, men and participants with low social position. Food insecure participants more often chose an unhealthy dietary pattern (43% vs. 32%) and they reported a higher energy intake. The food insecure spent the same amount of money on food as other participants but less on nutritious food and more on non-nutritious food. The cost per kilojoule (kJ) of the food of the food insecure was lower than that of the food secure (DKK 8.0 and 9.0 per 1000 kJ, respectively). The food insecure participants also spent considerably more on alcohol and tobacco. Conclusions The results suggest that it is not only unemployment and lack of money that creates food insecurity and unhealthy dietary patterns in Greenland. Food insecure participants gave higher priority to buying non-nutritious food, alcohol and tobacco than did food secure participants. There seems to be at least two population subgroups in Greenland with poverty and substance use, respectively, as the immediate determinants for food insecurity. The results are important for the design of interventions against food insecurity and unhealthy dietary patterns.

Hexachlorobenzene and omega-3 fatty acid intake from traditional foods in the northern Yukon: A risk and benefit analysis

Abstract:A human biomonitoring study was conducted in the community of Old Crow, Yukon, in 2019, finding that levels of hexachlorobenzene (HCB) in plasma were elevated in the community relative to the general Canadian population. The aim of this study was to estimate dietary intake of both hexachlorobenzene, and the nutrient omega-3 fatty acids from locally harvested traditional foods in Old Crow, with the aim of identifying possible regional sources of exposure. A stochastic model was constructed to estimate intake of both hexachlorobenzene and the omega-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). Average predicted hexachlorobenzene exposure levels from traditional food consumption in Old Crow were below the tolerable daily intake of 1600–1700 ng/kg body weight/day in both average (18 ng/kg body weight/day) and short-term maximum (27 ng/kg body weight/day) exposure models. The primary contributors to average hexachlorobenzene intake were caribou fat, bone marrow, ribs, and kidneys, and Chinook salmon muscle. Average estimated dietary EPA + DHA intake levels from traditional foods were below the recommendation of 2.1 to 3.2 g of EPA + DHA per week in the average (1.6 g/week) exposure model, but above this recommendation in the short-term maximum model (3.3 g/week). The primary contributors to average EPA + DHA intake were the meat of Chinook, coho, and, chum salmon muscle, and whitefish muscle and eggs. The results of this study support the message that traditional foods continue to be an important source of nutrients and other health benefits and that the health benefits of traditional foods generally outweigh contaminant risks.

Abstract:The 2021 Arctic Monitoring Assessment Program (AMAP)'s Human Health Assessment report presents a summary of the presence of contaminants in human populations across the circumpolar Arctic and provides an update to the previous assessment released in 2015. The primary objective of this paper is to summarise some of these findings by describing the current levels of metals across the Arctic, including key regional and temporal trends based on available national data and literature, and highlight knowledge gaps. Many Arctic populations continue to have elevated levels of these contaminants, and the highest levels of mercury (Hg) were observed in populations from Greenland, Faroe Islands, and Nunavik (Canada). Still, concentrations of several metals are declining in Arctic populations in regions where time trends data exist, although the declines are not consistent across all regions. The 2021 AMAP human health assessment report and this paper provide an extensive summary of levels of metals and trace elements in adults, pregnant women, and children across the Arctic.

Abstract:Understanding ethnic variations in body composition is crucial for assessing health risks. Universal models may not suit all ethnicities, and there is limited data on the Inuit population. This study aimed to compare body composition between Inuit and European adults using computed tomography (CT) scans and to investigate the influence of demographics on these measurements. A retrospective analysis was conducted on 50 adults (29 Inuit and 21 European) who underwent standard trauma CT scans. Measurements focused on skeletal muscle index (SMI), various fat indices, and densities at the third lumbar vertebra level, analyzed using the Wilcoxon-Mann-Whitney test and multiple linear regression. Inuit women showed larger fat tissue indices and lower muscle and fat densities than European women. Differences in men were less pronouncehd, with only Intramuscular fat density being lower among Inuit men. Regression indicated that SMI was higher among men, and skeletal muscle density decreased with Inuit ethnicity and age, while visceral fat index was positively associated with age. This study suggests ethnic differences in body composition measures particularly among women, and indicates the need for Inuit-specific body composition models. It higlights the importance of further research into Inuit-specific body composition measurements for better health risk assessment.

The Nordic Nutrition Recommendations 2022 - food consumption and nutrient intake in the adult population of the Nordic and Baltic countries

Abstract:Background: Knowledge about the nutrient intakes and food consumption in the Nordic and Baltic countries is important for the formulation of dietary reference values (DRVs) and food-based dietary guidelines (FBDGs), as part of the Nordic Nutrition Recommendations 2022 project (NNR2022). Objective: To describe nutrient intake and food consumption at a broad level in the adult population of each Nordic and Baltic country. This paper also provides guidance on where to find more information on the nutrient intake and food consumption reported from each country. Design: Information about the dietary surveys as well as the daily mean intakes was retrieved from the national dietary surveys in each of the Nordic and Baltic countries. Tabulation of the population intakes divided by sex for macronutrients, 20 micronutrients, and for the following broader food groups, Beverages, Cereals, Potatoes, Vegetables, Fruits and berries, Fish and seafood, Meat and meat products, Milk and dairy products, Cheese, Eggs, Fats and oils, and Sweets and sweet bakery products, was done. Results and discussion: The Nordic and Baltic countries share not only similarities but also differences in food consumption patterns, which is reflected in differences in average food consumption and nutrient intakes between the countries. This may be related to the dietary assessment method, prevalence of misreporting, and participation rates in the different dietary surveys. Other factors that may play a role are differences in the calculation procedures in the food composition databases and the definition of food groups. Conclusion: The nutrient intake and, especially, food consumption differ between the Nordic and Baltic countries because of differences in food patterns and factors related to the dietary surveying, food grouping, and calculation procedures in each country. To facilitate future comparisons between countries, it would be of interest to harmonize food groupings and the age groups reported on.
Environmental impact of Norwegian self-selected diets: comparing current intake with national dietary guidelines and EAT-Lancet targets

Abstract:Objectives: Dietary environmental impact in a Norwegian adult population was estimated for six environmental impact categories. Moreover, environmental benefits of scenario diets complying with the Norwegian Food-Based Dietary Guidelines (FBDG) and the EAT-Lancet reference diet were assessed. Design: The current diet of Norwegian adults was estimated according to 24-hour dietary recall data from a national dietary surveillance survey (Norkost 3). Scenario diets were modelled to represent the Norwegian FBDG and the EAT-Lancet healthy reference diet. Dietary environmental impact in terms of global warming potential, freshwater and marine eutrophication, terrestrial acidification, water use, and transformation and use of land, was estimated for the current and scenario diets using environmental impact data representative of the Norwegian market. Significant associations between impact and gender/educational attainment were assessed at P <0.05. Setting: Norway. Participants: Adults (n=1,787) aged 18-70 years who participated in the Norkost 3 survey (2010-2011). Results: Environmental impact varied significantly by gender and educational attainment. The food groups contributing most to environmental impact of Norwegian diets were meat, dairy, beverages, grains, and composite dishes. Compared to the current Norwegian diet, the FBDG scenario reduced impacts from 2% (freshwater eutrophication) to 32% (water use), while the EAT-Lancet scenario reduced impacts from 7% (marine eutrophication) to 61% (land use). The EAT-Lancet scenario resulted in 3-48% larger reductions in impact than the FBDG scenario. Conclusions: The Norwegian FBDG, while not as environmentally friendly as the EAT-Lancet reference diet, can still be an important tool in lessening environmental burden of Norwegian diets.

Abstract:Gastrointestinal function plays a pivotal role in nutrient absorption and overall digestive health. Abnormal gastric emptying is closely linked to type 2 diabetes, impacting blood glucose regulation and causing gastrointestinal symptoms. This study aims to investigate and compare segmental transit times, motility indices, and micromilieu between Greenlandic Inuit and Danish individuals with and without type 2 diabetes. We included forty-four Greenlandic Inuit, twenty-three of whom had type 2 diabetes, and age and gender-matched Danish individuals. Segmental transit time, motility, and luminal environment were measured using the SmartPill®. Greenlandic controls displayed shorter gastric emptying time (GET) (163 min), higher gastric median pH (2.0 pH) and duodenal median contractions (18.2 mm Hg) compared to Greenlanders with type 2 diabetes (GET: 235 min, pH:1.9, median duodenal contraction 18.4 mm Hg) and Danish controls (GET: 190, pH:1.2 median duodenal contraction 17.5 mmHg). Despite similar anti-diabetic management efforts, variations in gastrointestinal physiology were evident, highlighting the complexity of diabetes and its interaction with ethnicity, suggesting potential dietary or even genetic influences, emphasising the necessity for personalised diabetes management approaches. Finally, the study opens possibilities for future research, encouraging investigations into the underlying mechanisms linking genetics, diet, and gastric physiology, as an understanding of factors can lead to more effective, tailored strategies for diabetes care and improved digestive health in diverse populations.

Abstract:Sir—The transverse abdominis plane (TAP) block provides significant surgical and long-lasting postoperative analgesia in neonates and infants undergoing unilateral laparotomy (1,2). To date, there are no reports on using subcostal TAP block for major pediatric upper abdominal surgery via supra-umbilical transverse incision. We would therefore like to present the case of a 5-month-old girl (6.4 kg), who underwent a Bianchi procedure (3). Epidural analgesia for pain control was discussed, but declined by the parents. She was born at term via cesarean section for breech presentation in labor and underwent laparotomy on day two of life for intestinal obstruction. Atretic bowel from proximal jejunum to mid-transverse colon was found, and jejunocolic anastomosis was performed. Her postoperative recovery was complicated by shortbowel syndrome, and she was transferred to our institution for surgical management. Anesthesia was induced with 8% sevoflurane in oxygen. IV access was obtained, and rocuronium at a dose of 1 mgÆkg was given to facilitate endotracheal intubation. Bilateral subcostal TAP blocks (4) were sited under ultrasound guidance with a SonoSite Micromaxx portable ultrasound and 13–6 MHz 25 mm ‘hockeystick’ probe. Needle and probe were positioned as illustrated in Figure 1. Once the fascia dividing the transverse abdominis muscle and the internal oblique was identified, a 21 G hypodermic needle was inserted at an angle no >10 to the skin to enhance sonographic visibility. Care was taken to ensure the needle tip was visualized prior to needle advancement to reduce the risk of peritoneal puncture. Needle position in the TAP was confirmed by injection of a small amount of local anesthetic. The needle was subsequently levelled to an angle of zero degrees and its tip visualized during further advancement along the costal margin A total of 0.5 mlÆkg of 0.25% levobupivacaine was deposited onto each side. Anesthesia was maintained with isoflurane at an end-tidal concentration of 1.0% in a 30% oxygen and air mixture throughout. Intermittent boluses of rocuronium at a dose of 0.5 mgÆkg were used to facilitate positive pressure ventilation. Surgery was performed via a transverse supra-umbilical laparotomy with incision extending from left to right anterior axillary line. Prior to the induction of anesthesia, the heart rate was 135 b.p.m and blood pressure 79/40 mmHg. There was no physiological response to skin incision. Both heart rate and blood pressure remained within 20% of baseline values throughout the case. Occasional boluses of fentanyl at a dose of 1–2 mcgÆkg were used to cover the visceral pain not covered by the TAP block. In total, 10 mcgÆkg of fentanyl was used throughout the 6-h procedure. After skin closure, the child was extubated awake in the operating theater prior to transfer to the recovery area. Nurse-controlled analgesia (NCA) with morphine was commenced at a background infusion rate of 4 mcgÆkgÆh as opposed to our standard rate of 10 mcgÆkgÆh. Bolus dose and lockout were left unchanged at 20 mcgÆkg and 20 min, respectively. Regular intravenous paracetamol was prescribed at a dose of 15 mgÆkg 6 hourly. Pain scores via the FLACC scoring system were recorded hourly for the first 37 h, at which point the NCA was discontinued because of minimal usage. Twelve morphine boluses were administered over this period, giving a total of 388 mcgÆkg. Morphine consumption for the first 24 h was 11.5 mcgÆkgÆh. Maximal pain score recorded was 2 on the scoring system. Abstract:OBJECTIVE: In remote, Alaska Native communities, traditional foods remain a significant source of essential nutrients and appear to protect against the development of chronic diseases. Relatively low intake of traditional foods among Alaska Native children is therefore of concern. The aim of this study was to identify household and parental predictors of child traditional food (TF) consumption and weight in remote Yup'ik communities of Alaska.DESIGN: Children (10-18 years old) and parents in two communities (populations <500) were invited to participate in this cross-sectional study. Intake of traditional foods among children and parents was estimated from two-24 h recalls using NDS-R. Weight and height were measured and BMI calculated. Sociodemographic factors, including income and education, were collected from parents. A partial least square path modeling analysis and bootstrapping were performed to identify predictors of child TF consumption and weight.RESULTS: Parental intake of traditional foods, Yup'ik identity and income were positively associated with child intake of traditional foods. Further, parental intake of traditional foods predicted lower child BMI. Parental education was negatively associated with child traditional food intake and positively associated with child BMI.CONCLUSIONS: Findings suggest that interventions targeting parents may be an effective strategy to increase intake of traditional foods and improve diet quality among Alaska Native youth.

Abstract:Arctic Char (Salvelinus alpinus) is a salmonid fish that is the second-most consumed country food species by Nunavimmiut. Its nutritional quality is determined by omega-3 fatty acids and carotenoid pigments. Those molecules cannot be synthetized by fish and must be acquired through diet. We sampled Arctic Char in 10 rivers from three marine coastal regions of Nunavik (Hudson Bay, Hudson Strait and Ungava Bay) and described diet (stable isotopes δ13C & δ15N) and flesh quality (fatty acids and carotenoids) and assessed associations between diet and flesh quality (including also bio-impedance and colorimetry). Our results suggested inter-regional differences in the diet and nutritional quality of Arctic Char in Nunavik where δ13C values indicated that diet in Hudson Bay was more pelagic, while in Ungava Bay it was more coastal. We also observed inter-regional differences in omega-3 fatty acids and astaxanthin, the pigment responsible for the redness of the flesh color, where concentrations were highest in Ungava Bay Arctic Char. In all sampling locations, Arctic Char were an exceptional source of omega-3 fatty acids and astaxanthin, confirming its importance as a high-quality wild food. Our models suggest that astaxanthin, canthaxanthin and water content influence flesh redness. Our data highlight inter-regional differences that could be taken in consideration to better predict the impact of climate change on fish quality and, ultimately, on Inuit diet and health.
