Withdrawal reaction from Janus kinase inhibitor manifesting as hepatitis B virus outbreak: a case report

Abstract:A 60-year-old Chinese female was admitted to our hospital with joint swelling and pain for over 6 years and abnormal liver function for 1 day. She had been diagnosed with rheumatoid arthritis (RA) 6 years previously. A test for hepatitis B virus (HBV), performed over a year ago, was normal (). She was prescribed methotrexate (MTX) 10 mg weekly and tofacitinib (TOF) 5 mg twice daily, after which her joint pain improved continuously, with normal liver function tests every 2 months. Two months before the current admission, she had undergone a lymph-node biopsy at a local hospital owing to enlarged right axillary lymph nodes. Pre-biopsy liver function tests were normal, with hepatitis B surface antibody (HBsAb) 9.4 mIU/L (). The biopsy suggested angioimmunoblastic T-cell lymphoma, potentially related to MTX. MTX and TOF were abruptly discontinued, and she was treated with prednisone 5 mg daily. After changing the treatment, the patient experienced worsening symptoms of poor appetite, fatigue, and jaundice, and was hospitalized. On admission, she was alert without hepatomegaly. Laboratory tests showed severe liver dysfunction: alanine aminotransferase (ALT) 1183 U/L (normal 7–40), aspartate aminotransferase (AST) 1011 U/L (normal 13–35), gamma-glutamyl transferase (γ-GT) 411.60 U/L (normal 7–45), alkaline phosphatase (ALP) 195 U/L (normal 50–135), and total bilirubin (Tbil) 27.60 mol/L (normal 0–21). Repeat HBV tests revealed hepatitis B surface antigen (HBsAg) 624.291 IU/mL (normal <0.30), HBsAb 4.0 mIU/mL (normal <10), HBeAg 0.204 IU/mL (normal <0.25), HBeAb 6.000 IU/mL (normal <0.8), hepatitis B core antibody (HBcAb) 80 000 PEI U/mL (normal <1.8), and HBV-DNA 9.589 × 10 5 IU/mL (normal <5 × 10 2 ). Tests for hepatitis A, C, and E, cytomegalovirus, rubella virus, toxoplasma, herpes simplex virus, Epstein–Barr virus (EBV) shell membrane antigen antibody immunoglobulin M (IgM), EBV early antigen antibody IgG, self-exempted liver antigen profile assay, anti-nuclear antibody assay, copper blue protein, coagulation function, urine routine, C-reactive protein, complement, immunoglobulin, serum albumin, and serum white/globulin ratio were all negative.
