Abstract:To understand, in greater detail, the molecular mechanisms regulating the complex relationship between mechanical strain and alveolar bone metabolism during orthodontic treatment, passive cross-arch palatal springs were bonded to the maxillary molars of 6-wk-old rats, which were killed after 4 and 8 d. Outcome measures included serum assays for markers of bone formation and resorption and for the multifunctional hormone leptin, and histomorphometry of the inter-radicular bone. The concentration of the bone-formation marker alkaline phosphatase (ALP) was significantly reduced at both time points in the appliance group, accompanied by a 50% reduction in inter-radicular bone volume; however, osteocalcin (bone Gla protein) levels remained unaffected. Bone collagen deoxypyridinoline (DPD) crosslinks increased 2.3-fold at 4 d only, indicating a transient increase in bone resorption; in contrast, the level of the osteoclast-specific marker, tartrate-resistant acid phosphatase 5b (TRACP 5b), was unchanged. Leptin levels closely paralleled ALP reductions at both time points, suggesting an important role in the mechanostat negative-feedback loop required to normalize bone mass. These data suggest that an orthodontic appliance, in addition to remodeling the periodontal ligament (PDL)-bone interface, may exert unexpected side-effects on the tooth-supporting alveolar bone, and highlights the importance of recognizing that bone strains can have negative, as well as positive, effects on bone mass.

Abstract:Orthodontic treatment utilizes the remodeling of the alveolar bone in response to externally applied orthodontic force to a tooth. The rate of bone resorption is, therefore, one of the essential factors for understanding tooth movement during orthodontic treatment. There is the basic concept of differential force in orthodontics that an optimal force moves the canine without the movement of the molar, which is used for an anchor of the canine retraction. In this paper, the resorption rates of the alveolar bone around the molar and the canine were estimated from the clinical measurements of tooth movement and the stress analyses of tooth roots using a 3-D finite element model. The model consisted of an orthodontic spring, the canine, the second premolar, and the first and second molars. The bone resorption rate to unit stress at the molar was found to be about 0.5μm/(kPa·day) and almost the same as that of the canine. This result suggests that the concept of differential force can be described only by the difference in geometry of the roots of moved and fixed teeth.

Abstract:Abstract The structural process of bone and periodontal ligament (PDL) remodeling during long-term orthodontic tooth movement (OTM) has not been satisfactorily described yet. Although the mechanism of bone changes in the directly affected alveolar bone has been deeply investigated, detailed knowledge about specific mechanism of PDL remodeling and its interaction with alveolar bone during OTM is missing. This work aims to provide an accurate and user-independent analysis of the alveolar bone and PDL remodeling following a prolonged OTM treatment in mice. Orthodontic forces were applied using a Ni–Ti coil-spring in a split-mouth mice model. After 5 weeks both sides of maxillae were scanned by high-resolution micro-CT. Following a precise tooth movement estimation, an extensive 3D analysis of the alveolar bone adjacent to the first molar were performed to estimate the morphological and compositional parameters. Additionally, changes of PDL were characterized by using a novel 3D model approach. Bone loss and thinning, higher connectivity as well as lower bone mineral density were found in both studied regions. Also, a non-uniformly widened PDL with increased thickness was observed. The extended and novel methodology in this study provides a comprehensive insight about the alveolar bone and PDL remodeling process after a long-duration OTM.

Abstract:The tooth movement in the alveolus is possible due to bone remodeling. This process could be the risk factor for the formation of gingival recessions—the most common side effects of orthodontic therapy. Gingival recessions are found 5.8–11.5% more frequently among the orthodontically treated patients. What is more, anterior mandibular teeth are the ones most prone to gingival recession dehiscences and fenestrations. The aim of this narrative review was to evaluate, based on CBCT (Cone beam computed tomography) scans, the changes in the alveolar bone of lower incisors in adolescent and adult patients after orthodontic tooth movements. From the pool of 108 publications, a total of 15 fulfilled the criteria of this review. Both retrospective and prospective longitudinal studies—using CBCT or CT (Computed Topography) and evaluating alveolar bone changes in mandibular incisors during orthodontic treatment performed before and after teeth movement—were included. In the group of growing patients, either proclination or retroclination of mandibular incisors led to increase of the distance from CEJ (cementoenamel junction) to marginal bone crest. The difference in bone loss was greater on the lingual side of the incisors in both types of tooth movement. The results were similar for adults patients. The thickness of the alveolar bone was reduced after proclination (total bone thickness) among growing and non-growing patients and retraction (lingual and buccal) of lower anterior teeth in the group of growing patients. The only improvement was measured for buccal thickness of mandibular incisor in bimaxillary protrusion patients treated with extraction therapy. The control of retraction movement (more root than crown movement) enhanced preservation on bone height and thickness. In order to minimize possible deterioration and place teeth in the center of alveolus, CBCT monitoring and scrupulous clinical evaluation are recommended.

Abstract:Bone remodeling in orthodontic treatment has been one of the most concerned issues. The purpose of this study was to characterize the changes in bone remodeling on both labial and lingual aspects of the anterior teeth during treatment and retention. Data of 55 extraction cases (41 females and 14 males) were collected at pretreatment, post-treatment, and follow-up stages. Bone thickness on both labial and lingual aspects of the upper and lower incisors were measured at the level of the center of resistance (UC and LC) and that of 3 mm apical to the upper and lower incisors’ center of resistance (UAC and LAC). For LC, with the significant lingual movement of point LIR (center of resistance of lower incisor; P < 0.001), no statistically significant changes in alveolar width on the labial aspect were detected, whereas the reduction of lingual alveolar width was statistically significant ( P < 0.001). In retention, no significant movement of point LIR appeared to occur, while an increase in alveolar width on the lingual aspect was found at this stage. For LAC, the changes in alveolar width as well as the displacement of point LIA (apex of lower incisor) were similar to those of LC. On the maxilla, with the control lingual tipping movement of upper incisor, most of the anterior alveolar changes surrounding the upper incisor were analogous to those of lower incisor. But differently, during treatment, labial bone thickness was increased significantly as the upper incisor moved lingually, and in retention, no statistically significant changes were found on both labial and lingual bone thickness as the upper incisor labially proclined. It was concluded that, with lingual movement of the incisors, bone apposition on the lingual aspect may lag behind the incisor movement. In retention, the lower incisors are in a stable position, with sequential bone apposition being presented on the lingual aspect. However, the upper incisors turn out to be labially relapsed along with the lingual bone resorption and labial bone apposition occurring simultaneously.

Abstract:PURPOSE:Functional stimulus during orthodontic tooth movement into the grafted bone can lead to better alveolar bone grafting outcomes. The aim of this study was to analyze the biological effects of orthodontic tooth movement into the grafted alveolar cleft area with histologic staining, fluorescence staining, and real-time polymerase chain reaction (PCR).MATERIALS AND METHODS:An animal model of orthodontic tooth movement into the grafted alveolar cleft area was established in 8-week-old Sprague-Dawley rats. The animals were divided into the experimental group and the control group. Four checkpoints were observed: before orthodontic stimuli, day 1 after orthodontic stimuli, day 3 after orthodontic stimuli, and day 5 after orthodontic stimuli. The cleft bone formation conditions, including the collagen fibers and the activities of the osteoclasts and osteoblasts, were evaluated by histologic staining. The expression of tartrate-resistant acid phosphatase (TRAP), receptor activator nuclear factor κB ligand, and Runt-related transcription factor 2 was detected by real-time PCR in both groups.RESULTS:Hematoxylin-eosin staining showed that the remodeling process of iliac autografts was completed when the orthodontic stress was applied, whereas the bone tissues first showed osteoclastogenesis and then osteogenesis. On the basis of TRAP staining, the osteoclasts increased to the maximal amount on day 3 and decreased thereafter. Evidence from tetracycline fluorescence staining indicated that no obvious changes in osteoblast activity were detected at the early stage; however, it gradually increased, especially in the region close to the root surface. According to real-time PCR, the expression of TRAP increased in both the early and middle stages, that of receptor activator nuclear factor κB ligand increased in the early stage, and that of Runt-related transcription factor 2 increased in the late stage. Moreover, the results showed significant differences between the experimental and control groups.CONCLUSIONS:Orthodontic tooth movement into the alveolar cleft bone graft area promoted bone remodeling of embedded bone, thus inducing bone resorption and subsequent deposition.

Abstract:The alveolar bone, with a high turnover rate, is the most actively-remodeling bone in the body. Orthodontic tooth movement (OTM) is a common artificial process of alveolar bone remodeling in response to mechanical force, but the underlying mechanism remains elusive. Previous studies have been unable to reveal the precise mechanism of bone remodeling in any time and space due to animal model-related restrictions. The signal transducer and activator of transcription 3 (STAT3) is important in bone metabolism, but its role in osteoblasts during OTM is unclear. To provide in vivo evidence that STAT3 participates in OTM at specific time points and in particular cells during OTM, we generated a tamoxifen-inducible osteoblast lineage-specific Stat3 knockout mouse model, applied orthodontic force, and analyzed the alveolar bone phenotype. Micro-computed tomography (Micro-CT) and stereo microscopy were used to access OTM distance. Histological analysis selected the area located within three roots of the first molar (M1) in the cross-section of the maxillary bone as the region of interest (ROI) to evaluate the metabolic activity of osteoblasts and osteoclasts, indicating the effect of orthodontic force on alveolar bone. In short, we provide a protocol for using inducible osteoblast lineage-specific Stat3 knockout mice to study bone remodeling under orthodontic force and describe methods for analyzing alveolar bone remodeling during OTM, thus shedding new light on skeletal mechanical biology.
Local orthodontic force initiates widespread remodelling of the maxillary alveolar bone

Abstract:Objectives: To clarify the effects of a local orthodontic force on alveolar bone by analysing bone remodelling in different regions of the maxilla during orthodontic tooth movement (OTM). Methods: An OTM model was established in rats. Histological changes in the maxilla were analysed using TRAP staining, IHC staining for CTSK and haematoxylin and eosin (H and E) staining. The root bifurcation region of the alveolar bone of the first (M1), second (M2) and third (M3) molars were selected as the regions of interest (ROIs), which were further divided into a cervical and an apical level. Sequential fluorochrome labelling was performed to analyse bone deposition rates. Results: The maxillary left first molars were moved mesially. TRAP staining and IHC staining for CTSK showed orthodontic force increased osteoclast numbers in all six ROIs at both the cervical and apical levels. H and E staining indicated elevated osteoblast numbers in the OTM group in all induced regions. Sequential fluorochrome labelling exhibited increased bone deposition rates around M1, M2 and M3 in the OTM group. Conclusions: An orthodontic force applied to the first molar could initiate widespread remodelling of the maxillary alveolar bone, which was not restricted to the tension and pressure sites. This may revise the orthodontic biomechanical theory and provide new insights for clinical work.
Comparison of Orthodontic Tooth Movement of Regenerated Bone Induced by Carbonated Hydroxyapatite or Deproteinized Bovine Bone Mineral in Beagle Dogs

Abstract:Orthodontic treatments often involve tooth movement to improve dental alignment. In this study, we aimed to compare tooth movement in regenerated bone induced by two different bone fillers, carbonated hydroxyapatite (CAP) and deproteinized bovine bone mineral (DBBM). Four beagle dogs were used in this comparative study. The first, second, and fourth lower mandibular premolars (P1, P2, and P4) on both sides of the mouth were extracted, and CAP was implanted into the extraction site on the left side and DBBM into the right side. Following regenerative bone healing, orthodontic devices were attached to perform orthodontic tooth movement of the lower third mandibular premolar (P3) on both sides. X-ray examination, intraoral scan, and histological analysis were performed. The Mann-Whitney U test was used for statistical analysis, and p < 0.05 was considered significant. Bone regeneration and orthodontic tooth movement were observed in the CAP and DBBM groups. Histologically, normal periodontal tissue remodeling was observed on the compression and tension sides of CAP and DBBM. No statistical difference was observed in the number of osteoclasts around the periodontal ligament and the root resorption area. Orthodontic tooth movement of regenerated bone induced by CAP and DBBM was therefore achieved.

Abstract:The purpose of this review is to provide an update on the changes at the cellular and tissue level occurring during orthodontic force application. For the understanding of this process, knowledge of the mechanobiology of the periodontal ligament and the alveolar bone are essential. The periodontal ligament and alveolar bone make up a functional unit that undergoes robust changes during orthodontic tooth movement. Complex molecular signaling is responsible for converting mechanical stresses into biochemical events with a net result of bone apposition and/or bone resorption. Despite an improved understanding of mechanical and biochemical signaling mechanisms, it is largely unknown how mechanical stresses regulate the differentiation of stem/progenitor cells into osteoblast and osteoclast lineages. To advance orthodontics, it is crucial to gain a better understanding of osteoblast differentiation from mesenchymal stem/progenitor cells and osteoclastogenesis from the hematopoietic/monocyte lineage.

Abstract:Orthodontic tooth movement (OTM) relies on mechanical force-induced bone remodeling. As a metabolic intermediate of glycolysis, lactate has recently been discovered to participate in bone remodeling by serving as a signaling molecule. However, whether lactate could respond to mechanical stimulus during OTM, as well as whether lactate has an impact on the alveolar bone remodeling during orthodontics, remain to be further elucidated. In the current study, we observed physiologically elevated production of lactate along with increased osteogenic differentiation, proliferation, and migration of alveolar bone marrow mesenchymal cells (ABMMCs) under mechanical force. Inhibition of lactate, induced by cyclic mechanical stretch by GNE-140, remarkably suppressed the osteogenic differentiation, proliferation, and migration, yet enhanced apoptosis of ABMMCs. Mechanistically, these regulatory effects of lactate were mediated by histone lactylation. Taken together, our results suggest that force-induced lactate is involved in controlling bone remodeling-related cellular activities in ABMMCs and plays a vital role in the alveolar bone remodeling during OTM. Our findings indicate that lactate might be a critical modulator for alveolar bone remodeling during OTM, providing a novel therapeutic target for the purpose of more effectively controlling tooth movement and improving the stability of orthodontic results.

Abstract:The maxilla of a deceased 19-year-old young woman who had been treated with an edgewise appliance was removed during autopsy. The specimen was prepared histologically in the horizontal plane. The type of tooth movement was reconstructed by comparing the treatment documents at outset (photograph of dental cast, radiograph) with the photograph and radiographs of the specimen. This permitted the histological findings to be correlated to the type of tooth movement. Results: the localization and extent of tissue changes at the roots depend on the type of tooth movement and the structure of the bone. In case of an atrophied alveolar bone in front of the tooth movement direction, a partial increase of osseous tissue may be induced by bone apposition in the subperiosteal layer. After tooth movement in the maxillary sinus region, however, bone resorption was found in the subperiosteal layer in front of the roots. The histologic findings are more pronounced than the radiographs would suggest. Histologically verified bony dehiscences or fenestrations in the facial or oral cortical plate could not be diagnosed by macroscopic inspection of the specimen.

Abstract:This study aimed to assess the alveolar bone changes following maxillary and mandibular molars’ intrusion and extrusion movements using Clear Aligners using Cone-Beam Computed Tomography (CBCT). This is a retrospective clinical study in which 24 adult patients with pre-designed selection criteria and a mean age of 31.1 + 9.9 years were enrolled. Abstract:Orthodontic tooth movement is dependent upon the remodeling of the periodontal ligament and alveolar bone by mechanical means. Facial sutures are also fibrous articulations, and by remodeling these joints, one can alter the positional relationships of the bones of the facial skeleton. As might be expected from the structure and mobility of the temporomandibular joint (TMJ), this articulation is more resistant to mechanical deformation, and whether functional mandibular displacement can alter the growth of the condyle remains controversial. Clinical investigations of the effects of the Andresen activator and its variants on dentofacial growth suggest that the changes are essentially dento-alveolar. However, with the popularity of active functional appliances, such as the Herbst and twin-block based on ’jumping the bite’, attention has focused on how they achieve dentofacial change. Animal experimentation enables informed decisions to be made regarding the effects of orthodontic treatment on the facial skeleton at the tissue, cellular, and molecular levels. Both rat and monkey models have been widely used, and the following conclusions can be drawn from such experimentation: (1) Facial sutures readily respond to changes in their mechanical environment; (2) anterior mandibular displacement in rat models does not increase the mitotic activity of cells within the condyle to be of clinical significance, and (3) mandibular displacement in non-human primates initiates remodeling activity within the TMJ and can alter condylar growth direction. This last conclusion may have clinical utility, particularly in an actively growing child.

Alveolar Bone Changes In Maxillary And Mandibular Anterior Teeth During Orthodontic Treatment: A Systematic Review And Meta-Analysis

Abstract:The association between tooth movement and remodelling of surrounding bone is controversial. To analyse the effect of tooth movement on alveolar bone changes in maxillary and mandibular anterior teeth by cone-beam computed tomography (CBCT). The Embase, Cochrane Library and Medline databases were searched without any language restrictions. Longitudinal studies using CBCT to observe alveolar bone changes of maxillary and mandibular anterior teeth during orthodontic treatment were included. Two independent reviewers performed the study selection, data extraction and methodological quality assessment. A total of 26 studies were included in this review, 14 of which were eligible for quantitative synthesis. In extraction cases, meta-analysis showed vertical bone loss on the labial (0.36 mm) and lingual (0.94 mm) sides of maxillary incisors, and lingual bone thickness decreased significantly at the cervical level (0.57 mm). In non-extraction cases, vertical alveolar bone loss was significant on the labial side (0.97 mm) and lingual side (0.86 mm) of mandibular incisors. Subgroup analysis for skeletal class III patients indicated that vertical alveolar bone loss was 1.16 mm on the labial side and 0.83 mm on the lingual side of mandibular incisors. The absence of high-quality studies and the high heterogeneity of the included studies were limitations of this systematic review and meta-analysis. Based on limited evidence, alveolar bone height and thickness, especially at the cervical level, decreased during both labial and lingual movement of anterior teeth.

Abstract:PURPOSE:To study the biological effects of orthodontic tooth movement into surgical site on bone remodeling after bone graft.METHODS:Twelve SD rats were used as experimental animals.Critical size defects (CSD) were established on maxillary alveolar bone in SPF SD rats (8-week-old,200+20g).3D printed polycaprolactone scaffolds and rat bone marrow mesenchymal stem cells were co-cultured in vitro and then were used to be grafted into CSDs as substitution of autogenous bone.Eight weeks after surgery,3 rats were sacrificed to take samples.H-E staining was used to evaluate the osteogenesis.Meanwhile,orthodontic devices were used to move adjacent teeth into surgical area in one side,the other side was used as control.Three SD rats were sacrificed respectively 1,3 and 7 days after treatment.Real-time PCR was used to detect the expression of OPN,Trap,Runx2,Rank1,OCN and VEGF between the experimental group and control group.SPSS 19.0 software package was used for statistical analysis.RESULTS:H-E staining revealed abundant bone regeneration and osseous tissue was mature.RT-PCR showed that OCN expressed highest in late stage but had no significant difference between early stage and middle stage;expression of Runx2 rose from early stage to middle stage and declined to late stage;VEGF expression progressively decreased from early stage to late stage;expressions of each gene in the experimental group were significantly higher than the control group (P＜0.05).CONCLUSIONS:Tissue-engineered bone employing 3D printed scaffold can successfully substitute for autogenous bone to regenerate new bone.Using orthodontic method to move adjacent tooth into surgical area can promote bone regeneration,induce osteoblast and osteoclast to remodel bone biological structure.

Abstract:The aim of this study was to analyze alveolar bone morphology following periodontally accelerated osteogenic orthodontics. Treated patients were called for a full periodontal examination and a cone beam computed tomography scan. Mean treatment time was 6.08 months. Mean probing pocket depth was 2.7 mm. No gingival recessions were noted. In the maxilla, buccal plate thickness was 0.48 to 2.14 mm. In the mandible, bone thickness was 0.2 to 1.82 mm. Root fenestrations and dehiscences were present in up to 40% of the anterior teeth. Although clinical outcomes were favorable, due to the presence of multiple posttreatment bone fenestrations and dehiscences, a revision of the treatment protocol might be considered.

Osteoclastogenesis in Local Alveolar Bone in Early Decortication-Facilitated Orthodontic Tooth Movement.

Abstract:OBJECTIVE:In the current study, we aimed to investigate the effects of alveolar decortication on local bone remodeling, and to explore the possible mechanism by which decortication facilitates tooth movement. MATERIALS AND METHODS:Forty rabbits were included in the experiment. The left mandible was subjected to decortication-facilitated orthodontics, and the right mandible underwent traditional orthodontics as a control. The animals were sacrificed on the days 1, 3, 5, 7 and 14, after undergoing orthodontic procedures. Tooth movement was measured by Micro-CT, and the local periodontal tissues were investigated using H&E, Masson's trichrome and tartrate-resistant acid phosphatase (TRAP) staining. The mRNA levels of genes related to bone remodeling in the alveolar bone were analyzed using real-time PCR. RESULT:On days 3, 5, 7 and 14, tooth movement was statistically accelerated by decortication (P<0.05) and was accompanied by increased hyperemia. Despite the lack of new bone formation in both groups, more osteoclasts were noted in the decorticated group, with two peak counts (P<0.05). The first peak count was consistent with the maximum values of ctsk and TRAP expression, and the second peak counts accompanied the maximum nfatc1 and jdp2 expression. The increased fra2 expression and the ratio of rankl/opg also accompanied the second peak counts. CONCLUSIONS:Following alveolar decortication, osteoclastogenesis was initially induced to a greater degree than the new bone formation which was thought to have caused a regional acceleratory phenomenon (RAP). The amount of steoclastogenesis in the decorticated alveolar bone was found to have two peaks, perhaps due to attenuated local resistance. The first peak count in osteoclasts may have been due to previously existing osteoclast precursors, whereas the second may represent the differentiation of peripheral blood mononuclear cells which came from circulation as the result of hyperemia.
Orthodontic force influences the activity of osteoclasts in alveolar bone remodeling by regulating EphB 4 / ephrinB 2 expression

Abstract:EphB4/ephrinB2 signaling plays an important role in bone remodeling, but the role of orthodontic force regulated EphB4/ephrinB2 expression in influencing osteoclastogenesis in alveolar bone remodeling remains to be studied. A rat alveolar bone remodeling model was induced by orthodontic force, and we observed microstructural changes in the alveolar bone using Micro CT and hematoxylin-eosin (HE) staining methods. EphrinB2 expression localization was detected by immunofluorescence. EphrinB2 and EphB4 protein expressions were tested by western blot. EphrinB2, EphB4, osteoprotegerin (OPG), and receptor activator of nuclear factor κB ligand (RANKL) mRNA levels were determined by real-time PCR. After placing the orthodontic device, the rats did not change their drinking and eating habits, and the orthodontic side gingival tissue and the surrounding mucosa were slightly congested. Micro-CT (HECT) and HE staining showed large and multinucleated osteoclasts near the orthotopically absorptive lacunae. Immunofluorescence exhibited strong positive ephrinB2 expression in the orthodontic pressure zone. Western blot and real-time PCR demonstrated EphrinB2 upregulation and EphB4 downregulation in the pressure zone. OPG expression was reduced, but the RANKL level was elevated on the orthodontic side. In the alveolar bone remodeling model, orthodontic force can regulate EphB4/ephrinB2 expression and affect the activity of osteoclasts. It provided a theoretical basis for the mechanism of stress zone remodeling during orthodontic tooth movement, and helped to discover substances that can regulate the activity of osteoblasts and osteoclasts, thereby regulating bone remodeling.
