Value of 2D speckle tracking technique combined with real-time 3-dimensional echocardiography in the evaluation of the right atrial function in patients with 3-branch coronary artery disease without myocardial infarction

Abstract:The cardiomyocytes of patients with coronary artery disease (CAD) suffer from ischemia, hypoxia and necrosis, which leads to the enlargement of the heart and the remodeling of the ventricles. [ 1 ] Three-branch coronary artery disease (TBCAD) is a very high-risk type that can lead to systolic and diastolic insufficiency of the heart due to its wider blood supply than the range of the single and 2 branches. Over the years, researches on TBCAD have focused on coronary revascularization and assessment of cardiac function in patients with myocardial infarction caused by the lesion, and few scholars have studied the cardiac function of patients with TBCAD who have not yet had a myocardial infarction. As a new technique, 2D speckle tracking echocardiography (2D-STE) can evaluate cardiac function by measuring myocardial strains and strain rates. Strains and strain rates are very advantageous echocardiographic parameters that can directly reflect myocardial deformation during systole and diastole in both global and localized areas of the heart. [ 2 , 3 ] Due to myocardial ischemia, they can sensitively detect dysfunction in patients with CAD, and quantify the movement capacity of each component of the ventricle or atrium throughout the cardiac cycle. [ 4 , 5 ] 2D-STE is independent of angular dependence and has been shown to be useful for the accurate quantification of atrial function. [ 6–8 ] Real-time 3-dimensional echocardiography (RT-3DE) adopts advanced technologies such as ultra-matrix probe and 3-dimensional spatial localization to overcome the influence of complex morphology and structure of the left and right atrium, and the volume-time curves it obtains can make a highly accurate and reproducible assessment of the dynamic change of atrial volume in different phases of the cardiac cycle. [ 9 ] It has shown that changes in atrial function occur before changes in ventricular function in patients with ischemic heart disease. [ 10 ] This study aimed to evaluate the function of the right atrium in patients without myocardial infarction with TBCAD by applying 2D-STE combined with RT-3DE.
