Abstract:Over the last 40 years newborn screening has been an undoubted success and many thousands of children have been saved from mental retardation and other problems because of early diagnosis of their disorders. Now many diseases can be diagnosed early by newborn screening and many more are on the horizon. It must be a long-term goal to extend newborn screening tests to all children but, in areas of the world where healthcare delivery is insufficient, solving other health problems has to take precedence over introducing newborn screening. If it is decided to introduce newborn screening in a region where currently there is none screening for congenital hypothyroidism alone should be started before anything else at all is attempted so that proper systems can be put in place. There is an exciting future for newborn screening ahead. If new programmes are approached with proper caution maximal benefit will be achieved from newborn screening, which is one of the few clearly effective preventive strategies in healthcare.

Pulmonary sclerosing hemangioma with lymph node metastases: report of 4 cases

Abstract:Context: Sclerosing hemangioma is an unusual pulmonary tumor. Previously, 4 patients with pulmonary sclerosing hemangioma and lymph node metastases have been described in the literature. Objective: To report 4 additional cases of metastatic sclerosing hemangioma. Design: Retrospective review of the authors' consultation files and review of histologic sections of pulmonary tumors and lymph node metastases. Results: Four cases of a morphologically benign pulmonary sclerosing hemangioma with regional lymph node metastases (including hilar, peribronchial, and interlobar metastases) were identified. The patients (3 female, 1 male) had a mean age of 39 years (range, 10-56 years). The tumors ranged in size (greatest dimension) from 1.5 to 4.7 cm (mean, 3.1 cm). The pulmonary tumors were typical circumscribed sclerosing hemangiomas without mitotic activity, angiolymphatic invasion, or necrosis. One tumor had focal cytologic atypia. The metastases were identified in hilar lymph nodes that were removed at operation for the lung nodule. One patient received adjuvant chemotherapy for adenocarcinoma. All of the patients are alive. No recurrences or residual disease has been detected at a mean follow-up of 4.7 years (range, 2.3-10 years). Conclusions: On the basis of case data from the 4 patients described here and the 4 patients described previously, metastases to regional lymph nodes from pulmonary sclerosing hemangioma may occur but are rare and do not appear to affect prognosis.

Abstract:AIM OF THE STUDY To explore the content and structure of communication between patient and nurse at follow-up appointments concerning hypertension. BACKGROUND Hypertension is a chronic condition and calls for co-operation between health care providers and patients over a long period of time. One important purpose of the follow-up consultations is to transfer knowledge between patients and health care providers in order to empower patients. This is an important determinant of the quality of care. DESIGN/METHODS The study was based on 20 audio-recordings of actual follow-up appointments and was approved by ethics committees. The consultations took place at four different health care units for hypertensive patients. FINDINGS The average length of consultations was 18 minutes. In the consultations, patients initiated an average of eight new topics and nurses an average of 20. All nurses talked with patients about life style. Compared with previous studies of follow-ups with physicians, consultations with nurses addressed lifestyle factors and adherence to treatment to a higher degree. It was also observed that patients were more actively involved in interaction with nurses compared with the follow-ups with physicians. CONCLUSIONS Active patient participation in care is a critical factor in improving adherence to treatment. It would be of value to develop and assess a more patient-centred organization of hypertension care and thereby more individualized hypertension treatment. Nurses may have a pivot role in such care.

Abstract:We investigate methods of using CRC32 for compressing Web URL strings and sharing of URL lists between servers, caches, and URL switches. Using trace-based evaluation, we compare our new CRC32 digesting method against the existing Bloom filter and incremental CRC19 methods. Our CRC32 method requires less CPU resources, generates equal or smaller size digests, achieves equal collision rates, and simplifies switching.

Abstract:Pulmonary sclerosing pneumocytoma (PSP) is a rare tumor and the imaging appearance is usually a well-circumscribed nodule. Herein we present the clinicopathological features of a 25-year-old female patient with a 4 cm mass in the left upper lobe. She had undergone lobectomy with lymph node dissection with an incorrect intraoperative frozen section diagnosis of adenocarcinoma and diagnosed as a PSP with lymph node metastasis on permanent sections. The 3-year follow-up of the patient is uneventful.

Abstract:Objective To study the clinicopathologic characteristics of pulmonary sclerosing hemangioma(SH).Method Eleven cases of SH were studied by the clinical data and immunohistochemistry and review of the literature.Results SH has a characteristic architecture composed of round cell and surface cubic cell.It has a variety of tissular structure,mainly expression with four regions that contain solid regions,papillary regions,angiomatoid regions and sclerosing regions.Immunohistochemically,these two kinds of cells were both positive for TTF-1 and EMA.Conclusions SH is a tumor that originates from the epithelial cell of primary respiratory tract.Both the solid round cells and surface cubic cells are neoplastic.SH should differentiate with inflammatory,typeⅡpneumocyte papillary tumor,alveolus adenoma and typical carcinoid tumors.The parts of SH have invade grow tendency.Surgical cure of SH is suited for operating a lobe of the lung but not just peel of the tumor.

Abstract:Sclerosing hemangioma of the lung, a rare disease, is a low grade malignancy possibly originating from type II pneumocytes or Clara cells. We report the clinical characteristics of 8 patients who underwent surgical resection for sclerosing hemangioma between 2005 and 2010 in our hospital. All cases were female, and the average age was 50 (range: 28-83) years old. The median tumor doubling time was 965 days, suggesting they were slowly growing tumors. In the present cases, five patients had another lung disease: lung cancer in two, metastatic lung tumor in one and atypical adenomatous hyperplasia in two patients. Intraoperative frozen section examinations were performed in seven cases. Five patients were diagnosed correctly, but two patients were diagnosed with adenocarcinoma and organizing pneumonia. As a clinical characteristics, sclerosing hemangioma in the present study showed well-demarcated and slow-growing tumor. The postoperative clinical courses of all cases were uneventful, and no findings of recurrence distant metastasis, lymph node metastasis and local recurrence after surgery were observed in any of the patients.
A case of sclerosing hemangioma evaluated with diffusion-weighted magnetic resonance imaging and 18F-fluorodeoxyglucose positron emission tomography

Abstract:A 33-year-old female patient was referred to our hospital for further examination of an abnormal shadow evident on a chest X-ray film. Chest computed tomography (CT) revealed a solid nodule 1.9 cm in diameter in the hilum of the upper lobe of the left lung. Positron emission tomography showed high 18F-fluorodeoxyglucose accumulation in the nodule with a maximal standardized uptake value of 4.5, which favored a malignant lesion. Diffusion-weighted magnetic resonance imaging (DWI), which shows differences in the diffusion of water molecules and can discriminate between malignant and benign lesions, indicated that the nodule had a minimum apparent diffusion coefficient of 1.7 × 10-3 mm2/sec, which was higher than the cutoff value of 1.1 × 10-3 mm2/sec for discriminating between malignant and benign diseases; i.e., values equal to or lower than 1.1 × 10-3 mm2/sec favor malignant disease. The results of a CT-guided needle biopsy of the nodule favored sclerosing hemangioma. During surgery, the tumor did not appear to be invasive, and lymph node metastasis and dissemination were not apparent. On the basis of gross appearance, location, preoperative histological diagnosis, and DWI findings, the tumor was enucleated from the pulmonary parenchyma. Seven months after surgery, the patient was alive and had no evidence of recurrent disease.

Abstract:Sclerosing pneumocytoma (SP) is an uncommon benign tumor, and metastasis of SP has been rarely reported. Here, we report the case of a 26-year-old woman with surgically confirmed SP. The tumor diameter was 40 mm, and metastasis to mediastinal and regional lymph nodes was observed. Immunohistochemically, both surface and round cells were positive for epithelial membrane antigen, thyroid transcription factor 1, and vimentin. Only surface cells expressed creatine kinase, carcinoembryonic antigen, napsin A, and cytokeratin 7, and only round cells expressed progesterone receptor. Ki-67 was detected in ~3% of cells, and the rate of weak positive p53 staining was 3%. Both cell types were negative for chromogranin A, synaptophysin, CD3, and CK20. Multiple metastases in a young SP patient are very rare, and potential mechanisms of metastasis may be related to epithelial-mesenchymal transformation.
Mediastinal Hemangioma Mimicking as Bronchial NET

Abstract:We present a rare case of mediastinal capillary hemangioma in a 54-year-old female. She presented with back pain in the left suprascapular region, and the chest radiograph revealed left pleural effusion. On further workup with high-resolution computed tomography (CT) chest, a hypervascular pleural-based neoplastic lesion in the left upper hemithorax with gross left pleural effusion and subtotal collapse of the left lung was identified. 18F-fluorodeoxyglucose positron emission tomography/CT was suggestive of a weakly metabolic well-defined pleural-based soft tissue lesion in the left upper hemithorax along the mediastinal side. Neuroendocrine tumor was suspected. 68Ga-DOTATATE PET/CT was advised, which showed intense uptake in the lesion. The mass was resected completely. Histopathological examination established the final diagnosis as benign vascular tumor consistent with a capillary hemangioma.

Abstract:Primary pulmonary chondrosarcoma is a rare neoplasm that usually grows slowly, metastasizes late, and responds well to excision when localized. Herein, we present a 59-year-old man who manifested with hemoptysis, cough and dyspnea with a hemithorax mass. A chest computed tomography scan demonstrated a fast-growing mass in the right upper lobe, and the enhancement was partially heterogeneous. Bronchoscopy revealed a hemorrhagic neoplasm in the right upper bronchus. A bronchoscopy biopsy specimen revealed a variety of tissues, including mucoid cartilage, fibers, respiratory epithelium and squamous epithelium. Because malignancy was suspected, the patient underwent a right upper sleeve lobectomy with mediastinal lymphadenectomy. Macroscopically, we observed a firm white mass 9.5 cm in diameter with a central area of necrosis. Histopathology revealed neoplastic chondrocytes with enlarged and hyperchromatic nuclei in the myxoid matrix. The tumors were positive for S-100. The patient was diagnosed with tracheobronchial myxoid chondrosarcoma. Lymph node dissection indicated no metastasis. The tumor grows slowly in the initial symptom-free phase when localized. Then, a symptomatic phase ensues, during which the tumor progresses rapidly. The patient displayed pulmonary and subcutaneous skull metastases eight months after operation and was treated with adriamycin and ifosfamide chemotherapy.

Abstract:Pulmonary sclerosing pneumocytoma, formally named pulmonary sclerosing hemangioma, is a rare benign tumor with malignant potential often identified as solid pulmonary nodules. Sarcoidosis is an inflammatory, multisystemic disease of unknown cause with a wide range of clinical manifestations. The disorder is characterized by the formation of noncaseating granulomas in virtually any organ in the body. We present a case of a patient presenting with fever, weight loss, and respiratory symptoms found to have both a sclerosing pneumocytoma and pulmonary sarcoidosis. A diagnosis was made following the lobectomy. The patient was followed for two years with stable lymphadenopathy while remaining asymptomatic.

Abstract:A 69-year-old man was referred to our hospital in December 1993 because an abnormal mass had been detected in the right pulmonary hilum. Computed tomography (CT) of the chest revealed a swollen hilar lymph node between the right middle and lower lobe bronchi, and an adherent tumor in the right ventrobasal segment (S8). Chest roentgenogram in February 1994, however, showed no evident tumor in the right lung field. In March 1996, the mass in the right pulmonary hilum reappeared on chest roentgenogram. Chest CT revealed a swollen hilar lymph node between the right middle and lower lobe bronchi, but there was no tumor in right S8. The patient underwent video-assisted thoracoscopy on 17 May 1996. Intraoperative needle biopsy of the node revealed cancer cells. We performed right middle and lower bilobectomy with mediastinal dissection. Histological diagnosis revealed a large cell carcinoma almost completely occupying a hilar lymph node. The resected middle and lower lobes showed no tumors, except for a coagulation necrosis measuring 1.5 cm in diameter in S8b, corresponding to the site where a tumor shadow had been depicted on the CT image in December 1993. We concluded that the coagulation necrosis might have been the primary site of the tumor, which had spontaneously regressed and then appeared in the metastatic interlobar node.

Abstract:Rationale: Spindle cell hemangioma (SCH) is considered a benign vascular lesion. It typically develops as a solitary nodule or multiple masses located in the dermal or subcutaneous layers of the distal extremities. To the best of our knowledge, there are no prior reports of SCH in the spleen. Patient concerns: A 41-year-old male was admitted to our hospital with recurrent headaches, nausea, and vomiting persisting for 5 days. Ultrasound, computed tomography, and magnetic resonance imaging revealed multiple space-occupying lesions in the spleen, and the biggest lesion was 4.8 cm x 5.4 cm in size. Interventions: The patient underwent laparoscopic splenectomy. Diagnosis: A diagnosis of spindle cell hemangioma of the spleen was made based on the histopathology. Outcomes: No evidence of local recurrence or distant metastases was observed over 4-year follow-up. Lessons: Splenic SCH may exhibit relatively high proliferative activity and be comorbid with epithelioid hemangioendothelioma or angiosarcoma, raising the possibility of malignant potential. However, the patient remained alive and disease-free 4 years after the operation. The nature of SCH in deep soft tissues requires further study.
Clinicopathological analysis of pulmonary sclerosing hemangioma

Abstract:Background: Sclerosing hemangiomas of the lung are uncommon tumors and are thought to be benign. However, the histogenesis and clinicopathological features of these tumors have not been elucidated. Methods: We analyzed the clinicopathological features of 26 sclerosing hemangiomas. The immunoreactivity for Ki-67 and p53 of sclerosing hemangiomas was determined and compared with that of pathological stage 1 pulmonary papillary adenocarcinomas. Results: The patients of sclerosing hemangioma were predominantly female. Eighteen patients were detected as a result of routine medical examinations and 15 were nonsmokers. Seven patients underwent tumor enucleation, 10 underwent a wedge resection, and 9 underwent a lobectomy. The mean tumor size was 2.2 cm (range 1 to 5 cm). Pathological findings demonstrated a papillary pattern in 23 cases, sclerotic pattern in 26 cases, hemorrhagic pattern in 22 cases and a solid pattern in 25 cases. Twenty-five cases had an excellent prognosis with no evidence of recurrence following surgery. However, 1 patient who had undergone a wedge resection developed a local recurrence and required an additional wedge resection. The Ki-67 labeling index of sclerosing hemangiomas was significantly lower than that of adenocarcinomas, whereas the Ki-67 labeling index of the recurrent case was 0.4%. No significant immunohistochemical staining for p53 was observed in sclerosing hemangioma cases. Conclusions: Sclerosing hemangioma exhibits various histologic findings. Although we experienced one case with a recurrent tumor, sclerosing hemangiomas did not exhibit malignant behavior.

Abstract:Less than 1% of cancer occurs in children. With the progress made by national and international cooperative groups 75% of them are actually cured. However some entities have an incidence so weak that we can't actually establish standardized therapeutics guidelines. To improve our knowledge on these rare tumours a national organisation become necessary as well as an international collaboration. A French rare tumour committee was created within the French Society for Children Cancer (SFCE). Others European countries have such organisation. The objectives of these tasks groups are to enhance our knowledge of the real incidence of these rare tumours, their evolution, and to propose therapeutic recommendations for each of them. This article focuses on the specific French organization for rare tumours treatment. It also describes the draft for the creation of a new data base for prospective registry of clinical, therapeutics and follow up data. To provide a better understanding of these pathologies, the "Bulletin du Cancer's" editorial board decided to regularly publish an update on a rare paediatric tumour in a specific section.
Sclerosing mediastinitis: a report on 18 cases.

Abstract:BACKGROUND: Sclerosing mediastinitis is a rare condition which causes dense fibrosis of the mediastinum. Few large studies have been reported to date. The clinical and pathological features of cases have been studied in a specialist referral centre in the UK.METHODS: The pathological files of the Royal Brompton Hospital were examined and 18 cases of sclerosing mediastinitis were identified between 1970 and 1993. The clinical notes were obtained and the pathological specimens analysed.RESULTS: There were 12 men and six women of age range 9-64 years. Twelve patients presented with shortness of breath, six had haemoptysis, three had hoarseness, four had pleuritic chest pain, three general weakness, two had dysphagia, and one was asymptomatic. Nine patients had a previous history of pulmonary tuberculosis. Two had auto-immune disease--one rheumatoid arthritis and the other systemic lupus erythematosus. There were three cases of previous malignancy--two undifferentiated carcinoma of the lung and the other Hodgkin's disease. Serological tests revealed only one positive reaction to Histoplasma. The erythrocyte sedimentation rate and serum immunoglobulins were raised in nine patients. Diagnosis was usually by thoracotomy with biopsy. All cases had fibrosis and chronic inflammation with no active granulomas. No infective organisms or positive cultures were obtained in any case. Ten of the 18 cases are alive up to 15 years after diagnosis, with only two deaths and six lost to follow up.CONCLUSIONS: Sclerosing mediastinitis is a slowly progressive condition associated with previous tuberculosis, mediastinal malignancy, and autoimmune disease. The outlook is excellent for those cases without underlying malignancy.
