Abstract:BACKGROUND:The concept of pulmonary sclerosing hemangioma (PSH) was proposed only 50 years ago. PSH features several characteristics that should be differently diagnosed with lung cancer. The aim of this study is to retrospectively review the diagnosis and treatment of 48 cases of PSH, review recent publications about this topic, and address reasonable diagnosis and treatment methods.METHODS:Forty-eight patients with PSH were enrolled and consecutively treated from January 2001 to April 2011. Clinical presentations, image characteristics, pathologic morphologies, and prognostic data were analyzed.RESULTS:Twenty-seven PSH cases (56.3%) were asymptomatic. Tumor diameters ranged from 0.2 to 7.0 cm, with an average diameter of 2.1 cm, and involved both lobes of the lungs. Enlarged lymph nodes in the mediastinum were detected via computed tomography (CT) scans in 15 cases (31.3%). Only 47 patients underwent surgery. Pulmonary wedge resection was performed in 29 cases (61.7%) while lobectomy was performed in 14 (29.8%). The tumor was removed in 3 cases (6.4%) and anterior mediastinal tumor resection was performed in 1 case (2.1%). Forty-seven patients reported no recurrence during follow-up while one patient refused surgery after a CT-guided biopsy with no progression during the 28-month follow up period.CONCLUSIONS:The exact diagnosis of PSH is difficult prior to surgery. Surgery is an effective method of treatment that allows both definite diagnosis and accurate therapy. The prognosis of PSH is favorable.
[Diagnosis and therapeutics of 24 cases of pulmonary sclerosing hemangioma]

Abstract:Background & objective: Pulmonary sclerosing hemangioma (PSH) is an uncommon benign lung tumor. The study was to investigate the clinical features, diagnosis, treatment, and prognosis of PSH in order to promote the recognition of this disease. Methods: Data of 24 pathologically confirmed PSH patients treated in Sun Yat-sen University Cancer Center from Jan. 1999 to Jul. 2007 were reviewed. The clinical features, diagnosis, treatment, and prognosis were summarized. Results: Of the 24 patients, two (8.3%) were males, and 22 (91.7%) were females. The median age of the patients was 54.5 years old, ranging from 21 to 76 years old. Ten (41.7%) patients were detected upon routine medical examination, while 14 (58.3%) patients presented clinical symptoms, including cough, hemoptysis, chest pain, chest distress and tachypnea. The imaging examination revealed isolated round or similar round nodules with distinct margins and homogeneous density. No calcification and aerial semilunar sign appeared. All the patients received surgical resection without complications and mortality. Eight patients underwent lobectomy, 13 underwent wedge resection, two underwent tumor resection and one underwent segmentectomy. There was no recurrence or metastasis during follow-ups. Conclusions: Clinical and radiological characteristics of PSH are nonspecific. Thus, accurate diagnosis of PSH before operation is difficult. Confirmation of PSH depends on pathological examination. Surgical resection is an effective treatment for PSH, among which lobectomy or limited resection is advisable, while systematic lymph node dissection is not recommended.
Case report - Pulmonary Primary pulmonary artery sarcoma

Abstract:Primary pulmonary artery sarcoma is an uncommon tumor. We report a case of a 73-year-old male patient with a two-week history of palpitations and shortness of breath, aggravated for two days and was believed to be pulmonary hypertension. Emergency heart ultrasound after admission presented a massive pulmonary embolism in the pulmonary artery. The patient's condition was successfully managed with urgent pulmonary artery embolectomy. The patient demonstrated improvement in hemodynamics after the operation. Histologic and immunohistochemical assays were performed and a diagnosis was made as primary pulmonary artery sarcoma arising from the left pulmonary artery. Resection of the tumor is recommended for the treatment of this rare malignant tumor. The corresponding chemotherapy, follow-up and prognosis are described as well in this case report. 2008 Published by European Association for Cardio-Thoracic Surgery. All rights reserved.

Abstract:Objective To study the clinicopathologic characteristics of pulmonary sclerosing hemangioma(SH).Method Eleven cases of SH were studied by the clinical data and immunohistochemistry and review of the literature.Results SH has a characteristic architecture composed of round cell and surface cubic cell.It has a variety of tissular structure,mainly expression with four regions that contain solid regions,papillary regions,angiomatoid regions and sclerosing regions.Immunohistochemically,these two kinds of cells were both positive for TTF-1 and EMA.Conclusions SH is a tumor that originates from the epithelial cell of primary respiratory tract.Both the solid round cells and surface cubic cells are neoplastic.SH should differentiate with inflammatory,typeⅡpneumocyte papillary tumor,alveolus adenoma and typical carcinoid tumors.The parts of SH have invade grow tendency.Surgical cure of SH is suited for operating a lobe of the lung but not just peel of the tumor.
[Pulmonary Sclerosing Hemangioma: Report of 15 Cases and Review of the Literature].

Abstract:OBJECTIVE To improve the understanding of pulmonary sclerosing hemangioma (PSH). METHODS The clinical data of 15 cases of PSH were analyzed, and the literature was reviewed. The etiology, clinical manifestations, differential diagnosis, treatment and outcome of PSH were described. RESULTS The etiology and histological origin of PSH were unclear. Most cases were asymptomatic or only with mild symptoms. The radiology of PSH often showed isolated nodule with distinct margin in the lung field. The characteristics of its pathological manifestation were as follows: (1) background of cell gathering or mucin matrix with scattered white blood cells; (2) proliferation of hemangioma with sclerosis of vessel wall; (3) papillary proliferation of small vessels intruding into the air space; (4) existence of hemorrhage or sclerosis zone. Immunohistological studies had not defined the correct histological origin of PSH. A pre-operation diagnosis of PSH was difficult. Thirteen cases had been misdiagnosed as malignancy. The outcome of the disease was good when early surgical resection was performed. CONCLUSIONS PSH is an uncommon disease, and can be easily misdiagnosed. More attention should be paid to its clinical features and management.

Abstract:Objective:To investigate the imaging findings,histopathological features,surgical treatment and prognosis of pulmonary sclerosing hemangioma(PSH).Methods:Clinical data of 68 cases with postoperative pathologically proven PSH in Zhongshan Hospital,Fudan University between January 1999 and January 2012 were analyzed retrospectively.Results: Among the 68 cases,7 were males,61 were females(89.7%).The age of them ranged from 17 to 74 years(43.4±12.3 years).38(55.9%) cases of PSH were found in routine physical examination and those patients were free of symptoms.The lesions presented as round or nearly round shaped well-defined shadow on chest X-ray.On CT,the lesions presented as nearly round shaped mass or nodule,well-defined,homogeneous density,homogeneous and heterogeneous enhancement after contrast administration.The characteristic air-trapping zone and vessels at its periphery could be observed.Pathological morphology demonstrated that surface cuboidal cells and round mesenchymal cells formed four types of migratory structures,including solid,papillary,sclerotic and hemorrhagic pattern.The mixed-type made of two or three structures was more common than the other types.Immunohistochemisty results showed that PSH expressed surfactant protein A(SPA),cytokeratin-7(CK-7),epithelial membrane antigen(EMA) and thyroid transcription factor-1(TTF-1).All patients underwent operation and had no severe complications.There was no recurrence or metastasis during the postoperative follow-up of 3 months to 10 years.Conclusions: It is difficult to diagnose PSH before operation.There were some characteristic features in imaging findings and histopathology of PSH.Immunohistochemical findings demonstrated that PSH might originate from epithelium.Prognosis and diagnosis can be achieved through surgical treatment,and immunohistochemisty plays an important role in the diagnosis and differential diagnosis.Limited resection and lobectomy by video-assisted minimally surgery are good choice for PSH.

Abstract:Objective To improve the knowledge of pulmonary sclerosing hemangioma(PSH)especially with bilateral multiple lesions of the lung.Methods The clinical data of 3 cases of PSH(1 case with bilateral multiple lesions in the lung)were analyzed,and the related literatures were reviewed.Results All of the 3 cases were females.A 22-year-old female patient with bilateral multiple nodules in the lungs was complicated with thyroid multiple nodular goiter(with hypothyroidism),dysfunctional uterine bleeding(with anemia),nodular hyperplasia of the breast,and arteriovenous malformation over forearm.Thoracoscopic biopsy of left lung and resection of the right pulmonary mass were performed and both the lesions were confirmed as PSH.The clinical manifestations of multiorgan diseases and the presence of PSH suggested Cowden syndrome in this patient.The other 2 cases aged 50 and 53 were asymptomatic with solitary pulmonary nodules identified incidentally.The accessory examinations for malignancies,infections,and autoimmune diseases showed no specific findings.Resection of the lesions were performed by thoracoscopic surgery and thoracotomy respectively,and the histopathological results proved to be PSH.Literature review showed that PSH typically occurred in middle-aged women without clinical symptoms and signs,often presenting as a pulmonary solitary nodule/mass identified incidentally.The differential diagnosis should include peripheral carcinoma,hamartoma,inflammatory pseudotumor and tuberculoma.Multiple PSH,which mainly presented as multiple well-defined nodules/masses of different size in the lungs,was rather rare,but easily confused with metastatic neoplasm.Lung biopsy by surgical operation was a common way to confirm the diagnosis,while FDP-PET and fine needle aspiration biopsy showed some defects.Surgical resection was an effective method of treatment,the residual lesions of multiple PSH should be monitored.Cowden syndrome may be considered if a PSH coexisting with abnormity of multiple organs such as thyoid,breast and vessels.Conclusions PSH should be considered during the differential diagnosis for solitary or multiple nodules/masses in the lung.Surgical biopsy is a common way to confirm the diagnosis.Local excision and necessary follow-up are effective methods of treatment.

Abstract:Sclerosing hemangioma of the lung, a rare disease, is a low grade malignancy possibly originating from type II pneumocytes or Clara cells. We report the clinical characteristics of 8 patients who underwent surgical resection for sclerosing hemangioma between 2005 and 2010 in our hospital. All cases were female, and the average age was 50 (range: 28-83) years old. The median tumor doubling time was 965 days, suggesting they were slowly growing tumors. In the present cases, five patients had another lung disease: lung cancer in two, metastatic lung tumor in one and atypical adenomatous hyperplasia in two patients. Intraoperative frozen section examinations were performed in seven cases. Five patients were diagnosed correctly, but two patients were diagnosed with adenocarcinoma and organizing pneumonia. As a clinical characteristics, sclerosing hemangioma in the present study showed well-demarcated and slow-growing tumor. The postoperative clinical courses of all cases were uneventful, and no findings of recurrence distant metastasis, lymph node metastasis and local recurrence after surgery were observed in any of the patients.

Abstract:Pulmonary sclerosing hemangioma (PSH) is an uncommon benign tumor that presents as a solitary asymptomatic and slow-growing nodule. It occurs in both young and old persons; peak incidence is in the fifth decade. Both sexes are affected by this tumor, but women more frequently than men. On histological examination, PSH shows prominent sclerotization and vascularization of the tissue. Recent studies conclude that PSH derives from type II pneumocytes, but the potential for progression and histogenesis remains controversial. We report a case of pulmonary sclerosing hemangioma in a 61-year-old woman with a neoplastic node 1 cm in diameter. The karyotype was 46,XX,t(8;18),der(14;15),+14 in all the cells analyzed. PTEN (10q23) and IgH (14q32) probes were analyzed in interphase nuclei and paraffin-embedded tissues of tumor cells. These chromosome abnormalities could provide information about the relationship of genetic changes to the biological properties of sclerosing hemangioma tumors.

CT Diagnosis of Pulmonary Sclerosing Hemangioma

Abstract:Objective To investigate the CT manifestations of pulmonary sclerosing hemangioma (PSH), and improve the CT diagnostic accuracy of the disease.Methods The CT manifestations of 5 cases with pulmonary sclerosing hemangioma which were proved by pathology were analyzed retrospectively. Results Solitary pulmonary nodule was demonstrated in all 5 cases with clear demarcation on CT. The nodule was smooth and homogeneous in density in 4 cases, and was slightly irregular with cavity in another case. After intravenous contrast administration, all the lesions were homogeneously enhanced, similar to the nearby blood vessels. The vessels linked with the lesions were found generally.Conclusion The CT is helpful for the diagnosis of PSH.

Abstract:Objective To analyze the imaging findings of the pulmonary sclerosing hemangioma (PSH),and to improve the diagnosis and differential diagnosis of the PSH. Materials and Methods The imaging findings of 20 cases of PSH confirmed by pathology were analyzed retrospectively. All cases underwent anterior-posterior and lateral plain film and CT plain scanning,13 patients underwent contrast enhancement CT scans. Results Of 20 PSH,19 cases were solitary,one was multiple (two lesions).On DR,19 lesions were well-defined; the remaining 2 lesions had unclear margins. On pre-contrast enhanced CT imaging,well-defined round or oval lesions were showed in 18 lesions (86%). Three cases had ill defined margins. Seventeen lesions were homogeneous,while 4 lesions heterogeneous. Characteristic air-trapping zone (n=1),vessels at its periphery (n=2),a tail sign (n=1),and prominent pulmonary artery sign (n=1) were showed. Ten lesions enhanced homogeneously at the arterial phase and another 3 lesions enhanced heterogeneously. All lesions had delayed enhancement at the venous phase. Conclusion Well-defined and homogeneous masses with marked continuing enhancement are the main findings of PSH. comprehensive analysis all the signs will help to improve diagnosis.

Abstract:BACKGROUND:Pulmonary nodular lymphoid hyperplasia (PNLH) is a rare benign disease lacking systematically clinical assessment. This study aims to explore the clinical features of PNLH and evaluate the safety and efficacy of different surgical approaches for PNLH.METHODS:Patients with confirmed PNLH in our department between March 2007 and August 2017 were retrospectively reviewed. The detail information of patients, including demographic characteristics, medical history, preoperative investigations, perioperative findings, postoperative conditions, and recurrence status, was collected and analyzed.RESULTS:A total of 67 patients with PNLH were diagnosed in our department, and 15 of them were suspected to have lung cancer by radiological examination before the surgery. 43 of 67 patients underwent sublobar resection and 24 patients underwent lobectomy. Compared with the lobectomy, sublobar resection showed more significant advantages in operation time (P<0.001), blood loss (P=0.001), volume of chest drainage (P=0.007), duration of chest drainage (P=0.001) and postoperative hospital stay (P=0.001).CONCLUSIONS:The diagnosis of PNLH before operation is challenging and it should be taken into consideration in the differential diagnosis of lung nodules. Compared with lobectomy, sublobar resection is an alternative approach for the treatment of PNLH.
Increased expression of matrix metalloproteinase 9 and tubulin-alpha in pulmonary sclerosing hemangioma

Abstract:Pulmonary sclerosing hemangioma (PSH) is relatively rare and is usually considered a benign tumor because of its slow growth and solitary characteristics. However, several cases with lymph node metastasis have been reported, and its pathogenesis has not been fully elucidated. Three sets of PSH specimens from the Korea Lung Tissue Bank, obtained with IRB approval, were analyzed through the construction of an oligo-microarray that contained about 32,000 genes. The resulting data were confirmed by real-time RT-PCR. Protein expression levels were checked by performing immunohistochemistry (IHC) and immunoblot analysis. In the 3 specimens of PSH tissues, 72 of the 32,000 genes were commonly found up-regulated and 290 were commonly found down-regulated as compared to non-tumor tissues from each patient. Paraffin-embedded tissues from 11 cases were used to confirm the expression of matrix metalloproteinase 9 (MMP-9) and tubulin-alpha proteins in the non-tumor and PSH tissues via IHC. In addition, the upregulation of protein expression was confirmed by immunoblot analysis. As expected, in all cases MMP-9 and tubulin-alpha were expressed at significantly higher levels in the PSH than in the non-tumor tissues. This is the first report on a study of the whole genome of PSH. Increased expression of MMP-9 could induce the metastatic ability of PSH and tubulin-alpha might be responsible for the sclerotic character of this disease. The results of this study will be useful in helping to understand and effectively manage patients suffering from PSH.
Clinicopathological analysis of pulmonary sclerosing hemangioma

Abstract:Background: Sclerosing hemangiomas of the lung are uncommon tumors and are thought to be benign. However, the histogenesis and clinicopathological features of these tumors have not been elucidated. Methods: We analyzed the clinicopathological features of 26 sclerosing hemangiomas. The immunoreactivity for Ki-67 and p53 of sclerosing hemangiomas was determined and compared with that of pathological stage 1 pulmonary papillary adenocarcinomas. Results: The patients of sclerosing hemangioma were predominantly female. Eighteen patients were detected as a result of routine medical examinations and 15 were nonsmokers. Seven patients underwent tumor enucleation, 10 underwent a wedge resection, and 9 underwent a lobectomy. The mean tumor size was 2.2 cm (range 1 to 5 cm). Pathological findings demonstrated a papillary pattern in 23 cases, sclerotic pattern in 26 cases, hemorrhagic pattern in 22 cases and a solid pattern in 25 cases. Twenty-five cases had an excellent prognosis with no evidence of recurrence following surgery. However, 1 patient who had undergone a wedge resection developed a local recurrence and required an additional wedge resection. The Ki-67 labeling index of sclerosing hemangiomas was significantly lower than that of adenocarcinomas, whereas the Ki-67 labeling index of the recurrent case was 0.4%. No significant immunohistochemical staining for p53 was observed in sclerosing hemangioma cases. Conclusions: Sclerosing hemangioma exhibits various histologic findings. Although we experienced one case with a recurrent tumor, sclerosing hemangiomas did not exhibit malignant behavior.

Abstract:Objective To understand clinical manifestation and pathological diagnosis of pulmonary sclerosing hemangioma.Methods Clinical and pathological manifestations of six cases were reported.Clinical manifestations,pathological featrues,diagnosis and differentiation of the disease in integration were discussed with relevant documents and clinical materials of the six cases.Results Most of the patients were found with no symptoms and characteristics of imagination showed the isolated and surrounded focus.Four types of pathological manifestations i.e papillary,solid,sclerolic and angiomatous could be found.Under the microscope,two types of cell components could be identified:cubical cell and polygonal cell.Conclusions Pulmonary sclerosing hemangioma is a kind of seldom lung benign tumor with favourable prognosis.Diagnosis can be made through pathological inspection after operation.

Abstract:Context: Sclerosing hemangioma is an unusual pulmonary tumor. Previously, 4 patients with pulmonary sclerosing hemangioma and lymph node metastases have been described in the literature. Objective: To report 4 additional cases of metastatic sclerosing hemangioma. Design: Retrospective review of the authors' consultation files and review of histologic sections of pulmonary tumors and lymph node metastases. Results: Four cases of a morphologically benign pulmonary sclerosing hemangioma with regional lymph node metastases (including hilar, peribronchial, and interlobar metastases) were identified. The patients (3 female, 1 male) had a mean age of 39 years (range, 10-56 years). The tumors ranged in size (greatest dimension) from 1.5 to 4.7 cm (mean, 3.1 cm). The pulmonary tumors were typical circumscribed sclerosing hemangiomas without mitotic activity, angiolymphatic invasion, or necrosis. One tumor had focal cytologic atypia. The metastases were identified in hilar lymph nodes that were removed at operation for the lung nodule. One patient received adjuvant chemotherapy for adenocarcinoma. All of the patients are alive. No recurrences or residual disease has been detected at a mean follow-up of 4.7 years (range, 2.3-10 years). Conclusions: On the basis of case data from the 4 patients described here and the 4 patients described previously, metastases to regional lymph nodes from pulmonary sclerosing hemangioma may occur but are rare and do not appear to affect prognosis.

Pulmonary sclerosing pneumocytoma: a clinicopathologic analysis of 23 cases

Abstract:Objective:To investigate the clinical manifestations, imaging characteristics and pathological features of pulmonary sclerosing pneumocytoma (PSP). Methods: A total of 23 cases of PSP were enrolled,and the clinical and pathologic features, imaging findings and immunophenotype were collected and analyzed retrospectively. The associated literature was reviewed. Results: PSP was more common in adult females than male, most patients were symptom-free (13/23,56.5%). Of the 23 cases, 3 cases (3/23, 13.0%)were preoperatively diagnosed as PSP by imaging, and 13 cases were diagnosed as PSP(13/23, 56.5%)by frozen section examination at operation and the diagnostic rate was low. Gross exami-nation revealed that PSP presented with a well-circumscribed mass, with no capsule or pseudocapsule ,and a grey to tan color cut surface, 7 cases had focal dark red hemorrhage area. Microscopic examination revealed that PSP showed four histological patterns (solid, papillary, hemorrhagic and sclerotic) and two types of tumor cells (cuboidal surface cells and round or polygon stromal cells). For immunophenotype, TTF-1 was strongly expressed on both surface cells and stromal cells. AE1/AE3, CAM5.2 and EMA were strongly expressed on the cell membrane and cytoplasm of surface cells, while weakly expressed on the cell cytoplasm of only a minority of stromal cells. NapsinA and SP-A were only expressed on sur-face cells, and Vimentin on stromal cells. All the 23 patients were followed up from 4 to 50 months, one case was found having contralateral pulmonary nodule of 1.5 cm on CT at 9 months after surgery, and radiographic surveillance was to be continued. The other 22 patients had no recurrence or metastasis. Conclusions: PSP lacks special clinical and imaging manifestations and having a high misdiagnosis rate of frozen section pathology examination. The pathological diagnosis de-pends on the morphologic characteristics of paraffin-embedded tissue sections and immunohistochemical staining.
