Abstract:Purpose. Siderosis bulbi may occur as a result of retained intralenticular foreign bodies (ILFBs) that were missed during examination in patients with self-sealing wounds and without a significant decrease in visual acuity. This study aimed to explore the clinical characteristics and visual outcomes of ILFBs with self-sealing corneal penetrating wounds. Methods. Fifteen eyes of 15 patients with ILFBs and self-sealing corneal penetrating wounds, seen between October 2014 and September 2019, were retrospectively analyzed. Data regarding the patient demographics, clinical features, surgical procedure, and initial and final best-corrected visual acuity (BCVA) were analyzed. Results. All patients were male with a mean age of 41 years. The foreign bodies passed through the cornea, sometimes through the iris, through the anterior capsule, and finally localized in the lens. All ILFBs were pointed and metallic objects and were successfully removed with phacoemulsification and posterior chamber intraocular lens (IOL) implantation. Anterior capsule violation was found in three eyes, but no posterior capsule rupture was found. The IOL was placed in a capsule bag in all the cases. The BCVA ranged from 20/200 to 20/25 preoperatively and improved to between 20/32 and 20/20 at the last follow-up visit. The IOLs were well-centered. Apart from posterior capsule opacity in four eyes, no other postoperative complications were found. Conclusions. In patients with a pointed metallic ILFB and self-sealing corneal penetrating wounds (with or without cataracts), early diagnosis and removal of the metallic ILFB combined with lens removal and IOL implantation may avoid late complications and achieve good visual outcomes.
Laser in Situ Keratomileusis Flap Free-Floating on Interface Edema in Acute Corneal Hydrops

Abstract:A 62-year-old patient with a remote history of laser in situ keratomileusis (LASIK) awoke with cloudy vision, tearing, photophobia, and sharp pain in the left eye. She reported requiring a rigid gas permeable contact for 11 years due to postrefractive ectasia. Slitlamp examination was consistent with acute hydrops (Figure, A). Central pachymetry measured over 1400 μm (fellow eye, 588 μm). Optical coherence tomography identified fluid channels throughout the stroma and an unstable LASIK flap resting on fluid in the flap interface (Figure, B). She was treated conservatively with topical moxifloxacin, prednisolone, and timolol, and the edema resolved by 11 weeks, leaving a dense scar. Six weeks later, she developed redness, pain, and photophobia and was found to have a small, branching posterior stromal infiltrate concerning for fungal keratitis. Penetrating keratoplasty was performed, with pathology identifying Aspergillus at 80% stromal depth. The postoperative course was uncomplicated, and final visual acuity was correctable to 20/15. It was suspected that use of topical corticosteroid in conjunction with channels of edema communicating with tenuous surface bullae led to a deep fungal keratitis.

Small Incision Lenticule Extraction and Femtosecond-Assisted Laser in Situ Keratomileusis in Patients with Deep Corneal Opacity: Case Series.

Abstract:AIM To report the safety, efficacy, and accuracy of small-incision lenticule extraction (SMILE) or femtosecond-assisted laser in situ keratomileusis (FS-LASIK) for the correction of myopia or myopic astigmatism in patients with deep corneal opacity denoted by anterior segment optical coherence tomography (AS-OCT). METHODS Four patients with monocular corneal opacity (3 due to mechanical injury, 1 due to a firecracker wound) were recruited and treated with refractive surgery (3 for SMILE, 1 for FS-LASIK combined with limbal relaxing incision (LRI). Preoperative ocular manifestations, surgical details, postoperative visual outcomes, corneal opacity parameters, and corneal topography were analyzed. RESULTS Preoperatively, spherical diopter ranged from -3.0 D to -4.75 D with cylinder ranging from -0.75 to -5.0 D, and corrected distance visual acuity (CDVA) ranging from 20/25 to 20/20. One eye's corneal opacity was located in the central zone and three were in the mid-peripheral optical zone. Three patients underwent uneventful SMILE in both eyes, whilst one patient underwent FS-LASIK for high astigmatism in both eyes and LRI in the right eye. CDVA of the eye with corneal opacity ranged from 20/22 to 20/20 one to six weeks postoperatively. Two patients achieved better CDVA and no patients lost Snellen lines. The postoperative diopter was within ±0.75 D for all eyes. Significant edema existed above the corneal opacity in one eye and dissipated soon. No eccentric corneal topography or morphological anomaly of the corneal cap or flap was observed. CONCLUSION The cases demonstrate that SMILE or FS-LASIK is safe and effective to treat myopic astigmatism combined with deep corneal opacity lesions after comprehensive preoperative evaluation and appropriate candidate selection. FS-LASIK combined with LRI is also sufficient for correcting high astigmatism due to corneal scarring.
Presbyopic Corneal Inlay Extrusion Consequent to Sterile Keratolysis: Surgical Revision and Long-Term Management

Abstract:Purpose: The purpose of this study was to report a serious complication of early sterile keratolysis associated with a presbyopia inlay implantation combined with hyperopic laser in situ keratomileusis (LASIK). Methods: A 55-year-old hyperopic man underwent uneventful topography-guided bilateral femtosecond laser-assisted hyperopic LASIK, combined with same-day polymer refractive inlay implantation in the nondominant eye for additional presbyopia correction. Within 2 months, anterior sterile keratolysis and partial extrusion required surgical explantation of the inlay, and long-term medical management over 9 years achieved effective visual rehabilitation documented also with corneal imaging. Results: Immediate postoperative results were binocular 20/20 uncorrected distance visual acuity and J1 uncorrected near visual acuity. The early progressive anterior sterile keratolysis and partial extrusion necessitated surgical explantation at 2 months, severe anterior scarring of the perforated flap area and severe focal flattening of the cornea of over 10 diopters, developed despite aggressive topical corticosteroid use. The uncorrected distance visual acuity in the OS dropped from 20/25 to 20/400. The LASIK flap portion that underwent keratolysis and perforated, remodeled long-term from a scarred, epithelial plug with early significant surface concavity to evidently a mild subepithelial localized hazy area with regularized curvature over a period of 9 years. Conclusions: Some refractive synthetic corneal inlays have been recalled because they may result in significant corneal haze. We present herein a case of severe keratolysis that necessitated surgical explantation and further document long-term care for eventual visual rehabilitation.
Latency-Associated Nuclear Antigen of Kaposi's Sarcoma-Associated Herpesvirus (KSHV) Upregulates Survivin Expression in KSHV-Associated B-Lymphoma Cells and Contributes to Their Proliferation

Abstract:ABSTRACT Survivin is a master regulator of cell proliferation and cell viability and is highly expressed in most human tumors. The molecular network linked to survivin expression in tumors has not been completely elucidated. In this study, we show that latency-associated nuclear antigen (LANA), a multifunctional protein of Kaposi's sarcoma-associated herpesvirus (KSHV) that is found in Kaposi's sarcoma tumors, upregulates survivin expression and increases the proliferation of KSHV-infected B cells. Analysis of pathway-specific gene arrays showed that survivin expression was highly upregulated in BJAB cells expressing LANA. The mRNA levels of survivin were also upregulated in HEK 293 and BJAB cells expressing LANA. Similarly, protein levels of survivin were significantly higher in LANA-expressing, as well as KSHV-infected, cells. Survivin promoter activity assays identified GC/Sp1 and p53 cis-acting elements within the core promoter region as being important for LANA activity. Gel mobility shift assays revealed that LANA forms a complex with Sp1 or Sp1-like proteins bound to the GC/Sp1 box of the survivin promoter. In addition, a LANA/p53 complex bound to the p53 cis-acting element within the survivin promoter, indicating that upregulation of survivin expression can also occur through suppression of p53 function. Furthermore, immunohistochemistry analyses revealed that survivin expression was upregulated in KSHV-associated Kaposi's sarcoma tissue, suggesting that LANA plays an important role in the upregulation of survivin expression in KSHV-infected endothelial cells. Knockdown of survivin expression by lentivirus-delivered small hairpin RNA resulted in loss of cell proliferation in KSHV-infected cells. Therefore, upregulation of survivin expression in KSHV-associated human cells contributes to their proliferation.

Abstract:Objective:To observe the flap complications after LASIK and analyze their clinical characters and treatment. Methods:The flap complications after LASIK in 35 eyes of 27 patients was reviewed.Visual acuity were measured at 3,7,15,30,60 days after the operation,at the same time, the eyes were examined with slit lamp. Results:After the therapy,diffuse lamellar keratitis of 10 eyes were recovered in 1 to 2 months,the visual acuity were 0.8 to 1.5.Flap slippage of 9 eyes were treated with replacement and the visual acuity were 1.0 to 1.2.One eye with trauma induced flap lose underwent allogeneic keratomileusis and the visual actity was 0.5 to 0.6.One eye with interface infection was treated by removing the infected flap and PTK.The corneal surface was healed but remained with little haze.The visual acuity were 0.8 to 1.0 after the therapy in three eyes with epithelial ingrowth.Three eyes with fiap breaking in the procedure were reoperated half a year later,and the visual acuity were 1.0 to 1.2. Conclusion:Flap complications after LASIK occurred at various period and are related with surgical technic,patient~(,)s eyes conditions and the treatment after the surgery.Early detection and therapy can obtain an comparatively excellent recovery.
A Tikhonov Regularization Scheme for Focus Rotations With Focused Ultrasound-Phased Arrays

Abstract:Phased arrays have a wide range of applications in focused ultrasound therapy. By using an array of individually driven transducer elements, it is possible to steer a focus through space electronically and compensate for acoustically heterogeneous media with phase delays. In this paper, the concept of focusing an ultrasound-phased array is expanded to include a method to control the orientation of the focus using a Tikhonov regularization scheme. It is then shown that the Tikhonov regularization parameter used to solve the ill-posed focus rotation problem plays an important role in the balance between quality focusing and array efficiency. Finally, the technique is applied to the synthesis of multiple foci, showing that this method allows for multiple independent spatial rotations.

Abstract:Background: Although flap-based laser refractive surgeries are being extensively performed worldwide, complications due to the flap may occur years after the procedure leading to severe vision-threatening complications. Purpose: To describe the management of a case of traumatic displacement of a LASIK flap performed 8 years earlier. Synopsis: A 42-year-old male presented to us with complaints of decreased vision in his left eye following trauma with a toy gun sustained a day earlier. His best-corrected visual acuity was Counting fingers @ 2 m, and a slit-lamp evaluation revealed a LASIK flap with its temporal edge folded inwards. Flap repositioning was planned. The final surgical plan was interface irrigation with mechanical debridement, alcohol epitheliectomy, fibrin glue application, and bandage contact lens. His uncorrected visual acuity improved to 6/6, N6 at 5 weeks postoperatively, which was maintained at 8 months postoperatively along with a clear interface. Highlights: Traumatic displacement of LASIK flap years after the procedure is a known complication. However, simple flap repositioning may lead to various complications like epithelial ingrowth, DLK, astigmatism, etc., This video describes how to achieve anatomical and visual rehabilitation in such eyes without any long-term complications. Abstract:Abstract Hereditary pediatric retinal diseases are a diverse group of disorders with pathologies affecting different cellular structures or retinal development. Many can mimic typical pediatric retinal disease such as retinopathy of prematurity, vitreous hemorrhage, retinal detachment and cystoid macular edema. Multisystem involvement is frequently seen in hereditary pediatric retinal disease. A thorough history coupled with a good physical examination can oftentimes lead the ophthalmologist or pediatrician to the correct genetic test and correct diagnosis. In some instances, evaluation of parents or siblings may be required to determine familial involvement when the history is inconclusive or insufficient and clinical suspicion is high.
The Epithelial-Flap Abnormality of Laser Epithelial Keratomileusis

Abstract:OBJECTIVE:To investigate the technique of epithelial-flap performance and reposition and the factors related to the complication to improve the quality of laser epithelial keratomileusis (LASEK).METHODS:Fourteen eyes (9 cases) of epithelial-flap abnormality occurring in and after the operation in 309 eyes were observed and analyzed. The factors related to the abnormal flap were listed and discussed.RESULTS:The abnormal epithelial flaps of LASEK during and after the operation were free epithelial-flap, dissolution, fragments, fold, slip, and the late epithelial-flap abnormalities including epithelial defect and sub-epithelial foreign body 48 - 72 hours postoperatively. The related factors of complication included the shallow cut of micro-trephine, improper (too small) cut ring, improper concentration of alcohol, the invasion of alcohol solution, dry center of epithelial-flap, corneal epithelial injury by irrigation, and too early removal of contact lens.CONCLUSION:The epithelial-flap of LASEK is frailer than that of LASIK. The key point in the performance of a fine epithelial-flap is to manage every procedure carefully.

Abstract:We have previously shown that in vitro measurement of cytokine production prior to renal transplantation can provide predictive information on the risk of acute rejection. Our earlier studies demonstrated that patients who secreted high levels of interferon‐gamma (IFN‐γ) in OKT3‐stimulated or mixed lymphocyte culture had a significantly increased risk of acute rejection compared with patients who secreted lower levels. In this study, we performed a retrospective analysis of the same cohort of patients in order to determine the prognostic value of cytokine profiles and other variables on long‐term graft function. Our results show that high levels of IFN‐γ in pretransplant mixed lymphocyte culture are a highly significant predictor of poorer creatinine levels at 18, 24 and 36 months post‐transplant.

Abstract:UNLABELLED REQUIREMENT: Information given to the patient is a topic which is especially of current interest, and which has been taken into account in the recent law of March 4, 2002 on patient's rights. But what about non-steroidal anti-inflammatory agents, that is, one of the most widely prescribed classes of medicinal products worldwide and which induce the highest number of sometimes fatal adverse effects (adverse drug reactions)? FREQUENT MISREADING The aim of this update is thus to review once again the rare studies published on this topic, a review necessary prior to the formulation of recommendations regarding information to be given to the patient before prescribing him/her an NSAIDs. A review of the literature illustrates the common misunderstanding that patients have (nearly 1/4 to 1/2) regarding the potential serious gastro-intestinal adverse effects of NSAIDs. RECOMMENDATIONS We propose that the subjects at the very least, be informed first of the risk of dyspepsia, which is the most common side-effect but usually not predictive of a serious adverse effect; then we would point out the risk of gastro-duodenal ulcer whose complications certainly are rare but can be fatal; and lastly the existence of prophylactic strategies, including the recognition of main risk factors and some medical options, such as use of synthetic prostaglandins, proton pump inhibitors or the preferential use of coxibs.
Incidence and complications of retained lens fragment in the anterior chamber after uneventful cataract surgery in a United Kingdom tertiary center

Abstract:PURPOSE: To analyze the outcomes of retained lens fragment (RLF) in the anterior chamber after uneventful cataract surgery.SETTING: Moorfields Eye Hospital, NHS Foundation Trust, London, United Kingdom.DESIGN: Retrospective, observational cross-sectional study.METHODS: A single-center, retrospective review was performed to identify patients who underwent RLF removal after uneventful phacoemulsification surgery between October 2012 and November 2018; 122 eyes from 121 patients were identified. Patient characteristics, clinical findings, visual outcomes, and need for subsequent surgical procedure were recorded. Main outcomes were change in corrected distance visual acuity (CDVA), and number and risk factors associated with additional surgery following RLF removal.RESULTS: One hundred twenty-two RLF over a total of 98 467 uneventful phacoemulsification surgeries were identified, with an incidence of 1 (0.124%) in 807 operations. The mean CDVA improved significantly after RLF removal from 0.32 to 0.26 logMAR (SD 0.26) (P = .001). Six eyes (4.9%) had persistent corneal edema that required endothelial keratoplasty (EK) after a mean of 13 months after RLF removal (SD 12 months; median 8; range 4 to 35). Risk factors for EK include α-receptor blocker use (odds ratio [OR], 6.75; 95% CI, 1.069, 42.63), increased interval (month) between cataract surgery and diagnosis of RLF (OR, 1.29; CI: 1.080, 1.541), increased interval between cataract surgery and RLF washout (OR, 1.28; CI, 1.075, 1.522), and RLF diagnosis on gonioscopy (OR, 10.60; CI, 1.885, 59.59).CONCLUSIONS: RLF is a rare complication of uneventful cataract surgery and appears more frequently in challenging cataract cases and myopic eyes. CDVA improved significantly after RLF removal, but approximately 1 in 20 eyes needed additional surgery-mainly EK for corneal decompensation.

Abstract:Purpose To report a case of ulcerative keratopathy following implantation of acellular porcine corneal stroma (APCS) in a patient with keratoconus (KC). Methods A 58 year-old patient initially presented with an ulcerative keratopathy in the left eye. Previously, several corneal procedures (including radial keratotomy, laser-in-situ keratomileusis, crosslinking) were performed for KC. Eight months ago, an APCS lenticule (Xenia corneal implant, Gebauer Medizintechnik GmbH, Neuhausen, Germany) was implanted into a stromal pocket because of progressive keratectasia. Visual acuity was hand movement. Anterior segment optical coherence tomography showed a space between the APCS lenticule and the host stroma. Excimer laser-assisted penetrating keratoplasty (PKP, 8.0/8.1 mm) was performed in the left eye. The corneal explant was investigated by light and transmission electron microscopy. Results Best-corrected visual acuity was 20/40 six weeks after PKP. Light microscopy demonstrated a stromal ulceration down to the APCS lenticule. No stromal cells could be found within the APCS lenticule eight months after implantation. The APCS lenticule did not show a green stain of the collagens with Masson-Goldner staining and exhibited a strong Periodic acid-Schiff positive reaction. Electron microscopy of the APCS lenticule revealed cross-linked collagen lamellae without cellular components. Close to the interface, corneal collagen lamellae of the host cornea were disorganized. Few vital keratocytes were present on the surface of the lenticule and appeared to cause mechanical disruption of the host stroma along the lenticule-stroma interface. Conclusion APCS implantation may lead to severe complications such as ulcerative keratopathy in otherwise uncomplicated KC corneas. In such cases, excimer laser-assisted PKP or Deep Anterior Lamellar Keratoplasty are the methods of choice to restore visual acuity.

A Case of Transparent Cornea Maintained after Removal of Dislocated Graft from Descemet's Stripping Automated Endothelial Keratoplasty

Abstract:Introduction: Corneal graft detachment is a major postoperative complication of Descemet's stripping automated endothelial keratoplasty (DSAEK). When a corneal graft becomes detached, corneal endothelial function generally fails, and repeat corneal transplantation is required. Herein, we report a rare case in which a transparent cornea was maintained after the removal of a dislocated DSAEK graft. Case presentation: A 79-year-old woman with a residual lens cortex who had undergone cataract surgery was referred to our hospital. The cortex was removed, and bullous keratopathy progressed. Six months after the initial surgery, DSAEK was performed under topical anesthesia without any complications. Although the corneal graft had attached fairly well, it detached from the host cornea 3 weeks later. Two months after DSAEK, an air tamponade was used to treat the anterior chamber with single interrupted suturing; however, the graft detached again, except for the suture site. Because the detached cornea became cloudy in the anterior chamber, it was surgically removed 8 months after DSAEK. Accordingly, the host cornea transparency improved to a best-corrected visual acuity of 0.8 with a rigid gas permeable lens and a central corneal thickness of 580 μm. The corneal endothelial cell density was 995 cells/mm2. Conclusion: Removal of the corneal graft from the dislocated cloudy graft improved the visual acuity of this patient after DSAEK. The condition of the cornea should be carefully monitored after corneal endothelial transplantation, even after the graft has been dislocated.

K-6y4 and a bacterial bioluminescence operon (lux operon) from an ocean bacterium Vibrio fischeri were fused in avector plasmid. The resulting recombinant plasmids were cloned in Escherichia coli cells. The bioluminescence sensor systems responded to mercury chloride of 0.1 nM to 100 nM. The mercury bioluminescence sensor developed in this study can be used for monitoring of the bio-affecting mercury instead of total mercury that is measured by conventional analytical equipment. The fundamental feature of the bioluminescence sensor system is attractive for use as a monitoring system for bio-affecting environmental mercury contamination.
Laser-assisted Subepithelial Keratomileusis Substituted for an Aborted Small-Incision Lenticule Extraction Due to Large Black Area Formation.

Abstract:A 25-year-old woman with moderate myopia presented for refractive surgery. Bilateral femtosecond laser–assisted small-incision lenticule extraction (SMILE) was scheduled and her right eye was completed smoothly. However, during the lenticule cutting of her left eye, a large inferior black area was noted. The operation was abandoned after an immediate optical coherence tomography examination, which revealed the corneal epithelium defect with no laser scanning line at the corresponding site. The secondary surgery was assigned to laser-assisted subepithelial keratomileusis (LASEK) after 1 week. The uncorrected distance visual acuity of her left eye recovered to 20/25 on the 12th day and to 20/20 at 3-month follow-up, with ideal corneal topography profiles. Corneal epithelium defect induced by accidental alcohol contact during disinfection was suspected to cause the black area. The management of black area had to be determined according to the location and size. LASEK was a rational substitution for the aborted SMILE.

Abstract:Laser in situ keratomileusis (LASIK) is one of the primary types of refractive surgery. However, epithelial ingrowth due to traumatic flap dislocation is a serious complication that may occur after LASIK. [ 1 , 2 ] Ophthalmologists mainly treat this complication by lifting the flap and scraping the ingrowth or using scraping combined with phototherapeutic keratectomy (PTK), [ 3 ] mitomycin C, [ 4 ] neodymium-doped yttrium-aluminum-garnet laser, [ 5 ] and other treatments. This report presents 2 patients who developed post-LASIK epithelial ingrowth due to trauma and discusses the effect of nonsteroidal anti-inflammatory drugs (NSAIDs) on the inhibition of corneal epithelial cell proliferation. The potential usefulness of NSAIDs in such circumstances has not been reported before.
