Abstract:Objective:To explore the effect of lipoic acid on oxidative stress,inflammation and nutritional status in peritoneal dialysis patients.Methods:94 peritoneal dialysis patients were randomly divided into the observe group(n=47) the control group(n=47).They all accepted the conventional treatment,but the patients in the observed group were given the treatment of lipoic acid capsules(0.2 g tid) for 12 weeks.Superoxide dismutase(SOD),malondialdehyde(MDA),serum advanced oxidation protein products ( AOPPs ) were used to reflect the level of oxidative stress, high sensitive CRP, IL-6 and TNF-αwere used to reflect the micro inflammatory state, modified quantitative subjective global assessment ( MQSGA ) , mid-arm circumference, mid-arm muscle circumference,triceps skinfold thickness ( TSF ) , body mass index ( BMI ) , and serum albumin were used to reflect the nutritional status.The difference of oxidative stress, micro inflammatory state and nutritional status were compared between the two groups.Results:①In the observation group,the concentrations of AOPPs and MDA after treatment were significantly lower than that before treatment,and the concentration of SOD after treatment was significantly higher than that before treatment ( P<0.05 );after treatment,the concentrations of AOPPs and MDA in the observe group were significantly lower than that in the control group(P<0.05), and the concentration of SOD in the observe group was significantly higher than that in the control group ( P<0.05 ) .②In the observation group,the concentrations of high sensitive CRP,IL-6 and TNF-αafter treatment were significantly lower than that before treatment(P<0.05);after treatment,the concentrations of high sensitive CRP,IL-6 and TNF-αin the observe group were significantly lower than that in the control group ( P<0.05 ) .③In the observation group, the TSF, mid-arm circumference, mid-arm muscle circumference,BMI and albumin after treatment were significantly higher than that before treatment(P<0.05),and the MQSGA after treatment was significantly lower than that before treatment ( P<0.05 );after treatment, the MQSGA in the observe group was significantly lower than that in the control group(P<0.05),and serum albumin in the observe group was significantly higher than that in the control group(P<0.05).Conclusion:Lipoic acid could improve oxidative stress in peritoneal dialysis patients,so as to improve the micro inflammatory state and nutritional status.

Abstract:BACKGROUND: Oxidative stress is prevalent in dialysis patients and has been implicated in the pathogenesis of cardiovascular disease and anemia. Vitamin E is a fat-soluble antioxidant that plays a central role in reducing lipid peroxidation and inhibiting the generation of reactive oxygen species. The aim of this cross-over randomized study was to compare the effects of a vitamin E-coated polysulfone (Vit E PS) membrane and a non-vitamin E-coated polysulfone (PS) membrane on inflammatory markers and resistance to erythropoietin-stimulating agents (ESAs).METHODS: After a 1-month run-in period of standard bicarbonate dialysis with a synthetic membrane, 62 patients of both genders, and older than 18 years, dialysis vintage 48 ± 27 months, BMI 22 ± 3 (from 13 different dialysis units) were randomized (A-B or B-A) in a cross-over design to Vit E PS (treatment A) and to PS (treatment B) both for 6 months. C-reactive protein (CRP) and interleukin-6 (IL-6) concentrations were determined by a sandwich enzyme immunoassay at baseline and every 2 months; red blood cell count, ESA dose and ESA resistance index (ERI) were assessed monthly.RESULTS: Hemoglobin (Hb) levels significantly increased in the Vit E PS group from 11.1 ± 0.6 g/dl at baseline to 11.5 ± 0.7 at 6 months (p < 0.001) and remained unchanged in the PS group. Although ESA dosage remained stable during the observation periods in both groups, ERI was significantly reduced in the Vit E PS group from 10.3 ± 2.2 IU-dl/kg/g Hb week at baseline to 9.2 ± 1.7 at 6 months (p < 0.001). No significant variation of ERI was observed in the PS group. A significant reduction in plasma CRP and IL-6 levels was observed in the Vit E PS group: CRP from 6.7 ± 4.8 to 4.8 ± 2.2 mg/l (p < 0.001) and IL-6 from 12.1 ± 1.4 to 7.5 ± 0.4 pg/ml (p < 0.05). In the PS group, CRP varied from 6.2 ± 4.0 to 6.4 ± 3.7, and IL-6 from 10.6 ± 2.1 to 9.6 ± 3.5 (p = n.s.).CONCLUSIONS: Treatment with Vit E PS membranes seems to lead to a reduction in ESA dosage in HD patients; in addition, a low chronic inflammatory response may contribute to a sparing effect on exogenous ESA requirements.

Abstract:Objective To investigate the effect of puerarin on oxidative stress in continuous ambulatory peritoneal dialysis (CAPD) patients.Methods 36 stable CAPD patients were randomized into experimental group(n=18) dialyzed with the peritoneal dialysis fluid containing 50 mg·(2 L)-1 puerarin and control group(n=18) dialyzed with the regular peritoneal dialysis fluid.Serum albumin,KT/V,creatinine clearance rate(Ccr),oxidative stress markers of both serum and dialysis effluent such as glutathione(GSH),total-superoxide dismutase(T-SOD),malondialdehyde (MDA) were evaluated at the begining and the end of the 16-day observation.Results Sixteen days later,GSH,T-SOD and MDA in both serum and effluent were improved significantly in the experimental group as compared with the beginning as well as compared with the control group after 16 days(P0.05).However such improvement were not observed in the control group.And neither intragoup nor intergroup difference were found in serum albumin,KT/V,ultrafiltration and Ccr.Conclusion Supplementation of puerarin into peritoneal dialysis fluid can ameliorate oxidative stress in CAPD patients without apparent side effect.

Abstract:Objective:To investigate the impact of Puerarin on oxidative stress in patients undergoing continuous ambulatory peritoneal dialysis(CAPD).Methods: Thirty-eight patients CAPD were randomly divided into two groups,an experimental group(n=19) receiving peritoneal dialysis fluid containing Puerarin(50 mg/2L) in vivo for two weeks and a control group(n=19) receiving dialysis fluid without Puerarin.The concentrations of oxidative stress markers in the serum and effluent such as glutathione(GSH),total-superoxide dismutase(T-SOD),malondialdehyde(MDA),Kt/V and creatinine clearance rate of the two groups(Ccr) were measured and compared before and after the treatment.Results: After the treatment,the concentrations of GSH and T-SOD in the serum and effluent were significantly higher(P0.05) and that of MDA in the effluent was significantly lower in the experimental group than before the treatment(P0.05).No significant difference was found in the serum albumin,Kt/V,the volume of ultrafiltration and Ccr.Conclusion: Short-term supplementation of Puerarin to peritoneal dialysis fluid can ameliorate oxidative stress in patients on CAPD without apparent side effects.
[The effect of hemodialysis on some parameters of the antioxidant system in the blood of patients with chronic renal failure]

Abstract:Direct contact of the blood with the dialysis membrane during hemodialysis elicits a series of changes in blood cells. White blood cell count and total lymphocyte number are reduced, neutrocytes are stimulated and degranulate, platelet adhesiveness is enhanced. Interactions of granulocytes with the dialysis membrane stimulate the production of ROS and activate aerobic reactions triggering oxidative stress. Red blood cells form the "first line of defense" during contact of the blood with the dialysis membrane. The effect of ROS on erythrocytes appears in the form of reduced osmotic resistance of their cellular membrane and susceptibility to disintegration which results in the release of small quantities of hemoglobin and significant reduction in the lifetime of these cells. Oxidative stress caused by contact of the blood with the dialysis membrane triggers defense mechanisms in the erythrocyte that protect against oxidative damage. Reports on the effects of hemodialysis on the antioxidant system of the blood in patients with chronic renal failure are conflicting and the present work was aimed at resolving some of the discord. The following questions were addressed: (1) Does hemodialysis affect the antioxidant system of the blood in patients with chronic renal failure? (2) Are there associations between parameters of hemodialysis and of the blood's antioxidant system? (3) Is supplementation with antioxidants needed in hemodialysed patients? The experiments were done at the Department of Biochemistry and Chemistry, Pomeranian Medical University. Plasma and erythrocytes were obtained from 42 patients aged 67.7 +/- 12.3 years, treated at the Department of Nephrology, Transplantology and Internal Medicine, Pomeranian Medical University, due to chronic renal failure caused by chronic glomerulonephritis (n=16), pyelonephritis (n=12), diabetes (n=3), arterial hypertension (n=3) or other cause (gout, nephrolithiasis, n=8). The patients were assigned to two equal groups: 1--with end-stage renal failure on hemodialysis; 2--eated conservatively. Hemodialysis was done three times per week, 4 hours per session, using Fresenius dialysers with polysulfone membranes and bicarbonate buffer with glucose. The control group comprised 21 healthy volunteers. All groups were matched for age and gender. A 10 mL heparinized blood sample was collected twice from group 1 patients before and after dialysis directly from the dialysis catheter and once after an overnight fast from group 2 patients and controls. Glutathione concentration was determined in whole blood. Plasma was prepared by centrifugation and MDA, Se, Cu, Zn, and Fe levels were measured. SOD, GSH-Px, and CAT activities, MDA, Se, Cu, Zn, and Fe levels were determined in erythrocytes. Statistical methods were applied to determine significance of differences between groups and correlations between parameters studied. Conclusions: (1) Hemodialysis leads to significant changes in the antioxidant system of the blood of patients with chronic renal failure. The effect is noticeable with antioxidant enzyme activities and concentrations of nonenzymatic components of the system. (2) The activity of erythrocyte GSH-Px decreases with time of dialysis potentiating the effects of oxidative stress. (3) Reduced erythrocyte levels of Se in dialysed patients correlate with lower GSH-Px activities. Selenium supplementation may be of benefit to the antioxidative defense mechanisms.
Residual renal function affects lipid profile in patients undergoing continuous ambulatory peritoneal dialysis

Abstract:Objective: To determine whether lipoprotein abnormalities associated with continuous ambulatory peritoneal dialysis (CAPD) are influenced by residual renal function (RRF). Design: Open, nonrandomized prospective and comparative study. Setting: Single university teaching hospital dialysis unit and outpatient clinic. Patients: Twenty adult patients on standard CAPD (1-38 months) were divided into two groups: group A (RRF < or = 0.8 mL/min, n = 10) and group B (RRF > or = 1.1 mL/ min, n = 10), Patients in the two groups were matched for age, time on dialysis, body weight, body mass index, serum urea and albumin levels, peritoneal and urinary albumin losses, and peritoneal transport characteristics such as overnight 8-hour peritoneal creatinine and beta 2-microglobulin clearances and overnight B-hour effluent glucose concentrations. Results: The degree of uremia in patients with preserved RRF (group B) was obviously lower than in patients with negligible RRF (group A), that is, patients in group B had significantly lower serum creatinine and beta 2-microglobulin levels and significantly higher weekly KT/V than group A patients. Despite the prevalence of allele 4 of apolipoprotein E genotype in group A patients, their levels of serum total cholesterol, low-density lipoprotein cholesterol, lipoprotein (a) [Lp(a)], apolipoprotein B(ApoB), and apolipoprotein A1 (ApoA1) were significantly lower than those of patients with preserved RRF (group B). The two groups did not differ significantly in the serum levels of triglyceride or high-density lipoprotein cholesterol. Serum concentrations of Lp(a) and ApoA1, as well as ratios of ApoA1 to ApoB, were correlated significantly with RRF (r = 0.63, r = 0.51, and r = 0.61, respectively). Conclusions: The findings suggest that RRF affects the lipid profile of CAPD patients, especially serum levels of cholesterol-rich lipoproteins.
Peritoneal dialysis effluent, cytokine levels, and peritoneal mesothelial cell viability in CAPD: a possible relationship

Abstract:Recent studies have emphasized the role of peritoneal mesothelial cell (PMC) in peritoneal immune defense mechanisms in continuous ambulatory peritoneal dialysis (CAPD). The aim of this study was to evaluate a possible relationship between peritoneal dialysis effluent (PDE), cytokine (Cy) levels, and PMC viability and their impact on peritonitis morbidity. Fifteen patients initiating CAPD for end-stage renal failure participated in the study. The following parameters were evaluated: (1) the levels of interleukin-1 beta (IL-1 beta), tumor necrosis factor-alpha (TNF-alpha), interferon-gamma (IFN-gamma), interleukin-6 (IL-6), and interleukin-8 (IL-8) in PDE samples taken 7 days after initiating CAPD, at the end of the first, third, and sixth month of CAPD (determined by a solid phase enzyme amplified sensitivity immunoassay EASIA); (2) peritoneal mesothelial cell viability [determined by the release of lactate dehydrogenase (LDH) and by trypan blue extrusion test] by isolating and culturing peritoneal mesothelial cells at the moment of the placement of the peritoneal catheter and at the sixth month of CAPD; (3) peritonitis incidence during the 24 months after starting CAPD. At the first month of CAPD in all patients there was a slight increase in PDE IL-1 beta and TNF-alpha levels, while other Cy were almost undetectable. Time course studies showed that in 10 patients (Group I) there was a significant increase in PDE levels of IL-6, IL-8, and INF-gamma (p < 0.0005) in comparison to other Cy and a good PMC viability. In the other 5 patients (Group II) there were higher PDE levels of IL-1 beta and TNF-alpha (p < 0.0005). This was associated with a marked reduction in PMC viability determined by the release of LDH and by the trypan blue extrusion test. During the 24 months after starting CAPD, incidence of peritonitis was one episode per 24 patient-months in Group I and one episode per 9.2 patient-months in Group II. Our results show that from the beginning of CAPD there are distinct patterns of Cy in the PDE that correlate with a different PMC viability and peritonitis morbidity. Thus the analysis of the above-mentioned parameters may be useful in the early identification of the risk of peritonitis, thus allowing preventive measures.
Impacts of Tanshinone ⅡA on Oxidative Stress in Continuous Ambulatory Peritoneal Dialysis Patients

Abstract:Objective:To investigate the impact of Tanshinone ⅡA on oxidative stress in continuous ambulatory peritoneal dialysis(CAPD) patients.Methods:Thirty-eight stable CAPD patients were randomly divided into two groups.The experimental group(n=19) were dialyzed with peritoneal dialysis fluid along with 5 mg·L-1Tanshinone ⅡA,and the control group(n=19) were dialyzed with peritoneal dialysis fluid without Tanshinone ⅡA.The concentration of glutathione(GSH),total-superoxide dismutase(T-SOD),malondialdehyde(MDA) in serum and effluent and serum albumin were measured at the beginning and three weeks later.And dialysis adequancy indices such as KT/V and creatinine clearance rate(Ccr) were also calculated for each group.Results:Three weeks later,the serum and effluent GSH,T-SOD increased and MDA decreased significantly in the experimental group as compared with those indices at the beginning(P0.01 or P0.05).No such differences were found in the control group.The levels of the serum and effluent GSH,T-SOD were significantly higher,and MDA was lower in experimental group as compared with those in the control group after three weeks treatment(P0.01 or P0.05).There were no significant differences in serum albumin,KT/V,ultrafiltration volume and Ccr between two groups.The levels of GSH,T-SOD were significantly higher and the levels of MDA was significantly lower in experimental group compared with those in control group(P0.05).Conclusion:Short-term supplement of Tanshinone ⅡA into peritoneal dialysis fluid can reduce both local and systemic oxidative stress in CAPD patients.
Accumulation of Circulating Advanced Oxidation Protein Products is an Independent Risk Factor for Ischaemic Heart Disease in Maintenance Haemodialysis Patients

Abstract:AIM:Whether the burden of advanced oxidation protein products (AOPP) accumulation, a marker of oxidative stress, is affected by dialysis modality remains unclear. We compared the serum levels of AOPP in patients on haemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD) and tested the hypothesis that an accumulation of AOPP was an independent risk factor for cardiovascular disease.METHODS:This was a cross-section study. A total of 2095 patients (1539 HD, 556 CAPD) were recruited from the nine largest dialysis centres in China. Persons in medical centres for disease screening were selected as controls. Patients maintained on HD were dialyzed twice or thrice weekly. CAPD patients used lactate-buffered, glucose-containing solutions. The patients' data were abstracted from the medical record. The serum levels of AOPP were determined by spectrophotometric detection.RESULTS:The levels of AOPP were significantly elevated in both HD and CAPD patients compared to healthy controls. Accumulation of AOPP was more significant in HD compared to CAPD population. Meanwhile, AOPP accumulation was associated with the presence of ischaemic heart disease (IHD) only in HD, but not CAPD patients. A higher proportion of IHD was found in the HD population among those with higher levels of AOPP in each category of age and irrespective of the presence or absence of high triglyceride. Multivariate regression analysis indicated that accumulation of AOPP was an independent risk factor for IHD in HD population.CONCLUSION:Accumulation of AOPP was more significant in HD compared to CAPD patients. The level of AOPP was independently associated with IHD only in HD patients.
Alterations of erythrocyte rheology and cellular susceptibility in end stage renal disease: Effects of peritoneal dialysis

Abstract:In this study, we investigated the effects of peritoneal dialysis on hemorheological and hematological parameters and their relations with oxidant and antioxidant status of uremic patients. Hemorheological parameters (erythrocyte deformability, erythrocyte aggregation, osmotic deformability, blood and plasma viscosity) were measured in patients with renal insufficiency undergoing peritoneal dialysis (PD) and volunteers. Erythrocyte deformability, osmotic deformability and aggregation in both autologous plasma and 3% dextran 70 were measured by laser diffraction ektacytometry. Enzyme activities of glutathione peroxidase, superoxide dismutase and catalase were studied in erythrocytes; lipid peroxidation was studied by measuring the amount of malondialdehyde in both erythrocytes and plasma samples. Blood viscosity at native hematocrit was significantly lower in PD patients at all measured shear rates compared to controls, but it was high in PD patients at corrected (45%) hematocrit. Erythrocyte deformability did not show any difference between the two groups. Osmotic deformability was significantly lower in PD patients compared to controls. Aggregation index values were significantly high in PD patients in plasma Catalase and glutathione peroxidase activities in erythrocytes were decreased in PD patients whereas superoxide dismutase activity was increased compared to controls. Malondialdehyde was significantly increased in erythrocytes and plasma samples of PD patients which also shows correlations with aggregation parameters. It has been concluded that erythrocytes in PD patients are more prone to aggregation and this tendency could be influenced by lipid peroxidation activity in patient's plasma. These results imply that uremic conditions, loss of plasma proteins and an increased risk of oxidative stress because of decreasing levels of antioxidant enzymes affect erythrocyte rheology during peritoneal dialysis. This level of distortion may have crucial effects, impairing the blood flow dynamics and causing inadequate microcirculatory perfusion.

Abstract:OBJECTIVES:Accumulation of tissue advanced glycation end products (AGEs) is a marker of cumulative glycemic and/or oxidative stress. Cutaneous AGEs levels measured by skin autofluorescence correlate well with cardiovascular outcomes in diabetes and hemodialysis (HD) patients. The present study aimed to compare tissue AGEs levels with peritoneal dialysis (PD) and HD patients and to evaluate the relationship between skin autofluorescence and cardiovascular morbidity in patients on PD.METHODS:A total of 2388 maintenance dialysis patients (613 PD and 1775 HD) were enrolled in this cross-sectional study. Skin autofluorescence was measured non-invasively with an autofluorescence reader. Cardiovascular morbidity was defined as clinically diagnosed ischemic heart disease, heart failure, stroke or peripheral vascular disease from initiation of dialysis.RESULTS:More than 90% of patients on both PD and HD had met current dialysis adequacy targets. Compared to HD group, PD patients receiving conventional glucose-containing dialyzate had significantly higher skin autofluorescence values in each category of age and dialysis duration, irrespective of the presence or absence of diabetes. In PD patients, skin autofluorescence values were strongly correlated with the duration of PD and glucose exposure dose and independently associated with cardiovascular morbidity. Multivariate analysis revealed that glucose exposure dose and skin autofluorescence were the strongest risk factors for cardiovascular morbidity in PD patients after adjustment by age, gender, and other classic- or uremic-related risk factors.CONCLUSIONS:Accumulation of tissue AGEs provides a potential link between PD exposure of metabolic stress and progression of cardiovascular disease in patients on PD.

Abstract:Background: We report outcomes on >= 4 compared with <4 exchanges/day in a Chinese cohort on continuous ambulatory peritoneal dialysis (CAPD). Methods: Data were sourced from the Baxter (China) Investment Co. Ltd Patient Support Program database, comprising an inception cohort commencing CAPD between 1 January 2005 and 13 August 2015. We used cause-specific Cox proportional hazards and Fine-Gray competing risks (kidney transplantation, change to hemodialysis) models to estimate mortality risk on >= 4 compared with < 4 exchanges/day. We matched or adjusted for age, gender, employment, insurance, primary renal disease, size of CAPD program, year of dialysis inception, and treatment center. Results: We modeled 100,022 subjects from 1,177 centers over 239,876 patient-years. Of these subjects, 43,185 received <4 exchanges/day and 56,837 >= 4 exchanges/day. The proportion of patients on < 4 exchanges/day varied widely between centers. Those on < 4 exchanges/day were significantly older, more often female, of unknown employment, and from rural China. In the various models, >= 4 exchanges/day was associated with a significantly lower risk of death by 30% - 35% compared with < 4 exchanges/day. This beneficial effect was greatest in younger and rural patients. Conclusions: In this Chinese CAPD cohort, >= 4 exchanges/day was associated with significantly lower mortality risk than < 4 exchanges/day. Analyses are limited by residual confounding from unavailability of important prognostic covariates (e.g., comorbidity, socioeconomic factors) and data on residual renal function, peritoneal clearance, and transport status with which to judge the clinical appropriateness of CAPD prescription. Nonetheless, our study indicates this area as a high priority for further detailed study.
Assessment of iron metabolism and iron deficiency in incident patients on incident continuous ambulatory peritoneal dialysis

Abstract:Objective The aim of this study was to investigate iron status and iron deficiency in incident continuous ambulatory peritoneal dialysis (CAPD) patients and identify influencing factors. Methods Patients with end-stage renal disease were enrolled. Clinical data of iron metabolism and biochemical and dialysis parameters during the first peritoneal dialysis evaluation were collected. Serum ferritin (SF) and transferrin saturation (TSAT) levels were evaluated, and independent influencing factors were identified by correlation and regression analyses. Results Of 1,128 adult CAPD patients, 41.2% had iron deficiency (ID), 15.7% had absolute iron deficiency, and 8.2% had functional iron deficiency. The average SF level was (276.8 ± 277.9) μg/L, and iron saturation was (29.8 ± 12.7)%. Additionally, 50.2 and 69.3% of patients reached targets in SF level and iron saturation recommended by the Chinese Society of Nephrology. SF level and TSAT were not correlated with estimated glomerular filtration rate, whereas negatively correlated with platelet count and inflammatory factors. Low platelet count, presence of diabetes mellitus and high interleukin 6 levels were independent factors of lower TSAT. Conclusions ID is common in patients with CAPD. Women and those with thrombocytopenia, diabetes, and inflammation are at higher risk for iron storage or reduced iron utilization. In the initial CAPD stage, a reasonable iron supplement strategy may be established for CAPD patients with high-risk factors.

Risk factors and outcomes of high peritonitis rate in continuous ambulatory peritoneal dialysis patients: A retrospective study.

Abstract:Peritonitis remains a major complication of peritoneal dialysis (PD). A high peritonitis rate (HPR) affects continuous ambulatory peritoneal dialysis (CAPD) patients' technique survival and mortality. Predictors and outcomes of HPR, rather than the first peritonitis episode, were rarely studied in the Chinese population. In this study, we examined the risk factors associated with HPR and its effects on clinical outcomes in CAPD patients. This is a single center, retrospective, observational cohort study. A total of 294 patients who developing at least 1 episode of peritonitis were followed up from March 1st, 2002, to July 31, 2014, in our PD center. Multivariate logistic regression was used to determine the factors associated with HPR, and the Cox proportional hazard model was conducted to assess the effects of HPR on clinical outcomes. During the study period of 2917.5 patient-years, 489 episodes of peritonitis were recorded, and the total peritonitis rate was 0.168 episodes per patient-year. The multivariate analysis showed that factors associated with HPR include a quick occurrence of peritonitis after CAPD initiation (shorter than 12 months), and a low serum albumin level at the start of CAPD. In the Cox proportional hazard model, HPR was a significant predictor of technique failure. There were no differences between HPR and low peritonitis rate (LPR) group for all-cause mortality. However, when the peritonitis rate was considered as a continuous variable, a positive correlation was observed between the peritonitis rate and mortality. We found the quick peritonitis occurrence after CAPD and the low serum albumin level before CAPD were strongly associated with an HPR. Also, our results verified that HPR was positively correlated with technique failure. More importantly, the increase in the peritonitis rate suggested a higher risk of all-cause mortality. These results may help to identify and target patients who are at higher risk of HPR at the start of CAPD and to take interventions to reduce peritonitis incidence and improve clinical outcomes.

Abstract:Oxidative stress plays a key role in the pathophysiology of chronic kidney disease (CKD). Most studies have investigated peripheral redox state focus on plasma, but not in different immune cells. Our study analyzed several redox state markers in plasma and isolated peripheral polymorphonuclear (PMNs) and mononuclear (MNs) leukocytes from advanced-CKD patients, also evaluating differences of hemodialysis (HD) and peritoneal dialysis (PD) procedures. Antioxidant (superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx), reduced glutathione (GSH)) and oxidant parameters (xanthine oxidase (XO), oxidized glutathione (GSSG), malondialdehyde (MDA)) were assessed in plasma, PMNs and MNs from non-dialysis-dependent-CKD (NDD-CKD), HD and PD patients and healthy controls. Increased oxidative stress and damage were observed in plasma, PMNs and MNs from NDD-CKD, HD and PD patients (increased XO, GSSG and MDA; decreased SOD, CAT, GPX and GSH; altered GSSG/GSH balance). Several oxidative alterations were more exacerbated in PMNs, whereas others were only observed in MNs. Dialysis procedures had a positive effect on preserving the GSSG/GSH balance in PMNs. Interestingly, PD patients showed greater oxidative stress than HD patients, especially in MNs. The assessment of redox state parameters in PMNs and MNs could have potential use as biomarkers of the CKD progression.

Oxidation State in Peritoneal Dialysis in Patients with Type 2 Diabetes Mellitus

Abstract:End-stage renal disease (ESRD) progression is closely related to oxidative stress (OS). The study objective was to determine the oxidant and antioxidant status in peritoneal dialysis (PD) patients with type 2 diabetes mellitus (DM). An analytical cross-sectional study from the PD program was carried out with 62 patients, 22 with and 40 without DM. Lipoperoxides (LPO) levels in patients with DM, 3.74 ± 1.09 mM/L, and without DM, 3.87 ± 0.84 mM/L were found to increase compared to healthy controls (HC) 3.05 ± 0.58 mM/L (p = 0.006). The levels of the oxidative DNA damage marker (8-OH-dG) were found to be significantly increased in patients with DM, 1.71 ng/mL (0.19-71.92) and without DM, 1.05 ng/mL (0.16-68.80) front to 0.15 ng/mL (0.15-0.1624) of HC (p = 0.001). The antioxidant enzyme superoxide dismutase (SOD) activity was found to be significantly increased in patients with DM, 0.37 ± 0.15 U/mL, and without DM, 0.37 ± 0.17 compared to HC, 0.23 ± 0.05 U/mL (p = 0.038). The activity of the enzyme glutathione peroxidase (GPx) showed a significant increase (p < 0.001) in patients with DM, 3.56 ± 2.18 nmol/min/mL, and without DM, 3.28 ± 1.46 nmol/min/mL, contrary to the activity obtained in HC, 1.55 ± 0.34 nmol/min/mL. In conclusion, we found an imbalance of oxidative status in patients undergoing PD with and without DM through the significant increase in LPO oxidants and the marker of oxidative damage in DNA. The activity of the antioxidant enzymes SOD and GPx were significantly increased in patients with and without DM undergoing PD, possibly in an attempt to compensate for the deregulation of oxidants. Antioxidant enzymes could be promising therapeutic strategies as a complement to the management of chronic kidney diseases.
The Peritoneal Surface Proteome in a Model of Chronic Peritoneal Dialysis Reveals Mechanisms of Membrane Damage and Preservation

Abstract:Peritoneal dialysis (PD) fluids are cytotoxic to the peritoneum. Recent studies have shown that alanyl-glutamine (AlaGln) modulates the cellular stress response, improves mesothelial cell survival, reduces submesothelial thickening in experimental models of PD, and in clinical studies improves PD effluent cell stress and immune responses. However, the mechanisms of AlaGln-mediated membrane protection are not yet fully understood. Here, we explore those mechanisms through application of a novel proteomics approach in a clinically relevant in vivo model in rats. Experimental PD was performed for 5 weeks using conventional single-chamber bag (SCB) or neutral dual-chamber bag (DCB), PD fluid (PDF), with or without AlaGln supplementation, via a surgically implanted catheter. Rats subjected to a single dwell without catheter implantation served as controls. The peritoneal surface proteome was directly harvested by detergent extraction and subjected to proteomic analysis by two-dimensional difference gel electrophoresis (2D-DiGE) with protein identification by mass spectrometry. An integrated bioinformatic approach was applied to identify proteins significantly affected by the treatments despite biological variation and interfering high abundance proteins. From 505 of 744 common spots on 59 gels, 222 unique proteins were identified. Using UniProt database information, proteins were assigned either as high abundance plasma proteins, or as cellular proteins. Statistical analysis employed an adapted workflow from RNA-sequencing, the trimmed mean of M-values (TMM) for normalization, and a mixed model for computational identification of significantly differentially abundant proteins. The most prominently enriched pathways after 5 weeks chronic treatment with SCB or DCB, PDFs belonged to clusters reflecting tissue damage and cell differentiation by cytoskeletal reorganization, immune responses, altered metabolism, and oxidative stress and redox homeostasis. Although the AlaGln effect was not as prominent, associated enriched pathways showed mostly regression to control or patterns opposite that of the PDF effect. Our study describes the novel peritoneal surface proteome through combined proteomic and bioinformatic analyses, and assesses changes elicited by chronic experimental PD. The biological processes so identified promise to link molecular mechanisms of membrane damage and protection in the in vivo rat model to pathomechanisms and cytoprotective effects observed in vitro and in clinical PD.
Analysis of influencing factors of advanced glycation end products peptides in serum in continuous ambulatory peritoneal dialysis non-diabetic patients

Abstract:Objective To investgate the influencing factors of advanced glycation end products peptides(AGEs-P) in serum in continuous ambulatory peritoneal dialysis(CAPD) non-diabetic patients. Methods AGEs-P in serum of 25 CAPD non-diabetic patients was detect by flow injection assay(FIA),and the influencing factors,such as duration of CAPD,peritoneal glucose load,KT/V,creatinine clearance rate(CCr) of residual renal and peritoneum,oxidative stress markers such as glutathione (GSH),total-superoxide dismutase(T-SOD),malondialdehyde (MDA),high sensitivity C-reactive protein (hsCRP) were analyzed with multiple stepwise regression analysis and Pearson correlation analysis. Results The concentration of AGEs-P in serum was correlated negatively with residual renal function,GSH,T-SOD concentration (r=-0.785,-0.817,-0.826,P0.01) and positively with the duration of CAPD,peritoneal glucose load,hs-CRP,MDA concentration in long-term CAPD non-diabetic patients (r=0.883,0.901,0.715,0.828,P0.01). Multiple stepwise regression analysis revealed that peritoneal glucose load and residual renal function were main influencing factors of the concentration of AGEs-P in serum. Conclusion After CAPD,AGEs-P in serum in CAPD non-diabetic patients are correlated with duration of CAPD,peritoneal glucose load,residual renal function,inflammation and oxidative stress,but are influenced mainly by peritoneal glucose load and residual renal function.

Abstract:The key role of oxidative stress (OxSt) and inflammation for the induction of cardiovascular disease, the leading cause of excess morbidity/mortality in chronic kidney disease and dialysis patients, is known and both the activation of NADPH oxidase and RhoA/Rho kinase (ROCK) pathway are pivotal for their effects. While specific hemodialysis procedures, such as hemodiafiltration with on-line reinfusion of ultrafiltrate and/or the use of vitamin E coated dialyzers, are beneficial on OxSt and inflammation, studies in peritoneal dialysis (PD) are instead scarce and results seem not favorable.In nine patients under PD OxSt in terms of mononuclear cell protein level of p22phox (Western blot), subunit of NADPH oxidase, essential for the generation of OxSt, and MYPT-1 phosphorylation state (Western blot), a marker of ROCK activity, have been measured at the beginning and after 3 and 6 months of PD. Blood levels of Interleukin 6 (IL-6), Ferritin and Albumin have been considered for evaluating the inflammatory state.p22phox protein expression, MYPT-1-phosphorylation, and ferritin level were increased both at baseline vs healthy subjects (p=0.02, p<0.0001, p=0.004 respectively) and vs baseline after 3 and 6 months of peritoneal dialysis (p=0.007, p<0.001, p=0.004 respectively). Albumin was lower after 6 months of PD (p=0.0014). IL-6 was increased at baseline vs reference values and remained unchanged at 3 and 6 months.OxSt and inflammation increase during PD confirming via molecular biology approach a report at biochemical level. To improve OxSt state in PD, a multitarget approach is necessary. It might include the use of more physiologic pH, low glucose degradation products, low lactate and iso-osmolar PD solutions, patients' strict glycemic control, optimal volume management and antioxidants administration, such as N-acetylcysteine.
