Abstract:Sleep restriction causes impaired cognitive performance that can result in adverse consequences in many occupational settings. Individuals may rely on self-perceived alertness to decide if they are able to adequately perform a task. It is therefore important to determine the relationship between an individual's self-assessed alertness and their objective performance, and how this relationship depends on circadian phase, hours since awakening, and cumulative lost hours of sleep. Healthy young adults (aged 18-34) completed an inpatient schedule that included forced desynchrony of sleep/wake and circadian rhythms with twelve 42.85-hour "days" and either a 1:2 (n = 8) or 1:3.3 (n = 9) ratio of sleep-opportunity:enforced-wakefulness. We investigated whether subjective alertness (visual analog scale), circadian phase (melatonin), hours since awakening, and cumulative sleep loss could predict objective performance on the Psychomotor Vigilance Task (PVT), an Addition/Calculation Test (ADD) and the Digit Symbol Substitution Test (DSST). Mathematical models that allowed nonlinear interactions between explanatory variables were evaluated using the Akaike Information Criterion (AIC). Subjective alertness was the single best predictor of PVT, ADD, and DSST performance. Subjective alertness alone, however, was not an accurate predictor of PVT performance. The best AIC scores for PVT and DSST were achieved when all explanatory variables were included in the model. The best AIC score for ADD was achieved with circadian phase and subjective alertness variables. We conclude that subjective alertness alone is a weak predictor of objective vigilant or cognitive performance. Predictions can, however, be improved by knowing an individual's circadian phase, current wake duration, and cumulative sleep loss.

Can a Shorter Psychomotor Vigilance Task Be Usedas a Reasonable Substitute for the Ten‐Minute Psychomotor Vigilance Task?

Abstract:The 10 min psychomotor vigilance task (PVT) is commonly used in laboratory studies to assess the impact of sleep loss, sustained wakefulness, and/or time of day on neurobehavioral performance. In field settings, though, it may be impractical for participants to perform a test of this length. The aim of this study was to identify a performance measure that is sensitive to the effects of fatigue but less burdensome than a 10 min test. Sixteen participants (11 female, 5 male; mean age = 21.7 years) slept in the sleep laboratory overnight then remained awake for 28 h from 08:00 h. During every second hour, participants completed three PVTs of differing duration (10 min, 5 min, 90 sec). For the 5 min/10 min comparison, ANOVA indicated that response time was significantly affected by test length (F1,14 = 26.9, p < .001) and hours of wakefulness (F13,182 = 46.1, p < .001) but not by their interaction (F13,182 = 1.7, ns). There was a strong correlation between response time on the 5 and 10 min PVTs (r = .88, p < .001). For the 90 sec/10 min comparison, ANOVA indicated that response time was significantly affected by test length (F1,14 = 65.9, p < .001) and hours of wakefulness (F13,182 = 29.7, p < .001) as well as by their interaction (F13,182 = 6.0, p < .001). There was a strong correlation between response time on the 90 sec and 10 min PVTs (r = .77, p < .001). The effects of hours of wakefulness on neurobehavioral performance were similar for the 5 min and 10 min PVTs. In contrast, performance on the 90 sec PVT was less affected by hours of wakefulness than on the 10 min PVT. In addition, performance on the 10 min PVT was more highly correlated with the 5 min PVT than the 90 sec PVT. These data indicate that the 5 min PVT may provide a reasonable substitute for the 10 min PVT in circumstances where a test shorter than 10 min is required.

Abstract:Maintaining human alertness and behavioral capability under conditions of sleep loss and circadian misalignment requires fatigue management technologies due to: (1) dynamic nonlinear modulation of performance capability by the interaction of sleep homeostatic drive and circadian regulation; (2) large differences among people in neurobehavioral vulnerability to sleep loss; (3) error in subjective estimates of fatigue on performance; and (4) to inform people of the need for recovery sleep. Two promising areas of technology have emerged for managing fatigue risk in safety-sensitive occupations. The first involves preventing fatigue by optimizing work schedules using biomathematical models of performance changes associated with sleep homeostatic and circadian dynamics. Increasingly these mathematical models account for individual differences to achieve a more accurate estimate of the timing and magnitude of fatigue effects on individuals. The second area involves technologies for detecting transient fatigue from drowsiness. The Psychomotor Vigilance Test (PVT), which has been extensively validated to be sensitive to deficits in attention from sleep loss and circadian misalignment, is an example in this category. Two shorter-duration versions of the PVT recently have been developed for evaluating whether operators have sufficient behavioral alertness prior to or during work. Another example is online tracking the percent of slow eyelid closures (PERCLOS), which has been shown to reflect momentary fluctuations of vigilance. Technologies for predicting and detecting sleepiness/fatigue have the potential to predict and prevent operator errors and accidents in safety-sensitive occupations, as well as physiological and mental diseases due to inadequate sleep and circadian misalignment.

Using a Single Daytime Performance Test to Identify Most Individuals at High-Risk for Performance Impairment during Extended Wake

Abstract:We explored the predictive value of a neurobehavioral performance assessment under rested baseline conditions (evaluated at 8 hours awake following 8 hours of sleep) on neurobehavioral response to moderate sleep loss (evaluated at 20 hours awake two days later) in 151 healthy young participants (18-30 years). We defined each participant's response-to-sleep-loss phenotype based on the number of attentional failures on a 10-min visual psychomotor vigilance task taken at 20 hours awake (resilient: less than 6 attentional failures, n = 26 participants; non-resilient: 6 or more attentional failures, n = 125 participants). We observed that 97% of rested participants with 2 or more attentional failures (n = 73 of 151) and 100% of rested participants with 3 or more attentional failures (n = 57 of 151) were non-resilient after moderate sleep loss. Our approach can accurately identify a significant proportion of individuals who are at high risk for neurobehavioral performance impairment from staying up late with a single neurobehavioral performance assessment conducted during rested conditions. Additional methods are needed to predict the future performance of individuals who are not identified as high risk during baseline.

Abstract:The initial problem in studying sleep loss is the difficulty of finding any performance, either psychological or physiological, which is consistently impaired. The results are drawn from two studies of sleep loss, one in 1956 and the other in 1957. There are some differences between these two in the sample of subjects and in the design. In both studies, however, the experimental setting and general procedure reflect our aim of providing a non coercive, nonrestrictive atmosphere with a minimum of heavy physical work. With 49 subjects, deprived of sleep for 72-98 hr., performance deteriorated on a variety of tasks, an unusual result in studies of sleep loss. Deficit took the form of lapses (brief periods of no response accompanied by extreme drowsiness and a decline in EEG alpha amplitude). Four features of lapses were noted, (a) They occur in other conditions such as fatigue and hypoxia and appear to characterize impairment in general. (b) They increase in both frequency and duration as sleep loss progresses, (c) They are strongly affected by stimulus monotony, (d) Their specific effect on performance varies with the properties of the task. In subject-paced tasks, for example, speed is the critical measure; in experimenter-paced tasks, errors are critical. To identify the sensitive aspect of performance becomes the crucial problem.

Abstract:Abstract Introduction Sleep deprivation leads to detriments in performance, including alertness and vigilance. Less is known, however, about the cognitive impact of mild sleep restriction, including whether negative outcomes are time-dependent and cumulative. We tested the impact of sleep restriction on sustained attention in a multi-week, crossover experiment in a controlled laboratory setting. Methods Sixty-six young adults (Mage = 20.03; 53.0% female; 51.5% non-white) completed seven nights of in-laboratory testing, spread across 3-weeks. On the first week, participants completed a polysomnography adaptation night and baseline assessments including the psychomotor vigilance task (PVT) in the evening (9:00PM) and in the morning (7:30AM) with buffer time to prevent sleep inertia following initial awakening. During the following week, participants completed either three nights of sleep restriction (5 hours of sleep/night) or three nights of normal sleep (8 hours of sleep/night), with PVT testing each morning and evening. After a four-night washout period, participants returned to complete the other sleep condition (order counterbalanced). Research assistants who administered the PVT were kept blinded to the participant’s sleep condition. Results Relative to normal sleep, three nights of sleep restriction significantly worsened PVT mean reaction times (RTs; M = 361 ms versus M = 390 ms; t(65) = 4.38, p < .001, d = .54) and number of lapses (M= 3.78 versus M = 5.80; t(65) = 5.00, p < .001, d = .62]. There were no main effects of time of day or interactions with sleep condition (ps>.05). However, study day significantly interacted with sleep condition (F(2, 128) = 3.42, p = .036, η2 = .051) such that the negative impact of sleep restriction on lapses worsened across the week (night 1: Mdiff = 1.59, night 2: Mdiff = 0.88; night 3: Mdiff = 3.26). Conclusion Mild sleep restriction quickly produces detriments in sustained attention, with effects being cumulative across repeated days of sleep loss. Abstract:Abstract Introduction The importance of sleep in daily life and advancements in our ability to measure bio-signals have led to an unprecedented growth in the use of commercial wearable sleep-tracker devices. However, sleep parameters estimated from these tracker devices often differ from the gold-standard polysomnography (PSG). It is unclear to what extent we can tolerate these errors within the context of a particular clinical or operational application. Here, we sought to develop a method to quantitatively determine whether a sleep tracker yields acceptable sleep-parameter estimates for assessing alertness impairment. Methods Using literature data, we characterized sleep-measurement errors of 18 unique sleep-tracker devices with respect to PSG. Then, using predictions based on the well-validated Unified Model of Performance, we compared the temporal variation of alertness for simulations with and without added PSG-device sleep-measurement errors, for nominal schedules of 5, 8, or 9 hours of sleep/night, or an irregular sleep schedule each night, for 30 consecutive days. Finally, we deemed a device error acceptable when the predicted differences were smaller than the within-subject variability of 30 ms. Results On average, the 18 sleep-tracker devices overestimated sleep duration by 19 (standard deviation = 44) minutes. Using these errors for 30 consecutive days, we found that, regardless of sleep schedule, in nearly 80% of the time the resulting predicted alertness differences were smaller than 30 ms, suggesting that most devices yield acceptable sleep-measurement errors. We also established the capability to estimate the extent to which a specific sleep-tracker device meets this acceptance criterion. Conclusion We provide a method to quantitatively determine whether a sleep-tracker device produces sleep measurements that are operationally acceptable for fatigue management. Low Social Support Is Associated With Shorter Leukocyte Telomere Length in Late Life: Multi-Ethnic Study of Atherosclerosis

Abstract:Objective The primary goal was to test the hypothesis that limited social support (SS) is related to shorter leukocyte telomere length (LTL), particularly in an older adult population. Methods Cross-sectional analyses were performed on 948 participants aged 45 to 84 years at Examination 1 of the Multi-Ethnic Study of Atherosclerosis (18.4% white, 53.1% Hispanics, and 28.5% African American). LTL was determined by using quantitative polymerase chain reaction, and SS was measured with the Enhancing Recovery in Coronary Heart Disease SS inventory. Results Across the entire sample, SS was not associated with LTL (p = .87) after adjusting for demographic (age, sex, race/ethnicity, socioeconomic status), age × sex, age × race, health (body mass index, diabetes, pulse pressure), and life-style factors (smoking, physical activity, diet); however, the interaction term age (dichotomized) × SS was significant (p = .001). Stratification by age group revealed a positive association between SS (score range, 5–25) and LTL in the older (65–84 years; B[SE] = .005[.002]; p = .007) but not younger participants (45–64 years; p = .12) after adjusting for covariates. Conclusions These results from a racially/ethnically diverse community sample of men and women provide initial evidence that low SS is associated with shorter LTL in adults aged 65 years and older and is consistent with the hypothesis that social environment may contribute to rates of cellular aging, particularly in late life.
Effects of Sleep Deprivation on Performance during a Change Signal Task with Adaptive Dynamics

Abstract:Augmented cognition, which refers to real-time modifications to a human-system interface to improve performance and includes dynamic task environments with automated adaptations, can serve to protect against performance impairment under challenging work conditions. However, the effectiveness of augmented cognition as a countermeasure for performance impairment due to sleep loss is unknown. Here, in a controlled laboratory study, an adaptive version of a Change Signal task was administered repeatedly to healthy adults randomized to 62 h of total sleep deprivation (TSD) or a rested control condition. In the computerized task, a left- or right-facing arrow was presented to start each trial. In a subset of trials, a second arrow facing the opposite direction was presented after a delay. Subjects were to respond within 1000 ms of the trial start by pressing the arrow key corresponding to the single arrow (Go trials) or to the second arrow when present (Change trials). The Change Signal Delay (CSD)-i.e., the delay between the appearance of the first and second arrows-was shortened following incorrect responses and lengthened following correct responses so that subsequent Change trials became easier or harder, respectively. The task featured two distinct CSD dynamics, which produced relatively stable low and high error rates when subjects were rested (Low and High Error Likelihood trials, respectively). During TSD, the High Error Likelihood trials produced the same, relatively high error rate, but the Low Error Likelihood trials produced a higher error rate than in the rested condition. Thus, sleep loss altered the effectiveness of the adaptive dynamics in the Change Signal task. A principal component analysis revealed that while subjects varied in their performance of the task along a single dominant dimension when rested, a second inter-individual differences dimension emerged during TSD. These findings suggest a need for further investigation of the interaction between augmented cognition approaches and sleep deprivation in order to determine whether and how augmented cognition can be relied upon as a countermeasure to performance impairment in operational settings with sleep loss.
Sleepy Surgeons: A Multi-Method Assessment of Sleep Deprivation and Performance in Surgery

Abstract:Background: Minimum rest is mandated in high stake industries such as aviation. The current system of healthcare provision permits on-call surgeons to work in sleep deprived states when performing procedures. Fatigue has been demonstrated to negatively affect performance. This study aimed to explore measurements of sleep deprivation and their impact on simulated performance. Methods: This was a single site study conducted between September 2019 and February 2020. Surgical trainee and consultants were conveniently sampled from a single site. All testing was done between 7 AM and 9 AM. Participants completed electroencephalogram testing using a modified Multiple Sleep Latency Test testing for objective sleep measurement, the Pittsburgh Sleep Quality Index, Chalder Fatigue Scale and Epworth Sleepiness Scale for subjective sleep measurement. The Psychomotor Vigilance Task and the SIMENDO simulated tasks were used for standardized performance assessment. Results: Surgeons entered sleep in 6 min, on average pre-call. This significantly decreased to an average of 164 s post-call (P = 0.016). Pittsburgh Sleep Quality Index scoring was 5, indicating poor baseline sleep quality. There was higher self-reported fatigue and sleepiness in post-call states. Performance decrements were noted in cognitive performance reaction time and aspects of technical instrument proficiency. Conclusions: Surgeons are objectively sleep deprived pre-call according to internationally recognized guidelines. This sleep deprivation increases significantly in post-call states. Tasks with higher cognitive demands showed greater levels of diminished performance compared to those with lower cognitive demands. Current models of provision of surgical on-call are not conducive to optimizing sleep in surgeons. Prioritization of workload in post-call states, focusing on preserving individuals cognitive resources and utilizing lower cognitively demanding aspects of work is likely to have positive impacts on performance outcomes.
Computational Modeling of the Effects of Sleep Deprivation on the Vigilance Decrement

Abstract:Objective: We developed a computational model of the effects of sleep deprivation on the vigilance decrement by employing the methods of system dynamics modeling. Background: Situations that require sustained attention for a prolonged duration can cause a decline in cognitive performance, the so-called vigilance decrement. One factor that should influence the vigilance decrement is fatigue in the form of sleep deprivation. Method: We employed the methods of system dynamics modeling (numerical-integration techniques for modeling complex feedback systems) to create a computational model of the vigilance decrement. We then simulated the computational effects of sleep deprivation on the behavior of that model, using empirical data obtained from the literature for calibrating such effects. Results: Sleep deprivation of 2 hr over a 14-day period should produce an additional decline of 9% in detection performance over that found with the typical vigilance decrement, whereas 4 hr of sleep deprivation over 14 days should produce an additional decline of 14% in detection performance. Conclusion: With respect to dual-process theory, it is through its deleterious effects on analytical cognition that sleep deprivation should impact the vigilance decrement. Application: Such computational modeling may be advantageous for human-machine teaming by theoretically allowing a future autonomous software agent to anticipate the decline of human performance and compensate accordingly.

Abstract:Research on sleep loss and vigilance both focus on declines in cognitive performance, but theoretical accounts have developed largely in parallel in these two areas. In addition, computational instantiations of theoretical accounts are rare. The current work uses computational modeling to explore whether the same mechanisms can account for the effects of both sleep loss and time on task on performance. A classic task used in the sleep deprivation literature, the Psychomotor Vigilance Test (PVT), was extended from the typical 10-min duration to 35 min, to make the task similar in duration to traditional vigilance tasks. A computational cognitive model demonstrated that the effects of time on task in the PVT were equivalent to those observed with sleep loss. Subsequently, the same mechanisms were applied to a more traditional vigilance task-the Mackworth Clock Task-providing a good fit to existing data. This supports the hypothesis that these different types of fatigue may produce functionally equivalent declines in performance.

Abstract:IntroductionSleep disruption and teamwork are inherent features of 24/7 operational environments, yet little is known about how sleep and team/social processes interact to affect crew readiness and endurance.MethodsWe analyzed data of 3,434 active duty US sailors (80% male) from the Afloat Safety Climate Assessment Survey. Using structural equation modeling, we specified latent factors of Sleep Health (typical hours of sleep per day and sleep disturbances in shipboard environment); crew team and social factors of Team Transition Processes, Team Action Processes, Team Interpersonal Processes, Unit Cohesion, Psychological Safety, and Social Support; and operational outcome risks of Physical Health (no. days in previous 30 with physical illness or injury), Mental Health (no. days with stress, depression, or emotional problems), Performance (frequency of fatigue-induced functional impairments on duty), and Safety (individual and crew noncompliance, and rate of observed near misses).ResultsHigher Sleep Health reduced impairments in Physical Health, Mental Health, Performance, and Safety (standardized βs = −0.096 to −0.542, ps 0.980, RMSEA = 0.033). Higher Sleep Health improved Team Transition, Action, and Interpersonal Processes, Unit Cohesion, Psychological Safety, and Social Support (βs = 0.178 to 0.380, ps 0.982, RMSEA = 0.029). Social Support reduced risks to Physical Health, Mental Health, and Performance; Team Interpersonal Processes reduced Metal Health risk; Psychological Safety reduced Performance and Safety risks; Unit Cohesion reduced Safety risk (βs = −0.053 to −0.709, ps 0.979, RMSEA = 0.027). Mediation models indicated good Sleep Health enhances Social Support's beneficial impact on Physical Health, Mental Health, and Performance; Psychological Safety's impact on Performance and Safety; Team Interpersonal Processes' impact on Physical Health; and Unit Cohesion's impact on Safety (indirect effect βs = −0.032 to −0.127, ps 0.967, RMSEAs < 0.051).ConclusionSleep health improves team/social functioning, which serves an additive protective function and enhancement to crew operational health, performance, and safety. Abstract:The Karolinska Sleepiness Scale and Samn-Perelli fatigue ratings, and psychomotor vigilance task performance are proposed as measures for monitoring commercial pilot fatigue. In laboratory studies, they are sensitive to sleep/wake history and circadian phase. The present analyses examined whether they reliably reflect sleep/wake history and circadian phase during transmeridian flight operations. Data were combined from four studies (237 pilots, 730 out-and-back flights between 13 city pairs, 1-3-day layovers). Sleep was monitored (wrist actigraphy, logbooks) before, during and after trips. On duty days, sleepiness, fatigue and mean response speed were measured pre-flight and at the top of the descent. Mixed-model analysis of variance examined associations between these measures and sleep/wake history, after controlling for operational factors. Circadian phase was approximated by local (domicile) time in the city where each trip began and ended. More sleep in the 24 h prior to duty was associated with lower pre-flight sleepiness and fatigue and faster response speed. Sleepiness and fatigue were greater before flights departing during the domicile night and early morning. At the top of the descent, pilots felt less sleepy and fatigued after more in-flight sleep and less time awake. Flights arriving in the early-mid-morning (domicile time) had greater sleepiness and fatigue and slower response speeds than flights arriving later. Subjective ratings showed expected associations with sleep/wake history and circadian phase. The response speed showed expected circadian variation but was not associated with sleep/wake history at the top of the descent. This may reflect moderate levels of fatigue at this time and/or atypically fast responses among pilots.

Abstract:Study objectives: This study evaluated the utility and ecological validity of the 3-minute psychomotor vigilance test (PVT) completed by safety-critical personnel in an air medical transport operation as part of a fatigue risk management program. Methods: Crewmembers in an air medical transport operation self-administered an alertness assessment incorporating a 3-minute PVT at different time points during their duty schedule. The prevalence of alertness deficits was evaluated based on a failure threshold of 12 errors considering both lapses and false starts. To evaluate the ecological validity of the PVT, the relative frequency of failed assessments was evaluated relative to crewmember position, timing of the assessment within the duty schedule, time of day, and sleep quantity in the last 24 h. Results: 2.1% of assessments were associated with a failing PVT score. Crewmember position, timing of assessment within the duty shift, time of day, and sleep quantity in the last 24 h were found to affect the relative frequency of failed assessments. Obtaining less than 7-9 h of sleep was associated with systematic increases in the failure rate (F[1, 54 612] = 168.1, p < .001). Obtaining less than 4 h of sleep was associated with a frequency of a failed assessment 2.99 times higher than the frequency of a failed assessment when obtaining 7-9 h of sleep. Conclusions: Results provide evidence for the utility and ecological validity of the PVT as well as the suitability of the PVT failure threshold to support fatigue risk management in safety-critical operations.

Abstract:AIM To evaluate the effect of the application of plasma rich in growth factors (PRGF)-Endoret to the remaining intact bundle in partial anterior cruciate ligament (ACL) tears. METHODS A retrospective review of the rate of return to play in football players treated with the application of PRGF-Endoret in the remaining intact bundle in partial ACL injuries that underwent surgery for knee instability. Patients with knee instability requiring revision surgery for remnant ACL were selected. PRGF was applied in the wider part of posterolateral bundle and the time it took patients to return to their full sporting activities at the same level before the injury was evaluated. RESULTS A total of 19 patients were reviewed. Three had a Tegner activity level of 10 and the remaining 16 level 9. The time between the injury and the time of surgery was 5.78 wk (SD 1.57). In total, 81.75% (16/19) returned to the same pre-injury level of sport activity (Tegner 9-10). 17 males and 2 females were treated. The rate of associated injury was 68.42% meniscal lesions and 26.31% cartilage lesions. The KT-1000 values were normalized in all operated cases. One patient was not able to return to sport due to the extent of their cartilage lesions. The 15 patients with Tegner activity level 9 returned to play at an average of 16.20 wk (SD 1.44) while the 3 patients with Tegner activity level 10 did so in 12.33 wk (SD 1.11). CONCLUSION With one remaining intact bundle the application of PRGF-Endoret in instability cases due to partial ACL tear showed high return to sport rates at pre- injury level in professional football players.
