Abstract:Ankle exercises are useful for preventing deep vein thrombosis, as they increase venous blood flow velocity. The cause for the increased venous blood flow velocity during ankle exercises may be the skeletal-muscle pump, but the mechanism is not clearly understood. The purpose of this study was to investigate the effects of the dorsiflexion angle and gastrocnemius muscle contraction on venous blood flow velocity during ankle exercises and to investigate the mechanism of the increase in venous blood flow velocity. The blood flow velocity in the popliteal vein, ankle joint angle, and surface electromyographic activity of the gastrocnemius muscle were measured at rest and during ankle exercises in the prone position in young healthy volunteers. The significant increase in venous blood flow velocity was observed during dorsiflexion phase, max dorsiflexion and during planter flexion phase. The peak venous blood flow velocity was different in each subject and classified into four types. The correlations of venous blood velocity to ankle joint angle and with the surface electromyographic activity of the gastrocnemius muscle were not statistically significant. These findings suggest that venous blood flow velocity increases not only during plantar flexion and dorsiflexion.
Effects of ankle pump exercise frequency on venous hemodynamics of the lower limb

Abstract:Purpose: The purpose of this study was (1) to compare the effects of ankle pump exercise(APE) frequency on hemodynamics of the common femoral vein(CFV) and (2) to analyse the relationship between APE duration and lower limb fatigue. Methods: Twenty-seven males and thirty-three females performed APE. Among them, there were thirty participants with non-lower limb fracture (N-LLF) and thirty participants with lower limb fracture (LLF).The colour doppler ultrasound was used to record the time-averaged mean velocity (TAMV) of common femoral venous flow when the participants at rest and move at different frequencies of 6 times/min, 10 times/min, 30 times/min, 60 times/min. The ratings of perceived exertion (RPE) was used to assess the lower limb fatigue of the participants when performing APE at the frequency of 60 times/min after 1 min, 2 mins, 3 mins, 4 mins and 5 mins. Results: With the increase of frequency, TAMV increased significantly in both the participants with N-LLF and LLF (p < 0.01). The TAMV were 19.82±3.86, 33.78±8.76, 37.06±8.67, 43.82±10.40, 52.18±10.53, respectively in the participants with N-LLF and 16.98±3.01, 22.20±4.96, 24.01±5.78, 29.20±7.05, 35.75±9.28, respectively in the injured limb of patients with LLF when at rest and moving at the frequency of 6 times/min, 10 times/min, 30 times/min, 60 times/min. There was a positive correlation (p < 0.01) between lower limb fatigue and exercise duration.When the RPE was 16 points (the corresponding fatigue degree is "tired"), the exercise duration of the participants with N-LLF was 3 mins and that of the injured limb of patients with LLF was 2 mins. Conclusions: Both fast and slow-frequency APE can promote venous blood return in the lower limb. Despite of the equivalent APE duration, fast-frequency APE can promote venous blood return more effectively. When the frequency of APE was 60 times/min, participants with N-LLF can exercise for 3 mins, and the injured limb of patients with LLF can exercise for 2 mins.
Effects of different exercise methods of calf muscles on the hemodynamics of lower extremity vein

Abstract:Objective To compare the effects of active and passive calf muscle contraction on the hemodynamics of the lower extremity vein. Methods 30 females were selected by convenient sampling. The hemodynamic indexes of the common femoral vein were measured by Duplex ultrasound during the active ankle pump exercise, active circular exercise, passive ankle pump exercise, passive circular exercise, and massage the calf muscles. Results There was no significant difference in the velocity of common femoral vein when the subjects do active ankle pump exercise, active circular exercise, and massage the calf muscles ( p > .05), but the velocity of common femoral vein was faster than that of passive ankle pump exercise and passive circular exercise ( p < .01). Conclusion The effects of active ankle exercise and massage on promoting venous blood return of lower extremity are better than that of passive ankle exercise.

Abstract:Introduction Ankle pump exercise (APE) is an effective method to prevent deep vein thrombosis (DVT) in the lower extremities. However, traditional APE at a frequency of 3 times/min applied in the clinic lacked high compliance, and individuals were found to prefer an APE of 30 times/min. This study aimed to compare the effects of APEs with the above two different frequencies on the venous hemodynamics and fatigue of lower extremities. Methods The study applied a randomized cross-over design, and 307 healthy adults were randomly assigned into two groups, whose left feet performed 5 minutes of traditional APE (3 times/min, after 10 minutes of rest) and 5 minutes of selected APE (30 times/min, after 10 minutes of rest) in different orders. The diameters and blood flow velocities of the external iliac vein, femoral vein, and popliteal vein of the left extremities were recorded with the color doppler ultrasound when participants were at rest and after 5 minutes of traditional or selected APE. The fatigue of lower extremities was assessed with the Rating of Perceived Exertion (RPE) after the participants performed 5 minutes of different APEs. Results After both traditional APE and selected APE, the diameters and blood flow (peak systolic) velocities of the external iliac vein, femoral vein, and popliteal vein increased significantly in the participants (P .05). However, participants reported that traditional APE caused higher fatigue, and 252 (82.1%) participants preferred APE at 30 times/min. Conclusion Both traditional and selected APE could increase venous blood flow in the lower extremity. Despite the equivalent effects, the selected APE of 30 times/min may cause less fatigue than the traditional one of 3 times/min.

Abstract:Background: Ankle pumping exercises utilize a calf muscle pump function to pump blood to the heart by muscle contraction. Ankle pumping exercises are often used for the relief of edema and the prevention of deep vein thrombosis. So, the current study aimed to evaluate the effect of ankle pump exercise on fatigue sensation, comfort, and lower limb hemodynamics among deep vein thrombosis patients. Design: This study was carried out using a quasi-experimental research design. Setting: This study was applied in the Orthopedic Department at Ain Shams University Hospital. Subjects: It consisted of a convenient sampling technique enrolled to select a sample of 100 patients with deep vein thrombosis in the previously selected department who agreed to participate in the study and were randomly assigned into two equal groups, with 50 patients in each group (the intervention group who received ankle pump exercise and the control group who received routine hospital care only). Tools: three tools were used (I): A patient interviewing sheet, (II): Rating of Perceived Exertion (RPE), and (III): Comfort evaluation scale.

Abstract:Background and objective: The intravenous anaesthetic propofol has been reported to exert neuroprotective actions by several mechanisms. This study has been designed to investigate the effects of propofol on intracellular calcium increase in cultured cerebrocortical neurons after exposure to pathological concentrations of N‐methyl‐D‐aspartate (NMDA) mediated by potential direct interactions of propofol with NMDA receptors. Methods: The effects of propofol (0.1‐100 μmol) on intracellular calcium increase induced by 300 μmol NMDA (180 s) were measured in cultured cerebrocortical neurons using the calcium‐sensitive fluorochrome calcium green‐5N‐acetoxymethylester with confocal laser scanning microscopy. Results: The intraneuronal calcium increase after exposure to 300 μmol NMDA depended on extracellular calcium concentration. Propofol reduced the increase of NMDA receptor‐induced intraneuronal calcium concentration dependently with a threshold concentration for a significant effect of 10 μmol. The overall effect was small, since even high concentrations of propofol (100 μmol) diminished intraneuronal calcium rise by only 50%. Conclusions: The threshold concentration for significant effects of propofol on the NMDA‐induced increase of intraneuronal calcium turned out to be in the upper limit of propofol concentrations that are considered to be clinically relevant. However, in the presence of high propofol concentrations, inhibition of NMDA receptor‐mediated calcium increase might contribute to neuroprotective effects observed with propofol.
Evaluation of venous return in lower limb by passive ankle exercise performed by PHARAD

Abstract:This paper presents evaluation of venous return, i.e., blood flow volume of vein (BF), in the lower limb after passive exercise performed by our developed "parallel link type human ankle rehabilitation assistive device (PHARAD)". The PHARAD can perform complex passive exercises (plantar flexion/dorsiflexion, inversion/eversion, adduction/abduction, and combination of these motions) by reproducing input motions of a foot plate that is attached to a sole of foot. The passive exercise can be performed for not only rehabilitation but also prevention of deep vein thrombosis (DVT). In this study, we measured the concentration of Total hemoglobin (Total-Hb) using multi-channel near infra-red spectroscopy (NIRS)-based tissue oximeters and calculated a gradient of Total-Hb during a venous occlusion. We defined the gradient as BF and evaluated BF after 3 min passive exercise performed by the PHARAD comparing to BF of resting. Seven healthy young adult people were recruited for the experiment and we assessed passive exercise, active exercise, and walking. Experimental results show that BF after the passive exercises significantly increases compare to BF of resting and this indicates that passive exercises performed by the PHARAD increases BF and has a potential to prevent DVT.
The changes of the calf-vein deformation and femoral vein peak velocity during ankle pump exercise with or without graduated compression stockings

Abstract:Objectives: To analyze the changes of lower limb hemodynamics parameters before and after wearing graduated compression stockings (GCS) during ankle pump exercise in patients preparing for arthroplastic surgery. Method: The leg veins of 16 patients awaiting arthroplasty were analyzed using a Sonosite M-Turbo ultrasound system during ankle pump exercise with or without GCS. The age of them was 70 ± 7 years (mean ± SD) (range 56-82 years) and body mass index was 25.8 ± 3.0 kg/m2 (range 18.0-30.5 kg/m2). Measured data including the cross-sectional area (CSA), anteroposterior (AP) diameter and lateromedial (LM) diameter of the soleus vein (SV), posterior tibial vein (PTV) and great saphenous vein (GSV). Additionally, the peak velocities of femoral vein (FV) were also measured. Results: GCS could significantly decrease the cross-sectional area of SV, PTV and GSV in supine position at rest and maximum ankle plantar flexion. But the compression effect of GCS to SV and GSV was not observed during maximum ankle dorsiflexion. It was found that GCS application reduced the peak flow velocity of the femoral vein from 61.85 cm/s (95% CI = 50.94-72.75 cm/s) to 38.01 cm/s (95% CI = 28.42-47.59 cm/s) (P < 0.001) during ankle plantar flexion and decreased the femoral vein in these patients from 80.65 cm/s (95% CI = 70.37-90.92 cm/s) to 51.15 cm/s (95% CI = 42.58-59.73 cm/s) (P < 0.001) during ankle dorsiflexion. But this effect was not significant in supine position at rest. Conclusions: GCS could significantly reduce the peak flow velocity of the femoral vein during ankle pump exercise in the patients preparing for arthroplastic surgery.

Abstract:Purpose: Ankle pump exercises (APE) have been widely used in clinical practice. However, best practices for APE have not been established. Recognize the most effective frequency of APE for improving lower extremity hemodynamics and establish recommendations in clinical practice. Methods: Therefore, a systematic review and network meta-analysis (NMA) was performed according to PRISMA-NMA. Six English databases (Pubmed, Medline, CINAHL, Embase, the Cochrane library and ProQuest) and four Chinese databases (CNKI, Wanfang, VIP and Sinomed) were searched. Randomized controlled trials (RCTs) and quasi-experimental studies investigating the effects of different frequencies of APE on lower limb hemodynamics published before July 2022 were included. The reference list was also searched. Seven studies (one RCTs and six quasi-experimental studies) were included in the systematic review and five studies (one RCTs and four quasi-experimental studies) were included in the NMA. The risk of bias was assessed using the Cochrane and Joanna Briggs Institute tools. The NMA was performed using the R software (version 4.2.1) and OpenBUGS (version 3.2.3). Results: The results of the NMA showed that a frequency of every 3-4 s the most effective in improving lower extremity hemodynamics (P =.85), followed by every 1-2 s (P = .81), every 5-6 s (P=.32) and less than every 10 s (P =.02). Subgroup analysis failed to find a difference between healthy participants and those with unilateral total hip arthroplasty or fracture (MD = -0.23, 95% CI-5.92 to 4.61). Conclusions: Consequencely, for adult patients, with or without lower extremity disease, a frequency of every 3-4 s can be recommended as the optimal frequency of APE in clinical care practice. A novel device for lower leg intermittent pneumatic compression synchronized with active ankle exercise for prevention of deep vein thrombosis

Abstract:Objective: Intermittent pneumatic compression devices (IPCDs) and active ankle exercises have been shown to be efficacious in preventing venous thromboembolism (VTE) by increasing venous flow velocity and volume. However, IPCDs are expensive and require electricity; therefore, they cannot be used in the event of power loss. We developed a non-powered device that provides lower leg intermittent pneumatic compression synchronized with AAEs (LISA) and compared its efficacy with AAEs alone in increasing the peak velocity in the femoral vein. Methods: The study population consisted of 20 healthy younger men and 20 healthy older men who performed AAE every 2 s in a sitting posture under four conditions: AAE with LISA (AAE+LISA), AAE alone (AAE), AAE with IPCD, and AAE with a graduated compression stocking. Results: The PVs under all conditions were significantly higher than those at rest. The PVs in the AAE+LISA condition were significantly higher than those in the AAE alone condition in both younger and older groups (both p < .001). Conclusions: AAE with LISA significantly increased the PV, suggesting that LISA might be useful for preventing DVT.

The Impacts of Fast-track Surgery on Postoperative Deep Venous Thrombosis Among Patients with Lower Limb Fracture

Abstract:Purpose: This research was conducted to construct an ankle pump motion counter and system with orthopedic characteristics and analyze the impacts of fast-track surgery on postoperative deep venous thrombosis (DVT) among patients with lower limb fractures. Methods: First, an ankle pump motion counter system was set up to detect postoperative rehabilitation training (Hardware design: This involves components such as an accelerometer sensor, microcontroller, circuit design, power supply, and wireless module. The accelerometer sensor is used to monitor key points and capture motion signals, while the microcontroller handles frequency calculations and generates alerts for abnormal ankle pump motion parameters. Circuit design ensures the proper functioning of the device, and the power supply meets the requirements of the ankle pump motion counter. The wireless module is used for data transmission and communication with other devices. Software design: This includes software design for both the patient and doctor sides. The software design involves defining software requirements and module divisions, designing data acquisition and filtering programs, developing programs for data parameter reading and writing, implementing communication protocols, designing data communication programs, and creating rehabilitation training plans and training record programs). Then, a retrospective analysis was carried out for the subjects (100 patients with lower limb fractures treated in Zhejiang Hospital between June 2021 and June 2022. They were randomly enrolled into control and experimental groups (50 cases each). The ankle pump motion counter was utilized for the patients in the experimental group. Before surgery, gender, age, the incidence of venous thromboembolism (VTE), and the muscle strength of both lower limbs of the two groups were recorded. After surgery, numerical rating scale (NRS) pain scores, D-dimer (D-D), and average length of hospitalization 3 d after surgery and venous thrombosis of both lower limbs 5 d after surgery of two groups were compared. Results: D-D of the control group was significantly higher than that of the experimental group 3 days after surgery (P < .05), while the NRS pain score was relatively lower (P < .05). The average hospitalization length for the experimental group was 10.2 days versus 16.2 days for the control group. The incidence of VTE 5 days after the surgery was 2% for the experimental group compared to 6% for the control group (P < .05). Conclusion: The ankle pump motion counter system has the potential to improve VTE prevention, enhance patient compliance, streamline healthcare delivery, standardize care, and enable data-driven decision-making at a wider clinical level. By accurately monitoring ankle pump exercises and providing real-time feedback, this system can contribute to better patient outcomes, save time for healthcare providers, and facilitate evidence-based practices in the prevention of postoperative DVT among patients with lower limb fractures.
A hemodynamic study of popliteal vein blood flow: The effect of bed rest and electrically elicited calf muscle contractions

Abstract:Venous stasis, due to lack of activation of the calf muscle pump of postoperative patients, can result in the development of a thrombus which, in turn, can lead to a potentially fatal pulmonary embolism. The presented study investigates the effects that four hours of bed rest has on the lower limb hemodynamics of healthy subjects and, to what extent electrically elicited contractions of the calf muscles can alleviate these effects. Results indicated that the non-stimulated group experienced a decline in popliteal venous blood flow of approximately 45 % and a 10 % decrease in heart rate. The stimulated group maintained a higher venous blood flow and heart rate. The results suggest that even short periods of bed rest can significantly reduce lower limb blood flow which could have implications for DVT development in post-operative patients. Electrically elicited calf muscle contractions significantly improves lower limb blood flow and can alleviate the debilitating effects of bed rest.

The role of optical access trocars in laparoscopic surgery.

Abstract:A literature review between 2000 and 2005 has been conducted for laparoscopic access techniques. The blind insertion of a Veress needle or first trocar to create the pneumoperitoneum has been shown to cause vascular and visceral injuries. To reduce the risk of peritoneal entry, many surgeons prefer an open-access technique, like a Hasson trocar. Other trocars that can be used for laparoscopic entry are direct trocars and radially expanding trocars. No specific technique has been shown to be superior in preventing vascular and visceral complications. Optical trocars combine the advantages of the different entry techniques. This chapter describes the different entry techniques, indications, and clinical experiences of the optical trocars. An optical trocar provides a safe and feasible primary insertion method for laparoscopy in patients prone to access injuries.

Abstract:Background: Peripheral artery disease (PAD) is an atherosclerotic disease with luminal narrowing of blood vessels, leading to an obstruction of blood flow. It most commonly affects blood vessels of the lower extremities. The ankle-brachial index (ABI, the ratio of ankle pressure over brachial pressure) is a primary index in the diagnosis (ABI < 0.9) of PAD. However, resting ABI may not be sensitive enough to detect mild/moderate PAD in some patients. The ABI after a 5 -min exercise stress test is currently used to confirm a diagnosis of PAD in patients with classical symptoms of PAD, but with normal resting ABIs. However, not all of the patients can tolerate the treadmill test. The purpose of this study was to examine the effects of lower limb heating on the ABI measurement, since lower limb heating can also induce lower limb vasodilation as walking does. Hypothesis: We hypothesized that lower limb heating would specifically decrease ABI in PAD patients. Methods: In 10 PAD patients and 12 healthy control subjects, resting ABI was measured at first. Then, both feet and lower legs were immersed in warm water (~ 40 oC) for 30 min. Thereafter, ABI was measured again. This study is associated with a registered clinical trial (NCT03900832). Data and a summary of the results: Lower limb heating did not significantly alter the hemodynamic variables in both groups. Lower limb heating significantly decreased the ABI of both legs in PAD patients (0.75 ± 0.03 to 0.67 ± 0.04, P = 0.003), and did not alter the ABI in healthy controls (1.01 ± 0.01 to 1.03 ± 0.01, P = 0.103). Discussion and Conclusion: The presented data show that lower limb heating decreases ABI in PAD patients, while it does not alter ABI in healthy individuals. The results suggest that lower limb heating can increase the sensitivity of the ABI measurement in PAD patients. Many patients with PAD can have comorbidities that are either absolute or relative contraindications to exercise treadmill testing. Moreover, many patients with intermittent claudication may not tolerate exercise treadmill testing. We speculate that the lower limb heating approach can be applied in these patients. One major risk factor for its development is either long-term sitting or standing in the same position and the nature of the work performed. Material and methods: A group of 58 patients (mean age: 59.69 ±14.59 years) with CVI in CEAP (Clinical (C), Etiological (E), Anatomical (A), and Pathophysiological (P)) classification categories 2 and 3, and a group of 37 (mean age: 51.49 ±14.86 years) healthy volunteers performing sedentary work for at least 6 h during the working day were enrolled into the study. The total duration of observation lasted 8 months (8 visits), during which the following parameters were assessed at the beginning and end of this period: pain intensity (according to the visual analogue scale (VAS)), level of saturation on the toe, pulse rate, and lower limb Doppler ultrasound evaluation of reflux parameters. Results: The exercises used in people with CVI resulted in a significant reduction (p ≤ 0.01) in the occurrence of symptoms. Among all respondents, after 8 months of exercise, a significant reduction in pain level according to the VAS of the lower limbs, an improvement in saturation at the toe level, and a reduction in venous reflux was recorded (p ≤ 0.05). Conclusions: Home exercises with the use of an automatic exercise rehabilitation device alleviated significant symptoms in patients with CVI and improved the calf muscle pump.
Vascular adaptations to low-load resistance training with and without blood flow restriction

Abstract:PurposeTo examine the effects of low-load knee extensor training to fatigue with and without blood flow restriction (BFR) on calf vascular conductance, calf venous compliance, and peripheral arterial stiffness in middle-aged individuals.MethodsEleven men (55 ± 8 years) and five post-menopausal women (57 ± 5 years) completed 6 weeks of unilateral knee extensor training with one limb exercising with BFR (BFR limb) and the contralateral limb exercising without BFR (free flow, FF limb). Before and after the training, femoral pulse wave velocity (PWV), calf blood flow (normalized as conductance), and calf venous compliance were measured in each limb.ResultsPWV increased following training in both limbs (main effect of time, p = 0.036; BFR limb 8.9 ± 0.8 vs. 9.5 ± 0.9 m/s, FF limb 9.0 ± 1.2 vs. 9.0 ± 1.1; Pre vs. Post). Calf blood flow increased (p = 0.026) in the FF limb (25.0 ± 7.0 vs. 31.8 ± 12.0 flow/mmHg; Pre vs. Post) but did not change (p = 0.831) in the BFR limb (29.1 ± 11.3 vs. 28.7 ± 11.5 flow/mmHg; Pre vs. Post). Calf venous compliance did not change in either limb following training.ConclusionsThese results suggest low-load BFR resistance training to fatigue elicits small increases in peripheral arterial stiffness without eliciting concomitant changes in venous compliance. In addition, unlike low-load knee extensor training without BFR, training with BFR did not enhance calf blood flow.
