Abstract:Objectives: To analyze the changes of lower limb hemodynamics parameters before and after wearing graduated compression stockings (GCS) during ankle pump exercise in patients preparing for arthroplastic surgery. Method: The leg veins of 16 patients awaiting arthroplasty were analyzed using a Sonosite M-Turbo ultrasound system during ankle pump exercise with or without GCS. The age of them was 70 ± 7 years (mean ± SD) (range 56-82 years) and body mass index was 25.8 ± 3.0 kg/m2 (range 18.0-30.5 kg/m2). Measured data including the cross-sectional area (CSA), anteroposterior (AP) diameter and lateromedial (LM) diameter of the soleus vein (SV), posterior tibial vein (PTV) and great saphenous vein (GSV). Additionally, the peak velocities of femoral vein (FV) were also measured. Results: GCS could significantly decrease the cross-sectional area of SV, PTV and GSV in supine position at rest and maximum ankle plantar flexion. But the compression effect of GCS to SV and GSV was not observed during maximum ankle dorsiflexion. It was found that GCS application reduced the peak flow velocity of the femoral vein from 61.85 cm/s (95% CI = 50.94-72.75 cm/s) to 38.01 cm/s (95% CI = 28.42-47.59 cm/s) (P < 0.001) during ankle plantar flexion and decreased the femoral vein in these patients from 80.65 cm/s (95% CI = 70.37-90.92 cm/s) to 51.15 cm/s (95% CI = 42.58-59.73 cm/s) (P < 0.001) during ankle dorsiflexion. But this effect was not significant in supine position at rest. Conclusions: GCS could significantly reduce the peak flow velocity of the femoral vein during ankle pump exercise in the patients preparing for arthroplastic surgery.
Thigh-length graduated compression stocking cannot increase blood velocity of the common femoral vein in patients awaiting total hip arthroplasty

Abstract:Objectives: Graduated compression stocking (GCS) is one of the mechanical prophylaxes commonly used for deep vein thrombosis (DVT). The present study was designed to observe the effects of graduated compression stockings on the vein deformation and hemodynamics of lower limbs in patients awaiting total hip arthroplasty (THA). Methods: The lower extremity veins of 22 patients awaiting THA were examined by ultrasound, when they rested in supine position with or without thigh-length GCS. The deformation parameters we measured included antero-posterior (AP) diameters, latero-medial (LM) diameters, and cross-sectional area (CSA) of great saphenous vein (GSV), posterior tibial vein (PTV), popliteal vein (PV), gastrocnemius vein (GV), and superficial femoral vein (SFV). We measured peak velocity and mean velocity of GSV, common femoral vein (CFV), junction of GSV and CFV to represent for hemodynamics of veins. Results: Significant compression was observed in almost all measured veins with the use of thigh-length GCS, while it was unable to significantly compress GSV in latero-medial diameter. The mean latero-medial diameter reductions for GSV, PTV, GV, PV and SFV were 19.4, 30.2, 43.2, 29.7 and 20.4%, respectively. GCS significantly compressed antero-posterior diameter of GSV, PTV, GV, PV and SFV by 43.4, 33.3, 42.1, 37.5, and 27.8%, respectively. The mean reduction of cross-section area was 44.8% for GSV, 49.6% for PTV, 60.0% for GV, 57.4% for PV, and 36.2% for FV. No significant changes were observed in the mean blood velocity of GSV, CFV, and junction. GCS was able to significantly reduce peak velocity of CFV (17.6 ± 5.6 cm/s to 16.1 ± 6.0 cm/s) and junction (23.3 ± 9.5 cm/s to 21.3 ± 9.7 cm/s), while it did not change the peak velocity of GSV. Conclusion: Thigh-length GCS is sufficient to compress lower extremity veins in patients awaiting THA in supine position with the greatest compression in GV, while it was unable to significantly increase blood velocity of common femoral vein or GSV. GCS may prevent DVT through more than simply increasing blood flow. Further studies are needed to determine the specific effects of GCS.
A Randomized Controlled Trial of Standard versus Customized Graduated Elastic Compression Stockings in Patients with Chronic Venous Disease

Abstract:Objective This study aimed to compare the efficacy of customized graduated elastic compression stockings (c-GECSs) based on lower leg parameter models with standard graduated elastic compression stockings (s-GECSs) in patients with chronic venous disease (CVD). Methods In this randomized, single-blind, controlled trial, 79 patients with stage C2 or C3 CVD were assigned to one of two groups: c-GECSs or s-GECSs. The primary outcome was change to Venous Insufficiency Epidemiological and Economic Study-Quality of Life (VEINES-QOL) scores at months 1, 3, and 6 as compared to baseline. Secondary outcomes included compliance with wearing ECSs, interface pressure (IP) at the smallest circumference of the ankle (point B) and the largest circumference of the calf (point C), and calf volume (CV). Results 13 pairs of s-GECS and 2 pairs of c-GECS showed pressure values higher than the standard at either point B or C. The c-GECSs were significantly superior to s-GECSs in terms of score improvement at all three time points (month 1, 8.47 [7.47–9.45] vs. 5.89 [5.00–6.78]; month 3, 9.60 [8.47–10.72] vs. 6.72 [5.62–7.83]; month 6, 7.09 [5.93–8.24] vs. 3.92 [2.67–5.18], p < 0.0001). Besides, at month 1, the mean daily usage time of the c-GECS and s-GECS groups was 10.7 and 9.5 h, respectively ( p < 0.05). Correlation analysis indicated a negative relationship between local high pressure and daily duration in the s-GECS group (r pb = -0.388, n = 38, p < .05). Variances in pressure were greater in the s-GECSs group. The c-GECSs showed advantage in maintaining pressure. Both c-GECSs and s-GECSs effectively reduced CV (mL), with no significant differences between groups (month 1, 90.0 [71.4–108.5] vs. 85.0 [65.6–104.2]; month 3, 93.8 [69.7–117.8] vs. 85.9 [65.5–106.2]; month 6, 70.8 [46.5–95.2]) vs. 60.8 [44.1–77.5]). Conclusion The c-GECSs based on individual leg parameter models significantly improved VEINES-QOL scores and provided stable and enduring pressure as compared to s-GECSs for patients with stage C2 or C3 CVD. Although both c-GECSs and s-GECSs effectively reduced CV, the superior fit and comfort of c-GECSs improved patient compliance. Hence, c-GECSs are a viable alternative for patients who have difficulty tolerating s-GECSs.

Comparison of clinical and physiological efficacies of different intermittent sequential pneumatic compression devices in preventing deep vein thrombosis: a prospective randomized study

Abstract:Background: There are few comparative studies about the optimal method of pneumatic compression to prevent deep vein thrombosis (DVT). The aim of this prospective randomized study was to compare venous hemodynamic changes and their clinical influences between two graded sequential compression groups (an alternate sequential compression device [ASCD] vs. a simultaneous sequential compression device [SSCD]). Methods: In total, 34 patients (68 limbs) undergoing knee and spine operations were prospectively randomized into two device groups (ASCD vs. SSCD groups). Duplex ultrasonography examinations were performed on the 4th and 7th postoperative days for the detection of DVT and the evaluation of venous hemodynamics. Continuous data for the two groups were analyzed using a two-tailed, unpaired t-test. Relative frequencies of unpaired samples were compared using Fisher exact test. Mixed effects models that might be viewed as ANCOVA models were also considered. Results: DVT developed in 7 patients (20.6%), all of whom were asymptomatic for isolated calf DVTs. Two of these patients were from the ASCD group (11.8%) and the other five were from the SSCD group (29.4%), but there was no significant difference (p = 0.331). Baseline peak velocity, mean velocity, peak volume flow, and total volume flow were enhanced significantly in both device groups (p < 0.001). However, the degrees of flow and velocity enhancement did not differ significantly between the groups. The accumulated expelled volumes for an hour were in favor of the ASCD group. Conclusions: Both graded sequential compression devices showed similar results both in clinical and physiological efficacies. Further studies are required to investigate the optimal intermittent pneumatic compression method for enhanced hemodynamic efficacy and better thromboprophylaxis.

Abstract:Aspidosperma subincanum Mart. is widely used in Brazilian folk medicine to treat digestive disorders. In this study, acute and subchronic toxicity and cytotoxicity of stem bark ethanolic extract of Aspidosperma subincanum (EEAs) have been evaluated. In addition, phytochemical analysis was performed. The EEAs had low acute toxicity in mice with LD50 =1129 +/- 154mg/kg p.o. and 397 +/- 15 mg/kg i.p. The LC50 was 1340 +/- 428 microg/mL in the brine shrimp assay. There was no relevance of serious changes in behavioral, hematological and biochemical parameters and no deleterious effect on vital organs of rats that resulted after 30 days daily exposure to 5 and 100 mg/kg of EEAs. Phytochemical analysis of stem bark of A. subincanum revealed the presence of indole alkaloids, saponins, terpenoids, steroids and tannins and resulted in the isolation of oleic acid and guatambuine as major constituents. Using the method of the dose by factor approach, the human safe dose was 210 mg/70 kg/day. The EEAs appears to be safe and non-toxic in low doses in rodents and domestic preparations used by population have relatively security.

Abstract:OBJECTIVE:To compare the treatment effect of venous-arterial extracorporeal membrane oxygenation (VA-ECMO) patients in the prophylactic distal perfusion catheter (DPC) and the non-prophylactic DPC.METHODS:A prospective randomized controlled trial (RCT) was conducted. Patients who received VA-ECMO treatment were reviewed at Affiliated Jinhua Hospital, Zhejiang University School of Medicine from January 2019 to June 2020 were divided into two groups, the prophylactic DPC group (DPC placed immediately after the patient VA-ECMO) and the non-prophylactic DPC group (the DPC was placed after the early limb ischemic signs by using evaluation of the lower extremity perfusion assessment table). Comparing the differences of clinical data of two group patients. Pearson correlation analysis was used to analyze the correlation between peak velocity of dorsalis pedis artery and peak velocity of posterior tibial artery and transcutaneous oxygen partial pressure (TcPO2).RESULTS:A total of 62 patients were included in the analysis, with 31 cases in prophylactic DPC group and another 31 cases in non-prophylactic DPC group. There were no significant differences in sex, age, body mass index (BMI), smoking index, underlying disease, catheterization site, recovery time before on machine, extracorporeal membrane oxygenation (ECMO) operation time, mechanical ventilation time, length of stay in intensive care unit (ICU), mortality rate in hospital, and acute physiology and chronic health evaluation II (APACHE II) between the preventive DPC group and the non-preventive DPC group. There was no significant difference in ECMO indications, ECMO intubation location and pipeline type. The bleeding in the non-prophylactic DPC group was lower than that in the non-prophylactic DPC group [6.5% (2/31) vs. 29.0% (9/31), P < 0.05]. There were no significant differences in limb complications such as cyanosis, necrosis, amputation, compartment syndrome, arterial thrombosis, vascular reconstruction and repair, pseudoaneurysm, limb ischemic or limb infection. During the ECMO operation, except the blood stream infection in the non-prophylactic DPC group was lower than that in the non-prophylactic DPC group [3.2% (1/31) vs. 19.4% (6/31), P < 0.05], there was no other statistical difference in complications between the two groups. The peak velocity of dorsalis pedis artery in the preventive DPC group was significantly higher than that of the non-preventive DPC group (cm/s: 19.30±10.85 vs. 17.85±8.55, P < 0.05), and the peak velocity of posterior tibial artery was significantly lower than that of the non-preventive DPC group (cm/s: 19.90±10.94 vs. 21.58±9.77, P < 0.05). Pearson correlation analysis showed that the peak velocity of dorsalis pedis artery and peak velocity of posterior tibial artery of the preventive DPC group and the non-preventive DPC group were positively correlated with TcPO2 (r values were 0.747, 0.856, 0.850, 0.813, respectively, and P values were all 0.000).CONCLUSIONS:For patients with VA-ECMO treatment, the incidence of blood stream infection and bleeding during ECMO operation in non-prophylactic DPC implantation patients is lower than that of prophylactic DPC implantation patients. TcPO2 is positively correlated with peak velocity of posterior tibial artery and dorsal foot artery in the cannulated limb. In patients with VA-ECMO undergoing femoral artery and vein puncture,in addition to judging the blood supply of lower limbs according to symptoms and signs, ultrasound and TcPO2 monitoring can also be used as effective monitoring methods.
Impact of compression on velocity of venous and arterial main blood flow and cutaneous microcirculation of the lower extremity

Abstract:Objective. To study the impact of compression on velocity of venous and arterial main blood flow of the lower extremity, as well as cutaneous microcirculation in the back part of the foot in healthy individuals and patients with decompensated forms of varicose disease and postthrombophlebitis syndrome. Materials and methods. In the investigation 56 individuals took part and divided into three groups: Group I – 20 healthy persons; Group II – 15 patients with varicose disease in decompensated stage; Group III– 21 patients with decompensated stage of postthrombophlebitis syndrome. In all participants of the investigation the index of ankle–brachial pressure, deep–femoro–popliteal index, the regional perfusion index, transcutaneous partial pressure of oxygen and partial pressure of carbon dioxide, the arterial blood flow velocity in femoral artery and of venous blood flow distally from sapheno–femoral junction were measured before and after application of elastic medical knitwear of various Class of compression or the cuff pressure. Results. In the Class III compression in patients of Group III the transcutaneously registered indices crossing have occurred between partial pressure of oxygen and carbon dioxide, accompanied by domination of the carbon dioxide partial pressure over the oxygen partial pressure while further enhancement of the compression Class. In patients of Group II this tendency was observed while application of Class IV compression only. At the investigation beginning the values of partial pressure of carbon dioxide registered were higher in the Group III patients, than in the patients of Group II (p=0.0001). Conclusion. While application of the Class III compression the velocity of the hip venous blood flow, comparing with its initial values, have lowered at average by 78% in patients of Group II and at average in 7.4 times in the patients of |Group III (p=0.0001). It is affordable in patients, suffering decompensated postthrombophlebitic syndrome, to apply the elastic compression of Classes I–II, while in those, having varicose disease in decompensated stage, – the elastic compression of Classes III and iV as well.
Effects of different exercise methods of calf muscles on the hemodynamics of lower extremity vein

Abstract:Objective To compare the effects of active and passive calf muscle contraction on the hemodynamics of the lower extremity vein. Methods 30 females were selected by convenient sampling. The hemodynamic indexes of the common femoral vein were measured by Duplex ultrasound during the active ankle pump exercise, active circular exercise, passive ankle pump exercise, passive circular exercise, and massage the calf muscles. Results There was no significant difference in the velocity of common femoral vein when the subjects do active ankle pump exercise, active circular exercise, and massage the calf muscles ( p > .05), but the velocity of common femoral vein was faster than that of passive ankle pump exercise and passive circular exercise ( p < .01). Conclusion The effects of active ankle exercise and massage on promoting venous blood return of lower extremity are better than that of passive ankle exercise.
Comparative Efficacy of Postoperative Compression Methods After EVLT for Great Saphenous Vein Insufficiency

Abstract:Introduction: The preference for endovascular techniques in treating varicose veins, particularly in the great saphenous vein (GSV), has increased due to their minimally invasive nature and reduced complications. Post-operative care, especially involving compression therapy, remains crucial to improve outcomes, prevent varicose vein recurrence, and enhance overall recovery. This study aimed to evaluate the efficacy of eccentric compression therapy compared to alternative post-operative care methods following endovenous laser treatment (EVLT) for GSV insufficiency. Methods: This prospective randomized clinical trial encompassed 88 EVLT procedures for GSV insufficiency. The participants were divided into two groups, each receiving different postoperative compression methods, and were evaluated over a specified period. The primary outcome was the pain scale after EVLT; meanwhile, the secondary outcome measured in the present study was the rate of GSV occlusion after EVLT. Results: Both groups underwent all EVLT procedures successfully without any complications. At the one-month duplex ultrasound (DUS) follow-up, the sapheno-femoral junction occlusion rates were 97% (43 out of 44) for group A (eccentric compression plus gradual compression stocking) and 95% (42 out of 44) for group B (only gradual compression stocking). Ecchymosis was observed in only 12 patients across both groups, accounting for an overall occurrence of 13.6%. Group A patients reported significantly lower analgesic usage (10%) compared to group B (18%), although this difference did not reach statistical significance. Analysis of postoperative pain data utilizing the visual analog scale (VAS) showed a median value of 5.5 in group B patients, which decreased to 3.1 with the application of eccentric compression. Moreover, there was less ecchymosis in group A observed by one week. Conclusion: This study contributes to the ongoing discourse on the efficacy of postoperative compression in varicose vein treatment. It underscores the necessity for more comprehensive, well-designed studies to yield clearer conclusions and provide better guidance for post-procedure care.
Consistent application of compression: An under-considered variable in the prevention of venous leg ulcers

Abstract:Venous leg ulcers, the most common leg ulcer, occur in patients with chronic venous insufficiency due to venous hypertension. Evidence supports the conservative treatment with lower extremity compression, ideally between 30-40 mm Hg. Pressures in this range provide enough force to partially collapse lower extremity veins without restricting arterial flow in patients without peripheral arterial disease. There are many options for applying such compression, and those who apply these devices have varying levels of training and backgrounds. In this quality improvement project, a single observer utilised a reusable pressure monitor to compare pressures applied using different devices by individuals in wound clinics with diverse training from specialties of dermatology, podiatry, and general surgery. Average compression was higher in the dermatology wound clinic (n = 153) compared to the general surgery clinic (n = 53) (35.7 ± 13.3 and 27.2 ± 8.0 mm Hg, respectively, p < 0.0001), and wraps applied by clinic staff (n = 194) were nearly twice as likely as a self-applied wrap (n = 71) to have pressures greater than 40 mm Hg (relative risk: 2.2, 95% confidence interval: 1.136-4.423, p = 0.02). Pressures were also dependent upon the specific compression device used, with CircAid®s (35.5 mm Hg, SD: 12.0 mm Hg, n = 159) providing higher average pressures than Sigvaris Compreflex (29.5 mm Hg, SD: 7.7 mm Hg, n = 53, p = 0.009) and Sigvaris Coolflex (25.2 mm Hg, SD: 8.0 mm Hg, n = 32, p < 0.0001). These results indicate that the device-provided pressure may be dependent on both the compression device and the background and training of the applicator. We propose that standardisation in the training of compression application and increased use of a point-of-care pressure monitor may improve the consistency of applied compression, thus improving adherence to treatment and outcomes in patients with chronic venous insufficiency.
Blood Flow Analysis of the Great Saphenous Vein in the Su-Pine Position in Clinical Manifestations of Varicose Veins of Different Severities: Application of Phase-Contrast Magnetic Resonance Imaging Data

Abstract:The progression of clinical manifestations of lower-limb varicose veins remains unclear. This study investigated changes in lower-limb venous blood flow using phase-contrast magnetic resonance angiography. Data were collected on veins from 141 legs. We compared legs with and without varicose veins and related symptoms and examined varying levels of varicose vein symptom severity. Legs without varicose veins exhibited a lower absolute stroke volume (ASV, p < 0.01) and mean flux (MF, p = 0.03) for the great saphenous vein (GSV) compared with legs with symptomatic varicose veins. Legs with asymptomatic varicose veins exhibited lower MF for the GSV (p = 0.02) compared with legs with symptomatic varicose veins. Among legs with varicose veins, asymptomatic legs exhibited lower ASV (p = 0.03) and MF (p = 0.046) for the GSV compared with legs that exhibited skin changes or ulcers; however, no significant differences were observed between legs presenting with discomfort or edema and legs with skin changes or ulcers, and between legs presenting with discomfort or edema and asymptomatic legs. In conclusion, in the supine position, increased blood flow rate and blood flow volume in the GSV were associated with symptomatic varicose veins and increased symptom severity.

Abstract:Objective To investigate the feasibility and efficacy of assessing the calf perforating veins using the ankle pump method in a sitting position by color Doppler ultrasonography. Methods We performed a multicenter prospective clinical trial between November 2022 and October 2023. Eligible patients with chronic venous disease and healthy controls were enrolled. The calf perforating veins were assessed by three different methods, namely manual compression in a standing position, manual compression in a sitting position, and ankle pump in a sitting position. The reflux durations and detection rate of the incompetent perforating veins were compared among the three methods. The number and diameter of calf perforating veins and the distribution of incompetent perforating veins were analyzed. Results A total of 50 patients with chronic venous disease and 50 healthy controls were included. There were 173 calves analyzed, including 97 healthy calves and 76 calves with chronic venous disease. The median number of perforating veins per calf was higher in the diseased calves (7.0, interquartile range 6.0–8.0) than the healthy calves (5.0, 3.0–6.0) ( p < 0.001). The diameter of incompetent perforating veins (2.3, 2.0–3.1 mm) was larger than that of competent ones (1.4, 1.2–1.7 mm). Most of the incompetent perforating veins (78.8%) were located in the medial and posterior middle of the calf. The reflux duration induced by the ankle pump method was greater than that induced by the manual compression methods ( p < 0.001). Although the ankle pump method had a higher detection rate (92.0%) of incompetent perforating veins than the manual compression methods (71.7% and 74.3% for standing and sitting positions, respectively) ( p < 0.001), especially in the distal lower leg, the manual compression methods revealed the incompetent perforating veins that were not shown by the ankle pump method. Conclusions The diseased calves with chronic venous disease have more perforating veins than the healthy calves. Incompetent calf perforating veins are commonly larger than competent perforating veins, with the majority of the incompetent calf perforating veins being located in the medial and the posterior middle of the calf. Most importantly, the AP-sit method provides a convenient and effective approach for assessing the calf perforating veins, especially those located in the distal calf, as an alternative or complementary to traditional manual compression, which is valuable in the daily practice of sonographers.

The role of optical access trocars in laparoscopic surgery.

Abstract:A literature review between 2000 and 2005 has been conducted for laparoscopic access techniques. The blind insertion of a Veress needle or first trocar to create the pneumoperitoneum has been shown to cause vascular and visceral injuries. To reduce the risk of peritoneal entry, many surgeons prefer an open-access technique, like a Hasson trocar. Other trocars that can be used for laparoscopic entry are direct trocars and radially expanding trocars. No specific technique has been shown to be superior in preventing vascular and visceral complications. Optical trocars combine the advantages of the different entry techniques. This chapter describes the different entry techniques, indications, and clinical experiences of the optical trocars. An optical trocar provides a safe and feasible primary insertion method for laparoscopy in patients prone to access injuries.

Abstract:Introduction Ankle pump exercise (APE) is an effective method to prevent deep vein thrombosis (DVT) in the lower extremities. However, traditional APE at a frequency of 3 times/min applied in the clinic lacked high compliance, and individuals were found to prefer an APE of 30 times/min. This study aimed to compare the effects of APEs with the above two different frequencies on the venous hemodynamics and fatigue of lower extremities. Methods The study applied a randomized cross-over design, and 307 healthy adults were randomly assigned into two groups, whose left feet performed 5 minutes of traditional APE (3 times/min, after 10 minutes of rest) and 5 minutes of selected APE (30 times/min, after 10 minutes of rest) in different orders. The diameters and blood flow velocities of the external iliac vein, femoral vein, and popliteal vein of the left extremities were recorded with the color doppler ultrasound when participants were at rest and after 5 minutes of traditional or selected APE. The fatigue of lower extremities was assessed with the Rating of Perceived Exertion (RPE) after the participants performed 5 minutes of different APEs. Results After both traditional APE and selected APE, the diameters and blood flow (peak systolic) velocities of the external iliac vein, femoral vein, and popliteal vein increased significantly in the participants (P .05). However, participants reported that traditional APE caused higher fatigue, and 252 (82.1%) participants preferred APE at 30 times/min. Conclusion Both traditional and selected APE could increase venous blood flow in the lower extremity. Despite the equivalent effects, the selected APE of 30 times/min may cause less fatigue than the traditional one of 3 times/min.

Abstract:Abstract Objectives: To evaluate the effect of GCS on decreasing the incidence of postoperative DVT in patients after primary total knee arthroplasty.Method: A total of 226 patients who received primary TKA in Nanjing Drum Tower Hospital from Jan 1, 2018, to Jan 1, 2021 were enrolled. Depending on the presence of postoperative DVT, we divided the patients into 2 groups: the non-DVT group (n=192) and the DVT group (n=34). Parameters were compared using χ2 or t test between two groups. Binary logistic regression analysis was used to determine risk factors.Results: The incidence of postoperative DVT was 15% (n=34) in patients after TKA. Age, preoperative D-dimer and GCS usage were all associated with postoperative DVT. Compared to the non-DVT group, there was a significant increase in age (71.06±7.25 vs. 67.82±7.61, p=0.022) and preoperative D-dimer (1.51±2.21 vs. 0.92±1.39, p=0,042) in the DVT group. Moreover, the rate of GCS usage in the DVT group was significantly lower than that in the non-DVT group (23.5% vs. 48.4%, p=0.007). After adjusting for age, gender, preoperative D-dimer, and GCS usage by binary logistic regression analysis, GCS usage (OR = 2.950, 𝑃 = 0.013, 95% CI [1.261-6.901]) was an independent risk factor for postoperative DVT in patients after TKA.Conclusion: The overall prevalence of DVT was 15% in the current study, with 20.8% in non-GCS usage patients and 7.9% in GCS usage patients. GCS usage was an independent risk factor and could significantly decrease DVT risk in patients after TKA.
Sports compression garments improve resting markers of venous return and muscle blood flow in male basketball players

Abstract:Background: The benefits associated with sports compression garments are thought to be closely related to enhanced blood flow. However, findings are equivocal, possibly due to heterogeneity in the techniques used for measuring blood flow, the garment types used, and the pressures applied. This study combined Doppler ultrasound and near-infrared spectroscopy technologies to provide the first comprehensive assessment of the effects of 3 sports compression garment types on markers of venous return and muscle blood flow at rest. Methods: Resting lower-limb blood flow measures (markers of venous return, muscle blood flow, and muscle oxygenation) of 22 elite, junior, male basketball players (age = 17.2 ± 0.9 years, mean ± SD) were assessed in 4 separate conditions: no compression (CON), compression tights (TIGHTS), compression shorts (SHORTS), and compression socks (SOCKS). Markers of venous return (cross-sectional area, time-averaged mean and peak blood flow velocity, and venous blood flow) were measured via Doppler ultrasound at the popliteal and common femoral veins. Muscle blood flow and muscle oxygenation were measured in the gastrocnemius medialis and vastus lateralis using near-infrared spectroscopy. Results: Popliteal markers of venous return were higher in TIGHTS compared to CON (p < 0.01) and SHORTS (p < 0.01), with SOCKS values higher compared with CON (p < 0.05). Common femoral vein markers of venous return were higher for all conditions compared to CON (p < 0.05), with TIGHTS values also higher compared to SOCKS (p < 0.05). Gastrocnemius medialis blood flow was higher for TIGHTS compared to CON (p = 0.000), SOCKS (p = 0.012), and SHORTS (p = 0.000), with SOCKS higher compared to SHORTS (p = 0.046). Vastus lateralis blood flow was higher for TIGHTS compared to CON (p = 0.028) and SOCKS (p = 0.019), with SHORTS also higher compared to CON (p = 0.012) and SOCKS (p = 0.005). Gastrocnemius medialis oxygenation was higher for TIGHTS compared to CON (p = 0.003), SOCKS (p = 0.033), and SHORTS (p = 0.003), with SOCKS higher compared to CON (p = 0.044) and SHORTS (p = 0.032). Vastus lateralis oxygenation was higher for TIGHTS compared to CON (p = 0.020) and SOCKS (p = 0.006). Conclusion: Markers of venous return, muscle blood flow, and muscle oxygenation are increased with sports compression garments. TIGHTS are most effective, potentially because of the larger body area compressed.

Abstract:Background and objective: Diabetic foot (DF) complications often lead to severe vascular issues. This study investigated the effectiveness of enhanced external counterpulsation (EECP) and its derived innovative compression strategies in addressing poor perfusion in DF. Although developing non-invasive and efficient treatment methods for DF is critical, the hemodynamic alterations during EECP remain underexplored despite promising outcomes in microcirculation. This research sought to address this gap by developing a patient-specific 0D-1D model based on clinical ultrasound data to identify potentially superior compression strategies that could substantially enhance blood flow in patients with DF complications. Methods: Data were gathered from 10 patients with DF utilizing ultrasound for blood flow rate and computed tomography angiography (CTA) to identify lower limb conditions. Clinical measurements during standard EECP, with varying cuff pressures, facilitated the creation of a patient-specific 0D-1D model through a two-step parameter estimation process. The accuracy of this model was verified via comparison with the clinical measurements. Four compression strategies were proposed and rigorously evaluated using this model: EECP-Simp-I (removing hip cuffs), EECP-Simp-II (further removing the cuffs around the lower leg), EECP-Impr-I (removing all cuffs around the affected side), and EECP-Impr-II (building a loop circulation from the healthy side to the affected side). Results: The predicted results under the rest and standard EECP states were generally closely aligned with clinical measurements. The patient-specific 0D-1D model demonstrated that EECP-Simp-I and EECP-Impr-I contributed similar enhancement to perfusion in the dorsal artery (DA) and were comparable to standard EECP, while EECP-Simp-II had the least effect and EECP-Impr-II displayed the most significant enhancement. Pressure at the aortic root (AO) remained consistent across strategies. Conclusions: EECP-Simp-I is recommended for patients with DF, emphasizing device simplification. However, EECP-Simp-II is discouraged as it significantly diminished blood perfusion in this study, except in cases of limb fragility. EECP-Impr-II showed superior enhancement of blood perfusion in DA to all other strategies but required a more complex EECP device. Despite increased AO pressure in all the proposed compression strategies, safety could be guaranteed as the pressue remained within a safe range.

Abstract:Studies have primarily simulated the interface pressure and skin surface stress produced by compression textiles or garments with a single pressure level. However, only a few studies have reported the biomechanical transmission behaviors of compression fabrics from the skin to deeper soft tissues of the human body, especially within the context of compression fabrics providing multiple pressure levels. Therefore, to address this limitation, this study developed novel 3D finite element (FE) models to characterize and visualize the interface pressure exerted at the skin surface by elastic compression stockings (ECSs), a type of typical compression textiles, with either low- or high-pressure levels, and to analyze the resulting stress and stress transmission effects within lower limb tissues. The results of the validated FE models indicated that the simulated interface pressure favorably agreed with the measured pressure data. A close relationship existed between the pressure magnitudes and the lower limb surface curvatures, with the highest interface pressure occurring at the anterior and posterior bony regions of the ankle with greater surface curvatures than those of the posterior calf. The internal tissue stress distributions largely varied, which may be one of the potential causes for the inconsistency observed between the claimed pressure doses and those practically delivered inside. Overall, this study serves to improve the understanding of the working mechanisms of the ECS-lower limb system, allowing for improved functional material design and evaluation of the pressure effectiveness of compression textiles and garments used in physiotherapy.
