Abstract:Hypertension is a major risk factor for cardiovascular disease. Lowering blood pressure has been shown to decrease the incidences of stroke, heart attack, and heart failure (1,2). This report provides 2017-2018 U.S. hypertension prevalence estimates using the 2017 American College of Cardiology and American Heart Association definition of hypertension (3) and new guidelines, which redefine hypertension by lowering the previous threshold levels of 140/90 mmHg to 130/80 mmHg (4). This change categorizes a greater percentage of people as having hypertension.

Abstract:Hypertension, Ahead of Print. Background:Data from the US National Health and Nutrition Examination Survey are freely available and can be analyzed to produce hypertension statistics for the noninstitutionalized US population. The analysis of these data requires statistical programming expertise and knowledge of National Health and Nutrition Examination Survey methodology.Methods:We developed a web-based application that provides hypertension statistics for US adults using 10 cycles of National Health and Nutrition Examination Survey data, 1999 to 2000 through 2017 to 2020. We validated the application by reproducing results from prior publications. The application's interface allows users to estimate crude and age-adjusted means, quantiles, and proportions. Population counts can also be estimated. To demonstrate the application's capabilities, we estimated hypertension statistics for noninstitutionalized US adults.Results:The estimated mean systolic blood pressure (BP) declined from 123 mm Hg in 1999 to 2000 to 120 mm Hg in 2009 to 2010 and increased to 123 mm Hg in 2017 to 2020. The age-adjusted prevalence of hypertension (ie, systolic BP≥130 mm Hg, diastolic BP≥80 mm Hg or self-reported antihypertensive medication use) was 47.9% in 1999 to 2000, 43.0% in 2009 to 2010, and 44.7% in 2017 to 2020. In 2017 to 2020, an estimated 115.3 million US adults had hypertension. The age-adjusted prevalence of controlled BP, defined by the 2017 American College of Cardiology/American Heart Association BP guideline, among nonpregnant US adults with hypertension was 9.7% in 1999 to 2000, 25.0% in 2013 to 2014, and 21.9% in 2017 to 2020. After age adjustment and among nonpregnant US adults who self-reported taking antihypertensive medication, 27.5%, 48.5%, and 43.0% had controlled BP in 1999 to 2000, 2013 to 2014, and 2017 to 2020, respectively.Conclusions:The application developed in the current study is publicly available and produced valid, transparent, and reproducible results.

Abstract:Hypertension in youth tracks into adulthood and has long-term negative health effects. In 2017, the American Academy of Pediatrics released new Clinical Practice Guidelines on pediatric hypertension. The guidelines updated blood pressure (BP) thresholds and age-sex-height percentile references for youth 1–17 years. Pediatric hypertension prevalence estimates since the new guideline are lacking. This study aims to provide the latest nationally representative prevalence estimates of elevated BP and hypertension in youth aged 8–19 years, total and stratified by age group, sex, race and ethnicity, and body mass index (BMI) category. For adolescents aged 13–17 years, both percentile-based thresholds and adult BP thresholds are available in the guidelines. We therefore compared adult BP thresholds for elevated BP (120–129/<80 mmHg) and hypertension (≥130/≥80 mmHg) with percentile thresholds in adolescents. We used data from 2017–March 2020 National Health and Nutrition Examination Survey to obtain a sample of 2600 youth after excluding those with no BMI or BP measures. Prevalence estimates of elevated BP and hypertension were 8.7% (95% CI: 6.9-10.6) and 5.4% (95% CI: 4.0-6.7), respectively. Estimates were higher among males and increased with age (Table). Youth with severe obesity had the highest prevalence of hypertension. Hispanic youth had the lowest prevalence of elevated BP. In adolescents, elevated BP prevalence was similar using percentile thresholds (11.9%, 95% CI: 9.3-15.1) and the adult threshold (10.8%, 95% CI: 8.5-13.7). Hypertension prevalence in adolescents was also similar for percentile thresholds (4.8%, 95% CI: 3.1-7.6) and the adult threshold (4.8%, 95% CI: 2.0-7.5). Our findings suggest ongoing concern of elevated BP and hypertension among youth in the US. Either percentile or adult based thresholds for determining high BP could be used among adolescents by researchers. Knowledge of current national estimates can inform policies and interventions to support cardiovascular health in youth.

Abstract:Hypertension and high-normal blood pressure (BP) increase the risk for cardiovascular diseases. Examining trends in hypertension and high-normal BP among US adults is crucial. Participants aged 20 years or older from the 1999–2018 National Health and Nutrition Examination Surveys, were included. Trend analyses were performed to assess temporal changes in prevalence of hypertension and high-normal BP among US adults. Among the 48,580 participants included in this analysis, the mean (SD) age was 47.2 years (18 years) and 50.9% were women. Age-adjusted prevalence of hypertension was stable from 1999 to 2000 (29.5% [95% CI 26.6–32.3%]) through 2017–2018 (31.9%, [95% CI 29.0–34.7%]) ( P = 0.265 for linear trend). Age-adjusted prevalence of high-normal BP decreased from 10.9% (95% CI 9.4–12.5%) in 1999–2000 to 8.0% (95% CI 7.1–9.0%) in 2007–2008, then increased to 9.8% (95% CI 8.3–11.3%) in 2017–2018 ( P = 0.002 for nonlinear trend). Compared with men, hypertension and high-normal BP was less likely among women (multivariable-adjusted prevalence ratio, 0.90 [95% CI 0.84–0.97]; 0.68 [95% CI 0.52–0.88], respectively). Compared with non-Hispanic Black, high-normal BP was less likely among Mexican American, non-Hispanic White, and other race (multivariable-adjusted prevalence ratio, 0.59 [95% CI 0.44–0.79]; 0.53 [95% CI 0.41–0.69]; 0.56 [95% CI 0.74 − 0.71], respectively). The same held for hypertension.

Abstract:Background: Hypertension (HTN) stands as a significant risk factor for cardiovascular diseases. Identifying trends and disparities in HTN prevalence is vital for developing public health policies. Objective: Investigate the trends and disparities in HTN prevalence among U.S. adults from 2019 to 2022. Methods: Data from the CDC's National Health Interview Survey were utilized, with regression analysis including Joinpoint and ARIMA models performed by ChatGPT-4 to predict future trends. Results: The study observed fluctuations in the overall prevalence of diagnosed HTN starting at 27.0 % (95 % CI: 26.4-27.7, 2019), and reaching 27.2 % (95 % CI: 26.5-27.8, 2022). Males consistently showed higher HTN rates than females throughout the study period, with male prevalence increasing from 27.2 % (95 % CI: 26.3-28.1, 2019) to 27.9 % (95 % CI: 27.0-28.8, 2022), while females experienced decline from 26.9 % (95 % CI: 26.1-27.8, 2019) to 26.5 % (95 % CI: 25.7-27.3, 2022). Southern U.S. exhibited the highest prevalence at 30.1 % (95 % CI: 29.1-31.2, 2022), compared to the lowest in the West at 22.5 % (95 % CI: 21.4-23.8). Black adults showed a higher prevalence of 34.4 % (95 % CI: 32.4-36.4, 2022) compared to White adults at 27.4 % (95 % CI: 26.7-28.2), and significantly lower rates were observed in Asian adults at 14.5 % (95 % CI: 7.4-24.5). Conclusion: This study highlights stable trends in HTN prevalence among U.S. adults from 2019 to 2022, with significant disparities by gender, region, and race, underscoring the need for targeted public health interventions to address these inequalities.

Abstract:Overall hypertension prevalence has not changed in the United States in recent decades although awareness, treatment, and control improved. However, hypertension epidemiology and its temporal trends may differ in younger adults compared with older adults. Our study included 41 331 participants ≥18 years of age from 8 National Health and Nutrition Examination Surveys (1999–2014) and estimated temporal trends of hypertension, awareness, treatment, and control among young adults (age, 18–39 years) compared with middle-age (age, 40–59 years) and older adults (age, ≥60 years). In 2013 to 2014, 7.3% of the US young adults had hypertension. During 1999 to 2014, young adults saw larger increases in hypertension awareness, treatment, and control than did older adults. However, all of these components of hypertension control were lower among young adults compared with middle-aged or older adults (74.7% younger versus 81.9% middle versus 88.4% older for awareness; 50.0% versus 70.3% versus 83.0% for treatment; and 40.2% versus 56.7% versus 54.4% for control). Worse hypertension awareness, treatment, and control in young adults overall were mostly driven by worse measures in young adult men compared with young adult women. More frequent healthcare visits by young adult women explained ≈28% of the sex-related difference in awareness, 60% of the difference in treatment, and 52% of the difference in control. These findings suggest that improved access to and engagement in medical care might improve hypertension control in young adults, particularly young adult men, and reduce life-time cardiovascular risk.

Abstract:We evaluated the prevalence, awareness, treatment, and control of hypertension (defined as a systolic blood pressure [BP]) ≥140 mm Hg, diastolic BP ≥90 mm Hg, or a self-reported use of an antihypertensive agent) among US adults, stratified by race/ethnicity. This analysis included 16 531 nonpregnant US adults (≥18 years) in the three National Health and Nutrition Examination Survey cycles between 2013 and 2018. Race/ethnicity was defined by self-report as White, Black, Hispanic, Asian, or other Americans. Among 76 910 050 (74 449 985–79 370 115) US adults with hypertension, 48.6% (47.3%–49.8%, unadjusted) have controlled BP. When compared with BP control rates for White adults (49.0% [46.8%–51.2%], age-adjusted), BP control rates are lower in Black (39.2%, adjusted odds ratio [aOR], 0.71 [95% CI, 0.59–0.85], P <0.001), Hispanic (40.0%, aOR, 0.71 [95% CI, 0.58–0.88], P =0.003), and Asian (37.8%, aOR, 0.68 [95% CI, 0.55–0.84], P =0.001) Americans. Black adults have higher hypertension prevalence (45.3% versus 31.4%, aOR, 2.24 [95% CI, 1.97–2.56], P <0.001) but similar awareness and treatment rates as White adults. Hispanic adults have similar hypertension prevalence, but lower awareness (71.1% versus 79.1%, aOR, 0.72 [95% CI, 0.58–0.89], P =0.005) and treatment rates (60.5% versus 67.3%, aOR, 0.78 [95% CI, 0.66–0.94], P =0.010) than White adults. Asian adults have similar hypertension prevalence, lower awareness (72.5% versus 79.1%, aOR, 0.75 [95% CI, 0.58–0.97], P =0.038) but similar treatment rates. Black, Hispanic, and Asian Americans have different vulnerabilities in the hypertension control cascade of prevalence, awareness, treatment, and control. These differences can inform targeted public health efforts to promote health equity and reduce the burden of hypertension in the United States.

Trends in Prescription Fills for Blood Pressure-Elevating and Antihypertensive Medications, 2017-2023

Abstract:Background: Many medications can have blood pressure (BP)-elevating effects, which might negatively impact BP control among persons with hypertension. This study examines trends in prescription fills for BP-elevating and antihypertensive medications, individually and concurrently, among US individuals. Methods: Quarterly trends of concurrent and individual fills for BP-elevating and antihypertensive medications were reported using the national sample from IQVIA's Total Patient Tracker database, covering 94% of all retail prescription fills in the US. We identified 1,387 products containing BP-elevating medications and 240 products containing antihypertensive medications. Percentage change from Q1/2017 and average quarterly percent change from Joinpoint regression were used to present trends overall and by sex and age group (0-17, 18-44, 45-64, 65-74, and ≥75 years). Results: During 2017-2023, fills remained stable for BP-elevating medications alone and increased for antihypertensive medication alone (9.5% increase; from 10.1% to 11.0%; p<0.001). Concurrent fills for antihypertensive and BP-elevating medications increased by 15.9% (from 5.4% to 6.2%; p<0.001). Prescription fills for BP-elevating medications were higher among adult women compared to men; among women aged 18 to 44 years, this was primarily due to the use of combined oral contraceptives. In Q4/2023, prescription fills for BP-elevating medications were most common among those aged 65-74 years (females=30.7% vs males = 20.4%). Conclusions: These results provide the first national trends in concurrent prescription fills for BP-elevating and antihypertensive medications, indicating an increasing trend. Our findings might inform clinicians in their decision-making regarding medication selection for patients with hypertension.

Abstract:Self-reported hypertension is frequently used for health surveillance. However, little is known about the validity of self-reported hypertension among older Americans by nativity status. This study compared self-reported and measured hypertension among older black, white, and Hispanic Americans by nativity using the 2006 and 2008 Health and Retirement Study (n = 13,451). Sensitivity and specificity of self-reported hypertension were calculated using the Seventh report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure definition. Sensitivity was high among older blacks (88.9%), whites (82.8%), and Hispanics (84.0%), and both foreign-born (83.2%) and US-born (84.0%). Specificity was above 90% for both US-born and foreign-born, but higher for whites (92.8%) than blacks (86.0%). Despite the potential vulnerability of older foreign-born Americans, self-reported hypertension may be considered a reasonable estimate of hypertension status. Future research should confirm these findings in samples with a larger and more ethnically diverse foreign-born population.

Abstract:The key message from the article, "Blood Pressure Control Among US Adults, 2009 to 2012 Through 2017 to 2020" by Muntner et al, 1 is confirmation that hypertension control in the United States is declining. 2 The prior report documented that hypertension control in 2017 to 2018 was comparable to 2005 to 2006, rapidly reversing a decade of efforts to improve and sustain hypertension control. Evidence that hypertension control continues to fall is cause for alarm and further emphasizes the Surgeon General's call to action on a key vital sign of the nation's health. See related article, pp 1971–1980 Hypertension control among a representative sample of the US population civilian (National Health and Nutrition Examination Survey [NHANES]) fell from 52.8% in 2009 to 2012 to 48.2% in 2017 to 2020 ( P trend, 0.029). 1 NHANES was stopped in March 2020 due to the COVID-19 pandemic. Consequently, the 2019 to 2020 NHANES 2-year cycle was incomplete and only available for analysis as NHANES 2017 to 2020. Given the 4-year cycle for NHANES 2017 to 2020, the authors analyzed the 12-year period of NHANES 2009 to 2020 as 3 successive 4-year cycles 1 —a decision that may impact study findings. Two related impacts of the analytic approach shaped by COVID-19 include (1) the rate of decline in hypertension control may be greater than suggested from analysis of three 4-year cycles and the incomplete 2019 to 2020 2-year cycle and (2) the decline in hypertension control for 2017 to 2020 would likely have been greater if NHANES 2019 to 2020 had been completed as data collection stopped around the time of guidance to shelter in place to mitigate the adverse impact of COVID-19. In the prior report, 2 hypertension control reached a plateau from 2009 to 2010 through 2013 to 2014. Thus, the 2009 to 2012 period combined 2 comparable 2-year cycles during the plateau (Figure). In contrast, the 2013 to 2016 4-year cycle included the last 2 years of the plateau in 2013 to 2014 with hypertension control 53.8% and the first 2 years of the decline in 2015 to 2016 with hypertension control 48.4%. 2 In other words, the fall in hypertension control during 2013 to 2016 from levels in 2009 to 2012 was attenuated by the relatively high level of control in 2013 to 2014. The relatively sharp fall in hypertension control during 2015 to 2016 continued into 2017 to 2018 with control declining to 43.7%. The comparatively low level of hypertension control in 2017 to 2018 was likely mitigated by an increase in 2019 to 2020 pre-COVID as control for the 2017 to 2020 period was 48.2% versus 43.7% for 2017 to 2018 (Figure). Hypertension control is depicted for (1) NHANES 4-year blocks (2009–2012, 2013–2016, and 2017–2020), 1 which represented aggregation of data for adjacent 2-year cycles; (ii) NHANES 2-year cycles for 2009 to 2018, 2 and for 3 health plans for 2018 to 2020, 3 namely, commercial, Medicaid, and Medicare (no data, 2019). The rate of decline appears greater when analyzing NHANES 2-year cycles than 4-year blocks. Moreover, the COVID-19–related decline in hypertension control across health plans in 2020 was not captured in NHANES as examinations ended in March 2020. Data from health insurance plans are consistent with the impression that hypertension control rose in 2019. More specifically, from 2018 to 2019, hypertension control rose slightly in commercial health plans from 54.7% to 56.2% and from 58.9% to 60.8% in Medicaid health plans (Figure). 3 However, in 2020, hypertension control fell below 2018 levels among adults in commercial (54.7%–49.5%), Medicaid (58.9%–56.0%), or Medicare (69.5%–62.7%) health plans. Moreover, the decline in hypertension control during the full calendar year of 2020 is consistent with evidence that COVID-19 led to a 50% decline in office visits during 2020. 1 Although telemedicine visits increased rapidly, outpatient encounters with a blood pressure assessment declined 50%, since blood pressure values were only available on 10% of telemedicine encounters. Abstract:The prevalence, awareness, treatment, and control of hypertension in the United States are analyzed using the National Health and Nutrition Examination Survey (NHANES) database covering the period 1988-2002. Mean body mass index was 26.1+/-0.1 kg/m2 in 1988-1991 and 27.9+/-0.2 kg/m2 in 2001-2002 (p < 0.001). In the same period, the prevalence of diabetes mellitus increased from 5.0% to 6.5% (p = 0.03). Diastolic blood pressure was 73.3+/-0.2 mm Hg in 1988-1991 and 71.6+/-0.4 mm Hg in 2001-2002 (p < 0.001). Among the 18-39 years and 60 years and older age groups, the prevalence of hypertension increased significantly since 1988-1991. Multiple regression shows age, body mass index, and being non-Hispanic black were significantly associated with hypertension. In the period 1988-2002, the percentage receiving treatment and the percentage with blood pressure controlled increased significantly. In 2001-2002, significantly more people with hypertension and diabetes reached a blood pressure target of <130/85 mm Hg. Overall, the control rates were low, especially among middle-aged Mexican-American men (8%).

Abstract:Background: The Coronavirus Disease 2019 (COVID-19) pandemic disrupted health care, with particularly profound effects on persons with chronic conditions like hypertension. Objectives: In this study, we examined changes in the prevalence of blood pressure (BP) measurements by a healthcare professional among adults aged ≥ 18 years with hypertension before and during the COVID-19 pandemic in the United States (US). Methods: This study utilized the National Health Interview Survey data from April to December of the 2019 and 2021 modules of the survey. A total of 15,855 participants were included in the analytic sample. The prevalence of BP measurements taken by a health professional was calculated and the association between survey year and BP measurements was evaluated using adjusted and unadjusted logistic regression models. Results: Overall, the prevalence of BP measurements by a health professional among US adults with hypertension decreased from 95.9 % in the pre-pandemic period to 94.7 % in the pandemic period. Adults with hypertension were less likely (OR: 0.76, 95 % CI: 0.63-0.91) to report having had a BP measurement taken by a health professional during the pandemic compared to before the pandemic. Conclusion: Self-measured BP monitoring with clinical support could ensure continuous and improved care of individuals with hypertension, especially when circumstances could interrupt healthcare access.
Trends in the Prevalence of Multiple Chronic Conditions Among US Adults With Hypertension From 1999–2000 Through 2017–2020

Abstract:Abstract BACKGROUND The prevalence of many chronic conditions has increased among US adults. Many adults with hypertension have other chronic conditions. METHODS We estimated changes in the age-adjusted prevalence of multiple (≥3) chronic conditions, not including hypertension, using data from the National Health and Nutrition Examination Survey, from 1999–2000 to 2017–2020, among US adults with (n = 24,851) and without (n = 24,337 hypertension. Hypertension included systolic blood pressure (BP) ≥130 mm Hg, diastolic BP ≥80 mm Hg, or antihypertensive medication use. We studied 14 chronic conditions: arthritis, asthma, cancer, coronary heart disease, chronic kidney disease, depression, diabetes, dyslipidemia, hepatitis B, hepatitis C, heart failure, lung disease, obesity, and stroke. RESULTS From 1999–2000 to 2017–2020, the age-adjusted mean number of chronic conditions increased more among US adults with vs. without hypertension (2.2 to 2.8 vs. 1.7 to 2.0; P-interaction <0.001). Also, the age-adjusted prevalence of multiple chronic conditions increased from 39.0% to 52.0% among US adults with hypertension and from 26.0% to 30.0% among US adults without hypertension (P-interaction = 0.022). In 2017–2020, after age, gender, and race/ethnicity adjustment, US adults with hypertension were 1.94 (95% confidence interval: 1.72–2.18) times as likely to have multiple chronic conditions compared to those without hypertension. In 2017–2020, dyslipidemia, obesity, and arthritis were the most common 3 co-occurring chronic conditions among US adults with and without hypertension (age-adjusted prevalence 16.5% and 3.1%, respectively). CONCLUSIONS In 2017–2020, more than half of US adults with hypertension had ≥3 additional chronic conditions, a substantial increase from 20 years ago.

Age-Specific Prevalence and Factors Associated With Normal Blood Pressure Among US Adults

Abstract:Background: The mean systolic blood pressure (SBP) for US adults increases with age. Determining characteristics of US adults ≥65 years with normal blood pressure (BP) may inform approaches to prevent this increase. Methods: We analyzed US National Health and Nutrition Examination Survey 2011-2018 data (n = 21,581). BP was measured up to 3 times and averaged. Normal BP was defined as SBP <120 mm Hg and diastolic BP (DBP) <80 mm Hg among participants not taking antihypertensive medication. Those with SBP ≥120 mm Hg, DBP ≥80 mm Hg, self-reporting having hypertension or taking antihypertensive medication were categorized as having elevated BP or hypertension. Results: The prevalence of normal BP was 57.8%, 25.3%, 11.2%, and 5.0% among US adults who were 18-44, 45-64, 65-74, and ≥75 years, respectively. After multivariable adjustment, in US adults ≥65 years of age, normal BP vs. elevated BP/hypertension was more common among those with moderate and no vs. heavy alcohol consumption (prevalence ratio [PR] 3.03; 95% confidence interval [CI] 1.25-7.36 and 2.53; 95% CI 0.96-6.65, respectively), ≥150 vs. <150 minutes of physical activity per week (PR = 1.44; 95% CI 1.01-2.05), overweight and normal weight vs. obesity (PR = 1.88; 95% CI 1.22-2.90 and 2.94; 95% CI 1.89-4.59, respectively), and a high Dietary Approaches to Stop Hypertension score (PR = 1.43; 95% CI 1.00-2.05). US adults ≥65 years with normal BP vs. elevated BP/hypertension were less likely to have good or fair/poor vs. excellent/very good self-rated health, diabetes, albuminuria, atherosclerotic cardiovascular disease, and heart failure. Conclusions: Among US adults ≥65 years, normal BP was associated with healthy lifestyle factors and a lower prevalence of adverse health conditions.

Abstract:Objectives: Trends in prevalence and treatments of atherosclerotic cardiovascular disease (ASCVD) remains to be documented, with frequent update of relevant guidelines. We aimed to characterize trends in prevalence of ASCVD, and risk factor control and medications among ASCVD adults. Methods: We conducted a cross-sectional analysis of data from 55,081 adults in the National Health and Nutrition Examination Surveys (NHANES) 1999-2018. Results: The age -standardized prevalence of ASCVD did not change significantly from 1999-2002 (7.9 %, 95 % CI 7.1 %-8.7 %) to 2015-2018 (7.5 %, CI 6.8 %-8.3 %) (P for trend =0.18), representing an estimated 19.9 million individuals with ASCVD in 2015-2018. The prevalence of premature ASCVD was 2.0 % (CI, 1.6 %-2.5 %). Over 60.0 % of ASCVD participants were at very -high risk. From 1999-2002 to 2015-2018, the percentage with lipid control (non -high -density lipoprotein cholesterol <100 mg/dL) increased from 7.0 % (CI, 3.5 %-12.3 %) to 26.4 % (CI, 16.2 %-38.9 %). The percentage with blood -pressure control (<130/80 mmHg) increased from 51.2 % (CI, 41.0 %-61.3 %) in 1999-2002 to 57.2 % (CI, 48.4 %-65.6 %) in 2011-2014, but then declined to 52.8 % (CI, 44.4 %-81.3 %) in 2015-2018. The percentage with glycemic control (HbA1c <7.0 %) decreased from 95.0 % (CI, 90.2 %-97.9 %) to 84.0 % (CI, 75.9 %-90.3 %). The percentage who achieved all 3 targets was 18.6 % (CI, 8.2 %-33.8 %) in 2015-2018. The percentage of ASCVD participants who were taking statins increased from 1999-2002 to 2011-2014, but then leveled off. Approximately 60 % of individuals with ASCVD and less than 40 % of those with premature ASCVD were taking statins in 2015-2018. The utilization of blood -pressure -lowering drugs remained largely constant over time, whereas the use of glucose -lowering drugs increased. Conclusions: Based on NHANES data from US adults, the estimated prevalence of ASCVD remained relatively stable between 1999 and 2018. Substantial undertreatment with stains was found in individuals with ASCVD, and the percentage achieving optimal lipid control was low.

Abstract:BACKGROUND Less than half of US adults with hypertension (HTN) are controlled and a third are unaware of their condition. The emergency department (ED) represents a setting to improve HTN control by increasing awareness of asymptomatic hypertension (aHTN) according to the 2013 American College of Emergency Physicians (ACEP) asymptomatic elevated blood pressure clinical policy OBJECTIVE Estimate the prevalence and management of aHTN in United States (US) EDs. METHODS We examined the 2016-2019 National Hospital Ambulatory Medical Care Surveys (NHAMCS) to provide a more valid estimate of aHTN visits in US EDs. ED visits found to have aHTN is defined as adult patients with blood pressure ≥160/100mm Hg at triage and discharge without trauma or signs of end organ damage. We then stratified aHTN into a 160-179/100-109 mmHg subgroup and > 180/110mmHg subgroup and examined diagnosis and treatment outcomes. RESULTS Approximately 5.9% total visits between 2016-2019 met the definition for aHTN and 74% were discharged home representing an estimated 26.5 million visits. Among those discharged home, ED physicians diagnosed 13% (95%CI 10.6-15.8) and treated aHTN in 3.9% (95% CI 2.8-5.5) of visits in the higher aHTN subgroup. In the lower aHTN subgroup, diagnosis and treatment decreased to 3.1% (95%CI 2.4-4.1) and 1.2% (95%CI 0.7-2.0), respectively. CONCLUSIONS Millions of ED patient found to have aHTN are discharged home without diagnosis or treatment. While management practices follow clinical policy to delay treatment of aHTN there are missed opportunities to diagnosis aHTN.

Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aim:This study aimed to investigate awareness of having hypertension, diabetes and dyslipidaemia and their associated factors among US adults.Methods:Data from the National Health and Nutrition Examination Survey, including 21,399 adults aged ⩾20 years (pregnant women excluded) collected between 2011 and 2018, were used. Blood pressure was measured using a Baumanometer calibrated mercury true gravity wall model sphygmomanometer. Serum total cholesterol levels were measured using enzymatic assays. The percentage of haemoglobin A1C (HbA1c), which reflects long-term blood glucose levels, was measured and used to identify diabetes. Participants self-reported whether they were told by a doctor that they have hypertension, dyslipidaemia and diabetes. Awareness was defined as alignment between objective and self-reported measures for having the conditions. Sampling weights and the Taylor series linearisation variance estimation method were used in the analyses.Results:The findings showed that 64.06% of people with hypertension, 54.71% of those with dyslipidaemia and 78.40% of those with diabetes were aware of having the respective condition. Age, sex and health insurance were associated with awareness of having all three conditions, but marital status was not associated with any outcome. Weight status was associated with awareness of having hypertension and dyslipidaemia, whereas ethnicity was associated with awareness of having hypertension and diabetes. Relative family income was only associated with awareness of having hypertension.Conclusions:Large proportions of US adults with hypertension, dyslipidaemia and diabetes are not aware of having the conditions. Interventions targeting groups at higher risk of being unaware of these conditions are needed.

Abstract:Objective: The upward trends in the prevalence and control of hypertension in Chinese adults have been described, but recent trends based on the most recent guidelines are unavailable. We examined recent trends in the prevalence, awareness, treatment and control of hypertension among Chinese adults from 1991 to 2015 based on the 2018 Chinese Guideline. Methods: A total of 72 452 adults aged 20-79 years from the China Health and Nutrition Survey conducted between 1991 and 2015 were included in the study. Hypertension status and control rate were defined according to the 2018 Chinese Guideline. Age-standardized estimates were calculated based on the age distribution of the WHO standard population. Results: From 1991 to 2015, the crude/age-standardized hypertension prevalence (14.0/15.3 to 34.1/25.6%), awareness (29.4/24.2 to 43.8/27.2%), treatment (19.2/15.1 to 39.2/23.6%) and control rates (3.5/3.6 to 13.8/8.4%) increased (all P for trend <0.001). The prevalence of hypertension increased at a greater rate in rural regions compared with that in urban regions, whereas the control rate was higher in urban regions than that in rural regions. Compared with middle-aged and older adults aged 40-79 years, young adults aged 20-39 years had a larger increase in the prevalence of hypertension, but the awareness, treatment and control rates in the young adults did not increase. Conclusion: The prevalence of hypertension has increased markedly over the past two decades among Chinese adults, and the awareness, treatment and control rates have increased slightly or moderately and they have remained very low. These data underscore the need for effective measures to prevent hypertension and to increase the control of hypertension in Chinese adults.

Abstract:INTRODUCTION: Hypertension is a growing pandemic affecting over 1 billion people worldwide; about 46% of people with hypertension are unaware.METHOD: Data from the National Health and Nutrition Examination Survey (NHANES) 2011-2018 were analyzed to assess the relationship between access to a routine place of care and undiagnosed hypertension in adults aged 18 to 64 years old. We defined undiagnosed hypertension as those meeting the 2017 American Heart Association's guidelines for stage 1 or 2 hypertension who reported not being told by their healthcare provider that they had hypertension. We used a multivariable Poisson regression model to assess the relationship between access to a routine place of care and undiagnosed hypertension.RESULT: The final analytic sample was 5345 hypertensive American adults, with 56% unaware of their status. The results indicate that lack of awareness of hypertension status was highest among those without a routine place of care [PR = 1.20, CI = (1.12-1.29), p < 0.001] compared to those with access to a routine place of care, after adjustment for sociodemographic and clinical characteristics.CONCLUSION: Access to a routine place of care in a non-emergency department setting is essential to reduce the rate of undiagnosed hypertension among American adults. Policymakers should implement policies to address the shortage of primary care providers and increase access to a routine place of care.
