Abstract:The Africa Centers for Disease Control and Prevention declared mpox a Public Health Emergency of Continental Security (PHECS) in Africa. African public health systems have moved to mobilize a response against a backdrop of inherent significant challenges. With this commentary, we discuss how lessons from past public health emergencies, particularly COVID-19 and Ebola outbreaks, have prepared the region for improved disease surveillance, rapid response strategies, and effective public health communication and how these lessons can be applied to the mpox response, emphasizing the importance of strong healthcare infrastructure, effective data sharing, community engagement, targeted interventions, and robust contact tracing. Additionally, addressing misinformation and building public trust are crucial for controlling the spread of any disease. By leveraging these strategies, African countries can enhance their response to mpox. This includes improving diagnostic capabilities, strengthening cross-border collaborations, and prioritizing vaccination campaigns where needed. Ultimately, by applying the hard-earned lessons from the COVID-19 pandemic and Ebola outbreak, the East Africa region can better address the challenges posed by mpox and safeguard public health.

Abstract:The African continent is a crucial part of the historical and current narrative of mpox. The first human case of mpox (then termed monkeypox) was detected in 1970 in the Democratic Republic of the Congo (DRC), long before the recent spread of the disease to other parts of the world. Although mpox has been on the African continent for over five decades, it only gained worldwide attention when it began spreading to high-income countries in Europe and North America 1 . This is reminiscent of the global neglect of Ebola virus disease in the DRC and Sudan in the mid-1970s, which only gained international attention during the 2014–2015 Ebola outbreak when cases were exported to high-income countries. As with Ebola, mpox is emblematic of the global neglect of diseases that are endemic to Africa, up until they threaten the health security of the Global North. Progress must be made to control this disease and gaps need to be addressed to achieve global health security. Before the PHEIC was declared, Africa had registered several mpox outbreaks including the 2017 outbreak in Nigeria with 122 confirmed cases and 7 deaths (case fatality rate of 5.7%) from 17 states 5 . Similarly, between 2005 and 2007, at least 760 confirmed cases of mpox were reported from the DRC. Since then, the DRC has been reporting yearly increases in mpox cases (Table 1). Despite these high numbers, there was a global neglect of mpox outbreaks until the virus began spreading in the Global North (to access a more detailed dataset, please contact AfricaCDCEBS@africa-union.org). Abstract:On August 14, 2024, following a regional declaration by the Africa Centres for Disease Control and Prevention, the World Health Organization declared mpox a Public Health Emergency of International Concern, marking the second such declaration in two years. A series of outbreaks involving the more virulent clade I virus (compared to clade II, which caused a global outbreak in 2022), has now spread in 13 African countries, exposing the inadequacies of the public health infrastructure in these settings. There was significant investment during the 2022 global outbreak, but these efforts failed to address vaccine access and treatment in the Global South. Regulatory delays, unequal access to vaccines, and a lack of compassionate use treatments for severe cases have resulted in preventable cases and deaths, especially among vulnerable populations such as pregnant women, children, and the immunocompromised. The current outbreak also underscores critical knowledge gaps in our understanding of mpox, including its transmission, pathogenesis, and viral evolution. We join intensified calls for global solidarity and action to control mpox, emphasizing immediate containment measures and long-term local and international investment in African public health systems, to prevent future epidemics.

Abstract:Last month, when the world's most populus country, India, reported its first case of the new, highly transmissible clade Ib mpox variant, the challenge of containing the virus was once again evident. Only a few weeks before that in August, the World Health Organization (WHO) and the Africa Centres for Disease Control and Prevention (Africa CDC) declared mpox a public health emergency in response to its spread in Africa. Since then, cases of clade Ib mpox have been reported in Sweden, Thailand, and Pakistan. Although mpox is not yet a global pandemic, the new variant may tip the scales in that direction if the world does not act quickly to mitigate its spread in Africa.

Abstract:Background: The Mpox, a zoonotic viral disease that has historically affected Central and West Africa, has been declared a Public Health Emergency of International Concern by the World Health Organization. Rapid transmission and recent dissemination in Africa may imply significant challenges to global health, including mental health. Methods: We reviewed the mental health implications of the Mpox outbreak according to the published literature. We also discussed the psychological effects of the global spreading of this infection and public health preparedness strategies. Key areas of intervention may include the epidemiological surveillance, vaccine development, and mental health strategies. Comments: The Mpox outbreak calls for a robust global preparedness to address potential health emergencies. Strengthening epidemiological surveillance, ensuring equitable vaccine distribution, and building resilient public health infrastructure are crucial. Additionally, addressing mental health consequences may require immediate, intermediate, and long-term strategies, including telepsychiatry, stress management training, and the integration of mental health into primary care.

Abstract:The ongoing Mpox (Monkeypox) outbreak in Africa, now classified as a Public Health Emergency of International Concern (PHEIC) by the World Health Organization (WHO), presents a severe challenge, particularly for vulnerable populations like sex workers. Despite the endemic presence of Mpox in Africa since the 1970s, recent developments, including the emergence of a new clade Ib strain with increased transmissibility, have exacerbated the situation. Sex workers are at heightened risk due to their occupational exposure, compounded by stigma, criminalization, and limited access to healthcare. These factors significantly impede efforts to control the spread of the virus, leading to underreporting and inadequate intervention. This article highlights the urgent need for an inclusive public health response that prioritizes the health and safety of sex workers. Such a response should involve tailored health services, legal protections, and community engagement to ensure that this marginalized group is not overlooked. The decriminalization of sex work is also proposed as a critical public health measure to improve access to care and reduce stigma, ultimately curbing the spread of Mpox in Africa.

It did, but not quite enough. Despite some 91,000 cases of clade IIb mpox virus registered worldwide to-date [2], little progress has been made into gathering essential evidence as to what works or does not in preventing and treating mpox, with stark inequalities in access to therapy and vaccination between high-income and low- and middle-income countries, including the African countries with recurring outbreaks of clade I and clade IIa mpox virus [3].

Abstract:AbstractOn 23 July 2022, the World Health Organization reported monkeypox cases in over 75 countries and, as a result, declared the virus a Public Health Emergency of International Concern (PHEIC). Despite Africa being the centre of the zoonotic disease evolution, its healthcare systems have not provided optimum attention to the problem. The African healthcare system is under the threat of a repeat of the situation that transpired during the COVID‐19 pandemonium if stringent measures are not implemented immediately. Lessons learned from the COVID‐19 pandemic should inform emergency preparedness and response from African countries. Concerted and sustainable efforts can be achieved by reviewing and redesigning strategic preparedness plans for testing and vaccination as in previous pandemics. Africa needs to drive this response with collaboration and a united response, and with the use of strategic communication and a sense of urgency. Africa should prioritise access to available vaccines and invest in systems development for local vaccine production. In this article, we argue that monkeypox virus has serious public health implications in Africa and the region.

Abstract:Monkeypox (MPX) was a rare endemic disease in western and central Africa. In 1970, the first detected case of human MPX was reported in the Democratic Republic of Congo, and it was detected outside Africa in 2003. Currently, there are about 31,799 confirmed MPX cases which led the WHO to declare the disease a public health emergency of international concern which is considered the seventh deceleration by the WHO between 2009 and 2022. Herein, we aim to review the history behind the outbreak of the disease, its mode of transmission, and the target of WHO deceleration, while providing recommendations for disease prevention. The disease is prevalent mostly in the United States with a total case number of 10,676 which is considered a high-risk country. Meanwhile, other countries are at moderate risk. The disease can be transmitted directly through contact with different body fluids, infectious lesions, or sexual activity. We conclude that there should be high public awareness to stop the transmission of the disease. In addition, there is a great need to follow the instructions provided by public health institutions since vaccines, till now, are available only for high-risk populations secondary to their shortage.

Abstract:Mpox, caused by the Monkeypox virus (MPXV), has emerged as a significant global public health concern, particularly affecting vulnerable populations. The recent outbreak in the Democratic Republic of the Congo (DRC) is the largest recorded, driven by the highly virulent clade 1 strain. Transmission has shifted from animal contact to primarily sexual contact among Key Populations (KPs) such as Sex Workers (SW) and Men who have Sex with Men (MSM). In Zanzibar, where HIV prevalence is significantly higher among Key Populations, People Living with Human Immunodeficiency Virus (PLHIV) are at increased risk of Mpox infection due to socioeconomic challenges and immunosuppression. Despite no reported cases in Zanzibar, the spread of Mpox in non-endemic areas highlights the need for proactive measures. Leveraging Zanzibar's strengthened public health infrastructure, key strategies include tailored awareness campaigns, improved vaccine access through existing COVID-19 vaccination models, healthcare infrastructure enhancement, and mental health support. These targeted actions aim to protect Zanzibar's most vulnerable populations and bolster preparedness against Mpox, emphasizing the importance of resource-appropriate interventions to mitigate potential outbreaks.

Abstract:This study provides an in-depth analysis of mpox, encompassing its history, characteristics, epidemiology, diagnostics, treatment options, and the ongoing evolution of the virus and its transmission dynamics. Mpox, though once successfully eradicated, has re-emerged with new modes of transmission and a broader host range. Genomic analyses have revealed the virus's adaptability, posing challenges for diagnostics and vaccine efficacy. The epidemiology has shifted from sporadic zoonotic transmission in rural Africa to a significant presence in urban areas, particularly impacting high-risk populations. Advancements in diagnostics and therapeutics offer hope, but challenges persist. This work underscores the critical need for enhanced surveillance, vaccination strategies, and continued research to bolster global health systems and preparedness for future outbreaks.

Abstract:Mpox (formerly known as monkeypox) was declared a Public Health Emergency of International Concern (PHEIC) by the World Health Organization on 23rd July 2022, however cases of the disease have been detected in Nigeria since the 1970s and more recently since it began spreading in more urban areas of the country from 2017 onward. Nigeria has a strong track record of epidemic preparedness and response, spearheaded by the Nigeria Centre for Disease Control. Despite being somewhat separate architectures on paper, epidemic response (in particular, integrated disease surveillance and response) relies on a foundation of primary health care, which is inadequately funded not only in Nigeria, but globally. Situating mpox response within this wider landscape, we draw on ethnographic research from September 2022-March 2023 in southwestern Nigeria on lived experiences of mpox and mpox response, focusing on the perspectives of frontline health workers and community-based suspected or confirmed mpox cases. We aimed to understand how prioritization and resource constraints shape mpox response at a local level, including effects on the everyday work of frontline health workers in public health and clinical care who are left to "make do." We analyze their experiences interfacing with two intersecting infrastructures, community-based surveillance and primary health care. Health workers' improvisation and "repair work," which we detail, enables the surveillance system to function in some capacity. However, health workers must regularly contend with competing priorities and routine care that may be sidelined during an outbreak or epidemic. We argue that this reveals the limitations of a global health security agenda as it materializes at a local level and the need for strengthening primary health care for longer-term sustainability.
