Abstract:On August 14, 2024, following a regional declaration by the Africa Centres for Disease Control and Prevention, the World Health Organization declared mpox a Public Health Emergency of International Concern, marking the second such declaration in two years. A series of outbreaks involving the more virulent clade I virus (compared to clade II, which caused a global outbreak in 2022), has now spread in 13 African countries, exposing the inadequacies of the public health infrastructure in these settings. There was significant investment during the 2022 global outbreak, but these efforts failed to address vaccine access and treatment in the Global South. Regulatory delays, unequal access to vaccines, and a lack of compassionate use treatments for severe cases have resulted in preventable cases and deaths, especially among vulnerable populations such as pregnant women, children, and the immunocompromised. The current outbreak also underscores critical knowledge gaps in our understanding of mpox, including its transmission, pathogenesis, and viral evolution. We join intensified calls for global solidarity and action to control mpox, emphasizing immediate containment measures and long-term local and international investment in African public health systems, to prevent future epidemics.

Abstract:Just when the world started to adapt to the 'new normal' amid the coronavirus disease 19 (COVID‐19) pandemic, the world is witnessing the wrath of another viral disease, the monkeypox virus (MPXV). The virus is endemic to African countries, where several outbreaks have been reported in the past. However, the present cases have been reported in non‐endemic countries worldwide. Although MPX is considered to be a self‐limiting disease, recent reports on its incidence have proved otherwise. The 2022 multi‐country MPX outbreak has drawn the attention of global surveillance organizations and epidemiologists to trace its origin, however, there are existing gaps regarding the animal reservoirs, biological implications, and management of MPX. In view of the recent events, the World Health Organization (WHO) has also declared the ongoing MPX outbreak a global health emergency. Hence, the geographically expanding MPXV poses a significant threat to human health and public safety. In this review, the latest insights into the biology of MPXV have been provided by discussing its biological implications on human health, changing epidemiological footprint, and presently available intervention strategies. [Clinical value of high-resolution magnetic resonance tomography in the staging of tumors of the oral cavity, the pharynx and the larynx].

Abstract:In a prospective study 102 patients with malignant tumors of the tongue, oropharynx, hypopharynx, and larynx were staged by means of MRI. Special attention was directed at those tumour extensions that could influence the treatment strategy. The MR findings were correlated with the results of palpation, endoscopy, ultrasound, computerised tomography and histopathological findings. MRI showed a good delineation of pT2 to pT4 tumours. However, visualisation of small tumours in the soft palate, the tonsils, the pyriform sinuses and vocal cords was difficult. In conclusion our results suggest that in addition to endoscopy MRI is a valuable tool for the examination of tumours of tongue, oropharynx, hypopharynx and larynx.

Abstract:The African continent is a crucial part of the historical and current narrative of mpox. The first human case of mpox (then termed monkeypox) was detected in 1970 in the Democratic Republic of the Congo (DRC), long before the recent spread of the disease to other parts of the world. Although mpox has been on the African continent for over five decades, it only gained worldwide attention when it began spreading to high-income countries in Europe and North America 1 . This is reminiscent of the global neglect of Ebola virus disease in the DRC and Sudan in the mid-1970s, which only gained international attention during the 2014–2015 Ebola outbreak when cases were exported to high-income countries. As with Ebola, mpox is emblematic of the global neglect of diseases that are endemic to Africa, up until they threaten the health security of the Global North. Progress must be made to control this disease and gaps need to be addressed to achieve global health security. Before the PHEIC was declared, Africa had registered several mpox outbreaks including the 2017 outbreak in Nigeria with 122 confirmed cases and 7 deaths (case fatality rate of 5.7%) from 17 states 5 . Similarly, between 2005 and 2007, at least 760 confirmed cases of mpox were reported from the DRC. Since then, the DRC has been reporting yearly increases in mpox cases (Table 1). Despite these high numbers, there was a global neglect of mpox outbreaks until the virus began spreading in the Global North (to access a more detailed dataset, please contact AfricaCDCEBS@africa-union.org). [Laryngotracheal stenosis--etiology and therapeutic options].

Abstract:UNLABELLED Laryngotracheal stenosis, although not very common, is an important medical condition that represents a true diagnostic and therapeutic challenge. We present our experiences in treating 22 patients with laryngotracheal stenosis between 1995 and 2004 at Department of ENT, Head and Neck Surgery, Dr. Josip Bencević General Hospital in Slavonski Brod. PATIENTS AND METHODS Stenosis was caused by trauma in 9 patients, by endotrauma due to intubation in 3, and by war penetrating injury caused by shell and mine fragments in 4 patients. Hypertrophic glottic stenosis, after supraglottic laryngectomy and postoperative radiotherapy caused laryngeal obstruction in 2 patients. Tumors caused stenosis in 11 patients: tumors of the thyroid with compression in 9, and malignant infiltration of the trachea in 2 patients. Glottic stenosis was present in 4, glottic and subglottic in 4, subglottic and tracheal in 2, isolated tracheal stenosis in 12 patients. Surgical treatment was performed in 14 patients: endoscopic in two and classic open procedure in 12 patients. Laryngeal and tracheal stents were inserted in 3 patients with inopeable thyroid tumor. All treated patients except one were extubated. RESULTS Excellent functional results were achieved upon endoscopic resection, resection of stenosis and end-to-end anastomosis. Minimally invasive and good palliative treatment was achieved by the stent placement into the airway.

Abstract:Monkeypox (MPX) was a rare endemic disease in western and central Africa. In 1970, the first detected case of human MPX was reported in the Democratic Republic of Congo, and it was detected outside Africa in 2003. Currently, there are about 31,799 confirmed MPX cases which led the WHO to declare the disease a public health emergency of international concern which is considered the seventh deceleration by the WHO between 2009 and 2022. Herein, we aim to review the history behind the outbreak of the disease, its mode of transmission, and the target of WHO deceleration, while providing recommendations for disease prevention. The disease is prevalent mostly in the United States with a total case number of 10,676 which is considered a high-risk country. Meanwhile, other countries are at moderate risk. The disease can be transmitted directly through contact with different body fluids, infectious lesions, or sexual activity. We conclude that there should be high public awareness to stop the transmission of the disease. In addition, there is a great need to follow the instructions provided by public health institutions since vaccines, till now, are available only for high-risk populations secondary to their shortage.

Abstract:Study Design. Case report. Objective. To present the case of a patient with low back and lower extremity pain suspected to result from lumbar radiculopathy but who was discovered to have symptomatic uterine fibroid (UF). Summary of Background Data. UFs are usually asymptomatic but at times can produce symptoms such as excessive menstrual bleeding, intermenstrual bleeding, dysmenorrhea, pelvic pain, pelvic pressure, bloating, dyspareunia, urinary and bowel disturbance, subfertility, and pregnancy-related complications. In rare cases, they can cause neural impingement leading to lower extremity pain, which can be mistaken for lumbar radiculopathy. Methods. The patient was a 44-year-old woman who presented with low back pain and left leg pain. Examination suggested lumbar radiculopathy but lumbar magnetic resonance imaging was negative with the exception of a large mass in the pelvis. Pelvic magnetic resonance imaging confirmed this to be a large UF. Results. The patient's symptoms completely resolved following hysterectomy. Conclusion. In a female patient with suspected radiculopathy, in whom lumbar imaging is negative, or who does not respond as expected to treatment, UF should be considered.

Abstract:Mpox, the zoonotic disease caused by Monkeypox virus (MPXV), is currently a global health emergency. This review (Part I) aims to provide insights into the virus life cycle, epidemiology, host immune responses, and immune evasion mechanisms. Mpox symptoms is similar to smallpox but with lower mortality rates and lower transmissibility. In the past, the virus has been endemic in Central (Clade I) and West (Clade II) African countries. The first outbreak in outside Africa is reported in the United States in 2003. A multi-country outbreak across all continents occurred in 2022, predominantly driven by Clade II. Recently, the emergence of Clade Ib with sustained person-to-person transmission characteristic in the 2023-2024 outbreaks has raised significant public health concerns. Its apparent capacity for rapid spread and potential for causing severe disease highlight the need for enhanced surveillance, especially in regions not traditionally affected by Mpox. Immune responses induced by MPXV infection in humans and animal models provide the insights into the key step in which the host immune response recognizes and responds to the infection. The sophisticated immune evasion strategy by MPXV at both innate and adaptive arms also emerges that are useful for vaccine-based control measures. Taken together, understanding MPXV life cycle, epidemiology and immune response will facilitate better control, limit viral spread, and provide important insights for vaccine development.

Abstract:According to the WHO, more than 90,000 cases of mpox have been reported since the 2022 worldwide outbreak, which resulted in 167 deaths, while a new outbreak in Africa since 2023 has resulted in over 18,000 cases and 617 deaths. Mpox is a zoonosis caused by the monkeypox virus, a double-stranded DNA virus belonging to the Orthopoxvirus genus, which causes smallpox-like illness. Until 2022, cases were predominately located in West and Central Africa, with only sporadic cases and outbreaks reported in other parts of the world. During the 2022 outbreak, the primary mode of transmission was sexual contact among men who have sex with men. The changing epidemiology of mpox resulted in new disease phenotypes and populations at risk, disproportionally affecting people who live with HIV. Commonly presenting as a mild, self-limiting illness, mpox can cause severe and protracted disease in people with HIV with a CD4 count < 200 cell/mm3. The global emergence of mpox that followed and intersected with COVID-19 mobilized the scientific community and healthcare stakeholders to provide accurate diagnostics, preventive vaccines and treatment to those most affected. Despite existing gaps, this rapid response helped to contain the outbreak, but challenges remain as new variants emerge. Preparedness and readiness to respond to the next outbreak is crucial in order to minimize the impact to the most vulnerable.

Abstract:Abstract Background and Aims A double‐stranded DNA virus called monkeypox virus (MPV) belonging to the Poxviridae family and Orthopoxvirus genus causes monkeypox (mpox) infection. This virus used to infect only Central, East, and West Africa. However, it has spread to an extent outside Africa recently. The range of MPV outbreaks was so high that on July 23, 2022, the World Health Organization (WHO) declared it a Public Health Emergency of International Concern (PHEIC). About a year later, the WHO notified the end of a global public health emergency for mpox on May 11, 2023. Here, we aimed to assess the current pathogenicity and potential risk of MPV causing public health emergencies. Methods We searched information from published articles available in PubMed, Scopus, and ScienceDirect. We used monkeypox, mpox, monkeypox outbreak, and monkeypox virus as keywords during the literature search. Results Many new variants of MPV have emerged throughout the world that created PHEIC for mpox. Considering the low lethality and transmission rate, mpox is no longer a global public health threat. In addition, the availability of therapeutic and preventive measures helped the healthcare authorities fight the mpox infection in an efficient manner. In this review, we have portrayed the history and evolution of mpox from past to present and an idea of its future outcomes. Also, we have discussed the symptoms related to mpox and approved antiviral treatment strategies to fight off the infection in this piece. This review also emphasized the preventive guidelines set by the WHO for patients, caregivers, and healthcare providers to control the outbreak of mpox infection. Conclusion We believe this article would give an idea about the potential public health threats of the recent multi‐country monkeypox outbreak to the healthcare authorities for taking measures accordingly.

Abstract:Monkeypox (mpox), is a disease from the <i>Poxviridae</i> family that can cause several serious medical issues. This mini-review sought to analyze the existing literature regarding the current mpox outbreak with a focus on the prevalence, diagnostics, and containment measures. Mpox cases have been reported to World Health Organization (WHO) from 85 Member States in all six WHO regions during the period of January 1, 2022, through August 3, 2022. Standardized or optimized guidelines for the clinical care of patients with mpox are limited, particularly in low-resource settings. In an effort to achieve guidance and meet standards, special attention should be paid to this outbreak in order to eradicate such a rare infectious disease by analyzing prevention and control measures. Patient outcomes may also be poor, and their illnesses may last for a long time. The spectrum of clinical symptoms, including complications and sequelae, as well as aspects of the illness may be indicators of sickness severity and complications; therefore, its clinical presentation must be better understood to improve containment measures. In addition, it is important to create and evaluate a standard of care that takes a variety of parameters into account, including antiviral, immune therapies, and clinical metrics that are particular to mpox. The global emergence of mpox has presented new challenges for public health and has called for further investigation into its epidemiological profile across international contexts.

Abstract:Recently, monkeypox has become a global concern amid the ongoing COVID-19 pandemic. Monkeypox is an acute rash zoonosis caused by the monkeypox virus, which was previously concentrated in Africa. The re-emergence of this pathogen seems unusual on account of outbreaks in multiple nonendemic countries and the incline to spread from person to person. We need to revisit this virus to prevent the epidemic from getting worse. In this review, we comprehensively summarize studies on monkeypox, including its epidemiology, biological characteristics, pathogenesis, and clinical characteristics, as well as therapeutics and vaccines, highlighting its unusual outbreak attributed to the transformation of transmission. We also analyze the present situation and put forward countermeasures from both clinical and scientific research to address it.

Abstract:Mpox is a zoonotic illness caused by the Mpox virus (MPXV), a member of the Orthopoxvirus family. Although a few cases have been reported outside Africa, it was originally regarded as an endemic disease limited to African countries. However, the Mpox outbreak of 2022 was remarkable in that the infection spread to more than 123 countries worldwide, causing thousands of infections and deaths. The ongoing Mpox outbreak has been declared as a public health emergency of international concern by the World Health Organization. For a better management and control of the epidemic, this review summarizes the research advances and important scientific findings on MPXV by reviewing the current literature on epidemiology, clinical characteristics, diagnostic methods, prevention and treatment measures, and animal models of MPXV. This review provides useful information to raise awareness about the transmission, symptoms, and protective measures of MPXV, serving as a theoretical guide for relevant institutions to control MPXV.

Abstract:Background: The Mpox, a zoonotic viral disease that has historically affected Central and West Africa, has been declared a Public Health Emergency of International Concern by the World Health Organization. Rapid transmission and recent dissemination in Africa may imply significant challenges to global health, including mental health. Methods: We reviewed the mental health implications of the Mpox outbreak according to the published literature. We also discussed the psychological effects of the global spreading of this infection and public health preparedness strategies. Key areas of intervention may include the epidemiological surveillance, vaccine development, and mental health strategies. Comments: The Mpox outbreak calls for a robust global preparedness to address potential health emergencies. Strengthening epidemiological surveillance, ensuring equitable vaccine distribution, and building resilient public health infrastructure are crucial. Additionally, addressing mental health consequences may require immediate, intermediate, and long-term strategies, including telepsychiatry, stress management training, and the integration of mental health into primary care.

Abstract:The current monkeypox (MPX) outbreak has been declared a public health emergency of international concern (PHEIC) by the World Health Organization (WHO). It is a zoonotic disease that has persisted in the African basin for decades but suddenly exploded into the international sphere this year. In this paper, we provide a comprehensive overview of monkeypox, including a hypothesis of the rapid spread of the virus, its epidemiology and clinical features, a comparison with other orthopoxviruses such as chickenpox and smallpox, past and present outbreaks, and strategies for its prevention and treatment.
Insights into the challenging multi-country outbreak of Mpox: a comprehensive review

Abstract:Human monkeypox virus (hMpoxV) is of zoonotic origin and is closely related to the once-dreaded smallpox virus. It is largely endemic to the African continent but has moved out of the endemic regions as sporadic clusters in the past 20 years, raising concerns worldwide. Human Mpox is characterized by a mild to severe, self-limiting infection, with mortality ranging from less than 1% to up to 10% during different outbreaks caused by different clades of MpoxV. Bushmeat hunting is one of the primary reasons for its transmission from animals to humans. Various international and national health regulatory bodies are closely monitoring the disease and have laid down guidelines to manage and prevent hMpox cases. Emergency Use Status has been granted to Tecovirimat and Brincidofovir to treat severe cases and vaccination with the smallpox vaccine is recommended for high-risk group individuals. Strategies to repurpose and discover novel therapeutics and vaccines to control the outbreak are being researched. The current Mpox outbreak that has mainly affected men as approximately 96% of all cases are reported in men, is probably the result of a complex intersection of various factors. This necessitates a strong One Health response coordination involving human, animal and environmental health institutions. This review is an attempt to provide an all-inclusive overview of the biology, history, epidemiology, pathophysiology, diagnosis and management of hMpox in context to the recent 2022-2023 multi-country outbreak which is termed by WHO a 'Public Health Emergency of International Concern (PHEIC)'.
A critical review of mpox outbreaks, risk factors, and prevention efforts in Africa: lessons learned and evolving practices

Abstract:Objectives: In recent years, mpox, a zoonotic disease caused by the mpox virus, has transcended its primary association with Central and West Africa, emerging as a global public health concern. The virus poses a substantial threat, particularly, to vulnerable demographics such as young children and individuals with compromised immune systems. This critical literature review aimed to comprehensively evaluate the burden, risk factors, and current management strategies associated with mpox in Africa. Methods: This critical literature review was guided by Jesson & Laccy's guidelines on conducting critical literature reviews. We searched PubMed and Google Scholar databases and websites of the World Health Organization and health ministries in different African countries. We included articles written in English and published between 2010 and 2023. The synthesis of findings involved several steps, including summarizing themes, integrating themes, and linking themes to research questions. Results: A total of 25 articles were included in this review. The review revealed that mpox cases are concentrated in Central African countries. The risk factors for mpox identified include being in contact with bushmeat or rodents, not having been vaccinated against smallpox, being HIV-positive, and having close physical contact with someone with the disease. The clinical presentation of mpox revealed in this review includes a skin rash, fever, lymphadenopathy, headache, pruritus, sore throat, and body aches. Four themes arose on strategies to prevent and control mpox in Africa. Conclusions: The prevention and control of mpox in Africa require an improvement in community education, vaccination, disease surveillance, and infection control measures.
Monkeypox Virus and Other Emerging Outbreaks: An Overview and Future Perspective

Abstract:Monkeypox (MPX) is a zoonotic disease caused by the MPX virus from the poxviridae family of orthopoxviruses. Typically, endemic in central and west Africa, it has now become a matter of concern since cases have been reported in non-endemic countries around mid-June 2022, especially in the European region, with the transmission not related to travel. The diagnosis is made by PCR testing of the skin lesions. Even though treatment is symptomatic, antiretrovirals, such as tecovirimat, are used in severe cases. Vaccination with second and third generation vaccines is approved for prophylaxis in high risk individuals. Unfortunately, these options of treatment and prevention are only available in high income countries at the moment. This review, through a thorough literature search of articles from 2017 onward, focuses on epidemiology, clinical manifestations, challenges, treatment, prevention and control of MPX virus and how they can be corelated with other viral outbreaks including COVID-19, Acute Hepatitis of unknown origin, Measles and Dengue, to better predict and therefore prevent its transmission. The previous COVID-19 pandemic increased the disease burden on healthcare infrastructure of low-middle income countries, therefore, this recent MPX outbreak calls for a joint effort from healthcare authorities, political figures, and NGOs to combat the disease and prevent its further spread not only in high income but also in middle- and low-income countries.

Abstract:Historically, monkeypox (mpox) was a zoonotic disease endemic in Africa. However, in 2022, a global outbreak occurred following a substantial increase in cases in Africa, coupled with spread by international travellers to other continents. Between January 2022 and October 2023, about 91,000 confirmed cases from 115 countries were reported, leading the World Health Organization to declare a public health emergency. The basic biology of monkeypox virus (MPXV) can be inferred from other poxviruses, such as vaccinia virus, and confirmed by genome sequencing. Here the biology of MPXV is reviewed, together with a discussion of adaptive changes during MPXV evolution and implications for transmission. Studying MPXV biology is important to inform specific host interactions, to aid in ongoing outbreaks and to predict those in the future.
