Abstract:The purpose of this study was to investigate capabilities of Magnetic Resonance Angiography (MRA) in delineating neurovascular compression which has emerged as the most frequent etiology in cases of Hemi-Facial Spasm and Trigeminal Neuralgia. MR investigations were carried out at 1.5 t unit (Magnetom Vision, Siemens). The examination consisted of 3D time-of-flight MRA (3d TOF) protocol, with a FISP sequence (Fast Imaging Steady Precession) and magnetization transfer. Imaging parameters were; TR 39 ms, TE 7 ms; MX 256-512; FOV 210-210 mm. Time acquisition is 10 mm 30 s. The axially acquired angiographic slices were reformatted with a multiplanar reconstruction program (MPR) along and across the plane of facial and trigeminal nerves. Then millimetric raw data images were postprocessed with a maximum intensity projection (MIP) technique. 58 patients presented either with hemifacial spasm (36 cases, 16 women, 20 men, mean age: 58.5 y) or trigeminal neuralgia (22 cases, 13 women, 8 men, mean age: 51 y) were compared with 17 patients (9 women, 8 men, mean age: 57.5 y) as references. MRA with MIP and MPR techniques reliably demonstrates neurovascular compression at the root entry zone (REZ), and allows recognition of the implicated vascular loop in 34 the cases of hemifacial spasm investigated. Raw data were post-processed in an axially oblique plane parallel to the acoustico-facial bundle and secondarily reconstructed in coronally obliques views. These reconstructed images clearly delineate the cross-compression of the emerging facial nerve. The neurovascular conflict was related to an arterial loop from the postero-inferior cerebellar artery (PICA, 21 cases, 61%) and the antero-inferior cerebellar artery (AICA, 8 cases, 23%). Vertebral artery is implicated in conflicts in 5 cases. In 2 cases MRA exploration was negative. In HFS group distance between Vth nerve emergency and neuro vascular contact was measured and compared with control group; there is a significative difference (p < 0.01). In case of trigeminal neuralgia, MRA demonstrates neurovascular compression at the root entry zone of the Vth nerve of 16 patients. In theses cases, oblique and coronally reconstructed images are crucial in demonstrating neurovascular contacts. The vessel implicated in conflict is always the superior cerebellar artery (SCA). A supero-medial loop of the superior cerebellar artery was depicted in these 16 cases. Although 3 patients had negative MRA and MR studies, a case of compression of the origin of the Vth nerve by a enlarged draining vein in a case of brainstem arterio-venous malformation, a petrous apex meningioma was incidentally discovered and thoroughly investigated with gadolinium T1 weighted sequences and 1 demyelinating plaques located along the intraaxial course of the Vth nerve were noticed. In control group on 34 explanations only there are only 4 contacts between Vth nerve and SCA. MRA with a 3D-TOF FISP sequence allows recognition of neurovascular conflicts in case of hemifacial spasm and trigeminal neuralgia while MIP helps to characterize the implicated vessel. Sources images need to be carefully assessed in order to depict unsuspected pathology.

Abstract:The present report summarizes preliminary results of a retrospective study of the relationship between childbirth and mental disorder, the frequency of psychiatric symptoms post partum, and possible prognostic and predisposing factors of relevance to predict the course of such symptoms. A 49-item-questionnaire exploring symptoms of mental disease and gynaecological aberrations post partum was mailed to a representative selection of women delivered a t the Department of Obstetrics and Gynecology of Lund. The women investigated were delivered 3, 6, 9 and 12 months before the time of investigation. The sample consisted of 404 women, the answering percentage was 94,4 per cent. The psychiatric items were designed to map out symptoms of slight mental disorders, i.e. neurosis, asthenic reactions and mild affective disorders. As regards the psychiatric items we asked the subjects to state their condition a t the time of exploration in relation to the condition during the last year before pregnancy. The number of mental symptoms was correlated to a number of gynecological variables and background data, the latter obtained from the records of the maternity hospital. The number of psychiatric symptoms post partum was noteworthy high. About 25 per cent of the sample had seven or more symptoms, 20 per cent had 4 to 6, while the rest, about 55 per cent, had no symptoms at all or only a few ones. An interesting feature of the subjects with few mental symptoms was that the number of symptoms in them seemed t o diminish with increasing time from delivery. Such a trend was not found in the group with more than six symptoms. The background variables were to little avail in predicting psychiatric morbidity post partum. Abstract:The four engineering departments (electrical, mechanical, chemical and civil) at Rowan University (USA) rely extensively on semester-long project modules called "Clinics" to provide practical applications for coursework taught within the respective departments. The engineering clinics are typically team-taught by faculty from more than one discipline; the student project teams are also multidisciplinary. The focus is on product design and development, students and faculty bring skills acquired from their respective disciplines to bear on the design process. This paper describes a sophomore level clinic experience wherein students and faculty from the Electrical and Mechanical Engineering departments collaborated to design and develop a portable device that could be used to nondestructively inspect aircraft skin for cracks.
3D-TOF MRA in the Etiologic Diagnosis of Hemifacial Spasm and Trigeminal Neuralgia

Abstract:Objective To study the clinical significance of 3 dimensional time of flight magnetic resonance angiography(3D-TOF-MRA) for the pathogenesis of hemifacial spasm (HFS) and trigeminal neuralgia(TN).Methods 48 patients with HFS and 46 patients without HFS and 42 patients with TN and 40 patients without TN were examined by MRI and 3D-TOF-MRA by the enhancement of DTPA. Diagnosis of the presence of compressions in the root exit zone(REZ) of facial nerves and trigeminal nerves were done by two radiologists on an independent console. Results (1)In the patients, compression of the REZ of the facial nerves and trigeminal nerves were detected on 45 spastic sides (93.8%,neurovascular on 44 sides and tumor on 1 side) and 36 spastic sides ( 85.7%,neurovascular on 32 sides and tumor on 4 sides ), 8 and 4 on the asymptomatic sides (16.7% and 9.5%, all neurovascular ). In the controls, 4 and 5 sides ( 4.4% and 6.3% ) were found in the compression of the REZ of the facial nerves and trigeminal nerves. ( 2 ) The offending vessels of compression of the REZ of the facial nerves were the anterior inferior cerebellar artery (AICA) in 17 cases ( 38.6% ), the posterior inferior cerebellar artery (PICA) in 12 cases (27.3%), the vertebral artery (VA) in 6 cases (13.6%). The offending vessels of compression of the REZ of the trigeminal nerves were the superior cerebellar artery ( SCA ) in 18 cases ( 56.3% ), the anterior inferior cerebellar artery in 5 cases (15.6%), the difficult identified vessels (DIV) in 4 cases (12.5%). (3)The relative risks of microvascular compressions which cause HFS and TN were 26.6 and 9.84. (4) The compressions of the REZ of the facial nerves and trigeminal nerves were proved in 4 cases (neurovascular 3 cases and tumor 1 case) and 10 cases (neurovascular 6 cases and tumor 4 casee) in the operation.Conclusion MRI and enhanced 3D-TOF-MRA appeare to be the best imaging technology for the pathogenesis of HFS and TN now. The major causes of HFS and TN may be different neurovascular compressions in the REZ of the facial nerves and trigeminal nerves, some cases are caused by tumor compression.
Three-dimensional MRI of hemifacial spasm with surgical correlation

Abstract:MRI was used to investigate 100 patients with hemifacial spasm, using 3D-FT T2-weighted (CISS) and contrast-enhanced 3D-FT T1-weighted (turbo-FLASH) sequences in all cases. MR angiography was performed in 54 patients, using 3D-MT FISP images. Decompression of the facial nerve through a retromastoid craniotomy was performed in all patients. Hemifacial spasm caused by tumours in the cerebellopontine angle was not included. Vascular contact with the facial nerve root-exit zone or at the internal auditory canal was present in 96 of 100 patients with hemifacial spasm. The vessel responsible was the vertebral artery (VA) in 18 cases, the posterior inferior cerebellar artery (PICA) in 23, the anterior inferior cerebellar artery (AICA) in 22, the VA and PICA in 24, VA and AICA in 3, PICA and AICA in 1, VA, PICA and AICA in 4, and a vein in 1 case. CISS images showed compressive vascular loops better than contrast-enhanced turbo-FLASH images alone. The sensitivity of MRI was high, since only one false-negative case was found among the 100 patients who underwent surgery.

Abstract:BACKGROUND Exogenous and endogenous risk factors are involved in human carcinogenesis of the head and neck. Noteworthy hereditary factors include mutagen sensitivity and the individual's capacity for DNA repair. Repair mechanisms influence different phases of mutation and malignant transformation. The present study introduces a highly sensitive method for evaluating the repair capacity of human mucosal cells and lymphocytes. METHOD Human epithelia of the nose and peripheral lymphocytes were incubated with the tobacco-related carcinogen N'nitrosodiethylamine (NDEA). The solvent dimethylsulfoxide (DMSO) served as negative control. Following repair times of 0 min, 15 min and 30 min, the cells were subjected to a modified version of the alkaline microgel electrophoresis technique (Comet assay). The data were digitally analyzed after fluorescent staining. RESULTS Using the Comet assay, DNA repair could be quantified in human mucosal cells and in lymphocytes. The majority of DNA strand breaks induced by NDEA were repaired within 15 min in both cell types. CONCLUSIONS Up to now, the Comet assay has been the preferred method for demonstrating substance-induced DNA damage. It has been used in repair studies involving lymphocytes, bacterial systems and animal-derived cells. A modified version of this method, however, can be used to quantify DNA repair in human mucosal cells and peripheral lymphocytes targeted by carcinogens. It is thus possible to evaluate an endogenous factor involved in the development of malignant transformations in mucosal cells of the upper aerodigestive tract.

Abstract:Hemifacial spasm is a movement disorder characterized by involuntary contraction of unilateral facial muscles, usually starting with the orbicularis oculi muscle and progressing to the whole face.The vascular compression on the root exit zone of facial nerve is the most common cause of primary HFS.And the pathophysiological mechanism of the HFS may be the hyper excitability of the motor nucleus caused by the compression of the facial nerve, so it is particularly important to recognize the position between the facial nerve and offending vessels.Many new magnetic resonance imaging techniques have been applied to the identification and diagnosis of hemifacial spasm recently.The latest progresses in the field of magnetic resonance morphological imaging, resting-state functional magnetic resonance imaging (rs-fMRI), the voxel-based morphometry (VBM) and diffusion tensor imaging (DTI) were reviewed in this article.

Abstract:Purpose The specific neurovascular compression (NVC) event responsible for the symptomatic manifestation of hemifacial spasm (HFS) remains difficult to assess accurately using magnetic resonance imaging (MRI). We aim to evaluate the MRI characteristics of HFS. Method We retrospectively included patients with HFS and divided them into a test group (n = 186) and a validation group (n = 28). The presence, severity, and offending vessel type of NVC in each portion, and the orientation of the offending vessel around the facial nerve, were recorded. Conditional logistic regression analyses were performed to evaluate correlations using test group. The validation group was used to verify whether our findings improved diagnostic performance. Results Deformity in the proximal cisternal segment was significantly correlated with HFS occurrence (odds ratio [OR]: 256.58, p = .002), whereas contact was not (p = .233). Abstract:Background: High-resolution 3-dimensional (3-D) magnetic resonance imaging (MRI) is widely used to predict the neurovascular anatomy within the cerebellopontine angle. Objective: To assess the value of 3-D steady-state free precession imaging (SSFP) and time-of-flight magnetic resonance angiography (TOF MRA) in detecting the offending vessels in hemifacial spasm in comparison to intraoperative endoscopic visualization. Methods: 42 patients underwent endoscope-assisted microvascular decompression (MVD). All available preoperative 3-D SSFP and TOF MRA images were checked. Intraoperative videos were captured by a high-definition endoscopic camera attached to endoscopes while exploring the area of facial nerve root exit zone (REZ). Evaluation of the 3-D images was performed by 2 independent groups of observers and compared with the operative findings. Results: Three-D MRI had an average positive predictive value (PPV) of 89.1% in differentiating between simple and complex compression. Mean accuracy of the images in detection of the offending vessels was 83.3% and 77% according to the first and second groups of observers, respectively. Averaged inter-observer agreement between the 2 groups of observers was substantial, with an averaged Kappa coefficient (K) of 0.56. In the simple compression group, mean accuracy was 97% and 89.4% according to the first and second groups of observers, respectively. Averaged K for agreement was substantial (K = 0.65). Conclusion: According to endoscopic visualization, 3-D SSFP and TOF MRA images are accurate in detecting the offending vessels in simple compression of the facial nerve, and in predicting presence of a complex compression with variable sensitivity in identifying all offending vessels.
[Role of endoscopy and vascular decompression in the treatment of hemifacial spasm]

Abstract:Although it has long been hypothesized that hemifacial spasm could arise from a conflict between the nerve and the artery, it is very difficult to distinguish between a normal arterial loop and a pathological conflict leading to facial symptoms. Several new elements would help in the definition of the cause and allow less traumatic treatment of idiopathic hemifacial spasm. They include magnetic resonance imagery with CISS sequences, limited retrosigmoid approach, endoscopy of the cerebellopontine angle and monitoring the facial nerve. In our experience with 20 cases, retrosigmoid approach with a combined surgical and endoscopic procedure has led to total involution of the spasms in 80% of the cases.
[Hemifacial spasms and vertebral dolichoectasias]

Abstract:Introduction: Hemifacial spasm (HFS) is characterized by chronic twitching or spasm of one side of the face. It affects muscles activated by the facial nerve. HFS is believed to result from irritation of the facial nerve at its exit from the brainstem and most cases are associated with a small vascular loop compression. Rarely it is associated with a vertebral artery dolichoectasia. Case reports: We report three cases of HFS whose magnetic resonance imaging and magnetic resonance angiography revealed vertebral dolichoectatic arteries compressing the facial nerve and causing HFS. These cases were selected from the group of patients of the dystonic outpatient clinics, Neurology Department, between April 1st 1998 and December 30th 2007. Conclusions: Magnetic resonance imaging and magnetic resonance angiography are essential for the diagnosis of a vertebral dolichoectasia and should be considered the initial screening procedures in the assessment of patients with HFS. The role of these neuroradiologic studies in the diagnosis of HFS is not only to detect the vascular structures running adjacent to the root exit zone that might distort the facial nerve and compress the brain stem around the root exit zone, but also to exclude other causes in a single non-invasive examination.
Magnetic Resonance Imaging Assessment of Vascular Contact of the Facial Nerve in the Asymptomatic Patient

Abstract:Objective The objective of this study was to determine the prevalence of facial nerve vascular contact on magnetic resonance imaging (MRI) in patients without hemifacial spasm (HFS). Study Design Our radiology database was queried to identify consecutive adult patients without a history of HFS, intracranial tumor, brain radiation therapy, intracranial surgery, traumatic brain injury, or trigeminal nerve vascular compression. One hundred high-resolution MRIs of the posterior fossa were independently reviewed by two neuroradiologists for facial nerve vascular contact (200 sides). Main Outcome Measures The prevalence of vascular nerve contact in the non-HFS patient, the location of contact along the facial nerve, the culprit vessel, and severity of compression was recorded. Results The presence of vascular contact in the non-HFS patient may be as high as 53%. It is typically mild to moderate in severity, most commonly involves the cisternal portion, and usually caused by the anterior inferior cerebellar artery. Conclusion Vascular contact of the facial nerve is frequently identified in asymptomatic individuals but tends to be more peripheral and mild compared with previous descriptions of neurovascular contact in HFS patients. These results should be considered in assessing the candidacy of HFS patients for microvascular decompression.

Abstract:PurposeThe purpose of this study was to investigate and evaluate the accuracy and the preoperative diagnostic value of high-resolution magnetic resonance imaging (MRI) techniques, three-dimensional time-of-flight (3D-TOF) and three-dimensional constructive interference in steady state (3D-CISS) sequence, solely or in combination for the detection of the relationship between the facial nerve and adjacent vessels in patients with hemifacial spasm (HFS).MethodsA total of 95 patients with primary HFS were subject to 3D-TOF and 3D-CISS MRI. The MR images were then used to evaluate the anatomical neurovascular relationships, and the results were compared with the surgical findings. We categorized the neurovascular relationship into three types: compression, contact, and neighboring or distant. Compression and/or contacts between root exit zone (REZ) and vessels were defined as positive, whereas neighboring or distant was considered to be negative.Results3D-TOF combined with 3D-CISS assessment showed that 94 of 95 patients had artery compression or contact at REZ, whereas the remaining patient had compression at the peripheral branch of the facial nerve but not at REZ. The positive rates and the overall accuracy were 98.95 and 100 %, respectively, for the 3D-TOF combined with 3D-CISS assessment; 92.63 and 93.68 %, respectively, for the 3D-TOF assessment; and 85.26 and 86.32 %, respectively, for the 3D-CISS assessment. The positive rates and overall accuracy for the 3D-TOF combined with 3D-CISS assessment was significantly higher than those for the 3D-TOF or 3D-CISS assessment.ConclusionsCombination of 3D-TOF with 3D-CISS imaging well delineates the relationship between the facial nerve and adjacent vessels in terms of increased positive rates and accuracy.

Abstract:Bilateral hemifacial spasm and Meige syndrome can be easily confused due to their similar clinical manifestation. Here, we aimed to investigate the application of electrophysiological methods and magnetic resonance tomographic angiography (MRTA) in the differentiation between hemifacial spasm and Meige syndrome. 10 patients with bilateral hemifacial spasm and 9 patients with Meige syndrome received electrophysiological monitoring of nerves. There were two males and eight females with bilateral hemifacial spasm, aged 16–58 years with a course of 5–54 months. For the patients with Meige syndrome, there were three males and six females, aged 51–68 years with a course of 12–36 months. All patients received conventional MRTA of the brain blood vessels before decompression. We found that all patients with Meige syndrome showed synchronous contraction of bilateral orbicularis oculi muscles and (or) burst discharge from orbicularis oris muscles in surface electromyography (sEMG). However, those with hemifacial spasm presented with bilaterally asynchronous burst discharge. Electromyography for patients with Meige syndrome did not record abnormal muscle response (AMR), but recorded AMR for those with bilateral hemifacial spasm. The offending vessels were compressed in patients with hemifacial spasm in MRTA, while MRTA results were generally negative for those with Meige syndrome. Combining sEMG and AMR detection in EMG and MRTA, bilateral hemifacial spasm can be differentiated from Meige syndrome with a reduction of misdiagnosis rate.

Role of neuroimaging in cases of primary and secondary hemifacial spasm

Abstract:Purpose: The objective of this study was to analyze the role of neuroimaging and documenting various intracranial pathologies in primary and secondary hemifacial spasm. Methods: This retrospective study included patients with HFS who had undergone neuroimaging. The demographic profile, onset, progression, neuroimaging findings, and types of HFS were documented and analyzed. Results: A total of 202 patients (male = 110, female = 92) were included. The mean age of the study population was 51.81 ± 11.76 years. The right side was involved in 104 patients, the left side was involved in 97 patients and bilateral involvement was observed in one patient. Primary HFS: secondary HFS was 9.6:1. The mean age of onset of the spasms in the primary HFS group was 49.26 ± 8.35 years and in secondary HFS was 43.13 ± 12.12 years respectively. The anterior inferior cerebellar artery was the major vessel causing neurovascular conflict in primary HFS (n = 55). Facial nerve palsy was the most common cause (n = 13) of secondary HFS followed by cerebellopontine angle (CPA) tumors. Conclusion: The hemifacial spasm occurs mostly in the fifth decade of life. Primary HFS is more prevalent than secondary HFS. Clinical distinction between them is difficult. Neuroimaging is essential to detect the conflicting vasculature in cases of primary HFS and pathologies like CPA tumor, cyst, and aneurysms in cases of secondary HFS.
Feature Integration, Multi-image Queries and Relevance Feedback in Image Retrieval

Abstract:In this paper, we explore the effect of feature integration, multi-image queries, and relevance feedback in enhancing the performance of an image retrieval system. Weighted integration of structure, color and texture features is studied. In addition, we propose a methodology of retrieval consisting of multiple query images, as opposed to the traditionallyused model of a single query image. Two different mechanism of relevance feedback are also proposed and analyzed. Integration of features and feedback significantly improves the performance of the retrieval system.

Abstract:The diagnosis of primary hemifacial spasm (pHFS), due to a benign compression of the facial motor nerve by a vessel, within or close to its root exit zone, is often made with delay. Misdiagnosis includes psychogenic spasm, tics, facial myokymia or blepharospasm, but in fact post-facial palsy synkinesis (post-paralytic HFS) is the closest clinical condition, because it is limited to the territory of the facial nerve of a single hemiface. The differential diagnosis between these two entities, whose pathophysiological mechanisms are very different, can be made by electroneuromyographic (ENMG) examination. In addition, magnetic resonance imaging (MRI) is essential to show the offending vessel at the origin of pHFS and the absence of other causes of nerve compression. However, the diagnosis cannot be made on MRI basis alone, since a neurovascular conflict can be present in clinically asymptomatic subjects. This article reviews the clinical, MRI, and ENMG features in favour of a pHFS diagnosis as well as the various differential diagnoses of this involuntary facial movement disorder.
