Abstract:Background & Aim Donor-derived natural killer (NK) cells mediate graft-versus-leukemia effects low risk in inducing alloreactive graft-versus-host-disease and are used as component of T cell-depleted donor lymphocyte infusion (DLI) or purified. As clinical grade purification of NK cells requires two semi-automated separation procedures, including multiple manual hands-on steps, we developed an integrated fully automated process enabling either preparation of T cell-depleted DLI or clinical grade purification of NK cells within a single automated process on the CliniMACS Prodigy. Methods, Results & Conclusion The novel integrated CliniMACS Prodigy LP-3-56 System uses the CliniMACS Prodigy Tubing Set 320 for an fully automated procedure with integrated labeling and washing steps for both, CD3 and CD56 separation enabled through an integrated lytic rinsing sequence allowing subsequent CD56 enrichment in the same tubing set. By users choice the depletion of CD3+ cells will be performed with or without subsequent enrichment of CD56+ cells. For both strategies target cells are prepared with user-chosen formulation solution (e. g. sodium chloride or NK MACS medium). The new application software effectively depletes 9.6 × 109 CD3+ cells, and enriches 4.5 × 109 CD56+ cells from up to 40 × 109 total white blood cells. Inhouse evaluation runs with leukapheresis products (n= 29 in total, n=17 with CD3 depletion, n=12 with CD3 depletion and CD56 enrichment) were performed. After CD3 depletion, mean log depletion of T cells was 4.0. The mean recovery of NK cells was 89% with an average NK:T ratio-increase of 9509 fold. After CD3 depletion and CD56 enrichment, mean log depletion of T and B cells was 4.2 and 2.7, respectively. Purity of NK cells was 90.7% on average with a mean recovery of 49%. The majority of non-NK cells after the process were monocytes with a mean frequency of 5.9%. Functionality of purified NK cells was proven by their ability to lyse AML cells, to secrete cytokines after co-culture, and to perform antibody dependent cell-mediated cytotoxicity (ADCC) against Raji cells, using human CD20 antibody. In , the integrated fully automated CliniMACS Prodigy LP-3-56 System is capable of efficiently deplete CD3+ cells with or without subsequent CD56 enrichment to purify functional NK cells from leukapheresis products and provides target cells in formulation solution. The performance verification and submission to an European notified body for CE mark are the important next steps.

Projected quasiparticle theory for molecular electronic structure.

Abstract:We derive and implement symmetry-projected Hartree-Fock-Bogoliubov (HFB) equations and apply them to the molecular electronic structure problem. All symmetries (particle number, spin, spatial, and complex conjugation) are deliberately broken and restored in a self-consistent variation-after-projection approach. We show that the resulting method yields a comprehensive black-box treatment of static correlations with effective one-electron (mean-field) computational cost. The ensuing wave function is of multireference character and permeates the entire Hilbert space of the problem. The energy expression is different from regular HFB theory but remains a functional of an independent quasiparticle density matrix. All reduced density matrices are expressible as an integration of transition density matrices over a gauge grid. We present several proof-of-principle examples demonstrating the compelling power of projected quasiparticle theory for quantum chemistry.

Abstract:A method is described for the preparation of human peripheral blood mononuclear cell (PBMC) suspensions containing highly enriched natural killer (NK) cell cytotoxicity. The technique involved the negative selection of OKM1+ cells by the selective removal of nylon wool nonadherent PBMC which are reactive with the Leu-1 monoclonal antibody. The Leu-1+ cells are removed by subsequent rosette formation with anti-mouse IgG coated bovine erythrocytes. The resultant OKM1+ cell suspension had a greater number of large granular lymphocytes, K562 target binding effector cells, and lytic activity than concomitantly prepared fractions of Percoll gradients.
[Method for expansion in vitro of CD3-CD56+CD16+NK cells highly purified from human peripheral blood]

Abstract:The aim of this study was to establish an efficient method for expansion in vitro of natural killer (NK) cells highly purified from human peripheral blood. The CD3-CD56+CD16+ NK cells purified by the negative sorting method of MACS (magnetic microbeads activated cells sorting) were expanded with the different combinations of IL-2, SCF, IL-15 in SCGM (stem cell growth medium) supplemented with 10% human AB serum for 18 days. Cultures were fed with fresh medium and cytokines every 3 days. The sum of cells was counted for evaluating the efficiency of expansion. Then the purity of the CD3-CD56+CD16+ NK cells were determined by flow cytometry and the cytotoxicity to K562 targets was detected by CCK-8 assay in the end. Furthermore, the same way was used to explore the relationship between the efficiency of expansion, cytotoxicity to K562 targets of NK cells and the dose of IL-2. The results showed that after peripheral blood mononuclear cells (PBMNC) were purified by the negative sorting method of MACS, the purity of CD3-CD56+CD16+ NK cells increased from (12.70±2.66)% to (93.03±1.72)%. The CD3-CD56+CD16+ NK cells purified by MACS were expanded with the different combinations of IL-2, SCF, IL-15 in SCGM supplemented with 10% human AB serum for 18 days. The expanding multiple of IL-2/IL-15/SCF group was significantly higher than other groups (p<0.05). The purity of NK cells in the groups with cytokines was not significantly lower than that before expansion (p>0.05). The cytotoxicity of the groups with cytokines was significantly higher than that before expansion. Especially, the cytotoxicity (%) of NK cells in IL-2/IL-15 group and IL-2/IL-15/SCF group was more than 90%. The expanding multiples of low-dose group, medium-dose group and high-dose group were significantly higher than that of zero-dose group (p<0.05), but no significant difference was found between themselves (p>0.05). The cytotoxicity of the groups with IL-2 was significantly higher than that before expansion. Cytotoxicity to K562 cells in high-dose group was significantly higher than that in others (p<0.05); there was no significant difference between low-dose group and medium-dose group (p>0.05). It is concluded that cytokines in the 4 groups were efficient for expansion and the cytotoxicity of highly purified NK cells in vitro. IL-2/SCF/IL-15 combination is the most efficient one among different combinations, and enhanced significantly the cytotoxicity of NK cells against K562 targets. The efficiency of expansion and the cytotoxicity in vitro of NK cells are not related with the dose of IL-2, when IL-2<1,000 U/ml. It is indicated that IL-2 of high-dose (≥1,000 U/ml) may enhance the cytotoxicity of NK cells in vitro more efficiently.
[Ex vivo expansion of highly purified NK cells from human peripheral blood]

Abstract:Adoptive immunotherapy using allogeneic natural killer (NK) cells provides to be useful in recipients after allogeneic hematopoietic stem cell transplantation (Allo-HSCT), but its application has been limited by the inability to obtain sufficient numbers of pure NK cells. This study was aimed to optimize the expansion of high purity NK cells from human peripheral blood. First, the NK cells were isolated from PBMNC by using miniMACS (magnetic cell-selection) and NK Cell Isolation Kit II. Then the isolated cells were cultured in SCEM (Stemline Hematopoietic Stem Cell Expansion Medium, Sigma) supplemented with 10% human AB serum and different combinations of IL-2 and/or IL-12, IL-15 for 15 days. Cultures were fed with fresh media and cytokines every 3 days, and were evaluated for cell expansion, phenotype, and cytotoxicity at the end of the culture period. The results showed that in group IL2 + IL15 and IL2 + IL15 + IL12, cells were expanded 50.46 +/- 4.31 and 52.35 +/- 6.72-fold respectively, much higher than others (P<0.01), but no significant difference between themselves (P>0.05). And the purity of CD3(-)CD56(+) NK cells was over 94% in all groups except the control. The cytotoxicity of expanded NK cells cultured with cytokines was significantly higher than the starting population at different E:T ratio (P<0.01), although the cytotoxicity of IL2 + IL15 + IL12 group was slightly higher than that of IL2 + IL15 group, but no significant difference between themselves (P>0.05). It is concluded that high purity of NK cells can be efficiently expanded in culture with IL2 + IL15.

Abstract:Background: Immunotherapy with NK cells has been limited by the inability to obtain sufficient numbers of pure NK cells suitable for manipulation and expansion. The goal of this study was to isolate CD56(+) cells (CD3(-)/CD56(+), CD3(+)/CD56(+)) and expand them under culture conditions compliant with current good manufacturing practices. Methods: Magnetic cell-selection technology, using paramagnetic CD56 microbeads and cell selection columns, was used to isolate a CD56(+) population containing both CD3(-)/56(+) NK (60.6+/-10.8%) and CD3(+)/56(+) NK T cells (30.4+/-8.6%) to initiate the expansion studies. The isolated CD56(+) cells were cultured in X-Vivo10 serum-free media supplemented with 10% human AB serum and 500 U/mL recombinant human IL-2 or 500 U/mL IL-2 plus 10 ng/mL recombinant human IL-15 for 14 days. Cultures were fed fresh media and cytokines every 3-4 days, and were evaluated for cell expansion, phenotype, and cytotoxicity at the end of the culture period. Results: Significant expansion of CD56 cells occurred only during the second week of culture. Although an average of two log expansions was observed, there was substantial cell-expansion variability, depending on the donor, and even when the same donor was tested on different occasions. The cytotoxicity of selected and expanded CD56(+) cells at a low E:T ratio was significantly higher than the starting population, but was comparable to non-separated PBMC expanded for 2 weeks under the same conditions. IL-15 (in combination with IL-2) induced higher killing at the 1:1 E:T ratio than IL-2 alone. Since CD3 cells were not depleted upfront, the expansion of CD3(+)CD56(+) cells was 2-3 times that of CD3(-)CD56(+) cells. NK cells that express the FcgammaRIII (CD16) can mediate Ab-dependent cellular cytotoxicity, and can contribute to enhanced efficacy of MAb treatment. Under the given culture conditions, only moderate expansion of CD56(+)/CD3(-)/CD16(+) cells occurred, with the majority of cells being CD56(+)/CD3(+)/CD16(+) cells. Discussion: Our studies suggest that the positive magnetic cell-separation method provides a good basis for obtaining enriched CD56(+) cells but expansion conditions need to be optimized.

Abstract:Natural killer (NK) cells hold promise in cancer treatment due to their ability to spontaneously lyse cancer cells. For clinical use, high quantities of pure, functional NK cells are necessary. Combining adherence-based isolation with specialized media showed the unreliability of the isolation method, but demonstrated the superiority of the NK MACS® medium, particularly in suboptimal conditions. Neither human pooled serum, fetal calf serum (FCS), human platelet lysate, nor chemically defined serum replacement could substitute human AB serum. Interleukin (IL-)2, IL-15, IL-21, and combined CD2/NKp46 stimulation were assessed. IL-21 and CD2/NKp46 stimulation increased cytotoxicity, but reduced NK cell proliferation. IL-15 stimulation alone achieved the highest proliferation, but the more affordable IL-2 performed similarly. The RosetteSep™ human NK cell enrichment kit was effective for isolation, but the presence of peripheral blood mononuclear cells (PBMCs) in the culture enhanced NK cell proliferation, despite similar expression levels of CD16, NKp46, NKG2D, and ICAM-1. In line with this, purified NK cells cultured in NK MACS® medium with human AB serum and IL-2 demonstrated high cytotoxicity against primary glioblastoma stem cells.

Abstract:Summary: To test the efficacy of a sustainable community-level HIV intervention among sex workers, the Sonagachi Project was replicated, including community organizing and advocacy, peer education, condom social marketing, and establishment of a health clinic. Sex workers were randomly selected in 2 small urban communities in northeastern India (n = 100 each) and assessed every 5–6 months over 15 months (85% retention). Overall condom use increased significantly in the intervention community (39%) compared with the control community (11%), and the proportion of consistent condom users increased 25% in the intervention community compared with a 16% decrease in the control community. This study supports the efficacy of the Sonagachi model intervention in increasing condom use and maintaining low HIV prevalence among sex workers.

Abstract:OBJECTIVE: To study the method of expansion of CD3-、CD56 + NK cells from peripheral blood mononuclear cells(PBMCs). METHOD: PBMCs were cultured in stem cell growth medium (SCGM) supplemented with monoclonal anti-CD3 antibodies and IL-2 with different concentrations; The immune phenotypes of expanded cells were analyzed by Flow cytometry (ECM). RESULT: After being activated by monoclonal anti-CD3 antibodies and IL-2, PBMCs got the best expansion and expanded to 56. 2±5. 2 folds in 14th day after incubation, and contained (52.4±7.9)% CD3-、CD56+ NK cells. CONCLUSION: NK cells could be expanded from PBMCs using SCGM with the synergistic induction by monoclonal anti-CD3 antibodies and IL-2. This work provided a simple and effective method of expanding and enriching human NK cells for the adoptive immunotherapy of malignant tumors

Abstract:Techniques for CD34+ cell enrichment of hematopoietic progenitor cells in grafts destined for transplantation of certain patients with the aim of lowering the amount of infused T lymphocytes and subsequently decreasing the risk of graft versus host disease (GVHD) have been well developed. Adaptations of these techniques should be useful for isolation of other phenotypically defined stem cells. However, a major limitation of techniques now available consists of the number of total nucleated cells or phenotypically defined stem cells that can be processed in a single procedure. Here, we show that recommended levels are much lower than the levels of cells that can be effectively processed by immunomagnetic sorting. Twenty-nine procedures were performed using the Clini- MACS (Miltenyi Biotec) device, which is recommended for processing <6x10(10 )total nucleated cells or <6x10(8) CD34+ cells. Procedures were divided in groups according to their total cellular or CD34+ cell content. We achieved a median CD34+ cell recovery of 68.60% with a median purity of 98.56%, regardless of the loading dose when samples possessing 2-10x10(10) total nucleated cells and 0.8-12.5x10(8) CD34+ cells were applied to a single column. The median levels of CD3+ cells and CD19+ cells in the final product were depleted by 5 logs and 3.8 logs, respectively; no differences were noted when the initial loading dose was increased. Moreover, we found no correlation between the total number nucleated or CD34+ cells loaded and the resultant CD34+ cell recovery. In conclusion, levels of both total nucleated cells and CD34+ can be processed in a single procedure with satisfactory and similar CD34+ cell recovery when these columns are loaded with up to two times as many cells as recommended.

Abstract:Background aims: Interest in natural killer (NK) cell-based immunotherapy has resurged since new protocols for the purification and expansion of large numbers of clinical-grade cells have become available. Methods: We have successfully adapted a previously described NK expansion method that uses K562 cells expressing interleukin (IL)-15 and 4-1 BB Ligand (BBL) (K562-mb15-41BBL) to grow NK cells in novel gas-permeable static cell culture flasks (G-Rex). Results: Using this system we produced up to 19 × 10(9) functional NK cells from unseparated apheresis products, starting with 15 × 10(7) CD3(-) CD56 (+) NK cells, within 8-10 days of culture. The G-Rex yielded a higher fold expansion of NK cells than conventional gas-permeable bags and required no cell manipulation or feeding during the culture period. We also showed that K562-mb15-41BBL cells up-regulated surface HLA class I antigen expression upon stimulation with the supernatants from NK cultures and stimulated alloreactive CD8 (+) T cells within the NK cultures. However, these CD3 (+) T cells could be removed successfully using the CliniMACS system. We describe our optimized NK cell cryopreservation method and show that the NK cells are viable and functional even after 12 months of cryopreservation. Conclusions: We have successfully developed a static culture protocol for large-scale expansion of NK cells in the gas permeable G-Rex system under good manufacturing practice (GMP) conditions. This strategy is currently being used to produce NK cells for cancer immunotherapy.

Abstract:In human sepsis, myelocytosis and concomitant lymphopenia complicate the study of peripheral blood natural killer (NK) cells. Here, we present a protocol for isolating NK cells from peripheral blood of septic patients using magnetic cell separation. We describe steps for the depletion of non-NK cells and NK cell enrichment. We then detail procedures for comparing the results from this protocol to results obtained through the isolation procedures using two commercially available kits for NK cell isolation. Abstract:Background aims: This study developed a new method to expand CD3(-)CD56(+) natural killer (NK) cells from human peripheral blood mononuclear cells (PBMCs) without feeder cells for clinical trials. Methods: PBMCs from healthy subjects were co-stimulated with anti-CD3 and anti-CD52 monoclonal antibodies and cultured for 14 days in newly developed NKGM-1 medium containing autologous plasma and interleukin-2. Expanded NK cells were examined for cell number, phenotype, in vitro and in vivo cytotoxicity and interferon (IFN)-γ secretion. We also evaluated the proliferative ability of NK cells after cryopreservation. A patient with advanced pancreatic cancer was treated with autologous-expanded NK cells through the use of this method in combination with chemotherapy. Results: Expanded NK cells expressed higher levels of activating molecules compared with resting NK cells and exhibited potent cytotoxicity against K562 cells and IFN-γ secretion by cytokine stimulation. Significant anti-tumor activity was observed in immunodeficient mice injected with the human pancreatic cancer cell line BxPC-3. Large-scale cultures generated a median 5.7 × 10(9) NK cells from 20 mL of peripheral blood (n = 38) after 14 days of culture and 8.4 × 10(9) NK cells after 18 days of culture through the use of a cryopreservation procedure. The number of NK cells and cytotoxic activity in the peripheral blood of the patient with pancreatic cancer greatly increased, and successful clinical responses were observed after multiple infusions of expanded NK cells. Conclusions: These data demonstrate that this simple and safe methodology for the ex vivo expansion of NK cells can be used for cancer immunotherapy.

Abstract:CAROL RAVIOLA, MD: * The patient we would like to discuss today is a 63-year-old woman who was admitted 12 hours ago, having been injured when her runaway automobile crushed her against the door of her garage. She was admitted within thirty minutes of injury and was in shock with poor peripheral skin perfusion, and a blood pressure of 80/60 mm of mercury. The injuries appeared to be limited to the lower abdomen and pelvis. Her abdomen was diffusely tender and the pelvis was grossly unstable. Findings on x-ray studies included a fracture of the right iliac crest and superior and inferior pubic rami and a right sacroilic separation. On laparotomy, one hour after admission, a large retroperitoneal hematoma was noted, which filled the pelvis and extended upward on the right nearly to the diaphragm. This was not entered. After thorough exploration of the abdominal cavity showed no additional injuries, the abdomen was closed. The patient received 5 units of blood during the induction of anesthesia and the surgical procedure. Immediately after operation, peripheral perfusion was poor. The patient's toes appeared blue and mottled, the blood pressure was 140/90 mm of mercury and the pulse rate was 100 beats per minute. Central venous pressure was 10 cm of

Abstract:Adoptive natural killer (NK) cell therapy relies on the acquisition of large numbers of mature and functional NK cells. An option for future immunotherapy treatments is to use large amounts of NK cells derived and differentiated from umbilical cord blood (UCB) CD34+ hematopoietic stem cells (HSCs), mainly because UCB is one of the most accessible HSC sources. In our study, we compared the potential of two stromal cell lines, OP9 and M2-10B4, for in vitro generation of mature and functional CD56+ NK cells from UCB CD34+ HSC. We generated higher number of CD56+ NK cells in the presence of the OP9 cell line than when they were generated in the presence of M2-10B4 cells. Furthermore, higher frequency of CD56+ NK cells was achieved earlier when cultures were performed with the OP9 cells than with the M2-10B4 cells. Additionally, we studied in detail the maturation stages of CD56+ NK cells during the in vitro differentiation process. Our data show that by using both stromal cell lines, CD34+ HSC in vitro differentiated into the terminal stages 4-5 of maturation resembled the in vivo differentiation pattern of human NK cells. Higher frequencies of more mature NK cells were reached earlier by using OP9 cell line than M2-10B4 cells. Alternatively, we observed that our in vitro NK cells expressed similar levels of granzyme B and perforin, and there were no significant differences between cultures performed in the presence of OP9 cell line or M2-10B4 cell line. Likewise, degranulation and cytotoxic activity against K562 target cells were very similar in both culture conditions. The results presented here provide an optimal strategy to generate high numbers of mature and functional NK cells in vitro, and point toward the use of the OP9 stromal cell line to accelerate the culture procedure to obtain them. Furthermore, this method could establish the basis for the generation of mature NK cells ready for cancer immunotherapy.

Suppression of natural killer cells in the presence of CD34+ blood progenitor cells and peripheral blood lymphocytes

Abstract:Abstract Mobilization of CD34 + peripheral blood progenitor cells (PBPCs) with granulocyte-colony stimulating factor (G-CSF) may induce functional alterations in peripheral blood lymphocyte (PBL) subsets. We and others have shown that natural killer (NK) cells from PBPC collections are less expandable in vitro than those obtained during steady-state hematopoiesis. We show here that the extent of this proliferation deficit is related to the number of circulating CD34 + cells in vivo at the time of PBPC apheresis. Likewise, addition of autologous CD34 + cells to unseparated PBL reduced the expansion of the NK-cell subset by 22.2% +/- 6.0% (n = 10; P <.005). In contrast, when using purified NK cells, their proliferation remained unimpaired by autologous CD34 + cells. Supernatants from CD34 + cells cultured with autologous PBLs had an inhibitory effect on proliferation of purified NK cells (n = 16; P =.03), indicating that an interaction between CD34 + cells and lymphocytes is essential for the suppressive effect on NK cells. To investigate the role of T cells in this interaction, intracellular cytokines were determined in T cells cultured for 7 days with or without autologous CD34 + cells. When cultured with CD34 + cells, the frequency of IL-2-producing CD4 + and CD8 + T cells was reduced by 19% and 24%, respectively, compared with T cells cultured alone (n = 7; P =.016). Interferon-gamma-producing T cells were slightly reduced ( P = not statistically significant [ns]). Finally, the influence of T cells and NK cells on the recovery of myeloid colony-forming cells (CFU-GMs) from purified CD34 + cells was examined. In the presence of T cells, 16% +/- 6% of the input CFU-GM recovered after 7 days, compared with 5% +/- 4% in the presence of NK cells (n = 5; P = ns). Our findings point to an inhibition of NK-cell proliferation mediated by an interaction of CD34 + cells and T cells occurring during PBPC mobilization with G-CSF.

Abstract:Graft-versus-host disease (GVHD) is a major risk of the administration of allogeneic chimeric antigen receptor (CAR)-redirected T cells to patients who are HLA unmatched. Gene editing can be used to disrupt potentially alloreactive T-cell receptors (TCRs) in CAR T cells and reduce the risk of GVHD. Despite the high knockout rates achieved with the optimized methods, a subsequent purification step is necessary to obtain a safe allogeneic product. To date, magnetic cell separation (MACS) has been the gold standard for purifying TCRα/β- CAR T cells, but product purity can still be insufficient to prevent GVHD. We developed a novel and highly efficient approach to eliminate residual TCR/CD3+ T cells after TCRα constant (TRAC) gene editing by adding a genetically modified CD3-specific CAR NK-92 cell line during ex vivo expansion. Two consecutive cocultures with irradiated, short-lived, CAR NK-92 cells allowed for the production of TCR- CAR T cells with <0.01% TCR+ T cells, marking a 45-fold reduction of TCR+ cells compared with MACS purification. Through an NK-92 cell-mediated feeder effect and circumventing MACS-associated cell loss, our approach increased the total TCR- CAR T-cell yield approximately threefold while retaining cytotoxic activity and a favorable T-cell phenotype. Scaling in a semiclosed G-Rex bioreactor device provides a proof-of-principle for large-batch manufacturing, allowing for an improved cost-per-dose ratio. Overall, this cell-mediated purification method has the potential to advance the production process of safe off-the-shelf CAR T cells for clinical applications.

Abstract:Natural killer (NK) cells have been used in several clinical trials as adaptive immunotherapy. The low numbers of these cells in peripheral blood mononuclear cells (PBMC) have resulted in various approaches to preferentially expand primary NK cells from PBMC. While some clinical trials have used the addition of interleukin 2 (IL-2) to co-stimulate the expansion of purified NK cells from allogeneic donors, recent studies have shown promising results in achieving in vitro expansion of NK cells to large numbers for adoptive immunotherapy. NK cell expansion requires multiple cell signals for survival, proliferation and activation. Thus, expansion strategies have been focused either to substitute these factors using autologous feeder cells or to use genetically modified allogeneic feeder cells. Recent developments in the clinical use of genetically modified NK cell lines with chimeric antigen receptors, the development of expansion protocols for the clinical use of NK cell from human embryonic stem cells and induced pluripotent stem cells are challenging improvements for NK cell-based immunotherapy. Transfer of several of these protocols to clinical-grade production of NK cells necessitates adaptation of good manufacturing practice conditions, and the development of freezing conditions to establish NK cell stocks will require some effort and, however, should enhance the therapeutic options of NK cells in clinical medicine.
