Abstract:Randomized controlled trials (RCTs) usually enroll heterogeneous study population, and thus it is interesting to identify subgroups of patients for whom the treatment may be beneficial or harmful. A variety of methods have been developed to do such kind of post hoc analyses. Conventional generalized linear model is able to include prognostic variables as a main effect and predictive variables in an interaction with treatment variable. A statistically significant and large interaction effect usually indicates potential subgroups that may have different responses to the treatment. However, the conventional regression method requires to specify the interaction term, which requires knowledge of predictive variables or becomes infeasible when there is a large number of feature variables. The Least Absolute Shrinkage and Selection Operator (LASSO) method does variable selection by shrinking less clear effects (including interaction effects) to zero and in this way selects only certain variables and interactions for the model. There are many tree-based methods for subgroup identification. For example, model-based recursive partitioning incorporates parametric models such as generalized linear models into trees. The model incorporated is usually a simple model with only the treatment as covariate. Predictive and prognostic variables are found and incorporated automatically via the tree. The present article gives an overview of these methods and explains how to perform them using the free software environment for statistical computing R (version 3.3.2). A simulated dataset is employed for illustrating the performance of these methods.
Evaluating the Validity of Post-Hoc Subgroup Inferences: A Case Study

Abstract:In randomized experiments, the random assignment of units to treatment groups justifies many of the widely used traditional analysis methods for evaluating causal effects. Specifying subgroups of units for further examination after observing outcomes, however, may partially nullify any advantages of randomized assignment when data are analyzed naively. Some previous statistical literature has treated all post-hoc subgroup analyses homogeneously as entirely invalid and thus uninterpretable. The extent of the validity of such analyses and the factors that affect the degree of validity remain largely unstudied. Here, we describe a recent pharmaceutical case with First Amendment legal implications, in which post-hoc subgroup analyses played a pivotal and controversial role. Through Monte Carlo simulation, we show that post-hoc results that seem highly significant make dramatic movements toward insignificance after accounting for the subgrouping procedure presumably used. Finally, we propose a novel, randomization-based method that generates valid post-hoc subgroup p-values, provided we know exactly how the subgroups were constructed. If we do not know the exact subgrouping procedure, our method may still place helpful bounds on the significance level of estimated effects. This randomization-based approach allows us to evaluate causal effects in situations where valid evaluations were previously considered impossible.

Abstract:Subgroup analyses are commonly performed in the clinical trial setting with the purpose of illustrating that the treatment effect was consistent across different patient characteristics or identifying characteristics that should be targeted for treatment. There are statistical issues involved in performing subgroup analyses, however. These have been given considerable attention in the literature for analyses where subgroups are defined by a pre-randomization feature. Although subgroup analyses are often performed with subgroups defined by a post-randomization feature—including analyses that estimate the treatment effect among compliers—discussion of these analyses has been neglected in the clinical literature. Such analyses pose a high risk of presenting biased descriptions of treatment effects. We summarize the challenges of doing all types of subgroup analyses described in the literature. In particular, we emphasize issues with post-randomization subgroup analyses. Finally, we provide guidelines on how to proceed across the spectrum of subgroup analyses.

Abstract:Subgroup analysis is an integral part of comparative analysis where assessing the treatment effect on a response is of central interest. Its goal is to determine the heterogeneity of the treatment effect across subpopulations. In this paper, we adapt the idea of recursive partitioning and introduce an interaction tree (IT) procedure to conduct subgroup analysis. The IT procedure automatically facilitates a number of objectively defined subgroups, in some of which the treatment effect is found prominent while in others the treatment has a negligible or even negative effect. The standard CART (Breiman et al., 1984) methodology is inherited to construct the tree structure. Also, in order to extract factors that contribute to the heterogeneity of the treatment effect, variable importance measure is made available via random forests of the interaction trees. Both simulated experiments and analysis of census wage data are presented for illustration.

Abstract:When it is suspected that the treatment effect may only be strong for certain subpopulations, identifying the baseline covariate profiles of subgroups who benefit from such a treatment is of key importance. In this paper, we propose an approach for subgroup analysis by firstly introducing Bernoulli-gated hierarchical mixtures of experts (BHME), a binary-tree structured model to explore heterogeneity of the underlying distribution. We show identifiability of the BHME model and develop an EM-based maximum likelihood method for optimization. The algorithm automatically determines a partition structure with optimal prediction but possibly suboptimal in identifying treatment effect heterogeneity. We then suggest a testing-based postscreening step to further capture effect heterogeneity. Simulation results show that our approach outperforms competing methods on discovery of differential treatment effects and other related metrics. We finally apply the proposed approach to a real dataset from the Tennessee's Student/Teacher Achievement Ratio project.

Abstract:Case-mix is common in clinical trials and treatment effect can vary across different subgroups. Conventionally, a subgroup analysis is performed by dividing the overall study population by one or two grouping variables. It is usually impossible to explore complex high-order intersections among confounding variables. Latent class analysis (LCA) provides a framework to identify latent classes by observed manifest variables. Distal clinical outcomes and treatment effect can be different across these classes. This paper provides a step-by-step tutorial on how to perform LCA with R. A simulated dataset is generated to illustrate the process. In the example, the classify-analyze approach is employed to explore the differential treatment effects on distal outcomes across latent classes.

Abstract:The overall goal of this study is to introduce latent class analysis (LCA) as an alternative approach to latent subgroup analysis. Traditionally, subgroup analysis aims to determine whether individuals respond differently to a treatment based on one or more measured characteristics. LCA provides a way to identify a small set of underlying subgroups characterized by multiple dimensions which could, in turn, be used to examine differential treatment effects. This approach can help to address methodological challenges that arise in subgroup analysis, including a high Type I error rate, low statistical power, and limitations in examining higher-order interactions. An empirical example draws on N = 1,900 adolescents from the National Longitudinal Survey of Adolescent Health. Six characteristics (household poverty, single-parent status, peer cigarette use, peer alcohol use, neighborhood unemployment, and neighborhood poverty) are used to identify five latent subgroups: Low Risk, Peer Risk, Economic Risk, Household & Peer Risk, and Multi-Contextual Risk. Two approaches for examining differential treatment effects are demonstrated using a simulated outcome: 1) a classify-analyze approach and, 2) a model-based approach based on a reparameterization of the LCA with covariates model. Such approaches can facilitate targeting future intervention resources to subgroups that promise to show the maximum treatment response.

Abstract:Non-significant randomized control trials can hide subgroups of good responders to experimental drugs, thus hindering subsequent development. Identifying such heterogeneous treatment effects is key for precision medicine and many post-hoc analysis methods have been developed for that purpose. While several benchmarks have been carried out to identify the strengths and weaknesses of these methods, notably for binary and continuous endpoints, similar systematic empirical evaluation of subgroup analysis for time-to-event endpoints are lacking. This work aims to fill this gap by evaluating several subgroup analysis algorithms in the context of time-to-event outcomes, by means of three different research questions: Is there heterogeneity? What are the biomarkers responsible for such heterogeneity? Who are the good responders to treatment? In this context, we propose a new synthetic and semi-synthetic data generation process that allows one to explore a wide range of heterogeneity scenarios with precise control on the level of heterogeneity. We provide an open source Python package, available on Github, containing our generation process and our comprehensive benchmark framework. We hope this package will be useful to the research community for future investigations of heterogeneity of treatment effects and subgroup analysis methods benchmarking.

Abstract:Subgroup analysis may be used to investigate treatment effect heterogeneity among subsets of the study population defined by baseline characteristics. Several methodologies have been proposed in recent years and with these, statistical issues such as multiplicity, complexity, and selection bias have been widely discussed. Some methods adjust for one or more of these issues; however, few of them discuss or consider the stability of the subgroup assignments. We propose exploring the stability of subgroups as a sensitivity analysis step for stratified medicine to assess the robustness of the identified subgroups besides identifying possible factors that may drive this instability. After applying Bayesian credible subgroups, a nonparametric bootstrap can be used to assess stability at subgroup‐level and patient‐level. Our findings illustrate that when the treatment effect is small or not so evident, patients are more likely to switch to different subgroups (jumpers) across bootstrap resamples. In contrast, when the treatment effect is large or extremely convincing, patients generally remain in the same subgroup. While the proposed subgroup stability method is illustrated through Bayesian credible subgroups method on time–to–event data, this general approach can be used with other subgroup identification methods and endpoints.

Abstract:Abstract Methods for extending – generalizing or transporting – inferences from a randomized trial to a target population involve conditioning on a large set of covariates that is sufficient for rendering the randomized and non-randomized groups exchangeable. Yet, decision-makers are often interested in examining treatment effects in subgroups of the target population defined in terms of only a few discrete covariates. Here, we propose methods for estimating subgroup-specific potential outcome means and average treatment effects in generalizability and transportability analyses, using outcome model-based (g-formula), weighting, and augmented weighting estimators. We consider estimating subgroup-specific average treatment effects in the target population and its non-randomized subset, and provide methods that are appropriate both for nested and non-nested trial designs. As an illustration, we apply the methods to data from the Coronary Artery Surgery Study to compare the effect of surgery plus medical therapy versus medical therapy alone for chronic coronary artery disease in subgroups defined by history of myocardial infarction.

Abstract:Heterogeneity exists in populations, and people may benefit differently from the same treatments or services. Correctly identifying subgroups corresponding to outcomes such as treatment response plays an important role in data-based decision making. As few discussions exist on subgroup analysis with measurement error, we propose a new estimation method to consider these two components simultaneously under the linear regression model. First, we develop an objective function based on unbiased estimating equations with two repeated measurements and a concave penalty on pairwise differences between coefficients. The proposed method can identify subgroups and estimate coefficients simultaneously when considering measurement error. Second, we derive an algorithm based on the alternating direction method of multipliers algorithm and demonstrate its convergence. Third, we prove that the proposed estimators are consistent and asymptotically normal. The performance and asymptotic properties of the proposed method are evaluated through simulation studies. Finally, we apply our method to data from the Lifestyle Education for Activity and Nutrition study and identify two subgroups, of which one has a significant treatment effect.

Abstract:By ‘partially post‐hoc’ subgroup analyses, we mean analyses that compare existing data from a randomized experiment—from which a subgroup specification is derived—to new, subgroup‐only experimental data. We describe a motivating example in which partially post hoc subgroup analyses instigated statistical debate about a medical device's efficacy. We clarify the source of such analyses' invalidity and then propose a randomization‐based approach for generating valid posterior predictive p‐values for such partially post hoc subgroups. Lastly, we investigate the approach's operating characteristics in a simple illustrative setting through a series of simulations, showing that it can have desirable properties under both null and alternative hypotheses. Abstract:Heterogeneity of treatment effects due to heterogeneous patient characteristics often arises in clinical trials. Subgroup analysis and the analysis of interactions are the most common approaches for evaluating such heterogeneous effects but do not explicitly address multiplicity issues. Another common challenge of analyzing treatment effect heterogeneity is the large number of possible covariates which inevitably causes problems related to multiplicity and lack of power. In this article, we develop a Bayesian credible subgroups method using continuous shrinkage priors to assess heterogeneity in treatment effects and multiplicity-adjusted benefiting subgroup identification for zero-inflated count data, which are often encountered in medical and public health studies. Our proposed method provides two bounding subgroups for the true benefiting subgroup: one that is probably contained by the true benefiting subgroup and one that probably contains the true benefiting subgroup. A simulation study has been conducted to compare the performance of the proposed method with other methods through frequentist properties. We apply our method to a clinical bladder tumor trial studying the effect of thiotepa treatment on the reduction of the recurrence of bladder tumor.

Using shrinkage methods to estimate treatment effects in overlapping subgroups in randomized clinical trials with a time-to-event endpoint

Abstract:In randomized controlled trials, forest plots are frequently used to investigate the homogeneity of treatment effect estimates in subgroups. However, the interpretation of subgroup-specific treatment effect estimates requires great care due to the smaller sample size of subgroups and the large number of investigated subgroups. Bayesian shrinkage methods have been proposed to address these issues, but they often focus on disjoint subgroups while subgroups displayed in forest plots are overlapping, i.e., each subject appears in multiple subgroups. In our approach, we first build a flexible Cox model based on all available observations, including categorical covariates that identify the subgroups of interest and their interactions with the treatment group variable. We explore both penalized partial likelihood estimation with a lasso or ridge penalty for treatment-by-covariate interaction terms, and Bayesian estimation with a regularized horseshoe prior. One advantage of the Bayesian approach is the ability to derive credible intervals for shrunken subgroup-specific estimates. In a second step, the Cox model is marginalized to obtain treatment effect estimates for all subgroups. We illustrate these methods using data from a randomized clinical trial in follicular lymphoma and evaluate their properties in a simulation study. In all simulation scenarios, the overall mean-squared error is substantially smaller for penalized and shrinkage estimators compared to the standard subgroup-specific treatment effect estimator but leads to some bias for heterogeneous subgroups. We recommend that subgroup-specific estimators, which are typically displayed in forest plots, are more routinely complemented by treatment effect estimators based on shrinkage methods. The proposed methods are implemented in the R package bonsaiforest.

Abstract:While the conventional forest plot is useful to present results within subgroups of patients in clinical studies, it has been criticized for several reasons. First, small subgroups are visually overemphasized by long confidence interval lines, which is misleading. Second, the point estimates of large subgroups are difficult to discern because of the large box representing the precision of the estimate within subgroups. Third, confidence intervals depicted by lines might incorrectly convey the impression that all points within the interval are equally likely. Rainforest plots have been proposed to overcome these potentially misleading aspects of conventional forest plots. The metaviz package enables to generate rainforest plots for meta-analysis within the statistical computing environment R. We suggest the application of rainforest plots for the depiction of subgroup analysis in clinical trials. In this tutorial, detailed step-by-step guidance on the generation of rainforest plot for this purpose is provided.
Evaluating sensitivity to classification uncertainty in latent subgroup effect analyses

Abstract:Background: Increasing attention is being given to assessing treatment effect heterogeneity among individuals belonging to qualitatively different latent subgroups. Inference routinely proceeds by first partitioning the individuals into subgroups, then estimating the subgroup-specific average treatment effects. However, because the subgroups are only latently associated with the observed variables, the actual individual subgroup memberships are rarely known with certainty in practice and thus have to be imputed. Ignoring the uncertainty in the imputed memberships precludes misclassification errors, potentially leading to biased results and incorrect conclusions. Methods: We propose a strategy for assessing the sensitivity of inference to classification uncertainty when using such classify-analyze approaches for subgroup effect analyses. We exploit each individual's typically nonzero predictive or posterior subgroup membership probabilities to gauge the stability of the resultant subgroup-specific average causal effects estimates over different, carefully selected subsets of the individuals. Because the membership probabilities are subject to sampling variability, we propose Monte Carlo confidence intervals that explicitly acknowledge the imprecision in the estimated subgroup memberships via perturbations using a parametric bootstrap. The proposal is widely applicable and avoids stringent causal or structural assumptions that existing bias-adjustment or bias-correction methods rely on. Results: Using two different publicly available real-world datasets, we illustrate how the proposed strategy supplements existing latent subgroup effect analyses to shed light on the potential impact of classification uncertainty on inference. First, individuals are partitioned into latent subgroups based on their medical and health history. Then within each fixed latent subgroup, the average treatment effect is assessed using an augmented inverse propensity score weighted estimator. Finally, utilizing the proposed sensitivity analysis reveals different subgroup-specific effects that are mostly insensitive to potential misclassification. Conclusions: Our proposed sensitivity analysis is straightforward to implement, provides both graphical and numerical summaries, and readily permits assessing the sensitivity of any machine learning-based causal effect estimator to classification uncertainty. We recommend making such sensitivity analyses more routine in latent subgroup effect analyses.

Abstract:The importance of subgroup analyses has been increasing due to a growing interest in personalized medicine and targeted therapies. Considering designs with multiple nested subgroups and a continuous endpoint, we develop methods for the analysis and sample size determination. First, we consider the joint distribution of standardized test statistics that correspond to each (sub)population. We derive multivariate exact distributions where possible, providing approximations otherwise. Based on these results, we present sample size calculation procedures. Uncertainties about nuisance parameters which are needed for sample size calculations make the study prone to misspecifications. We discuss how a sample size review can be performed in order to make the study more robust. To this end, we implement an internal pilot study design where the variances and prevalences of the subgroups are reestimated in a blinded fashion and the sample size is recalculated accordingly. Simulations show that the procedures presented here do not inflate the type I error significantly and maintain the prespecified power as long as the sample size of the smallest subgroup is not too small. We pay special attention to the case of small sample sizes and attain a lower boundary for the size of the internal pilot study.

Subgroup Identification Using the personalized Package

Abstract:A plethora of disparate statistical methods have been proposed for subgroup identification to help tailor treatment decisions for patients. However a majority of them do not have corresponding R packages and the few that do pertain to particular statistical methods or provide little means of evaluating whether meaningful subgroups have been found. Recently, the work of Chen, Tian, Cai, and Yu (2017) unified many of these subgroup identification methods into one general, consistent framework. The goal of the personalized package is to provide a corresponding unified software framework for subgroup identification analyses that provides not only estimation of subgroups, but evaluation of treatment effects within estimated subgroups. The personalized package allows for a variety of subgroup identification methods for many types of outcomes commonly encountered in medical settings. The package is built to incorporate the entire subgroup identification analysis pipeline including propensity score diagnostics, subgroup estimation, analysis of the treatment effects within subgroups, and evaluation of identified subgroups. In this framework, different methods can be accessed with little change in the analysis code. Similarly, new methods can easily be incorporated into the package. Besides familiar statistical models, the package also allows flexible machine learning tools to be leveraged in subgroup identification. Further estimation improvements can be obtained via efficiency augmentation.

Abstract:Subgroup analysis for modeling longitudinal data with heterogeneity across all individuals has drawn attention in the modern statistical learning. In this paper, we focus on heterogeneous quantile regression model and propose to achieve variable selection, heterogeneous subgrouping and parameter estimation simultaneously, by using the smoothed generalized estimating equations in conjunction with the multi-directional separation penalty. The proposed method allows individuals to be divided into multiple subgroups for different heterogeneous covariates such that estimation efficiency can be gained through incorporating individual correlation structure and sharing information within subgroups. A data-driven procedure based on a modified BIC is applied to estimate the number of subgroups. Theoretical properties of the oracle estimator given the underlying true subpopulation information are firstly provided and then it is shown that the proposed estimator is equivalent to the oracle estimator under some conditions. The finite-sample performance of the proposed estimators is studied through simulations and an application to an AIDS dataset is also presented.
