Abstract:OBJECTIVE: To investigate the clinical significance of plasma homocysteine Hcy in patients with coronary atherosclerosis. METHODS Plasma Hcy levels of 85 patients with coronary atherosclerosis and 68 normal controls were determined by fluorescence polarization immunoassay. RESULTS The mean levels of plasma Hcy were (9.31+/-3.80)&mgr;mol/L in normal controls and (13.39+/-6.06)&mgr;mol/L in patients with coronary atherosclerosis. That was (11.36+/-3.86)&mgr;mol/L in the patients with micro-pathological changes of coronary artery, (13.32+/-6.09)&mgr;mol/L with single-vessel disease,(13.39+/-4.92)&mgr;mol/L with double-vessel disease, and (18.23+/-8.98)&mgr;mol/L with three-vessel disease by coronary angiography. Statistically, the mean plasma Hcy concentrations in male and female patients was higher than that in the corresponding control subjects(13.77+/-6.68 compared with 10.50+/-4.07, 11.50+/-3.58 compared with 7.80+/-2.85 &mgr;mol/L,P<0.001 respectively). CONCLUSION The patients with coronary atherosclerosis present hyperhomocysteinemia is very important to determine plasma homocysteine for diagnosis and therapy in the patients with coronary heart disease.
Long-term trends in solar radiation and the associated climatic factors over China for 1961-2000

Abstract:Abstract. Long-term trends in downwelling solar irradiance and associated climatic factors over China are studied in the paper. Decreasing trends in global and direct radiation are observed over much of China. The largest decrease occurs in South and East China (east of about 100° E and south of about 40° N). The spatial pattern of observed trends in diffuse irradiance is complex and inhomogeneous. An intriguing aspect of trends in global and direct irradiance is the rather abrupt decrease in annual and seasonal mean values from 1978 onward. The decreasing trends in solar radiation in China did not persist into the 1990s. The spatial and temporal patterns of trends in sunshine duration are consistent with that of global and direct irradiance. A decreasing trend in rainy days is observed over much of China, which is in agreement with the secular trend in cloud amount. The fact that trends in cloud amount and solar radiation are quite similar suggests that the cloud amount is not the primary cause for the decrease in solar radiation. Visibility in the eastern part of China has deteriorated heavily as a result of the rapid increase in aerosol loading. The statistical analysis showed that atmospheric transmission under clear conditions decreased rapidly. These facts suggest that the rapid increase in aerosol loading should be one of the principle causes for the decrease in solar radiation. The observed diurnal temperature range decreases remarkably in China, which is closely related to the increase in aerosols. The effects of anthropogenic air pollutants on climate should be further studied and included in the simulation of climate and projection of climate scenario. Keywords. A Significant Inverse Relationship Between Concentrations of Plasma Homocysteine and Phospholipid Docosahexaenoic Acid in Healthy Male Subjects.

Abstract:The aim of this study was to investigate the possibility of a relationship between plasma homocysteine (Hcy) and phospholipid FA (PUFA) in healthy Australian males. One hundred thirty six healthy male subjects aged 20–55 yr were recruited from the Melbourne metropolitan area. Each volunteer completed a semiquantitative food frequency questionnaire and gave a blood sample. Plasma Hcy concentrations were determined by an established HPLC method; the plasma phospholipid FA were determined by standard methods. Plasma Hcy concentration was significantly negatively correlated with plasma phospholipid concentration of the PUFA 20∶5n−3 ( r =−0.226, P =0.009), 22∶5n−3 ( r =−0.182, P =0.036), 22∶6n−3 ( r =−0.286, P =0.001), total n−3 ( r =−0.270, P =0.002) and the ratio n−3/n−6 PUFA ( r =−0.265, P =0.002), and significantly positively correlated with 20∶4n−6 ( r =0.180, P =0.037). In the partial correlation analysis, after controlling for serum vitamin B 12 and folate concentration, plasma Hcy was significantly negatively correlated with the plasma phospholipid concentration of 22∶6n−3 ( r =−0.205, P =0.019), total n−3 ( r =−0.182, P =0.038) and the ratio n−3/n−6 PUFA ( r =−0.174, P =0.048). Evidence indicates that an increased concentration of n−3 PUFA in tissues has a beneficial effect on cardiovascular health. Our findings provide further evidence that increased consumption of dietary n−3 PUFA increases the concentration of n−3 PUFA in plasma phospholipid, which is associated with a protective effect on cardiovascular diseases and lower plasma Hcy levels. The mechanism that might explain the association between plasma 22∶6n−3 and Hcy levels is not clear.
Plasma homocysteine and longitudinal change in cognitive function among urban adults

Abstract:Background: Cross-sectional and longitudinal studies have inconsistently linked cognitive performance and change over time to an elevated level of homocysteine (Hcy), with few conducted among urban adults. Methods: Longitudinal data [Visit 1 (2004-2009) and Visit 2 (2009-2013)] were analyzed from up to 1430 selected Healthy Aging in Neighborhoods of Diversity across the Life Span (HANDLS) participants. Baseline and follow-up blood Hcy was measured, while 11 cognitive function test scores were assessed at either of these two visits. Overall, sex- and race-stratified associations were evaluated using mixed-effects linear regression models, adjusting for key potential confounders. Interaction effects between Hcy and serum levels of folate and vitamin B-12 were also tested. Results: We found that greater LnHcyv1 was significantly associated with poorer baseline attention based on higher Loge (TRAILS A, in seconds) [β (SE): 0.101 (0.031), P = 0.001]. Heterogeneity was also found by sex and by race. Most notably, among men only, LnHcyv1 was associated with faster decline on the BVRT (# of errors), a measure of visuo-spatial memory (β (SE): 0.297(0.115), P = 0.010, reduced model); while among African American adults only, an elevated and increasing LnHcy over time was associated with faster rate of decline on Loge (TRAILS B, in seconds) [β (SE): +0.012 (0.005), p = 0.008], a measure of executive function. Interactions between Hcy, folate and vitamin B-12 blood exposures were also detected. Conclusions: In summary, sex- and race-specific adverse association between elevated Hcy and cognitive performance over time were detected among middle-aged urban adults, in domains of attention, visuo-spatial memory and executive functioning.

Abstract:This article presents updated diagnostic and therapeutic guidelines for the management of pancreatic neuroendocrine tumours (PNEN), proposed by the Polish Network of Neuroendocrine Tumours. The guidelines contain new data received in the years 2013-2016, which confirm previous recommendations, and have led to modification of previous guidelines or have resulted in the formulation of new guidelines. Biochemical and imaging (anatomical and functional) tests are of great importance in diagnostics, as well as histopathological diagnosis to determine the management of PNEN patients, but they must be confirmed by an immunohistochemical examination. PNEN therapy requires collaboration among the members a multidisciplinary team of specialists experienced in the management of these neoplasms. Surgery is the basic form of treatment in many cases. Further therapy requires a multidirectional procedure; therefore, the rules of biotherapy, peptide receptor radionuclide therapy, molecular targeted therapy, and chemotherapy are discussed.

Abstract:The distribution profile of plasma homocysteine (Hcy) in young adults and its related factors are not well understood. We performed a generalized estimating equations (GEE) analysis for plasma-Hcy-correlated factors in 2436 young adults, aged 20–39 years, from a health checkup population. We observed that the mean Hcy concentration in males (16.7 ± 10.3 μmol/L) was significantly higher than that in females (10.3 ± 4.0 μmol/L), and hyperhomocysteinemia (HHcy) prevalence in males was 5.37 times than that in females (33.3% vs. 6.2%). A GEE analysis stratified by sex indicated that age (B = −0.398, p < 0.001) and LDL-C (B = −1.602, p = 0.043) were negatively correlated, while BMI (B = 0.400, p = 0.042) was positively correlated, with the Hcy level in young males. ALT (B = −0.021, p = 0.033), LDL-C (B = −1.198, p < 0.001) and Glu (B = −0.446, p = 0.006) were negatively correlated, while AST (B = 0.022, p = 0.048), CREA (B = 0.035, p < 0.001), UA (B = 0.004, p = 0.003) and TG (B = 1.042, p < 0.001) were positively correlated, with the Hcy level in young females. These results suggest that young males have a significantly higher plasma Hcy level and HHcy prevalence than young females; therefore, more attention should be paid to the reason for and effect of the higher HHcy prevalence in young males.
Racial variation in fasting and random homocysteine levels

Abstract:Homocysteine (Hcy) levels have been shown to be a predeterminant of thrombotic diseases. We measured the Hcy levels of 50 blacks and 50 whites equally divided by gender to determine if there is a significant racial difference in either fasting or random Hcy levels. Dietary, medication, smoking, alcohol, past medical, educational, and occupational histories were obtained, and the body mass index calculated. Total serum fasting and random Hcy levels, B12, folate, BUN, creatinine, and lipid profiles were drawn from each participant. Analysis of the results showed that white males have the highest fasting Hcy levels, 10.5 microM/l, whereas random Hcy levels were not significantly different. Correlation between fasting and random Hcy levels was poor (R = 0.61). B12 levels in black subjects were significantly higher, 490.8 pg/ml, compared to whites, 382.8 pg/ml, P = 0.001, but contributed little to total Hcy levels (R(2) = 0.08). Folic acid levels, all within normal range, were not significantly different between the two racial groups and also did not appear to greatly affect Hcy levels (R(2) = 0.06). Our study demonstrates that, despite the genetic diversity of these two racial groups in the U.S., white males in this age group have higher fasting Hcy levels than black males, and white males, but not black males, have higher fasting homocysteine levels than females. This discrepancy in Hcy levels may reflect methylene-tetrahydrofolate reductase (MTHFR) enzyme polymorphisms, known to be higher in whites, rather than socioeconomic influences.

Abstract:Although methylenetetrahydrofolate reductase (MTHFR) genetic variants are associated with plasma homocysteine (Hcy) and cardiovascular disease (CVD), little s known whether dietary fatty acid intake modulates these associations. Tie goal was to examine the interaction o MTHFR variants with d etary fatty acids influencing plasma Hey in 995 Bostor Pier to Rican adults. We found that plasma Hcy concentration was negatively correlated with (n-3) PUFA intake (r = -0.117; P - 0.022), and the rat o of (n-3):(n-6) PUPA in the diet (r = -0.1 22; P = 0.009). Further, 2 functional MTHER variarts, 1298A>C and 677C>T, which are not in linkage disequilibrium in this population, were significantly associated with hypertension (OR = 1.72. P = 0.024, and OR = 1.60. P = 0 002, respectively). In addition, :he 1298A>C variant was sigmficantly associated with CVD (OR = 3.32; P = 0.030). Importantly. this variant exhibited significant interactions with intakes of total and (n-6) PUFA and the (n-3):(n-6) PUPA ratio of the d e:. The plasma Hcy concentration of carriers of risk allele 1298C was greater than that of roncarr ers only when participants had consumed a high-PUFA diet (>7.8% energy) but was not greater when :hey had low intake of PUFA (<= 7.8% energy). In addition, participants with combined aerotypes of both SNP (677 : with 1298 AC or CC) who consumed high levels of (n-3) PUFA (>0.66% energy) had lower plasma Hcy compared with :hose who had the same genotype and consumed low levels of (n-3) PUFA (>0.66% energy). Our study suggests that dietary PJFA intake modulates the ef ect of 2 MIHFR variants or plasma Hcy in Boston Puerto Rican adults. J. Nutr. 141: 654-659, 2011.
Associations of plasma homocysteine levels with peripheral systolic blood pressure and noninvasive central systolic blood pressure in a community-based Chinese population

Abstract:Previous studies indicated that homocysteine (Hcy) is associated with higher peripheral systolic blood pressure (pSBP). There have been few data on the relationship between Hcy and central SBP (cSBP). A total of 4,364 Chinese subjects from the Shijingshan community in Beijing were included. cSBP and pSBP were measured with an Omron HEM-9000AI device. Subjects were 57.20 ± 8.9 years old, 37.9% were male. The median of Hcy was 11.96 μmol/L. The mean of cSBP and pSBP was 129.94 ± 18.03 mmHg and 133.25 ± 18.58 mmHg. lnHcy was associated with cSBP (adjusted β = 2.17, SE = 0.80, P = 0.007) and pSBP (adjusted β = 2.42, SE = 0.75, P = 0.001). With increasing Hcy, there were enhanced correlations of Hcy with pSBP and cSBP (p for trend between quartiles <0.01). Using Q1 for reference, the Q4 was associated with cSBP (adjusted β = 1.77, SE = 0.89, P = 0.047) and pSBP (adjusted β = 2.15, SE = 0.84, P = 0.011). The correlations were more significant in non-obese subjects than in obese subjects (cSBP: β = 4.30 vs 0.46, pSBP: β = 5.04 vs 1.18, P for interaction <0.001). Our study showed that Hcy was associated with higher cSBP and pSBP, especially in non-obese subjects.

Abstract:Objective: To investigate genetic and lifestyle factors and their interactions on plasma homocysteine (Hcy) concentrations in the Boston Puerto Rican population. Design: Cross-sectional study. Plasma concentrations of Hcy, folate, vitamin B-12 and pyridoxal phosphate were measured, and genetic polymorphisms were determined. Data on lifestyle factors were collected in interviews. Setting: A population survey of health and nutritional measures. Subjects: A total of 994 Puerto Rican men and women residing in the Boston metropolitan area. Results: Smoking status was positively associated with plasma Hcy. Genetic polymorphisms MTHFR 677C -> T, FOLH1 1561C -> T, FOLH1 rs647370 and PCFT 928A -> G interacted significantly with smoking for Hcy. MTHFR 1298A -> C (P = 0.040) and PCFT 928A -> G (P = 0.002) displayed significant interactions with alcohol intake in determining plasma Hcy. Subjects with PCFT 928GG genotype had significantly higher plasma Hcy concentrations compared with carriers of the A allele (AA+AG; P = 0.030) among non-drinking subjects. When consuming alcohol, GG subjects had lower plasma Hcy levels compared with AA+AG subjects. Physical activity interacted significantly with MTR 2756A -> G in determining plasma Hcy (P for interaction = 0.002). Smoking interacted with physical activity for plasma Hcy (P for interaction = 0.023). Conclusions: Smoking and drinking were associated plasma Hcy concentrations. Genetic variants involved in folate metabolism further modify the effects of lifestyle on plasma Hcy.
A Case-Control Study on the Relationship Between Abnormal Homocysteine Metabolism and Essential Hypertension

Abstract:OBJECTIVE:To examine the relationship between abnormal homocysteine metabolism and essential hypertension in a Chinese population, a community-based case-control study was conducted.METHODS:127 essential hypertensive patients aged 35 to 75 were randomly selected from a community. Another 170 control subjects with blood pressure < 140/90 mmHg were selected from the same community. Serum homocysteine was determined using HPLC. Folate and vitamin B(12) were measured by radioimmunoassay. MTHFR genotypes were identified by PCR and restriction fragment length polymorphism analysis with Hinf I digestion.RESULTS:After adjusting for age and sex, the mean homocysteine level was 10.56 micromol/L for hypertensive patients and 10.34 micromol/L for controls (F = 0.234, P = 0.63). No association between either SBP or DBP and Hcy concentration was found in subjects without anti-hypertensive medications. The prevalence of homozygousity for thermolabile MTHFR variant for this population was 13.1%, and the mutant allele frequency was 38.7%. There was no significant difference on genotype distributions and the mutant allele frequency for the two studied groups. However, the concentrations of folate and B(12) for the hypertensive subjects were generally higher than the controls.CONCLUSIONS:The present study did not discover that the elevation of Hcy levels or MTHFR mutation were independent risk factors for essential hypertension. The higher folate and B(12) in the hypertensive subjects might contribute to a lower risk.
The Association Between Total Homocysteine and Blood Pressure in Two Independent Chinese Populations.

Abstract:Hypertension and hyperhomocystinemia have a joint effect on the risk of stroke. We aimed to evaluate the relationship between plasma total homocysteine (tHcy) and blood pressure in two independent Chinese populations. Four thousand five hundred and fifty-five participants who underwent health examinations between March 2016 and September 2016 at Peking University First Hospital were enrolled as ‘Population 1’, and 2689 participants who were admitted to Peking University First Hospital between January 2014 and December 2015 were enrolled as ‘Population 2’. None of the study participants were taking antihypertensive medication or vitamins, or had cardio-cerebrovascular disease or chronic kidney disease stages 4 or 5. In Population 1, a 5 μmol/L increase in tHcy was associated with a 0.47 mmHg (95% confidence interval [CI]: 0.23–0.70 mmHg, p < 0.01) increase in systolic blood pressure (SBP) and a 0.14 mmHg (95% CI: −0.02 to 0.30 mmHg, p = 0.08) increase in diastolic blood pressure (DBP). In Population 2, a 5 μmol/L increase in tHcy was associated with a 0.42 mmHg (95% CI: 0.13–0.72 mmHg, p < 0.01) increase in SBP and a 0.29 mmHg (95% CI: 0.09–0.49 mmHg, p < 0.01) increase in DBP. The prevalence of hypertension was significantly higher in Population 1 (by 47%; odds ratio [OR] 1.47, 95% CI: 1.09–1.98, p = 0.01) and in Population 2 (by 55%;OR 1.55, 95% CI: 1.15–2.08, p < 0.01) in participants with tHcy ≥ 15 μmol/l than in those with tHcy < 10 μmol/L. Stratified analysis showed that the association was stronger in women than in men.

Abstract:Aims Hyperhomocysteinemia (HHcy) is known to increase the risk of many diseases. Factors influencing HHcy in healthy and hypertensive subjects remain under-researched. Methods A large population-based study was conducted in 60 communities from Shenzhen, China. Responses to standardized questions on lifestyle factors and blood samples were collected from all participants after a 12-h overnight fast. Multiple linear and multivariate logistic regressions were used to explore risk factors for HHcy. Results were then compared to those from a systematic review of English-language articles listed in Pubmed, EBSCOhost, Web of Science, Embase and Cochrane libraries that investigated HHcy risk factors in healthy and hypertensive subjects. Results A total of 1586 healthy (Male/Female = 642/944) and 5935 hypertensive subjects (Male/Female = 2928/3007) participated in our population-based study. In logistic regression analyses, age, BMI and creatinine (Cr) were risk factors, while being female, fruit intake and physical activity were protective factors for HHcy in healthy subjects. In hypertensive subjects, seven [age, smoking, salt intake, systolic blood pressure (SBP), uric acid, triglycerides (TG), and Cr] and four [female, fruit intake, total cholesterol (TC), and glucose] factors were associated with higher and lower HHcy respectively. The review of 71 studies revealed that potential risk factors for Hcy included nutritional, physiologic, lifestyle habits, ethnicity, genetics, interactions between gene–environment, gene–gene, gene–nutritional, environment–environment, nutritional–nutritional. Conclusion Our study indicates the potential importance of increasing folic acid and vitamin B supplementation, daily fruit and vegetable intake, regular exercise and refraining from tobacco smoking and alcohol consumption as preventive strategies for Hcy.

Abstract:Background: Homocysteine (Hcy) is an independent risk factor for cardiovascular disease, while mechanisms are unclear. Despite inconsistent and limited, epidemiological and experimental studies indicated that hyperhomocysteinemia (HHcy) affected lipid metabolism. This study aims to investigate the association of plasma Hcy with traditional lipid profiles and remnant cholesterol (RC) in Chinese adults. Methods: In total, 7,898 subjects aged 20–79 years who underwent a physical examination at Beijing Chao-Yang Hospital in Beijing were included in this study. Fasting plasma total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), apolipoprotein A1 (ApoA1), apolipoprotein B (ApoB), lipoprotein (a) [Lp(a)], Hcy, and other metabolic risk factors were measured by routine automated laboratory methods. RC was calculated as TC minus HDL-C and LDL-C. The linear regression model and logistic regression model were used to assess the relationship between Hcy and lipids after adjusting potential confounders. Results: Of the subjects, the median level of plasma Hcy was 13.0 μmol/L and 32.3% had HHcy. Plasma Hcy was negatively associated with HDL-C, ApoA1, and Lp(a) and positively associated with TG levels after adjusting age, sex, body mass index, blood pressure, alanine transaminase, aspartate transaminase, creatinine, uric acid, and glucose. HHcy significantly increased the risk of low HDL-C [odds ratio (OR) 1.26; 95%CI (1.11–1.44); p < 0.001]. The net mediation effects of ApoA1 on the relationship between Hcy and HDL-C before and after adjusting confounders were 46.9 and 30.6%, respectively. More interestingly, the RC level was significantly elevated in subjects with HHcy after adjusting other influencing factors ( p = 0.025). Hcy presented a positive correlation with RC levels after adjusting the above confounding factors (β = 0.073, p = 0.004), and the correlation was still significant even after controlling other lipids, including TG, LDL-C, HDL-C, ApoA1, ApoB, and Lp(a). Conclusion: Our study showed that plasma Hcy was not only significantly associated with conventional atherogenic lipids but also independently correlated with RC levels beyond other lipids after controlling potential confounders. This finding proposes that identifying Hcy-related dyslipidemia risk, both traditional lipids and RC residual risk, is clinically relevant as we usher in a new era of targeting Hcy-lowering therapies to fight against dyslipidemia or even cardiovascular disease.

Abstract:Homocysteine (Hcy) is a heritable biomarker for CVD, peripheral artery disease, stroke, and dementia. Little is known about genetic associations with Hcy in individuals of African ancestry. We performed a genome-wide association study for Hcy in 4927 AAs from the Jackson Heart Study (JHS), the Multi-Ethnic Study of Atherosclerosis (MESA), and the Coronary Artery Risk in Young Adults (CARDIA) study. Analyses were stratified by sex and results were meta-analyzed within and across sex. In the sex-combined meta-analysis, we observed genome-wide significant evidence ( p < 5.0 × 10 −8 ) for the NOX4 locus (lead variant rs2289125, β = −0.15, p = 5.3 × 10 11 ). While the NOX4 locus was previously reported as associated with Hcy in European-American populations, rs2289125 remained genome-wide significant when conditioned on the previously reported lead variants. Previously reported genome-wide significant associations at NOX4 , MTR , CBS , and MMACHC were also nominally ( p < 0.050) replicated in AAs. Associations at the CPS1 locus, previously reported in females only, also was replicated specifically in females in this analysis, supporting sex-specific effects for this locus. These results suggest that there may be a combination of cross-population and population-specific genetic effects, as well as differences in genetic effects between males and females, in the regulation of Hcy levels.
Prevalence and clinical correlates of hyperhomocysteinemia in Chinese urban population with hypertension

Abstract:Context: The coexistence of hypertension and elevated homocysteine (Hcy) levels has a mutually reinforcing impact on the susceptibility to cardio-cerebrovascular disease. Objective: The aim was to assess the prevalence, clinical correlation, and demographic characteristics of hyperhomocysteinemia (HHcy) within the Chinese urban population with hypertension. Methods: A cohort of 473 individuals with hypertension were selected from four communities in Shenzhen, China. Demographic attributes, clinical profiles, and lifestyle behaviors were gathered and compared between individuals with and without HHcy. A logistic regression model was employed to examine potential factors associated with the prevalence of HHcy. Correlation between Hcy levels and clinical characteristics was assessed through multiple linear regression analysis. Results: The prevalence of HHcy in the population with hypertension was 31.3%. In comparison to individuals without HHcy, those with HHcy exhibited a higher proportion of males, a higher prevalence of smoking and alcohol consumption, and a higher proportion of cases with the homozygous (TT) genotype at the MTHFR C677T polymorphism. Moreover, individuals with HHcy had lower levels of folic acid (FA), and lower fruit and vitamin B12 intake. Furthermore, the risk factors for HHcy were male ( B = 1.430, OR = 4.179) and MTHFR (TT) ( B = 1.086, OR = 2.961). In addition, the multiple linear regression analysis revealed a significant association between Hcy levels and gender ( B = -2.784, P = 0.004), MTHFR genotypes ( B = 1.410, P = 0.005), and FA levels ( B = -0.136, P = 0.030). Conclusion: The high prevalence of HHcy among hypertensive patients in this Chinese urban population underscores the necessity for interventions targeting modifiable risk factors such as dietary choices and lifestyle practices.

Association between total homocyst(e)ine and the likelihood for a history of acute myocardial infarction by race and ethnicity: Results from the Third National Health and Nutrition Examination Survey

Abstract:Background: Few studies examining the association between total homocyst(e)ine and coronary heart disease have included blacks or Hispanics. Methods: Data from the third National Health and Nutrition Examination Survey (3173 patients), a nationally representative survey of US adults, were used to examine the relation between total homocyst(e)ine and an electrocardiogram or a physician's diagnosis of acute myocardial infarction (259 patients) among whites, blacks, and Mexican Americans >/=40 years old. Results: Vitamin B(12) and serum folate concentrations were significantly lower among persons with a total homocyst(e)ine concentration >/=15 micromol/L than among those with a total homocyst(e)ine concentration </=10 micromol/L. Persons with a total homocyst(e)ine concentration >/=15 micromol/L were also older and more likely to be hypertensive, have a higher cholesterol concentration, and smoke. Compared with persons with a total homocyst(e)ine concentration </=10 micromol/L, persons with a concentration >/=15 micromol/L had an odds ratio (OR) for myocardial infarction of 1.8 (95% confidence interval [CI], 1.2-2.9) after adjustment for cardiovascular disease risk factors. Similar associations were noted among whites (OR 1.8, 95% CI, 1.1-3.1) and blacks (OR 1.9, 95% CI, 0.8-4.2); a more modest association was noted among Mexican Americans (OR 1.2, 95% CI, 0.3-5.0). The association between total homocyst(e)ine and myocardial infarction was also more pronounced in persons without hypertension or diabetes. Conclusions: Almost a 2-fold increased likelihood of myocardial infarction among persons with a total homocyst(e)ine concentration >/=15 micromol/L was noted in this nationally representative survey. The magnitude of the association did not differ by race or ethnicity.
No Causal Association Between Plasma Homocysteine Levels and Atrial Fibrillation: A Mendelian Randomization Study

Abstract:Background and aims: Although several studies have shown an association between plasma homocysteine (Hcy) levels and atrial fibrillation (AF), the causality remains unclear. We undertook a Mendelian randomization (MR) study to investigate the causal association between Hcy and AF. Methods and results: Single-nucleotide polymorphisms (SNPs) which genome-wide significantly associated with plasma Hcy levels were obtained from a genome-wide meta-analysis (N = 44 147). MR analyses including the random-effect inverse variance-weighted (IVW) meta-analysis, weighted median analysis, and MR-Egger regression were used to estimate the associations between the selected SNPs and AF based on a meta-analysis of genome-wide association study for AF (N = 588 190). The MR analyses revealed no causal role of genetically elevated plasma Hcy levels with AF risk (random-effect IVW, odds ratio per 1 SD increase in Hcy levels = 0.972, 95% confidence interval = 0.919 to 1.027, P = 0.308). The results were consistent with the weighted median method, MR-Egger and the analysis after excluding the pleiotropic SNPs. No heterogeneity and directional pleiotropy were observed in sensitivity analyses. Conclusion: The findings suggested that plasma Hcy levels were not causally associated with AF. The differential diagnostic value of serum homocysteine for white coat hypertension.

Abstract:Objective: To assess the value of serum homocysteine (Hcy) in differential diagnosis of white coat hypertension (WCH). Results: In this retrospective study, serum Hcy levels were elevated in hypertensive patients (P < 0.001) compared to WCH patients. Serum Hcy levels were positively correlated with 24-h mean systolic blood pressure, r = 0.1378, P < 0.001. The results of the receiving operating characteristic (ROC) curve showed that the AUC value of Hcy was 0.80 (95% CI, 0.77-0.83), the cut-off value was 13.8 mu mol/L, the sensitivity was 68.58% and the specificity 87.21%. In the prospective study, the AUC value of Hcy was 0.73 (95% CI: 0.67-0.78), higher than N -terminal pro -brain natriuretic peptide(NT-pro-BNP) (0.64, 95% CI: 0.58-0.70) and cystatin C (Cys-C) (0.62, 95% CI: 0.55-0.68). Hcy, NT-proBNP and Cys-C combined, provided a better indication of a differential diagnosis of WCH, than Hcy alone. Materials and Methods: This investigation involved both a retrospective and a prospective study. Clinical data including blood pressure, age, sex, height, weight, BMI, smoking status, past history, and behavioral electrocardiogram of patients who had undergone 24-hour ambulatory blood pressure monitoring (ABPM) with elevated clinical blood pressure (BP) were recorded. Pearson correlation analysis was used to test the correlation between Hcy and BP. The ROC curve was used to analyze the value of measuring Hcy levels in differential diagnosis of WCH. Conclusions: Serum Hcy was decreased in WCH patients and therefore could be a biomarker for differential diagnosis of WCH.
