Abstract:Background: Uric acid (UA), the end product of purine nucleotide metabolism, participates in the processes of metabolic and cardiovascular diseases. Experimental evidence suggests it is an important mediator in the physiological response to blood pressure increase. Objective: To evaluate the association between serum UA levels and pre-hypertension and hypertension in a Chinese population. Methods: A cross-sectional study was conducted from March to September 2017, and 1,138 participants aged 35 to 75 were enrolled in this study, where 223 normotensive, 316 pre-hypertensive, and 599 hypertensive subjects were selected to evaluate the association between serum UA levels and hypertension. A p-value <0.05 was considered statistically significant. Results: Serum UA levels were significantly higher in the pre-hypertension and hypertension group compared to the control group in the entire population (p<0.05 for all). Quantitative trait analysis indicated that serum UA levels were (2.92 +/- 0.81, 3.06 +/- 0.85, 3.22 +/- 0.98 mg/d) linearly increased in normotensive, pre-hypertensive and hypertensive females, with a p value of 0.008. Serum UA levels in the quartiles were positively correlated with DBP (p<0.05), particularly in females. After adjusting for age, gender, body mass index (BMI), glucose (GLU), total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-c), the odds ratios (ORs) and 95% confidence intervals (CIs) of pre-hypertension from the lowest (referent) to the highest levels of serum UA were 1.718 (1.028-2.872), 1.018 (0.627-1.654) and 1.738 (1.003-3.010). Additionally, the second quartile of serum UA levels were significantly associated with hypertension, with an OR (95% CI) of 2.036 (1.256-3.298). Conclusions: This study suggests that higher serum UA levels are positively associated with pre-hypertension and hypertension among Chinese adults.
Association of Serum Fatty Acid and Estimated Desaturase Activity with Hypertension in Middle-Aged and Elderly Chinese Population

Abstract:We aimed to investigate the cross-sectional associations of serum fatty acid (FA) and related Δ-desaturase with hypertension among 2,447 community-dwellers aged 35–79 years living in Zhejiang Province, China. Individual FA was determined in serum, Δ5-desaturase (D5D) and Δ6-desaturase (D6D) activities were indirectly estimated by FA product/precursor ratios. Participants in the highest quartile of D5D component scores (20:4n–6, 20:5n–3, 22:6n–3 and D5D) have significantly lower odds of hypertension compared with individuals in the lowest (multivariate-adjusted odds ratio (OR) = 0.68, 95% CI: 0.46–0.98). When further stratified by gender, high D5D component scores were significantly associated with lower odds of hypertension in women (OR = 0.53, 95% CI: 0.35–0.80), but not in men (OR = 0.78, 95% CI: 0.52-1.18). Multivariate-adjusted prevalent OR for an interquartile increment of individual FA and estimated desaturase was 1.27 (95% CI: 1.08–1.50) for 16:0, 1.15 (95% CI: 1.01–1.30) for 16:1n–7, 0.89 (95% CI: 0.80–0.99) for 22:6n–3, 1.32 (95% CI: 1.01–1.72) for D6D (18:3n–6/18:2n–6) and 0.74 (95% CI: 0.56, 0.98) for D5D (20:4n–6/20:3n–6). Present findings suggested that high serum 22:6n–3 and D5D as well as low 16:0, 16:1n–7 and D6D were associated with a low prevalence of hypertension in this Chinese population.
Exploratory serum fatty acid patterns associated with blood pressure in community-dwelling middle-aged and elderly Chinese

Abstract:Background Epidemiological studies have assessed relationships between circulating levels of fatty acid (FA) and blood pressure (BP), and their results remain controversial. Nevertheless, data are sparse on serum FA as biomarker and BP in China. The aim of the study was to investigate the association between serum FA and BP in Chinese populations. Methods We conducted a cross-sectional study nested within a community-based cohort of 2447 Chinese participants aged 35–79 years who completed a baseline assessment between October 2012 and April 2013. Baseline assessment included the collection of fasting blood samples, anthropometric measurements and a personal interview using a validated questionnaire. Serum FA was determined by gas-liquid chromatography. Exploratory factor analyses were employed to identify FA-factor as a reflection of serum FA pattern. A multiple regression model was conducted to estimate adjusted mean of BP with 95 % confidence interval (CI) by tertile groups of the generated FA-factor scores. Results Hypertensive patients have significantly higher serum 14:0, 16:0, 16:1n-7, 18:3n-6, 20:3n-6 and Δ 6 -desaturase index (18:3n-6/18:2n-6) as well as lower 18:2n-6, 22:6n-3 and Δ 5 -desaturase index (20:4n-6/20:3n-6) compared with normotensive participants. Factor 1 (low linoleic acid/high saturated FA pattern: 14:0, 16:0, 16:1n-7, 18:2n-6, 18:3n-6, 20:3n-6) and Factor 2 (n-3 PUFA pattern: 20:5n-3, 22:5n-3, 22:6n-3, 18:1n-9) were identified as indicators of the serum FA pattern. After adjustment for age, gender, body mass index, hypertension treatment, smoking, alcohol intake, education, profession, exercise habit, salt intake, family history of hypertension, heart rate, blood lipids and fasting blood-glucose levels, per a standard deviation (SD) increment of Factor 1 scores was associated with an increment of 2.44 (95 % CI: 1.73, 3.15) mm Hg for systolic BP, whereas per a SD increment of Factor 2 scores was associated with a reduction of 1.40 (95 % CI: 0.80, 2.04) mm Hg for diastolic BP. Conclusions The serum FA pattern characterized by low proportions of 14:0, 16:0, 16:1n-7 and 18:3n-6 as well as high 18:2n-6 and 22:6n-3 was beneficially associated with BP levels in this Chinese population. This evidence well supports the current dietary recommendations in the communities to replace saturated fat with polyunsaturated fat.

Abstract:Context: The coexistence of hypertension and elevated homocysteine (Hcy) levels has a mutually reinforcing impact on the susceptibility to cardio-cerebrovascular disease. Objective: The aim was to assess the prevalence, clinical correlation, and demographic characteristics of hyperhomocysteinemia (HHcy) within the Chinese urban population with hypertension. Methods: A cohort of 473 individuals with hypertension were selected from four communities in Shenzhen, China. Demographic attributes, clinical profiles, and lifestyle behaviors were gathered and compared between individuals with and without HHcy. A logistic regression model was employed to examine potential factors associated with the prevalence of HHcy. Correlation between Hcy levels and clinical characteristics was assessed through multiple linear regression analysis. Results: The prevalence of HHcy in the population with hypertension was 31.3%. In comparison to individuals without HHcy, those with HHcy exhibited a higher proportion of males, a higher prevalence of smoking and alcohol consumption, and a higher proportion of cases with the homozygous (TT) genotype at the MTHFR C677T polymorphism. Moreover, individuals with HHcy had lower levels of folic acid (FA), and lower fruit and vitamin B12 intake. Furthermore, the risk factors for HHcy were male ( B = 1.430, OR = 4.179) and MTHFR (TT) ( B = 1.086, OR = 2.961). In addition, the multiple linear regression analysis revealed a significant association between Hcy levels and gender ( B = -2.784, P = 0.004), MTHFR genotypes ( B = 1.410, P = 0.005), and FA levels ( B = -0.136, P = 0.030). Conclusion: The high prevalence of HHcy among hypertensive patients in this Chinese urban population underscores the necessity for interventions targeting modifiable risk factors such as dietary choices and lifestyle practices.

Associations of Plasma N-3 Polyunsaturated Fatty Acids with Blood Pressure and Cardiovascular Risk Factors among Chinese

Abstract:The relationship between plasma fatty acid (FA) levels and hypertension in Chinese is not fully understood. The aim of this study was to examine relationships between plasma phospholipid (PL) FAs and hypertension in Chinese subjects. One thousand one hundred and fifty-four subjects in Hangzhou, China, were recruited in this cross-sectional study. Two hundred and fourteen (160 males, 54 females) subjects with hypertension and 940 (664 males, 276 females) healthy subjects were identified. The prevalence of hypertension in females (19.6%) was significantly higher than that in males (16.4%). Compared with healthy subjects, hypertensive subjects showed significantly lower plasma PL 22:5n-3 (p = 0.017), 22:6n-3 (p = 0.008), PL polyunsaturated fatty acids (PUFAs; p < 0.001), n-3 PUFA (p = 0.015), n-6 PUFA (p < 0.001) and 20:4n-6 (p < 0.010). PL n-3 PUFA [odds ratio (OR) = 0.43, 95% confidence interval (CI) = 0.29-1.19] and n-3:n-6 (OR = 0.48, 95% CI = 0.12-1.86) were inversely associated with hypertension. However, plasma saturated fatty acid (SFA; OR = 2.01, 95% CI = 1.05-2.98) was significantly positively associated with hypertension. PL SFA was significantly positively associated with systolic blood pressure (p = 0.048), whereas plasma PL monounsaturated FA was significantly positively associated with diastolic blood pressure (DBP; p = 0.009) in hypertensive subjects. PL PUFA (p = 0.022) and n-3 PUFA (p = 0.047) were significantly negatively associated with DBP in hypertensive subjects. Our results suggest that plasma PL n-3 PUFA was significantly inversely associated with hypertension in Chinese. It would seem appropriate for hypertensive subjects to increase their dietary n-3 PUFA which may help reduce BP.
High Sensitivity C-reactive Protein Associated with Different Health Predictors in Middle-Aged and Oldest Old Chinese.

Abstract:Objective To describe the distribution of plasma high sensitivity C-reactive protein (hsCRP) and explore the relationship between hsCRP and metabolic risk factors among residents living in longevity areas of China.Methods 268 individuals aged between 40 and 59 years and 506 individuals aged over 90 years were selected from 5 longevity areas of China to participate in a cross section longitudinal cohort study. The participants were interviewed with general health related questionnaire to collect their demographic, behavioral and lifestyle data, as well as their chronic conditions, and meanwhile their physical and biomedical parameters including waist circumference (WC), blood pressure (BP), hsCRP, plasma lipids, and fasting blood glucose (FBG) were measured.Results The median of hsCRP was 0.99 mg/L in the middle-aged group and 1.76 mg/L in the oldest old group. No significant gender difference was observed between the above two groups. Among the oldest old individuals, 36.56% had an hsCRP level >3.0 mg/L. The prevalence of high hsCRP was 16.79% in the middle-aged group. The results of stepwise multiple linear regression analyses showed that HDL-C was independently associated with In (hsCRP) concentration in the middle-aged group, whereas In (TG), HDL-C and FBG were correlated after adjustment for gender, study site, smoking, drinking, education and BMI in the oldest old group.Conclusion HDL-C is a stronger predictor of elevated hsCRP than other metabolic factors in the middle-aged population. For the oldest old persons, high TG, low HDL-C, and FBG predict elevated plasma hsCRP.
Associations of serum uric acid with hypertension status, stages, phenotypes and progressions among Chinese middle-aged and elderly

Abstract:Background and aims: No consensus has been reached on the association between serum uric acid (SUA) and hypertension. This study aimed to investigate the associations between SUA and hypertension, including its status, stages, phenotypes and progressions, among middle-aged and older Chinese. Methods and results: Data were obtained from the China Health and Retirement Longitudinal Study 2011-2015. Binary logistic regression was used to evaluate the association between SUA and hypertension status. Multinomial logistic regression was used to explore the associations of SUA with hypertension stages, phenotypes and hypertension status progressions. Models were adjusted for potential confounders and stratified by sex. A total of 7931 individuals aged ≥45 years were included, with 39.16 % of hypertension. Significant associations were found of SUA with stage2 and above hypertension (quartile 4 [Q4] vs quartile 1 [Q1]: odds ratio 1.78, 95 % confidence interval 1.31-2.42, P < 0.001), and systolic diastolic hypertension (SDH) (Q4 vs Q1: 1.53, 1.14-2.06, P = 0.005). In sex stratification, significant associations were found between SUA and stage2 and above hypertension and SDH only for men. Moreover, higher quartiles of baseline SUA showed increased risks of maintained hypertension from 2011 to 2015 (Q3 vs Q1: 1.23, 1.03-1.48, P = 0.024; Q4 vs Q1: 1.73, 1.43-2.10, P < 0.001). Conclusion: Higher SUA was associated with hypertension and maintained hypertension among Chinese middle-aged and elderly. Sex-specific associations of SUA with hypertension stages and phenotypes were observed. Regular measurement of SUA in clinical practice may indicate hypertension and its progression, particularly among men.

Abstract:Objectives: Limited evidence of association of high-sensitivity C-reactive protein (hs-CRP) levels and cardiovascular diseases (CVDs) has been conducted in Chinese population whose median levels are lower than western standards. Thus, this study aimed to estimate the association between hs-CRP and CVD in a Chinese middle-aged population. Methods: 1,000 participants aged 35–64 years in each 12 populations were under study, finally, 11,623 were potential respondents. Fasting blood sample was collected for all the participants and tested the hs-CRP. Cardiovascular risk factors were also investigated. Participants in 9 out of 12 sites (n = 8,427) were measured of hs-CRP at baseline and followed up for 6–7 years. A subset (54.76%) of subjects had a repeat hs-CRP measurement during the follow-up. Participants were categorized into four groups regarding to median (interquartile). Cox proportional hazards regression model was used to analyze the relationships between hs-CRP and CVD after adjusting for confounding factors. Estimation of hs-CRP was done using transmission turbidimetry. Results: The median (interquartile) of hs-CRP in baseline was higher in male than female [0.9 (0.4–1.8) vs. 0.7 (0.3–1.6), P < 0.001]. Only 11.65% in male and 10.98% in female met the standard cut points in white populations for hs-CRP > 3 mg/L. During a median of 6.3 years (52,552 person-years), 187 participants occurred CVDs. After adjusted for CVD risk factors, we found that baseline hs-CRP quartiles were linearly associated with CVD in male not female. Irrespective of hs-CRP concentration in baseline, any rise or decline in hs-CRP over time did not affect the HR of CVD. Conclusion: The concentrations of hs-CRP are lower in Chinese adults than reported for white populations. However, higher serum hs-CRP were associated with CVD in Chinese men.
Combined Associations of Hs-Crp and Cognitive Function with All-Cause Mortality among Oldest-Old Adults in Chinese Longevity Areas: a Prospective Cohort Study

Abstract:Background Inflammatory markers, such as high sensitivity C-reactive protein (hs-CRP), and cognitive impairment (CI) are associated with mortality; CRP is related to the deterioration of CI. However, it is still unknown whether these two indices predict mortality independent of each other. Furthermore, their joint effect on all-cause mortality has not been well established, especially in oldest-old adults. Methods Based on data from the 2012 wave of the Chinese Longitudinal Healthy Longevity Survey (CLHLS), we included 1447 oldest-old adults (mean age 84.7 years and 58.7% were female, weighted) with information on hs-CRP (stratified by a cutoff value of 3.0 mg/L) and cognition (quantified by Mini-Mental Status Examination (MMSE) scored according to the personal educational level) at baseline. Mortality was assessed in followed 2014 and 2017 waves. Cox proportional hazards regression models were used, with adjustment for hs-CRP and cognition (mutually controlled) and several traditional mortality risk factors. Results During a median follow-up period of 32.8 months (Q1-Q3, 9.7–59.0 months), 826 participants died. Hs-CRP [HR > 3.0 mg/L vs ≤ 3.0 mg/L : 1.64 (95% CI, 1.17, 2.30)] and cognition [HR CI vs normal : 2.30 (95% CI, 1.64, 3.21)] each was independent predictor of all-cause mortality, even after accounting for each other and other covariates. Monotonic and positive associations were observed in combined analyses, in which the highest mortality risk was obtained in elders with both high hs-CRP > 3.0 mg/L and CI [HR: 3.56 (95% CI, 2.35, 5.38)].The combined effects were stronger in male and younger oldest-old (aged 80–89 years). Conclusion High hs-CRP and CI, both individually and jointly, were associated with increased all-cause mortality risks in Chinese oldest-old. Intervention strategies for preventing inflammation and maintaining adequate cognitive function may be more important in male and younger oldest-old for reducing mortality risk.
Plasma Lipidomic Subclasses and Risk of Hypertension in Middle-Aged and Elderly Chinese

Abstract:While disrupted lipid metabolism is a well-established risk factor for hypertension in animal models, the links between various lipidomic signatures and hypertension in human studies remain unclear. We aimed to examine associations between plasma lipidomic profiles and prevalence of hypertension among 2248 community-living Chinese aged 50–70 years. Hypertension was defined according to 2020 International Society of Hypertension global hypertension practice guidelines and 2018 Chinese guidelines. In total, 728 plasma lipidomic species were profiled using high-coverage targeted lipidomics. After multivariate adjustment, including lifestyle, body mass index, blood lipids, and sodium intake, 110 metabolites from nine lipidomic subclasses showed significant associations with hypertension, among which phosphatidylethanolamines (PEs) had the strongest association. Eleven lipidomic signals for hypertension risk were further identified from the nine subclasses, including PE(18:0/18:2) (OR per SD, 1.49; 95% confidence intervals, 1.30–1.69), phosphatidylcholine (PC) (18:0/18:2) (1.27; 1.13–1.43), phosphatidylserine (18:0/18:0) (1.24; 1.09–1.41), lysophosphatidylinositol (18:1) (1.17; 1.06–1.29), triacylglycerol (52:5) (1.38; 1.18–1.61), diacylglycerol (16:0/18:2) (1.42; 1.19–1.69), dihydroceramide (24:0) (1.25; 1.09–1.43), hydroxyl-sphingomyelins (SM[2OH])C34:1 (1.19; 1.07–1.33), lysophosphatidylcholine (20:1) (0.86; 0.78–0.95), SM(OH)C38:1 (0.87; 0.79–0.96), and PC (18:2/20:1) (0.84; 0.75–0.94). Principal component analysis also showed that a factor mainly containing specific PEs was positively associated with hypertension (1.20; 1.09–1.33). Collectively, our study revealed that disturbances in multiple circulating lipidomic subclasses and signatures, especially PEs, were significantly associated with the prevalence of hypertension in middle-aged and elderly Chinese. Future studies are warranted to confirm our findings and determine the mechanisms underlying these associations.
The mechanism study of low-temperature brittle fracture of bulk Sn-based solder

Abstract:In aerospace applications, some devices, instruments and equipment suffer the harsh environment, as extremely low temperature and wide temperature ranges for instance. The solder joints which are fragile and sensitive to the thermal and mechanical environment are part of the electronic control systems that are important to the devices. Sn-based alloy has been the mostly used solder material of electronic circuit in recent years. Sn-based solder alloy has better performance such as low melting point and good plasticity. However, it shows brittle behavior at low temperatures and brittle failures may eventually lead to catastrophic failures. Study on mechanical behaviors of Sn-based solder alloy at cryogenic temperatures becomes more and more important. Charpy test was performed to measure the dissipated energy in the impact process. There were 6 kinds of solder alloys, as follows: Sn, 99.3Sn0.7Cu, 96.5Sn3Ag0.5Cu, 62Sn36Pb2Ag, 63Sn37Pb, 10Sn90Pb. There is obvious ductile-to-brittle transition for all the Pb-free solders at lower temperature. For Sn solder, this transition begins between −40 °C and −60 °C. The ductile-to-brittle transition temperature (DBTT) of 96.5Sn3Ag0.5Cu solder is between −30°C and −40°C which is higher than Sn solder. When adding Pb to solder, the toughness will be so perfect that the ductile-to-brittle transition tend to be gradual and not obvious. The results shows that for Sn solder the fracture modes have both intergranular fracture and transgranular fracture. A rod-like structure has been found in 99.3Sn0.7Cu solder and 96.5Sn3Ag0.5Cu solder that it facilitates transgranular fracture. The composition of the specific structure is likely to be Cu6Sn5 for 99.3Sn0.7Cu solder and Ag3Sn for 96.5Sn3Ag0.5Cu solder measured by EDS (Energy Disperse Spectroscopy). Ag3Sn particles are observed in the fracture morphology of 62Sn36Pb2Ag solder and the fracture mode is quasi-cleavage fracture.
The Association Between Total Homocysteine and Blood Pressure in Two Independent Chinese Populations.

Abstract:Hypertension and hyperhomocystinemia have a joint effect on the risk of stroke. We aimed to evaluate the relationship between plasma total homocysteine (tHcy) and blood pressure in two independent Chinese populations. Four thousand five hundred and fifty-five participants who underwent health examinations between March 2016 and September 2016 at Peking University First Hospital were enrolled as ‘Population 1’, and 2689 participants who were admitted to Peking University First Hospital between January 2014 and December 2015 were enrolled as ‘Population 2’. None of the study participants were taking antihypertensive medication or vitamins, or had cardio-cerebrovascular disease or chronic kidney disease stages 4 or 5. In Population 1, a 5 μmol/L increase in tHcy was associated with a 0.47 mmHg (95% confidence interval [CI]: 0.23–0.70 mmHg, p < 0.01) increase in systolic blood pressure (SBP) and a 0.14 mmHg (95% CI: −0.02 to 0.30 mmHg, p = 0.08) increase in diastolic blood pressure (DBP). In Population 2, a 5 μmol/L increase in tHcy was associated with a 0.42 mmHg (95% CI: 0.13–0.72 mmHg, p < 0.01) increase in SBP and a 0.29 mmHg (95% CI: 0.09–0.49 mmHg, p < 0.01) increase in DBP. The prevalence of hypertension was significantly higher in Population 1 (by 47%; odds ratio [OR] 1.47, 95% CI: 1.09–1.98, p = 0.01) and in Population 2 (by 55%;OR 1.55, 95% CI: 1.15–2.08, p < 0.01) in participants with tHcy ≥ 15 μmol/l than in those with tHcy < 10 μmol/L. Stratified analysis showed that the association was stronger in women than in men.
Hyperuricemia is Associated with More Cardiometabolic Risk Factors in Hypertensive Younger Chinese Adults Than in Elderly

Abstract:BackgroundNumerous studies have shown that hyperuricemia (HUA) is associated with cardiovascular and renal outcomes, but few studies specifically explored the effect of age on this relationship. Therefore, our study aimed to explore the relationship between HUA and other cardiometabolic risk factors in different age groups.MethodsThis cross-section study used the data from Survey on uric acid in Chinese subjects with essential hypertension (SUCCESS). We performed multivariate logistic regressions in different age groups.ResultsAfter adjusting for potential confounders, among young and middle-aged adults less than 60, HUA was associated with higher body mass index (BMI, adjusted OR = 1.114, 95% CI: 1.057–1.174), higher fasting blood glucose (FBG, adjusted OR = 1.099, 95% CI: 1.003–1.205), triglycerides (TG, adjusted OR = 1.425, 95% CI: 1.247–1.629), higher low-density lipoprotein cholesterol (LDL-C, adjusted OR = 1.171, 95% CI: 1.025–1.337), and lower estimated glomerular filtration rate (eGFR, adjusted OR = 0.992, 95% CI: 0.988–0.996). Among elderly adults 60 years or older, HUA was associated with higher SBP (adjusted OR = 1.024, 95% CI: 1.005–1.042), higher TG (adjusted OR = 1.716, 95% CI: 1.466–2.009), and higher LDL-C (adjusted OR = 1.595, 95% CI: 1.366–1.863).ConclusionHUA is associated with more cardiometabolic risk factors in younger adults with hypertension (HT). Comprehensive management of HT with HUA is needed in clinical settings.
Synergistic Effects of Overweight/obesity and High Hemoglobin A1c Status on Elevated High-Sensitivity C-reactive Protein in Chinese Adults: a Cross-Sectional Study.

Abstract:BackgroundHigh-sensitivity C-reactive protein (hs-CRP) is an inflammatory marker that has been suggested as a predictor of cardiovascular diseases. High glycated hemoglobin (HbA1c) levels and overweight/obesity are independently associated with elevated hs-CRP; meanwhile, high HbA1c levels are frequently accompanied by overweight or obesity. However, their joint effect on elevated hs-CRP levels has not been well-established. Therefore, we evaluated whether overweight/obesity modified the association between high HbA1c levels and elevated hs-CRP. MethodsBased on cross-sectional data from the Chinese Urban Adults Diet and Health Study (CUADHS) in 2016, we included 1,630 adults aged 18-75 years (mean age 50.16 years and 33.6% male). Elevated hs-CRP was defined as serum hs-CRP >= 3 and <10 mg/L. The interactive effects of BMI and HbA1c levels on the risk of elevated hs-CRP levels were calculated by using multiple logistic regression models, followed by strata-specific analyses. ResultsIndividuals with elevated hs-CRP had a higher rate of HbA1c level than those without elevated (25.3 vs. 11.3%, P < 0.001), as well as a higher rate of overweight/obesity (67.1 vs. 43.5%, P < 0.001). Higher HbA1c levels were independently associated with an increased risk of elevated hs-CRP [adjusted odds ratio (aOR) = 2.31, 95% confidence interval (CI): 1.47, 3.65], as well as overweight/obesity with the risk of elevated hs-CRP (aOR = .31, 95% confidenc-3.73). Furthermore, overweight/obesity showed a significant synergistic effect on high HbA1c levels with a higher aOR of 5.25 (2.77, 9.95) (P-interaction < 0.001). This synergistic effect was more prominent when stratified by age (in 18-44 years old, aOR, 95% CI = 30.90, 4.40-236.47 for interaction vs. 6.46, 1.38-30.23 for high HbA1c only) and gender (in women, aOR, 95% CI = 8.33, 3.80-18.23 for interaction vs. 2.46,1.38-4.40 for high HbA1c only). ConclusionThere are synergistic effects of high HbA1c levels and overweight/obesity on the risk of elevated hs-CRP in Chinese adults, with more significant effects in adults aged 18-44 years or females. Intervention strategies for preventing high blood glucose levels and body weight simultaneously may be important for reducing hs-CRP-related diseases. Further studies are needed to confirm this finding in other populations, and its molecular mechanisms need to be elucidated.

Abstract:BACKGROUND: A balanced intake of trace elements is beneficial for chronic diseases such as hypertension. However, the available information regarding trace elements that may be independently associated with hypertension is limited, and the relationship between this disorder and element ratios also remains unclear.METHODS: A total of 6,754 subjects from rural China were selected, after exclusion of patients who were under 18, had incomplete data or had additional related disorders, by multi-stage simple random and cluster sampling (participation rate: 95.22 %). Subjects were divided into a hypertensive (H) and a control (C) group. Data were collected on blood pressure and 12 serum trace elements were measured by flame atomic absorption spectrometry and inductively coupled plasma-mass spectrometry. Other basic information was collated from questionnaires and biochemical indicators were measured via kits.RESULTS: Differences in serum levels of magnesium (Mg(mg/l): H: 27.43 ± 12.72; C: 26.33 ± 12.16), iron (Fe(mg/l): H: 1.99 ± 1.24; C: 1.84 ± 1.16), copper (Cu(mg/l): H: 1.19 ± 0.37; C: 1.10 ± 0.36), boron (B(μg/l): H: 50.00 ± 25.21; C: 47.57 ± 26.25), selenium (Se(μg/l): H: 125.12 ± 32.81; C: 118.80 ± 29.72) and chromium (Cr(μg/l): H: 8.77 ± 10.12; C: 10.12 ± 10.72) between the hypertensive and control groups were found. There were no differences in serum contents of calcium (Ca(mg/l): H: 112.43 ± 58.25; C: 111.00 ± 59.49), zinc (Zn(mg/l): H: 1.50 ± 1.97; C: 1.44 ± 1.88), arsenic (As(μg/l): H: 4.17 ± 3.94; C: 4.10 ± 4.00), manganese (Mn(μg/l): H: 4.15 ± 4.03; C: 4.07 ± 4.05), cadmium (Cd(μg/l): H: 1.14 ± 1.11; C: 1.18 ± 1.12) or lead (Pb(μg/l): H: 4.22 ± 8.90; C: 4.26 ± 10.25). The serum Cr and Cd concentrations of hypertensive men were lower than that of male controls while Mg, Cu, Ca and Se concentrations in male controls were lower. Further differences were apparent and Fe, B, Se, Mg and Cu all showed higher levels in hypertensive females whereas Cr concentrations were higher in female controls. Serum Zn and B levels showed age-related variations among hypertensive patients and concentrations of serum Cu, Zn, Se and B showed age-related variations among control subjects. For hypertensive patients, the odds ratio (OR) and 95 % confidence interval (CI) for the association of serum Cu, Se and Cr levels with hypertension were Cu: 1.36 (1.12-1.66); Se: 1.03 (1.01-1.05); Cr: 0.89 (0.83-0.96). Moreover, when the participants in the grouping with the highest copper/zinc (Cu/Zn) and magnesium/manganese (Mg/Mn) ratios were compared with the reference group, the OR and 95 % CI for hypertension were 1.22 (1.04-1.44) and 1.20 (1.01-1.42), respectively.CONCLUSIONS: Levels of serum trace elements showed age- and sex-related differences in a group of rural Chinese adults with hypertension and healthy participants. Serum concentrations of Cu, Se and Cr may be independently associated with hypertension. Higher serum ratios of Cu:Zn and Mg:Mn may also be associated with hypertension. Further randomized trials are necessary to elucidate the true relationship between levels of Cu, Se, Cr, Cu:Zn, Mg:Mn and hypertension.

Impact of the Serum Uric Acid Level on Subclinical Atherosclerosis in Middle-aged and Elderly Chinese.

Abstract:Aim: The carotid intima-media thickness (CIMT) is now validated as a sensitive marker of atherosclerosis and is directly associated with an increased risk of cardiovascular disease. Considering that the independent association between the serum uric acid level and CIMT remains controversial due to the complex interrelationship with other known cardiovascular risk factors, further studies are needed. The aim of the present study is to explore the association between the serum uric acid level and CIMT in a general Chinese population and determine whether the association differs according to varied metabolic status. Methods: The present study was cross-sectional in design. A total of 10,281 community-based participants 40 years of age or older from Shanghai, China were included in the current analysis. All participants underwent a detailed questionnaire interview, anthropometric measurements and ultrasonography to assess the CIMT. Blood and urine samples were collected for the biochemical measurements. Results: The serum uric acid levels were positively associated with obesity-and diabetes-related parameters and the CIMT. In a logistic regression model controlling for potential confounders, compared with the participants in the first quartile of the uric acid level, those in the fourth quartile had a higher odds of an elevated CIMT in both men (odds ratio [OR] = 1.37; 95% confidence interval [CI] = 1.07-1.75) and women (OR = 1.48; 95% CI = 1.12-1.94). The subgroup analyses revealed that an association between an elevated CIMT and the serum uric acid level persisted regardless of diuretic use and the hypertension, diabetes mellitus and chronic kidney disease status. However, the association disappeared in the patients who consumed alcohol and in premenopausal women. Conclusions: The serum uric acid level is positively associated with an elevated CIMT in middle-aged and elderly Chinese subjects, independent of known risk determinants of cardiovascular disease.

Abstract:Increased plasma homocysteine (Hcy) has been identified as one of the important risk factors for cardiovascular disease. However, the association between plasma Hcy and peripheral artery disease (PAD) is still controversial. This study aimed to investigate the association between plasma Hcy and PAD and the potential modifier factors in Chinese hypertensive adults. A total of 25 300 hypertensive patients aged 18 years or older were included in the analysis in this cross‐sectional study. The outcome was PAD, which defined as an ankle‐brachial index ≤0.90 in either limb. Multiple logistic regression was used to analyze the relationship between plasma Hcy and PAD. The median plasma Hcy was 14.00 (interquartile range: 11.60–17.80) μmol/L. There was a significant positive association between plasma Hcy and PAD (per SD increment; OR: 1.13; 95% CI: 1.06–1.19). Patients in the upper plasma Hcy tertile (≥16.16 μmol/L) were associated with a 53% increased risk of PAD compared with patients in the lower tertile (<12.33 μmol/L) after adjustment for multiple potential confounders. Subgroup analyses showed the association between Hcy and PAD was robust among various strata. Among Chinese adults with hypertension, plasma Hcy is an independent risk factor for PAD. This finding may improve the risk stratification of PAD.

Abstract:We aimed to investigate the prevalence of hyperhomocysteinaemia (total plasma homocysteine (tHcy) >= 10 mu mol/l) and its major determinants in rural Chinese hypertensive patients. A cross-sectional investigation was carried out in Lianyungang of Jiangsu province, China. This analysis included 13 946 hypertensive adults. The prevalence of hyperhomocysteinaemia was 51.6% (42.7% in women and 65.6% in men). The OR of hyperhomocysteinaemia were 1.52 (95% CI 1.39, 1.67) and 2.32 (95% CI 2.07, 2.61) for participants aged 55-65 and 65-75 v. 45-55 years; 1.27 (95% CI 1.18, 1.37) for participants with a BMI >= 25 v. <25 kg/m(2); 1.14 (95% CI 1.06, 1.23) for participants with v. without antihypertensive treatment; 1.09 (95% CI 1.00, 1.18) for residents inland v. coastal; 0.89 (95% CI 0.82, 0.97) and 0.83 (95% CI 0.74, 0.92) for participants with moderate and high v. low physical activity levels; 1.54 (95% CI 1.41, 1.68) and 2.47 (95% CI 2.17, 2.81) for participants with a glomerular filtration rate 60-90 and <60 v. >= 90 ml/min per 1.73 m(2); and 1.20 (95% CI 1.07, 1.35) and 3.81 (95% CI 3.33, 4.36) for participants with CT and TT v. CC genotype at methylenetetrahydrofolate reductase 677C. T polymorphism, respectively. Furthermore, higher tHcy concentrations were observed in smokers of both sexes (men: geometric mean 12.1 (interquartile range (IQR) 9.2-14.5) v. 11.9 (IQR 9.3-14.0) mu mol/l, P=0.005; women: geometric mean 10.3 (IQR 8.3-13.0) v. 9.6 (IQR 7.8-11.6) mu mol/l, P = 0.010), and only in males with hypertension grade 3 (v. grade 1 or controlled blood pressure) (geometric mean 12.1 (IQR 9.2-14.4) v. 11.7 (IQR 9.2-14.0), P = 0.016) and in male non-drinkers (yes v. no) (geometric mean 12.3 (IQR 9.4-14.8) v. 11.7 (IQR 9.1-13.9), P = 0.014). In conclusion, there was a high prevalence of hyperhomocysteinaemia in Chinese hypertensive adults, particularly in the inlanders, who may benefit greatly from tHcy-lowering strategies, such as folic acid supplementation and lifestyle change.
Serum Concentration of Uric Acid and Prehypertension among Chinese Population

Abstract:Objective To evaluate the association between serum uric acid (SUA) and prehypertension in Chinese population. Method A cross-sectional study was performed during 2005–2007 and a total of 11,199 participants without hypertension and other cardiovascular diseases, aged 35 years or older were available in our analysis. Multivariate logistic regress method was applied to estimate odds ratios (ORs) for prehypertension by quartiles of the serum uric acid value, with corresponding 95% confidence intervals (CIs) calculated using floating absolute risk method. Result The prevalence of prehypertension was 61.99% in the lowest quartile of serum concentration of uric acid, 62.47% in the second quartile, 64.67% in the third quartile, and 67.85% in the highest quartile (P for trend <0.0001). Linear regression analysis of uric acid levels in a model including 10 variables identified serum creatinine, systolic blood pressure (SBP), body mass index (BMI) and in women age, triglycerides and diastolic blood pressure (DBP) and in men fasting glucose as significant independent covariates whereby the largest contribution was generated by serum creatinine. After adjusting age, alcohol consumption, smoking status, BMI, diabetes, total cholesterol, triglyceride, serum creatinine, the ORs and 95% CIs of the prehypertension from the lowest (referent) to the highest levels of SUA were 1.00 (95% CI, 0.91–1.10), 1.12 (95% CI, 1.03–1.21), 1.17 (95% CI, 1.09–1.27), and 1.25 (95% CI, 1.13–1.39) (linear trend P=0.0016). This association persisted in subgroup analysis by gender and was also consistent with separately analysis by classification of age, smoking status, alcohol usage, overweight and diabetes mellitus. Conclusion Independently of other cardiovascular risk factors, higher serum uric acid levels are positive associated with prehypertension in Chinese general population without hypertension and other cardiovascular diseases. Our study further expands the role of uric acid in the development of hypertension. Prospective trials would be useful to evaluate interventions aimed at lowering the uric acid level.
Joint Association of Serum Homocysteine and High-Sensitivity C-Reactive Protein with Arterial Stiffness in Chinese Population: A 12-Year Longitudinal Study

Abstract:Background: Elevated plasma homocysteine (Hcy) and high-sensitivity C-reactive protein (hsCRP) levels are independent risk factors for cardiovascular diseases. However, it is unclear whether the coexistence of these conditions accelerates the risk of arterial stiffness. Our study aimed to evaluate the association of combined Hcy and hsCRP with arterial stiffness in Chinese middle-aged adults. Material/Methods: We conducted a 12-year longitudinal study in 220 individuals in Hanzhong, China, from 2005 to 2017. The average age at follow-up was 41.83 ± 3.10 years. Demographic information, medical history, anthropometric measurements, and blood pressure as well as urine and fasting blood samples, including Hcy, hsCRP, and brachial-ankle pulse wave velocity (baPWV) were measured and analyzed. Results: BaPWV levels showed a linear growth trend with the increasing of hsCRP (p for trend <0.01). The ORs in the highest quartile compared to the lowest quartile were 1.985 (95% CI 0.776–5.077; p = 0.152) and 3.960 (95% CI 1.468–10.684; p= 0.007) for Hcy and hsCRP, respectively. When Hcy and hsCRP were combined, subjects in both the highest quartile of Hcy and hsCRP (Hcy ≥15.50 μmol/L and hsCRP ≥0.82 μmol/L) had a 12.68-fold increased risk of developing arterial stiffness at the 12-year follow-up compared to those in the lowest quartile of Hcy and hsCRP (Hcy ≤9.91 μmol/L and hsCRP ≤0.19 μmol/L) after adjusting for potential confounders. Conclusions: The present study demonstrated that the combination of elevated serum Hcy and hsCRP may contribute to an increased risk of arterial stiffness.
