Abstract:Objective:To analysis the clinical characteristics of The clinical characteristics of using automated external defibrillation in the public place,To explore the feasibility and effectiveness of AED application in public places in China.Methods:From January 2014 to April 5, 2019, 54 cases of on-site emergency medical records of AED use in public places in China were analyzed retrospectively from three aspects: patient and AED user attributes, and AED clinical performance.Results:After field application of AED analysis, 54 patients did not have out of hospital cardiac arrest in 9 patients; cardiac arrest in 45 patients, cerebral resuscitation in 40 patients (88.9%), death in 5 patients (11.1%), one of them died in hospital. The accuracy of AED for defibrillation rhythm recognition and defibrillation recommendations was 100%. The success rate of shock to VF was 97.22%, and that of non pulse VT was 100%. The data shows that AEDs of different brands show clinical effectiveness in the core indicators of work. The operation level of the rescuer determines the critical time of AED shock, which is closely related to the prognosis of the patient ( P<0.05) . Conclusions:AED is reliable and effective in electric shock decision and performance.The overall efficiency of AED application can be improved by strengthening training, shortening the critical time of electric shock, rational configuration and effective management.

Abstract:The Hispanic population is growing rapidly and is composed of individuals from many countries with varying levels of acculturation, education, income, and citizenship status. The Genesis Health and Empowerment Program was developed locally in 1993 to improve the health status of Hispanics living in Des Plaines, Illinois, USA. Understanding the quality of life and its cultural patterning for the community is an essential aspect of planning and implementing a health care delivery program. Using Leininger's Theory of Culture Care: Diversity and Universality as a framework, adapted the Concerns Report Method was essential for data collection. This paper describes the method used for learning about the Hispanic community. Steps included conducting three focus groups, constructing a structured interview guide, collecting data, analyzing data, and reporting the findings to the community. Some very preliminary findings are presented and implications for transcultural health care are described.

Abstract:Abstract Public access defibrillation initiatives make automated external defibrillators available to the public. This facilitates earlier defibrillation of out-of-hospital cardiac arrest victims and could save many lives. It is currently only used for a minority of cases. The aim of this systematic review was to identify barriers and facilitators to public access defibrillation. A comprehensive literature review was undertaken defining formal search terms for a systematic review of the literature in March 2017. Studies were included if they considered reasons affecting the likelihood of public access defibrillation and presented original data. An electronic search strategy was devised searching MEDLINE and EMBASE, supplemented by bibliography and related-article searches. Given the low-quality and observational nature of the majority of articles, a narrative review was performed. Sixty-four articles were identified in the initial literature search. An additional four unique articles were identified from the electronic search strategies. The following themes were identified related to public access defibrillation: knowledge and awareness; willingness to use; acquisition and maintenance; availability and accessibility; training issues; registration and regulation; medicolegal issues; emergency medical services dispatch-assisted use of automated external defibrillators; automated external defibrillator-locator systems; demographic factors; other behavioural factors. In conclusion, several barriers and facilitators to public access defibrillation deployment were identified. However, the evidence is of very low quality and there is not enough information to inform changes in practice. This is an area in urgent need of further high-quality research if public access defibrillation is to be increased and more lives saved. PROSPERO registration number CRD42016035543.
Kinetic analysis of endocytosis and intracellular fate of liposomes in single macrophages.

Abstract:Endocytosis and the intracellular fate of liposomes in single mouse peritoneal macrophages were examined kinetically by fluorescence microphotometry. Liposomes labeled with N-(7-nitro-2,1,3-benzoxadiazol-4-yl)phosphatidylethanolamine or containing 8-amino-naphthalene-1,3,6-trisulfonate were promptly incorporated into macrophages on incubation at 37 degrees C, but fluorescence increase caused by hydrolysis of 4-methylumbelliferyl-beta-D-glucoside encapsulated in the liposomes was observed after 30 min of incubation. The fluorescences of calcein and 8-hydroxy-1,3,6-pyrenetrisulfonate (HPTS) in liposomes, which were respectively quenched statically due to high concentration and dynamically by a co-entrapped fluorescence quencher, p-xylene-bis-pyridinium bromide, also increased from 30 min after the start of liposome incorporation, indicating that macrophages require this period for intracellular delivery of liposomes from the cell surface to lysosomes. Measurement of the intraendosomal pH change in a single macrophage at 37 degrees C with liposomes containing a pH-sensitive fluorescent marker, HPTS, showed that the pH value decreased continuously to a constant value of 5.5 in 30-40 min after endocytosis, and this decrease was reversed on addition of NH4Cl, suggesting that acidification of endosomes is not a stepwise reaction and is coupled with delivery of liposomes. These fluorescence microphotometric systems using liposomes containing different fluorescent dyes should be useful for kinetic analyses of the endocytosis and intracellular fate of liposomes in various phagocytes.

Abstract:Purpose of review: Survival statistics for out-of-hospital cardiac arrests remain unsatisfactory. Prompt defibrillation of shockable rhythms, such as ventricular fibrillation and pulseless ventricular tachycardia, is crucial for improving survival. Automated external defibrillators (AEDs) and wearable cardiac defibrillators (WCDs) seek to improve the survival rates following out-of-hospital cardiac arrests. We aim to review the indications, utility, advancements, and limitations of AEDs and WCDs, as well as their role in contemporary and future clinical practice. Recent findings: Recent advancements in these technologies, such as smartphone applications and drone delivery of AEDs and less inappropriate shocks and decreased size of WCDs, have increased their ubiquity and efficacy. However, implementation of this technology remains limited due to lack of resources and suboptimal patient adherence. Summary: Out of hospital cardiac arrests continue to pose a significant public health challenge. Advancements in AEDs and WCDs aim to facilitate prompt defibrillation of shockable rhythms with the goal of improving survival rates. However, they remain underutilized due to limited resources and suboptimal patient adherence. As these technologies continue to evolve to become smaller, lighter and more affordable, their utilization and accessibility are expected to improve.
[Early defibrillation in the community: breaking barriers to save lives]

Abstract:It is considered that in Spain, every year, we have more than 24,500 out-of-hospital cardiac arrests. Around 85% of these are secondary to ventricular fibrillation, with possibility of reversion in more than 90% if defibrillation is performed in the first minute of arrhythmia. However, if we delay this defibrillation, survival possibilities disappear in a few minutes. Clinical advances in last decades have not achieved satisfactory results in the treatment of cardiac arrest as survival rates at hospital discharge do not exceed 7%. Aware of this situation, the International Scientific Societies are recommending decreasing time to defibrillation, advising, at best, a time less than five minutes between the 112-call (emergency) and adequate electric discharge. Development of automated defibrillators in Emergency Medical Systems and their use by <<first responders>> of <<non-health care>> emergency services (police, fire fighters, etc) contribute to reach this objective. Because of this, Emergency Medical Systems are modifying their assistance strategies, to implement the early defibrillation as <<key to survival>>. Literature showed the effective value of automated defibrillators in the public areas but their efficiency level is less than that reached with the Emergency Services. Efficiency depends on multiple factors such as type of installation, accessibility level to emergency medical services or incidence rate of sudden cardiac arrest. Thus, their introduction should be preceded by a cost-effectiveness study. Effectiveness of automated defibrillators at home, where up to 80% of cardiac arrest are produced, has still not been evaluated. Nevertheless, in the USA, its marketing with this indication has been authorized.

Abstract:According to the "Chinese Cardiovascular Health and Disease Report" published by the National Health Commission, sudden cardiac arrest (SCA) affects approximately 544,000 people in China each year, with 75% of cases occurring outside of the hospital setting, such as in homes or public places. [ 1 ] Unfortunately, many out-of-hospital cardiac arrest (OHCA) patients suffer permanent disability or death due to the lack of timely assistance or improper rescue efforts, underscoring the importance of prompt and accurate on-site emergency treatment over hospital treatment. Statistics have shown that 85% to 90% of OHCA patients experience ventricular fibrillation (VF) at the onset, and defibrillation is the fastest and most effective way to terminate VF. However, the effectiveness of defibrillation is time-sensitive, as every minute of delay in defibrillation reduces the survival rate of OHCA patients by 7% to 10%. [ 2 ] If bystanders can use automatic external defibrillators (AEDs) to rescue patients in a timely manner, the success rate of rescuing OHCA patients would significantly increase. Reports indicate that the survival rate of patients with shockable rhythms is 38%, while the survival rate of patients with non-shockable rhythms is 9%. [ 3 ] Recognizing the importance of AED usage, the American Heart Association (AHA) passed legislation in 1995 promoting public access defibrillation (PAD), which encourages the use of AED by the general public in public places. However, research shows that different countries have varying degrees of PAD implementation, with less than 3% of patients receiving defibrillation before the arrival of emergency medical services (EMS), and China having a lower implementation rate of less than 1%. [ 4 , 5 ] The knowledge and willingness of the general public to perform cardiopulmonary resuscitation (CPR) and defibrillation in a timely manner are crucial factors that affect the effectiveness of first aid before professional medical personnel intervene. Therefore, this study aims to investigate the knowledge and willingness of AED use among the general public in Hubei Province, analyze the factors that influence AED knowledge and willingness, and provide scientific guidance to increase awareness of AED usage and improve the implementation rates of PAD treatments in the public. By understanding these factors, appropriate measures can be taken to enhance public engagement and improve the overall response to cardiac emergencies.

Abstract:Introduction Public access defibrillation (PAD) prior to ambulance arrival is a key determinant of survival from out-of-hospital (OOH) cardiac arrest. Implementation of PAD has been underway in the UK for the past 12 years, and its importance in strengthening the chain of survival has been recognised in the government's recent ‘Cardiovascular Disease Outcomes Strategy’. The extent of use of PAD in OOH cardiac arrests in the UK is unknown. We surveyed all OOH cardiac arrests in Hampshire over a 12-month period to ascertain the availability and effective use of PAD. Methods A retrospective review of all patients with OOH cardiac arrest attended by South Central Ambulance Service (SCAS) in Hampshire during a 1-year period (1 September 2011 to 31 August 2012) was undertaken. Emergency calls were reviewed to establish the known presence of a PAD. Additionally, a review of all known PAD locations in Hampshire was undertaken, together with a survey of public areas where a PAD may be expected to be located. Results The current population of Hampshire is estimated to be 1.76 million. During the study period, 673 known PADs were located in 278 Hampshire locations. Of all calls confirmed as cardiac arrest (n=1035), the caller reported access to an automated external defibrillator (AED) on 44 occasions (4.25%), successfully retrieving and using the AED before arrival of the ambulance on only 18 occasions (1.74%). Conclusions Despite several campaigns to raise public awareness and make PADs more available, many public areas have no recorded AED available, and in those where an AED was available it was only used in a minority of cases by members of the public before arrival of the ambulance. Overall, a PAD was only deployed successfully in 1.74% OOH cardiac arrests. This weak link in the chain of survival contributes to the poor survival rate from OOH cardiac arrest and needs strengthening.

Abstract:In their cohort study in this issue of JAMA Internal Medicine , Kolkailah et al 1 found that rates of bystander automated external defibrillator (AED) use were similarly low in both states that mandate and those that do not mandate such devices in recreational facilities: 19% vs 18%. The fact that cardiac arrests with ventricular tachycardia or ventricular fibrillation rhythms have significantly better odds of survival when early cardiopulmonary resuscitation is performed has likely motivated policies to expand AED availability. 2 However, availability alone does not guarantee appropriate use by bystanders.

Abstract:Currently every day in the USA 1000 people die of sudden cardiac arrest (SCA) outside of hospitals or ambulances - before emergency medical help arrives - in the streets, workplaces, schools and homes of our cities, adults and children. Brain death commences in 3 minutes, and often the ambulance just can't be there in time. Citizen cardiopulmonary resuscitation (CPR) and automated external defibrillator (AED) use can save precious minutes and lives. Using public access AED's saves lives in SCA- however AEDs are used in <2% of cardiac arrests, though could save lives in 80% if available, findable, functioning, and used. The systems problem to solve is that there is no comprehensive or real time accessible database of the AED locations, and also it is not known that they are actually being positioned where they are needed. The iRescU project is designed to bridge this gap in SCA survival, by substantially augmenting the AED database. Utilizing a combination of AED crowd sourcing and geolocation integrated with existing 911 services and SCA events and projected events based on machine learning data information to help make the nearest AED accessible and available in the setting of a SCA emergency and to identify the areas of greatest need for AEDs to be positioned in the community. Helping to save lives and address preventable death with a social good approach and applied big data.

Abstract:Background Sudden cardiac death is the leading cause of death in athletes. Evidence on current sudden cardiac death prevention through preparticipation history, physicals, and noninvasive cardiovascular diagnostics has demonstrated a low sensitivity for detection of athletes at high risk of sudden cardiac death. Data are lacking on automated external defibrillator programs specifically initiated to respond to rare dysrhythmia in younger, relatively low-risk populations. Methods Surveys were mailed to the head athletic trainers of all National Collegiate Athletic Association Division I athletics programs listed in the National Athletic Trainers’ Association directory. In all, 303 surveys were mailed; 186 departments (61%) responded. Results Seventy-two percent (133) of responding National Collegiate Athletic Association Division I athletics programs have access to automated external defibrillator units; 54% (101) own their units. Proven medical benefit (55%), concern for liability (51%), and affordability (29%) ranked highest in frequency of reasons for automated external defibrillator purchase. Unit cost (odds ratio = 1.01; 95% confidence interval, 1.01-1.0), donated units (odds ratio = 1.92; confidence interval, 3.66-1.01), institution size (odds ratio = .0001; confidence interval, 1.3 E-4 to 2.2E-05), and proven medical benefit of automated external defibrillators (odds ratio = 24; confidence interval, 72-8.1) were the most significant predictors of departmental defibrillator ownership. Emergency medical service response time and sudden cardiac death event history were not significantly predictive of departmental defibrillator ownership. The majority of automated external defibrillator interventions occurred on nonathletes. Conclusions Many athletics medicine programs are obtaining automated external defibrillators without apparent criteria for determination of need. Usage and maintenance policies vary widely among departments with unit ownership or access. Programs need to approach the issue of unit acquisition and implementation with knowledge of the surrounding emergency medical service system, geography of their individual sports medicine facilities, numbers and relative risk of their athletes, and budgetary constraints.
The General Public's Ability to Operate Automated External Defibrillator: A Controlled Simulation Study.

Abstract:BACKGROUND:Automated external defibrillators (AEDs) enable laypeople to provide early defibrillations to patients undergoing cardiac arrest, but scant information is available on the general public's ability to use AEDs. This study assessed the ability of laypeople to operate AEDs, the effect of a 15-minute training, and whether skills differed by age.METHODS:From May 1 to December 31, 2018, a prospective simulation study was conducted with 94 laypeople aged 18-65 years (32 aged 18-24 years, 34 aged 25-54 years, and 28 aged 55-65 years) with no prior AED training. The participants' AED skills were assessed individually pre-training, post-training, and at a three-month follow-up using a simulated cardiac arrest scenario. The critical actions and time intervals were evaluated during the AED operating process.RESULTS:Only 14 (14.9%) participants (eight aged 18-24 years, four aged 25-54 years, and two aged 55-65 years) successfully delivered defibrillations before training. AED operation errors were more likely to occur among the participants aged 55-65 years than among other age groups. After training, the proportion of successful defibrillations increased significantly (18-24 years old: 25.0% vs. 71.9%, P<0.01; 25-54 years old: 11.8% vs. 70.6%, P<0.01; 55-65 years old: 7.1% vs. 67.9%, P<0.01). After three months, 26.1% of the participants aged 55-65 years successfully delivered defibrillations, which was significantly lower than that of participants aged 18-24 years (54.8%) and 25-54 years (64.3%) (P=0.02). There were no differences in time measures among three age groups in each test.CONCLUSIONS:The majority of untrained laypeople cannot effectively operate AEDs. More frequent training and refresher courses are crucial to improve AED skills.

Abstract:Introduction: Out of hospital sudden cardiac arrest is a major health concern with a low survival rate. Proper Automated External Defibrillator use has been shown to decrease mortality associated with OHCA. In 2014, New Jersey passed Janet’s Law to mandate automated external defibrillator access in all public and private schools. Our project surveyed NJ schools to evaluate the implementation of AEDs and to analyze their usage and outcomes. Hypothesis: Schools that were adherent to AED placement and training as outlined in Janet’s Law will likely have more positive outcomes from sudden cardiac arrests on school properties than schools that do not. Methods: An observational, cross sectional study was conducted via an online form (Redcap 12.4.2) sent to New Jersey public school nurses. Data was collected from February to June 2023 and analyzed using SAS version 9.4. Results: 301 (99.7%) responses reported having automated external defibrillators. 85.3% of schools provided AED staff training. 42 schools (14.4%) utilized an AED in a cardiac emergency, most often occurring to students in classrooms. An AED was administered by school employees 95.8% of the time. In 80.5% of cases, the patient was resuscitated on scene with no long term neurological complications. 87.2% of schools have an emergency response plan. Conclusions: Our data highlights the impact of proper AED utilization to positively influence out of hospital sudden cardiac arrest outcomes in pediatric populations. Despite Janet’s law being an unfunded mandate, we found near complete compliance among New Jersey schools. The majority of reported sudden cardiac arrests occurred in a classroom setting, necessitating available resources throughout school facilities. In these events, the percentage of positive patient outcomes in our study was significantly higher than the national average with excellent neurological recovery. This discovery may be a product of excellent response rates by trained staff as mandated in Janet’s law. Our findings highlight the need for awareness, training, and accessibility regarding AEDs.

Abstract:BACKGROUND:Public access of automated external defibrillator (AED) is an important public health strategy for improving survival of cardiac arrest. Major metropolitan cities in China are increasingly investing and implementing public access defibrillator programs, but the effectiveness of these programs remains unclear. This study aims to evaluate the public accessibility of AED in Shanghai, a major metropolitan city in China.METHODS:From July 1 to September 30, 2018, all AED locations indicated by AED Access Map Apps were visited and investigated in three most densely distributing areas of AED (Huangpu District, Xuhui District, and Central Area of the Pudong New District) in Shanghai. Two AED Access Map APPs were used to identify the location of AEDs. Characteristics of and the barriers to access, the AED sites were recorded. Awareness and skills of first aid and AED among on-site staff of the AED installation sites were evaluated.RESULTS:A total of 283 sites were marked on two AED Apps. One hundred and seventy (60%) locations were accessible, and 142 (50%) were actually with AEDs installed. Among those AED installed sites, 112 (79%) were completely identifiable to the information on the maps, 20 (14%) were inconsistent and 10 (7%) were inaccurate on the maps. Ninety-four (66%) AEDs had visible signs and information around the location, 7 (5%) AEDs had signs outside of the location, and 107 (75%) sites had educational instructions. In addition, 230 individuals who were around the AED site were interviewed. Among them, 79 (34%) had good knowledge of AED. After shown the picture of AED, 112 (49%) knew whether there was AED in the site, and 108 (47%) knew the AED's location. Eighty-seven (38%) staff have received first aid training, and among them 26 (30%) reported that they had skills in operating the AED.CONCLUSIONS:Public placement and accessibility of AEDs, related public signs and information on AED, and staff's awareness about AED were not optimal in Shanghai. Continuing efforts should be made to improve public accessibility and public awareness, knowledge, and user skills of AED.

Abstract:Objective: To assist high school and college athletic programs prepare for and respond to sudden cardiac arrest (SCA). This consensus statement summarizes our current understanding of SCA in young athletes, defines the necessary elements for emergency preparedness, and establishes uniform treatment protocols for the management of SCA. Background: SCA is the leading cause of death in young athletes. The increasing presence of and timely access to automated external defibrillators (AEDs) at sporting events provides a means of early defibrillation and the potential for effective secondary prevention of sudden cardiac death. An Inter-Association Task Force was sponsored by the National Athletic Trainers' Association to develop consensus recommendations on emergency preparedness and management of SCA in athletes. Recommendations: Comprehensive emergency planning is needed for high school and college athletic programs to ensure an efficient and structured response to SCA. Essential elements of an emergency action plan include establishing an effective communication system, training of anticipated responders in cardiopulmonary resuscitation and AED use, access to an AED for early defibrillation, acquisition of necessary emergency equipment, coordination and integration of onsite responder and AED programs with the local emergency medical services system, and practice and review of the response plan. Prompt recognition of SCA, early activation of the emergency medical services system, the presence of a trained rescuer to initiate cardiopulmonary resuscitation, and access to early defibrillation are critical in the management of SCA. In any collapsed and unresponsive athlete, SCA should be suspected and an AED applied as soon as possible for rhythm analysis and defibrillation if indicated.
Identification of priority areas for public-access automated external defibrillators (AEDs) in metropolitan areas: A case study in Hangzhou, China

Abstract:Out-of-hospital cardiac arrest (OHCA) is a major health concern in China. The use of automated external de-fibrillators (AEDs) has received great attention from Chinese public healthcare departments as a useful inter-vention to address OHCA, and there is a growing need to place AEDs throughout the urban built environment. However, due to the lack of a publicly available, geocoded dataset of historical OHCA cases, there is not a coherent approach for identifying priority areas for AEDs placement in this country. This study attempts to fill this gap by examining the spatial disparity of OHCA risk in Hangzhou, China, and then proposing a Geographic Information Systems(GIS) based model that incorporates the urban land cover, vulnerable groups, and trans-portation accessibility to identify priority areas at the hexagon level for future AEDs placement. Results show that: (1) the hexagons with the highest level of OHCA risk (ranked Group 1) are accounting for 9.87%-16.71% and are spatially dispersed. (2) A large high-risk cluster was identified in the western region of the Qiantang district. (3) Nearly one-fifth of the new AEDs are recommended to install in Phase I, more than 60% in Phase II, and the remainder in Phase III. (4) Xiaoshan district requires the most AEDs, while Linping district requires the fewest. Ultimately, these results guide future placement of the AED facilities in the cities, particularly those with no geocoded OHCA historical data.
