Abstract:More than 20 years ago, a report on unequal treatment by the Institute of Medicine 1 made several recommendations to address racial and ethnic health disparities; chief among them was ensuring accurate and complete race and ethnicity data to identify disparities and inform interventions. Since then, electronic health records (EHRs) have become one of the main sources of population-level race and ethnicity data used for research, quality improvement efforts, and resource allocation for equitable interventions. However, despite its importance, numerous studies have demonstrated that EHR race and ethnicity data are incomplete and inaccurate, especially for minoritized populations at the highest risk for health disparities. 2 Salhi et al 3 add to this growing body of literature with their retrospective cohort study. Using a statewide clinical registry, they measured the frequency of discordant EHR documentation of race and ethnicity for patients with at least 2 emergency department visits to the same hospital or health system between December 1, 2018, and November 30, 2021. Discordance was defined as a change in race and ethnicity categorization across visits. In their large sample of 403 587 adult and 121 839 pediatric patients, they found that the rate of discordance was 1.7% for adults and 7.0% for children. 3 The process of race and ethnicity data collection was unknown, and authors suggested that the discordance observed could be due to variability in data collection. The authors state that this level of discordance may widen disparities or limit the ability to detect them. This may have downstream effects and highlights the need for best practices in race and ethnicity data collection. The study by Salhi et al 3 is one of only a few studies that has looked specifically at EHR discordance across emergency department visits and stratified for both adult and pediatric data. Most studies on EHR discordance have been conducted among adults and compared EHR data with self-reported surveys; pediatric studies have typically had smaller sample sizes or have been from single clinical sites. Sahil et al 3 found higher rates of discordance for children vs adults, which is not surprising considering there is a lack of guidelines and consistent practices in demographic data collection among pediatric populations. 4 In pediatrics there are unique challenges to address: Should the data be provided by a caregiver or child? What age is appropriate for individuals to self-report? How do we reconcile discordance among caregivers or between caregiver and the child? It may be helpful to have a designated area in the EHR to document who provided the data (eg, self, caregiver, or staff observation). Also, because multiracial children are one of the largest growing racial groups in the US, rendering clear data collection processes that have expansive race and ethnicity categories is very important. The complexity of collecting race and ethnicity data can be used to understand how discordance may occur. Overall, these complexities can be distilled into the 5 Ws (who, what, where, when, and why): who collects and provides the data, what race and ethnicity categories are available to choose from, when are data collected, where are data collected, and making clear to patients why data are collected and what will be done with them. Of note, the Salhi et al 3 study was conducted on data before recent calls to more clearly describe the processes used for race and ethnicity data collection from funding agencies, health systems, and studies published in medical journals. For example, the National Committee on Quality Assurance highlighted the issue of discordance from multiple data sources and developed guiding principles on how to prioritize sources of truth and the need for data management systems to allow documentation of how data are collected to better inform reconciliation processes. 5 In addition, the Centers for Medicare & Medicaid Services, the biggest payer of health care in the US, has made expanding the collection, reporting, and analysis of standardized data, including race and ethnicity, 1 of their 5 priority areas in their Framework for Health Equity 2022-2032. 6 Because minoritized groups are more likely than majority groups to have incomplete and inaccurate data, Centers for Medicare & Medicaid Services and others use various imputation methods to indirectly approximate data on the population level, but there is a tension between using those analytic methods over focusing on the more time-intensive and person-centered approach of improvement efforts to consistently implement the criterion standard of self-reported race and ethnicity in health care spaces. 5 ,6 Our society's understanding of race and ethnicity as fluid, socially constructed identit -Abstract Truncated-

Abstract:Introduction Home healthcare (HHC) enables patients to receive healthcare services within their homes to manage chronic conditions and recover from illnesses. Recent research has identified disparities in HHC based on race or ethnicity. Social determinants of health (SDOH) describe the external factors influencing a patient's health, such as access to care and social support. Individuals from racially or ethnically minoritized communities are known to be disproportionately affected by SDOH. Existing evidence suggests that SDOH are documented in clinical notes. However, no prior study has investigated the documentation of SDOH across individuals from different racial or ethnic backgrounds in the HHC setting. This study aimed to (1) describe frequencies of SDOH documented in clinical notes by race or ethnicity and (2) determine associations between race or ethnicity and SDOH documentation. Design Retrospective data analysis. Methods We conducted a cross‐sectional secondary data analysis of 86,866 HHC episodes representing 65,693 unique patients from one large HHC agency in New York collected between January 1, 2015, and December 31, 2017. We reported the frequency of six SDOH (physical environment, social environment, housing and economic circumstances, food insecurity, access to care, and education and literacy) documented in clinical notes across individuals reported as Asian/Pacific Islander, Black, Hispanic, multi‐racial, Native American, or White. We analyzed differences in SDOH documentation by race or ethnicity using logistic regression models. Results Compared to patients reported as White, patients across other racial or ethnic groups had higher frequencies of SDOH documented in their clinical notes. Our results suggest that race or ethnicity is associated with SDOH documentation in HHC. Conclusion As the study of SDOH in HHC continues to evolve, our results provide a foundation to evaluate social information in the HHC setting and understand how it influences the quality of care provided. Clinical Relevance The results of this exploratory study can help clinicians understand the differences in SDOH across individuals from different racial and ethnic groups and serve as a foundation for future research aimed at fostering more inclusive HHC documentation practices.

Abstract:Objective We assessed the concordance of patient-reported race and ethnicity for emergency department (ED) patients compared with what was recorded in the electronic health record. Methods We conducted a single-center, prospective, observational study of 744 ED patients (English- and/or Spanish-speaking), asking them to describe their race and ethnicity. We compared the distributions of ethnicity and race between patient-reported and electronic health record data using McNemar's test. We calculated percent agreement and Cohen's kappa, with 95% confidence intervals (CI), for the concordance of patient-reported race and ethnicity with electronic health record data. Results Of 744 ED patients, 731 participants who completed the survey reported their ethnicity, resulting in 98.2% of electronic health records obtained ethnicities matched self-reported data (kappa = 0.95; 95% CI: 0.92 to 0.98). For those who self-reported as Hispanic, only 92.3% agreement was observed between the self-reported and electronic health record values. For all patients who had race recorded, 85.4% agreement was observed (kappa = 0.75; 95% CI 0.71 to 0.79). High rates of agreement were observed for Black or African American patients (98.7%) and White patients (96.6%), with low rates for those who identified as "More than one race" (22.9%) or "Other" race (1.8%). In the subset of Hispanic patients, low rates of agreement (25.0%) were observed for race (kappa = 0.10; 95% CI 0.01 to 0.19). Conclusions Documentation discordance regarding race and ethnicity exists between electronic health records and self-reported data for our ED patients, particularly for ethnically Hispanic and Latino/a patients. Future efforts should focus on ensuring that demographic information in the electronic health record is accurately collected.

Abstract:Patient race/ethnicity data collection in most U.S. health systems abide by federal standards, determined by the federal Office of Management and Budget. Yet, decades of research show that reliance on these categories alone limits understanding of within-group health disparities, systematically erasing key groups from health data. Because granular race/ethnicity data is complex and patients may be hesitant to disclose this personal information, it is important for health leaders to consider community perspectives when making decisions about race/ethnicity data procedures. As such, this study uses community focus groups to understand: (1) how individuals representing different racial/ethnic identities perceive the collection of race/ethnicity in healthcare settings; (2) differences in opinions between disaggregated race/ethnicity data collection instruments and those using federal standards; and (3) recommended practices for collecting race/ethnicity from patients. Participants self-selected into 13 focus groups and one key informant interview based on the race/ethnicity with which they most closely identified. Audio recordings from these groups were transcribed and evaluated using thematic content analysis. Among the 83 total participants in this study, there was a strong preference for more flexible and specific options for self-identifying race/ethnicity in healthcare settings. Participants also felt comfortable disclosing granular race/ethnicity to health providers but expressed discomfort with disclosing this information for other purposes. Recommendations for healthcare leaders include ensuring patients receive detailed communication about race/ethnicity data use and purpose, allowing multiple category selection, keeping the list of disaggregated response options short so as to not overwhelm patients, and providing a free text option to ensure inclusivity.

Abstract:Despite recent therapeutic advances, ethnic minorities in the U.S. continue to have disproportionately poor outcomes in many hematologic malignancies such as AML. We identified 162 adult AML patients treated at a non-transplant safety net hospital from 2007 to 2022 and evaluated differences in disease characteristics, treatment and clinical outcomes based on race and ethnicity. Our cohort consisted of 82 (50.6%) Hispanic, 36 (22.2%) non-Hispanic black and 44 (27.2%) non-Hispanic white and Asian patients. Median age at diagnosis was 42.5, 49.0 and 52.5 years respectively (p=0.025). Hispanics had higher rates of intermediate and high-risk disease (p=0.699) and received high intensity induction and consolidation chemotherapy at lower rates (p=0.962), although differences did not reach statistical significance. Despite this, similar remission rates were achieved. Hispanics with high-risk disease had longer overall survival (OS) than the combined non-Hispanic cohort (mOS 14 m vs 7 m, p=0.030). Multivariate regression analysis showed that OS was negatively associated with age (HR 1.023, p=.006), intermediate (HR 3.431, p=.0003) and high-risk disease (HR 4.689, p<0.0001) and positively associated with Hispanic ethnicity (HR 0.614, p=.026). This report suggests that contrary to other studies, Hispanics, particularly those with high-risk AML, may have improved OS compared to other ethnic groups. These results are unique to our safety net hospital setting where common barriers to medical care and healthcare disparities are largely mitigated.

Abstract:Personal implicit biases may contribute to inequitable health outcomes, but the mechanisms of these effects are unclear at a system level. This study aimed to determine whether stigmatizing subjective terms in electronic medical records (EMR) reflect larger societal racial biases. A cross-sectional study was conducted using natural language processing software of all documentation where one or more predefined stigmatizing words were used between January 1, 2019 and June 30, 2021. EMR from emergency care and inpatient encounters in a metropolitan healthcare system were analyzed, focused on the presence or absence of race-based differences in word usage, either by specific terms or by groupings of negative or positive terms based on the common perceptions of the words. The persistence ("stickiness") of negative and/or positive characterizations in subsequent encounters for an individual was also evaluated. Final analyses included 12,238 encounters for 9135 patients, ranging from newborn to 104 years old. White (68%) vs Black/African American (17%) were the analyzed groups. Several negative terms (e.g., noncompliant, disrespectful, and curse words) were significantly more frequent in encounters with Black/African American patients. In contrast, positive terms (e.g., compliant, polite) were statistically more likely to be in White patients' documentation. Independent of race, negative characterizations were twice as likely to persist compared with positive ones in subsequent encounters. The use of stigmatizing language in documentation mirrors the same race-based inequities seen in medical outcomes and larger sociodemographic trends. This may contribute to observed healthcare outcome differences by disseminating one's implicit biases to unknown future healthcare providers.

Abstract:e13733 Background: Racial disparities in healthcare refer to differences in health outcomes, access to healthcare &quality of care among racial or ethnic groups. Aim of study is to investigate the characteristics and clinical outcomes of racial differences in patients admitted with ALL. Methods: We queried the National Inpatient Sample between 2017-2020 for adult patients of different races who were hospitalized with ALL. The primary outcome was inpatient mortality; secondary outcomes were cardiac arrest, invasive mechanical ventilation, length of stay & hospital cost. Multivariable logistic, linear & Poisson regression analyses were used to estimate clinical outcomes with p-value < 0.05 was significant. Results: Total 110999 hospitalizations with ALL. Mean age 46 years & 55.8% male. Among admitted, 59599 (53.7 %) were Caucasian, 9205 (8.3 %) were Black, 27565 (24.8%) were Asian, 4540 (4.1%) were Hispanic, and 10090 (9.1%) were others. Subgroup with Caucasian, Blacks, Asians, Hispanics, and other races had Medicare 34.9% vs 30.2% vs 13.5% vs 17.9% vs 12.5%, Medicaid 13.3% vs 29.7% vs 41.4% vs 19.2 vs 34.8%, Non-insured 2.1% vs 5.1% vs 10.6% vs 4.2% vs 7.7%; DM 19% vs 23.8% vs 24.6% vs 17.4% vs 20.7%; Protein calorie malnutrition 11 % vs 10.6% vs 10.1% vs 12.6% vs 10.2%, respectively. All-cause mortality was 3810, where subgroup with Caucasian, Black, Asian, Hispanic, and others were 2085 (3.5%) vs 410 (4.4%) vs 885 (3.2%) vs 145 (3.2%) vs 285 (2.8%). Conclusions: Among the racial categories, Caucasians comprised the largest portion followed by Asians, Blacks, Hispanics. Mortality and intubation proportion was highest among Blacks followed by Hispanics & Asians. Cardiac arrest proportion was highest among Blacks followed by Hispanics and Asians. LOS was highest among Hispanics followed by Asians & Blacks. Resource utilization was highest in Hispanics followed by Asians & Blacks. Understanding these disparities is essential for improving healthcare equity and ensuring that all patients receive appropriate & equitable care regardless of their racial/ethnic background. Abstract:Context: Sociological research has linked racism and discrimination among children to poorer health outcomes and social conditions later in life. Objectives: Given the change in the political climate in the United States, highly publicized deaths of Black men and women by police, and the rise in hate crimes against Asian Americans from 2016 through 2020, our primary objective was to assess trends in racial or ethnic discrimination among children in the United States. Methods: We conducted a cross-sectional analysis of the National Survey of Children's Health (NSCH), a nationally representative survey, utilizing data from 2016 to 2020. We calculated yearly population estimates of whether a child had experienced discrimination based on race/ethnicity via a parent-reported item. We further divided the estimates by race/ethnicity and plotted linear trends over time. Results: Data from the NSCH show that racial/ethnic discrimination reported by parents of children who are minorities increased from 6.7% in 2016 to approximately 9.3% in 2020. Indigenous children were reported to experience discrimination at high rates ranging from 10.8% in 2016 to 15.7% in 2020, as well as Black children ranging from 9.69% in 2018 to 15.04% in 2020. The percent of Asian, Hawaiian or Pacific Islander, and Hispanic children reported to have experience discrimination was between 4.4 and 6.8% during this time. Conclusions: Discrimination negatively impacts the developmental experiences of children, disproportionately affecting those identifying as Indigenous and Black. Therefore, addressing harmful stereotyping of Indigenous and Black cultures is necessary, especially in media targeted toward children. Providing culturally competent healthcare, critically in the Indigenous and Black pediatric population, may improve long-term outcomes by reducing discriminatory barriers to healthcare access.

Abstract:INTRODUCTION:Early reports highlighted racial/ethnic disparities in the severity of COVID-19 seen across the USA; the extent to which these disparities have persisted over time remains unclear. Our research objective was to understand temporal trends in racial/ethnic variation in severity of COVID-19 illness presenting over time. METHODS:We conducted a retrospective cohort analysis using longitudinal data from Cedars-Sinai Medical Center, a high-volume health system in Southern California. We studied patients admitted to the hospital with COVID-19 illness from 4 March 2020 through 5 December 2020. Our primary outcome was COVID-19 severity of illness among hospitalised patients, assessed by racial/ethnic group status. We defined overall illness severity as an ordinal outcome: hospitalisation but no intensive care unit (ICU) admission; admission to the ICU but no intubation; and intubation or death. RESULTS:A total of 1584 patients with COVID-19 with available demographic and clinical data were included. Hispanic/Latinx compared with non-Hispanic white patients had higher odds of experiencing more severe illness among hospitalised patients (OR 2.28, 95% CI 1.62 to 3.22) and this disparity persisted over time. During the initial 2 months of the pandemic, non-Hispanic blacks were more likely to suffer severe illness than non-Hispanic whites (OR 2.02, 95% CI 1.07 to 3.78); this disparity improved by May, only to return later in the pandemic. CONCLUSION:In our patient sample, the severity of observed COVID-19 illness declined steadily over time, but these clinical improvements were not seen evenly across racial/ethnic groups; greater illness severity continues to be experienced among Hispanic/Latinx patients.

Abstract:Objective Health disparities may be driven by hospital‐level factors. We assessed whether racial and ethnic composition of populations hospitals serve explain or modify disparities in hospital outcomes of children with systemic lupus erythematosus (SLE). Methods In this retrospective cohort study of patients 5‐26 years old with SLE at 47 children's hospitals in the Pediatric Health Information System (2006–2021), race and ethnicity were assessed at the patient‐level and hospital‐level (proportion of total admissions comprised by Black or Hispanic patients, respectively). Outcomes included intensive care unit (ICU) admission or adverse renal outcome (end‐stage renal disease, dialysis or transplant) during follow‐up. We estimated racial‐ethnic disparities, adjusted or stratified by hospital racial or ethnic composition. Results Of 8125 patients with SLE, 2293 (28%) required ICU admission and 698 (9%) had an adverse renal outcome. Black/non‐Hispanic White disparities in ICU admission were observed only at hospitals serving higher proportions of Black patients (OR 1.29, 95% CI [1.04‐1.59] vs. 1.07, [0.83‐1.38]). Larger Black/non‐Hispanic White disparities in adverse renal outcomes were observed at hospitals with higher Black racial composition (OR 2.0, [1.4‐2.8] vs. 1.7, [1.1‐2.4]). Conversely, Hispanic vs. non‐Hispanic disparities in renal outcomes persisted after adjustment for hospital‐reported Hispanic ethnic composition but were observed only at hospitals with lower proportions of Hispanic patients. Conclusion Worse Black/White disparities in SLE outcomes are observed at children's hospitals serving more Black children, whereas distinct patterns are observed for Hispanic/non‐Hispanic disparities. Reporting of hospital characteristics related to populations served is needed to identify modifiable drivers of hospital‐level variation.

Examining Linguistic Differences in Electronic Health Records for Diverse Patients With Diabetes: Natural Language Processing Analysis

Abstract:Background: Individuals from minoritized racial and ethnic backgrounds experience pernicious and pervasive health disparities that have emerged, in part, from clinician bias. Objective: We used a natural language processing approach to examine whether linguistic markers in electronic health record (EHR) notes differ based on the race and ethnicity of the patient. To validate this methodological approach, we also assessed the extent to which clinicians perceive linguistic markers to be indicative of bias. Methods: In this cross-sectional study, we extracted EHR notes for patients who were aged 18 years or older; had more than 5 years of diabetes diagnosis codes; and received care between 2006 and 2014 from family physicians, general internists, or endocrinologists practicing in an urban, academic network of clinics. The race and ethnicity of patients were defined as White non-Hispanic, Black non-Hispanic, or Hispanic or Latino. We hypothesized that Sentiment Analysis and Social Cognition Engine (SEANCE) components (ie, negative adjectives, positive adjectives, joy words, fear and disgust words, politics words, respect words, trust verbs, and well-being words) and mean word count would be indicators of bias if racial differences emerged. We performed linear mixed effects analyses to examine the relationship between the outcomes of interest (the SEANCE components and word count) and patient race and ethnicity, controlling for patient age. To validate this approach, we asked clinicians to indicate the extent to which they thought variation in the use of SEANCE language domains for different racial and ethnic groups was reflective of bias in EHR notes. Results: We examined EHR notes (n=12,905) of Black non-Hispanic, White non-Hispanic, and Hispanic or Latino patients (n=1562), who were seen by 281 physicians. A total of 27 clinicians participated in the validation study. In terms of bias, participants rated negative adjectives as 8.63 (SD 2.06), fear and disgust words as 8.11 (SD 2.15), and positive adjectives as 7.93 (SD 2.46) on a scale of 1 to 10, with 10 being extremely indicative of bias. Notes for Black non-Hispanic patients contained significantly more negative adjectives (coefficient 0.07, SE 0.02) and significantly more fear and disgust words (coefficient 0.007, SE 0.002) than those for White non-Hispanic patients. The notes for Hispanic or Latino patients included significantly fewer positive adjectives (coefficient -0.02, SE 0.007), trust verbs (coefficient -0.009, SE 0.004), and joy words (coefficient -0.03, SE 0.01) than those for White non-Hispanic patients. Conclusions: This approach may enable physicians and researchers to identify and mitigate bias in medical interactions, with the goal of reducing health disparities stemming from bias.

Abstract:Objective: To determine the association of patient race, patient-provider racial congruence, patient ethnicity, and family primary language with patient family experience (PFE) survey responses. Methods: Cross-sectional review of PFE survey responses from all ambulatory medical encounters at a large, urban children's hospital system June 1, 2020-May 31, 2022. Exposures were patient race, patient-provider racial congruence, patient ethnicity, and family primary language. We adjusted analyses for neighborhood-level socioeconomic deprivation, patient sex and age, encounter specialty, and location of care. Outcomes were PFE survey scores for five questions focused on overall experience, respect, and safety; categorized using industry standard metric of presence of a "top-box" score, defined as a nine or 10 for questions on an 11-point scale or as four on a four-point scale. Results: We included 89,175 surveys (15.6% response rate). The odds of having optimal, "top-box" responses for several assessed questions were lower for patients identified as Asian (eg, adjusted odds ratio [OR] 0.46; 95% confidence interval [CI] 0.40, 0.52) or Black (eg, OR 0.65; CI 0.60, 0.70) compared to White, and for Hispanic (eg, OR 0.84; CI 0.72, 0.97) compared to non-Hispanic. Similarly, the odds of having "top-box" scores were lower for Spanish-primary-language (eg, OR 0.38; CI 0.30, 0.48) compared to English-primary-language patients. Patient-provider racial congruence had higher odds of "top-box" responses for two of five assessed questions (eg, OR 1.18; CI 1.04, 1.35). Conclusions: We found previously unreported inequities in ambulatory pediatric PFE outcomes, with worse experiences reported by Asian, Black, Hispanic, and Spanish-language patients.
