Abstract:Background/Aims. To investigate the long-term efficacy and rotational stability of toric intraocular lenses (IOLs) implanted for the correction of moderate-to-high corneal astigmatism. Methods. A total of 57 cataract patients (57 eyes) with regular corneal astigmatism (≥2.57 D) were enrolled in this retrospective cohort study. Phacoemulsification with toric IOL implantation was performed for all patients. The uncorrected visual acuity (UCVA) and best corrected visual acuity (BCVA) were recorded before and one year after surgery, and statistical analysis of preoperative corneal astigmatism, postoperative residual astigmatism, aberrations, IOL rotation, and related factors was performed to evaluate the efficacy, safety, and stability of toric IOLs in correcting moderate-to-high corneal astigmatism. Results. One year after surgery, visual acuity was significantly improved compared with that before surgery (preoperative log MAR 0.87 ± 0.34 vs. postoperative log MAR 0.31 ± 0.26, p < 0.001 ), and the self-reported spectacle independence rate was 68.42%. The total residual astigmatism was 1.18 ± 0.85 D, which was significantly less than the preoperative value (3.41 ± 0.99 D) ( p < 0.001 ). The degree of toric IOL rotation was 4.93 ± 3.02°, and 54.39% of patients had a lens rotation of less than 5°. The IOLs of 5.26% (3 eyes) of patients rotated more than 10°, and these patients received glasses instead of undergoing IOL repositioning. Conclusions. Toric IOL implantation provided optimal vision outcomes and low spectacle dependence during a one-year follow-up period. The results from our study show that toric IOL implantation is a safe and effective option for cataract patients with moderate-to-high corneal astigmatism.

Corneal Relaxing Incision Combined with Phacoemulsification and IOL Implantation

Abstract:Objective: To analyze the effectiveness and safety of corneal relaxing incisions (CRI) in correcting keratometric astigmatism during cataract surgery. Methods: A prospective study of two groups: control group and treatment group. A treatment group included 25 eyes of 25 patients who had combined clear corneal phacoemulsification, IOL implantation and CRI. A control group included 25 eyes of 25 patients who had clear corneal phacoemulsification and IOL implantation. Postoperative keratometric astigmatism was measured at 1 week, 1 month., 3 months and 6 months. Results: CRI significantly decreased keratometric astigmatism in patients with preexisting astigmatism compared with astigmatic changes in the control group. In eyes with CRI, the mean keratometric astigmatism was 0.29±0.17 D (range 0 to 0.5 D) at 1 week, 0.41±0.21 D (range 0 to 0.82 D) at 1 month, respectively reduced by 2.42 D and 2.30 D at 1 week and 1 month postoperatively (P=0.000,P=0.000) and postoperative astigmatism was stable until 6 months follow-up. The keratometric astigmatism of all patients decreased to less than 1.00 D postoperatively. Conclusions: CRI is a practical, simple, safe and effective method to reduce preexisting astigmatism during cataract surgery. A modified nomogram is proposed. The long-term effect of CRI should be investigated.
Clinical Application and Evaluation of Toric Intraocular Lens

Abstract:Objective Objective To evaluate the clinical results and rotational stability of AcrySof Toric intraocular lens (IOLs), and the impact of three measurements of keratometry on the results of astigmatism correction. Methods This prospective study included 23 eyes of 20 patients having cataract surgery. The IOL was calculated randomly using the results of automatic refractor, corneal topography or IOL Mastor. Implantation of the toric IOL was performed. The uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA), residual refractive cylinders, residual keratometry, and toric IOL axis were measured. And the differences of three measurements were analyzed. Results The UCVAs were 0.69±0.16 and BCVAs were 0.83±0.15 at 3 months follow-up. The mean refractive cylinder decreased significantly after surgery from 3.23 D to 0.59 D. The pre-surgical and post-surgical keratometry and corneal astigmatism showed no statistical difference. By analyzing the differrence from the post-surgical to the aimed residual astigmatism, there was no significant difference among these three measurements. Conclusions The results indicate that AcrySof toric IOL showed good clinical results and rotational stability. Automatic measurements of keratometry have no significant impact on its correction of astigmatism.
Long-term efficacy of implantation of AcrySof Toric intraocular lens on corneal astigmatism in cataract surgery

Abstract:AIM: To evaluate the clinical outcome of correction of corneal astigmatism and rotational stability of implantation of the AcrySof Toric intraocular lens(IOL) in cataract surgery.METHODS: Twenty-eight patients (thirty-two eyes) with more than 1.0 diopter(D) of preexisting astigmatism underwent similar phacoemulsification combined with AcrySof toric IOL implantation. The uncorrected visual acuity (UCVA), best-corrected visual acuity(BCVA), preoperative corneal astigmatism, anticipated residual astigmatism, postoperative residual astigmatism and toric lens axis were detected and compared.RESULTS: At 1 year following surgery, 85% of eyes showed the 0.5 or better in UCVA; 86% of eyes was 0.8 or better in BCVA and 92% of eyes achieved 0.6 or better in BCVA. The mean preoperative corneal astigmatism was (2.20 ±0.65)D and the postoperative residual astigmatism was (0.54±0.13)D, indicating a significant decrease in refractive cylinder after surgery(t=10.134, P<0.01). Preoperative predictive astigmatism(0.52±0.14)D and postoperative residual astigmatism (0.54±0.13)D showed no significant difference(t=0.364, P>0.05). After postoperative 6 month, the mean rotation of toric IOLs was 3.4°±2.2°, and after postoperative 1 year it was 3.82°±1.27°.CONCLUSION:AcrySof Toric IOL implantation is a viable and highly predictable method of correcting the corneal astigmatism, with no injury of the cornea. Careful selection of the patient, accurate keratometry and precise alignment of the cylindrical axes are some of the factors to be considered for a superior outcome.
Effectiveness of toric intraocular lens implantation for correcting irregular corneal astigmatism in cataract eyes

Abstract:A retrospective cohort study was conducted to observe the correction effect of Toric intraocular lens (IOL) implantation in cataract eyes with specific types of irregular corneal astigmatism. Thirty-four eyes with either the "asymmetric bow-tie" pattern (Type I) or the "angled bow-tie" pattern (Type II) were included. Corneal topography was assessed using Pentacam HR, and changes in preoperative corneal astigmatism, visual acuity, manifest refraction, and objective visual quality were measured and compared. The average uncorrected distance visual acuity improved significantly from 0.86 ± 0.40 logMAR to 0.22 ± 0.15 logMAR (P < 0.001). Preoperative corneal astigmatism of 2.05 ± 0.90 D was corrected to a postoperative residual astigmatism of 0.78 ± 0.57 D (P < 0.001), with 32% of eyes within 0.50 D. The residual astigmatism prediction errors in Type I and Type II cases were (0.97 ± 0.68 D) and (0.66 ± 0.37 D), respectively (P = 0.100). The mean spherical equivalent prediction error in Type II cases (0.07 ± 0.36 D) was significantly smaller than that in Type I cases (− 0.29 ± 0.52 D) (P = 0.030). This study concludes that Toric IOL implantation effectively corrects specific types of irregular corneal astigmatism in cataract surgery. Eyes with the "angled bow-tie" pattern show higher accuracy in refractive predictions compared to eyes with the "asymmetric bow-tie" pattern.

Abstract:Abstract This study tried to compare the clinical outcomes of femtosecond laser-assisted astigmatic keratotomy (FSAK) and toric intraocular lens (IOL) implantation for astigmatism correction and identify factors affecting the efficacy of FSAK and toric IOL implantation in astigmatism correction. This retrospective case series comprised patients with corneal astigmatism ranging between 0.5 D and 4.5 D. Patients underwent FSAK or toric IOL implantation for cataract treatment and correction of astigmatism at the Samsung Medical Center, a tertiary surgical center, between April 2016 and December 2018. All patients underwent examination before and at three months after the surgery for comparative evaluation of refractive astigmatism, corneal high order aberrations and irregularity index. The astigmatism correction was analyzed by the Alpins method. Subgroup analysis of preoperative factors was based on the extent of target-induced astigmatism (TIA), the degree of astigmatism, and astigmatism classification based on topography. Thirty-one eyes underwent toric IOL implantation and 35 eyes underwent FSAK. The refractive astigmatism was significantly decreased in both toric IOL ( P = 0.000) and FSAK group ( P = 0.003). The correction index (CI) of refractive astigmatism was 0.84 ± 0.39 in the toric IOL and 0.71 ± 0.60 in the FSAK group. There was no difference between the two groups ( P = 0.337). The CI of the FSAK group was significantly lower than in the toric IOL group when TIA was more than 1.5 D ( P = 0.006), when correcting against-the-rule ( P = 0.017), and limbus-to-limbus astigmatism ( P = 0.008). In conclusion, toric IOL implantation is an effective and safe procedure for correcting preoperative astigmatism in cataract surgery in the short-term observation.

[Phacoemulsification and Foldable Intraocular Lens Implantation Through a Clear Corneal Incision].

Abstract:OBJECTIVE:To evaluate the therapeutic effects of phacoemulsification with foldable intraocular lens implantation through a clear corneal incision.METHODS:The surgery was performed through a 3.2 mm clear corneal incision on 100 eyes of 90 patients with senile, complicated, congenital and traumatic cataracts.RESULTS:Postoperatively, the visual acuities with spherical correction or without correction were 0.5 or better in 93.0% of eyes at one day, and 1.0 or better in 56.0%, 65.1% and 67.5% at one week, one month and three months respectively. The mean postoperative astigmatism was 0.93 +/- 0.58 D at one week and 0.20 D more than the mean preoperative one (P < 0.001). There was no statistical difference between the mean preoperative astigmatism and postoperative astigmatism at one and three months which were respectively 0.78 +/- 0.60 D and 0.73 +/- 0.59 D.CONCLUSION:The phacoemulsification with foldable intraocular lens implantation through a clear corneal incision can reduce early postoperative astigmatism, shorten the period of visual rehabilitation and simplify operative procedures.

Abstract:OBJECTIVE:To evaluate the therapeutic effects of phacoemulsification and the foldable intraocular lens implantation. METHODS:The surgery was performed through a 3.2 mm incision on 72 eyes of 65 patients with senile, complicated, congenital and traumatic cataracts. RESULTS:Post-operatively, the visual acuities with spherical correction or without correction were 0.5 or better in 94.4% at one day, and 1.0 or better in 70.8%, 75.8%, and 76.5% at one week, one month and three months respectively. The mean post-operative astigmatism was 0.89 +/- 0.83 D at one week and 0.73 +/- 0.76 D at one month which was respectively less than that in the control group with a 6.5 mm incision (P < 0.001), but no statistical difference between them at three months (P > 0.05). CONCLUSION:The foldable intraocular lens implantation through a small sutureless incision could reduce early post-operative astigmatism and shorten the period of visual rehabilitation.
Astigmatism correction with toric implantable collamer lens in low and high astigmatism groups

Abstract:Purpose: To analyze the target induced astigmatism (TIA), surgically induced astigmatism (SIA), difference vector (DV), and correction index (CI) in the correction of astigmatism with phakic lenses, and its influence on visual acuity, and to analyze the safety and efficacy indexes of the correction of high and low power astigmatism with toric phakic lenses. Design: Retrospective comparative study. Methods: The medical records of patients that were operated on at the research center during the period were analyzed. Results were divided into Low Astigmatism Group - LAG (33 eyes) and High Astigmatism Group - HAG (93 eyes) according to the implanted toric ICL lens power. Preoperative refraction and resultant postoperative refraction were analyzed by vector analysis. Visual acuity pre and postop, with and without optical correction, were compared. Results: A total of 126 eyes were studied. The average preop refraction was -5.02 D sphere with -2.61 D cylinder. The average ICL lens power implanted was -8.31 D sphere +2.77 D cylinder. Refractive remaining was -0.01 ± 0.11 D sphere -0.15 ± 0.28 D cylinder. The arithmetic average angle of error in the astigmatism correction was 1.08°. The resultant cylinder was -0.03 ± 0.12 D and -0.19 ± 0.30 D in the low and high astigmatism groups, respectively, with a mean UDVA -0.01 ± 0.10 and 0.01 ± 0.16 and CDVA -0.03 ± 0.08 and -0.01 ± 0.17 for each group. The safety and efficacy indexes for the low astigmatism group were 1.09 ± 0.16 and 1.05 ± 0.17, respectively, with 1.11 ± 0.17 and 1.06 ± 0.16 for the high astigmatism group. Conclusions: The correction of astigmatism by the implantation of toric phakic lenses of the posterior chamber is safe and effective, independently of the amount of cylinder corrected.
Clinical Results with a Supplementary Toric Intraocular Lens for the Correction of Astigmatism in Pseudophakic Patients

Abstract:Purpose To evaluate the visual outcomes of pseudophakic patients who underwent supplementary toric intraocular lens (IOL) implantation to correct astigmatic refractive errors. Methods Pseudophakic patients referred for the implantation of a supplementary toric IOL (Sulcoflex Toric 653T) were evaluated. Uncorrected and corrected distance visual acuities (UDVA and CDVA, respectively), spherical equivalent (SE) refraction, rotational stability, higher order aberrations (HOA), and photopic glare and no-glare contrast sensitivity (CSV-1000, VectorVision) were evaluated. Results A total of 10 eyes of 10 patients were included. The mean age was 56.42 ± 5.9 years (range 45-65 years). Mean follow-up was 6.99 ± 5.1 months (6-18 months). Postoperatively, UDVA improved to 0.10 ± 0.12 (0.3 to −0.1) (p = 0.004) and CDVA to 0.07 ± 0.12 (0.3 to −0.1) (p = 0.021). Mean SE was −0.30 ± 0.56 D (-1.00 to +0.75) (p = 0.001). Mean toric IOL axis rotation at 6-month follow-up was 3.0° ± 2.45° (0-6). Ocular aberrometry values decreased after surgery (for average HOA root mean square, p = 0.008). Photopic contrast sensitivity (for all spatial frequencies) showed a trend for improvement after surgery; however, this was not borne out from the analysis (p>0.05). Conclusions The implantation of the Sulcoflex Toric IOL to correct astigmatism in pseudophakic patients provided excellent visual outcomes, predictability of refractive results, rotational stability, and optical performance. The implantation of this IOL is a safe and effective technique to correct pseudophakic cylindrical refractive errors and reduce spectacle dependence in these patients.

Abstract:Purpose: We present a case of internal astigmatism correction using a toric scleral contact lens in a patient with high myopia after implantable collamer lens (ICL) surgery.Case summary: A 45-year-old man presented with a significant decrease in visual acuity despite previous ICL surgery. Initial attempts to fit a corneal rigid gas permeable contact lens to the left eye were unsuccessful due to fitting challenges. Subsequently, the use of a spherical scleral contact lens effectively corrected corneal astigmatism but failed to address internal astigmatism, thereby limiting improvement in visual acuity. Postoperative corneal ectasia and ICL rotation were suspected to be the underlying causes of this astigmatism. The introduction of a front toric scleral lens resulted in a significant improvement in visual acuity and sustained comfort throughout the 3-month follow-up period.Conclusions: The use of toric scleral lenses can significantly improve visual acuity and provide exceptional comfort in cases where corneal morphology and lens position change after ICL surgery. This modality presents a promising alternative for future consideration.
Efficacy Comparison Between Toric Intraocular Lens and Aspheric Intraocular Lens Plus Steep-Axis Incision in Cataract Patients with Low Corneal Astigmatism.

Abstract:Background: The present study aimed to evaluate and compare the efficacy of toric intraocular lens (IOL) implantation and aspheric IOL implantation with steep-axis incision for correcting mild to moderate corneal astigmatism in cataract patients. Methods: A prospective, single-center, open label, randomized, two-arm study was designed. Cataract patients with corneal astigmatism of 1.0-2.0D were enrolled and divided into test and control groups. The test group had the AcrySof (R) IQ Toric IOL implanted and the control group had the AcrySof (R) IQ IOL implanted with a steep-axis corneal incision. All patients underwent examinations of uncorrected distance visual acuity (UCDVA), corrected distance visual acuity (CDVA), subjective refraction, and corneal astigmatism before surgery and at the 1-day, 1-month, and 3-month follow-ups. Vector astigmatism analysis was evaluated using the Alpins method. Results: We enrolled 26 eyes of 20 patients in the study, with 13 eyes in each group. The test group had better vision than the control group at the 3-month follow-up and had more cases with a UCDVA of 20/20 (10/13 vs. 4/13). The surgically induced astigmatism (SIA) vector of the test group was higher than that of the control group (1.22 +/- 0.64 vs. 0.84 +/- 0.45). The correction index of the test group was closer to 1 compared to that of the control group (0.7 vs. 0.46). Approximately 85% of patients in the test group had an angle of error within-15 degrees to 15 degrees. However, only 23% of patients in the control group were within that range. Conclusions: Though steep-axis corneal incision was cost-saving and easy-to perform, its astigmatism-correcting efficacy was not as good as the Toric IOL implantation for cataract patients with low to moderate corneal astigmatism.

Abstract:High levels of corneal astigmatism are prevalent in a significant proportion of the population. During cataract surgery pre-existing astigmatism can be corrected using single or paired incisions on the steep axis of the cornea, using relaxing incisions or with the use of a toric intraocular lens. This review provides an overview of the conventional methods of astigmatic correction during cataract surgery and in particular, discusses the various types of toric lenses presently available and the techniques used in determining the correct axis for the placement of such lenses. Furthermore, the potential causes of rotation in toric lenses are identified, along with techniques for assessing and quantifying the amount of rotation and subsequent management options for addressing post-operative rotation.

[The role of cholecystokinin in the regulation of intestinal blood circulation and metabolism].

Abstract:These studies were conducted to assess whether cholecystokinin (CCK) play a role in the regulation of intestinal blood flow and metabolic activity. Anesthetized dogs were subjected to laparotomy and implantation of transducers to measured intestinal blood flow (MBF), intestinal oxygen extraction (AVO2) and intraluminal pressure (IP). Intestinal oxygen consumption (VO2) was calculated as the product of the AVO2 and MBF. CCK8 infused intraarterially in doses 12.5, 25, 50, 75 and 100 pmol/kg-h increased significantly MBF by 5, 8, 11, 15 and 19%. IP increased significantly with the last three doses of CCK by 55, 87 and 145%, respectively. Glucose and oleic acid increased MBF by 12 and 37% and VO2 by 26 and 17%, respectively. Pretreatment with a highly selective CCK-receptor antagonist L-364 718 (1 mg/kg i.v.) abolished the vascular and motor responses to exogenous CCK and significantly reduced intestinal hyperemia and increase in VO2 in response to luminal oleic acid but not glucose. These findings indicate that endogenous CCK may play a role in the intestinal hyperemia after fatty meal and that this effect is primarily mediated by the CCK receptors.
Two-year results of AcrySof toric intraocular lens implantation in patients with combined microincision vitrectomy surgery and phacoemulsification

Abstract:AIM: To evaluate the effects and stability of AcrySof toric intraocular lens (IOL) implantation in patients who had combined microincision vitrectomy surgery (MIVS) and phacoemulsification for vitreoretinal diseases and cataract with corneal astigmatism.METHODS: A retrospective comparative study with 20 patients (20 eyes) who had combined 23-gauge MIVS and phacoemulsification with regular corneal astigmatism (>1.00 dioptres) was done. 10 eyes had toric IOL and 10 eyes had non-toric IOL implantation. The main outcome measures were uncorrected visual acuity (UCVA), refractive cylinder and toric IOL axis rotation at postoperative months 1, 6, 12, 18 and 24.RESULTS: The mean UCVA of toric IOL was better than non-toric IOL at each postoperative period (p=0.019, 0.001, 0.007, 0.004 and 0.001, respectively). The mean absolute residual refractive cylinder of toric IOL was less than non-toric IOL at each postoperative period (p=0.001, <0.001, <0.001, <0.001 and <0.001, respectively). At month 24, the mean toric IOL axis rotation was 3.3 ± 2.1°, which was within 5° in 80% and within 10° in 100%.CONCLUSIONS: Toric IOL implantation could be an effective method of correcting corneal astigmatism in patients who have vitreoretinal diseases and cataract. The toric IOL showed good rotational stability, even in vitrectomised eyes for 24 months.

Abstract:Background To compare the clinical outcomes after implantation of AcrySof Toric IOLs with different clear corneal incisions. Methods Sixty cataract patients with regular corneal astigmatism who underwent phacoemulsification combined with implantation of an AcrySof Toric IOL were enrolled. They were divided into two groups according to the position of the clear corneal incision: steep-axis and non-steep-axis. Main outcome measurements included visual acuity, residual astigmatism and changes of corneal astigmatism 3 months postoperatively. Deviation of IOL axis according to the re-calculation using the actual surgically induced astigmatism (SIA) and visual function 3 months after surgery were also evaluated. Results The corneal astigmatism decreased significantly in steep-axis group 3 months postoperatively ( P < 0.05). Besides, more patients in non-steep-axis group were with irregular corneal astigmatism after the surgery (43.33% vs 10%, P = 0.004). The deviation of IOL axis according to the re-calculation using the actual SIA was significantly larger in non-steep-axis group than that of steep-axis group ( P < 0.001). Moreover, the postoperative visual function was better in eyes of steep-axis group in various measurements, including point spread function, modulation transfer function and high-order aberrations. Conclusion Steep-axis clear corneal incision could reduce the cylinder power of toric IOL and lower the chance of postoperative irregular astigmatism, which may consequently improve the postoperative visual quality.
Outcomes for Acrysof Toric Intraocular Lenses to Correct Corneal Astigmatism in Eyes after Cataract Surgery

Abstract:Objective To compare the Acrysof toric intraocular lens (IOL) with the Acrysof SA60AT spherical IOL for patients with cataracts and preexisting corneal astigmatism and to investigate the rotational stability of the Acrysof toric IOL.Methods This prospective case-control study was comprised of 104 eyes with more than 1.0 D of preexisting corneal astigmatism.One group of 56 eyes were implanted with Acrysof toric IOL (toric group) and the other 48 eyes were implanted with Acysof SA60AT IOL(control group). Uncorrected (UCVA) and best corrected visual acuity (BCVA), residual refractive cylinder,spectacle independence,total lower order (LOA) and higher order aberrations (HOA),modulation transfer function (MTF) under 5, 10, 15,20,25, 30 cpd,and toric IOL axis were recorded 3 months postoperatively. Data were analyzed using a paired samples t test and an independent samples t test.Results Postoperatively, UCVA (logMAR) was 0.22 ±0.12 in toric group,versus 0.36 ±0.20 in control group (t=4.526,P＜0.01).BCVA was 0.05±0.07in toric group,versus 0.06 ±0.08 in control group (t=0.894,P=0.37).Mean residual refractive cylinder was 0.36 D in toric group versus 1.19 D in control group (t=4.275,P＜0.01).Three-month distance vision spectacle independence was 59％ (toric IOL) versus 32％ (control IOL).LOA in the toric group was significantly lower than that in the control group (t=11.146, P＜0.01), but no significant differences were found in higher order abberration between the two groups.MTF values in the toric group were higher than values in the control group at all spatial frequencies with a 3 mm pupil size (t=8.894,5.750,4.573,4.475,2.968,4.756,P＜0.01).Mean rotation was 5.84°±4.12° (range 0°～18°)for the toric IOL.Conclusion The favorable visual acuity,distance vision spectacle independence,visual quality, predictability, and rotational stability support the use of the Acrysof toric IOL for cataractous patients with corneal astigmatism.

Abstract:Objective Acrysof IQ Toric intraocular lens( IOL) are new intraocular lens which can effectively reduce corneal astigmatism in cataract patients. The aim of this study is to observe the clinical outcomes on pseudophakic patients with implanted Acrysof IQ Toric intraocular lens. Methods Thirty patients( thirty-four eyes) were treated with cataract phacoemulsification and implanted with the Acrysof IQ Toric IOL by the same surgeon from October 2012 to March 2013. Uncorrected visual activity( UCVA), best corrected visual activity( BCVA),preoperative corneal astigmatism,anticipated residual astigmatism postoperative,residual refractive cylinder and IOL axis and position were observed and compared preoperative and postoperative 1 day,1 week,1 month and 3 months. Results Cataract patients' preoperative UCVA was( 0. 13 ± 0. 10) D and BCVA was( 0. 22 ± 0. 16) D. The mean uncorrected visual acuity was( 0. 35 ± 0. 21) D 1 week postoperatively,( 0. 50 ± 0. 20) D 1 month postoperatively and( 0. 60 ± 0. 26) D 3 months postoperatively,and all of them were significantly higher than the preoperative BCVA( P 0. 05). There were significant differences between the preoperative corneal astigmatism( [2. 07 ± 1. 13]D) and the mean residual astigmatism 1 week( [0. 26 ± 0. 13]D),1 month( [0. 30 ± 0. 16]D) and 3 months postoperatively( [0. 34 ± 0. 29]D)( P 0. 01). There was no significant difference between the preoperative estimated residual astigmatism( [0. 28 ± 0. 22]D) and the residual astigmatism 1 week,1 month and 3 months postoperatively( P 0. 05). The mean IOL rotation was( 3. 17 ± 1. 83) °1 week postoperatively,( 3. 22 ± 1. 94) °1 month postoperatively and( 3. 57 ± 1. 69) °3 months postoperatively. No intraocular lens rotated more than 10° 3 month postoperatively. Conclusion Implantation of Acrysof IQ Toric intraocular lens for correction of astigmatism is effective and the IOL has good rotation stability.
Rotational stability of two different piggyback toric intraocular lenses for correction of high post-keratoplasty pseudophakic ametropia

Abstract:We present the case of a 65-year-old man with bilateral keratoconus and history of bilateral penetrating keratoplasty (PK) who developed gradual visual decline in the left eye due to cataract formation. Following successful left eye cataract surgery and monofocal, non-toric intraocular lens (IOL) in-the-bag implantation, the patient experienced persistently low uncorrected distance visual acuity (UDVA) due to high residual refractive error and intolerance to contact lenses. A supplementary toric IOL was placed in the ciliary sulcus, but subsequent rotational instability of the lens required repeated realignment. Despite two attempts at IOL repositioning, the rotational instability persisted, necessitating the replacement of the original Sulcoflex IOL with a toric, implantable Collamer lens. Following the implantation of the toric ICL, the patient achieved excellent UDVA with no adverse events over a 4-year follow-up period. This case highlights the potential rotational instability associated with toric piggyback IOLs in keratoconic, post-PK, pseudophakic eyes and the special considerations on choosing the type of piggyback lens in these eyes.
