[Clinical Evaluation of Hemofiltration Without Anticoagulation in Critically Ill Patients at High Risk of Bleeding].

Abstract:OBJECTIVE:To explore the safety and therapeutic efficacy of hemofiltration without anticoagulation in critically ill patients at high risk of bleeding. METHODS:We retrospectively analyzed 41 patients undergoing bedside hemofiltration in the Intensive Care Unit of PUMC Hospital from December 2005 to December 2006. The hemofiltration cases with or without anticoagulation were compared. RESULTS:Totally 224 hemofiltration circuits were performed in these 41 patients, including 114 (50.89%) circuits without anticoagulation in 27 patients at high risk of bleeding and 110 (49.11%) with anticoagulation by heparin. The circuit life of hemofiltration without anticoagulation was (13.21 +/- 9.73) hours (3-55 hours), and that of hemofiltration with anticoagulation was (28. 35 +/- 26.43) hours (9-126) hours (P < 0.01). The circuit life had no correlation with prothrombin time (PT), activitated partial thromboplastin time (APTT), fibrinogen (Fbg), and platelet count (PLT) levels before the therapy (P > 0.05). The levels of blood creatinine, blood urea nitrogen, potassium, and pH value significantly improved after both therapies (P < 0.05). Urea reduction rates were (38.4 +/- 26.7)% and (24.3 +/- 13.2)% in hemofiltration with and without anticoagulation, respectively. The levels of Fbg and PLT significantly decreased after therapy in both groups (P < 0.01). PT and APTT did not change in hemofiltration without anticoagulation (P > 0.05), while APTIT was prolonged in hemofiltration with heparin (P < 0.01). Of the 21 patients who initially received hemofiltration with heparin, 7 patients turned to hemofiltration without anticoagulation for hemorrhagic complications. Seventeen (41.5%) of 41 patients died during hospital stay, but none of them died of complications of hemofiltration. CONCLUSIONS:Hemofiltration without anticoagulation reduces bleeding risks and achieves an acceptable circuit life. The strategy can be applied as an alternative to critically ill patients at high risks of bleeding who need continuous blood purification.

Abstract:Dose determination in continuous renal replacement therapy (CRRT) is controversial. Most clinical trials use effluent flow rate as a surrogate of the dose. However, such definition may overestimate actually delivered dose due to declining filter function. The current study aimed to determine the difference between prescribed and delivered clearance and its association with transmembrane pressure. Hemofiltration was done in a mixed pre- and postdilution mode. Creatinine concentrations in serum and effluent fluid were measured simultaneously at 4, 10, 16, 28, 40, 52, and 64 hours for an individual hemofilter. Prescribed clearance (K) was estimated as the effluent flow rate corrected for predilution, and delivered clearance (Kx) was estimated using the ratio of serum and effluent creatinine. A total of 60 patients involving 248 filters were included in our analysis. The mean filter life span was 37.7 hours (standard deviation: 17.6). K overestimated Kx by 9.3% (95% confidence interval: -4.4% to 32.3%). The differences between K and Kx increased progressively over time. Transmembrane pressure was significantly correlated to the reduction with a Spearman's rho of 0.44 (p < 0.001). K significantly overestimates Kx during CRRT, and the difference increases progressively over time. Filters are recommended to be changed at 48-72 hours on a routine basis.
In vitro Removal of Protein-Bound Retention Solutes by Extracorporeal Blood Purification Procedures

Abstract:Introduction: When the kidneys or liver fail, toxic metabolites accumulate in the patient's blood, causing cardiovascular and neurotoxic complications and increased mortality. Conventional membrane-based extracorporeal blood purification procedures cannot remove these toxins efficiently. The aim of this in vitro study was to determine whether commercial hemoperfusion adsorbers are suitable for removing protein-bound retention solutes from human plasma and whole blood as well as to compare the removal to conventional hemodialysis. Methods: For in vitro testing of the removal of protein-bound substances, whole blood and plasma were spiked with uremic retention solutes (homocysteine, hippuric acid, indoxyl sulfate, 3-carboxy-4-methyl-5-propyl-2-furanpropionic acid) and the toxins of liver failure (bilirubin, cholic acid, tryptophan, phenol). Subsequently, the protein binding of each retention solute was determined. The adsorption characteristics of the hemoperfusion adsorbers, Jafron HA and Biosky MG, both approved for the adsorption of protein-bound uremic retention solutes and Cytosorb, an adsorber recommended for adsorption of cytokines, were tested by incubating them in spiked whole blood or plasma for 1 h. Subsequently, the adsorption characteristics of the adsorbers were tested in a dynamic system. For this purpose, a 6-h in vitro hemoperfusion treatment was compared with an equally long in vitro hemodialysis treatment. Results: Hippuric acid, homocysteine, indoxyl sulfate, and tryptophan were most effectively removed by hemodialysis. Bilirubin and cholic acid were removed best by hemoperfusion with Cytosorb. A treatment with Jafron HA and Biosky MG showed similar results for the adsorption of the tested retention solutes and were best for removing phenol. 3-Carboxy-4-methyl-5-propyl-2-furanpropionic acid could not be removed with any treatment method. Discussion/Conclusion: A combination of hemodialysis with hemoperfusion seems promising to improve the removal of some toxic metabolites in extracorporeal therapies. However, some very strongly protein-bound metabolites cannot be removed adequately with the adsorbers tested.

Abstract:Introduction: Online hemodiafiltration (OL-HDF) and hemodialysis (HD) using high-performance membranes such as adsorptive, medium cut-off (MCO), and super high-flux (SHF) dialyzers have been implemented to enhance the removal of middle molecules (MM). The aim of this study was to compare the efficacy of different dialysis strategies and dialyzers on small solutes and MM reduction ratio (RR) and mass removal. Methods: We performed a prospective study in 8 HD patients. Each patient underwent 9 dialysis sessions: seven sessions on HD using either Theranova 500TM, Elisio 21HTM, Renak PS-2.0WTM, Filtryzer BK-2.1FTM, Vie 21XTM, TS-2.1ULTM or FDY 210-GWTM dialyzers and two sessions on OL-HDF using Elisio 21HTM or Renak PS-2.0WTM dialyzers. Results: Urea mass removal and RR were similar between all dialysis strategies. The lowest beta2-microglobulin RR was achieved with Filtryzer BK-2.1FTM HD ( p #x3c; 0.05). Compared to Elisio 21HTM HD, Renak PS-2.0WTM OL-HDF produced higher beta2-microglobulin mass removal (181 ± 46 vs. 317 ± 161 mg, p #x3c; 0.05). Theranova 500TM HD, Vie 21XTM HD, FDY 210-GWTM HD, Elisio 21HTM OL-HDF, and Renak PS-2.0WTM OL-HDF induced higher RR for kappa and lambda FLC, as compared to Elisio 21HTM HD and Filtryzer BK-2.1FTM HD ( p #x3c; 0.05). Renak PS-2.0WTM OL-HDF achieved higher kappa FLC mass removal compared to Elisio 21HTM HD (563 ± 515 vs. 141 ± 47 mg, p #x3c; 0.01) and to Renak PS-2.0WTM HD (563 ± 515 vs. 153 ± 25 mg, p #x3c; 0.05). Albumin loss varied from 0.02 ± 0.05 to 7.6 ± 3.8 g/session with Elisio 21HTM HD and Renak PS-2.0WTM OL-HDF, respectively. Compared to all other strategies, Renak PS-2.0WTM OL-HDF induced a significantly higher albumin loss ( p #x3c; 0.05). Conclusion: This study confirms that albumin loss and removal of MM are similar using conventional Elisio 21HTM OL-HDF, MCO-HD, and SHF type V dialyzers. Although Renak PS-2.0WTM OL-HDF provides high performance for MM depuration, this protein-permeable dialyzer should not be used in OL-HDF because of excessive albumin loss. Blood Purif

Comparison of Plasma Exchange with Different Membrane Pore Sizes in the Treatment of Severe Viral Hepatitis

Abstract:Plasma exchange has become an effective mode of blood purification in patients suffering from liver failure. To assist in patient recovery, we compared two plasma separators to identify a plasma separator with suitable pore sizes to remove toxic substances effectively, and retain important plasma components. The study focused on severe viral hepatitis patients. Of 206 rounds of plasma exchange, 137 were completed with the PS-06 plasma separator (membrane pore size = 0.2 mu m) and 69 with the EC4A lasma separator (membrane pore size = 0.03 mu m).The efficacy of different plasma separators was compared using survival rate, changes in liver biochemistry, immunoglobulin, and complement parameters. The survival rate of patients treated with PS-06 was 43.3% (13 of 30 patients). For patients treated with EC-4A, two patients were bridged to liver transplantation successfully, and 57.9% (11 of 19 patients) survived. In both groups, the levels of total bilirubin, prothrombin time, and bile acid declined significantly. Compared to PS-06, EC-4A could retain significantly larger amounts of immunoglobulin and complements. Our study revealed that plasma exchange implementation with membrane pore size 0.03 mu m could remove adequate bilirubin and bile acid, a class of toxins bound to plasma protein in severe viral hepatitis patients, and reduce the loss of essential plasma macromolecules.
Human Proenkephalin A 119-159 (penKid) in Extracorporeal Therapies: Ex vivo Sieving Coefficient, Diffusive Clearance, and Hemoadsorption Kinetics

Abstract:Introduction: Enkephalins, endogenous opioid peptides, are involved in the regulation of renal function. One derived molecule, proenkephalin A, also known as penKid, has been demonstrated to be a reliable biomarker for kidney function and its plasma concentration correlates with measured glomerular filtration rate. penKid is used for prediction and diagnosis of AKI and need of renal replacement therapy (RRT). penKid has also been used to predict the successful weaning from RRT in patients with AKI. Whether the concentration of penKid is affected or not by RRT is a controversial point and there are no studies describing the kinetics of the molecule in such conditions. The low molecular weight (4.5 kDa) would imply free removal by the glomerulus and the dialysis membranes. During RRT, this reduction could not be detected in clinical practice due to the complex kinetics involving either low dialytic clearance or increased production in response to impaired kidney function. The aim of this study was to determine the sieving coefficient and the diffusive clearance of the penKid molecule in conditions of in vitro continuous veno-venous hemofiltration (CVVH) and continuous veno-venous hemodialysis (CVVHD), respectively, and also the penKid removal ratio in conditions of in vitro hemoadsorption (HA) using a synthetic microporous resin. Methods: Blood spiked with a lyophilized penKid peptide solved in 20 m<sc>m</sc> dipotassium phosphate and 6 m<sc>m</sc> disodium EDTA [pH 8] to reach target concentrations is used as testing solution. In each experiment, the blood batch was adjusted at a volume of 1,000 mL, maintained at 37°, and continuously stirred. Samples were collected from blood, ultrafiltrate, and spent dialysate at different times during the experiments. Sieving, clearance, and removal ratio were calculated. Results: Significant removal of penKid was observed in CVVH (sieving 1.04 ± 0.27), in CVVHD (clearance 23.08 ± 0.89), and in HA (removal ratio 76.1 ± 1% after 120 min). Conclusion: penKid is effectively removed by extracorporeal therapies. In presence of anuria, penKid generation kinetics can be calculated based on extracorporeal removal and volume variation. In steady state conditions, declining values may be the result of an initial renal function recovery and may suggest discontinuation and successful liberation from RRT.
Citrate anticoagulation versus systemic heparinisation in continuous venovenous hemofiltration in critically ill patients with acute kidney injury: a multi-center randomized clinical trial

Abstract:IntroductionBecause of ongoing controversy, renal and vital outcomes are compared between systemically administered unfractionated heparin and regional anticoagulation with citrate-buffered replacement solution in predilution mode, during continuous venovenous hemofiltration (CVVH) in critically ill patients with acute kidney injury (AKI).MethodsIn this multi-center randomized controlled trial, patients admitted to the intensive care unit requiring CVVH and meeting inclusion criteria, were randomly assigned to citrate or heparin. Primary endpoints were mortality and renal outcome in intention-to-treat analysis. Secondary endpoints were safety and efficacy. Safety was defined as absence of any adverse event necessitating discontinuation of the assigned anticoagulant. For efficacy, among other parameters, survival times of the first hemofilter were studied.ResultsOf the 139 patients enrolled, 66 were randomized to citrate and 73 to heparin. Mortality rates at 28 and 90 days did not differ between groups: 22/66 (33%) of citrate-treated patients died versus 25/72 (35%) of heparin-treated patients at 28 days, and 27/65 (42%) of citrate-treated patients died versus 29/69 (42%) of heparin-treated patients at 90 days (P = 1.00 for both). Renal outcome, i.e. independency of renal replacement therapy 28 days after initiation of CVVH in surviving patients, did not differ between groups: 29/43 (67%) in the citrate-treated patients versus 33/47 (70%) in heparin-treated patients (P = 0.82). Heparin was discontinued in 24/73 (33%) of patients whereas citrate was discontinued in 5/66 (8%) of patients (P < 0.001). Filter survival times were superior for citrate (median 46 versus 32 hours, P = 0.02), as were the number of filters used (P = 0.002) and the off time within 72 hours (P = 0.002). The costs during the first 72 hours of prescribed CVVH were lower in citrate-based CVVH.ConclusionsRenal outcome and patient mortality were similar for citrate and heparin anticoagulation during CVVH in the critically ill patient with AKI. However, citrate was superior in terms of safety, efficacy and costs.Trial registrationClinicaltrials.gov NCT00209378. Registered 13th September 2005.

Abstract:[Objective] The research aimed to get the optimized separation and purification conditions of the hirudin produced from Bacillus subtilis DB403(pUBH5).[Method]Through the systemic pretreatment,preliminary chromatography and fine chromatography.[Result]The optimized separation and purification conditions were that:Supernatant was treated by trichloroacetic acid,then by ultrafiltration desalt and anion exchange chromatography.Strong anion Q F.F.was better than weak anion DEAE F.F.The proper balanced solution was Tris-HCl(pH 8.0).The proper conductivity was 6.0 mS/cm.The maximum applied sample was 240.0 ATU/ml to matrix of strong anion Q F.F.This optimized procedure was magnified in strong anion exchange HiPrep 16/10Q with the 90.0% recovery and 70.2% purity.The purification of gel filtration of Sephacryl S-100 to hirudin was not relative to flow rate within certain scope.The application size of sample was 10ml.The purity checked by HPLC was 95.1%,and the recovery was 93.0%,and the band of SDS-PAGE was single.[Conclusion] The research provided the reference of the further industrialization separation and purification of hiruin.
High Cut-off Point Membranes in Septic Acute Renal Failure: A Systematic Review

Abstract:Objectives To review the literature on the experimental, physiological and clinical effects of blood purification with high cut-off (HCO) point membranes in septic acute renal failure (ARF). Study Design MEDLINE and PubMed database search combining relevant terms and integrating data from studies on the use of HCO membranes. Setting and Population Ex vivo studies of endotoxemia, animal studies of bacteremia and clinical studies using HCO membranes in patients with septic ARF. Selection Criteria for Studies: Original data from primary publications. Interventions: HCO membrane-based hemodialysis, hemodiafiltration or hemofiltration. Outcomes: Plasma cytokine clearance, immunological and physiological effects and safety parameters of HCO membranes. Results HCO membranes effectively remove cytokines from blood. Treatment using HCO membranes has beneficial effects on immune cell function and increases survival in animal models of sepsis. Preliminary clinical studies show that HCO membranes decrease plasma cytokine levels and the need for vasopressor therapy. HCO membrane-based blood purification has now been applied in four pilot randomized controlled studies of 70 patients with septic ARF with no reports of serious adverse effects. Limitations Because of substantial heterogeneity, no formal quantitative analysis could be performed. Conclusions The available evidence on HCO blood purification justifies larger randomized controlled trials in patients with septic ARF.
Depurative capacity toward medium molecules of the dialyzer Toray NV-U® Hydrolink™: A new hydrophilic membrane to perform online hemodiafiltration

Abstract:Introduction: New dialysis membranes with new properties are being developed to improve efficacy and tolerance. The hemocompatibility of a polymeric biomaterial is influenced by the layer of water at the blood membrane interface. The new dialyzer TORAY NV-U® has a membrane Hydrolink™, designed to suppress platelet adhesion and to improve the hemocompatibility. Until now, there is no experience in online hemodiafiltration (OL-HDF).The objective of the present study is to evaluate the efficacy of this new membrane in OL-HDF therapy compared to another membrane commonly used. Other objectives are to evaluate the inflammatory response, hemodynamic tolerance, and the anticoagulation regimes. Methods: This is a prospective pilot study performed in five anuric patients receiving OL-HDF. For 1 month patients were kept with their usual dialyzer FX1000® (FMC). Subsequently, the dialyzer was changed to TORAY NV-U® (Hydrolink®) for 1 month. In the last dialysis session of each dialyzer, blood tests were performed to evaluate inflammation and depurative capacity. Results: We did not find differences in medium size removal molecules and convective volume: FX1000®: 31 ± 9 l per session and Hydrolink™ 30 ± 8 l; p = 0.7); β2microglobulin reduction ratio (RR) FX1000® FMC 83 ± 3%; Hydrolink™ 79 ± 4; p = 0.14; Myoglobin RR FX1000® FMC 72 ± 7%; Hydrolink™ 76 ± 4; p = 0.28. We did not find differences in inflammation parameters: serum IL6 with FX1000® 6.0 ± 4.2 pg/mL; Hydrolink™ 7.6 ± 5.0 pg/mL; p = 0.3.During all sessions with the two dialyzers there was adequate plasmatic filling, reaching 85 % filling. All patients had "good" dialyzer status in all dialysis sessions with TORAY NV-U®, while the dialyzer status with FX1000® was "good" in 20% of the sessions, "medium" in 30%, and "dirty" in the remaining 50% dialysis sessions. Conclusions: The new dialyzer Hydrolink™, TORAY NV-U® is not inferior to perform OL-HDF compared to dialyzers usually used for this therapy, and could allow decrease heparin doses. Further studies with a bigger sample size and longer follow-up will answer if Hydrolink improves inflammation and assess a better hemodynamic tolerance.
Removal of β2-microglobulin during on-line hemodiafiltration using a domestic hemofilter

Abstract:Objective To investigate the removal of serum β2-microglobulin during on-line hemodiafiltration using a domestic hemofilter.Methods In this single center,self-control,open-label clinical study,30 stable hemodialysis patients whose serum β2-microglobulin increased to more than 10 times above the upper normal limit were chosen to receive post-dilution online hemodiafiltration using a domestic hemofilter (Lengthen,membrane area 1.4m2) or an imported hemofilter (Toray,membrane area 1.6m2).Reduction ratio (RR) of β2-microglobulin was assessed.Results The RR of serum β2-microglobulin was similar after online hemodiafiltration using a Lengthen (71.729％± 7.393％) hemofilter and a Toray hemofiter (74.707％± 7.889％,t=1.508,P=0.137).Conclusion There was a good removal effectiveness of β2-microglobulin when the domestic hemofilter was used in post-dilution online hemodiafiltration.

We did not find differences in inflammation parameters: serum IL6 with FX1000® 6.0 ± 4.2 pg/mL; HydrolinkTM 7.6 ± 5.0 pg/mL; p = 0.3.During all sessions with the two dialyzers there was adequate plasmatic filling, reaching 85 % filling. All patients had "good" dialyzer status in all dialysis sessions with TORAY NV-U®, while the dialyzer status with FX1000® was "good" in 20% of the sessions, "medium" in 30%, and "dirty" in the remaining 50% dialysis sessions.Conclusions:The new dialyzer HydrolinkTM, TORAY NV-U® is not inferior to perform OL-HDF compared to dialyzers usually used for this therapy, and could allow decrease heparin doses. Further studies with a bigger sample size and longer follow-up will answer if Hydrolink improves inflammation and assess a better hemodynamic tolerance.

Abstract:BACKGROUND: Regional anticoagulation with citrate during renal replacement therapy (RRT) reduces the risk of bleeding, extends dialyzer lifespan and is cost-effective. Therefore, current guidelines recommend its use if patients are not anticoagulated for another reason and if there are no contraindications against citrate. RRT with regional citrate anticoagulation has been established in critically ill patients as continuous veno-venous hemodialysis (CVVHD) to reduce citrate load. However, CVVHD is inferior regarding middle molecule clearance compared to continuous veno-venous hemofiltration (CVVH). The use of a high cut-off dialyzer in CVVHD may thus present an option for middle molecule clearance similar to CVVH. This may allow combining the advantages of both techniques.METHODS: In this prospective, randomized, single-blinded single-center-trial, sixty patients with acute renal failure and established indication for renal replacement therapy were randomized 1:1 into two groups. The control group was put on CVVHD using regional citrate anticoagulation and a high-flux dialyzer, while the intervention group was on CVVHD using regional citrate anticoagulation and a high-cut-off dialyzer. The concentrations of urea, creatinine, β2-microglobulin, myoglobin, interleukin 6 and albumin were measured pre- and post-dialyzer 1, 6, 12, 24 and 48 hours after initiating CVVHD.RESULTS: Mean plasma clearance for β2-microglobulin was 19.6±5.8 ml/min in the intervention group vs. 12.2±3.6 ml/min in the control group (p<0.001). For myoglobin (8.0±4.5 ml/min vs. 0.2±3.6 ml/min, p<0.001) and IL-6 (1.5±4.3 vs. -2.5±3.5 ml/min, p = 0.002) a higher mean plasma clearance using high-cut-off dialyzer could be detected too, but no difference for urea, creatinine and albumin could be observed concerning this parameter between the two groups.CONCLUSION: CVVHD using a high cut-off dialyzer results in more effective middle molecule clearance than that with high-flux dialyzer.TRIAL REGISTRATION: German Clinical Trials Register (DRKS00005254, registered 26th November 2013).

Abstract:To evaluate the solute clearance of three modes of continuous renal replacement therapy(CRRT) at various flow rates of dialysate and ultrafiltrate. METHODOLOGY Five patients with acute renal failure who were indicated for CRRT were enrolled in this study . A PRISMA pre dilution system with Multiflow 100 filter was applied .Solute clearance of urea nitrogen (UN) ,creatinine (Cr), uric acid( Ua), phosphate (P) and β 2 microglobulin (β 2 M) were determined at various dialysate and ultrafiltrate during CVVH(continuous venovenous hemofiltration), CVVHD(continuous venovenous hemodialysis), CVVHDF (continuous venovenous hemodiafiltration). RESULTS The determined clearance of various solutes (Kd) were calculated by the concentration of the cleared solutes in effluents (E) based on the formula Kd=(E×Q E)/P, where Q E for the flow rate of the effluent and P for the plasma concentration of the solute.Clearances of UN,Cr,Ua and P during CVVHD were greater than those during CVVH,but clearance of β 2 M during CVVHD was less than that during CVVH.The interaction between convection and diffusion was found during CVVHDF. CONCLUSION Clearance of small solutes is better in CVVHD than in CVVH,but clearance of β 2 M is better in CVVH than in CVVHD.In CVVHDF,interaction exists between diffusion and convection.
Elution-free hollow-fiber membranes of block copolymers for hemodialysis balancing protein retaining and toxin clearance

Abstract:Hemodialysis has been used as the primary treatment for patients with end-stage renal disease, however, the current hemodialysis membranes still need complicated modifications to enhance the hemocompatibility and overcome the additive leaching issue. Herein, we prepare hemodialysis hollow-fiber membranes (HFMs) via the melt spinning and selective swelling of the block copolymers of polysulfone (PSF) and polyethylene glycol (PEG), PSF- b -PEG. PSF- b -PEG is first melt-extruded to form dense hollow fibers, and then soaked in selective solvents to transform the PEG microdomains into nanopores following the mechanism of selective swelling-induced pore generation, thus producing HFMs with three dimensionally interconnected porosities. As neither additives nor involatile solvents are involved in the manufacturing process of the HFMs, no elution of any organic matters could be detected for the HFMs during hemodialysis. The HFMs possess a symmetrical structure ensuring tight selectivity, and hydrophilic PEG chains are enriched on the pore surfaces, thus endowing the membranes with enhanced hydrophilicity and durable biocompatibility. We systematically investigate the effect of PEG contents and swelling conditions on pore sizes, porosities, surface properties, and consequently the hemodialysis performance. The optimized HFMs reject >99 % serum albumin while clear ∼70 % middle molecular toxins such as lysozyme and 93–95 % small molecular toxins including urea, phosphate and creatinine. This work provides a strategy to prepare elution-free hemodialysis membranes by taking advantage of selective swelling of block copolymers to balance high protein retaining and high clearance of middle and small molecular toxins, and demonstrates their superiority in hemodialysis performance and safety than conventional membranes.

Abstract:Abstract Introduction The optimal regional anticoagulation (RA) of dialysis filters in patients at risk of bleeding remains elusive. Inducing hypocalcemia within the filter by using a calcium-free dialysate has emerged as an easy-to-use heparin-free RA, including in critically ill patients, but comparative studies are lacking. Methods We conducted a multicentre, randomized, crossover trial to compare the efficacy and tolerance of two RAs (heparin-coated membrane (HCM) or calcium free dialysate with calcium reinjection according to ionic dialysance (CFD)) in patients requiring hemodialysis and at risk of bleeding. During the study period, each patient received two dialysis sessions (one with each RA in a randomly assigned order). The primary endpoint was the proportion of dialysis sessions completed (≥ 240 min). Results 94 patients were included in the intention-to-treat analysis, including 16 critically ill patients (17.0%). Coagulation and inflammation parameters, as well as hemodynamic status at baseline, were balanced between groups. Premature coagulation of the filter occurred in 19 HCM (20.9%) compared to 3 (3.2%) CFD sessions. In half of the sessions with premature termination, coagulation occurred before 180 minutes. The proportion of patients who completed the CFD session while failing to complete the HCM session (n = 17) was significantly higher than the proportion of patients who completed the HCM session while failing to complete the CFD session (n = 1; p < 0.001). Hemodynamic and metabolic tolerance were not different between groups. Conclusions In individuals at risk of bleeding, RA with calcium-free dialysate significantly reduces the incidence of premature dialysis termination compared to heparin-coated membrane without safety concerns. Abstract:Introduction: Membrane fouling is a significant complication potentially reducing clinical effects of extracorporeal blood purification (EBP) in critically ill septic patients with acute kidney injury. Although fascinating, the effect of heparin coating in preventing membrane fouling is currently unknown. This multicenter prospective study aims to preliminary describe the incidence, associated factors, and clinical consequences of premature circuit clotting in a cohort of adult critically ill septic patients treated with EBP using a high biocompatible heparin-coated hemodiafilter characterized by advanced adsorption properties. Methods: This study was a retrospective analysis of prospectively entered data in the oXirisNet Registry; overall, 97 septic patients undergoing EBP with oXiris between May 2019 and March 2020 were enrolled in this study. Patients were divided into two groups according to the occurrence of filter clotting (premature vs. nonpremature). Logistic regression analysis was used to identify factors associated with premature circuit clotting. Results: Premature clotting occurred in 18 (18.6%) patients. Results of the multivariate logistic regression analysis demonstrated that hematocrit ( p = 0.02, odds ratio [OR] 1.15 [1.05; 1.30]), serum procalcitonin (PCT) ( p = 0.03, OR 1.1 [1.05; 1.2]), and anticoagulation strategy ( p = 0.05 at Wald’s test) were independent predictors of circuit clotting. Systemic anticoagulation ( p = 0.02, OR 0.03 [0.01; 0.52]) and regional citrate anticoagulation ( p = 0.10, OR 0.23 [0.04; 1.50]) were both protective factors if compared to no-anticoagulation strategy. Patients with nonpremature circuit clotting showed more rapid recovery from hemodynamic instability, pulmonary hypo-oxygenation, and electrolyte disorders and greater improvement of inflammatory markers and SOFA scores. Conclusion: Although in this study the incidence of premature circuit clotting was relatively low (18.6%) compared to previously reported values (54%), membrane clotting in adult critically ill septic patients could cause clinically relevant interferences with treatment performances. Prevention of clotting should be based on avoiding higher patients’ hematocrit, high serum PCT, and no-anticoagulation strategy which resulted as independent predictors of circuit clotting.
