Abstract:This investigation detects the inherent precision of four navigation systems, of different structural type, for computer-assisted surgery, ranging from 0.1 to 1.8 mm: the Viewing Wand with a mechanical arm, and three new systems, the SMN microscope and STP pointer with infrared technology and the MKM system with laser autofocus. For this purpose, a new standard to detect separately the inherent deviations of navigation systems from the deviations caused by acquisition of CT data sets, is introduced. The measurements are performed within a complete three-dimensional room, consisting of three orthogonal planes of a geometric model. The method introduced is valid for regular measurements of the inherent precision of navigation systems for quality assurance in order to prevent intraoperative failure caused by insufficient potentiometers, infrared transmitters or receivers.

Abstract:Abstract Objective To explore navigation-related factors interfering with accuracy of robot-assisted surgery. Methods We made a measurement model to test the accuracy of the TianJi Robot system when performing the stimulated screw placement procedure. The three-coordinate machine was used to measure the deviation between the actual position and the planned position. We designed corresponding experiments to explore the effects of different navigation-related factors on the screw placement accuracy. The deviations were measured at different distance (ranging from 1.2 m to 2.2 m) between the navigation optical stereo camera and the tracker and each distance was measured 50 times. The distance between the optical camera and the patient tracker was set at 1.4 m and the deviations were measured at different angles between the camera and the robot tracker, each angle was measured more than 25 times. Data was donated with mean and standard deviation. The line charts were employed to describe the changes of deviations over one clinical factor including distance and angle. Results Within the available scope of navigation optical system (1.2 m-2.2 m), the deviation increased with the distance (χ2=479.107, P<0.001). The robotic system accuracy was high and stable (mean deviation 0.332 mm ± 0.067 mm) when the relative angle between the optical camera and the tracker less than 40 degrees. Conclusions Accuracy of robot system was affected by the relative distance and angle between the optical camera and the tracker. When placing and adjusting the optical tracking devices, surgeons should set the relative distance between the optical camera and the patient tracker as 1.4 m- 1.5 m and the relative angle less than 40 degrees.

Abstract:This study aims to explore factors related to optical navigation that interfere with the accuracy of robot-assisted surgery, specifically focusing on the TIANJI Robot system. A measurement model was created to assess the accuracy of the TIANJI Robot system in simulated screw placement. The robotic system demonstrated high and consistent accuracy (mean deviation: 0.332 mm ± 0.067 mm) when the relative angle between the optical camera and tracker was below 40°. The accuracy of the TIANJI Robot system was found to be influenced by relative distance and angle between the optical camera and tracker during screw placement procedures. Surgeons are recommended to set a relative distance of 1.4-1.5 m between the optical camera and patient tracker, with a relative angle below 40° when placing and adjusting optical tracking devices.
Positional Accuracy and Transmitter Orientation of the 3D Electromagnetic Tracking System

Abstract:This research investigates the positional accuracy and effects of transmitter orientation of a 3D electromagnetic tracking (EMT) system. EMT systems, capable of real-time position and orientation monitoring, are commonly used in computer-aided surgical navigation and path monitoring. In this study, positional information is evaluated for accuracy by comparing the EMT system against laser interferometer measurements in three orthogonal axes with step sizes between 0.1 and 0.5 mm. The effect of transmitter orientation is evaluated by placing the transmitter with either the front or the side facing the magnetic sensor. Gauge repeatability and reproducibility results demonstrate that the EMT system can accurately measure the motion with a tolerance of 0.2 mm with 0.5 s measurement time. The transmitter oriented with the front facing the sensor has a higher positional accuracy than that of the side transmitter orientation. High accuracy of the EMT system combined with the knowledge of transmitter orientation information presents the potential for accurate navigation and path monitoring in medical procedures.

Abstract:Abstract Object Preoperative image-based neuronavigation-assisted endoscopy during intracranial procedures is gaining great interest. This study aimed to analyze the precision of navigation-assisted endoscopy according to the navigation setup, the type of optic and its working angulation. Methods A custom-made box with four screws was referenced. The navigation-assisted endoscope was aligned on the screws (targets). The precision on the navigation screen was defined as the virtual distance-to-target between the tip of the endoscope and the center of the screws. Three modifiers were assessed: (1) the distance D between the box and the reference array (CLOSE 13 cm – MIDDLE 30 cm – FAR 53 cm), (2) the distance between the tip of the endoscope and the navigation array on the endoscope (close 5 cm – middle 10 cm – far 20 cm), (3) the working angulation of the endoscope (0°-endoscope and 30°-endoscope angled at 90° and 45° with the box). Results The median precision was 1.3 mm (Q1: 1.1; Q3: 1.7) with the best setting CLOSE/close. The best setting in surgical condition (CLOSE/far) showed a distance-to-target of 2.3 mm (Q1: 1.9; Q3: 2.5). The distance D was correlated to the precision (Spearman rho = 0.82), but not the distance d (Spearman rho = 0.04). The type of optic and its angulation with the box were also correlated to the precision (Spearman rho = − 0.37). The best setting was the use of a 30°-endoscope angled at 45° (1.4 mm (Q1: 1.0; Q3: 1.9)). Conclusion Navigated-assisted endoscopy is feasible and offers a good precision. The navigation setup should be optimized, reducing the risk of inadvertent perifocal damage.

Abstract:Several causes of spatial errors in surgical navigation system have been investigated for several systems. These include identification error of the external features used for registration, geometrical distortion in the preoperative images, and tracking error of the surgical instruments. Another potentially important source of error is the patient head position detection retrospectively and concurrent with surgery. The visualization of target zone is carried out according to reference landmark points inside the center of the brain as well as an automatic algorithm for contour recognition (based on 3D segmentation and computer morphometry) was applied to edge the contour. Moreover, the optical navigation system was used to assess the accuracy of determination the position of the surgical instrument inside patient head construction using the "internal map" describing the anatomy of the brain. The major sources of errors occurring in the unit of position detection of the optical navigation system were considered. Furthermore, parts of the errors that could appear were simulated while operating the surgical navigation system. Eventually the research included the possibility of using different filters depending on natural source of noise that was also simulated and analyzed.

Abstract:Background: The optoelectronic camera source and data interpolation serve as the foundation for navigational integrity in the robotic-assisted surgical platform. The objective of the current systematic review serves to provide a basis for the numerical disparity that exists when comparing the intrinsic accuracy of optoelectronic cameras: accuracy observed in the laboratory setting versus accuracy in the clinical operative environment. It is postulated that there exists a greater number of connections in the optoelectronic kinematic chain when analyzing the clinical operative environment to the laboratory setting. This increase in data interpolation, coupled with intraoperative workflow challenges, reduces the degree of accuracy based on surgical application and to that observed in controlled musculoskeletal kinematic laboratory investigations. Methods: Review of the PubMed and Cochrane Library research databases was performed. The exhaustive literature compilation obtained was then vetted to reduce redundancies and categorized into topics of intrinsic optoelectronic accuracy, registration accuracy, musculoskeletal kinematic platforms, and clinical operative platforms. Results: A total of 147 references make up the basis for the current analysis. Regardless of application, the common denominators affecting overall optoelectronic accuracy are intrinsic accuracy, registration accuracy, and application accuracy. Intrinsic accuracy of optoelectronic tracking equaled or was less than 0.1 mm of translation and 0.1° of rotation per fiducial. Controlled laboratory platforms reported 0.1 to 0.5 mm of translation and 0.1°-1.0° of rotation per array. There is a huge falloff in clinical applications: accuracy in robotic-assisted spinal surgery reported 1.5 to 6.0 mm of translation and 1.5° to 5.0° of rotation when comparing planned to final implant position. Total Joint Robotics and da Vinci urologic robotics computed accuracy, as predicted, lies between these two extremes-1.02 mm for da Vinci and 2 mm for MAKO. Conclusions: Navigational integrity and maintenance of fidelity of optoelectronic data is the cornerstone of robotic-assisted spinal surgery. Transitioning from controlled laboratory to clinical operative environments requires an increased number of steps in the optoelectronic kinematic chain and error potential. Diligence in planning, fiducial positioning, system registration, and intraoperative workflow have the potential to improve accuracy and decrease disparity between planned and final implant position. The key determining factors limiting navigation resolution accuracy are highlighted by this Cochrane research analysis.

Accuracy Study of a Binocular-Stereo-vision-based Navigation Robot for Minimally Invasive Interventional Procedures

Abstract:BACKGROUND:Medical robot is a promising surgical tool, but no specific one has been designed for interventional treatment of chronic pain. We developed a computed tomography-image based navigation robot using a new registration method with binocular vision. This kind of robot is appropriate for minimal invasive interventional procedures and easy to operate. The feasibility, accuracy and stability of this new robot need to be tested. AIM:To assess quantitatively the feasibility, accuracy and stability of the binocular-stereo-vision-based navigation robot for minimally invasive interventional procedures. METHODS:A box model was designed for assessing the accuracy for targets at different distances. Nine (three sets) lead spheres were embedded in the model as puncture goals. The entry-to-target distances were set 50 mm (short-distance), 100 mm (medium-distance) and 150 mm (long-distance). Puncture procedure was repeated three times for each goal. The Euclidian error of each puncture was calculated and statistically analyzed. Three head phantoms were used to explore the clinical feasibility and stability. Three independent operators conducted foramen ovale placement on head phantoms (both sides) by freehand or under the guidance of robot (18 punctures with each method). The operation time, adjustment time and one-time success rate were recorded, and the two guidance methods were compared. RESULTS:On the box model, the mean puncture errors of navigation robot were 1.7 ± 0.9 mm for the short-distance target, 2.4 ± 1.0 mm for the moderate target and 4.4 ± 1.4 mm for the long-distance target. On the head phantom, no obvious differences in operation time and adjustment time were found among the three performers (P > 0.05). The median adjustment time was significantly less under the guidance of the robot than under free hand. The one-time success rate was significantly higher with the robot (P < 0.05). There was no obvious difference in operation time between the two methods (P > 0.05). CONCLUSION:In the laboratory environment, accuracy of binocular-stereo-vision-based navigation robot is acceptable for target at 100 mm depth or less. Compared with freehand, foramen ovale placement accuracy can be improved with robot guidance.
Analysis of influence of surgical instruments on accuracy of magnetic navigation system for craniofacial surgery robots

Various surgical instruments have been evaluated to effectively reduce the interference of magnetic navigation systems with surgical instruments. In the surgical environment, the use of surgical instruments during mandibular surgery was simulated by selecting several conventional surgical instruments to record errors in the magnetic navigation system. The fluctuation values of the magnetic navigation errors were subsequently estimated and changes in its accuracy measured. MATLAB was used to calculate and analyze the fluctuations of the magnetic navigation errors. As results, the high-frequency electrosurgical system caused the greatest interference with the magnetic navigation system during surgery while powered on, with a maximum fluctuation error value of 1.8120 mm, and the maximum fluctuation error values of the stitch scissors, teeth forceps, and a needle holder were 1.3662, 1.3781, and 0.3912 mm, respectively. The closer the instrument is to the magnetic field generator or navigation target, the greater its impact. In conclusion, stitch scissors, teeth forceps, a needle holder, and the high-frequency electrosurgical system all affect magnetic navigation system accuracy. Therefore, it is necessary to avoid magnetic navigation system use and surgical instrument disturbances during surgery or select surgical instruments that do not interfere with the system. Surgical instruments must be evaluated for electromagnetic interference before they can be used in surgery with a magnetic navigation system.
System to assess accuracy of micromanipulation

Abstract:The authors had previously developed an optical micro motion sensing system (M2S2) using a pair of orthogonally placed position sensitive detectors (PSD) to track 3D displacement of the tip of a microsurgical instrument in real-time. In the M2S2 system, an infrared (IR) diode is used to illuminate the workspace. A ball is attached to the tip of an intraocular shaft to reflect IR rays onto the PSDs. Instrument tip position is then calculated from the centroid position of reflected IR light on the PSDs. The M2S2 system together with a test platform is used as an evaluation system to assess the accuracy and physiological tremor of subjects performing micromanipulation tasks. Since the need to use the ball at the instrument tip prevents the subjects from performing the manipulation tasks precisely, a laser light is provided as a guide for the subjects to aim at the target precisely. A laser diode module is placed inside the instrument to provide the required laser light. The instrument intraocular shaft is replaced with a same-sized hollow tube to let the laser light from the instrument pass through down to the target. The laser light spot position on the platform is used to access the performance of the subjects. The laser light spot position is calculated from the tilt angle information provided by an accelerometer placed inside the instrument, and the instrument tip position information given by M2S2.
The influence of the da Vinci surgical robot on electromagnetic tracking in a clinical environment

Abstract:Robot-assisted surgery is increasingly used in surgery for cancer. Reduced overview and loss of anatomical orientation are challenges that might be solved with image-guided surgical navigation using electromagnetic tracking (EMT). However, the robot's presence may distort the electromagnetic field, affecting EMT accuracy. The aim of this study was to evaluate the robot's influence on EMT accuracy. For this purpose, two different electromagnetic field generators were used inside a clinical surgical environment: a table top field generator (TTFG) and a planar field generator (PFG). The position and orientation of sensors within the electromagnetic field were measured using an accurate in-house developed 3D board. Baseline accuracy was measured without the robot, followed by stepwise introduction of potential distortion sources (robot and robotic instruments). The absolute accuracy was determined within the entire 3D board and in the clinical working volume. For the baseline setup, median errors in the entire tracking volume within the 3D board were 0.9 mm and 0.3° (TTFG), and 1.1 mm and 0.4° (PFG). Adding the robot and instruments did not affect the TTFG's position accuracy ( p = 0.60), while the PFG's accuracies decreased to 1.5 mm and 0.7° ( p < 0.001). For both field generators, when adding robot and instruments, accuracies inside the clinical working volume were higher compared to the entire tracking 3D board volume, 0.7 mm and 0.3° (TTFG), and 1.1 mm and 0.7° (PFG). Introduction of a surgical robot and robotic instruments shows limited distortion of the EMT field, allowing sufficient accuracy for surgical navigation in robotic procedures.
[Grading of astrocytomas and oligodendrogliomas].

Abstract:The histologic determination of the degree of tissue anaplasia and grade of malignancy of gliomas is based upon qualitative histological features (nuclear pleomorphism, mitoses, endothelial proliferation, tumor necrosis). This grading approach is influenced by the subjective interpretation of the pathologist, especially concerning the weighting of criteria (scant, moderate, pronounced). An observer-independent approach seems to be feasible by abandoning the concept of parameter weighting in favor of an binary approach noting only the presence or absence of these structure parameters. This grading procedure is recognized in the revised WHO classification of brain tumors for common type astrocytomas (Ste. Anne-Mayo System, SAMS). Our results indicate that a similar approach is also suitable for grading purposes of oligodendrogliomas and mixed gliomas. Our recent investigations on glioma grading showed, both for astrocytomas and oligodendrogliomas, that a two-tiered grading scheme distinguishing only "low-grade" and "high grade" cases was prognostically relevant. For all glioma entities the onset of tumor angiogenesis with endothelial proliferation and contrast enhancement in CT and MRI seems to be the key criterion indicating irreversible tumor progression to the "high" malignancy grade.
Neuronavigation in endoscopic skull base surgery and the accuracy of different MRI sequences

Abstract:Objective: Neuronavigation is common technology used by skull base teams when performing endoscopic endonasal surgery. A common practice of MRI imagining is to obtain 3D isotopic gadolinium enhanced T1W magnetisation prepared rapid gradient echo (MPRAGE) sequences. These are prone to distortion when undertaken on 3 T magnets. The aim of this project is to compare the in vivo accuracy of MRI sequences between current and new high resolution 3D sequences. The goal is to determine if geometric distortion significantly affects neuronavigation accuracy. Methods: Patients were scanned with a 3D T1 MPRAGE sequence, 3D T1 SPACE sequence and a CT stereotactic localisation. Following general anaesthesia, patients were registered on the Stealth Station (Medtronic, USA) using a side mount emitter for Electromagnetic navigation. A variety of surgically relevant anatomical landmarks in the sagittal and coronal plane were selected with real and virtual data points measured. Results: A total of 10 patients agreed be enrolled in the study with datapoints collected during surgery. The distance between real and virtual datapoints trended to be lower in SPACE sequences compared to MPRAGE. Paired t test did not demonstrate a significant difference. Conclusion: We have demonstrated that navigational accuracy is not significantly affected by the type of MRI sequence selected and that current corrective algorithms are sufficient. Navigational accuracy is affected by many factors, with registration error likely playing the most significant role. Further research involving real time imaging such as endoscopic ultrasound may hopefully address this potential error.

Abstract:Purpose: The integration of a surgical robotic instrument tracking module within optical microscopes holds the potential to advance microsurgery practices, as it facilitates automated camera movements, thereby augmenting the surgeon's capability in executing surgical procedures. Methods: In the present work, an innovative detection backbone based on spatial attention module is implemented to enhance the detection accuracy of small objects within the image. Additionally, we have introduced a robust data association technique, capable to re-track surgical instrument, mainly based on the knowledge of the dual-instrument robotics system, Intersection over Union metric and Kalman filter. Results: The effectiveness of this pipeline was evaluated through testing on a dataset comprising ten manually annotated videos of anastomosis procedures involving either animal or phantom vessels, exploiting the Symani®Surgical System-a dedicated robotic platform designed for microsurgery. The multiple object tracking precision (MOTP) and the multiple object tracking accuracy (MOTA) are used to evaluate the performance of the proposed approach, and a new metric is computed to demonstrate the efficacy in stabilizing the tracking result along the video frames. An average MOTP of 74±0.06% and a MOTA of 99±0.03% over the test videos were found. Conclusion: These results confirm the potential of the proposed approach in enhancing precision and reliability in microsurgical instrument tracking. Thus, the integration of attention mechanisms and a tailored data association module could be a solid base for automatizing the motion of optical microscopes.
Experimental Research on Laser Tracking System with Galvanometer Scanner for Measuring Spatial Coordinates of Moving Target

Abstract:The spatial position of industrial object, such as robot end-effector, is an important geometric parameter whose accuracy determines whether robot can perform accurately. Therefore, we have established a laser tracking and coordinate measuring system with galvanometer scanner for high accuracy, large range, non-contact, and spatial dynamic measurement. Zn this paper, the laser tracking system and its setup are illuminated at first. Then, the formulae for calculating coordinates are deduced, and the calibration method of the initial distance from tracking mirror to target is presented. After that, two preliminary experiments in different distances are described. One is on CMM; the other is with grating ruler as reference. In the former, the maximum measurement error of coordinates is 70 mum and the maximum error of length is 35 mum in the 85x100x100mm(3) measurement volume, and in the Im initial distance. In the latter, the maximum error of length is 140 mum in the range of 480mm, and in the 5m initial distance. At the end of the paper, the error sources are analyzed and simulated.

Abstract:BACKGROUND & AIMS Reduced bone mass and increased fracture rate are complications of primary biliary cirrhosis (PBC). The effect of intermittent administration of human parathyroid hormone (hPTH) 1-34 on bone mass and architecture in bile duct-ligated (BDL) rats was studied. METHODS Six-month-old male rats were subjected to BDL or sham operation (SO) and were treated from the second postoperative week intermittently with either hPTH 1-34 40 microg/kg per day, 80 microg/kg per day, or a vehicle for 4 weeks. Femoral and tibial bones were evaluated ex vivo by dual x-ray absorptiometry, microcomputed tomography, and histomorphometry. Serum osteocalcin and urinary deoxypyridinoline cross-links (DPD) were determined. RESULTS BDL rats had decreased bone mass compared with SO rats as indicated by a 6% decrease in femoral and tibial bone mineral density (BMD), 18% reduction in femoral trabecular bone volume (bone volume/total volume [BV/TV]), 17% decrease in trabecular thickness, and 10% decrease in tibial cortical thickness. The administration of hPTH 1-34 at 40 microg/kg per day increased femoral and tibial BMD (9% and 9%), femoral trabecular BV/TV (50%), trabecular thickness (50%), tibial cortical thickness (17%), and serum osteocalcin (82%). On the other hand, hPTH 1-34 80 microg/kg per day had no effect on BMD and tibial cortical thickness, was associated with a smaller increase in trabecular BV/TV (24%), and had a higher osteoclast number and DPD compared with untreated BDL rats and the lower hPTH 1-34 dose treatment group. CONCLUSIONS BDL rats exhibit loss of bone mass and structure, which can be prevented by the intermittent administration of hPTH 1-34, a potential therapy for osteoporosis in PBC.
Performance of Robotic Assistance for Skull Base Biopsy: A Phantom Study.

Abstract:Objectives This study aims to evaluate the feasibility of a custom robot system guided by optical cone beam computed tomography (CBCT)-based navigation for skull base biopsy.Design An accuracy study was conducted.Setting Platform for navigation and robot-aided surgery technology.Participants Phantom skull.Main Outcome Measures The primary outcome measure was to investigate the accuracy of robot-assisted needle biopsy for skull base tumors. A 14-gauge needle was automatically inserted by the five degrees of freedom robot into the intended target, guided by optical navigation. The result was displayed on the graphical user interface after matrix transformation. Postoperative image scanning was performed, and the result was verified with image fusion.Results All 20 interventions were successfully performed. The mean deviation of the needle tip was 0.560.22mm (measured by the navigation system) versus 1.73 +/- 0.60mm (measured by image fusion) ( p <0.05). The mean insertion depth was 52.3mm (range: 49.7-55.2 mm). The mean angular deviations off the x-axis, y-axis, and z-axis were 1.51 +/- 0.67, 2.33 +/- 1.65, and 1.47 +/- 1.16 degrees, respectively.Conclusions The experimental results show the robot system is efficient, reliable, and safe. The navigation accuracy is a significant factor in robotic procedures.
Surgical Tracking, Registration, and Navigation Characterization for Image-guided Renal Interventions

Abstract:Most image-guided interventions rely on surgical tracking and image/model to patient registration to establish a spatial relationship between the patient and the pre- and intraprocedural images, by using surgical tracking and localization systems. In this work, we characterize the tracking, registration and navigation accuracy using two different surgical localization systems - the NDI Polaris Spectra optical tracking system and the NDI Aurora electromagnetic tracking system - in the context of an image-guided renal intervention, using a 3D printed life-size model of a patient-specific kidney phantom generated from a CT image. Our results reported a 0.05 mm fiducial localization error, 0.70 mm fiducial registration error, and 0.78 mm target registration error, and 0.63 mm overall navigation error using the optical tracking, and 0.12 mm fiducial localization error, 0.78 mm fiducial registration error, 0.93 mm target registration error and 0.89 mm overall navigation error using electromagnetic tracking. Additionally, our study also showed similarity between the overall navigation accuracy using optical (0.63 mm RMS error) or electromagnetic tracking (0.89 mm RMS error) and the overall navigation accuracy achieved using direct visualization of the surgical scene (0.68 mm and 1.06 RMS error respectively), which serves as a baseline control metric.
