Abstract:Objective To evaluate oncoplastic techniques on breast conservative surgery.Methods Clinical data of 50 breast cancer patients who received oncoplastic breast conservative surgery from October 2009 to August 2011 were retrospectively analyzed.The indication,methods,outcome and morbility of oncoplastic surgery were assessed. Results All patients had solitary tumors.Twelve patients received neoadjuvant therapy and thirty-eight received upfront surgical therapy.Different oncoplastic surgery techniques were applied based on the location of tumor.There were 20 tumors located in upper outer quadrant receiving lateral radial or horizontal excision; Four lower inner and seven lower outer tumors received V and J shape excision respectively; Three low pole and seven upper pole tumors received inverted T and batwing excision respectively; Six tumors located periareolar area and three tumors located retroareolar area were removed using round block and J shape excision.Two patients experienced postoperative hematoma and three patients had incisional dehiscence followed by secondary infection. Another developed skin ischemia and necrosis. Thirty-five patients have finished the postoperative adjuvant radiotherapy. The appearances of the breasts were satisfactory in all patients except one having deviation of nipple and two patients suffering from dimpling in incisional region.Patients gave high scores about their breast appearance.After 4- 27 months follow-up,two patients had local recurrence and received mastectomy.Other patients were free of recurrence. Conclusions Breast oncoplastic surgery technique benefits those whose tumors located in special area of the breast resulting in a satisfactory cosmetical appearance of the breast.
Efficacy and safety of oncoplastic breast-conserving surgery versus conventional breast-conserving surgery: An updated meta-analysis

Abstract:Introduction: Breast cancer is the most common cancer among women. The surgical treatment of breast cancer has transitioned progressively from radical mastectomy to breast-conserving surgery. In this meta-analysis, we are aiming to compare oncoplastic breast-conserving surgery (OS) with conventional breast-conserving surgery (BCS) in terms of efficacy and safety. Methods: We searched Medline, Web of Science, Embase, Cochrane databases, Clinicaltrial.gov, and CNKI until April 30, 2024. Data from cohort studies and randomized controlled trials (RCTs) were included. Outcomes included primary outcomes (re-excision, local recurrence, positive surgical margin, mastectomy), secondary outcomes and safety outcomes. The Cochrane Risk of Bias Assessment Tool and Newcastle-Ottawa Scale were used to evaluate the quality of outcomes. Results: Our study included 52 studies containing 46,835 patients. Primary outcomes comprise re-excision, local recurrence, positive surgical margin, and mastectomy, there were significant differences favoring OS over BCS (RR 0.68 [0.56, 0.82], RR 0.62 [0.47, 0.82], RR 0.76 [0.59, 0.98], RR 0.66 [0.44, 0.98] respectively), indicating superior efficacy of OS. Additionally, OS demonstrated significant aesthetic benefits (RR 1.17 [1.03, 1.33] and RR 1.34 [1.18, 1.52]). While total complications were significantly fewer in the OS group (RR 0.70 [0.53, 0.94]), the differences in specific complications were not significant. Furthermore, subgroup analyses were conducted based on nationality, sample size, quality, and type. Conclusion: OS demonstrates either superior or at least comparable outcomes across various aspects when compared to BCS.

Abstract:Objective To evaluate the short-term clinical effect and cosmetic outcome of oncoplastic techniques applied in breast-conserving surgery. Method Clinical data of 25 breast cancer patients who received breast-conserv-ing surgery with gland tissue flap or latissimus dorsi myocutaneous flap following breast reconstruction (observation group), and of another 25 patients that received traditional breast conservative surgery (control group) were retrospec-tively analyzed. The resection range, margin, RTOG cosmetic outcome evaluation, postoperative complications and re-currence or metastasis of the two groups were compared. Result Among the observation group, 21 cases received immediate breast reconstruction with adjacent gland tissue flap and 4 cases were with latissimus dorsi myocutaneous flap. The resection volume of the breast in observation group ranged from 26 to 1105 cm3, with a median volume of 343 cm3, while that of the control group ranged from 15 to 504 cm3, with a median volume of 262 cm3. And there were 3 cases in observation group, and 5 cases in control group that had positive surgical margins confirmed by intra-operative pathologic test of the frozen sections, while no significant differences were observed (χ2=0.504, P=0.478). As for RTOG cosmetic outcome evaluation, number of patients with excellent or good results were 22 (88%) and 20 (80%) in the observation group and control group, respectively, and there were still no significant differences between the two groups (χ2=0.611, P=0.737). One patient in the observation group had multiple bone metastasis in 2 months after surgery, and no recurrence or death was observed after a median follow-up of 5 months (range 2-12 months). Conclusion Application of plastic breast-conserving surgery in early breast cancer on the basis of negative surgical margins may bring about better cosmetic effect, with satisfactory short-term clinical outcome, making it an option that is worth further clinical practice.

Abstract:Abstract Background Oncoplastic surgery is an expanding approach for managing breast conservation, as it offers improved cosmetic outcomes while maintaining oncological safety. Objectives This study aimed to assess the oncologic and surgical outcomes of patients who underwent oncoplastic procedures at our institution. Methods The study population includes all consecutive breast cancer patients who underwent lumpectomy followed by immediate oncoplastic breast reconstruction with contralateral breast adjustment between 2010 and 2021. Following IRB approval, we evaluated patient demographics, tumor characteristics, surgical details, complications, and follow-up for this group. Quality of life (QOL) and patient-reported outcome measures (PROM) were assessed through questionnaires. Results Following multidisciplinary tumor board discussion 77 patients (82 breasts) underwent oncoplastic surgery. 92.2% underwent breast MRI prior to surgery with mean mass enhancement of 2.35 cm (0-8.5) and non-mass enhancement of 5.77 cm [0-14 cm]), 44% presented with multifocal disease. The final positive surgical margin rate was 2.4%, and all conserved breasts received adjuvant radiation therapy. At 5 years, local recurrence rate was 4.1% for invasive tumors. QOL scores were generally high, with most patients reporting good-excellent cosmetic outcomes and high personal satisfaction with breast appearance and softness. Physicians reported breast symmetry in 89.5%. Conclusions The findings suggest that oncoplastic surgery with contralateral breast adjustment is safe, effective, and excellent option for breast cancer patients wishing for breast conservation even in large or multifocal masses, with favorable oncologic and cosmetic outcomes. The QOL analysis indicates high patient satisfaction. These results support the use of oncoplastic surgery in the management of breast cancer.

Abstract:Abstract Purpose Breast cancer patients who are in line with breast conserving are faced more challenges in making surgery decision, because breast conservation and mastectomy have equivalent oncologic outcomes. The purpose of this study is to analyze the surgical decision-making process for breast cancer patients and further explore the reasons why do Chinese women who are in line with breast conserving are more likely to choose mastectomy. Methods A qualitative study was conducted. With the sample saturation principle, data collected by semi-structured interviews with 14 breast cancer patients who are in line with breast conserving. Colaizzi’s method of phenomenology was used for data analysis. Results Three major themes emerged from the data. The themes were the following: (1) lack of significant decision-making support (stereotyping of breast cancer surgery, Inferior quality of interaction with health professions, and without the help of decision aids); (2) can’t mull under the strike (immediacy of the decision, breasts are out of deliberating, and escape); and (3) edified by the Chinese culture (deep family values, hiding concerns about femininity, and conservative character). Conclusions Our findings explore the reasons why more Chinese breast cancer patients with breast conserving conditions choose mastectomy, and help patients to choose knowledge-based treatment options which are concordant with their needs, values, and preferences.

Abstract:Breast aesthetics is becoming increasingly important in breast cancer surgery due to changes in patient expectations and greater emphasis been placed on the psychosocial outcomes. Studies have shown no difference in local recurrence risk between mastectomy and breast conserving surgery (BCS) and also a higher overall survival rate after BCS. Breast preservation improves the quality of life substantially compared to mastectomy. Oncoplastic breast-conserving surgery (O-BCS) involves tumour excision whilst overcoming the limitations of standard breast conserving surgery (S-BCS) by allowing larger resection volumes, avoiding deformities with better aesthetic results. Our study aims to compare the oncosurgical and aesthetic outcomes of O-BCS versus S-BCS among women in Sri Lanka.

Abstract:To determine the impact of breast conservation on quality of life and identify treatment-related and other demographic factors associated with post-breast cancer treatment quality of life. A prospective study was conducted on 392 women who underwent breast cancer surgery at Hangzhou Cancer Hospital from January 1, 2013, to December 31, 2022. Operable breast cancer patients who had completed all treatments except endocrine therapy were included. Patients with tumor recurrence/metastasis, bilateral or male breast cancer, and other primary malignancies were excluded. After enrollment, patients were asked to complete the BREAST-Q scale, and their pathological and medical records were reviewed. Analysis of variance was used to compare the quality of life scores among the groups. Univariate and multivariate linear regression analyses were performed to identify independent factors associated with quality of life scores in different domains. Participants completed the BREAST-Q scale at a median of 4.6 years after surgery. Quality of life scores varied based on the therapeutic strategy. Breast conservation has significant advantages over mastectomy in terms of breast satisfaction, psychosocial, and sexual well-being. Compared to oncoplastic breast-conserving surgery, mastectomy was independently associated with decreased breast satisfaction, psychosocial, and sexual well-being, while conventional breast-conserving surgery showed comparable outcomes to oncoplastic breast-conserving surgery in terms of these factors. Breast conservation leads to an improvement in quality of life compared to mastectomy. Oncoplastic breast-conserving surgery does not lead to a decrease in quality of life compared to conventional breast-conserving surgery and offers better outcomes compared to mastectomy.

Abstract:Introduction: Local treatment can be distressful to breast cancer patients. We aimed to evaluate how different types of local treatment impact the quality of life of patients. Methods: In this retrospective cohort study, one-year postoperative Breast-Q Satisfaction with Breasts scores were used as a surrogate for Quality of Life. Linear regression was used to estimate the impact of breast conservation, oncoplastic surgery, breast reconstruction, and radiation therapy on Breast-Q scores. All analyses were adjusted for multiple covariates. Results: Of the 711 eligible patients, 349 female patients answered both the pre- and one-year postoperative questionnaires and were included in the final analysis. In total, 237 (68%) patients underwent breast-conserving surgeries and 112 (32%) underwent mastectomies. All mastectomy patients underwent breast reconstruction and 176 (74% of breast-conserving surgeries) underwent concomitant oncoplastic surgery. After multivariate analysis, mastectomy was associated with lower scores compared to breast-conserving surgery (-21.3; 95%CI: -36.2, -6.4, p=0.005), and oncoplastic surgery was associated with higher scores (9.2; 95%CI: 0.8, 17.6, p=0.032). There was a tendency for higher scores with the use of flaps in breast reconstruction and a tendency for lower scores with the use of radiation therapy, but the difference was not significant. Conclusions: Breast-conserving surgery is associated with better quality of life than mastectomy. Additionally, oncoplastic surgery is associated with a better quality of life than standard breast-conserving surgery. Patients should be counseled whenever multiple options for surgery are possible, and efforts should be made to increase the availability of trained surgeons in oncoplastic techniques.

Abstract:Background: Oncoplastic breast-conserving surgery may allow women with early breast cancer to avoid a mastectomy, but many women undergo more extensive surgery, even when breast-conserving options are offered. The aim of the ANTHEM qualitative study was to explore factors influencing women's surgical decision-making for and against oncoplastic breast-conserving surgery. Methods: Semi-structured interviews were conducted with a purposive sample of women who had received either oncoplastic breast-conserving surgery or a mastectomy with or without immediate breast reconstruction to explore their rationale for procedure choice. Interviews were transcribed verbatim and analysed thematically. Trial registration number: ISRCTN18238549. Results: A total of 27 women from 12 centres were interviewed. Out of these, 12 had chosen oncoplastic breast-conserving surgery and 15 had chosen a mastectomy with or without immediate breast reconstruction. Overwhelmingly, women's decisions were guided by their surgical teams. Decision-making for and against oncoplastic breast-conserving surgery was influenced by three key inter-related factors: perceptions of oncological safety; the importance of maintaining/restoring femininity and body image; and practical issues. Oncological safety was paramount. Women who reported feeling reassured that oncoplastic breast-conserving surgery was oncologically safe were happy to choose this option. Those who were not reassured were more likely to opt for a mastectomy, as a perceived 'safer' option. Most women wished to maintain/restore femininity, with the offer of immediate breast reconstruction essential to make a mastectomy an acceptable option. Practical issues such as the perceived magnitude of the surgery were a lesser concern. Conclusion: Decision-making is complex and heavily influenced by the surgical team. High-quality, accurate information about surgical options, including appropriate reassurance about the short- and long-term oncological safety of oncoplastic breast-conserving surgery is vital if women are to make fully informed decisions.

Abstract:The development of surgical technology, advances in a chemotherapy and radiation therapy for the treatment of the breast cancer is a solution of not only medical, but also aesthetic problems. A multidisciplinary approach to the treatment of breast cancer has made it possible to use less aggressive tactics of surgical treatment, complementing the operation with other methods of neoadjuvant and adjuvant therapy. Despite the fact is a modern conditions the issue of rehabilitation and preservation of the quality of life of patients after surgical treatment is more acute than ever, today there are no collecting recommendations for the use of breast conserving treatment methods for breast cancer, it is can be useful to adequately assess the risk of tumor recurrence and it is advisable to determine the scope of surgical treatment. However, the experience accumulated by surgeons allows us to identify risk factors for tumor recurrence depending on the characteristics of its location and morphological structure of the tumor. Considering the possibilities of modern neoadjuvant chemotherapy, even with an unfavorable immunohistochemical type of tumor, we can offer patients the choice of breast conserving treatment. This issue is especially acute among young patients, the proportion of whom is increasing every year. In this connection, we can achieve faster treatment of breast cancer, as well as achieve a good aesthetic result with minimal surgical interventions, without the help of foreign materials (implants or other mesh).

Abstract:We read with great interest the article by Klement et al. regarding the impact of types of postmastectomy reconstruction on complications and patient-reported outcomes in the obese group, entitled "Microsurgical Breast Reconstruction in the Obese: A Better Option Than Tissue Expander/Implant Reconstruction?"<sup><sup><a class="ejp-citation-link js-ejp-citation-link">1</a></sup></sup> This article studied the impact of types of postmastectomy reconstruction on complications and patient-reported outcomes in an obese group of patients, which enables surgeons and patients to make better choices for breast reconstruction by providing communication with accurate risk data. Inspired by the authors, we would like to express our considerations of this study.

Abstract:Background: This study explores the surgical management and reconstruction options for augmented breasts in the context of breast conservation therapy (BCT) for breast cancer. We hypothesized that there would be no difference in the rates of complications, revisions, or patient satisfaction in patients who maintained their breast augmentation versus those that had their implants removed in the context of BCT. Methods: We conducted a retrospective review of 142 patients who underwent BCT at a single center from March 2016 to March 2022. The study included patients who had a preexistent cosmetic augmentation at the time of breast cancer diagnosis and BCT. Patient demographics, clinical and treatment characteristics, breast implant details, reconstructive technique, complications, and revisions were recorded. Patient-reported outcomes were assessed using the BREAST-Q Breast-Conserving Therapy module. Results: Ninety-three (65.5%) patients chose to maintain their implants, while 49 (34.5%) elected to have them removed during BCT. Patients with submuscular implants were more likely to maintain their implants. Oncoplastic mastopexy was associated with higher complication rates, particularly in patients opting for implant downsizing. However, multivariate logistic regression did not identify implant management strategy as an independent predictor for complications or revisions. Surgical site infection was the only predictor of implant explantation. Patient-reported outcomes did not differ significantly between the different implant management cohorts. Conclusions: This study demonstrates that maintaining breast implants during BCT does not increase the risk of complications or revisions. Overall, BCT in augmented women was found to be a safe approach, with high patient satisfaction.

Abstract:Background Compared with older patients, young women with breast cancer (YWBCs) have a poorer prognosis and a higher risk of recurrence. Ages ≤35 years are independent risk factors for local recurrence of breast cancer. Surgery is the most important local treatment for YWBC, and there is still a lack of prospective studies comparing surgical options for recurrence and survival. We retrospectively compared the effects of surgical options on disease-free survival (DFS) and overall survival (OS) of YWBC at Fudan University Shanghai Cancer Center (FUSCC). Methods YWBCs (age ≤35 years) who underwent surgery at FUSCC between 2008 and 2016 were retrospectively analyzed and divided into three groups according to surgical options: 1) breast-conserving surgery (BCS), 2) mastectomy alone (M), and 3) mastectomy with reconstruction (RECON). The DFS and OS outcome rates from the three surgical options were compared using the Kaplan–Meier method and Cox regression model. Propensity score matching (PSM) was also used to balance the baseline characteristics to eliminate selection bias. Results A total of 1,520 YWBCs were enrolled with a median follow-up of 5.1 years, including 524 patients (34.5%) who underwent BCS, 676 patients (44.5%) who underwent M, and 320 patients (21.1%) who underwent RECON. The 5-year DFS rates were 96%, 87%, and 93%, respectively ( P < 0.001); the 5-year OS rates were 98%, 94%, and 97%, respectively ( P = 0.002). Multivariate Cox analysis showed that DFS and OS were significantly improved in patients undergoing BCS compared with those undergoing M, with hazard ratios (HR) of 0.448 (95% CI 0.276–0.728; P = 0.001) and 0.405 (95% CI 0.206–0.797, P = 0.009), respectively. After PSM, DFS and OS rates were significantly improved in patients undergoing BCS compared to patients undergoing M (DFS, P = 0.001; OS, P = 0.009); RECON was also improved compared to patients undergoing M in terms of DFS and OS, but the difference was not statistically significant (DFS, P = 0.164; OS, P = 0.130). Conclusions The surgical options were independent factors affecting DFS and OS in YWBC, and the DFS and OS rates were significantly improved in the BCS group compared to those in the M group. BCS is preferred for early YWBC, and RECON is the best option for remodeling the body images of YWBC who do not have breast-conserving conditions.

Abstract:Objective To evaluate the safety of breast-conserving surgery for ductal carcrnoma in situ (DCIS).Methods One hundred and nineteen patients with pathologically confirmed DCIS were analyzed retrospectively. The relationship among local recurrence and distant metastasis with relevant factors including surgical procedure, age, tumor size, estrogen/progesterone status and human epithelial receptor 2 (HER-2) status were analyzed by univariate and multivariate methods. Results In this study 48 patients received breast conserving surgery,71 patients received mastectomy. All patients were followed up for 7-132 months (median 35 months). Chest wall recurrence was found in mastectomy group in one case and none in ipsilateral breast tumor recurrence (IBTR) in breast conservation group (P =0. 384). Two patients had distant metastasis in mastectomy group, and one patient in breast conserving group ( P =0. 383 ). The three year disease-free survival (DFS) were 100％ and 94. 4％ in breast conserving and mastectomy groups respectively(P =0. 225). Univariate analysis showed that age( ＜40 years)was the risk factor for distant metastasis(P =0. 035) ,a factor not confirmed by multivariate analysis (P =0. 086). Conclusions There is no significant difference on local recurrence and distant metastasis between breast conserving surgery and mastectomy for DCIS of the breast.

Abstract:Objective To study different surgical methods for breast cancer patients under the influence of the quality of life at different stages. Methods The clinical data of finishing our hospital 160 cases of breast cancer patients , ac-cording to the patient's surgical approach , divided into breast-conserving resection of breast conserving surgery group , mastectomy breast reconstruction group, modified radical group, using the method of returning scale investigation, the statistical quality of life of patients at different stages. Results The patients in the perioperative period, physical, psy-chological and other breast-conserving surgery group and mastectomy breast reconstruction group than modified radical group ( P<0 . 05 ) , with significant difference , but there were no significant difference in the quality of life between the two groups (P>0.05); after 2 years and 5 years, breast-conserving surgery and mastectomy breast reconstruction group's quality of life was higher than that in the modified radical group (P<0.05), with significant difference between the two groups. The patient's physiological , psychological and other aspects of the situation was not statistically signif-icant (P>0.05);perioperative period, after 2 years, a period of 5 years, breast-conserving surgery group and mastecto-my breast reconstruction patients physical, psychological status, quality of life was not statistically significance (P>0.05). Conclusion Breast cancer treatment involves the patient's physical structure , resulting in a greater impact on quality of life and psychological state of the patient's will to a large extent, in the optional cases, give priority to breast-con-serving surgery or mastectomy breast reconstruction may be to some extent , improve the quality of life of patients.

Abstract:In addition to conventional breast‐conserving surgery (BCS), oncoplastic breast surgery (OBS) is an operation technique that strives simultaneously to increase oncological safety and patient's satisfaction. It is the combination of the best‐proven techniques in plastic surgery with surgery for breast cancer. In a growing number of indications, OBS overcomes the limit of conventional BCS by allowing larger resection volumes while avoiding deformities. The aim of our retrospective study (2012–2014) was to compare oncological outcomes of OBS versus BCS.We compared two groups of patients with primary non‐metastatic breast tumours: group A (n = 291), where BCS was performed, versus group B (n = 52), where OBS was performed. Surgical interventions were performed in German and Swiss teaching hospital settings. The surgeon for group B had subspecialist training in OBS. We assessed outcome in term of re‐excision rates, resection margin and complications. Groups were homogenous (no significant differences in terms of age, tumour size, tumour type or grade). The resection margin was larger in group B (7 mm) than in group A (3 mm). Re‐excision rate of group B (8%) was significantly lower than in group A (31%). Complication rates were comparably low in groups A and B.Despite the limits of retrospective design, our study confirms that OBS is safe and reduces the re‐excision rates and the need for further surgery. OBS has the potential to improve oncological care and should be more widely adopted.
