Abstract:Laser tissue welding is a hot issue in surgical research due to its advantages of non-contact, instant water tightness, no suture removal, no rejection and anti-infection. However, the contradiction between strength and thermal damage has not been solved completely. The addition of dye can solve the problem. Therefore, in order to improve the welding strength and quality, this paper studies the influence of different position and concentration of dye on the welding strength and determines the optimal concentration and applied position. Two kinds of flux were prepared, which were based on bovine serum albumin solution and stained with indocyanine green and methylene blue respectively. Comparing the welding effect of flux, the indocyanine green group is more remarkable. When the concentration of indocyanine green is 2.0 mg/mL, the maximum tensile strength reaches 19.32 +/- 1.22 N/cm2, which is significantly higher than that of the control group without flux. When the flux is applied to the front side, the tensile strength is higher than the reverse side group. Two kinds of flux can effectively reduce the thermal damage. Our study demonstrates that indocyanine green and methylene blue are significant organic chromophores in laser welding process, which can promote the efficiency of photo-thermal conversion, improve welding strength and reduce thermal damage.
Reconstruction of Soft Biological Tissues Using Laser Soldering Technology with Temperature Control and Biopolymer Nanocomposites

Abstract:Laser soldering is a current biophotonic technique for the surgical recovery of the integrity of soft tissues. This technology involves the use of a device providing laser exposure to the cut edges of the wound with a solder applied. The proposed solder consisted of an aqueous dispersion of biopolymer albumin (25 wt.%), single-walled carbon nanotubes (0.1 wt.%) and exogenous indocyanine green chromophore (0.1 wt.%). Under laser exposure, the dispersion transforms into a nanocomposite due to the absorption of radiation and its conversion into heat. The nanocomposite is a frame structure of carbon nanotubes in a biopolymer matrix, which provides adhesion of the wound edges and the formation of a strong laser weld. A new laser device based on a diode laser (808 nm) has been developed to implement the method. The device has a temperature feedback system based on a bolometric infrared matrix sensor. The system determines the hottest area of the laser weld and adjusts the current supplied to the diode laser to maintain the preset laser heating temperature. The laser soldering technology made it possible to heal linear defects (cuts) in the skin of laboratory animals (rabbits) without the formation of a fibrotic scar compared to the control (suture material). The combined use of a biopolymer nanocomposite solder and a laser device made it possible to achieve a tensile strength of the laser welds of 4 ± 0.4 MPa. The results of the experiment demonstrated that the addition of single-walled carbon nanotubes to the solder composition leads to an increase in the ultimate tensile strength of the laser welds by 80%. The analysis of regenerative and morphological features in the early stages (1-3 days) after surgery revealed small wound gaps, a decrease in inflammation, the absence of microcirculatory disorders and an earlier epithelization of laser welds compared to the control. On the 10th day after the surgical operation, the laser weld was characterized by a thin cosmetic scar and a continuous epidermis covering the defect. An immunohistochemical analysis proved the absence of myofibroblasts in the area of the laser welds.
Nanothermometry-Enabled Intelligent Laser Tissue Soldering

Abstract:While often life-saving, surgical resectioning of diseased tissues puts patients at risk for post-operative complications. Sutures and staples are well-accepted and routinely used to reconnect tissues, however, their mechanical mismatch with biological soft tissue and invasiveness contribute to wound healing complications, infections, and post-operative fluid leakage. In principle, laser tissue soldering offers an attractive, minimally-invasive alternative for seamless soft tissue fusion. However, despite encouraging experimental observations, including accelerated healing and lowered infection risk, critical issues related to temperature monitoring and control during soldering and associated complications have prevented their clinical exploitation to date. Here, intelligent laser tissue soldering (iSoldering) with integrated nanothermometry is introduced as a promising yet unexplored approach to overcome the critical shortcomings of laser tissue soldering. It demonstrates that adding thermoplasmonic and nanothermometry nanoparticles to proteinaceous solders enables heat confinement and non-invasive temperature monitoring and control, offering a route to high-performance, leak-tight tissue sealing even at deep tissue sites. The resulting tissue seals exhibit excellent mechanical properties and resistance to chemically-aggressive digestive fluids, including gastrointestinal juice. The iSolder can be readily cut and shaped by surgeons to optimally fit the tissue defect and can even be applied using infrared light from a medically approved light source, hence fulfilling key prerequisites for application in the operating theatre. Overall, iSoldering enables reproducible and well-controlled high-performance tissue sealing, offering new prospects for its clinical exploitation in diverse fields ranging from cardiovascular to visceral and plastic surgery.
Laser assisted vascular welding with real time temperature control

Abstract:Background and objective: Previous studies in laser assisted vascular welding have been limited by the lack of a reliable end point for tissue fusion. As a means of improving the reproductibility of laser assisted repairs, a system incorporating real time temperature monitoring and closed loop feedback was used. Study design/materials and methods: The system consisted of a direct view infrared thermometer for monitoring the laser heated spot, a 1.9 microns diode laser, and a microprocessor for data acquisition and feedback control of the laser power to maintain a constant tissue temperature. Rat aortas were welded under constant surface temperature conditions. Results: In vivo temperature stability of +/- 2 degrees C was achieved over a temperature range of 70-90 degrees C pertinent to welding small vessels. When welds were completed using the feedback system to maintain the tissue temperature at 80 degrees C, the acute success rate was 100% and the burst pressure was 290 +/- 70 mmHg. Conclusion: These studies demonstrate that the use of real time monitoring and feedback control results in improved consistency for vascular tissue welding.
Temperature-controlled laser photocoagulation of soft tissue: in vivo evaluation using a tissue welding model

Abstract:Background and objectives: Laser surgical procedures involving photocoagulation of soft tissue have relied on subjective visual endpoints. The thermal damage to the denatured tissue in these procedures is highly dependent on the tissue temperatures achieved during laser irradiation. Therefore, a system capable of real time temperature monitoring and closed loop feedback was used to provide temperature controlled photocoagulation (TCPC). Study design/materials and methods: The TCPC system consisted of a 1.32 microns Nd:YAG laser, an infrared thermometer, and a microprocessor for data acquisition and feedback control. A porcine skin model was used. Tissue welds were completed to evaluate the photocoagulation effects at different predetermined temperatures. A quantitative measurement of tissue photocoagulation was obtained by tensile strength measurements of the laser repairs. Histology of the irradiated tissue was used to determine the extent of thermal injury associated with different photocoagulation temperatures. Results: The TCPC system was capable of maintaining a relatively constant temperatures (+/- 4 degrees C) during laser irradiation. The tensile strengths of acute repairs increased with temperature over the range studied (65-95 degrees C). Tensile measurements made after several days of healing showed that higher temperature (95 degrees C) welds had lower strengths than repairs completed at lower (65 degrees C or 75 degrees C) temperatures and were significantly lower at 3 days. Acute histology showed that the amount thermal damage was strongly dependent on the tissue temperature and increased both in tissue depth and lateral to the repair with temperature. The histologic results suggest that the increase in the acute repair tensile strength as the weld temperature increased was due to an increase in the depth of tissue photocoagulation. The increase in the lateral tissue injury measured histologically for higher temperature welds likely resulted in the decreased chronic tensile strengths, as a healing response to excessive thermal damage. Conclusion: Tissue temperatures can be controlled during laser photocoagulation of skin. The degree of acute and chronic tissue damage is highly dependent on the temperature during welding. By controlling the tissue temperature during laser procedures, the surgical outcome can be more reliably predicted and reproduced, as compared to the conventional open loop methods. In addition, the use of a TCPC system should significantly reduce the learning curve for photothermal surgical procedures.
Laser tissue welding by using collagen excitation at a 1,720 nm near-infrared optical window III

Abstract:Laser tissue welding (LTW) is a method of fusing incised tissues together. LTW has the potential to revolutionize plastic surgery and wound healing techniques by its ability to produce watertight, scarless seals with minimal foreign body reaction. While using thermal mechanisms to achieve LTW, energy from the incident laser is absorbed by water in the tissue. As the water temperature increases, partial denaturing of the collagen triple helix briefly occurs, which is quickly followed by renaturation of collagen as the tissue cools, thus providing a watertight seal. This research study investigates the efficacy of direct collagen excitation at 1,720 nm to accomplish LTW. This wavelength falls within the near-infrared (NIR) optical window III. The tensile strengths of pig skin that have been welded with NIR continuous-wave (CW) diode lasers at 1,455 nm, which promote thermal mechanisms of tissue welding, and 1,720 nm wavelengths, are compared. Near-infrared lasers tuned to 1,455 and 1,720 nm were used to weld incised pieces of porcine skin together without extrinsic solders or dyes. The tensile force of the welded tissues was measured using a digital force gauge. The average tensile force of the welded pig skin using the 1,720 nm laser was approximately four times greater than that using the CW 1,455 nm laser, suggesting that LTW accomplished through direct collagen excitation in the NIR optical window III provides greater tensile strengths.
Closed-Feedback Control of Laser Soldering of Rat Skin Using Diode Laser

Abstract:Tissue laser soldering is a method of closure of incisions that in principle may have advantages over conventional closure methods. It is a non-contact technique, continuous and watertight and the procedure is faster and requires less skill to master. However, in practice there have been difficulties in obtaining strong and reliable welding. The quality of the weld depends on the ability to monitor and control the surface temperature of soldering zone during the procedure. Our goal was to use laser soldering to seal cuts in rat skin under temperature feed-back control and compare the results with standard sutures. Full-thickness incision created on the dorsum of rats and albumin solder with Indocyanine- Green (ICG) dye was added between the incision edges and photothermally coagulated with diode laser. We have developed an automated soldering system based on diode laser, IR detector, photodiode, digital thermocouple and camera. The true temperature of the heated tissue was determined by using and improved calibration soft ware method. Laser soldering was carried out spot by spot until it reached a preset value of 70–75°C at each point. The tensile strength of laser soldered cuts was measured after 2–10 days postoperatively and was found comparable to that of sutured cuts. Histopathological studies showed better healing and less inflammatory reactions than that caused by standard sutures after 7th day.

Abstract:Background and objective: Thermal denaturation of proteins is recognized as a rate process governed by the local temperature-time response. Since rate processes are exponential with temperature, laser-assisted tissue welding was performed with and without temperature feedback control (TFC) to investigate the efficacy of temperature feedback in enhancing the photothermal welding process in vivo. Study design/materials and methods: An automated system was developed for temperature feedback controlled laser irradiation. An experimental device incorporating co-aligned laser delivery and temperature detection was used to perform argon laser welded (with and without TFC) enterotomies. The weld strength and histology of laser welded and control sutured enterotomies were compared in an in vivo rat model. Animals (n = 41) were sacrificed at 1, 3, 7, and 21 days postoperatively, and the anastomotic site was removed for bursting/leaking pressure measurements and histological examination. Results: Laser-welded (with and without TFC) and control sutured anastomoses in surviving animals healed comparably. Some laser-welded anastomoses without TFC ruptured spontaneously (4 out of 15) leading to the animals' death within the first 24-36 hours postoperatively. None of the animals in the other groups had this problem (control suture 0/6; laser with TFC 1 leak/8). The bursting/leaking pressures of the laser welded anastomoses were not significantly different than those of the sutured controls. Conclusion: TFC improves the quality of laser-welded rat intestinal anastomoses in vivo in the critical first postoperative 36 hours.
Effect of optical temperature feedback control on patency in laser-soldered microvascular anastomosis

Abstract:Feedback control has been postulated to improve the efficacy of laser welding in microsurgery, but alteration of outcome has not been clearly shown. The authors evaluated the ability of an optical closed loop temperature feedback control to improve patency, aneurysm rate, and to histologically limit thermal damage. Rat femoral artery anastomoses were performed under operating microscope magnification. One hundred and twenty-four anastomoses were performed in five groups using 1) free-hand (FH) 1.9-microm laser soldering without feedback; 2) temperature controlled (TC) 1.9-microm laser soldering with optical feedback; 3) FH 808 nm laser; 4) TC 808 nm laser soldering; and 5) 10-0 nylon suture control. In Groups 2 and 4, an optical feedback system controlling laser exposure to produce a preset temperature was used. Anastomotic time was significantly less for all laser groups (p < 0.05). Late patency for all 1.9-microm laser anastomoses was almost 0. Temperature controlled 808-nm anastomoses showed no significant difference from sutures in terms of patency (88 percent vs. 96 percent), bursting pressure, and aneurysm rate, while freehand 808-nm anastomoses had a significantly lower patency (71 percent) and more tissue damage (ANOVA, p < 0.05). The authors conclude that temperature control improves outcome in microvascular anastomosis by reducing transmural thermal injury caused by variations in surgeon technique.

Abstract:Electron microscopic analysis of gap-junction-like structures isolated from an anthropod (Nephrops norvegicus) and composed of a 16-kDa polypeptide, show the functional unit to be a star-shaped hexamer of protein arranged around a central channel which runs perpendicular to the plane of the membrane. Estimations of the molecular volume carried out on an averaged projection are consistent with a subunit mass of 16-18 kDa. Fourier transform infrared spectroscopy indicates a high alpha-helical content for the protein, supporting secondary-structure predictions of four transmembrane alpha helices/monomer. The averaged projection shows a close resemblance to a hexamer of the 16-kDa protein built on the basis of a four alpha-helical bundle [Finbow, M. E., Eliopoulos, E. E., Jackson, P. J., Keen, J. N., Meagher, L., Thompson, P., Jones, P. C. & Findlay, J. B. C. (1992) Protein Eng. 5, 7-15]. The reconstructed image is also similar to that obtained for gap-junction-like channels isolated from a related arthropod [Homarus americanus; Sikerwar, S. S., Downing, K. H. & Glaeser, R. M. (1991) J. Struct. Biol. 106, 255-263] whose protein content was unknown but which we demonstrate may be composed of a related 16-kDa protein. Previous studies have shown a high sequence identity of the Nephrops 16-kDa protein with the 16-kDa proteolipid subunit c of the vascular H(+)-ATPase, both of which in turn bear similarity to the 8-kDa proteolipid subunit of the F1F0-ATP synthase. Expression of cDNA coding for the Nephrops 16-kDa protein in Saccharomyces cerevisiae, in which the endogenous gene coding for the V-ATPase proteolipid has been inactivated, restores V-ATPase activity and cell growth.
Study on mechanism underlying effect of scanning paths on healing performances in dual-beam laser welding

Abstract:A dual-beam laser system was developed for closing skin wounds and investigated the healing performance of these wounds after welding using five different laser scanning paths with increasing sparsity. The five paths included Peano, Circle, Jagged, Airwave, and Surgical suture needle point paths. These paths exhibited symmetrical structures along the incision, with decreasing sparsity in the aforementioned order. Total Energy density is 102.96 J/cm 2 with the interspace between beams is 0.1865 mm. Soldering time is 300 s and effective fusion area is 0.2625 mm 2 . Macroscopic morphology and biomechanical strength of skin samples under different laser scanning paths were evaluated using metallographic microscopy and high-precision electronic push–pull force gauge measurements. HE staining was employed to characterize the influence of laser scanning paths on microscopic tissue structure of samples, while Masson staining represented the degree of collagen damage under different scanning paths. Fluorescence spectroscopy was used to analyze energy transfer between tyrosine and phenylalanine, Fourier transform near-infrared spectroscopy demonstrated collagen molecular structural changes, and Raman spectroscopy further analyzed peptide chain folding mechanisms. Finally, Sirius Red staining was utilized to analyze fibrosis tendency in wound healing by examining distribution and content of type I and III collagens after welding with different laser scanning paths. The final results showed that dual-beam laser welding of skin incisions under Jagged path conditions can achieve stable biomechanical strength, fine microstructural texture, and full-thickness fusion. The functionality of tyrosine remains unaffected by laser energy damage, primarily due to the reconstruction of acylamide III band accompanied by the formation of new peptide chains. The ratio between collagen I and collagen III in welded area is close to 0.6, with a rich presence of type III collagen around the denatured region of collagen, contributing in high elasticity and reduced tendency for scar formation during later stages of healing process.

Controlled temperature tissue fusion: argon laser welding of canine intestine in vitro

Abstract:Background and objective: Thermal denaturation of proteins is recognized as a rate process governed by the local temperature-time response and is believed to be the principal mechanism for photothermal tissue welding. Since rate processes are exponential with temperature, feedback control of tissue surface temperature is hypothesized to create a quasi-constant rate of denaturation that will enhance the tissue welding process. Study design, materials and methods: Controlled temperature tissue welding of severed edges of fresh canine jejunum was performed in vitro by remote sensing of tissue surface temperature with an infrared sensor. A hardware controlled temperature feedback system opened and closed a shutter located in the beam path of an argon ion laser to provide constant temperature welding. Results: Strong tissue fusion was not possible at or below a surface temperature of 70 degrees C, but was accomplished at 80 degrees, 90 degrees, and 95 degrees, and 100 degrees C. Fusion was achieved with thermal coagulation of the collagenous submucosa and mucosal tissues. The bursting strength of welds created at 90 degrees C and 95 degrees C were significantly stronger than those performed at 80 degrees C. Conclusion: Laser-assisted intestinal anastomoses created in vitro are optimally strong at 90-95 degrees C feedback control temperatures.
Amendments of weld formation in human skin laser soldering

Abstract:A computational modeling is employed for quantitative assessment of weld formation and area of tissue temperature necrosis during the human skin laser soldering. The evaluation is carried out depending on the components composition of using solders, including bovine serum albumin (BSA), indocyanine green (ICG), and carbon nanotubes (CNTs), as well as the angle of incidence of laser light and its pulse duration. The influence of CNT on the change of thermodynamic characteristics of albumin denaturation and the rate of formation of the laser weld is investigated. The obtained results suggest to limit the duration of laser light pulse by temperature relaxation time to minimize transfer of thermal energy to reduce the heating of human skin tissues. The developed model has a great potential for further optimization of laser soldering of biological tissues technology with greater efficiency in minimizing the weld area.
Investigation of collagen reconstruction mechanism in skin wound through dual-beam laser welding: Insights from multi-spectroscopy, molecular dynamics simulation, and finite element multiphysics simulation

Abstract:Since the mechanism underlying real-time acquisition of mechanical strength during laser-induced skin wound fusion remains unclear, and collagen is the primary constituent of skin tissue, this study investigates the structural and mechanical alterations in collagen at temperatures ranging from 40 °C to 60 °C using various spectroscopic techniques and molecular dynamics calculations. The COMSOL Multiphysics coupling is employed to simulate the three-dimensional temperature field, stress-strain relationship, and light intensity distribution in the laser thermal affected zone of skin wounds during dual-beam laser welding process. Raman spectroscopy, synchronous fluorescence spectroscopy and circular dichroism measurement results confirm that laser energy activates biological activity in residues, leading to a transformation in the originally fractured structure of collagen protein for enhanced mechanical strength. Molecular dynamics simulations reveal that stable hydrogen bonds form at amino acid residues within the central region of collagen protein when the overall temperature peak around the wound reaches 60 °C, thereby providing stability to previously fractured skin incisions and imparting instantaneous strength. However, under a 55 °C system, Type I collagen ensures macrostructural stability while activating biological properties at amino acid bases to promote wound healing function; this finding aligns with experimental analysis results. The COMSOL simulation outcomes also correspond well with macroscopic morphology after laser welding samples, confirming that by maintaining temperatures between 55 °C–60 °C during laser welding of skin incisions not only can certain instantaneous mechanical strength be achieved but irreversible thermal damage can also be effectively controlled. It is anticipated that these findings will provide valuable insights into understanding the healing mechanism for laser-welded skin wounds.

Laser tissue welding: a comprehensive review of current and future clinical applications

Abstract:Laser techniques for joining tissue, in combination with other surgical technologies, will be a hallmark of surgery in the next century. At present, there are many clinical applications of tissue welding and soldering which are beginning to achieve wide spread acceptance. These exciting clinical developments are the result of many advances which have been made in the past few years in our understanding of the mechanism of laser tissue welding. Also contributing to this progress are many important technical refinements such as tissue solders and feedback control of the laser device. In this article, we describe in depth the history and development of laser tissue welding including key theoretical concepts as well as crucial experiments which have added to our insight into this phenomenon. We also review the evolving concepts of its clinical application and indicate clinical applications which are likely to become more important in the future.

Visual tracking problems are often cast as a sequential inference problem within the DS framework and a compact way to model the observation distributions (i.e., object appearances) is through probabilistic principal component analysis (PPCA). PPCA is a classic Gaussian based subspace representation method and a popular tool for appearance modeling. Although Gaussian density has theoretically nice properties, resulting in models that are always tractable, they are also severely limited in practical settings. One of the central issues in the use of PPCA for target appearance modeling is that it is very sensitive to outliers. The Gaussian density has a very light tail, while real world data with outliers exhibit heavy tails. Recently, more heavy-tailed distributions (e.g., Student's t-distribution) have been introduced to increase the robustness of the original PPCA. We propose to augment the traditional target tracking DS by adding a set of auxiliary latent variables to adjust the shape of the observation distribution. We show that by carefully choosing the probability density of these auxiliary latent variables, a more robust observation distribution can be obtained with tails heavier than Gaussian. Numerical experiments verify that the proposed SDS has a better capability to handle considerable amount of outlier noise and an improved tracking performance over DS with a Gaussian based observation model.
Effect of Process Parameters on the Performance of Laser Repair of Esophageal Tissue Incisions

Abstract:To study the influence of laser parameters on the mechanical properties and thermal damage of esophageal tissue, the experiment of laser repair of esophageal tissue incision is made. The effects of parameters including the laser power, scanning speed, defocusing amount, scanning path on the bursting pressure strength of esophageal tissue are analyzed. The influence of different energy densities on the appearance of esophageal tissue morphology, microstructure, and the degree of thermal denaturation of collagen is analyzed. The results showed that the laser power and the interaction factor of laser power x scanning path is the most important factor affecting the bursting pressure strength of esophageal tissues. The optimal parameters are laser power 6 W, scanning speed 150 mm/s, defocusing amount 0 mm, and scanning path 3, the highest bursting pressure strength of 93.3 mmHg is achieved. Meanwhile the esophageal tissues do not show obvious scarring, and the degree of thermal denaturation of tissue collagen is the smallest, which is only 1.6 x 10-5, the texture of the tissue microscopic images is uniformly changed, and the texture clarity is high.

Abstract:In clinical surgery, high frequency electric welding is routinely utilized to seal and fuse soft tissues. This procedure denatures collagen by electrothermal coupling, resulting in the formation of new molecular crosslinks. It is critical to understand the temperature distribution and collagen structure changes during welding in order to prevent thermal damage caused by heat generated during welding. In this study, a method combining optical measurement and simulation was presented to evaluate the temperature distribution of vascular tissue during welding, with a fitting degree larger than 97% between simulation findings and measured data. Integrating temperature distribution data, strength test data, and Raman spectrum data, it is discovered that optimal parameters exist in the welding process that may effectively prevent thermal damage while assuring welding strength.
Impact of SWCNTs and CMCS on efficacy of anastomosis and optical properties in vivo during laser biological tissue soldering

Abstract:In order to improve the mechanical properties of skin tissue incision, enhance laser absorption efficiency, further shorten healing time and reduce skin fibrosis tendency, this study investigated the effects of CMCS or SWCNT combined with 25 % BSA and 0.4 % ICG on laser tissue welding. Nd:YAG pulsed laser was used for wound closure test on live rats with an energy density of 67.3 J/cm 2 and linear scanning of skin incisions. The effects of different types and concentrations of biological dressings on wound closure were tested at different sites. The results showed that biocompatible dressings containing CMCS and SWCNTs had a significant effect in promoting wound healing after laser welding. After laser welding, the tensile strength of the control group without CMCS or SWCNTs was only between 6.5 and 9.8 N/cm 2 , while that with CMCS reached as high as 26.5 N/cm 2 . Macroscopic morphology analysis showed that components such as CMCS and SWCNTs could significantly shorten healing time from 14 days to 9–10 days. Optical performance tests demonstrated that a combination of 5 %CMCS and 4 %SWCNTs can significantly increase skin's absorption coefficient for laser photons up to7-8 Lg -1 cm −1 and reduce photon diffuse reflectance. HE and Masson staining revealed that when internalized by dressing containing 0.4 %SWCNTs, nanocarbon particles promoted collagen skeleton regeneration but increased fibrosis tendency after 14 days post-laser welding, resulting in unfavorable long-term healing outcomes. However, the combination of CMSC with SWCNTs can significantly improve incision mechanical properties, laser photon absorption rate, and shorten healing time while reducing scar proliferation tendencies.
