Abstract:Objective:To explore the sensitivity and specificity of fetal intelligent navigation echocardiography(FINE, 5D Heart) in the prenatal detection of congenital heart defect(CHD), and to compare its diagnostic performance with spatiotemporal image correlation(STIC) loop (STICloop).Methods:A total of 250 pregnant women having a singleton pregnancy in the second and third trimesters from April 2018 to May 2019 in Sir Run Run Shaw Hospital, Zhejiang University College of Medicine were enrolled in this prospective study. 2D ultrasound diagnosis has been made before acquiring STIC volume data set. After the appropriateness of each volume data set was preliminarily determined, all identifying informations were concealed and the appropriate volumes were randomly distributed to less experienced sonographer for analysis using 5D Heart software. The sensitivity and specificity, positive and negative likelihood ratios, as well as area under the receiver operating characteristic curve (AUC) of the two methods were determined respectively, and the differences between the two methods were statistically analyzed.Results:The diagnostic performance of 5D Heart for the prenatal detection of CHD was: sensitivity of 95.45% (105/110), specificity of 94.29% (132/140), positive likelihood ratio of 16.72, negative likelihood ratio of 0.05, and AUC of 0.95, respectively. Among cases with confirmed CHD, the 5D Heart diagnosis completely matched the final diagnosis in 77.27% (85/110). The diagnostic performance of STICloop for the prenatal detection of CHD was: sensitivity of 90.91% (100/110), specificity of 92.86% (130/140), positive likelihood ratio of 12.73, negative likelihood ratio of 0.10, and AUC of 0.92. The sensitivity and secificity of 5D Heart in diagnosing CHD were both higher than 90%, and it could successfully detect a broad spectrum of severe fetal CHD. Among cases with confirmed CHD, even the STICloop diagnosis completely matched the final diagnosis in 61.81%(68/110), its diagnostic accuracy was relatively lower than 5D Heart ( P=0.013). However, there were no statistical differences between the two methods in sensitivity and specificity(all P>0.05). Conclusions:The sesitivity and specifity of 5D Heart in diagnosis CHD were both higher than 90%, and it could successfully detect a broad spectrum of severe fetal CHD. STICloop could be used for preliminary screening of CHD, as it has the same high sensitivity and specificity as 5D Heart. Further examination may be recommended when fetal heart abnormalities are suspected.

Abstract:Background: The ultrasound scan represents the first tool that obstetricians use in fetal evaluation, but sometimes, it can be limited by mobility or fetal position, excessive thickness of the maternal abdominal wall, or the presence of post-surgical scars on the maternal abdominal wall. Artificial intelligence (AI) has already been effectively used to measure biometric parameters, automatically recognize standard planes of fetal ultrasound evaluation, and for disease diagnosis, which helps conventional imaging methods. The usage of information, ultrasound scan images, and a machine learning program create an algorithm capable of assisting healthcare providers by reducing the workload, reducing the duration of the examination, and increasing the correct diagnosis capability. The recent remarkable expansion in the use of electronic medical records and diagnostic imaging coincides with the enormous success of machine learning algorithms in image identification tasks. Objectives: We aim to review the most relevant studies based on deep learning in ultrasound anomaly scan evaluation of the most complex fetal systems (heart and brain), which enclose the most frequent anomalies.

Abstract:In the past few decades, great progress has been made in prenatal diagnosis of congenital heart disease (CHD). Fetal echocardiography is recognized as the main prenatal screening and diagnostic tool that can accurately detect approximately 85 % of fetal cardiac abnormalities. Evaluation of the fetal heart remains a major challenge in prenatal ultrasound screening and diagnosis due to fetal position, involuntary movement, small and complex fetal cardiac anatomy, maternal abdominal wall conditions, and lack of expertise in fetal echocardiography by some physicians engaged in obstetric ultrasound. Artificial intelligence (AI) can automate and standardize the display of each diagnostic section of the fetal heart and thus contribute to accurate diagnosis, which significantly optimizes the clinical application of fetal echocardiography. In this review, we not only clarify the role of AI but also highlight its significance and future solutions in the field of fetal echocardiography.

Abstract:Congenital heart disease (CHD) is a severe health risk for newborns. Early detection of abnormalities in fetal cardiac structure and function during pregnancy can help patients seek timely diagnostic and therapeutic advice, and early intervention planning can significantly improve fetal survival rates. Echocardiography is one of the most accessible and widely used diagnostic tools in the diagnosis of fetal CHD. However, traditional fetal echocardiography has limitations due to fetal, maternal, and ultrasound equipment factors, and is highly dependent on the skill level of the operator. AI technology, with its rapid development utilizing advanced computer algorithms, has great potential to empower sonographers in time-saving and accurate diagnosis and to bridge the skill gap in different regions. In recent years, AI-assisted fetal echocardiography has been successfully applied to a wide range of ultrasound diagnoses. This review systematically reviews the applications of AI in the field of fetal echocardiography over the years in terms of image processing, biometrics, and disease diagnosis, and provides an outlook for future research. Abstract:In this Technical note, the authors describe a how do‐we‐do it 3D ultrasound technique in the examination of the fetal face, either in normal as in pathological cases, using a conventional 3D surface application compared with the use of artificial intelligence (AI) platform. Legal considerations are also discussed. The use of artificial intelligence (AI) platforms is revolutionizing the performance in managing metadata and big data. Medicine is another field where AI is spreading. However, this technological advancement is not amenable to errors or fraudulent misconducts. International organization and recently the European Union have released principles and recommendations for an appropriate use of AI in healthcare. In prenatal ultrasound diagnosis, the use of AI in daily practice is having a revolutionary impact. Notwithstanding, the diagnostic enhancement should be regulated, and AI applications should be developed to guarantee correct imaging acquisition and further postprocessing.

Given the unique position of ultrasound (US) in prenatal screening, the research on AI in prenatal US has practical significance with its application to prenatal US diagnosis improving work efficiency, providing quantitative assessments, standardizing measurements, improving diagnostic accuracy, and automating image quality control. This review provides an overview of recent studies that have applied AI technology to prenatal US diagnosis and explains the challenges encountered in these applications.

Abstract:Objective This proof-of-concept study assessed how confidently an artificial intelligence (AI) model can determine the sex of a fetus from an ultrasound image. Study Design Analysis was performed using 19,212 ultrasound image slices from a high-volume fetal sex determination practice. This dataset was split into a training set (11,769) and test set (7,443). A computer vision model was trained using a transfer learning approach with EfficientNetB4 architecture as base. The performance of the computer vision model was evaluated on the hold out test set. Accuracy, Cohen's Kappa and Multiclass Receiver Operating Characteristic area under the curve (AUC) were used to evaluate the performance of the model. Results The AI model achieved an Accuracy of 88.27% on the holdout test set and a Cohen's Kappa score 0.843. The ROC AUC score for Male was calculated to be 0.896, for Female a score of 0.897, for Unable to Assess a score of 0.916, and for Text Added a score of 0.981 was achieved. Conclusion This novel AI model proved to have a high rate of fetal sex capture that could be of significant use in areas where ultrasound expertise is not readily available. Key Points This is the first proof-of-concept AI model to determine fetal sex. This study adds to the growing research in ultrasound AI. Our findings demonstrate AI integration into obstetric care. Abstract:Prenatal screening with ultrasound can lower neonatal mortality significantly for selected cardiac abnormalities. However, the need for human expertise, coupled with the high volume of screening cases, limits the practically achievable detection rates. In this paper we discuss the potential for deep learning techniques to aid in the detection of congenital heart disease (CHD) in fetal ultrasound. We propose a pipeline for automated data curation and classification. During both training and inference, we exploit an auxiliary view classification task to bias features toward relevant cardiac structures. This bias helps to improve in F1-scores from 0.72 and 0.77 to 0.87 and 0.85 for healthy and CHD classes respectively.

Abstract:Purpose: Since it is fast, inexpensive and increasingly portable, ultrasound can be used for early detection of Developmental Dysplasia of the Hip (DDH) in infants at point-of-care. However, accurate interpretation\is highly dependent on scan quality. Poor-quality images lead to misdiagnosis, but inexperienced users may not even recognize the deficiencies in the images. Currently, users assess scan quality subjectively, based on image landmarks which are prone to human errors. Instead, we propose using Artificial Intelligence (AI) to automatically assess scan quality. Methods: We trained separate Convolutional Neural Network (CNN) models to detect presence of each of four commonly used ultrasound landmarks in each hip image: straight horizontal iliac wing, labrum, os ischium and midportion of the femoral head. We used 100 3D ultrasound (3DUS) images for training and validated the technique on a set of 107 3DUS images also scored for landmarks by three non-expert readers and one expert radiologist. Results: We got AI ≥ 85% accuracy for all four landmarks (ilium = 0.89, labrum = 0.94, os ischium = 0.85, femoral head = 0.98) as a binary classifier between adequate and inadequate scan quality. Our technique also showed excellent agreement with manual assessment in terms of Intraclass Correlation Coefficient (ICC) and Cohen's kappa coefficient (K) for ilium (ICC = 0.81, K = 0.56), os ischium (ICC = 0.89, K = 0.63) and femoral head (ICC = 0.83, K = 0.66), and moderate to good agreement for labrum (ICC = 0.65, K = 0.33). Conclusion: This new technique could ensure high scan quality and facilitate more widespread use of ultrasound in population screening of DDH.

Abstract:Introduction: Ultrasonography in the first trimester of pregnancy offers an early screening tool to identify high risk pregnancies. Artificial intelligence (AI) algorithms have the potential to improve the accuracy of diagnosis and assist the clinician in early risk stratification. Objective: The objective of the study was to conduct a systematic review of the use of AI in imaging in the first trimester of pregnancy. Methods: We conducted a systematic literature review by searching in computerized databases PubMed, Embase, and Google Scholar from inception to January 2024. Full-text peer-reviewed journal publications written in English on the evaluation of AI in first-trimester pregnancy imaging were included. Review papers, conference abstracts, posters, animal studies, non-English and non-peer-reviewed articles were excluded. Risk of bias was assessed by using PROBAST. Results: Of the 1,595 non-duplicated records screened, 27 studies were included. Twelve studies focussed on segmentation, 8 on plane detection, 6 on image classification, and one on both segmentation and classification. Five studies included fetuses with a gestational age of less than 10 weeks. The size of the datasets was relatively small as 16 studies included less than 1,000 cases. The models were evaluated by different metrics. Duration to run the algorithm was reported in 12 publications and ranged between less than one second and 14 min. Only one study was externally validated. Conclusion: Even though the included algorithms reported a good performance in a research setting on testing datasets, further research and collaboration between AI experts and clinicians is needed before implementation in clinical practice.

Abstract:Fetal magnetic resonance imaging (MRI) has become an appealing technology adjunct to ultrasound in the detection of fetal additional abnormalities, especially in the central nervous system (CNS) and cardiovascular system. During the last decade, artificial intelligence (AI) has made remarkable progress, and it is widely available for clinical utility. Compared with the large number of research articles regarding the use of AI tools for ultrasound, the studies of fetal MRI using AI tools are very limited. In this review, we summarized the advances in the application of AI in fetal brain and cardiac MRI. We conclude that AI can be used as a promising tool in fetal brain and cardiac MRI, as an approach to assist clinicians and parents in decision-making.

Abstract:Objectives Although there have been remarkable strides in fetal medicine and prenatal diagnosis of congenital heart disease, a significant percentage of newborns with isolated coarctation of the aorta (CoA) ‐ around 60 percent ‐ are still not identified prior to birth. The prenatal detection of CoA has been shown to have a notable impact on the survival rates of affected infants. To this end, the implementation of artificial intelligence (AI) in fetal ultrasound may represent a groundbreaking advancement. Our hypothesis is that leveraging automated cardiac biometric measurements with AI during the 18‐22‐week anomaly scan will enhance the identification of fetuses that are at risk of developing CoA. Methods We have developed an AI model capable of identifying standard cardiac planes and conducting automated cardiac biometric measurements. Our data consisted of pregnancy ultrasound image and outcome data spanning from 2008 to 2018 and collected from four distinct regions in Denmark. The CoA cases from the period were paired with healthy controls in a ratio of 1:100 and matched on gestational ages of ±2 days. The cardiac biometrics on the four‐chamber view and three vessel view were included in a logistic regression‐based prediction model. To assess the predictive capabilities, we visualized sensitivity and specificity on Receiver Operating Characteristic (ROC) curves. Results At the 18‐22 week scan, the right ventricle (RV)area and length, left ventricle (LV) width, and the ratios of RV/LV areas and main pulmonary artery/ascending aorta diameters showed significant differences with z‐scores above 0.7 when comparing subjects with a postnatal diagnosis of CoA (n=73) and healthy controls (n=7300). Using logistic regression and backward feature selection, our prediction model produced a ROC curve with an AUC (Area Under the Curve) of 0.96 and a specificity of 88.9% at a sensitivity level of 90.4%. Conclusion The integration of AI technology with automated cardiac biometric measurements conducted during the 18‐22‐week anomaly scan in fetal medicine has the potential to substantially enhance the screening for fetal CoA and subsequently the rate of CoA detection. Future research should clarify how AI technology can be used to aid in screening and detection of congenital heart anomalies to improve neonatal outcomes. This article is protected by copyright.
