Abstract:Challenges to epidemiological studies of eating and related body image disturbance disorders in males include, in addition to low base rates and the predominance of residual diagnostic categories, the female-centric nature of current classification schemes and the consequent lack of appropriate assessment instruments. In this narrative review, we summarise epidemiological data regarding the prevalence and correlates of eating disorders, related body image disturbance disorders, and eating disorder features in males. Attention is focused on disorders most likely to be observed among males, such as muscle dysmorphia and muscularity-oriented excessive exercise. It is argued that, given the multiple challenges involved in research of this kind, a focus on features is more likely to advance the field than a focus on diagnoses. In terms of correlates, we focus on impairment and help-seeking, since these issues are most relevant in informing public health burden, service provision, and related issues. We end with some thoughts about current gaps in the knowledge base and directions for future research that we consider to be most promising.

Abstract:BACKGROUND: Eating disorders are seen mainly as a problem affecting women, not just by the public at large, but also in specialized circles. Although it is true that more women than men suffer from all types of eating disorder, pertinent reviews have clearly shown that they do indeed occur in men, and that the available evidence on the matter is limited. The stigmatization of men with eating disorders makes it harder for these men, and for the relevant professionals, to recognize the symptoms and to seek or provide help.METHODS: This review is based on publications retrieved by a selective search in PubMed on the epidemiological, diagnostic, clinical, and therapeutic aspects of eating disorders in men.RESULTS: Current estimated lifetime prevalences in men are 0.2% for anorexia nervosa, 0.6% for bulimia nervosa, and 1% for bingeeating disorder; the corresponding figures for women are 1.4%, 1.9%, and 2.8%. Men and women may display different manifestations. Women are thought to be mainly seeking a slim figure and weight reduction; men, a muscular build. The established Germanlanguage screening and diagnostic tools, however, do not cover the types of symptoms that are more common in men. Little is known about whether treatment yields comparable results in men and women.CONCLUSION: It is important to combat the stigmati - zation of men with eating disorders and to remove the obstacles to their appropriate diagnosis and treatment. The current methods of screening and diagnosis need to be adapted to take account of the special aspects of abnormal eating behavior in men. It remains unclear whether and how the disorderspecific treatment of these conditions in men should differ from their treatment in women.

Abstract:Purpose of ReviewThere is growing recognition that eating disorder (ED) symptoms, particularly those of a muscularity-oriented nature, are more common in men than previously understood. The purpose of the current review is to describe contemporary directions and implications of research on traditional and muscularity-oriented ED symptoms among males.Recent FindingsEvidence indicates that ED symptoms occur in a substantial minority of men. Importantly, recent research has focused on muscularity-oriented body image and disordered eating in males, demonstrating the prevalence, correlates, and consequences of maladaptive muscularity-oriented attitudes and behaviors. A growing number of assessments are available to measure these constructs in males, and preliminary treatment considerations have begun to be addressed in the literature.SummaryResearch on male EDs and body image is increasingly focusing on muscularity-oriented manifestations. Continued empirical work will be critical to improve our understanding of the onset, maintenance, and treatment of muscularity-oriented disordered eating in males.

Abstract:Most studies on the epidemiology of eating disorders have focused on young females in Western countries, being the high-risk group for the development of eating disorders. However, eating disorders do occur in older women and in men and they also do occur in persons in nonwestern countries, as the reviews in this issue show [Raevuori et al.(pp. 426–430), Mangweth-Matzek et al.(pp. 431–435), Pike et al.(pp. 436–442)]. Abstract:In the eating disorder section in this issue, Keski-Rahkonen and Mustelin state that a basic requirement for advances in the detection and treatment of eating disorders is a better understanding of their epidemiology [1]. They compare epidemiologists with news reporters, who also attempt to answer five basic questions: who, what, when, where, and why?[1]. In previous years, the review articles in the eating disorder section in this journal showed that eating disorders most frequently occur in the highrisk group of young Western females, but do occur in older women, in men and in persons in non-Western countries [2, 3]. Abstract:Objective: Despite the recent surge of eating disorders among women in large Asian cities, male eating disorder cases remain rare. The current article described 5 male eating disorder cases that presented within a period of 2 years in Wuhan, a city in central China.Methods: The authors described 4 cases of anorexia nervosa (2 restrictive, 2 bulimic) and 1 case of normal weight bulimia nervosa.Results: Fear of fat was reported for all 5 cases, and none of the cases reported homosexuality.Discussion: Sociocultural changes and westernization most probably accounted for the increasing incidence of eating disorders among male and female youngsters in China today. Abstract:This study aimed to explore experiences of men currently using eating disorder services. Eight men from two eating disorder services were interviewed about their experiences of seeking and receiving treatment. Two superordinate themes emerged from Interpretative Phenomenological Analysis: (1) difficulty seeing self as having an eating disorder; and (2) experiences of treatment: how important is gender? The underlying themes varied in their specificity to men, with some echoing findings from the female eating disorder literature. Difficulty admitting the eating disorder may link with eating disorder psychopathology as well as gender-specific issues. Implications for clinical practice and future research are discussed.

Supportive Interventions in Managing Eating Disorders: A Review of the Evolving Medical Professional Landscape

Abstract:Background: The landscape of healthcare for medical professionals is undergoing significant changes during a global rise in obesity and mental health issues, particularly in the context of eating disorders. The COVID-19 pandemic, coupled with sedentary lifestyles and job loss, has exacerbated food obsession and mental health challenges, highlighting the need for effective treatments. Objective: This review aims to explore the impact of supportive interventions in managing eating disorders within the evolving landscape of medical professionals' approaches, focusing on the adoption of new tools and approaches. Methods: A comprehensive analysis of current literature and data sources was conducted to examine the healthcare landscape's transformation and its implications for treating eating disorders. Various treatment modalities, including therapy, education, medication, and support groups, were evaluated in light of recent changes in medical practice. Results: The review identified a growing body of research highlighting the effectiveness of supportive interventions, such as cognitive behavior therapy, mindfulness, and participation in peer support groups in improving emotional eating patterns and facilitating long-term weight management. The findings underscore the growing prevalence of obesity and mental health issues, with a significant portion of individuals experiencing food obsession and overeating. Despite this, recognition and treatment of obesity-related psychological issues remain inadequate, partly due to a shortage of mental health professionals. Treatment options for eating disorders may include bariatric surgery, therapy, education, medication, and supportive interventions. Support groups such as Overeaters Anonymous (OA) have shown promise in helping individuals manage eating disorders and achieve healthier lifestyles. Conclusions: The shifting healthcare landscape necessitates a proactive approach from medical professionals to address the complex relationship between obesity, mental health, and eating disorders. Integrating peer support groups and holistic treatment approaches alongside traditional medical interventions can enhance outcomes and promote long-term weight management.
Epidemiology of anorexia nervosa in men: a nationwide study of Finnish twins

Abstract:Background To examine the epidemiology of anorexia nervosa in men, we screened Finnish male twins born in 1975–79. Methods and Findings Men (N = 2122) from FinnTwin16 birth cohorts were screened for lifetime eating disorders by a questionnaire. The screen positives (N = 18), their male co-twins (N = 10) and those with lifetime minimum BMI≤17.5 (N = 21) were administered the Structured Clinical Interview for DSM-IV anorexia nervosa. The incidence rate of anorexia nervosa for the presumed peak age of risk (10–24y) was 15.7 per 100 000 person-years; its lifetime prevalence was 0.24%. All probands had recovered from eating disorders, but suffered from substantial psychiatric comorbidity, which also manifested in their co-twins. Additionally, male co-twins displayed significant dissatisfaction with body musculature, a male-specific feature of body dysmorphic disorder. Conclusions Anorexia nervosa in males in the community is more common, transient and accompanied by more substantial comorbidity than previously thought.

Abstract:Objective To review the literature on the incidence and prevalence of eating disorders. Methods We searched Medline using several key terms relating to epidemiology and eating disorders and we checked the reference lists of the articles that we found. Special attention has been paid to methodologic problems affecting the selection of populations under study and the identification of cases. Results An average prevalence rate for anorexia nervosa of 0.3% was found for young females. The prevalence rates for bulimia nervosa were 1% and 0.1% for young women and young men, respectively. The estimated prevalence of binge eating disorder is at least 1%. The incidence of anorexia nervosa is 8 cases per 100,000 population per year and the incidence of bulimia nervosa is 12 cases per 100,000 population per year. Abstract:Although eating disorders were long considered a typical female disorder, it is now clear that men are also affected. However, the literature on eating disorders in men is still very limited, and the actual extent is not known. Even less is known about the epidemiology of eating disorders in older individuals. In this focused review, we will present an update of the available data on disordered eating and eating disorders in middle-aged and older males. In addition, we will highlight the relationship of eating disorders with excessive sports as a purging method of choice for this age group and discuss the impact of age-related hormonal imbalances in aging men.

Abstract:This paper reviews the incidence and prevalence studies of anorexia nervosa and bulimia nervosa. The incidence of anorexia nervosa has been estimated at 8.1 and of bulimia nervosa at 11.4 per 100,000 population per year. The registered incidence rates of anorexia nervosa have increased sharply, but it is still unclear whether the rates in the community have increased. It has been established, however, that the incidence rates of anorexia nervosa in females 15 to 24-years-old—the most vulnerable group—have increased during the past 50 years. The (point-)prevalence among young females is 0.28% for anorexia nervosa and 1% for bulimia nervosa. A model of the pathway to care is described to show the one-year prevalence rates at different levels of care. The role of social and cultural factors will be discussed.

Medical complications because of age, the stigma of eating disorders in a still ‘untypical’age, and the glorification of sports activity often hinder the recognition of eating disorders in midlife and older persons. Treatment approaches should consider treatment strategies tailored for older women and men, addressing the context of midlife and aging.

Abstract:Depression comorbid with eating disorders is common and can worsen the severity of both disorders. Little is published regarding depression and eating disorders in male adolescents. This retrospective observational study compared eating disorder presentation and depression comorbidity between medically-hospitalized male and female adolescents. Standardized chart abstraction was performed for 148 subjects (n=127 females, =21 males). Male adolescents had significantly greater pre-hospitalization weight loss and longer eating disorder duration, and were 1.6 times more likely to have comorbid depression compared to female patients. These findings suggest increased detection and treatment of both disorders in adolescent males is warranted.

Abstract:This study is the continuation of a broader eating disorder study with a focus on males, which examines the influence of body image, sociocultural attitudes, appearance anxiety, and depression on disordered eating behaviors among college students. Based on EAT-26 scores, 10.5% of males were at risk of an eating disorder. Body image dissatisfaction was almost as common among males (65.2%) as females (68.6%) and BMI was not a significant predictor of dissatisfaction. Among participants who reported dissatisfaction with their bodies, eating disorder risk was higher among females than males. While there were differences between men and women in general, at-risk men and women showed quite similar self-report ratings on most measures of the SATAQ scale. The main gender difference was that Social Appearance Anxiety and Depression were significant predictors of eating disorder risk among females, but not males. This study indicates that risk of an eating disorder is more prevalent among males than previously reported. The risk is significant for college students as a whole and it is important to have preventative measures before disordered eating behaviors turn into an eating disorder.

Abstract:Abstract Background Eating disorders are serious conditions that cause major suffering for patients and their families. Better knowledge about perceptions of eating disorders and their treatment, and which factors that facilitate or hinder recovery, is desired in order to develop the clinical work. We aimed to explore and synthesise experiences of eating disorders from the perspectives of those suffering from an eating disorder, their family members and health care professionals through an overarching meta-review of systematic reviews in the field. Methods A systematic literature search was conducted in the databases PubMed, PsycInfo, Scopus, and CINAHL. Inclusion criteria were systematic reviews of qualitative research on experiences, perceptions, needs, or desires related to eating disorders from the perspective of patients, family members or health care professionals. Systematic reviews that fulfilled the inclusion criteria were assessed for relevance and methodological limitations by at least two researchers independently. The key findings were analysed and synthesised into themes. Results We identified 17 systematic reviews that met our inclusion criteria. Of these, 13 reviews reported on the patients’ perspective, five on the family members’ perspective, and three on the health care professionals’ perspective. The study population in the reviews was predominantly girls and young women with anorexia nervosa, whilst systematic reviews focusing on other eating disorders were scarce. The findings regarding each of the three perspectives resulted in themes that could be synthesised into three overarching themes: 1) being in control or being controlled, 2) balancing physical recovery and psychological needs, and 3) trusting relationships. Conclusions There were several similarities between the views of patients, family members and health care professionals, especially regarding the significance of building trustful therapeutic alliances that also included family members. However, the informants sometimes differed in their views, particularly on the use of the biomedical model, which was seen as helpful by health care professionals, while patients and family members felt that it failed to address their psychological distress. Acknowledging these differences is important for the understanding of anorexia nervosa and other eating disorders, and may help clinicians to broaden treatment approaches to meet the expectations of patients and family members.

Eating disorders in the male athlete

Abstract:Eating disorders do occur in male athletes. They are less prominent than in female athletes, and therefore in danger of being missed. The high-risk sports fall into the same categories as with females: aesthetic sports, sports in which low body fat is advantageous, such as cross-country and marathon running, and sports in which there is a need to "make weight", including wrestling and horse racing. Athletic involvement may foster the development of an eating disorder. Some male athletes, in their preoccupation with body image, will abuse anabolic steroids. While sports participation may contribute to the aetiology of an eating disorder, the converse is also true. Exercise may be used as therapy for some cases of eating disorder. In order to adequately treat eating disorders in the male athlete, it is first essential to identify cases. Psychoeducation of athletes, their families, coaches and trainers is an important first step. Counselling an athlete to pursue a sport appropriate to his body type, or to leave his sport behind altogether (an unpopular recommendation from a coach's perspective) can be important to treatment. Treatment of co-morbid psychiatric conditions is essential. Treatment can be structured using a biopsychosocial approach, and all appropriate modalities of therapy, including individual, family and group, as well as psychopharmacotherapy, where appropriate, should be applied.

Abstract:<p>This article presents current diagnostic conceptualisations of eating disorders, including new disorders such as binge eating disorder (BED) and avoidant/restrictive food intake disorder (ARFID). This is followed by contemporary findings in the epidemiology of eating disorders, their broad sociodemographic distribution and the increases in community prevalence. Advances and the current status of evidence‐based treatment and outcomes for the main eating disorders, anorexia nervosa, bulimia nervosa and BED are discussed with focus on first‐line psychological therapies. Deficits in knowledge and directions for further research are highlighted, particularly with regard to treatments for BED and ARFID, how to improve treatment engagement and the management of osteopenia.</p>
