Abstract:Humans are highly dependent on the ability to process audio in order to interact through conversation and navigate from sound. For this, the shape of the ear acts as a mechanical audio filter. The anatomy of the outer human ear canal to approximately 15-20 mm beyond the Tragus is well described because of its importance for customized hearing aid production. This is however not the case for the part of the ear canal that is embedded in the skull, until the typanic membrane. Due to the sensitivity of the outer ear, this part, referred to as the bony part, has only been described in a few population studies and only ex-vivo. We present a study of the entire ear canal including the bony part and the tympanic membrane. We form an average ear canal from a number of MRI scans using standard image registration methods. We show that the obtained representation is realistic in the sense that it has acoustical properties almost identical to a real ear.

Abstract:Ear acoustic authentication is a new biometrics method and it utilizes the differences in acoustic characteristics of the ear canal between users. However, there have been few reports on the factors that cause differences in the acoustic characteristics. We investigate the relationship between ear canal shapes and acoustic characteristics in terms of user-to-user similarity. We used magnetic resonance imaging (MRI) to measure ear canal geometry. As a result, the correlation coefficient between shape similarity and acoustic characteristic similarity is higher than 0.7 and the coefficient of determination is higher than 0.5. This suggests that the difference in the shape of the ear canal is one of the important factors.

Abstract:The human ear canal couples the external sound field to the eardrum and the solid parts of the middle ear. Therefore, knowledge of the acoustic impedance of the human ear is widely used in the industry to develop audio devices such as smartphones, headsets, and hearing aids. In this study acoustic impedance measurements in the human ear canal of 32 adult subjects is presented. Wideband measurement techniques developed specifically for this purpose enable impedance measurement to be obtained in the full audio band up to 20kHz. Full ear canal geometries of all subjects are also available from the first of its kind in vivo based magnetic resonance imaging study of the human outer ear. These ear canal geometries are used to obtain individual ear moulds of all subjects and to process the data. By utilizing a theoretical Webster's horn description, the measured impedance is propagated in each ear canal to a common theoretical reference plane across all subjects. At this plane the mean human impedance and standard deviation of the population is found. The results are further demographically divided by gender and age and compared to a widely used ear simulator (the IEC711 coupler).

Abstract:In many applications, knowledge of the sound pressure transfer to the eardrum is important. The transfer is highly influenced by the shape of the ear canal and its acoustic properties, such as the acoustic impedance at the eardrum. Invasive procedures to measure the sound pressure at the eardrum are usually elaborate or costly. In this work, we propose a numerical method to estimate the transfer impedance at the eardrum given only input impedance measurements at the ear canal entrance by using one-dimensional first-order finite elements and Nelder-Mead optimization algorithm. Estimations on the area function of the ear canal and the acoustic impedance at the eardrum are achieved. Results are validated through numerical simulations on ten different ear canal geometries and three different acoustic impedances at the eardrum using synthetically generated data from three-dimensional finite element simulations.

Ground-truth validation of the "earbuds method" for measuring acoustic nasalance

Abstract:Among the variety of methodological tools available for quantifying the degree of nasalization, nasometry has remained a popular choice for both academic and clinical research on speech due to its relatively low cost, portability, ease of use, and interpretability. In its simplest form, nasometry involves two microphones that independently capture the acoustic radiation from the mouth and nose. Recently, a method was proposed by Stewart and Kohlberger [(2017). Lang. Doc. Conserv. 11, 49-80] of using earbuds to achieve a similar result. Although the method has been widely used since, most notably in the context of linguistic fieldwork, it has yet to be tested against ground-truth measurements of acoustic nasalance from a commercially established device; the current study attempts to provide this ground-truth comparison. A wide variety of characteristics of the relationship between nasalance measurements created using acoustic data co-registered from a commercial nasometer, silicone earbuds, and flat earbuds are thoroughly examined. The results suggest that the "earbuds method" may provide reliable estimates of nasality when assessed at a global level (i.e., overall averages, relatively high/low nasalance scores coinciding with the presence/absence of nasalization), but that the accuracy of these estimates may decline at more fine-grained levels in different ways that are outlined in the discussion.

Abstract:The ear canal is usually described as an S-shaped funnel. In attempting to classify ear-canal shapes obtained from point clouds digitized from molds of 300 ears, the problem of designing criteria for distinguishing and organizing the canal shapes arose. In this work, we extracted features inspired by the S-shape characteristic (critical point, maximum, minimum, twist, writhe, translation, rotation) and, through them, introduced 14 types of ear-canal shapes. This classification allowed comparison of ears within a type and of ears between different types. It expanded our range of descriptors of canal shapes and unlocked perspectives for applications.

Effect of curvature on sound propagation in the ear canal

Abstract:In this paper, we investigate the effect of the curvature and torsion of the ear canal on its resonance through a comparison between several ear canal models. Utilizing Stinson's ear canal geometries as a reference, we build and analyze several ear canal models using both transmission matrix and numerical methods for the purpose of comparative assessment. A conical transmission unit, which considers visco-thermal effects, is employed for the modeling of the human ear canal. While the transfer matrix and numerical method agree well for a straight axis model, this simplification results in up to 20% deviation from a curved canal. We propose the curve twist ratio as a metric to quantify the influence of curvature on the ear canal and find that our proposed metric can effectively express the error introduced by the simplified straight axis model. Upon this metric, an empirical equation is proposed for incorporating the curvature effect in the transmission matrix method, enabling it to generate comparable results to those of the numerical method, which considers the effect of the curvature and torsion, thus dramatically accelerating computation.

Morphology and anatomical variability of the external auditory canal: A population-based MRI study

Abstract:Background: The external auditory canal (EAC) exhibits a complex morphology and strong inter-individual variations. However, these have not yet been comprehensively described in the literature. Purpose: This study aims to determine the width, height and cross-sectional area of the cartilaginous portion of the EAC and to describe the three-dimensional morphology and variability of different EACs. Methods: Magnetic resonance imaging was performed on 870 subjects (401 male, 469 female, resulting in 1740 EACs) who participated in the longitudinal, population-based cohort study 'Study of Health in Pomerania-START-3'. The height and width were measured in the cartilaginous part of the EAC, between the first and second bend. The variability of the EAC morphology was visualized in three-dimensional models. Results: The mean height (vertical length) of the EAC was 8.62 mm (SD = 2.42) on the right, 8.47 mm (SD = 2.36) on the left. The width (horizontal length) was 4.08 mm (SD = 1.6) on the right, 3.93 mm (SD = 1.64) on the left. The EAC cross-section was 28.6 mm2 (SD = 15.19) on the right, 27.15 mm2 (SD = 14.33) on the left. The average cross-sectional area of the EAC in men was higher than in women. Subjects with larger body size had larger cross-sectional areas. Subjects with higher body mass index tended to have smaller cross-sections. Although the average EAC had an oval shape, a three-dimensional comparison of different EACs revealed strong individual variation in morphology. Conclusion: This study enhances the understanding of otolaryngologists and anatomists regarding the complex morphology and variability of the cartilaginous portion of the EAC.

Abstract:The ear canal (EC) is essential for sound transfer and crucial for hearing. Some pathological conditions may modify its morphology, leading to EC sound pressure redistribution, and stapes footplate displacement (FPD) gain alteration. However, no consensus regarding pathological EC and its impact on sound transfer has yet been achieved. To address the effect of morphology of EC on sound pressure redistribution and FPD gain. Varied pathological EC finite element (FE) models were constructed and analyzed based on FE analysis. The results indicated that canal wall down mastoidectomy decreases the second resonance frequency of the EC. The canal wall down mastoidectomy, with conchaplasty increased the first resonance frequency, but decreased the second along with the interval sound pressure gain increased, following which the FPD gain was altered. Stenosis of the EC at the internal portion decreased the second resonance frequency with minimal effect to the first part. When the stenosis moved to the outer portion of the EC, the first resonance frequency decreased, and the second one increased, along with the interval sound pressure gain decreased and FPD gain. Finally, the simplified EC model exerted a minimal effect on sound transfer. The minimal change in EC, such as simplification, straightening, canal wall down mastoidectomy, or enlargement, moderately affects the sound transfer; however, the EC stenosis deteriorates the sound transfer remarkably.
Audiologic changes after pinna augmentation

Abstract:No studies to date have objectively assessed whether pinna morphology affects sound intensity detected within the external auditory canal (EAC). Commonly performed procedures on the EAC are carried out for acquired and congenital pathology, together with correction of ear deformities. Our aim was to use an experimental model to identify whether a relationship exists between pinna shape and its subsequent effect on the hearing subject. An anatomically accurate and life-size model made of rubber composite was used for this study. Serial sections (small wedge, defect open; small wedge, defect closed; large wedge, defect open; large wedge, defect closed [equivalent to a protruding ear]; and pinnectomy) were undertaken, and the sound intensity changes assessed at the junction between the EAC and middle ear (tympanic membrane position) using an AURICAL Plus (Otometrics; Taastrup, Denmark) sound processor. A statistically significant loss was demonstrated for wedge-excised models, which was greatest at 180° azimuth. This loss was significantly reduced when the wedge defects were closed. A statistically significant improvement was demonstrated in the protruding ("bat") ear model compared with the normal ear at 0° azimuth. In this model, gain in sound intensity is adversely affected by pinna wedge resection. Because this change may be increased in those with protruding ears, this factor is important to consider for all cosmetic and noncosmetic operations to the pinna, and it supports the notion that the pinna is not a simple funnel.
Fully Automated Measurement of Cochlear Duct Length From Clinical Temporal Bone Computed Tomography

Abstract:Objectives/hypothesis: To present and validate a novel fully automated method to measure cochlear dimensions, including cochlear duct length (CDL). Study design: Cross-sectional study. Methods: The computational method combined 1) a deep learning (DL) algorithm to segment the cochlea and otic capsule and 2) geometric analysis to measure anti-modiolar distances from the round window to the apex. The algorithm was trained using 165 manually segmented clinical computed tomography (CT). A Testing group of 159 CTs were then measured for cochlear diameter and width (A- and B-values) and CDL using the automated system and compared against manual measurements. The results were also compared with existing approaches and historical data. In addition, pre- and post-implantation scans from 27 cochlear implant recipients were studied to compare predicted versus actual array insertion depth. Results: Measurements were successfully obtained in 98.1% of scans. The mean CDL to 900° was 35.52 mm (SD, 2.06; range, [30.91-40.50]), the mean A-value was 8.88 mm (0.47; [7.67-10.49]), and mean B-value was 6.38 mm (0.42; [5.16-7.38]). The R2 fit of the automated to manual measurements was 0.87 for A-value, 0.70 for B-value, and 0.71 for CDL. For anti-modiolar arrays, the distance between the imaged and predicted array tip location was 0.57 mm (1.25; [0.13-5.28]). Conclusion: Our method provides a fully automated means of cochlear analysis from clinical CTs. The distribution of CDL, dimensions, and cochlear quadrant lengths is similar to those from historical data. This approach requires no radiographic experience and is free from user-related variation. Abstract:The first objective of the study was to compare approaches to eardrum electrode insertion as they relate to the likelihood of introducing an acoustic leak between the ear canal and eartip. A common method for placing a tympanic membrane electrode involves securing the electrode in the canal by routing it underneath a foam eartip. This method is hypothesized to result in a slit leak between the canal and foam tip due to the added bulk of the electrode wire. An alternative approach involves creating a bore in the wall of the foam tip that the electrode can be threaded through. This method is hypothesized to reduce the likelihood of a slit leak before the electrode wire is integrated into the foam tip. The second objective of the study was to investigate how sound transmission in the ear is affected by placing an electrode on the eardrum. It was hypothesized that an electrode in contact with the eardrum increases the eardrum's mass, with the potential to reduce sound transmission at high frequencies.

Three-dimensional finite element modeling of the human external ear: simulation study of the bone conduction occlusion effect

Abstract:A linear three-dimensional (3D) elasto-acoustic finite element model was used to simulate the occlusion effect following mechanical vibration at the mastoid process. The ear canal and the surrounding soft and bony tissues were reconstructed using images of a female cadaver head (Visible Human Project(®)). The geometrical model was coupled to a 3D earplug model and imported into comsol Multiphysics (COMSOL(®), Sweden). The software was used to solve for the sound pressure at the eardrum. Finite element modeling of the human external ear and of the occlusion effect has several qualities that can complement existing measuring and modeling techniques. First, geometrically complex structures such as the external ear can be reconstructed. Second, various material behavioral laws and complex loading can be accounted for. Last, 3D analyses of external ear substructures are possible allowing for the computation of a broad range of acoustic indicators. The model simulates consistent occlusion effects (e.g., insertion depth variability). Comparison with an experimental dataset, kindly provided by Stenfelt and Reinfeldt [Int. J. Audiol. 46, 595-608 (2007)], further demonstrates the model's accuracy. Power balances were used to analyze occlusion effect differences obtained for a silicone earplug and to examine the increase in sound energy when the ear canal is occluded (e.g., high-pass filter removal).

Abstract:Recently directed methods of inner ear drug delivery highlight the necessity for understanding critical anatomical dimensions. This study compares anatomical measurements of the human middle and inner ear, relevant for safe inner ear drug delivery, using three different methods. Objective(s) Recently directed methods of inner ear drug delivery underscore the necessity for understanding critical anatomical dimensions. This study examines anatomical measurements of the human middle and inner ear relevant for inner ear drug delivery studied with three different imaging modalities. Methods Post‐mortem human temporal bones were analyzed using human temporal bone histopathology (N = 24), micro computerized tomography (μCT; N = 4), and synchrotron radiation phase‐contrast imaging (SR‐PCI; N = 7). Nine measurements involving the oval and round windows were performed when relevant anatomical structures were visualized for subsequent age‐controlled analysis, and comparisons were made between imaging methods. Results Combined human temporal bone histopathology showed the mean distance to the saccule from the center of the stapes footplate (FP) was 2.07 ± 0.357 mm and the minimum distance was 1.23 mm. The mean distance from the round window membrane (RWM) to the osseous spiral lamina (OSL) was 1.75 ± 0.199 mm and the minimum distance was 1.43 mm. Instruments inserted up to 1 mm past the center of the FP are unlikely to cause saccular damage, provided there are no endolymphatic hydrops. Similarly, instruments inserted up to 1 mm through the RWM in the trajectory toward the OSL are unlikely to cause OSL damage. Conclusion The combined analyses of inner‐ear dimensions of age‐controlled groups and imaging modalities demonstrate critical dimensions of importance to consider when inserting delivery vehicles into the human cochlea. Abstract:INTRODUCTION:Studies have shown that higher response levels can be obtained when the bone conduction stimulation position is closer to the cochlea. However, the morphological characteristics of round window niche and posterior tympanum in congenital aural atresia (CAA) and stenosis (CAS) patients were different from the normal. These affected the position of the cochlea at the cranial base. It was still unknown whether the distances from the cranium of CAA and CAS patients to the cochlea were the same as those of normal patients or not.OBJECTIVE:To measure distances from various points on the lateral surface of the cranium to the cochlea and the cranium thickness on these points among a CAA group, CAS group and normal control group, which may provide valuable information for the better position of bone conduction stimulation.METHODS:CT images of CAA, CAS patients and these patients' healthy sides were analyzed. Firstly, the Frankfurt horizontal plane (Pfrkt) was established. Secondly, a model of part of the cranium was three-dimensionally reconstructed. Then, the Pfrkt plane was rotated down 20, 30 and 40° according to the superior margin of the external auditory canal. At every angle, points 25, 30, 35 and 40 mm away from the superior margin of the external auditory canal were marked out on the surface of the model and recorded as P20A, P30A, P40A, P20B, etc. The spatial distances between the cranium and ipsilateral cochlea were defined as lengths of points on the surface of the model to the cochlea apex (CA), cochlear base (CB) and modiolus midpoint (MM), respectively, recorded as P20A/CA, P20A/CB, P20A/MM, P30A/CA, etc. Results and Conclusions: In all groups, the length of P20D/CA was the shortest compared to P30D/CA and P40D/CA (p < 0.05). The P20A/CB and P20A/MM were also the shortest (p < 0.05). When the Pfrkt plane was rotated down 30 and 40°, the results were the same as at 20° (p < 0.05). However, P20D, P30D and P40D were almost on the mastoid air cells. We suggest that the bone conduction stimulation position is placed closer to the ear, while avoiding the mastoid air cells in the CAA and CAS patients.
Variations in cochlear duct shape revealed on clinical CT images with an automatic tracing method

Abstract:Cochlear size and morphology vary greatly and may influence the course of a cochlear implant electrode array during insertion and its final intra-cochlear position. Detailed insight into these variations is valuable for characterizing each cochlea and offers the opportunity to study possible correlations with surgical or speech perception outcomes. This study presents an automatic tracing method to assess individual cochlear duct shapes from clinical CT images. On pre-operative CT scans of 479 inner ears the cochlear walls were discriminated by interpolating voxel intensities along radial and perpendicular lines within multiplanar reconstructions at 1 degree intervals from the round window. In all 479 cochleas, the outer wall could be traced automatically up to 720 degrees. The inner wall and floor of the scala tympani in 192 cochleas. The shape of the cochlear walls were modelled using a logarithmic spiral function including an offset value. The vertical trajectories of the scala tympani exhibited a non-monotonous spiral slope with specific regions at risk for CI-related insertion trauma, and three slope categories could be distinguished. This presented automatic tracing method allows the detailed description of cochlear morphology and can be used for both individual and large cohort evaluation of cochlear implant patients.

Abstract:OBJECTIVES: The internal acoustic meatus is an osseous canal that connects the inner ear to the posterior cranial fossa. It is located in the petrous portion of the temporal bone. A thin cribriform osseous plate known as the fundus is situated at the lateral end of the canal. This study assesses the structural and numerical variations of the fundus formations.METHODS: Fifty-four temporal bones of unknown gender and age were examined with the surgical microscope.RESULTS: The temporal bones analyzed were 46.2% right-sided and 53.7% left-sided. Only one temporal bone had two parallel transverse crests, while three had a single anterior crest that split into two branches posteriorly. The number of foramina at the transverse crest varied, with 29.6% having none, 48.1% having a single foramen, and 22.2% having several foramina. An anterior crest structure was seen in 53.7% of the temporal bones, with 5% having a slightly constricted entry to the facial canal. In cases with a single nerve foramen, 48.1% had one, while 51.8% had more than one, including examples with three or four foramina. A crest was found between the foramina of the single nerve in 7% of patients. Furthermore, a crest between the saccular nerve foramen and the high fiber foramina was seen in 25.9% of cases, and 5% had two saccular nerve foramina.CONCLUSION: We think that revealing the anatomical, structural and numerical variations in the fundus will be useful in explaining the disease-symptom relationship.LEVEL OF EVIDENCE: Level 4.

Abstract:CONCLUSIONS:With the aggravation of the external auditory canal malformation, the size of extra-niche fossa became smaller, providing concrete data and valuable information for the better design, selecting and safer implantation of the transducer in the area of round window niche. Three-dimensional measurements and assessments before surgery might be helpful for a safer surgical approach and implantation of a vibrant soundbridge.OBJECTIVES:The aim of this study was to investigate whether differences exist in the morphology of the posterior tympanum related to the round window vibroplasty among congenital aural atresia (CAA), congenital aural stenosis (CAS), and a normal control group, and to analyze its effect on the round window implantation of vibrant soundbridge.METHODS:CT images of 10 normal subjects (20 ears), 27 CAS patients (30 ears), and 25 CAA patients (30 ears) were analyzed. The depth and the size of outside fossa of round window niche related to the round window vibroplasty (extra-niche fossa)and the distances between the center of round window niche and extra-niche fossa were calculated based on three-dimensional reconstruction using mimics software. Finally, the data were analyzed statistically.RESULTS:The size of extra-niche fossa in the atresia group was smaller than in the stenosis group (p < 0.05); furthermore, the size of extra-niche fossa in the stenosis group was smaller than that of the control group (p < 0.05). There was no statistically significant difference of the depth of extra-niche fossa among different groups.
A new impedance method for measuring middle ear mechanics and eustachian tube function

Abstract:The main principle of this new impedance method is a continuous recording of changes in tympanic membrane compliance in relation to its neutral position during changes in pressure, using a small pressure chamber. The subject sitting in the chamber can be placed in the recumbent position by rotation of the chamber. The working principles of the impedance method are: a servosystem regulates displacement of the loudspeaker membrane in order to keep the sound pressure in the external ear canal constant, independent of tympanic membrane compliance. From this recording it is possible to isolate tympanic membrane compliance during changes in intratympanic pressure as well as during ambient pressure changes. The results indicate that the new regulated impedance method combined with the small pressure chamber should be useful in routine clinical examination of middle ear mechanics and eustachian tube function.
