Abstract:Ledoux and colleagues 1 should be congratulated on conducting one of the largest studies to date on concussions in our youth population, defined as children aged 5 to 12 years, a historically challenging population to study. This multicenter study 1 examined 1747 youths who presented to emergency departments with both sport- and non–sport-related concussions, excluding concussions involving motor vehicle crashes (MVCs) and assaults. The authors dichotomized the sample into children aged 5 to 7 years and 8 to 12 years and examined changes in symptoms over 5 time points (1, 2, 4, 8, and 12 weeks postconcussion). Approximately 43% of the concussions in children aged 5 to 7 years were sport-related, compared with 68% in children aged 8 to 12 years. 1 When comparing sport-related and non–sport-related concussions, results did not reveal significant differences in symptom recovery trajectories in either age group, nor were there recovery trajectory differences among different sport classifications (noncontact, limited contact or impact, and contact or collision). The current study 1 has several strengths, including the prospective and multicenter nature. Moreover, patients were assessed at various time points, which allows us to see detailed symptom trajectories over time. Interestingly, the authors 1 used the term persisting symptoms after concussion (PSAC) to describe lingering symptoms instead of more traditional terms like persistent postconcussive symptoms or postconcussive syndrome. These traditional terms often connotate a permanent syndrome and/or assumption that the concussion itself is solely responsible for 100% of symptoms, which can be harmful to a patient's recovery. Conversely, PSAC offers room for the clinician to discuss how other causes may be maintaining, magnifying, or mimicking concussion symptoms. 2 Several thought-provoking ideas are brought forth by the current study 1 that can help improve our knowledge of concussions in youth athletes. First, although these results demonstrate similar outcomes between sport-related and non–sport-related concussions, we must consider the context and sample characteristics of this study 1 and avoid broad assumptions that sport and nonsport injuries are the same. All patients in this study 1 presented to an emergency department, whereas many youths with concussions present in a clinic setting, either to a pediatrician or other health care professionals. 3 Furthermore, this study excluded concussions from MVCs and assaults, likely to mitigate the effects of multiple trauma and/or psychological symptoms. Roby et al 4 found that non–sport-related and non–recreation-related concussions, such as assaults and MVCs, had higher symptom burdens compared with sport- and recreation-related concussions. Lumba-Brown et al 5 compared pediatric postconcussive symptom burdens across various mechanisms and found that individuals with concussions after MVCs presented with lower symptoms immediately postinjury but also had a slower rate of symptom improvement compared with those who sustained a concussion through other mechanisms. We must remember the overarching mantra in sport-related concussion management is to treat each concussion individually, depending on the injury and the athlete, and we should avoid sweeping conclusions about such a unique and heterogeneous injury. Second, the authors 1 astutely separated the youth cohort into 2 groups, and the older youth athletes were starkly different than the younger cohort in terms of the sports they participated in. Youth sports in the US quickly escalate in rigor and popularity as one progresses through elementary school. Utilizing the age groups defined by Ledoux et al, 1 children aged 8 to 12 years may begin their maturation into adolescence, both physically and mentally, and sports become more competitive and more intense. However, children aged 5 to 7 years are often receiving an introduction to sports and typically participate with less intensity. Although the minimum age for Pop Warner American football is 5 years, 6 among 186 online survey responses of middle-aged American men who participated in high school football, approximately one-half reported beginning their football participation before age 12 years. 7 This finding illustrates that more children may be starting their journeys into sports with enhanced competition and vigor when they are aged 8 to 12 years. Third, this large epidemiologic investigation 1 allows us to examine potential differences between youth ages 5 to 7 years and aged 8 to 12 years.
