Abstract:Up to 35% of individuals diagnosed with epilepsy continue to have seizures despite treatment, commonly referred to as drug-resistant epilepsy. Uncontrolled seizures can directly, or indirectly, negatively impact an individual's quality of life. To inform clinical management and life decisions, it is important to be able to predict the likelihood of seizure control. Those likely to achieve seizure control will be able to return sooner to their usual work and leisure activities and require less follow-up, whereas those with a poor prognosis will need more frequent clinical attendance and earlier consideration of epilepsy surgery. This is a systematic review aimed at identifying demographic, clinical, physiological (e.g., electroencephalographic), and imaging (e.g., magnetic resonance imaging) factors that may be predictive of treatment outcomes in patients with newly diagnosed epilepsy (NDE). MEDLINE and Embase were searched for prediction models of treatment outcomes in patients with NDE. Study characteristics were extracted and subjected to assessment of risk of bias (and applicability concerns) using the PROBAST (Prediction Model Risk of Bias Assessment Tool) tool. Baseline variables associated with treatment outcomes are reported as prognostic factors. After screening, 48 models were identified in 32 studies, which generally scored low for concerns of applicability, but universally scored high for susceptibility to bias. Outcomes reported fit broadly into four categories: drug resistance, short-term treatment response, seizure remission, and mortality. Prognostic factors were also heterogenous, but the predictors that were commonly significantly associated with outcomes were those related to seizure characteristics/types, epilepsy history, and age at onset. Antiseizure medication response was often included as a baseline variable, potentially obscuring other factor relationships at baseline. Currently, outcome prediction models for NDE demonstrate a high risk of bias. Model development could be improved with a stronger adherence to recommended TRIPOD (Transparent Reporting of a Multivariable Prediction Model for Individual Prognosis or Diagnosis) practices. Furthermore, we outline actionable changes to common practices that are intended to improve the overall quality of prediction model development in NDE.

Abstract:Epilepsy is a neurological disease characterized by recurrent and spontaneous seizures. It affects approximately 50 million people worldwide. In majority of the cases accurate diagnosis of the disease can be made without using any technologically advanced techniques and seizures are controlled using standard treatment in the form of regular use of anti-epileptic drugs. However, approximately 30% of the patients suffer from medically refractory epilepsy, wherein seizures are not controlled by the use of anti-epileptic drugs. Understanding the mechanisms underlying these forms of drug resistant epileptic seizures and the development of alternative effective treatment strategies is a fundamental challenge in modern epilepsy research. In this context, the need for integrative approaches combining various modalities of treatment strategies is high. Computational modeling has gained prominence in recent years as an important tool for tackling the complexity of the epileptic phenomenon. In this review article we present a survey of different computational models for epilepsy and discuss how computer models can aid in our understanding of brain mechanisms in epilepsy and the development of new epilepsy treatment protocols.

Abstract:Abstract Although antiepileptic drugs (AEDs) are the most effective treatment for epilepsy, 30–40% of patients with epilepsy would develop drug-refractory epilepsy. An accurate, preliminary prediction of the efficacy of AEDs has great clinical significance for patient treatment and prognosis. Some studies have developed statistical models and machine-learning algorithms (MLAs) to predict the efficacy of AEDs treatment and the progression of disease after treatment withdrawal, in order to provide assistance for making clinical decisions in the aim of precise, personalized treatment. The field of prediction models with statistical models and MLAs is attracting growing interest and is developing rapidly. What’s more, more and more studies focus on the external validation of the existing model. In this review, we will give a brief overview of recent developments in this discipline.

Abstract:With the advancement in artificial intelligence (AI) and machine learning (ML) techniques, researchers are striving towards employing these techniques for advancing clinical practice. One of the key objectives in healthcare is the early detection and prediction of disease to timely provide preventive interventions. This is especially the case for epilepsy, which is characterized by recurrent and unpredictable seizures. Patients can be relieved from the adverse consequences of epileptic seizures if it could somehow be predicted in advance. Despite decades of research, seizure prediction remains an unsolved problem. This is likely to remain at least partly because of the inadequate amount of data to resolve the problem. There have been exciting new developments in ML-based algorithms that have the potential to deliver a paradigm shift in the early and accurate prediction of epileptic seizures. Here we provide a comprehensive review of state-of-the-art ML techniques in early prediction of seizures using EEG signals. We will identify the gaps, challenges, and pitfalls in the current research and recommend future directions.

Abstract:Epilepsy is a disorder characterised by spontaneous, recurrent seizures. Both local and network abnormalities have been associated with epilepsy, and the exact processes generating seizures are thought to be heterogeneous and patient-specific. Due to the heterogeneity, treatments such as surgery and medication are not always effective in achieving full seizure control and choosing the best treatment for the individual patient can be challenging. Predictive models constrained by the patient's own data therefore offer the potential to assist in clinical decision making. In this chapter, we describe how personalised patient-derived networks from structural or functional connectivity can be incorporated into predictive models. We focus specifically on dynamical systems models which are composed of differential equations capable of simulating brain activity over time. Here we review recent studies which have used these models, constrained by patient data, to make personalised patient-specific predictions about seizure features (such as propagation patterns) or treatment outcomes (such as the success of surgical resection). Finally, we suggest future research directions for patient-specific network models in epilepsy, including their application to integrate information from multiple modalities, to predict long-term disease evolution, and to account for within-subject variability for treatment.
A sustainable artificial-intelligence-augmented digital care pathway for epilepsy: Automating seizure tracking based on electroencephalogram data using artificial intelligence

Abstract:Objective: Scalp electroencephalograms (EEGs) are critical for neurological evaluations, particularly in epilepsy, yet they demand specialized expertise that is often lacking in many regions. Artificial intelligence (AI) offers potential solutions to this gap. While existing AI models address certain aspects of EEG analysis, a fully automated system for routine EEG interpretation is required for effective epilepsy management and healthcare professionals' decision-making. This study aims to develop an AI-augmented model for automating EEG seizure tracking, thereby supporting a sustainable digital care pathway for epilepsy (DCPE). The goal is to improve patient monitoring, facilitate collaborative decision-making, ensure timely medication adherence, and promote patient compliance. Method: The study proposes an AI-augmented framework using machine learning, focusing on quantitative analysis of EEG data to automate DCPE. A focus group discussion was conducted with healthcare professionals to find the problem of the current digital care pathway and assess the feasibility, usability, and sustainability of the AI-augmented system in the digital care pathway. Results: The study found that a combination of random forest with principal component analysis and support vector machines with KBest feature selection achieved high accuracy rates of 96.52% and 95.28%, respectively. Additionally, the convolutional neural networks model outperformed other deep learning algorithms with an accuracy of 97.65%. The focus group discussion revealed that automating the diagnostic process in digital care pathway could reduce the time needed to diagnose epilepsy. However, the sustainability of the AI-integrated framework depends on factors such as technological infrastructure, skilled personnel, training programs, patient digital literacy, financial resources, and regulatory compliance. Conclusion: The proposed AI-augmented system could enhance epilepsy management by optimizing seizure tracking accuracy, improving monitoring and timely interventions, facilitating collaborative decision-making, and promoting patient-centered care, thereby making the digital care pathway more sustainable.

Abstract:Introduction Machine learning is a rapidly expanding field and is already incorporated into many aspects of medicine including diagnostics, prognostication and clinical decision-support tools. Epilepsy is a common and disabling neurological disorder, however, management remains challenging in many cases, despite expanding therapeutic options. We present a systematic review protocol to explore the role of machine learning in the management of epilepsy. Methods and analysis This protocol has been drafted with reference to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) for Protocols. A literature search will be conducted in databases including MEDLINE, Embase, Scopus and Web of Science. A PRISMA flow chart will be constructed to summarise the study workflow. As the scope of this review is the clinical application of machine learning, the selection of papers will be focused on studies directly related to clinical decision-making in management of epilepsy, specifically the prediction of response to antiseizure medications, development of drug-resistant epilepsy, and epilepsy surgery and neuromodulation outcomes. Data will be extracted following the CHecklist for critical Appraisal and data extraction for systematic Reviews of prediction Modelling Studies checklist. Prediction model Risk Of Bias ASsessment Tool will be used for the quality assessment of the included studies. Syntheses of quantitative data will be presented in narrative format. Ethics and dissemination As this study is a systematic review which does not involve patients or animals, ethics approval is not required. The results of the systematic review will be submitted to peer-review journals for publication and presented in academic conferences. PROSPERO registration number CRD42023442156.

Abstract:Epilepsy stands as one of the prevalent and significant neurological disorders, representing a critical healthcare challenge. Recently, machine learning techniques have emerged as versatile tools across various healthcare domains, encompassing diagnostics, treatment assessment, and prognosis. We compared 11 machine learning model to find the best ML model to predict drug treatment outcomes for our cohort, which we previously evaluated using classical statistical methods. Methods: In our study, we evaluated patients who presented to the pediatric neurology department of our university hospital with seizures at the age of 1 to 24 months and were diagnosed with epilepsy. We utilized 11 different machine learning techniques namely Decision Tree, Bagging, K-Nearest Neighbour, Linear Discriminant Analysis, Logistic Regression, Neural Networks, Deep Neural Networks, Support Vector Machine. Besides, we compared these techniques using various performance metrics to identify anti-seizure medicine response. We also utilized the chi-square feature selection methods to enhance performance in machine learning algorithms. Results: Two hundred and twenty-nine patients (110 male and 119 female) who were diagnosed between the ages of 1-24 months were included in the study. Support Vector Machine algorithm was found to be effective in drug resistant epilepsy detection, with the highest aure under curve value (0.9934) and achieving a test accuracy of 97.06 %. Conclusion: This study can shed light on future studies by showing that the Support Vector Machine algorithm can effectively determine the drug resistant epilepsy. The pediatric neurologist and experts should be referred to non-medical treatment (epilepsy surgery, ketogenic diet) at the early stages and multidisciplinary approach should be provided.

Abstract:In this study, we developed and tested machine learning models to predict epilepsy surgical outcome using noninvasive clinical and demographic data from patients. Methods: Seven dif-ferent categorization algorithms were used to analyze the data. The techniques are also evaluated using the Leave-One-Out method. For precise evaluation of the results, the parameters accuracy, precision, recall and, F1-score are calculated. Results: Our findings revealed that a machine learning-based presurgical model of patients' clinical features may accurately predict the outcome of epilepsy surgery in patients with drug-resistant lesional epilepsy. The support vector machine (SVM) with the linear kernel yielded 76.1% in terms of accuracy could predict results in 96.7% of temporal lobe epilepsy (TLE) patients and 79.5% of extratemporal lobe epilepsy (ETLE) cases using ten clinical features. Significance: To predict the outcome of epilepsy surgery, this study recommends the use of a machine learning strategy based on supervised classification and se-lection of feature subsets data mining. Progress in the development of machine learning-based prediction models offers optimism for personalised medicine access.

Abstract:<p>Big Data is no longer a novel concept in health care. Its promise of positive impact is not only undiminished, but daily enhanced by seemingly endless possibilities. Epilepsy is a disorder with wide heterogeneity in both clinical and research domains, and thus lends itself to Big Data concepts and techniques. It is therefore inevitable that Big Data will enable multimodal research, integrating various aspects of "‐omics" domains, such as phenome, genome, microbiome, metabolome, and proteome. This scope and granularity have the potential to change our understanding of prognosis and mortality in epilepsy. The scale of new discovery is unprecedented due to the possibilities promised by advances in machine learning, in particular deep learning. The subsequent possibilities of personalized patient care through clinical decision support systems that are evidence‐based, adaptive, and iterative seem to be within reach. A major objective is not only to inform decision‐making, but also to reduce uncertainty in outcomes. Although the adoption of electronic health record (EHR) systems is near universal in the United States, for example, advanced clinical decision support in or ancillary to EHRs remains sporadic. In this review, we discuss the role of Big Data in the development of clinical decision support systems for epilepsy care, prognostication, and discovery.</p>

Artificial intelligence and telemedicine in epilepsy and EEG: A narrative review

Abstract:The emergence of telemedicine and artificial intelligence (AI) has set the stage for a possible revolution in the future of medicine and neurology including the diagnosis and management of epilepsy. Telemedicine, with its proven efficacy during the COVID-19 pandemic, offers the advantage of bridging the gap between patients in resource-limited areas and specialized care, where in one study telemedicine reduced the epilepsy treatment gap from 43 % to 9 %. AI innovations promise a transformation in epilepsy care by possibly enhancing the accuracy of electroencephalogram (EEG) interpretation and seizure prediction through machine and deep learning. In one study, abnormal EEG recordings were classified into different categories using a convolutional neural networks (CNN) model showing a specificity of 90 % and an accuracy of 88.3 %. Other models constructed to predict seizures have also achieved a sensitivity of 96.8 % and specificity of 95.5 %. Various machine learning (ML) models highlight the potential AI holds in identifying interictal biomarkers and localizing seizure onset zones aiding in epilepsy treatment decision and outcome prediction. An ML model highlighted in this review localized seizure onset zone with an accuracy reaching 73 % and predicted surgical outcomes with an accuracy reaching 79 % compared to the 43 % accuracy of clinicians. However, limitations and challenges hinder the application of such technologies to reach their full potential in epilepsy care. Limitations include access to compatible devices, integration into clinical workflows, data bias, and availability of sufficient data. Extensive validated research is needed to guide future clinical practice with the implementation of technology-enhanced epilepsy care. This narrative review article will explore the use of AI and telemedicine in EEG and epilepsy care, examining their individual and combined impacts in shaping the future of epilepsy care and discussing the challenges and limitations faced in their usage.

However, due to the variations in their modes of action, identification of effective AEDs often relies on ad hoc trials, which is particularly challenging for pediatric patients. Thus, there is significant value in computational methods capable of assisting in the selection of AEDs, aiming to minimize unnecessary medication and improve treatment efficacy.

Predicting efficacy of antiseizure medication treatment with machine learning algorithms in North Indian population

Abstract:Purpose: This study aimed to develop a classifier using supervised machine learning to effectively assess the impact of clinical, demographical, and biochemical factors in accurately predicting the antiseizure medications (ASMs) treatment response in people with epilepsy (PWE). Methods: Data was collected from 786 PWE at the Outpatient Department of Neurology, Institute of Human Behavior and Allied Sciences (IHBAS), New Delhi, India from 2005 to 2015. Patients were followed up at the 2nd, 4th, 8th, and 12th month over the span of 1 year for the drugs being administered and their dosage, the serum drug levels, the frequency of seizure control, drug efficacy, the adverse drug reactions (ADRs), and their compliance to ASMs. Several features, including demographic details, medical history, and auxiliary examinations electroencephalogram (EEG) or Computed Tomography (CT) were chosen to discern between patients with distinct remission outcomes. Remission outcomes were categorized into 'good responder (GR)' and 'poor responder (PR)' based on the number of seizures experienced by the patients over the study duration. Our dataset was utilized to train seven classical machine learning algorithms i.e Extreme Gradient Boost (XGB), K-Nearest Neighbor (KNN), Support Vector Classifier (SVC), Decision Tree (DT), Random Forest (RF), Naïve Bayes (NB) and Logistic Regression (LR) to construct classification models. Results: Our research findings indicate that 1) among the seven algorithms examined, XGB and SVC demonstrated superior predictive performances of ASM treatment outcomes with an accuracy of 0.66 each and ROC-AUC scores of 0.67 (XGB) and 0.66 (SVC) in distinguishing between PR and GR patients. 2) The most influential factor in discerning PR to GR patients is a family history of seizures (no), education (literate) and multitherapy with Chi-square (χ2) values of 12.1539, 8.7232 and 13.620 respectively and odds ratio (OR) of 2.2671, 0.4467, and 1.9453 each. 3). Furthermore, our surrogate analysis revealed that the null hypothesis for both XGB and SVC was rejected at a 100 % confidence level, underscoring the significance of their predictive performance. These findings underscore the robustness and reliability of XGB and SVC in our predictive modelling framework. Significance: Utilizing XG Boost and SVC-based machine learning classifier, we successfully forecasted the likelihood of a patient's response to ASM treatment, categorizing them as either PR or GR, post-completion of standard epilepsy examinations. The classifier's predictions were found to be statistically significant, suggesting their potential utility in improving treatment strategies, particularly in the personalized selection of ASM regimens for individual epilepsy patients.
A Predictive Risk Model for Medical Intractability in Epilepsy.

Abstract:OBJECTIVE:This study aimed to investigate early predictors (6 months after diagnosis) of medical intractability in epilepsy.METHODS:All children <12 years of age having two or more unprovoked seizures 24 h apart at Xinhua Hospital between 1992 and 2006 were included. Medical intractability was defined as failure, due to lack of seizure control, of more than 2 antiepileptic drugs at maximum tolerated doses, with an average of more than 1 seizure per month for 24 months and no more than 3 consecutive months of seizure freedom during this interval. Univariate and multivariate logistic regression models were performed to determine the risk factors for developing medical intractability. Receiver operating characteristic curve was applied to fit the best compounded predictive model.RESULTS:A total of 649 patients were identified, out of which 119 (18%) met the study definition of intractable epilepsy at 2 years after diagnosis, and the rate of intractable epilepsy in patients with idiopathic syndromes was 12%. Multivariate logistic regression analysis revealed that neurodevelopmental delay, symptomatic etiology, partial seizures, and more than 10 seizures before diagnosis were significant and independent risk factors for intractable epilepsy. The best model to predict medical intractability in epilepsy comprised neurological physical abnormality, age at onset of epilepsy under 1 year, more than 10 seizures before diagnosis, and partial epilepsy, and the area under receiver operating characteristic curve was 0.7797. This model also fitted best in patients with idiopathic syndromes.CONCLUSION:A predictive model of medically intractable epilepsy composed of only four characteristics is established. This model is comparatively accurate and simple to apply clinically.
A comprehensive prediction model of drug-refractory epilepsy based on combined clinical-EEG microstate features

Therapeutic Advances in Neurological Disorders

Abstract:Background: Epilepsy is a chronic neurological disorder characterized by recurrent seizures that significantly impact patients’ quality of life. Identifying predictors is crucial for early intervention. Objective: Electroencephalography (EEG) microstates effectively describe the resting state activity of the human brain using multichannel EEG. This study aims to develop a comprehensive prediction model that integrates clinical features with EEG microstates to predict drug-refractory epilepsy (DRE). Design: Retrospective study. Methods: This study encompassed 226 patients with epilepsy treated at the epilepsy center of a tertiary hospital between October 2020 and May 2023. Patients were categorized into DRE and non-DRE groups. All patients were randomly divided into training and testing sets. Lasso regression combined with Stepglm [both] algorithms was used to screen independent risk factors for DRE. These risk factors were used to construct models to predict the DRE. Three models were constructed: a clinical feature model, an EEG microstate model, and a comprehensive prediction model (combining clinical-EEG microstates). A series of evaluation methods was used to validate the accuracy and reliability of the prediction models. Finally, these models were visualized for display. Results: In the training and testing sets, the comprehensive prediction model achieved the highest area under the curve values, registering 0.99 and 0.969, respectively. It was significantly superior to other models in terms of the C-index, with scores of 0.990 and 0.969, respectively. Additionally, the model recorded the lowest Brier scores of 0.034 and 0.071, respectively, and the calibration curve demonstrated good consistency between the predicted probabilities and observed outcomes. Decision curve analysis revealed that the model provided significant clinical net benefit across the threshold range, underscoring its strong clinical applicability. We visualized the comprehensive prediction model by developing a nomogram and established a user-friendly website to enable easy application of this model ( https://fydxh.shinyapps.io/CE_model_of_DRE/ ). Conclusion: A comprehensive prediction model for DRE was developed, showing excellent discrimination and calibration in both the training and testing sets. Potential merits and flaws of large language models in epilepsy care: A critical review

Abstract:The current pace of development and applications of large language models (LLMs) is unprecedented and will impact future medical care significantly. In this critical review, we provide the background to better understand these novel artificial intelligence (AI) models and how LLMs can be of future use in the daily care of people with epilepsy. Considering the importance of clinical history taking in diagnosing and monitoring epilepsy—combined with the established use of electronic health records—a great potential exists to integrate LLMs in epilepsy care. We present the current available LLM studies in epilepsy. Furthermore, we highlight and compare the most commonly used LLMs and elaborate on how these models can be applied in epilepsy. We further discuss important drawbacks and risks of LLMs, and we provide recommendations for overcoming these limitations.

Abstract:Studies on treatment of epilepsy have been actively conducted in multiple avenues, but there are limitations in improving its efficacy due to between-subject variability in which treatment outcomes vary from patient to patient. Accordingly, there is a growing interest in precision medicine that provides accurate diagnosis for seizure types and optimal treatment for an individual epilepsy patient. Among these approaches, computational studies making this feasible are rapidly progressing in particular and have been widely applied in epilepsy. These computational studies are being conducted in two main streams: 1) artificial intelligence-based studies implementing computational machines with specific functions, such as automatic diagnosis and prognosis prediction for an individual patient, using machine learning techniques based on large amounts of data obtained from multiple patients and 2) patient-specific modeling-based studies implementing biophysical in-silico platforms to understand pathological mechanisms and derive the optimal treatment for each patient by reproducing the brain network dynamics of the particular patient per se based on individual patient's data. These computational approaches are important as it can integrate multiple types of data acquired from patients and analysis results into a single platform. If these kinds of methods are efficiently operated, it would suggest a novel paradigm for precision medicine.

Abstract:Epilepsy is a common neurological disease where the earlier disease prediction significantly impacts those patients' lives. In this paper, a novel epilepsy seizure prediction approach is designed using deep learning. The proposed model is applied to the Electroencephalogram (EEG) recordings collected from Children's Hospital Boston (CHB-MIT). The recording data is grouped into 23 cases, including 17 females and five males of different ages. The recordings are sampled at 256 samples/s of 16-bit resolution. The target is to analyze the brain's state and evaluate the changes encountered from the interictal state. The earlier prediction process helped in timely disease identification and treatment to rescue the patients. Feeding the raw EEG signals over the feature extractor reduces the computational complexity and execution time. An Adaptive Grey Wolf Optimizer (AGWO) is used for learning the features and promoting those discriminative features to enhance the prediction rate and classification accuracy. To optimize the features integrating the auto-encoder concept with Genetic Algorithm (GA) in an adaptive manner termed as to enhance the prediction rate. The functionality of is tested over the subjects of the CHB-MIT EEG dataset to achieve resourceful outcomes. The proposed attains higher accuracy of 99% and reduces the False Alarm Rate (FAR) with little prediction time. The model's functionality is evaluated using the MATLAB simulation environment and shows a better trade-off than existing approaches.

JAMA Neurol. 2022;79(10):986-996. doi:10.1001/jamaneurol.2022.2514 Importance:Selection of antiseizure medications (ASMs) for epilepsy remains largely a trial-and-error approach. Under this approach, many patients have to endure sequential trials of ineffective treatments until the "right drugs" are prescribed.Objective:To develop and validate a deep learning model using readily available clinical information to predict treatment success with the first ASM for individual patients.Design, Setting, and Participants:This cohort study developed and validated a prognostic model. Patients were treated between 1982 and 2020. All patients were followed up for a minimum of 1 year or until failure of the first ASM. A total of 2404 adults with epilepsy newly treated at specialist clinics in Scotland, Malaysia, Australia, and China between 1982 and 2020 were considered for inclusion, of whom 606 (25.2%) were excluded from the final cohort because of missing information in 1 or more variables.Exposures:One of 7 antiseizure medications.Main Outcomes and Measures:With the use of the transformer model architecture on 16 clinical factors and ASM information, this cohort study first pooled all cohorts for model training and testing. The model was trained again using the largest cohort and externally validated on the other 4 cohorts. The area under the receiver operating characteristic curve (AUROC), weighted balanced accuracy, sensitivity, and specificity of the model were all assessed for predicting treatment success based on the optimal probability cutoff. Treatment success was defined as complete seizure freedom for the first year of treatment while taking the first ASM. Performance of the transformer model was compared with other machine learning models.Results:The final pooled cohort included 1798 adults (54.5% female; median age, 34 years [IQR, 24-50 years]). The transformer model that was trained using the pooled cohort had an AUROC of 0.65 (95% CI, 0.63-0.67) and a weighted balanced accuracy of 0.62 (95% CI, 0.60-0.64) on the test set. The model that was trained using the largest cohort only had AUROCs ranging from 0.52 to 0.60 and a weighted balanced accuracy ranging from 0.51 to 0.62 in the external validation cohorts. Number of pretreatment seizures, presence of psychiatric disorders, electroencephalography, and brain imaging findings were the most important clinical variables for predicted outcomes in both models. The transformer model that was developed using the pooled cohort outperformed 2 of the 5 other models tested in terms of AUROC.Conclusions and Relevance:In this cohort study, a deep learning model showed the feasibility of personalized prediction of response to ASMs based on clinical information. With improvement of performance, such as by incorporating genetic and imaging data, this model may potentially assist clinicians in selecting the right drug at the first trial.

Abstract:Epileptic encephalopathy (EE) represents a challenging group of disorders characterized by severe epilepsy and significant cognitive, behavioral, and neurological impairments. This comprehensive review aims to elucidate the current insights into the pathogenesis and therapeutic strategies for these disorders. Pathogenesis involves a complex interplay of genetic factors, neurobiological mechanisms, and environmental influences that contribute to the severity and progression of symptoms. Clinical manifestations are diverse, encompassing various seizure types, cognitive and behavioral impairments, and developmental delays. Current therapeutic strategies include pharmacological treatments, nonpharmacological interventions, and emerging therapies such as gene and stem cell therapy. Despite advancements, significant challenges and limitations remain, highlighting the need for ongoing research and innovation. This review synthesizes existing knowledge, identifies research gaps, and proposes future directions, emphasizing the potential for personalized medicine to improve patient outcomes and quality of life.
