Abstract:Abstract Introduction Placement of an inflatable penile prosthesis (IPP) is indicated for patients with severe Peyronie's disease and erectile dysfunction (ED). Most cases involving plaque incision and graft placement are performed at the point of maximal curvature which result in a large defect that requires placement of a large graft while postoperative penile shortening remains a significant concern among patients. Objective To present a video of a novel modification of the multiple slit technique (MUST) for the treatment of severe PD and ED during IPP placement. This technique involves multiple relaxing incisions made around the point of maximal curvature (POMC) which minimizes the size or even the need of a graft while allowing for length restoration. Methods This technique involves elevation of the neurovascular bundle for dorsal or lateral curvatures along with elevation of the urethra for ventral curvatures. After the POMC is identified, 2 to 4 transverse relaxing incisions (slices) are made around the POMC approximately 1 to 1.5cm apart in order to minimize the size of the defect and hence the need or size of the graft. Each slice allows for maximal restoration of length. Results The SAPOC technique results in correction of curvature and length restoration. Conclusions The SAPOC technique at the time of IPP placement is a novel, safe and feasible technique for the management of complex PD and ED. This technique allows the surgeon to complete the procedure without graft placement in some occasions which theoretically will decrease operative time. It also provides for length restoration which will increase patient satisfaction. Abstract:Severe Peyronie’s disease (PD) and concomitant erectile dysfunction (ED) may require plaque incision/excision and grafting (PIG) as an adjunct to penile prosthesis implantation (IPP). Currently, there is no available consensus on the best graft material to use. Our aim was to systematically review graft materials used as patches following PIG + IPP. Literature search was performed in March 2021. Only original articles in English with a series of 10 or more patients were included. Overall, a total of 17 studies were included, corresponding to a cohort of 662 patients. The mean age ranged from 45 to 65 years and most patients had curvatures >45°. Average penile lengthening ranged from 1 to 3.5 cm, average residual curvatures from 0 to 20% and decreased glans sensitivity from 0 to 20%. Eighty to 100% of patients were satisfied with cosmetic and functional results. PIG + IPP with the use of various grafts offers promising results for the treatment of patients suffering from severe PD with concomitant ED. Unfortunately, the absence of high quality and comparative studies makes it difficult to establish the optimum graft. Therefore, the level of experience of the surgical team with one or more methods should guide their choice.

A common complaint is reduced engorgement of the glans penis, which can manifest as subjective reduction in penile length or the feeling of a cold insensitive glans. Patient factors often ensure a sound justification for this complaint, namely vasculogenic erectile dysfunction (ED) or their corporo-glandular morphology despite a well-sized and well-positioned implant. Objective In this study, we sought to identify factors that may contribute to reduced glans engorgement and possible management strategies. Methods Intervention: Any method of treating or avoiding reduced glans engorgement post-IPP. Population: Men undergoing IPP insertion or men with glans complaints post-operatively Inclusion Criteria: Articles published in English with full text available and objective outcome measures Search Strategy: Literature search was performed in June 2023 using Medline, PubMed, Embase and Scopus. PubMed search included both "Mesh" protocols and "free text" to capture the correct study population and intervention utilized. Review Type & Analysis: Although an initial systematic model was considered, a scoping model was ultimately selected due to the limited literature and inconsistent outcome measures. This approach aimed to identify gaps in knowledge and provide an overview of the existing literature, thus informing future studies in this area. The variability in outcome measures across studies prevented higher-powered analyses from being conducted. Results Initial search strategy retrieved 98 non-duplicate studies with 40 being selected for initial screening after applying inclusion criteria. Ultimately, a total of 7 articles were included and categorized by theme: Cavernous-sparing technique (1–3); Pharmacotherapy for post-IPP glans complaints (4–7) (Table 1 & 2). The cavernous-sparing technique described aims to limit disruption of cavernous tissue around the implanted cylinder. This was achieved by omitting dilatation and using Furlow directly for measurements, or by intraoperative intracavernosal (IC) prostaglandin followed by dilatation only with size 7 or 8 Hegar alone. In all studies that compared penile dimensions, girth was significantly greater in the cavernous-sparing group. A higher proportion of men in this group also had detectable cavernous arteries, thicker residual corporal tissue, better residual tumescence post-IPP and demonstrated significant increases in both PSV and EDV post-PDE5i, compared to no PDE5i. Men who had standard dilatation also responded to post-IPP PDE5i but fewer had identifiable cavernous artery and had significant increase in only PSV, compared to no PDE5i. Other articles relating to post-IPP pharmacotherapy unanimously demonstrated that post-IPP glans engorgement was improved. This equated to reported statistically improved satisfaction when compared to the IPP alone groups, as measured by IIEF and EDITS questionnaires. Post-IPP PDE5i were better tolerated than intraurethral alprostadil, which was associated with discomfort. Conclusions Post-IPP pharmacotherapy appears to enhance residual erectile tissue engorgement. It can be used as an adjunctive after-care measure. Abstract:Surgical options for Peyronie's Disease (PD) can be broadly broken down into three approaches: penile plication, plaque incision (partial excision) and grafting, and penile prosthesis (PP) with or without adjunctive straightening maneuvers [1]. Multiple factors influence the decision to pursue one approach over another. These factors include curvature severity, presence of indentation or hourglass, perceived and actual penile shortening, and baseline erectile function. For patients with underlying medication-refractory erectile dysfunction (ED), the surgical treatment of choice is likely PP with straightening maneuvers [2]. In contrast, patients without ED are candidates for plication or grafting without an implant, thereby maintaining spontaneous erectile function. In contrast to this classic surgical paradigm, Moncada et al. report their experience with PP for men with PD including in those with intact erectile function [3]. The authors identified 168 patients among five centers who opted for PP as first-line treatment for PD. The cohort included 33 patients (nearly 20%) without ED who opted for PP placement in lieu of other treatments. It was suggested that these patients had "significant or complex penile curvature", although the average curvature was 46 degrees and only 18% had hourglass deformity or narrowing. It was also suggested that patients without ED who elected PP were at higher risk for postoperative ED based on risk factors (older age, vasculopathy, etc.). Adjunctive maneuvers included modeling in the majority of patients (roughly 85%) and grafting in the other 15%. Importantly, patient-reported satisfaction rates based on a 5-point Likert scale were high in both patients with and without baseline ED (89 and 88%, respectively).

Abstract:Despite significant scientific advances in the modern three-piece inflatable penile prosthesis implant surgery, it is not without surgical risks and can carry additional cosmetic and psychosocial consequences in poorly selected and consented individuals. To address this problem, an international group of key opinion leaders and high-volume prosthetic surgeons reviewed the current guidelines and clinical evidence, discussed their experiences, and formed a consensus regarding inflatable penile prosthesis surgery. The findings of this consensus panel were presented at the 17th biennial Asia Pacific Society of Sexual Medicine scientific meeting. The experts concluded that proper patient selection, informed consent and strict adherence to safe surgical principles are important to optimize clinical outcomes. Furthermore, most intraoperative complications, if recognized, can be addressed intraoperatively to enable placement of the device at the time of initial surgery. Men with significant corporal fibrosis due to Peyronie’s disease, prior prosthesis explantation and priapism, and men who have undergone construction of a neophallus, as well as men who receive concurrent continence surgery, are complex cases requiring additional care and advanced techniques to obtain optimal surgical outcomes. Variability in patient care — in terms of postoperative antibiotic use, pain management, scrotal care, and cycling of the penile prosthesis implant — must be reduced to enable optimization and assessment of outcomes across patient groups.

Abstract:The mechanisms leading to Erectile dysfunction (ED) in Peyronie's disease (PD) are still debatable [1,2,3]. One plausible mechanism is the lack of activation of the veno-occlusive mechanism due to the plaque in PD [1] and some authors highlight the psychological burden and pain being the factors for poor erections in PD [2]. According to the theory proposed by Goldstein and Udelson [1], the inelasticity of the plaque causes an inadequate stretching of the sub-tunical venules leading to a lack of activation of the veno-occlusive mechanism and subsequent venous leakage. It is very common that the rigidity of the erection is normal in the base of the penis but is less firm distally from the point of plaque. This also may explain the de novo ED that some patients experience after grafting in PD due to a lack of tunica expansion and venous leakage. Geometrical alterations of the penile anatomy may also cause a lack of rigidity, a bent penis has less axial rigidity than a straight penis based on the geometrical properties of a column-like erect penis. The traditional concept in the management of PD has been to do penile prosthesis (PP) implantation when there is refractory ED [4]. However, there has been an evolving trend where some men with PD having normal erections have undergone PP during surgical correction of bothersome curvature [5, 6]. Recently, Moncada et al. reported that 19.6% ( n = 33/135) of PD patients without ED opted for PP when curvature correction surgery was planned [5]. Another study by Kurtzman et al. analysed the New York State PP Registry which revealed that 1 in 5 PD patients without ED had received a PP between 2003 and 2016 [6]. Intraoperative manual modelling [7] and post-operative home modelling [8] over inflatable PP in PD can help in the correction of primary or residual curvature respectively. Hence adjunct procedures such as plication or grafting can be avoided, which otherwise may lead to post-operative poor patient satisfaction.

Abstract:Abstract Introduction Historically, inflatable penile prostheses (IPP) insertion in a neophallus has an estimated 40-50% surgical revision rate. Objective To review the surgical outcomes of a single center’s experience with the infrapubic approach to insertion of the IPP after phalloplasty in transmen. Methods The infrapubic prosthesis insertion after phalloplasty (IPIP) technique involves a horizontal incision anterior to the pubic symphysis, which allows dissection of the neophallus channel, the Space of Retzius, and the scrotal pocket. Surgical outcomes using the Titan (Coloplast, Minneapolis, MN) IPP by a single surgeon (MLC) were analyzed between October 2017 and June 2023. Complications were reviewed and categorized into erosions, infections, device malposition and malfunction, and less common events like necrosis and urinary tract injury. Results The IPIP technique was performed on 175 phalloplasty patients by a single surgeon (MLC): 156 patients had a prior RF phalloplasty, and 19 had a prior ALT phalloplasty. The mean followup was 32.8 months. Surgical revision was required in 35 patients (20%). There were 10 pump erosions through the neoscrotum, 1 glans erosion, 9 infections, 6 pump malpositions in the neoscrotum, 6 cylinder malposition in the neophallus, 3 mechanical malfunctions with broken tubing, 2 urethral injuries, 2 bladder injuries, and 1 glans necrosis. There were no instances of implant detachment from the pubic bone anchor site, osteomyelitis, nor total phallus loss. Conclusions Data for the IPIP technique using the commercially available Coloplast Titan IPP suggest a reduction in neophallus prosthetic surgical revision rates compared to previous reports. Longer term studies with patient reported outcomes are required for further evaluation. Abstract:Inflatable penile prosthesis (IPP) surgery is an effective treatment for erectile dysfunction (ED), but infections pose a significant threat to its success. Current guidelines lack antifungal recommendations, despite rising fungal infection rates post-IPP surgery. This review examines epidemiology, risk factors (including diabetes mellitus, immunosuppression, and obesity), and pathogenesis, highlighting the role of biofilm formation in device contamination. Clinical manifestations vary from acute to delayed, with fungal biofilms presenting challenges in diagnosis. Prophylactic strategies, including broad-spectrum antibiotics and antifungals, are crucial, with evidence suggesting a 92% reduction in infections. With fungal infections showing lower salvage rates, management involves culture-guided treatment, irrigation, and oral antibiotics. Future research aims to understand biofilm mechanisms and develop biomaterials to reduce infection rates. Implementing antifungal therapy, along with standard practices like the no-touch technique and antibiotic dips, is crucial in preventing IPP infections.

Two-piece inflatable and semi-rigid penile implants: an effective alternative?

Abstract:Penile prostheses (PPs) are an excellent treatment to be considered for patients suffering from erectile dysfunction (ED), refractory to oral medications, or unwilling to consider intracavernosal injections or other therapeutic strategies. Nowadays the market is dominated by three-piece inflatable penile prosthesis (IPP), which are considered the best option available. However, in some specific cases three-piece IPP may be superseded by semi-rigid penile prosthesis (SRPP) and two-piece IPP. Despite the advantages of these implants, the literature is quite scarce, not robust and rarely takes into consideration the partner's satisfaction. This clearly affects the strength of our conviction. Most of the studies confirm that SRPPs and two-piece IPPs can be considered a valid alternative in patients with hand dexterity issues, refractory ischaemic priapism, neophallus and in cases of complex anatomies. In addition, the ease of placement and the high confidence level of surgeons make them a powerful tool in the armamentarium of urologists.

Abstract:Abstract Introduction Inflatable penile prostheses (IPP) are used in a multitude of urologic disorders including Peyronie’s disease and medication-refractory erectile dysfunction. The IPP procedure is performed by inserting a penile pump to help generate and maintain penile erections. Objective To analyze self-perceptions and body dysmorphia of patients who have had an IPP placement. Methods An electronic survey was delivered through REDCap to patients who had undergone an IPP placement and who were identified using diagnosis codes associated with the procedure within the 06/01/2007 - 09/01/2022 observation interval. The survey asked each patient about their comfort with their penis’s appearance and usage in sexual settings, with answer choices corresponding to a five-point Likert-Scale in addition to a free-form response section. T-tests were utilized to compare mean respondent scores before and after IPP placement using SPSS software. Results 436 surveys were sent out, and 69 responses were received (15.83%). When asked about comfort with their penis’s appearance, the respondents had a slight, non-significant increase after IPP (T-value = -0.771, p = .221). In response to inquiring about the patient’s ease using their penis in sexual relationships, patients reported an increase due to their IPP placement (T-value = -3.00, p = .0016). When reverse coded, patients demonstrated a great decrease in concern about their penis in social situations after the IPP procedure (T-value = -3.88, p = .00008). The total score of all questions before and after IPP placement increased by an average of 15.1% (T-value = -3.21, p = .00082). 44.78% of respondents noted a positive change in how the IPP changed how they view themselves and their body; 40.30% noted neutral change and 14.93% noted a negative change. In response to how the IPP has affected sexual and romantic relations, 68.66% reported a positive difference after IPP, 16.42% were neutral, and 14.93% reported a negative difference. Conclusions Our data demonstrate that IPPs have a stronger impact on sexual function, as opposed to body image; however, the vast majority of patients felt a positive or neutral change of how they view themselves and their bodies post-IPP. These findings support the continued use of IPPs in patients who are non-responsive to previous erectile dysfunction treatments. Abstract:Residual curvature correction during penile prosthesis implantation (PPI) is usually needed in patients with severe erectile dysfunction (ED) and concomitant Peyronie's disease (PD). The aim of the study was to assess the different existing techniques for treating residual penile curvature during PPI in patients with severe PD and ED. We generated a comprehensive narrative review of the literature until August 2020 on the use of PPI in combination with straightening reconstruction techniques, in treating patients with severe PD and ED. We included studies published in English, assessing the PPI as primary intervention in patients with PD and ED. Secondary research studies and studies with insufficient data were excluded from final analyses. We included a total of 33 clinical articles with 1,612 patients that assessed the effects of PPI combined with straightening surgical techniques for the treatment of severe PD and ED. Based on the severity of penile curvature, the concomitance of additional penile deformities (i.e., hourglass deformity), the penile length, the presence of previous penile operations and the surgeon's experience, four main categories of surgical techniques were identified: (I) PPI with plication of the penis on the convex side of the curvature, (II) transcorporeal plaque incision/excision, (III) PPI with plaque/tunical incision(s) on the concave side of the curvature and (IV) PPI with plaque incision/excision plus grafting. Patients with severe PD and ED can expect excellent outcomes with PPI and surgical correction of residual penile curvature and minimal side effects. Overall, all the above techniques seem to able to correct the residual penile curvature during prosthesis implantation. Grafting techniques seem to be favorable in patients with additional severe penile shortening. Still, no definite conclusions can be drawn regarding the superiority of one technique over the other.

Abstract:Implantation of inflatable penile prostheses (IPPs) has been established as a pivotal interventional option for patients suffering from erectile dysfunction with increased popularity in the past few decades. Data from the United States, which is the largest consumer country for IPPs, showed that a total of 54,146 implantation surgeries were performed between 2005 and 2012 with over half being IPPs [1]. During the implantation of the three-piece IPPs, the step of reservoir implantation may necessitate additional time and expertise. The reservoir is most commonly implanted in either the retropubic space of Retzius (the standard approach for placement) or the intra-abdominal submuscular space (the ectopic approach). Faulty reservoir placement technique can lead to complications such as displacement and subcutaneous reservoir resulting in bulging and disfigurement. Both techniques for insertion were described in a recent article by Gupta et al. [2]. The ectopic approach relies on retracting the external inguinal ring to create a space just above the pubic tubercle and under the Scarpa's fascia to ensure a submuscular insertion instead of an intramuscular one. Since the first penile implant was introduced in 1973, there have been several advances in the devices' design over years. We describe a novel advancement in the standard implantation technique of IPP through reservoir adjustment. For the proposed adjusted modification in the reservoir, using this approach is suggested to decrease the learning curve for young surgeons. Likewise, this can also reduce time of implantation which can lead to a decreased rates of infection post- operatively. It was reported that a shorter surgical time is associated with less post-operative infection rates [3]. It is also established that surgeon's experience can play a paramount role in the resulting infection and reoperation rates [4, 5]. Moreover, patients can benefit during the recovery times since using this method can prevent reservoir movement and decrease the probability for tubes rotation which can be associated with reservoir displacement leading to leakage and blockage of fluid transportation.

Abstract:Introduction Inflatable penile prosthesis (IPP) is seen as the gold standard for surgical treatment for ED, specifically for patients that are not amenable to lifestyle changes or oral medications. Objective This study aims to explore which patients progress to seeking treatment with an IPP along their journey with erectile dysfunction. Methods This retrospective study identified patients with ED as a chief complaint during the primary visit or follow-up from 2009-2022. If patients eventually progressed to treatment with IPP, data was collected to find common features and analyzed. Results Of our cohort of 361 men presenting with erectile dysfunction (ED), 21 men progressed to the placement of an inflatable penile prosthesis (IPP). 20 men classified as non-Hispanic, with 13 white, 6 African American/black, and 1 Asian males. Only 1 man was Hispanic. 5 men had either hypertension (n = 11), hyperlipidemia (n = 10), or DM (n = 3), with 6 having both hypertension and hyperlipemia, 1 with hypertension and diabetes, and 1 with hypertension, hyperlipidemia, and diabetes. Median age of the group at surgery was 76 years (IQR: 71 – 82), with median time from initial presentation to surgery of 12 (IQR = 11.5-27.5) months. N = 10 men had trialed oral phosphodiesterase inhibitor type 5 (PDE-5i) therapy, and 7 men failed intracavernosal injection (ICI) therapy. 8 men had been treated for hypogonadal symptoms with testosterone replacement therapy (TRT). Of patients who had data available on their reason for IPP placement, indications included were Peyronie's disease inhibiting intercourse (8/21), longstanding ED refractory to medical therapy (4/21), post-prostatectomy ED (4/21), history of priapism requiring surgical intervention (1/21), and history of spinal cord tumor (1/21). Among those treated with IPP, the median patient waited almost 1 year from initial presentation following failure with PDE5i, ICI, and TRT. 17/21 men reported satisfaction after IPP treatment. In those who progressed to IPP, the majority of indications were organic forms of ED (Peyronie's disease, neurogenic, surgery). Conclusions Only 5.8% of men in this cohort went on to proceed with an IPP. In summary, our results indicate that IPP is best suited for those presenting with organic ED who have failed more conservative management techniques. Further studies are needed to assess why so few men go on to proceed with an IPP and wait an average of 2 years. Abstract:Peyronie's disease (PD) presents a multifaceted challenge in contemporary urological practice, marked by penile deformity, pain, and the potential for erectile dysfunction. We meticulously explored the existing literature of intralesional/topical interventions, aiming to provide clinicians with a nuanced understanding of available options for comprehensive PD management. To conduct this review, we performed a systematic search using the PubMed, Scopus, and ScienceDirect databases, including the keywords of combination of the "Peyronie's disease/plastic induration of the penis (PIP) and intralesional/topical treatments". The study selection was based on adherence to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, resulting in the inclusion of 16 articles. We delve into the effectiveness and safety profiles of collagenase Clostridium histolyticum (CCH), interferon, platelet-rich plasma (PRP), hyaluronic acid, botulinum toxin, stem cell, extracorporeal shock wave therapy (ESWT), and traction therapy, assessing their impact on penile curvature, length improvement, and patient-reported symptoms and outcomes. The best options evaluated are intralesional injections of CCH and penile traction devices, alone or in combination. Despite PD remains a challenge for urologists, the objective of this review is to contribute to the evolving landscape of PD management, fostering informed decision-making, and personalized care for individuals grappling with this challenging condition.

Abstract:Abstract Introduction The Satisfaction Survey for Inflatable Penile Implants (SSIPI) is a recently validated 16-question survey used to quantitatively measure patient outcomes after penile prosthetic surgery. The survey consists of four subdomain scores representing the appearance, pain, satisfaction, and function of the penile implant. Since its validation, there have been limited studies examining its use in patients with Peyronie’s disease (PD). Objective The objective of this study is to explore the use of the SSIPI in men with Peyronie’s Disease and to determine if any factors predict patient-perceived postoperative outcomes. Methods Preoperative, intraoperative, and postoperative data for patients with PD undergoing implantation of an inflatable penile prosthesis (IPP) by a single surgeon at a tertiary care institution was retrospectively collected into a secure database. Only those patients with at least one completed SSIPI postoperatively were included in the analysis. For those patients with multiple completed SSIPI’s, only the most recently completed one was used. Linear regression and Fischer’s exact test were used to determine if there were any significant relationships between variables and the SSIPI scores. Results There were a total of 44 men with PD who underwent surgery for an IPP. 31 of them were Caucasian, 11 were African-American, and one each were Hispanic and American Indian/Native American. Median time between surgery and completion of the SSIPI at a follow-up visit was 291 days (IQR 107.75-403.50). 35 men received a Coloplast Titan and 9 received an AMS 700 CX. There was no significant difference in the overall SSIPI score and any of the subdomains between men who received different types of implants or between men of different races. All four subdomains were significantly negatively correlated with the residual curvature which was measured intraoperatively after the IPP was implanted and any adjunctive straightening techniques were used (Fig 1a-d). The appearance domain was also significantly negatively correlated with the preoperative total testosterone indicating that men with a lower total testosterone level were more satisfied with the appearance of their penis. (p < 0.05, R2 = 0.32). Interestingly, those men who underwent manual modeling (n=36) had a significantly lower score in the appearance domain (p < 0.05). Conclusions Post-operative residual curvature in men with PD undergoing implantation of an IPP is a predictor of satisfaction with the IPP. The role of preoperative testosterone and intraoperative manual modeling in how men with PD perceive the appearance of their penis postoperatively requires further investigation. Abstract:Introduction Patient medical and surgical history factors, such as prior prostatectomy, may lead surgeons to opt for ectopic reservoir placement rather than the standard reservoir location in the retropubic space (RPS) during inflatable penile prosthesis (IPP) placement. Objective To examine the safety and effectiveness of ectopic reservoir placement used with three-piece IPPs in relation to reservoir placement in the RPS. Methods A systematic review of MEDLINE/Pubmed and Embase databases was performed for literature between 1970 and 2022. Clinical studies and case reports describing three-piece IPP reservoir placement and clinical outcomes on AMS 700, similar products (such as Coloplast Titan), and three-piece IPPs where the manufacturer is not specified were included. Results Seventy articles were identified that reported clinical outcomes on three-piece IPP reservoir placement, which included data on 9565 patients. Of these, 67% of the reservoirs (n = 6413) were placed in ectopic locations. These locations were defined as submuscular (n = 5207), retroperitoneal (n = 405), sub-external oblique (n = 50), peritoneal (n = 42), subcutaneous (n = 10), and did not specify the ectopic location (n = 694). A total of 670 patients had ectopic placement of the AMS 700 reservoirs specifically. Overall, there were no elevated rates in safety outcomes between RPS and ectopic placement. Fourteen studies directly compared safety and/or efficacy outcomes between RPS and ectopic placement and did not report any significant differences between patient groups. Conclusions Ectopic reservoir placement of three-piece IPPs, including AMS 700, is comparable in terms of safety, efficacy, and patient satisfaction to RPS reservoir placement. Ectopic reservoir placement of the AMS 700 device is also similarly comparable to ectopically placed reservoirs of other IPPs as reported in the literature. Surgeons should consider ectopic implantation for patients at higher risk of complications associated with reservoir placement into the RPS.

Abstract:Penile prosthesis (PP) is the mainstay of treatment in Peyronie's disease (PD) with co-existent refractory erectile dysfunction (ED). This study aimed to assess the clinical outcomes of patients who underwent PP as the first-line surgical treatment in PD without ED. A total of 636 patients underwent PP for PD from 2012 to 2022, but only 168 patients who underwent PP as first-line surgical management for PD with or without ED were included in the study. The mean (SD) age of 168 patients was 56.3 years (12.4). The mean curvature of the "PD with ED" group and the "PD without ED" group were 38.2 (5.6) degrees and 42.2 (5.9) degrees. The median (IQR) follow-up was 56.0 months (34.5– 61.4). Most (86.9%) patients underwent 3-piece inflatable PP. An important finding is that 33 patients (19.6%) without ED had undergone PP. Mechanical failure requiring revision surgery was less common in the 'without ED' group than in the ED group (6.8% vs. 10.2%, p 0.04). Most PD patients without ED (87.9%, 29/33) and with ED (88.9%, 120/135) were "satisfied" after PP implantation at six months, as defined by a score of ≥4 on a 5-point Likert scale. If surgery is offered in PD, PP may be considered a safe and effective first-line surgical treatment regardless of the ED, given the acceptable complications and high satisfaction rates. However, this new concept warrants further research.
