VARIABILITY OF TOTAL IGE CORRELATES WITH THE ANNUAL SEVERE ASTHMA EXACERBATIONS: A PROSPECTIVE COHORT STUDY IN REAL‐WORLD SETTING

Abstract:characteristics of adults with exacerbation prone asthma (EPA) in Chinese population. Methods. This study based on the Australasian Severe Asthma Network (ASAN) which provided requirements of data collection. Baseline data of eligible adults with asthma (n=546) were analyzed. Patients were divided into three groups by the number of exacerbations in the past year as none, few (1 or 2), or exacerbation prone (≥3). Multivariable analysis was performed by using univariable negative binomial regression models in which the frequency of exacerbations outcome was examined as a discrete count. Results. The EPA group had more controller therapies but worst ACT scores and AQLQ (all P < 0.05). Further, the EPA group also had a greater proportion of subjects with comorbidities such as COPD, bronchiectasis and anxiety. A more proportion of participants with EPA were symptomtriggered by upper respiratory tract infection and food. Those with EPA were featured by reduced lung function at baseline and increased blood eosinophils. In multivariable models, blood and sputum eosinophils, bronchodilator responsiveness were positively associated with exacerbation frequency even after adjustment for age and sex (RR [95% CI], 2.38 [1.24–4.60] for every unit of blood eosinophils, 1.58 [1.22–2.05] for every log unit of sputum eosinophils and 1.15 [1.05–1.27] for every 10% increase in bronchodilator responsiveness). Baseline FEV1 was positively associated with exacerbation frequency (0.88 [0.83–0.94] for every 10% increase). COPD (2.36 [1.39–3.99]), bronchiectasis (3.51 [1.94–6.38]) and anxiety (3.31 [1.05–5.05]) were associated with exacerbation frequency. Upper respiratory tract infection and food were associated with exacerbation frequency (1.98 [1.33–2.96] and 1.84 [1.28–2.65]). Conclusion. Therapies aiming at modifying these treatable traits are likely to reduce morbidities and medical burden in these patients.
