Screening and follow-up of the indeterminate samples by HIV antibody confirmation test

Abstract:Objective To analyze the follow-up results and the screening test characteristics of indeterminate samples from Western blotting (WB) test, and to explore the possibility of WB indetermination and the times of screening positive reaction in predicting HIV infection. Methods A total of 209 samples with indeterminate WB test results for the first time were collected from Hangzhou HIV confirmatory laboratory from 2015 to 2017, and 113 subjects were followed up. The follow-up results of samples with different bands and screening results were observed to analyze the first WB results among different population, different age groups and different screening methods. Results Of the 113 subjects who were successfully followed up, 83 subjects confirmed positive for HIV-1 antibody and 30 subjects showed no progress. WB uncertain results were most common with p24+gp160, accounting for 64.60% (73/113) . Four screening methods were used to detect the number of responses and the follow-up results were statistically significant (revise χ2=21.23, P 0.01) . Conclusions The analysis on HIV WB indetermination result can be judged by the number of bands and the positive reaction of antigen. Therefore, two or more screening methods should be used in conditional laboratories, and preferably a 4-generation reagent should be used to improve the detection of early infection samples.
