Abstract:Abstract Introduction Clomiphene, and its trans isomer enclomiphene, have been used off-label in recent years for hypogonadal men seeking to maintain fertility. Both drugs have become increasingly widespread within andrology clinics; thus, understanding the comparative benefits and risks of these medications are crucial for optimizing treatment decisions and providing improved therapeutic options. Objective To investigate the longitudinal benefits and risks associated with enclomiphene compared to clomiphene to provide valuable insights for clinicians when making treatment decisions in the management of hypogonadism and male fertility preservation. Methods We retrospectively studied patients who had been prescribed clomiphene, and later enclomiphene, at a university hospital within the observation range of 1/1/2021-12/31/2022, encompassing 66 patients. All included patients were prescribed clomiphene prior to switching to enclomiphene. Baseline laboratory values for each patient prior to being prescribed clomiphene were documented, followed by subsequent values for each of these variables in the most recent visit prior to stopping clomiphene as well as any noted adverse effects experienced during this time. Laboratory values and adverse effects were noted for the same patients in their most recent visit while taking enclomiphene. Adverse events were defined by 1) depressive thoughts; 2) weak muscle strength; 3) gynecomastia; 4) mood changes; or 5) agitation as well as changes in estradiol and hematocrit laboratory values. Two-tailed T-Tests were employed using SPSS to analyze the longitudinal impacts of clomiphene and enclomiphene on serum hormone values. Results In our cohort, men on clomiphene experienced any adverse events 18.18% of the time, whereas men on enclomiphene experienced any adverse events 3.45% of the time. From our data, men on enclomiphene had statistically lower rates of documented adverse events than the same men on clomiphene, indicating no increased risk on the newer drug; neither treatment yielded any serious adverse events. In looking specifically at hormone-related adverse events, estradiol increased significantly on clomiphene but not on enclomiphene (17.25 versus -0.29 pg/mL; t=4.271, p=.00005). However, there was no difference in change in hematocrit between the two medications (0.10% versus 0.79% increase; t=-1.103, p=.314). On average, testosterone increased 149.33 ng/dL from their baseline scores while on clomiphene and increased 108.55 ng/dL from their clomiphene values while on enclomiphene. When comparing the improvement of testosterone values attributed to clomiphene versus enclomiphene, the change of testosterone levels over time attributed to either medication are not statistically different (t-value = .883; p-value = .379). Lastly, men stayed on clomiphene for an average of 32.92 months before switching to enclomiphene. Conclusions Our findings show that enclomiphene provides the same magnitude of improvement in testosterone but through safer means, as noted by its lower rate of documented adverse events. These findings support the use of enclomiphene as a comparable treatment option for hypogonadal men while minimizing the risk of adverse effects. Further research and larger studies are warranted to validate these conclusions and explore additional long-term effects of enclomiphene. Abstract:Objective: The aim was to assess the efficacy of post-operative adjuvant treatment with goserelin acetate for reducing the recurrence of pelvic symptoms and total recurrence rates in Chinese patients with moderate to severe endometriosis. Methods: This largest non-interventional, observational study enrolled 426 Chinese patients with moderate to severe endometriosis across 15 centers in China. Goserelin acetate depot (3.6 mg) was prescribed as adjuvant post-operative treatment, subcutaneously every 4 weeks till 18 months. The primary efficacy end-point was improvement of pelvic symptom recurrence rate and total recurrence rates (including pelvic symptom and physical findings) up to 18 months post-surgery. Secondary endpoints were pregnancy rate among infertile subjects receiving post-operative goserelin treatment 18 months following the operation; and information on goserelin administration. Results: The total recurrence rate (8.2%; 95% confidence interval [CI], 5.8-11.3%) and symptom recurrence rate (5.6%; 95% CI, 3.6-8.2%) in Chinese patients with moderate to severe endometriosis was comparatively lower than the previously reported studies. Less than 28% of the patients required add-back therapy to offset the effects of gonadotropin-releasing hormone (GnRH), and indicated that the GnRH agonist was efficacious in the majority of the patients. No serious adverse events were reported during the study. Conclusion: Goserelin acetate was effective in reducing symptom and total recurrence rate when used post-operatively in Chinese patients with mild to severe endometriosis. The authors therefore offered goserelin acetate as a viable treatment for Chinese patients with moderate to severe endometriosis. Clomiphene citrate for men with hypogonadism: a systematic review and meta‐analysis

Abstract:BACKGROUND: Male hypogonadism is a clinical and biochemical androgen insufficiency syndrome, becoming more prevalent with age. Exogenous testosterone is first-choice therapy, with several side effects, including negative feedback of the hypothalamic-pituitary-gonadal axis, resulting in suppression of intratesticular testosterone production and spermatogenesis. To preserve these testicular functions while treating male hypogonadism, clomiphene citrate is used as off-label therapy. This systematic review and meta-analysis aimed to evaluate the effectiveness and safety of clomiphene citrate therapy for men with hypogonadism.METHODS: The EMBASE, PubMed, Cochrane databases were searched in May 2021, for effectiveness studies of men with hypogonadism treated with clomiphene citrate. Both intervention and observational studies were included. The Effective Public Health Practice Project Quality Assessment Tool, a validated instrument, was used to assess methodological study quality. The primary outcome measure was the evaluation of serum hormone concentration. Secondary outcomes were symptoms of hypogonadism, metabolic and lipid profile, side effects, safety aspects.RESULTS: We included 19 studies, comprising four randomized controlled trials and 15 observational studies, resulting in 1642 patients. Seventeen studies were included in the meta-analysis, with a total of 1279 patients. Therapy and follow-up duration varied between one and a half and 52 months. Total testosterone increased with 2.60 (95% CI 1.82-3.38) during clomiphene citrate treatment. An increase was also seen in free testosterone, luteinizing hormone, follicle stimulating hormone, sex hormone-binding globulin and estradiol. Different symptom scoring methods were used in the included studies. The most frequently used instrument was the Androgen Deficiency in Aging Males questionnaire, whose improved during treatment. Reported side effects were only prevalent in less than 10% of the study populations and no serious adverse events were reported.CONCLUSION: Clomiphene citrate is an effective therapy for improving both biochemical as well as clinical symptoms of males suffering from hypogonadism. Clomiphene citrate has few reported side effects and good safety aspects.

Abstract:Male hypogonadism, which is characterized by low testosterone levels, has a significant impact on male sexual function, overall health, and fertility. Testosterone replacement therapy (TRT) is the conventional treatment for this condition, but it has potential adverse effects and is not suitable for men seeking to conceive. Testosterone plays an essential role in male sexual function, metabolism, mood, and overall well-being. Clomiphene citrate, a drug originally developed for female infertility, has recently gained attention as an off-label treatment for male hypogonadism. By blocking the negative feedback of estrogen on the hypothalamus and pituitary glands, clomiphene stimulates gonadotropin secretion, leading to increased endogenous testosterone production, which, in turn, improves sperm parameters and fertility and alleviates the symptoms of hypogonadism. Regarding the safety profile of clomiphene compared with TRT, clomiphene appears to confer a lower risk than TRT, which is associated with adverse effects such as polycythemia. Furthermore, combination therapy with clomiphene and anastrozole or human chorionic gonadotropin has been investigated as a potential approach to enhancing the effectiveness of treatments for improving hypogonadism symptoms. In conclusion, clomiphene citrate may offer a promising alternative to TRT for men with hypogonadism, particularly those desiring fertility preservations. However, its long-term efficacy and safety remain inadequately understood. Future research should focus on exploring the benefits of combination therapies and personalized treatment strategies based on individual patient characteristics.

Abstract:OBJECTIVES: To determine the effects of daily oral doses of enclomiphene citrate compared with topical testosterone gel treatment on serum total testosterone (TT), luteinising hormone (LH), follicle-stimulating hormone (FSH), and sperm counts in men with secondary hypogonadism.PATIENTS AND METHODS: Two parallel randomised, double-blind, double-dummy, placebo-controlled, multicentre, phase III studies were undertaken to evaluate two doses of enclomiphene citrate vs testosterone gel (AndroGel(®) 1.62%) on TT, LH, FSH, and sperm counts in overweight men aged 18-60 years with secondary hypogonadism. Men were screened and enrolled in the trials (ZA-304 and ZA-305). All enrolled men had early morning serum TT levels in the low or low normal range (≤300 ng/dL; ≤10.4 nmol/L) and had low or normal LH (<9.4 IU/L) levels measured on two separate occasions 2-10 days apart. Serum samples were obtained over the course of the study to determine relevant hormone levels at baseline and after 16 weeks of treatment. Men provided semen samples twice to enroll at the beginning and twice at the end of the study.RESULTS: TT levels increased between baseline and after 16 weeks of treatment in all the treatment groups. FSH and LH levels increased in the enclomiphene citrate groups and decreased in the testosterone gel group at 16 weeks. Enclomiphene citrate maintained sperm concentration in the normal range over the treatment period, while there was a marked reduction in spermatogenesis in the testosterone gel group.CONCLUSIONS: Enclomiphene citrate consistently increased serum TT, LH and FSH, restoring normal levels of serum TT. Enclomiphene citrate treatment maintained sperm concentrations in the normal range. The effects on TT were also seen with testosterone replacement via testosterone gel but sperm counts were not maintained.

Abstract:Objectives It is already a common clinical practice to not use hypothalamic estrogen receptor antagonists and aromatase inhibitors in hypergonadotropic hypogonadism, as gonadotropins will rise. From clinical observation, normo-gonadotropic hypogonadal males may have elevated gonadotropin levels on hypothalamic estrogen receptor antagonists. The purpose of this study is to evaluate erectile function, hormonal and semen parameters changes in clomiphene-induced high LH and FSH levels. Methods A retrospective study was done on 83 normo-gonadotropic hypogonadal males with symptoms of ED and infertility, mean age 35,56±9,09 years, to whom treatment with Clomiphene citrate was recommended, and at follow-up visits, elevated gonadotropins were found. Pre-treatment and on treatment LH (mIU/mL), FSH (mIU/mL), total T (ng/dL), E (pg/mL), and semen parameters were compared. IIEF-5 scoring was used for ED evaluation. Results Accordingly, with IIEF-5 scoring, 17.5% reported severe ED, 31.3% with moderate, 21% with mild-moderate, and 31.2% with mild ED. Before treatment hormones and semen mean values: LH (4.04±3.62), FSH (5.43±4.62), total T (208±87.9), E (27.4±15.22), sperm concentration (12.45±8.61), PR motility (27.56±18.13). On treatment 28 (33.73%) male were found with LH only high level with mean value 13.54±6.48. 55 (66.27%) male with Clomiphene induced hypergonadotropic (LH and FSH) hypogonadism was reported with semen and hormonal mean values: LH (12.04±4.98), FSH (15.43±5.62), total T (475.8±244.9), E (48.4±19.22), sperm concentration (18.15±12.87), PR motility (36.56±13.47). In 13 (15.66%) induced high LH and FSH males, semen parameters got worse, with mean values: sperm concentration (7.63±4.91) and PR motility (22.21±7.63). Conclusions Follow-up hormonal evaluation during hypothalamic estrogen receptor antagonist treatment should be mandatory, as elevated gonadotropin levels may have a negative impact on semen parameters, gynecomastia, and overall testicular function. Complete reversal of adult-onset isolated hypogonadotropic hypogonadism with clomiphene citrate

Abstract:Objective: Inhibition of pituitary gonadotropin secretion in men by T is principally mediated by aromatization to estrogen (E), which inhibits hypothalamic secretion of GnRH. We hypothesized that adult-onset isolated hypogonadotropic hypogonadism (IHH) might result from an altered central set-point for E-mediated negative feedback. Design and setting: Longitudinal clinical investigation unit-based evaluation of the clinical and biochemical response to E-receptor blockade. Patient(s): A 31-year-old man presenting with an 18-month history of sexual dysfunction resulting from severe adult-onset IHH (LH 1.7 U/L, FSH 2.0 U/L, T 3.5 nmol/L). Intervention(s): Initial therapy with 50 mg of clomiphene citrate (CC) three times a day for 7 days, with overnight LH pulse profiling and 9 am T levels evaluated at baseline and on completion. A 2-month washout period, followed by low-dose maintenance therapy (25-50 mg/d) for 4 months. Main outcome measure(s): Baseline and stimulated T levels and LH pulsatility; effect on sexual function. Result(s): Clomiphene therapy resulted in complete normalization of pulsatile gonadotropin secretion, serum T level, and sexual function. Conclusion(s): Isolated hypogonadotropic hypogonadism may result from an acquired defect of enhanced hypothalamic sensitivity to E-mediated negative feedback. Whereas direct T replacement therapy can further suppress endogenous gonadotropin secretion, treating IHH men with gonadotropins can stimulate endogenous T secretion and enhance fertility potential. On theoretical grounds, reversal of gonadotropin deficiency with CC might be expected to have a similar biological effect.

Abstract:Many male patients are discovered on screening to suffer from hypogonadism and age related hypogonadism is being increasingly recognized. However, secondary causes of hypogonadism should not be overlooked, especially in patients who may have concomitant morbidity as highlighted in this case. Our patient with vascular hypogonadism was treated with testosterone and clomiphene citrate in cycles; with a hope of improving not only androgen levels but overall pituitary function as there were co-existing endocrine pathologies of albeit primary hypothyroidism and low IGF-1 levels. Treatment with exogenous testosterone is fairly well established; but there is also increasing evidence of the effectiveness and short-term safety of clomiphene citrate in restoring not only biological levels but functional states in males as well. As such, we report an unusual case of a patient seen at our Men's Health & Andrology clinic in which both the cause of some otherwise unremarkable symptoms and the treatment, using a combination of clomiphene citrate and testosterone, were remarkable.

Abstract:<h3 class="article-section__sub-title section1"> Background</h3><p>Clomiphene citrate (CC) has been shown to restore the hypothalamic pituitary‐gonadal (HPG) axis by increasing testosterone (T) levels to physiological levels in patients with dysmetabolic conditions such as obesity, metabolic syndrome and type 2 diabetes mellitus (T2DM). However, the data are unclear regarding the effects on Sertoli cell (SC) function.</p><h3 class="article-section__sub-title section1"> Aim</h3><p>To study SC function by assessing Inhibin B (IB) and Anti‐Mullerian Hormone (AMH) levels at baseline and after 3 month of CC treatment.</p><h3 class="article-section__sub-title section1"> Materials and methods</h3><p>This is an ancillary study of a cross‐over, randomised, double‐blind, placebo‐controlled trial performed to evaluate androgen response to CC treatment in dysmetabolic obese subjects with low T levels treated with metformin. We evaluated SC function by assessing IB and AMH levels at baseline and after three months of each treatment in ten dysmetabolic obese subjects with low T levels. In all subjects,the influence of the clinical characteristics, metabolic and hormonal baseline parameters on SC and Leydig (LC) function, evaluated respectively with AMH, IB, FSH and T levels, were tested.</p><h3 class="article-section__sub-title section1"> Results</h3><p>No significant changes were observed for IB and AMH concentrations after each treatment period. Whereas T and estradiol (E2) levels were shown to be significantly higher in the CC plus metformin phase (CC/Met) only. No clinical, metabolic or hormonal parameters showed significant effects on serum AMH at baseline or after treatments. However, baseline T, dihydrotestosterone (DHT) andE2 positively affected IB levels during CC/Met therapy (P=0.003, P=0.038 and P=0.049 respectively). Baseline leptin, and FSH had a negative (P=0,031) and positive (P=0.048) respectively role on T levels during CC/Met, as they were statistically significant compared to the placebo period (Plac/Met).</p><h3 class="article-section__sub-title section1"> Conclusion</h3><p>Unlike the LC activity, CC was unable to influence SC function, as shown by the lack of IB and AMH serum modifications, thus suggesting an intrinsic non‐reversible defect of SC cells in patients with dysmetabolic conditions.</p>

(253) Comparative Effectiveness in Testosterone Levels and Sperm Parameters of Medical Treatments for Primary Infertility and Testicular Deficiency

Abstract:Introduction The goal of treatment for infertile men with testosterone deficiency is to improve intratesticular androgen levels with the aim of increasing the last step of maturation in spermatogenesis. Medical therapy for male factor infertility remains largely empiric and relies on off-label use of medications known to increase intratesticular testosterone levels, such as selective estrogen receptor modulators (clomiphene citrate and enclomiphene citrate) and gonadotropins (human chorionic gonadotropin). There is scarce data of whether the above-mentioned treatment options offer different success rates in improving testosterone levels and sperm parameters. Objective To compare the improvement in total testosterone, intratesticular testosterone, and sperm parameters between patients receiving medical treatment with clomiphene citrate (CC), enclomiphene citrate (ECC), or clomiphene citrate + human chorionic gonadotropin (HCG). Methods We conducted a retrospective study from 2018 to 2022 of men with primary infertility, testicular deficiency, and secondary hypogonadism receiving ECC, CC or CC+HCG for at least 3 months. Among the different treatment groups, we assessed baseline and follow-up total testosterone, 17-hydroxyprogesterone (17-OH), semen parameter(s), and demographics. Semen quality improvement was based on assisted reproduction eligibility according to the total motile sperm count (TMSC) value: in-vitro fertilization ( 9 million). Intratesticular testosterone upgrading was based on 17-OH level, with expected values being ≥55 ng/dL. Variables were compared using paired t-test and ANOVA. Results A total of 18 men received ECC, 19 received CC and 9 received CC+HCG. Mean age was 41.1±2.28 years in the ECC group, 40.62±1.17 in the CC group, and 45±2.75 in the CC+HCG group (p=0.458). Overall, the mean follow-up was 3.64 ±2.14 months (p=0.242). The 3 treatment groups exhibited a statistically significant increase in total testosterone (table 1), but only the CC and CC+HCG groups had a statistically significant improvement in the level of 17-OH (p 9 million. Conclusions Our study showed that ECC, CC and CC+HCG are effective treatment regimens for improving total testosterone without negatively affecting spermatogenesis. Nonetheless, their effects on intratesticular testosterone and sperm parameters varied, suggesting that choosing the best treatment option for improving sperm parameters should be an individualized patient decision. Abstract:Male hypogonadism is a clinical syndrome that results in low testosterone levels and frequently leads to infertility. The syndrome occurs due to disruption at one or more levels of the hypothalamic-pituitary-gonadal axis. Testosterone replacement therapy (TRT) is the most common treatment utilised for male hypogonadism. However, long-acting forms of TRT leads to infertility and so is inappropriate for patients wishing to conceive. For patients who wish to remain fertile, nasal TRT, clomiphene citrate, exogenous gonadotropins, gonadotropin releasing hormone and aromatase inhibitors have been used as alternative treatment options with different degrees of success. A review of the literature was performed to identify the safety and efficacy of alternative treatment options. Gonadotropin releasing hormone can successfully induce spermatogenesis but is impractical to administer. Likewise, aromatase inhibitors have limited use due to inducing osteopenia. Nasal TRT may be a good treatment option for these patients, but its efficacy has so far only been demonstrated in small sample sizes. However, clomiphene citrate and exogenous gonadotropins are safe, offer good symptom control and can successfully induce fertility in hypogonadism patients.

Abstract:Background. Infertility in men is a common and worldwide problem with limited registered treatment options and licensed therapies. Clomiphene citrate (CC) is an empirical off-label therapy. CC is a selective estrogen receptor modulator that may improve testicular function, including sperm quality, by stimulating steroidogenesis and spermatogenesis. However, there is a lack of evidence regarding the efficacy of CC as therapy for male infertility. Objectives. To assess the improvement of CC on sperm parameters, pregnancy, hypogonadism, and measurement of safety. Methods. In this retrospective study, 52 men treated with CC for idiopathic nonobstructive infertility were analyzed. Data of sperm parameters, pregnancy rate, hormonal values, side effects, improvement in hypogonadal symptoms, and potential predictors of treatment response were collected. Results. A total of 52 infertile men underwent CC treatment. An increase in sperm concentration was observed in 19 patients (37%). An upgrade in sperm concentration classification according to the WHO criteria was noted in 21% of the patients receiving CC therapy. Additionally, 15% of initially ineligible patients for intrauterine insemination became eligible during CC. In total, 33% of couples achieved pregnancy, either spontaneously or with assisted reproductive techniques (ART). Low-normal follicle-stimulating hormone and larger testis volume before CC treatment were predictors for better semen concentration during treatment. The retrieval rate was 75% for azoospermic patients who underwent testicular sperm extraction or percutaneous epididymal sperm aspiration, enough for intracytoplasmic sperm injection. With CC, testosterone levels increased and hypogonadal symptoms improved. Mild side effects were reported by 8% of patients. Conclusion. The results of this study contribute to the existing evidence that CC is a safe and noninvasive therapy for idiopathic infertile males. It could be considered as a step-up therapy before initiating more invasive ART procedures.

Abstract:Men with macroprolactinoma can present persistent hypogonadism despite normoprolactinemia achieved with clinical and/or neurosurgical treatment. Usually, testosterone replacement therapy is indicated. Nevertheless, although off-label, clomiphene citrate (CC), a selective estrogen receptor modulator, has also been used, mainly when fertility is an issue. The aim of this study is to evaluate the effectiveness of CC in recovering the gonadal axis in men with macroprolactinoma, with or without hyperprolactinemia, and evaluate its safety as a long-term therapy.

Abstract:Abstract Introduction Depression of spermatogenesis is a significant concern for hypogonadal patients on testosterone therapy. Human chorionic gonadotropin (hCG) is commonly prescribed in these men to prevent testicular atrophy and to maintain some degree of spermatogenesis. Furthermore, hCG dosages are significantly increased when a pregnancy is desired. Historically, we have concomitantly used Clomid to stimulate FSH and testosterone production since both are needed for optimal spermatogenesis. However, with the availability of compounded FSH, we have introduced its usage instead of Clomid to evaluate if it yields superior quantitative sperm production. Objective To investigate which medication pairings yields the greatest improvement in hormone and semen parameters over time. Methods Including men with azoospermia or oligozoospermia who were prescribed hCG, we noted whether the men were additionally taking FSH or Clomid. The patient population of men in the hCG + FSH group had 32, and the hCG + Clomid group totaled 14. Baseline laboratory values were documented for testosterone and total sperm count for each patient prior to being prescribed their combined medications, followed by subsequent values for these variables in the months following this prescription. ANOVAs were employed using SPSS to analyze the change in the two assessed metrics across follow up visits. Results By analyzing increase in testosterone over time, the hCG + FSH cohort experienced the greatest improvement after four follow up visits (600.89 ng/dL after four visits, 150.22 (ng/dL)/visit), outperforming the hCG + Clomid group (505.63 ng/dL after four visits, 126.41 (ng/dL)/visit). The ANOVAs across the subsequent visits for each treatment group are as follows: hCG + FSH: f-ratio = 3.226, p-value = 0.0137; hCG + Clomid: f-ratio = 1.456, p-value = 0.219. Regarding total sperm count, the hCG + FSH group surpassed the HCG + Clomid group across the follow-up visits as well: hCG + FSH: 41.47 million after three follow-up visits; hCG + Clomid: 26.20 million after three follow-up visits. The ANOVAs across the subsequent visits for each treatment group are: hCG + FSH: f-ratio = 3.015, p-value = 0.0339; hCG + Clomid: f-ratio = 1.485, p-value = 0.234. Conclusions Those taking hCG with FSH experienced a greater improvement in both testosterone and total sperm counts compared to men on hCG and Clomid. It would seem that the use of FSH gives a more rapidly successful outcome for hypogonadal patients, since its combination with hCG acts directly to stimulate spermatogenesis, within the testes rather than requiring, as with Clomid, secondary stimulation through the pituitary gland's secretion of LH and FSH. With the relatively recent introduction of FSH as compounded biologics and its reduction in cost compared to brand FSH, the use of this trophic hormone can now be more widely used. Moreover, these data suggest that hCG with FSH results in a more rapid improvement in both spermatogenesis and serum testosterone than historically alternative therapies. Abstract:Background Existing literature does not provide accurate epidemiological data regarding the true prevalence of men with non‐obstructive azoospermia (NOA) who would be eligible for hormonal optimization therapy, according to specific pre‐treatment criteria. Objectives To investigate the characteristics of those men with NOA who would qualify for the medical therapy prior to any SR procedure in a large multi‐centric cross‐sectional study. Materials and methods Complete data from 1644 NOA patients seeking medical help for primary infertility at three tertiary referral centers from USA, Brazil, and Italy were analyzed. Baseline serum hormone levels were collected for all patients. NOA was confirmed after two consecutive semen analyses. Abstract:Background: Male factor infertility affect up to 50% of couples unable to conceive spontaneously. Several non-hormonal pharmacological treatments have been proposed to boost spermatogenesis and increase chances of conception in men with infertility. Still, no clear evidence exists on the most effective treatment strategy. Objective: We aimed to compare the effectiveness of non-hormonal pharmacological treatment options for men with infertility using a systematic review and network meta-analysis. Methods: We searched MEDLINE, EMBASE, and CENTRAL until October 2023 for randomised/quasi-randomised trials that evaluated any non-hormonal pharmacological treatment options for men with idiopathic semen abnormalities or those with hypogonadism. We performed pairwise and network meta-analyses using a random effect model. We assessed risk of bias, heterogeneity, and network inconsistency. We calculated the mean rank and the surface under the cumulative ranking curve (SUCRA) for each intervention the maximum likelihood to achieve each of reported outcomes. We reported primarily on sperm concentration and other important semen and biochemical outcomes using standardised mean difference (SMD) and 95% confidence-intervals(CI). Results: We included 14 randomised trials evaluating four treatments (Clomiphene citrate, Tamoxifen, Aromatase inhibitors, anti-oxidants) and their combinations in 1342 men. The overall quality of included trials was low. Sperm concentration improved with clomiphene compared to anti-oxidants (SMD 2.15, 95%CI 0.78-3.52), aromatase inhibitor (SMD 2.93, 95%CI 1.23-4.62), tamoxifen (SMD - 1.96, 95%CI -3.57; -0.36) but not compared to placebo (SMD - 1.53, 95%CI -3.52- 0.47). Clomiphene had the highest likelihood to achieve the maximum change in sperm concentration (SUCRA 97.4). All treatments showed similar effect for sperm motility, semen volume, and normal sperm morphology. FSH levels showed significant improvement with clomiphene vs.anti-oxidant (SMD 1.48, 95%CI 0.44-2.51) but not compared to placebo. The evidence networks for LH and testosterone suffered from significant inconsistency (p = 0.01) with similar trend of improvement with clomiphene compared to other treatments but not compared to placebo. Conclusion: There is insufficient evidence to support the routine use of Clomiphene, tamoxifen, and aromatase inhibitors to optimise semen parameters in men with infertility. Future randomised trials are needed to confirm the efficacy of clomiphene in improving fertility outcomes in men. Prospero: CRD42023430179.
Recovery of persistent hypogonadism by clomiphene in males with prolactinomas under dopamine agonist treatment

Abstract:Context: Persistence of hypogonadism is common in male patients with prolactinomas under dopamine agonist (DA) treatment. Conventional therapy with testosterone causes undesirable fluctuations in serum testosterone levels and inhibition of spermatogenesis. Objective: To evaluate the use of clomiphene as a treatment for persistent hypogonadism in males with prolactinomas. Design: Open label, single-arm, prospective trial. Patients: Fourteen adult hypogonadal males (testosterone <300 ng/dl and low/normal LH) with prolactinomas on DA, including seven with high prolactin (range: 29-1255 microg/l; median: 101 microg/l) despite maximal doses of DA. Intervention: Clomiphene (50 mg/day orally) for 12 weeks. Measures: Testosterone, estradiol, LH, FSH, and prolactin were measured before and 10 days, 4, 8, and 12 weeks after clomiphene. Erectile function, sperm analysis, body composition, and metabolic profiles were evaluated before and after clomiphene. Results: Ten patients (71%), five hyperprolactinemic and two normoprolactinemic, responded to clomiphene (testosterone >300 ng/dl). Testosterone levels increased from 201+/-22 to 457+/-37 ng/dl, 436+/-52, and 440+/-47 ng/dl at 4, 8, and 12 weeks respectively (0.001<P<0.01). Estradiol increased significantly and peaked at 12 weeks. LH increased from 1.7+/-0.4 to 6.2+/-2.0 IU/l, 4.5+/-0.7, and 4.6+/-0.7 IU/l at 4, 8, and 12 weeks respectively (0.001<P<0.05). FSH levels increased in a similar fashion. Prolactin levels remained unchanged. Erectile function improved (P<0.05) and sperm motility increased (P<0.05) in all six patients with asthenospermia before clomiphene. Conclusions: Clomiphene restores normal testosterone levels and improves sperm motility in most male patients with prolactinomas and persistent hypogonadism under DA therapy. Recovery of gonadal function by clomiphene is independent of prolactin levels.

Abstract:Introduction Clomiphene is a selective estrogen receptor modulator used to induce ovulation by increasing luteinizing (LH) and follicle-stimulating hormone (FSH) levels. Since the 1970's, it has been used in males with hypogonadism as an alternative to testosterone therapy as it does not suppress the hypothalamic-pituitary-gonadal axis. Objective While there have been systematic reviews looking at the effectiveness of clomiphene, we sought to better characterize the average testosterone levels, as well as the rate of deep venous thrombosis (DVT), myocardial infarction (MI), stroke, and polycythemia in clomiphene vs. testosterone users. Methods We performed a retrospective chart review for 537 men with a hypogonadism diagnosis that were seen by two urology providers at our center. Of these 537 patients, 84 presented with prior use or were prescribed clomiphene. We measured the incidence of DVT, MI, stroke, polycythemia and average testosterone levels with clomiphene. Statistical analysis was conducted using both a beta binomial and linear model. Results Patients receiving testosterone (n=217) versus clomiphene (n=84) demonstrated a 99% chance of higher mean levels of testosterone (Difference=216.55, 95% CrI = 20.67-408.16). Among those taking testosterone or clomiphene at the final visit (N=237; testosterone n=185; clomiphene=52) adverse events included, polycythemia (n=28), one DVT, VTE's (n=4), MI's (n=4), and strokes (n=4). The absolute risk (AR) of experiencing at least one of these events (i.e. polycythemia, DVT, VTE, MI or stroke) was 13.8% (95% CrI = 10.1-17.9%) and 3.1% (95% CrI = 0.44-9.4%) for testosterone and clomiphene respectively (Absolute Risk Difference (ARD) = 10.5% (95% CrI = 3.4-15.7%); Posterior Probability (ARD>0) > 0.99. The AR of polycythemia was 12.6% (95% CrI = 8.9-16.7%) and 1.5% (95% CrI = 0.1-7.4%) for testosterone and clomiphene respectively (ARD = 10.7% (95% CrI = 4.3-15.6%); Posterior Probability (ARD > 0) > 0.99. The AR of stroke was 0.94% (95% CrI = 0.14-2.98%) and 1.56% (95% CrI = 0.1-7.4%) for testosterone and clomiphene respectively (ARD = -0.6% (95% CrI = -6.5-2.1%); Posterior Probability (ARD 0) = 0.96. The AR of DVT was 0.37% (95% CrI = 0.01-1.99%) and <0.00001% (95% CrI = 0) = 0.88. Conclusions The absolute risk of experiencing at least one of the events was found to be significantly higher in patients receiving testosterone compared to those receiving clomiphene. The results should be interpreted with caution, given the broad credible intervals due to its small size.
