Abstract:Almost all cases of cervical cancer are caused by the human papilloma virus (HPV). Detection of pre-cancerous cervical changes provides a window of opportunity for cure of an otherwise lethal disease when metastatic. With a greater understanding of the biology and natural course of high-risk HPV infections, screening methods have shifted beyond subjective Pap smears toward more sophisticated and objective tactics. This has led to a substantial growth in the breadth and depth of HPV-based cervical cancer screening tests, especially in developed countries without constrained resources. Many low- and middle-income countries (LMICs) have less access to advanced laboratories and healthcare resources, so new point-of-care (POC) technologies have been developed to provide test results in real time, improve the efficiency of techniques, and increase screening adoption. In this Review, we will discuss how novel decentralized screening technologies and computational strategies improve upon traditional methods and how their realized promise could further democratize cervical cancer screening and promote greater disease prevention.

Predicting breast cancer with AI for individual risk-adjusted MRI screening and early detection

Abstract:Women with an increased life-time risk of breast cancer undergo supplemental annual screening MRI. We propose to predict the risk of developing breast cancer within one year based on the current MRI, with the objective of reducing screening burden and facilitating early detection. An AI algorithm was developed on 53,858 breasts from 12,694 patients who underwent screening or diagnostic MRI and accrued over 12 years, with 2,331 confirmed cancers. A first U-Net was trained to segment lesions and identify regions of concern. A second convolutional network was trained to detect malignant cancer using features extracted by the U-Net. This network was then fine-tuned to estimate the risk of developing cancer within a year in cases that radiologists considered normal or likely benign. Risk predictions from this AI were evaluated with a retrospective analysis of 9,183 breasts from a high-risk screening cohort, which were not used for training. Statistical analysis focused on the tradeoff between number of omitted exams versus negative predictive value, and number of potential early detections versus positive predictive value. The AI algorithm identified regions of concern that coincided with future tumors in 52% of screen-detected cancers. Upon directed review, a radiologist found that 71.3% of cancers had a visible correlate on the MRI prior to diagnosis, 65% of these correlates were identified by the AI model. Reevaluating these regions in 10% of all cases with higher AI-predicted risk could have resulted in up to 33% early detections by a radiologist. Additionally, screening burden could have been reduced in 16% of lower-risk cases by recommending a later follow-up without compromising current interval cancer rate. With increasing datasets and improving image quality we expect this new AI-aided, adaptive screening to meaningfully reduce screening burden and improve early detection.

Abstract:Early screening for cancer has proven to improve the survival rate and spare patients from intensive and costly treatments due to late diagnosis. Cancer screening in the healthy population involves an initial risk stratification step to determine the screening method and frequency, primarily to optimize resource allocation by targeting screening towards individuals who draw most benefit. For most screening programs, age and clinical risk factors such as family history are part of the initial risk stratification algorithm. In this paper, we focus on developing a blood marker-based risk stratification approach, which could be used to identify patients with elevated cancer risk to be encouraged for taking a diagnostic test or participate in a screening program. We demonstrate that the combination of simple, widely available blood tests, such as complete blood count and complete metabolic panel, could potentially be used to identify patients at risk for colorectal, liver, and lung cancers with areas under the ROC curve of 0.76, 0.85, 0.78, respectively. Furthermore, we hypothesize that such an approach could not only be used as pre-screening risk assessment for individuals but also as population health management tool, for example to better interrogate the cancer risk in certain sub-populations.

Current Problems in Cancer

Abstract:The early detection of cancer is a key goal of the National Cancer Plan formally released by the National Institutes of Health's (NIH) National Cancer Institute (NCI) in April 2023. To support this effort, many laboratories and vendors are developing multi-cancer detection (MCD) assays that interrogate blood and other bodily fluids for cancer-related biomarkers, most commonly circulating tumor DNA (ctDNA). While this approach holds promise for non-invasively detecting early signals of multiple different cancers and potentially reducing cancer-related mortality, there is a dearth of prospective clinical data to inform the deployment of MCD assays for cancer screening in the general adult population. In this review we highlight differing technologies that underpin various MCD assays in clinical development, the importance of achieving adequate performance specifications for MCD assays, ongoing clinical studies investigating the utility of MCD assays in cancer screening and detection, and efforts by the NCI's Division of Cancer Prevention (DCP) to establish a network infrastructure that has the capacity to comprehensively address the scientific and logistical challenges of evaluating blood-based MCD approaches and other cancer screening tools.

Abstract:Multi-cancer early detection (MCED) liquid biopsy screening tests provide great promise for both first-time cancer detection, and recurrence monitoring applications. The prominent feature of asymptomatic cancer screening solutions is their demand for ultra-high specificity (e.g., circa 99.99%), while maintaining a reasonable sensitivity. However, the challenges of screening are not limited to these technical specifications. Among other challenges, psychological and economical barriers-to-entry can be mentioned. In this work, we discuss the value of adding a high-sensitivity liquid-biopsy prescreening system to a screening solution. Such a tandem system can retain the high specificity offered by the screening solution, while relieving some of the economical concerns in cancer screening, by providing an opportunity to forgo screening in individuals with negative prescreening results. In order to serve in a general multi-cancer scheme, the prescreening solution must be MCED in nature. While our screening solution is primarily single-cell genomics/proteomics-based, we adopted a predominantly proteomics-based solution for prescreening. A proteomics solution has the potential to be notably more economical due to its simplicity/maturity. This is in contrast to the relatively high cost of the ′omics-based screening methods. Nevertheless, proteomics-based solutions, historically, have suffered from limited sensitivity and low specificity. In order to attain a high sensitivity while maintaining a workable specificity, we have used a complex Artificial Intelligence (AI) based system, comprising machine learning (ML) and Expert Systems (ES). Our MCED blood-based prescreening solution has been successfully tested on a series of cancers, including (among others) the cancers of breast, lung, prostate, ovary, uterus, and cervix, with cancer signal detection sensitivity of ≈92% for early stages (Stage I or II). While a tandem prescreening-screening system is designed primarily for testing asymptomatic individuals for early detection of cancer, a prescreening method has other potential uses. For instance, it can be used for: 1) minimal residual disease (MRD) detection after treatment; 2) recurrence monitoring after achieving remission. In particular, in cases where the original cancer is aggressive and has a high chance of recurrence, e.g., triple-negative breast cancer (TNBC), such a prescreening test can be repeated several times in the course of a year, providing the patient with measurements that are affordable and near real-time. In conclusion, the introduced proteomics/AI-based prescreening system could provide an economical, yet high-sensitivity solution for general adoption, acting primarily as a molecular risk assessment tool, to be performed prior to using a high-specificity screening modality. Pre-screening breast cancer with machine learning and deep learning

Abstract:We suggest that deep learning can be used for pre-screening cancer by analyzing demographic and anthropometric information of patients, as well as biological markers obtained from routine blood samples and relative risks obtained from meta-analysis and international databases. We applied feature selection algorithms to a database of 116 women, including 52 healthy women and 64 women diagnosed with breast cancer, to identify the best pre-screening predictors of cancer. We utilized the best predictors to perform k-fold Monte Carlo cross-validation experiments that compare deep learning against traditional machine learning algorithms. Our results indicate that a deep learning model with an input-layer architecture that is fine-tuned using feature selection can effectively distinguish between patients with and without cancer. Additionally, compared to machine learning, deep learning has the lowest uncertainty in its predictions. These findings suggest that deep learning algorithms applied to cancer pre-screening offer a radiation-free, non-invasive, and affordable complement to screening methods based on imagery. The implementation of deep learning algorithms in cancer pre-screening offer opportunities to identify individuals who may require imaging-based screening, can encourage self-examination, and decrease the psychological externalities associated with false positives in cancer screening. The integration of deep learning algorithms for both screening and pre-screening will ultimately lead to earlier detection of malignancy, reducing the healthcare and societal burden associated to cancer treatment.

Abstract:e15035 Background: Early cancer detection has the potential to significantly improve patient outcomes and reach the Cancer Moonshot goals of reducing the death rate from cancer by at least 50 percent over the next 25 years. Harbinger Health is pioneering early cancer detection with a blood-based test that combines recent genomic and epigenomic discoveries of early cancer and biology-informed artificial intelligence. Unlike prior approaches that are purely statistical to identify informative biomarkers, Harbinger’s approach is informed by insights into specific biological events early during tumorigenesis and is therefore optimized for detecting cancer in patients with very low levels of circulating tumor DNA. Methods: Here, we present data generated using the Harbinger Health assay from 1,046 subjects, 621 with newly diagnosed, treatment-naive cancer (15 different cancer types) and 425 individuals with no history, diagnosis, or cancer symptoms. Using this sample cohort, we developed and applied a rigorous framework for training, calibration and predicting likelihood of cancer using a multi-layered logistic regression-based machine learning algorithm, generating final outputs of binary classification (cancer yes/no). Our framework involved multiple iterations of 10-fold cross-validation of the full dataset, and we report solely on samples that appear in the held-out test set in each iteration. Additionally, we developed a similar framework to predict tissue of origin (TOO) for cancer samples. Results: The overall sensitivity of cancer detection was 82% (95% confidence interval (CI): 72.7-91.0%) at 95% specificity. Notably, the sensitivity was 74% (95% CI: 54.8-92.7%) for stage 1 and 84% (95% CI: 65.3-100%) for stage 2. Furthermore, we were able to correctly predict cancer in 95% of patient samples with at least 0.037% tumor fraction. The overall sensitivity for high incident cancers were: breast (73%), prostate (82%), lung (85%), and colorectal (96%) at 95% specificity and the overall accuracy of TOO prediction was 86% when the tumor fraction was greater than 0.1% in the top 3 most prevalent cancer types (breast, colorectal and lung). Importantly, we noted that technical variability was introduced when performing assay optimization strategies, yet we observed comparable performance when assessing subsets of stably processed samples, indicating that our performance is robust. Conclusions: Overall, these results demonstrate that the Harbinger Health platform, with its biology-informed approach, has the potential for highly sensitive multi-cancer diagnostic accuracy and specifically those with early-stage cancer. The platform is now being validated in a 10,000-subject prospective clinical trial (CORE-HH/NCT05435066).

Abstract:Effective risk assessment in early breast cancer is essential for informed clinical decision-making, yet consensus on defining risk categories remains challenging. This paper explores evolving approaches in risk stratification, encompassing histopathological, immunohistochemical, and molecular biomarkers alongside cutting-edge artificial intelligence (AI) techniques. Leveraging machine learning, deep learning, and convolutional neural networks, AI is reshaping predictive algorithms for recurrence risk, thereby revolutionizing diagnostic accuracy and treatment planning. Beyond detection, AI applications extend to histological subtyping, grading, lymph node assessment, and molecular feature identification, fostering personalized therapy decisions. With rising cancer rates, it is crucial to implement AI to accelerate breakthroughs in clinical practice, benefiting both patients and healthcare providers. However, it is important to recognize that while AI offers powerful automation and analysis tools, it lacks the nuanced understanding, clinical context, and ethical considerations inherent to human pathologists in patient care. Abstract:Guideline-recommended screening programs exist for only a few cancer types. Although all these programs are understood to lead to reductions in cancer-related mortality, standard-of-care screening tests vary in accuracy, adherence and effectiveness. Recent advances in high-throughput technologies and machine learning have facilitated the development of blood-based multi-cancer cancer early detection (MCED) tests. MCED tests are positioned to be complementary to standard-of-care screening and they may broaden screening availability, especially for individuals who are not adherent with current screening programs and for individuals who may harbor cancers with no available screening options. In this article, we outline some key features that should be considered for study design and MCED test development, provide an example of the developmental pathway undertaken for an emerging multi-biomarker class MCED test and propose a clinical algorithm for an imaging-based diagnostic resolution strategy following MCED testing.
Deep Learning for Breast Cancer Risk Prediction: Application to a Large Representative UK Screening Cohort

Abstract:Purpose To develop an artificial intelligence (AI) deep learning tool capable of predicting future breast cancer risk from a current negative screening mammographic examination and to evaluate the model on data from the UK National Health Service Breast Screening Program. Materials and Methods The OPTIMAM Mammography Imaging Database contains screening data, including mammograms and information on interval cancers, for more than 300 000 female patients who attended screening at three different sites in the United Kingdom from 2012 onward. Cancer-free screening examinations from women aged 50-70 years were performed and classified as risk-positive or risk-negative based on the occurrence of cancer within 3 years of the original examination. Examinations with confirmed cancer and images containing implants were excluded. From the resulting 5264 risk-positive and 191 488 risk-negative examinations, training (n = 89 285), validation (n = 2106), and test (n = 39 351) datasets were produced for model development and evaluation. The AI model was trained to predict future cancer occurrence based on screening mammograms and patient age. Performance was evaluated on the test dataset using the area under the receiver operating characteristic curve (AUC) and compared across subpopulations to assess potential biases. Interpretability of the model was explored, including with saliency maps. Results On the hold-out test set, the AI model achieved an overall AUC of 0.70 (95% CI: 0.69, 0.72). There was no evidence of a difference in performance across the three sites, between patient ethnicities, or across age groups. Visualization of saliency maps and sample images provided insights into the mammographic features associated with AI-predicted cancer risk. Conclusion The developed AI tool showed good performance on a multisite, United Kingdom-specific dataset. Abstract:Abstract Cancer early detection aims at reducing cancer deaths. Unfortunately, many established cancer screening methods are not suitable for use in low- and middle-income countries (LMICs) due to cost, complexity, and dependency on extensive medical infrastructure. Nearly 10,000 participants (2003 cancer cases and 7888 non-cancer cases) were divided into one training and five independent validation cohorts across different races, sample types and platforms. One tube of peripheral blood was collected from each participant and quantified using a panel of seven protein tumor markers (PTMs) consisting of AFP, CA125, CA15-3, CA19-9, CA72-4, CEA and CYFRA 21-1 by common clinical immunoassay analyzers. An algorithm named OncoSeek was established using artificial intelligence (AI) to distinguish cancer cases from non-cancer cases by calculating the probability of cancer (POC) index based on the quantification of the seven PTMs and clinical information including sex and age, and to predict the possible affected tissue of origin (TOO). The conventional clinical method that relied only on a single threshold for each PTM would make a big problem when combining the results of those markers as the false positive rate would accumulate as the number of markers increased. Nevertheless, OncoSeek was empowered by AI to significantly reduce the false positive rate, increasing the specificity from 54.0% to 93.0%. The overall sensitivity of OncoSeek was 51.7%, resulting in 84.6% accuracy. The performance was consistent in the training and the five validation cohorts from three countries (Brazil, China and United States) including two sample types (plasma and serum) and three different platforms (Roche, Luminex and ELISA). The sensitivities ranged from 39.0% to 77.6% for the detection of the nine common cancer types (breast, colorectum, liver, lung, lymphoma, oesophagus, ovary, pancreas and stomach), which account for 59.2% of global cancer deaths annually. Furthermore, it has shown excellent sensitivity in several high-mortality cancer types for which there are lacking routine screening tests in the clinic, such as the sensitivity of pancreatic cancer was 77.6%. The overall accuracy of TOO prediction in the true positives was 65.4%, which could assist the clinical diagnostic workup. OncoSeek significantly outperforms the conventional clinical method, representing a novel blood-based test for multicancer early detection (MCED) that is non-invasive, easy, efficient and robust. Moreover, the accuracy of TOO facilitates the follow-up diagnostic workup. OncoSeek is affordable (~$20) and accessible requiring nothing more than a blood draw at the screening sites, which makes it adoptable in LMICs. Promoting Artificial Intelligence for Global Breast Cancer Risk Prediction and Screening in Adult Women: A Scoping Review

Abstract:Background: Artificial intelligence (AI) algorithms can be applied in breast cancer risk prediction and prevention by using patient history, scans, imaging information, and analysis of specific genes for cancer classification to reduce overdiagnosis and overtreatment. This scoping review aimed to identify the barriers encountered in applying innovative AI techniques and models in developing breast cancer risk prediction scores and promoting screening behaviors among adult females. Findings may inform and guide future global recommendations for AI application in breast cancer prevention and care for female populations. Methods: The PRISMA-SCR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews) was used as a reference checklist throughout this study. The Arksey and O'Malley methodology was used as a framework to guide this review. The framework methodology consisted of five steps: (1) Identify research questions; (2) Search for relevant studies; (3) Selection of studies relevant to the research questions; (4) Chart the data; (5) Collate, summarize, and report the results. Results: In the field of breast cancer risk detection and prevention, the following AI techniques and models have been applied: Machine and Deep Learning Model (ML-DL model) (n = 1), Academic Algorithms (n = 2), Breast Cancer Surveillance Consortium (BCSC), Clinical 5-Year Risk Prediction Model (n = 2), deep-learning computer vision AI algorithms (n = 2), AI-based thermal imaging solution (Thermalytix) (n = 1), RealRisks (n = 2), Breast Cancer Risk NAVIgation (n = 1), MammoRisk (ML-Based Tool) (n = 1), Various MLModels (n = 1), and various machine/deep learning, decision aids, and commercial algorithms (n = 7). In the 11 included studies, a total of 39 barriers to AI applications in breast cancer risk prediction and screening efforts were identified. The most common barriers in the application of innovative AI tools for breast cancer prediction and improved screening rates included lack of external validity and limited generalizability (n = 6), as AI was used in studies with either a small sample size or datasets with missing data. Many studies (n = 5) also encountered selection bias due to exclusion of certain populations based on characteristics such as race/ethnicity, family history, or past medical history. Several recommendations for future research should be considered. AI models need to include a broader spectrum and more complete predictive variables for risk assessment. Investigating long-term outcomes with improved follow-up periods is critical to assess the impacts of AI on clinical decisions beyond just the immediate outcomes. Utilizing AI to improve communication strategies at both a local and organizational level can assist in informed decision-making and compliance, especially in populations with limited literacy levels. Conclusions: The use of AI in patient education and as an adjunctive tool for providers is still early in its incorporation, and future research should explore the implementation of AI-driven resources to enhance understanding and decision-making regarding breast cancer screening, especially in vulnerable populations with limited literacy.

Abstract:Abstract Background: Approximately 1 in 8 women will be impacted by breast cancer in their lifetimes. Earlier detection of breast cancer through screening has improved survival. Liquid biopsies have the potential to complement existing screening methods by enabling earlier detection and differentiating invasive cancer (IBC) from ductal carcinoma in-situ (DCIS). We previously demonstrated high sensitivity and specificity for early detection of IBC by using a blood-based liquid biopsy platform to analyze a novel category of cancer-associated small RNAs, termed orphan RNAs (oncRNAs). Here, we developed a test that could not only detect the presence of cancer, but also classify the invasiveness of breast cancer. Methods: We utilized The Cancer Genome Atlas (TCGA) small RNA profiles to discover a library of 20,538 oncRNAs that were significantly enriched among 1,103 breast tumors compared to 349 controls from normal tissues spanning multiple tissue sites, limited to female samples. The diagnostic performance of these oncRNAs was assessed in an independent cohort of serum samples from 708 women, including 380 breast cancer patients (221 IBC and 159 DCIS; mean age: 58.0 ± 13.4 years) and 328 age-matched controls (mean age: 58.4 ± 13.7 years). We sequenced the small RNA content from 1 ml of serum from these patients at an average depth of 21.6 million 50-bp single-end reads. We detected 19,736 (96%) of the breast cancer-specific oncRNA library within at least one sample in this cohort. We then trained a multi-class generative AI model using 5-fold cross-validation to predict IBC, DCIS, and absence of breast cancer (IBC or DCIS). Results: Our oncRNA-based generative AI model achieved an overall AUC of 0.95 (95% CI: 0.94-0.97) for prediction of breast cancer versus cancer-free controls. At 90% specificity, overall model sensitivity is 90.0% (86.5%-92.8%). For DCIS and stage I IBC, the model has a sensitivity of 88.1% (82.0%-92.7%) and 90.4% (81.9%-95.8%) respectively, both at 90% specificity. In the second step, restricting to samples flagged as cancer, we observed an overall AUC of 0.9 (0.87-0.93) and sensitivity of 62.4% (55.3%-69.1%) at 90% specificity for discriminating against invasive breast cancer. Conclusions: We have demonstrated the potential utility of oncRNAs as the foundation for a liquid biopsy platform for sensitive and accurate early detection of breast cancer. Abstract:Early detection can significantly reduce mortality due to lung cancer. Presented here is an approach for developing a blood-based screening panel based on clonal hematopoietic mutations. Animal model studies suggest that clonal hematopoietic mutations in tumor infiltrating immune cells can modulate cancer progression, representing potential predictive biomarkers. The goal of this study was to determine if the clonal expansion of these mutations in blood samples could predict the occurrence of lung cancer. A set of 98 potentially pathogenic clonal hematopoietic mutations in tumor infiltrating immune cells were identified using sequencing data from lung cancer samples. These mutations were used as predictors to develop a logistic regression machine learning model. The model was tested on sequencing data from a separate set of 578 lung cancer and 545 non-cancer samples from 18 different cohorts. The logistic regression model correctly classified lung cancer and non-cancer blood samples with 94.12% sensitivity (95% Confidence Interval: 92.20-96.04%) and 85.96% specificity (95% Confidence Interval: 82.98-88.95%). Our results suggest that it may be possible to develop an accurate blood-based lung cancer screening panel using this approach. Unlike most other "liquid biopsies" currently under development, the approach presented here is based on standard sequencing protocols and uses a relatively small number of rationally selected mutations as predictors.

Abstract:Artificial intelligence (AI) holds promise for improving breast cancer screening, but many challenges remain in implementing AI tools in clinical screening services. AI readers compare favourably against individual human radiologists in detecting breast cancer in population screening programs. However, single AI or human readers cannot perform at the level of multi-reader systems such as those used in Australia, Sweden, the UK, and other countries. The implementation of AI readers in mammographic screening programs therefore demands integration of AI readers in multi-reader systems featuring collaboration between humans and AI. Successful integration of AI readers demands a better understanding of possible models of human-AI collaboration and exploration of the range of possible outcomes engendered by the effects on human readers of interacting with AI readers. Here, we used a large, high-quality retrospective mammography dataset from Victoria, Australia to conduct detailed simulations of five plausible AI-integrated screening pathways. We compared the performance of these AI-integrated pathways against the baseline standard-of-care "two reader plus third arbitration" system used in Australia. We examined the influence of positive, neutral, and negative human-AI interaction effects of varying strength to explore possibilities for upside, automation bias, and downside risk of human-AI collaboration. Replacing the second reader or allowing the AI reader to make high confidence decisions can improve upon the standard of care screening outcomes by 1.9-2.5% in sensitivity and up to 0.6% in specificity (with 4.6-10.9% reduction in the number of assessments and 48-80.7% reduction in the number of reads). Automation bias degrades performance in multi-reader settings but improves it for single-readers. Using an AI reader to triage between single and multi-reader pathways can improve performance given positive human-AI interaction. This study provides insight into feasible approaches for implementing human-AI collaboration in population mammographic screening, incorporating human-AI interaction effects. Our study provides evidence to support the urgent assessment of AI-integrated screening pathways with prospective studies to validate real-world performance and open routes to clinical adoption.

Abstract:Abstract Background: Image-derived AI risk models for breast cancer have shown high discriminatory performances compared with clinical risk models based on family history and lifestyle factors. However, little is known about their generalizability across different screening settings and clinical feasibility. Purpose: To investigate the predictive performance of a clinically used image-derived AI-based breast cancer risk model in multiple European screening populations. Methods: Four European mammographic screening populations in three countries screened between 2009-2020 for women aged 45-69 was used to perform a nested case-control study. In total, 739 women with incident breast cancers were included together with 7,812 controls matched to cases on year of study-entry. Mammographic features (density, microcalcifications, masses, left-right breast asymmetries of these features) for risk assessment were extracted using AI from full-field digital mammograms. Breast cancer occurrence was assessed after two years of follow-up. Absolute risks of breast cancer were predicted using the risk model from negative mammograms at study-entry. Adjusted Area Under the receiver operating characteristic Curves (aAUC) estimated discriminatory performance and, adjusted risk-ratios estimated the stratification performance of women at high/general risk per the clinical guidelines. Results: The overall aAUC of the AI risk model was 0.72 (95%CI 0.70-0.75), range 0.71 (95%CI 0.67-0.75) to 0.74 (95%CI 0.69-0.78) for breast cancers developed in four screening populations. In the 4.6% of women classified at high risk using the NICE guidelines thresholds, cancers were more likely diagnosed after 2 years follow-up, risk-ratio (RR) 6.7 (95%CI 5.6-8.0), compared with the 71% of women classified at general risk by the model. Similar risk-ratios were observed across tertiles of mammographic density. In the high-risk group, 22% of the 2-year future cancers were diagnosed, and 29% of stage 2 and higher cancers, p< 0.01. Conclusions: The AI risk model showed generalizable discriminatory performances across European populations and, captured ~30% of clinically relevant stage 2 and higher breast cancers in ~5% of high-risk women who were sent home with a negative mammogram. Similar results were seen in fatty and dense breasts. An image-derived AI model is feasible for personalized screening to improve screening outcomes. Abstract:e13574 Background: Current United States Preventive Services Task Force (USPSTF) recommendations include routine screening for breast, cervical, colorectal, and lung cancer; however, two out of every three cancer cases occur in other indications leading to diagnosis in advanced stages of the disease and higher likelihood of mortality. Current recommended cancer screening and early detection tests also face several hurdles hindering equitable access in a real-world setting. Blood-based MCED tests can impact cancer screening and early detection by monitoring for multiple different cancer types at once via a blood draw that can detect cancers in multiple indications, including indications where screening is not performed routinely today. Given their nascency, the role of blood-based MCED tests in clinical practice is currently undefined. To help facilitate the appropriate integration of MCED tests into patient care, it is important to understand the perspectives of stakeholders on the anticipated challenges to implementation. Methods: We conducted three separate double-blinded surveys in July 2023 of representative cohorts of U.S. Healthcare providers (HCPs) (n=238) that are or act as primary care providers (general practitioners and obstetrics and gynecologists (OB-GYNs)), payers (n=40), and patients (n=116), to evaluate the current cancer screening and early detection landscape and gather perspectives on barriers to implementation of new cancer screening technologies, including blood-based MCED tests. Results: Of the surveyed HCPs and payers, >90% and 100% respectively reported to be aware of MCED tests. However, <10% of HCPs reported having ordered an MCED test and 80% of payers had not yet evaluated an MCED test for coverage under their plan at the time of the survey. The survey results indicated that there is broad agreement on the value proposition that blood-based MCED tests would improve patient outcomes with earlier intervention. Despite this recognition, the survey highlighted that there is limited use today due to the perceived lack of clinical accuracy and utility data, high out-of-pocket patient costs, and lack of payer coverage. Conclusions: To overcome the hurdles for future widespread adoption of blood-based MCED tests, increased investment in data generation, education, and implementation of logistical support for HCPs will be critical. Initiatives include collaborations between stakeholders to generate near-term clinical utility data, equipping HCPs with the ability to educate patients on test accuracy, and seamlessly integrating the testing process into the patient visit workflow. The findings of this study can help build understanding of the anticipated barriers and inform strategies to alleviate them and build a foundation for clinical adoption of blood-based MCED tests.

Abstract:Background: Artificial intelligence (AI) readers, derived from applying deep learning models to medical image analysis, hold great promise for improving population breast cancer screening. However, previous evaluations of AI readers for breast cancer screening have mostly been conducted using cancer-enriched cohorts and have lacked assessment of the potential use of AI readers alongside radiologists in multi-reader screening programs. Here, we present a new AI reader for detecting breast cancer from mammograms in a large-scale population screening setting, and a novel analysis of the potential for human-AI reader collaboration in a well-established, high-performing population screening program. We evaluated the performance of our AI reader and AI-integrated screening scenarios using a two-year, real-world, population dataset from Victoria, Australia, a screening program in which two radiologists independently assess each episode and disagreements are arbitrated by a third radiologist. Methods: We used a retrospective full-field digital mammography image and non-image dataset comprising 808,318 episodes, 577,576 clients and 3,404,326 images in the period 2013 to 2019. Screening episodes from 2016, 2017 and 2018 were sequential population cohorts containing 752,609 episodes, 565,087 clients and 3,169,322 images. The dataset was split by screening date into training, development, and testing sets. All episodes from 2017 and 2018 were allocated to the testing set (509,109 episodes; 3,651 screen-detected cancer episodes). Eight distinct AI models were trained on subsets of the training set (which included a validation set) and combined into our ensemble AI reader. Operating points were selected using the development set. We evaluated our AI reader on our testing set and on external datasets previously unseen by our models. Findings: The AI reader outperformed the mean individual radiologist on this large retrospective testing dataset with an area under the receiver operator characteristic curve of 0.92 (95% CI 0.91, 0.92). The AI reader generalised well across screening round, client demographics, device manufacturer and cancer type, and achieved state-of-the-art performance on external datasets compared to recently published AI readers. Our simulations of AI-integrated screening scenarios demonstrated that a reader-replacement human-AI collaborative system could achieve better sensitivity and specificity (82.6%, 96.1%) compared to the current two-reader consensus system (79.9%, 96.0%), with reduced human reading workload and cost. Our band-pass AI-integrated scenario also enabled both higher sensitivity and specificity (80.6%, 96.2%) with larger reductions in human reading workload and cost. Interpretation: This study demonstrated that human-AI collaboration in a population breast cancer screening program has potential to improve accuracy and lower radiologist workload and costs in real world screening programs. The next stage of validation is to undertake prospective studies that can also assess the effects of the AI systems on human performance and behaviour.
