Abstract:Combining virtual reality (VR) with rehabilitation robots has the potential to enhance rehabilitation training and neural functional recovery. However, there is limited research on designing VR scenes and evaluating the impact of such systems on participants' cognitive and experiential aspects when using rehabilitation robots. This study aimed to examine the effects of different gaming modes (first-person perspective and third-person perspective) and robot involvement on participants' motivation, experience, task load, and engagement. Thirty-two participants underwent gait rehabilitation training, providing feedback on their experiences after each condition. The findings revealed that the first-person perspective mode increased motivation, experience, task performance, and engagement. On the other hand, robot-assisted participation improved motivation but decreased the overall experience. These results indicate that the first-person perspective mode takes precedence over the third-person perspective mode in terms of embodied gaming and the development of rehabilitation tasks.

Increasing patient engagement during virtual reality-based motor rehabilitation

Abstract:Objective: To investigate the influence of different design characteristics of virtual reality exercises on engagement during lower extremity motor rehabilitation. Design: Correlational study. Setting: Spinal cord injury (SCI) rehabilitation center. Participants: Subjects with SCI (n=12) and control subjects (n=10). Interventions: Not applicable. Main outcome measures: Heart rate and electromyographic activity from both legs at the tibialis anterior, the gastrocnemius medialis, the rectus femoris, and the biceps femoris were recorded. Results: Interactivity (ie, functionally meaningful reactions to motor performance) was crucial for the engagement of subjects. No significant differences in engagement were found between exercises that differed in feedback frequency, explicit task goals, or aspects of competition. Conclusions: Functional feedback is highly important for the active participation of patients during robotic-assisted rehabilitation. Further investigations on the design characteristics of virtual reality exercises are of great importance. Exercises should thoroughly be analyzed regarding their effectiveness, while user preferences and expectations should be considered when designing virtual reality exercises for everyday clinical motor rehabilitation.

Abstract:Virtual Reality (VR) has emerged as a new safe and efficient tool for the rehabilitation of many childhood and adulthood illnesses. VR-based therapies have the potential to improve both motor and functional skills in a wide range of age groups through cortical reorganization and the activation of various neuronal connections. Recently, the potential for using serious VR-based games that combine perceptual learning and dichoptic stimulation has been explored for the rehabilitation of ophthalmological and neurological disorders. In ophthalmology, several clinical studies have demonstrated the ability to use VR training to enhance stereopsis, contrast sensitivity, and visual acuity. The use of VR technology provides a significant advantage in training each eye individually without requiring occlusion or penalty. In neurological disorders, the majority of patients undergo recurrent episodes (relapses) of neurological impairment, however, in a few cases (60-80%), the illness progresses over time and becomes chronic, consequential in cumulated motor disability and cognitive deficits. Current research on memory restoration has been spurred by theories about brain plasticity and findings concerning the nervous system's capacity to reconstruct cellular synapses as a result of interaction with enriched environments. Therefore, the use of VR training can play an important role in the improvement of cognitive function and motor disability. Although there are several reviews in the community employing relevant Artificial Intelligence in healthcare, VR has not yet been thoroughly examined in this regard. In this systematic review, we examine the key ideas of VR-based training for prevention and control measurements in ocular diseases such as Myopia, Amblyopia, Presbyopia, and Age-related Macular Degeneration (AMD), and neurological disorders such as Alzheimer, Multiple Sclerosis (MS) Epilepsy and Autism spectrum disorder. This review highlights the fundamentals of VR technologies regarding their clinical research in healthcare. Moreover, these findings will raise community awareness of using VR training and help researchers to learn new techniques to prevent and cure different diseases. We further discuss the current challenges of using VR devices, as well as the future prospects of human training.

Abstract:Virtual reality, augmented reality, and mixed reality (VR/AR/MR) as information and communication technologies have been recognised and implemented in healthcare in recent years. One of the popular application ways is games, due to the potential benefits of providing an engaging and immersive experience in a virtual environment. This study presents a systematic literature review that evaluates the state-of-the-art on VR/AR/MR game applications in healthcare by collecting and analysing related journal and conference papers published from 2014 through to the first half of 2020. After retrieving more than 3,000 papers from six databases, 88 articles, from both computer science and medicine, were selected and analysed in the review. The articles are classified and summarised based on their (1) publication information, (2) design, implementation, and evaluation, and (3) application. The presented review is beneficial for both researchers and developers interested in exploring current research and future trends in VR/AR/MR in healthcare.

Abstract:Background: Virtual reality (VR) technology has been widely used in rehabilitation training because of its immersive, interactive, and imaginative features. A comprehensive bibliometric review is required to help researchers focus on future directions based on the new deﬁnitions of VR technologies in rehabilitation, which reveal new situations and requirements. Objective: Herein, we aimed to summarize effective research methods for and potential innovative approaches to VR rehabilitation by evaluating publications from various countries to encourage research on efficient strategies to improve VR rehabilitation. Methods: The SCIE (Science Citation Index Expanded) database was searched on January 20, 2022, for publications related to the application of VR technology in rehabilitation research. We found 1617 papers, and we created a clustered network, using the 46,116 references cited in the papers. CiteSpace V (Drexel University) and VOSviewer (Leiden University) were used to identify countries, institutions, journals, keywords, cocited references, and research hot spots. Results: A total of 63 countries and 1921 institutes have contributed publications. The United States of America has taken the leading position in this field; it has the highest number of publications; the highest h-index; and the largest collaborative network, which includes other countries. The reference clusters of SCIE papers were divided into the following nine categories: kinematics, neurorehabilitation, brain injury, exergames, aging, motor rehabilitation, mobility, cerebral palsy, and exercise intensity. The research frontiers were represented by the following keywords: video games (2017-2021), and young adults (2018-2021). Conclusions: Our study comprehensively assesses the current research state of VR rehabilitation and analyzes the current research hot spots and future trends in the field, with the aims of providing resources for more intensive investigation and encouraging more researchers to further develop VR rehabilitation.

Abstract:Dysfunctions of upper limbs caused by diseases such as stroke result in difficulties in conducting day-to-day activities. Studies show that rehabilitation training using virtual reality games is helpful for patients to restore arm functions. It has been found that ensuring active patient participation and effort devoting in the process is very important to obtain better training results. This article introduces a method to help patients increase their engagement and provide motion assistance in virtual reality-mediated upper-limb rehabilitation training. Attention enhancement and motion assistance is achieved through an illusion of virtual forces created by altering the drag speed between the cursor and the object presented on a screen to the patient as the only feedback. We present two game forms using the proposed method, including a target-approaching game and a maze-following game. The results of evaluation experiments with human participants showed that the proposed method could provide path guidance that significantly improved path-following performance of users and required more involvement of the users when compared to playing the game without attention enhancement and motion assistance.
The Effectiveness of Virtual Reality Exercise on Individual’s Physiological, Psychological and Rehabilitative Outcomes: A Systematic Review

Abstract:Objective purpose: This review synthesized the literature examining the effects of virtual reality (VR)-based exercise on physiological, psychological, and rehabilitative outcomes in various populations. Design: A systematic review. Data sources: 246 articles were retrieved using key words, such as “VR”, “exercise intervention”, “physiological”, “psychology”, and “rehabilitation” through nine databases including Academic Search Premier and PubMed. Eligibility criteria for selecting studies: 15 articles which met the following criteria were included in the review: (1) peer-reviewed; (2) published in English; (3) randomized controlled trials (RCTs), controlled trials or causal-comparative design; (4) interventions using VR devices; and (5) examined effects on physiological, psychological, and/or rehabilitative outcomes. Descriptive and thematic analyses were used. Results: Of the 12 articles examining physiological outcomes, eight showed a positive effect on physical fitness, muscle strength, balance, and extremity function. Only four articles examined the effects on psychological outcomes, three showed positive effects such that VR exercise could ease fatigue, tension, and depression and induce calmness and enhance quality of life. Nine articles investigated the effects of VR-based exercise on rehabilitative outcomes with physiological and/or psychological outcomes, and six observed significant positive changes. In detail, patients who suffered from chronic stroke, hemodialysis, spinal-cord injury, cerebral palsy in early ages, and cognitive decline usually saw better improvements using VR-based exercise. Conclusion: The findings suggest that VR exercise has the potential to exert a positive impact on individual’s physiological, psychological, and rehabilitative outcomes compared with traditional exercise. However, the quality, quantity, and sample size of existing studies are far from ideal. Therefore, more rigorous studies are needed to confirm the observed positive effects.
Immersive Virtual Reality Games in Neuromotor Rehabilitation with Brain-Computer Interfaces: A Scoping Review

Abstract:The use of immersive virtual reality (VR) is gaining traction among the scientific community as it opens great opportunities in the field of rehabilitation. By making use of video game mechanics and Brain computer interface (BCI) that is based on electroencephalography (EEG) signals, patients undergoing neurorehabilitation can be more engaged in rehabilitation training. This paper reviews the available literature which uses BCI and VR in game rehabilitation and analyses the gaming elements and brain machine interfaces (BMI) used in each study. Four databases (IEEE Xplore, PubMed, Web of Science, Scopus) from inception until October 2023 were queried using a comprehensive searching strategy and then screened by two independent reviewers. A total of 18 articles were found eligible for qualitative synthesis. These are the main findings: (1) A diverse participant demographic spanned age and health conditions; (2) Oculus Rift has gained prominence as a VR device, replacing older CAVE systems; (3) All surveyed studies unanimously relied on the Motor Imagery (MI) paradigm, reflecting its importance in neuro motor rehabilitation and neuroplasticity; (4) Rehabilitation games displayed varied characteristics, emphasizing scoring, embodiment, and customization. Notably, the lack of gamification elements in some games suggests an area for potential enhancement and future research.

Psychological benefits of virtual reality for patients in rehabilitation therapy

Abstract:Context: Whether virtual rehabilitation is beneficial has not been determined. Objective: To investigate the psychological benefits of virtual reality in rehabilitation. Design: An experimental group underwent therapy with a virtual-reality-based exercise bike, and a control group underwent the therapy without virtual-reality equipment. Setting: Hospital laboratory. Patients: 30 patients suffering from spinal-cord injury. Intervention: A designed rehabilitation therapy. Main outcome measures: Endurance, Borg's rating-of-perceived-exertion scale, the Activation-Deactivation Adjective Check List (AD-ACL), and the Simulator Sickness Questionnaire. Results: The differences between the experimental and control groups were significant for AD-ACL calmness and tension. Conclusion: A virtual-reality-based rehabilitation program can ease patients' tension and induce calm.

Abstract:Conventional motor rehabilitation therapies followed by stroke survivors during their recovery process, are typically intense and involve numerous repetitions of task-specific recommended exercises. However, due to the lack of short-term or immediate results, patients tend to lose motivation on the rehabilitation process, and on performing the therapist recommended exercises at home between sessions. The work described in this article proposes to engage the patients with the therapeutic exercises by transforming the latter into specially designed serious games. After an initial evaluation by therapists, a pilot study was conducted on a rehabilitation clinic that showed that this system is adequate for about 75% of the clinical cases treated. The proposed game was globally well-accepted by patients, showing to be motivational and engaging. These promising results lead us to believe that immersive-based games can indeed be a valuable aid for motor rehabilitation therapies.

Abstract:Purpose of review: This review explores recent advances in using immersive virtual reality to improve bodily perception and motor control in rehabilitation across musculoskeletal and neurological conditions, examining how virtual reality's unique capabilities can address the challenges of traditional approaches. The potential in this area of the emerging metaverse and the integration of artificial intelligence in virtual reality are discussed. Recent findings: In musculoskeletal rehabilitation, virtual reality shows promise in enhancing motivation, adherence, improving range of motion, and reducing kinesiophobia, particularly postsurgery. For neurological conditions like stroke and spinal cord injury, virtual reality's ability to manipulate bodily perceptions offers significant therapeutic potential, with reported improvements in upper limb function and gait performance. Balance and gait rehabilitation, especially in older adults, have also seen positive outcomes. The integration of virtual reality with brain-computer interfaces presents exciting possibilities for severe speech and motor impairments. Summary: Current research is limited by small sample sizes, short intervention durations, and variability in virtual reality systems. Future studies should focus on larger, long-term trials to confirm findings and explore underlying mechanisms. As virtual reality technology advances, its integration into rehabilitation programs could revolutionize treatment approaches, personalizing treatments, facilitating home training, and potentially improving patient outcomes across a wide variety of conditions.

Abstract:This narrative review focuses on upper-limb stroke rehabilitation and virtual reality (VR) exergaming interventions that seek to facilitate the rehabilitation process. We examine exergaming interventions from the perspective of diegesis ("narration"), an aspect often overlooked despite its significance in neuronal rehabilitation. The importance of diegesis and narrative engagement in rehabilitation exergames started becoming clear only recently, with findings in the field of neurology underscoring the impact of purpose-driven task engagement on neuroplasticity. We begin this review by examining various frameworks for stroke rehabilitation exergames and identifying the gaps in the existing literature. We continue with summarizing the literature on exergames in upper-limb stroke rehabilitation, emphasizing the contribution of diegesis on exercise motivation and engagement. Finally, we conclude this review by offering insights into the current state of research, along with future perspectives on the topic.

Abstract:AbstractThe purpose of this study was to explore the clinical feasibility of virtual reality (VR) for the rehabilitation of upper limbs of stroke. In this study, it was found and suggested that future research should focus on the content design and application of VR rehabilitation games. While using VR to increase the interestingness of rehabilitation, one can also integrate VR and other technologies to achieve complementary benefits. In addition, in terms of the design of VR rehabilitation games, it was suggested that VR rehabilitation game researchers investigate the needs of the target users and design VR games that meet the needs of the target users in future work. Finally, this study demonstrates the clinical feasibility of applying VR technology for the rehabilitation of upper limbs after stroke, as well as highlights the aspects that still need to be addressed by researchers. These aspects are important targets of designing a VR system suitable for stroke upper limb rehabilitation.
CFI: a VR motor rehabilitation serious game design framework integrating rehabilitation function and game design principles with an upper limb case

Abstract:Virtual reality (VR) Rehabilitation holds the potential to address the challenge that patients feel bored and give up long-term rehabilitation training. Despite the introduction of gaming elements by some researchers in rehabilitation training to enhance engagement, there remains a notable lack of in-depth research on VR rehabilitation serious game design methods, particularly the absence of a concrete design framework for VR rehabilitation serious games. Hence, we introduce the Clinical-Function-Interesting (CFI): a VR rehabilitation serious game design framework, harmonizing rehabilitation function and game design theories. The framework initiates with clinic information, defining game functions through the functional decomposition of rehabilitation training. Subsequently, it integrates gaming elements identified through the analysis and comparison of related literature to provide enduring support for long-term training. Furthermore, VR side-effect and enhancement are considered. Building upon this design framework, we have developed an upper limb VR rehabilitation serious game tailored for mild to moderate stroke patients and aligned our framework with another developed VR rehabilitation serious game to validate its practical feasibility. Overall, the proposed design framework offers a systematic VR rehabilitation serious game design methodology for the VR rehabilitation field, assisting developers in more accurately designing VR rehabilitation serious games that are tailored to specific rehabilitation goals.

Abstract:Prolonged bed rest is a known contributor to muscle atrophy, weakness, and deconditioning. Early active mobilization protocols aim to combat acquired weakness and loss of function in patients in the intensive care unit. Despite these benefits, mobilization of patients in the intensive care unit remains a challenge, most notably for patients with limited ability to get out of bed because of invasive devices and prolonged hospitalization. Virtual reality has gained favor for use in critical care to mitigate patients' stress, pain, and anxiety and to provide distraction and socialization. This case report demonstrates a novel application of virtual reality and the ease with which virtual reality can be used to facilitate early mobility and activity progression in the critically ill.

Abstract:Objective: Physical rehabilitation by virtual reality (VR) gamification is gaining acceptance. This study was designed to verify whether neck movements invoked by a fully immersive VR game environment may be physiotherapist-prescribed rehabilitation exercise. Methods: This was a single-visit prospective clinical trial (NCT03104647). Healthy participants put on VR headsets and entered a fully immersive game environment (VRPhysio, XRHealth, Tel Aviv, Israel) that prompted neck movement (flexion, extension, rotation, lateral bend, and combinations repeated twice in random order) accompanied by feedback encouraging correct performance. Four board-certified physiotherapists independently viewed videotapes recorded during the session, identified movements, and determined whether they were recommended as neck rehabilitation exercises. Results: Twenty ( n = 20) participants (male–female ratio = 13:7; age = 38 ± 14 years old) completed the training session (16 movements per participant). All movements were identified correctly and determined to be appropriate for neck rehabilitation. No adverse events were reported. Conclusions: The VRPhysio software invoked movements identified by board-certified physiotherapists as appropriate for neck rehabilitation. The potential advantage of home-based VR gamification of cervical spine rehabilitation programs over common practice in motivating patient adherence warrants evaluation by randomized controlled trials.

Abstract:This paper presents a novel evaluation method for assessing the effectiveness of virtual reality serious games (In our work, we used virtual reality as a simulated 3D environment, not immersive headsets) in post-stroke rehabilitation. Stroke, which is a leading cause of long-term disability, requires effective rehabilitation approaches to facilitate motor recovery and enhance patients’ quality of life. The study aims to investigate the potential benefits of incorporating VR technology into upper limb rehabilitation for post-stroke patients, with a specific focus on evaluating patient motivation and engagement. The proposed evaluation method provides insights into the effectiveness of VR serious games as a rehabilitation tool and contributes to the understanding of their impact on post-stroke recovery. This study involved the recruitment of a diverse sample of 20 patients, both male and female, who had experienced left hemiplegia as a result of an ischemic stroke. To ensure a representative sample, patients of different ages were included. Participants were divided into three groups, with each group invited to participate in different stages over a one-year period. During an eight-week period, all groups participated in a rehabilitation program consisting of virtual reality exercises twice a week, alongside five traditional rehabilitation sessions per week. Furthermore, this study introduces a novel approach using camera-based sensors to evaluate patient engagement in virtual reality exercises. By monitoring and analyzing facial expressions throughout each rehabilitation session, our objective is to leverage emotion recognition technology to understand patients’ emotional states and levels of motivation and engagement. The findings from this study indicate that a significant number of patients showed motivation during virtual reality rehabilitation exercises. The analysis of facial expressions utilizing camera-based sensors,yielded valuable insights into the emotional state and level of engagement exhibited by the participants. These findings substantiate the efficacy of virtual reality as a potent tool in fostering motivation among post-stroke patients, thereby actively involving them in rehabilitation exercises and facilitating their journey toward recovery.

Abstract:This review is aimed to assess the effectiveness of virtual reality (VR) games on cognition, mobility, and emotion in elderly stroke patients. We selected relevant articles from eight databases from 2011 to 2022 and extracted articles on cognitive ability (general cognition, mini-mental state examination (MMSE), Montreal cognitive assessment (MoCA) et al.), mobility (modified Barthel index (MBI), Fugl-Meyer assessment (FMA), Berg balance scale (BBS), functional independence measure motor (FIM MOT)), and emotion (depression/anxiety). Twenty-nine studies including 1311 participants were included in the analysis. In the results, virtual reality games were more effective in improving overall cognitive function in stroke patients compared to conventional therapies. In addition, the intervention group in the MMSE (SMD = 0.6, 95%CI = 0.26–0.95, P = 0.0007), MoCA (MD = 1.97, 95%CI = 1.3–2.64, P < 0.00001), and attention test (MD = 0.25, 95% CI = 0.01–0.49, P < 0.00001) scores were also higher. In terms of physical function, MBI (SMD = 0.61, 95%CI = 0.14–1.08, P = 0.01), FMA (SMD = 0.47, 95%CI = 0.02–0.93, P = 0.04), BBS (SMD = 0.78, 95%CI = 0.42–1.15, P < 0.0001), and FIM MOT (MD = 5.87, 95%CI = 2.57–9.17, P = 0.0005) indicators showed better results. It is also observed that virtual reality games can effectively relieve depression and improve mental health in stroke patients. Sports game training, especially with VR equipment, had a positive impact on improving the cognitive performance, mobility, and emotional state of stroke patients compared to a control group. Although the improvement in cognitive ability is relatively low, the effect of improving physical activity and depression is obvious.

Abstract:Stroke is the second most common cause of death worldwide, and it greatly impacts the quality of life for survivors by causing impairments in their upper limbs. Due to the difficulties in accessing rehabilitation services, immersive virtual reality (IVR) is an interesting approach to improve the availability of rehabilitation services. This systematic review evaluates the technological characteristics of IVR systems used in the rehabilitation of upper limb stroke patients. Twenty-five publications were included. Various technical aspects such as game engines, programming languages, headsets, platforms, game genres, and technical evaluation were extracted from these papers. Unity 3D and C# are the primary tools for creating IVR apps, while the Oculus Quest (Meta Platforms Technologies, Menlo Park, CA, USA) is the most often used headset. The majority of systems are created specifically for rehabilitation purposes rather than being readily available for purchase (i.e., commercial games). The analysis also highlights key areas for future research, such as game assessment, the combination of hardware and software, and the potential integration incorporation of biofeedback sensors. The study highlights the significance of technological progress in improving the effectiveness and user-friendliness of IVR. It calls for additional research to fully exploit IVR's potential in enhancing stroke rehabilitation results.

Abstract:Background: Restoring hand and finger function after a traumatic hand injury necessitates a regimen of consistent and conscientious exercise. However, motivation frequently wanes due to unchallenging repetitive tasks or discomfort, causing exercises to be performed carelessly or avoided completely. Introducing gamification to these repetitive tasks can make them more appealing to patients, ultimately increasing their motivation to exercise consistently. Objective: This study aims to iteratively develop a serious virtual reality game for hand and finger rehabilitation within an appealing and engaging digital environment, encouraging patient motivation for at least 2 weeks of continuous therapy. Methods: The development process comprised 3 distinct stages, each of which was subject to evaluation. Initially, a prototype was created to encompass the game's core functionalities, which was assessed by 18 healthy participants and 7 patients with impaired hand function. Subsequently, version 1 of the game was developed and evaluated with 20 patients who were divided into an investigation group and a control group. On the basis of these findings, version 2 was developed and evaluated with 20 patients who were divided into an investigation group and a control group. Motivation was assessed using the Intrinsic Motivation Inventory (IMI), while the application's quality was rated using the Mobile Application Rating Scale and the System Usability Scale. User feedback was gathered using semistructured interviews. Results: The prototype evaluation confirmed the acceptance and feasibility of the game design. Version 1 significantly increased motivation in 2 IMI subscales, effort (P<.001) and usefulness (P=.02). In version 2, a significant increase in daily performed exercises was achieved (P=.008) compared to version 1, with significantly higher motivation in the IMI subscale effort (P=.02). High Mobile Application Rating Scale scores were obtained for both versions 1 and 2, with version 2 scoring 86.9 on the System Usability Scale, indicating excellent acceptability. User feedback provided by the semistructured interviews was instrumental in the iterative development regarding improvements and the expansion of the playable content. Conclusions: This study presented a virtual reality serious game designed for hand and finger rehabilitation. The game was well received and provided an environment that effectively motivated the users. The iterative development process incorporated user feedback, confirming the game's ease of use and feasibility even for patients with severely limited hand function.
