Abstract:Recent studies have demonstrated the potential for myocardial regeneration at the time of myocardial infarction. Our lab focuses on understanding how stem cells home to injured tissue. We believe that a detailed analysis of the biological responses to tissue injury not only gives us insight into how damage occurs, but can also yield insight into how the body attempts to repair itself. In the case of the heart, we have shown that these "healing pathways" are only expressed for a short time following injury, and this may be why myocardial damage is usually irreversible. Furthermore, work suggests that deciphering these "healing pathways" and re-establishing their expression at time remote from injury offers an avenue for developing novel therapeutics that ultimately may allow us to orchestrate and exaggerate the healing process at time remote from tissue injury.

Abstract:Stem cell biology holds great promise for a new era of cell-based therapy, sparking considerable interest among scientists, clinicians, and their patients. However, the translational arm of stem cell science is in a relatively primitive state. Although a number of clinical studies have been initiated, the early returns point to several inherent problems. Abstract:Palliative surgery for congenital heart disease has allowed patients with previously lethal heart malformations to survive and, in most cases, to thrive. However, these procedures often place pressure and volume loads on the heart, and over time, these chronic loads can cause heart failure. Current therapeutic options for initial surgery and chronic heart failure that results from failed palliation are limited, in part, by the mammalian heart's low inherent capacity to form new cardiomyocytes. Surmounting the heart regeneration barrier would transform the treatment of congenital, as well as acquired, heart disease and likewise would enable development of personalized, in vitro cardiac disease models. Although these remain distant goals, studies of heart development are illuminating the path forward and suggest unique opportunities for heart regeneration, particularly in fetal and neonatal periods. Here, we review major lessons from heart development that inform current and future studies directed at enhancing cardiac regeneration.

Abstract:Although cardiac stem cells (CSCs) and tissue engineering are very promising for cardiac regenerative medicine, studies with model organisms for heart regeneration will provide alternative therapeutic targets and opportunities. Here, we present a review on heart regeneration, with a particular focus on the most recent work in mouse and zebrafish. We attempt to summarize the recent progresses and bottlenecks of CSCs and tissue engineering for heart regeneration; and emphasize what we have learned from mouse and zebrafish regenerative models on discovering crucial genetic and epigenetic factors for stimulating heart regeneration; and speculate the potential application of these regenerative factors for heart failure. Abstract:Some lower vertebrates such as zebrafish and axolotl have incredible cardiac regenerative potential while mammals have very limited ones. Comparative studies among species have revealed that cardiomyocyte polyploidy, endothermy, and injury-induced activation of certain transcriptional factors including AP1 complexes are critical for cardiomyocyte proliferation and heart regeneration during animal evolution. Gaining insights into these evolutionarily conserved mechanisms will likely lead to achieving heart regeneration in non-regenerative mammals including humans.

Abstract:Cardiomyocytes are terminally differentiated and are unable to proliferate in response to injury. Genetic modulation, cell transplantation and tissue engineering promise a revolutionary approach for myocardial regeneration and tissue repair after myocardial injury. Current data derived from animal models suggest that it may be possible to treat heart failure by inserting genetic materials or myogenic cells into injured myocardium. Success with animal models has raised the hope for new treatment after heart attacks and could prove an alternative to transplantation, particularly in elderly patients for whom there is often a lack of donor hearts. This exciting research, however, still faces significant difficulties before it can develop into a clinical therapeutic tool and many challenges need to be overcome before cell transplantation, gene therapy and tissue engineering can be considered efficient, therapeutic strategies for myocardial regeneration.

Abstract:Myocardial infarction, caused by a thrombus or coronary vascular occlusion, leads to irreversible ischaemic injury. Advances in early reperfusion strategies have significantly reduced short‐term mortality after myocardial infarction. However, survivors have an increased risk of developing heart failure, which confers a high risk of death at 1 year. The capacity of the injured neonatal mammalian heart to regenerate has stimulated extensive research into whether recapitulation of developmental regeneration programmes may be beneficial in adult cardiovascular disease. Restoration of functional blood and lymphatic vascular networks in the infarct and border regions via neovascularisation and lymphangiogenesis, respectively, is a key requirement to facilitate myocardial regeneration. An improved understanding of the endogenous mechanisms regulating coronary vascular and lymphatic expansion and function in development and in adult patients after myocardial infarction may inform future therapeutic strategies and improve translation from pre‐clinical studies. In this review, we explore the underpinning research and key findings in the field of cardiovascular regeneration, with a focus on neovascularisation and lymphangiogenesis, and discuss the outcomes of therapeutic strategies employed to date. © 2023 The Authors. Abstract:Ischemic heart disease is the leading cause of death globally, largely driven by inability of the adult heart to restore tissue structure and function after injury. However, the neonatal mouse heart is able to regenerate up to one week after birth, circumventing scarring and functional impairment. Neonatal heart regeneration is a spatially and temporally regulated process, involving coordinated communication of different cardiac cell types. Our previous work identified a regenerative cardiomyocyte population (CM4) that proliferates after myocardial infarction (MI), but the intercellular communication that induces their regenerative response is not fully understood. To study the cellular environment of CM4 cells during neonatal heart regeneration, we profiled spatial transcriptomes of regenerative P1 hearts at 3 days and 7 days after MI and compared them to nonregenerative P8 hearts. We identified unique spatial regions with distinct gene expression signatures, that we refer to as “molecular niches”, in the ischemic and border zones (IZ, BZ) of the P1 heart post-MI compared to P8 hearts. Cell type deconvolution of molecular niches showed a distinct temporal change of fibroblast and macrophage abundance in the IZ of P1 hearts compared to P8 hearts and also showed the localization of CM4 cells to the BZ of P1 hearts at 7 days post-MI. To identify intercellular communication that contributes to the differential injury response in P1 and P8 hearts, we analyzed ligand-receptor pairs that were expressed at higher levels in the IZ and BZ of P1 compared to P8 hearts. Among the nearly 50 signaling pairs identified, OSM and SLIT signaling have been previously implicated in heart regeneration. Notably, many of these signaling factors, including cytokines, chemokines and growth factors, have receptors expressed in multiple cardiac cell types. Thus, ongoing studies are systematically distilling the effect of each secreted factor on individual cell types in the heart using a cardiac cell co-culture and multiplexed single-cell RNA sequencing approach. In summary, our study provides insight into the spatial organization and tissue microenvironment of neonatal heart regeneration and identifies a number of candidate signaling pathways that potentially mediate critical intercellular communication during neonatal heart regeneration.

Abstract:The neonatal mammalian heart exhibits a remarkable regenerative potential, which includes fibrotic scar resolution and the generation of new cardiomyocytes. To investigate the mechanisms facilitating heart repair after apical resection in neonatal mice, we conducted bulk and spatial transcriptomic analyses at regenerative and non-regenerative timepoints. Importantly, spatial transcriptomics provided near single-cell resolution, revealing distinct domains of atrial and ventricular myocardium that exhibit dynamic phenotypic alterations during postnatal heart maturation. Spatial transcriptomics also defined the cardiac scar, which transitions from a proliferative to secretory phenotype as the heart loses regenerative potential. The resolving scar is characterized by spatially and temporally restricted programs of inflammation, epicardium expansion and extracellular matrix production, metabolic reprogramming, lipogenic scar extrusion, and cardiomyocyte restoration. Finally, this study revealed the emergence of a regenerative border zone defined by immature cardiomyocyte markers and the robust expression of Sprr1a. Taken together, our study defines the spatially and temporally restricted gene programs that underlie neonatal heart regeneration and provides insight into cardio-restorative mechanisms supporting scar resolution.

Abstract:The cost of tissue damage due to degenerative disease and injury is enormous in terms of health care costs, lost economic productivity, diminished quality of life and premature death. Advances in cell, developmental and molecular biology, and the discovery of regeneration-competent cells in many non-regenerating mammalian tissues, have given impetus to systematic investigations that will enable us to regenerate these tissues by cell transplantation or the pharmaceutical induction of regeneration from the body's own tissues. A significant avenue of research is the identification of the soluble and insoluble signals and their transduction pathways that govern the proliferation and differentiation of regeneration-competent cells, and the signals that inhibit their activity after injury. The most direct experimental strategy to identify the chemical and physical signals that promote regeneration and the factors that inhibit it is to make genomic and proteomic comparisons, using bioinformatic analyses, of regeneration-competent vs regeneration-deficient tissues. Once identified, the molecular promoters of regeneration can be used in clinical treatments. How far can we go in our quest for regeneration? We will probably be able to induce the regeneration of some tissues, such as skin or even spinal cord, within a few years. The regeneration of others, such as heart, lung, kidney or appendages, may be more complex and difficult, but we should not view them as impossible. They will just take a little longer.

Abstract:The stem cells keep us young by endogenously repairing us upon need. They do so bysmartly one step forward towards differentiation while another step backward to nurturethe undifferentiated stem cells. They are building blocks for every organ witha differential rate of repair of worn out tissues. Since stem cells can be cultured with a normal karyo type, they could be the ideal source for heart repair after myocardial infarction. As opposed to lower vertebrates and neonatal mice, cardiac regeneration in adult mammalian heart seems to be difficult to assess with a solid evidence of cytokinesis. It becomes more difficult to quantify the level of regeneration after myocardial infarction injury against a background of a large invasion of proliferating inflammatory cells. The question to beraised is how the renewal of a piece of myocardium follows the time line of picking upcell types in series: cardiomyocytes, endothelial cells, smooth muscle cells, fibroblast, pacemaker cells, conducting and Purkinje cells to bring the orchestration of rhythmically contracting and relaxing heart. This review focuses on where we are onthe status of heart repair and cardiac regeneration.

Abstract:Cardiovascular diseases are one of the leading causes of death globally. The primary pathological mechanism in myocardial infarction and heart failure is irreversible cardiomyocyte loss. The conventional knowledge is that mammals have a transient cardiac regenerative potential shortly after birth, but adult cardiomyocytes, as terminally differentiated cells, cannot regenerate. In contrast, some lower vertebrates, such as zebrafish, regenerate cardiac muscle throughout their lifetime. Zebrafish are extremely sensitive to cardiac injury and, within a short period, stimulate the proliferation of cardiomyocytes in numbers that fully regenerate to pre-injury levels; meanwhile, the initial scarring is gradually absorbed until there is little or no visible sign of fibrosis in the regenerating myocardium. The newborn mouse heart has the same remarkable regenerative potential before birth as one week after cardiac injury. The complex process of regeneration restores tissue structure through a series of cascade events, a coordinated process of cell proliferation, differentiation and dedifferentiation, and rearrangement of tissue morphogenesis. Understanding the ontogeny and development of cardiac regeneration and the molecular mechanisms of cardiac regeneration in zebrafish and mice is the cornerstone of regenerative biology and provides clues to investigate the cardiac repair process in adult mammals, resulting in extensive scarring rather than cardiomyocyte regeneration.

The utility of magnetic resonance imaging in cardiac tissue regeneration trials

Abstract:The past decade has seen the emergence of paradigm shifts in concepts involving cardiovascular tissue regeneration, including the idea that adult stem cells originate in hematopoietic or bone marrow cells, the belief that even adult organs, such as the heart and nervous system, are capable of post-mitotic regeneration, and the concept of inherent plasticity in cells that have undergone limited lineage differentiation. There has consequently been a flurry of proposed regenerative strategies, and safety and limited efficacy data from both animal and limited human trials have been presented. The drive to push these advances from the bench to the bedside has created a unique environment where the therapeutic agents, delivery approaches, and methods of measuring efficacy (often imaging technology) are evolving practically in parallel. The encouraging results of recent cell-therapy trials should therefore be assessed cautiously and in consonance with an understanding of the advantages and limitations of delivery strategies and end points. Arguably, the use of imaging technologies to evaluate surrogate end points might help overcome the difficulty posed by large sample sizes required for hard end point trials in cardiovascular therapeutics. Cardiac magnetic resonance imaging is one of the most sensitive techniques available to assess spatial and temporal changes following local or systemic therapies, and the availability of a bevy of complementary techniques enables interrogation of physiology, morphology, and metabolism in one setting. We contend that cardiac magnetic resonance imaging is ideally suited to assess response to myocardial regeneration therapy and can be exploited to yield valuable insights into the mechanism of action of myocardial regeneration therapies.
Leveraging chromatin state transitions for the identification of regulatory networks orchestrating heart regeneration

Abstract:The limited regenerative capacity of the human heart contributes to high morbidity and mortality worldwide. In contrast, zebrafish exhibit robust regenerative capacity, providing a powerful model for studying how to overcome intrinsic epigenetic barriers maintaining cardiac homeostasis and initiate regeneration. Here, we present a comprehensive analysis of the histone modifications H3K4me1, H3K4me3, H3K27me3 and H3K27ac during various stages of zebrafish heart regeneration. We found a vast gain of repressive chromatin marks one day after myocardial injury, followed by the acquisition of active chromatin characteristics on day four and a transition to a repressive state on day 14, and identified distinct transcription factor ensembles associated with these events. The rapid transcriptional response involves the engagement of super-enhancers at genes implicated in extracellular matrix reorganization and TOR signaling, while H3K4me3 breadth highly correlates with transcriptional activity and dynamic changes at genes involved in proteolysis, cell cycle activity, and cell differentiation. Using loss- and gain-of-function approaches, we identified transcription factors in cardiomyocytes and endothelial cells influencing cardiomyocyte dedifferentiation or proliferation. Finally, we detected significant evolutionary conservation between regulatory regions that drive zebrafish and neonatal mouse heart regeneration, suggesting that reactivating transcriptional and epigenetic networks converging on these regulatory elements might unlock the regenerative potential of adult human hearts.

Abstract:The heart is one of the least regenerative organs in humans, and ischemic heart disease is the leading cause of death worldwide. Understanding the cellular and molecular processes that occur during cardiac wound healing is an essential prerequisite to reducing health burden and improve cardiac function after myocardial tissue damage. By integrating single-cell RNA-sequencing with imaging-based spatial transcriptomics, we reconstructed the spatio-temporal dynamics of the fibrotic niche after ventricular injury in adult mice. Our analysis reveals dynamic regulation of local cell communication niches over time. We identified interactions that regulate cardiac repair, including fibroblast proliferation silencing by Trem2high macrophages that prevents excessive fibrosis. Moreover, we discovered a rare population of dedifferentiating cardiomyocytes during early post-lesion stages, which was sustained by signals from myeloid and lymphoid cells. Culturing non-regenerative mouse cardiomyocytes or human heart tissue with these niche factors reactivated progenitor gene expression and cell cycle activity. In summary, this spatio-temporal cell type atlas provides valuable insights into the heterocellular interactions that control cardiac repair.

Abstract:Heart failure (CHF) is the direct cause of death of a variety of cardiovascular diseases, culminating in irreversible myocardial necrosis and fibrosis. Traditional drug treatment, intervention, and surgical treatment have their own limitations. Increasing cardiac regeneration and repair of the injured heart is undoubtedly a promising approach to solve these clinical problems.
