Abstract:Poly-pharmacy means the use of multiple medications for multiple Diseases, with impact to the increases of the risk of drug-drug interactions (DDIs), and it may cause a threat to patient safety. Traditional DDI detection methods are often manual and leads to errors. This study investigates the potential of large language models (LLMs) to improve the efficiency of personalized DDI prediction and to use the AI advancements. By using LLMs’ natural language processing capabilities, we will develop a system that analyzes comprehensive patient data, including medical history, and individual characteristics. The system aims to enabling healthcare providers to make informed decisions and improve the treatment plans. Initial results, while promising, highlight the need for further refinement and larger datasets to improve prediction accuracy. However, this research demonstrates the significant potential of LLM-based systems in transforming medication safety, optimizing treatment regimens, and ultimately enhancing patient care and treatment process.

Abstract:Understanding and mitigating medication errors is critical for ensuring patient safety and improving patient care. Correctly identifying medication errors in the United States Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) reports can be difficult because of the complexities of medication error concepts. We took a user-centered design approach to support the medication error categorization workflow process with artificial intelligence (AI). We developed machine learning models to categorize medication error terms. The average F1-score, precision, recall, and area under the precision recall curve for 18 Medical Dictionary for Regulatory Activities (MedDRA) Lower Level Term (LLT) relating to medication errors were 0.88, 0.92. 0.85, and 0.83 respectively. We developed a framework to help evaluate opportunities for artificial intelligence integration in the medication error categorization workflow. The framework has four attributes: technical deployment, process rigidity, AI assistance, and frequency. We used the framework to compare two AI integration opportunities and concluded that the quality assurance (QA) opportunity to be a more feasible initial option for AI integration. We then extended these insights into the development and user testing of a prototype application. The user testing identified the highlighting and commenting capabilities of the application to be more useful and sliders and similar report suggestions to be less useful. This suggested that different AI interactions with human highlighting should be explored. While the medication error quality assurance prototype application was developed for supporting the review of direct FAERS reports, this approach can be extended to assist in the workflow for all FAERS reports.

Abstract:Errors in pharmacy medication directions, such as incorrect instructions for dosage or frequency, can increase patient safety risk substantially by raising the chances of adverse drug events. This study explores how integrating domain knowledge with large language models (LLMs)—capable of sophisticated text interpretation and generation—can reduce these errors. We introduce MEDIC (medication direction copilot), a system that emulates the reasoning of pharmacists by prioritizing precise communication of core clinical components of a prescription, such as dosage and frequency. It fine-tunes a first-generation LLM using 1,000 expert-annotated and augmented directions from Amazon Pharmacy to extract the core components and assembles them into complete directions using pharmacy logic and safety guardrails. We compared MEDIC against two LLM-based benchmarks: one leveraging 1.5 million medication directions and the other using state-of-the-art LLMs. On 1,200 expert-reviewed prescriptions, the two benchmarks respectively recorded 1.51 (confidence interval (CI) 1.03, 2.31) and 4.38 (CI 3.13, 6.64) times more near-miss events—errors caught and corrected before reaching the patient—than MEDIC. Additionally, we tested MEDIC by deploying within the production system of an online pharmacy, and during this experimental period, it reduced near-miss events by 33% (CI 26%, 40%). This study shows that LLMs, with domain expertise and safeguards, improve the accuracy and efficiency of pharmacy operations.

Abstract:Background: Medication errors, including dispensing errors, represent a substantial worldwide health risk with significant implications in terms of morbidity, mortality, and financial costs. Although pharmacists use methods like barcode scanning and double-checking for dispensing verification, these measures exhibit limitations. The application of artificial intelligence (AI) in pharmacy verification emerges as a potential solution, offering precision, rapid data analysis, and the ability to recognize medications through computer vision. For AI to be embraced, it must be designed with the end user in mind, fostering trust, clear communication, and seamless collaboration between AI and pharmacists. Objective: This study aimed to gather pharmacists' feedback in a focus group setting to help inform the initial design of the user interface and iterative designs of the AI prototype. Methods: A multidisciplinary research team engaged pharmacists in a 3-stage process to develop a human-centered AI system for medication dispensing verification. To design the AI model, we used a Bayesian neural network that predicts the dispensed pills' National Drug Code (NDC). Discussion scripts regarding how to design the system and feedback in focus groups were collected through audio recordings and professionally transcribed, followed by a content analysis guided by the Systems Engineering Initiative for Patient Safety and Human-Machine Teaming theoretical frameworks. Results: A total of 8 pharmacists participated in 3 rounds of focus groups to identify current challenges in medication dispensing verification, brainstorm solutions, and provide feedback on our AI prototype. Participants considered several teaming scenarios, generally favoring a hybrid teaming model where the AI assists in the verification process and a pharmacist intervenes based on medication risk level and the AI's confidence level. Pharmacists highlighted the need for improving the interpretability of AI systems, such as adding stepwise checkmarks, probability scores, and details about drugs the AI model frequently confuses with the target drug. Pharmacists emphasized the need for simplicity and accessibility. They favored displaying only essential information to prevent overwhelming users with excessive data. Specific design features, such as juxtaposing pill images with their packaging for quick comparisons, were requested. Pharmacists preferred accept, reject, or unsure options. The final prototype interface included (1) checkmarks to compare pill characteristics between the AI-predicted NDC and the prescription's expected NDC, (2) a histogram showing predicted probabilities for the AI-identified NDC, (3) an image of an AI-provided "confused" pill, and (4) an NDC match status (ie, match, unmatched, or unsure). Conclusions: In partnership with pharmacists, we developed a human-centered AI prototype designed to enhance AI interpretability and foster trust. This initiative emphasized human-machine collaboration and positioned AI as an augmentative tool rather than a replacement. This study highlights the process of designing a human-centered AI for dispensing verification, emphasizing its interpretability, confidence visualization, and collaborative human-machine teaming styles.

Abstract:Large language models (LLMs) are useful tools with the capacity for performing specific types of knowledge work at an effective scale. However, LLM deployments in high-risk and safety-critical domains pose unique challenges, notably the issue of ``hallucination,'' where LLMs can generate fabricated information. This is particularly concerning in settings such as drug safety, where inaccuracies could lead to patient harm. To mitigate these risks, we have developed and demonstrated a proof of concept suite of guardrails specifically designed to mitigate certain types of hallucinations and errors for drug safety, and potentially applicable to other medical safety-critical contexts. These guardrails include mechanisms to detect anomalous documents to prevent the ingestion of inappropriate data, identify incorrect drug names or adverse event terms, and convey uncertainty in generated content. We integrated these guardrails with an LLM fine-tuned for a text-to-text task, which involves converting both structured and unstructured data within adverse event reports into natural language. This method was applied to translate individual case safety reports, demonstrating effective application in a pharmacovigilance processing task. Our guardrail framework offers a set of tools with broad applicability across various domains, ensuring LLMs can be safely used in high-risk situations by eliminating the occurrence of key errors, including the generation of incorrect pharmacovigilance-related terms, thus adhering to stringent regulatory and quality standards in medical safety-critical environments.

Unlocking the potential of advanced large language models in medication review and reconciliation: A proof-of-concept investigation

Abstract:Background: Medication review and reconciliation is essential for optimizing drug therapy and minimizing medication errors. Large language models (LLMs) have been recently shown to possess a lot of potential applications in healthcare field due to their abilities of deductive, abductive, and logical reasoning. The present study assessed the abilities of LLMs in medication review and medication reconciliation processes. Methods: Four LLMs were prompted with appropriate queries related to dosing regimen errors, drug-drug interactions, therapeutic drug monitoring, and genomics-based decision-making process. The veracity of the LLM outputs were verified from validated sources using pre-validated criteria (accuracy, relevancy, risk management, hallucination mitigation, and citations and guidelines). The impacts of the erroneous responses on the patients' safety were categorized either as major or minor. Results: In the assessment of four LLMs regarding dosing regimen errors, drug-drug interactions, and suggestions for dosing regimen adjustments based on therapeutic drug monitoring and genomics-based individualization of drug therapy, responses were generally consistent across prompts with no clear pattern in response quality among the LLMs. For identification of dosage regimen errors, ChatGPT performed well overall, except for the query related to simvastatin. In terms of potential drug-drug interactions, all LLMs recognized interactions with warfarin but missed the interaction between metoprolol and verapamil. Regarding dosage modifications based on therapeutic drug monitoring, Claude-Instant provided appropriate suggestions for two scenarios and nearly appropriate suggestions for the other two. Similarly, for genomics-based decision-making, Claude-Instant offered satisfactory responses for four scenarios, followed by Gemini for three. Notably, Gemini stood out by providing references to guidelines or citations even without prompting, demonstrating a commitment to accuracy and reliability in its responses. Minor impacts were noted in identifying appropriate dosing regimens and therapeutic drug monitoring, while major impacts were found in identifying drug interactions and making pharmacogenomic-based therapeutic decisions. Conclusion: Advanced LLMs hold significant promise in revolutionizing the medication review and reconciliation process in healthcare. Diverse impacts on patient safety were observed. Integrating and validating LLMs within electronic health records and prescription systems is essential to harness their full potential and enhance patient safety and care quality.

Abstract:Introduction : Artificial intelligence or machine learning (AI/ML) based systems can help personalize prescribing decisions for individual patients. The recommendations of these clinical decision support systems must relate to the "label" of the medicines involved. The label of a medicine is an approved guide that indicates how to prescribe the drug in a safe and effective manner. Areas covered : The label for a medicine may evolve as new information on drug safety and effectiveness emerges, leading to the addition or removal of warnings, drug-drug interactions, or to permit new indications. However, the speed at which these updates are made to these AI/ML recommendation systems may be delayed and could influence the safety of prescribing decisions. This article explores the need to keep AI/ML tools 'in sync' with any label changes. Additionally, challenges relating to medicine availability and geographical suitability are discussed. Expert Opinion : These considerations highlight the important role that pharmacoepidemiologists and drug safety professionals must play within the monitoring and use of these tools. Furthermore, these issues highlight the guiding role that regulators need to have in planning and oversight of these tools. Artificial intelligence or machine learning (AI/ML) based systems that guide the prescription of medications have the potential to vastly improve patient care, but these tools should only provide recommendations that are in line with the label of a medicine. With a constantly evolving medication label, this is likely to be a challenge, and this also has implications for the off-label use of medicines.

Abstract:Medication errors continue to be the leading cause of avoidable patient harm in hospitals. This paper sets out a framework to assure medication safety that combines machine learning and safety engineering methods. It uses safety analysis to proactively identify potential causes of medication error, based on expert opinion. As healthcare is now data rich, it is possible to augment safety analysis with machine learning to discover actual causes of medication error from the data, and to identify where they deviate from what was predicted in the safety analysis. Combining these two views has the potential to enable the risk of medication errors to be managed proactively and dynamically. We apply the framework to a case study involving thoracic surgery, e.g. oesophagectomy, where errors in giving beta-blockers can be critical to control atrial fibrillation. This case study combines a HAZOP-based safety analysis method known as SHARD with Bayesian network structure learning and process mining to produce the analysis results, showing the potential of the framework for ensuring patient safety, and for transforming the way that safety is managed in complex healthcare environments.

Abstract:Aim This paper aims to explore the possibility of employing large language models (LLMs) – a type of artificial intelligence (AI) – in clinical pharmacology, with a focus on its possible misuse in bioweapon development. Additionally, ethical considerations, legislation, and potential risk reduction measures are analysed. Methods A comprehensive review of existing literature investigates the potential misuse of AI and LLMs in bioweapon creation. The search includes articles from PubMed, Scopus, and Web of Science Core Collection that were identified using a specific protocol. To explore the regulatory landscape, the OECD.ai platform was used. Results he review highlights the dual‐use vulnerability of AI and LLMs, with a focus on bioweapon development. Subsequently, a case study is used to illustrate the potential of AI manipulation resulting in harmful substance synthesis. Existing regulations inadequately address the ethical concerns tied to AI and LLMs. Mitigation measures are proposed, including technical solutions (explainable AI), establishing ethical guidelines through collaborative efforts, and implementing policy changes to create a comprehensive regulatory framework. Conclusion The integration of AI and LLMs into clinical pharmacology presents invaluable opportunities, while also introducing significant ethical and safety considerations. Addressing the dual‐use nature of AI requires robust regulations, as well as adopting a strategic approach grounded in technical solutions and ethical values following the principles of transparency, accountability, and safety. Additionally, AI's potential role in developing countermeasures against novel hazardous substances is underscored. By adopting a proactive approach, the potential benefits of AI and LLMs can be fully harnessed while minimizing the associated risks.

Abstract:Background: Medication-related harm has a significant impact on global healthcare costs and patient outcomes, accounting for deaths in 4.3 per 1000 patients. Generative artificial intelligence (GenAI) has emerged as a promising tool in mitigating risks of medication-related harm. In particular, large language models (LLMs) and well-developed generative adversarial networks (GANs) showing promise for healthcare related tasks. This review aims to explore the scope and effectiveness of generative AI in reducing medication-related harm, identifying existing development and challenges in research. Methods: We searched for peer reviewed articles in PubMed, Web of Science, Embase, and Scopus for literature published from January 2012 to February 2024. We included studies focusing on the development or application of generative AI in mitigating risk for medication-related harm during the entire medication use process. We excluded studies using traditional AI methods only, those unrelated to healthcare settings, or concerning non-prescribed medication uses such as supplements. Extracted variables included study characteristics, AI model specifics and performance, application settings, and any patient outcome evaluated. Findings: A total of 2203 articles were identified, and 14 met the criteria for inclusion into final review. We found that generative AI and large language models were used in a few key applications: drug-drug interaction identification and prediction; clinical decision support and pharmacovigilance. While the performance and utility of these models varied, they generally showed promise in areas like early identification and classification of adverse drug events and support in decision-making for medication management. However, no studies tested these models prospectively, suggesting a need for further investigation into the integration and real-world application of generative AI tools to improve patient safety and healthcare outcomes effectively. Interpretation: Generative AI shows promise in mitigating medication-related harms, but there are gaps in research rigor and ethical considerations. Future research should focus on creation of high-quality, task-specific benchmarking datasets for medication safety and real-world implementation outcomes.

The development of a custom-made large language model (LLM) (DELSTAR) trained on a wide range of internationally recognised scientific publication databases, pharmacovigilance sites and international product characteristics to help identify and summarise medication related information on delirium, as a proof-of-concept model, identified new facilitators and barriers for robust clinical pharmacy practice research. This technology holds great promise for the development of much more comprehensive prescribing guidelines, practice support applications for clinical pharmacy, increased patient and prescribing safety and resultant implications for healthcare costs. The challenge will be to ensure its methodologically robust use and the detailed and transparent verification of its information accuracy.

Abstract:The language used by physicians and health professionals in prescription directions includes medical jargon and implicit directives and causes much confusion among patients. Human intervention to simplify the language at the pharmacies may introduce additional errors that can lead to potentially severe health outcomes. We propose a novel machine translation-based approach, PharmMT, to automatically and reliably simplify prescription directions into patient-friendly language, thereby significantly reducing pharmacist workload. We evaluate the proposed approach over a dataset consisting of over 530K prescriptions obtained from a large mail-order pharmacy. The end-to-end system achieves a BLEU score of 60.27 against the reference directions generated by pharmacists, a 39.6% relative improvement over the rule-based normalization. Pharmacists judged 94.3% of the simplified directions as usable as-is or with minimal changes. This work demonstrates the feasibility of a machine translation-based tool for simplifying prescription directions in real-life.

Abstract:This qualitative method research aims to explain the use of artificial intelligence (AI) as an alternative means of optimizing interpersonal communication between doctors and pharmacists in terms of prescribing drugs. Because in practice, there are still many drug administration errors due to miscommunication between what the doctor writes or prescribes and what the pharmacist understands and gives to the patient. One that can be done by applying AI to electronic recipes. Unfortunately, the expansion of the use of AI technology in Indonesia in the pharmaceutical sector (especially drug prescribing) is still rarely found. In electronic prescribing, AI technology will help doctors to diagnose diseases suffered by patients through the analysis of previous medical reports. Then the doctor can directly send the prescription to the pharmacist through the computer. So that pharmacists can directly read, prepare and mix medicines. Through this mechanism, errors in prescribing that often occur can be minimized, thereby improving patient safety. The results of the study also show that although electronic prescribing can help overcome many medical problems, it can also cause new problems. For example electronic prescription systems can affect the workload of doctors and pharmacists by creating new types of prescribing errors such as delivery of prescriptions with medications, dosages or incorrect drug regimens. Not only that, but electronic prescribing systems can also raise concerns about information security and patient privacy when sharing prescription information with pharmacies. When sharing and storing patient prescription information, data security must be ensured to avoid changes that could potentially compromise the patient's health. The use of AI in electronic prescribing does not necessarily indicate clinical efficiency. Because it is also influenced by several factors, one of which is related to the knowledge and ability to use AI technology and the electronic prescribing system itself.

Abstract:The remarkable capabilities of Large Language Models (LLMs) make them increasingly compelling for adoption in real-world healthcare applications. However, the risks associated with using LLMs in medical applications have not been systematically characterized. We propose using five key principles for safe and trustworthy medical AI: Truthfulness, Resilience, Fairness, Robustness, and Privacy, along with ten specific aspects. Under this comprehensive framework, we introduce a novel MedGuard benchmark with 1,000 expert-verified questions. Our evaluation of 11 commonly used LLMs shows that the current language models, regardless of their safety alignment mechanisms, generally perform poorly on most of our benchmarks, particularly when compared to the high performance of human physicians. Despite recent reports indicate that advanced LLMs like ChatGPT can match or even exceed human performance in various medical tasks, this study underscores a significant safety gap, highlighting the crucial need for human oversight and the implementation of AI safety guardrails.

Abstract:The article examines the impact of artificial intelligence on scientific writing, with a particular focus on its application in hospital pharmacy. It analyses artificial intelligence tools that enhance information retrieval, literature analysis, writing quality, and manuscript drafting. Chatbots like Consensus, along with platforms such as Scite and SciSpace, enable precise searches in scientific databases, providing evidence-based responses and references. SciSpace facilitates the generation of comparative tables and the formulation of queries regarding studies, while ResearchRabbit maps the scientific literature to identify trends. Tools like DeepL and ProWritingAid improve writing quality by correcting grammatical, stylistic, and plagiarism errors. A.R.I.A. enhances reference management, and Jenny AI assists in overcoming writer's block. Python libraries such as langchain enable advanced semantic searches and the creation of agents. Despite their benefits, artificial intelligence raises ethical concerns including biases, misinformation, and plagiarism. The importance of responsible use and critical review by experts is emphasised. In hospital pharmacy, artificial intelligence can enhance efficiency and precision in research and scientific communication. Pharmacists can use these tools to stay updated, enhance the quality of their publications, optimise information management, and facilitate clinical decision-making. In conclusion, artificial intelligence is a powerful tool for hospital pharmacy, provided it is used responsibly and ethically.

For example more companies are looking towards cloud- based solutions, mobile applications, robotic automation, artificial intelligence and big data analytics as a vital part of clinical safety and regulatory operations in the pharmaceutical industry. Applying innovative technology automation tools and processes to PV strategies is now a critical requirement for managing the safety of pharmaceutical products. The role of artificial intelligence and machine learning in pharmacovigilance can enhance the productivity in identification, detection, management and reporting of ADRs. The main objective ofArtificial intelligence is meant to challenges to implementing intelligent automatic solutions include finding / having appropriate training data for machine learning models and the need for harmonised regulatory guidance. AI can analyse and interpret data at lightning speed, never gets tried or sick and can simply work by 24/7.
