Abstract:Objective The aim of this study was to explore the relationship between the cardiac function changes and depressive and anxiety symptom in depression patients.Methods The cardiac function indices including the left stroke index (LSI), ejection fraction (EF), heart rate (HR), diastolic pressure mean (DPM), systolic pressure mean (SPM), left ventricle diastolic volume (LVDV), effective circulating volume (ECV), resistance total mean (RTM) and blood flow smooth degree (BFSD) were determined in 65 patients with major depressive disorders and 31 healthy normal controls. The clinical symptoms were assessed with the Hamilton Depression Rating Scale (HAMD) and Hamilton Anxiety Rating Scale (HAMA).Results In patients with depression without anxiety, ECV, SPM, DPM was significantly lower than that in controls, while RTM higher than that in controls. The indexes of LVDV, LSI, EF, which represented left cardiac function, were decreased significantly in patients. The patients with comorbidity of depression and anxiety showed decreased ECV, LVDV, BFSD, and increased HR in comparison with the controls. The HAMD anxiety/somatization factor score was positively correlated with the LSI, RSI, EF, LVDV, whereas negatively correlated to RTM. There was negative correlation between retardation factor score and DPM, SPM, LVDV.Conclusions The study indicates that there are noticeable changes of left ventricle preload and afterlord, blood pressure, microcirculation, peripheral resistance in patients with depressive episode,and the accompanying anxiety would make it more complex.

Abstract:Objective: To analyze the sociodemographic and clinical factors related to anxiety in patients with major depressive disorder (MDD).Methods: This study involved a secondary analysis of data obtained from the Diagnostic Assessment Service for People with Bipolar Disorders in China (DASP), which was initiated by the Chinese Society of Psychiatry (CSP) and conducted from September 1, 2010 to February 28, 2011. Based on the presence or absence of anxiety related characteristics, 1,178 MDD patients were classified as suffering from anxious depression (n=915) or non-anxious depression (n=263), respectively.Results: Compared with the non anxious group, the anxious depression group had an older age at onset (t=-4.39, p<0.001), were older (t=-4.69, p<0.001), reported more lifetime depressive episodes (z=-3.24, p=0.001), were more likely to experience seasonal depressive episodes (chi(2)=6.896, p=0.009) and depressive episodes following stressful life events (chi(2)=59.350, p<0.001), and were more likely to have a family history of psychiatric disorders (chi(2)=6.091, p=0.014), Their positive and total scores on the Mood Disorder Questionnaire (MDQ) and the 32-item Hypomania Checklist (HCL-32) (p<0.05) were also lower. The logistic regression analysis indicated that age (odds ratio [OR]=1.03, p<0.001), a lower total MDQ score (OR=0.94, p=0.011), depressive episodes following stressful life events (OR=3.04, p<0.001), and seasonal depressive episodes (OR=1.75, p=0.039) were significantly associated with anxious depression.Conclusion: These findings indicate that older age, fewer subclinical bipolar features, an increased number of depressive episodes following stressful life events, and seasonal depressive episodes may be risk factors for anxiety related characteristics in patients with MDD.

Abstract:<span>Depression and anxiety often co-occur in coronary artery disease (CAD) patients undergoing percutaneous coronary intervention (PCI). This study was conducted to examine the predictive value of depression, anxiety, and their combination on the prognosis of Chinese PCI patients. A multicenter prospective study was undertaken with a sample of 309 primary PCI patients. The Hospital Anxiety and Depression Scale (HADS) was completed at baseline to assess anxiety and depression symptoms. Major adverse cardiovascular events (MACEs) were recorded for 12 months after PCI . Days from baseline to the first MACE was the outcome variable. MACEs occurred in 26 (8.4%) patients. After adjustments for socio-demographic and clinical characteristics, a Cox proportional hazards regression model revealed that depression (HR = 2.70, 95% confidence interval [CI] 1.22–5.95; p =.014) and anxiety (HR = 2.56, 95% CI 1.18–5.54; p =.017) <b>symptoms</b> were independent predictors of MACEs. A subanalysis showed that the cumulative event-free survival curves did not differ among depressed and anxious patients and depressed but non-anxious patients (Log Rank p =.52). In conclusion, depression is associated with an increased risk for MACEs post-PCI, independent of anxiety. Although anxiety is associated with MACEs, it has no additional predictive value when co-occurring with depression.</span>
Predictors of quality of life in Chinese patients with acute coronary syndrome.

Abstract:Predictors of quality of life in Chinese patients with acute coronary syndrome remain unknown. Between January 2006 and March 2009, a prospective cohort study of 782 patients with acute coronary syndrome was carried out, with follow-up 7 months later. Depression and anxiety symptoms were measured using the Hospital Anxiety and Depression Scale. Quality of life was assessed using the 36-Item Short-Form Health Survey. To identify predictors of decreased quality of life, both the physical and mental component summary scores were analyzed using multiple and ordinal logistic regression. In our quality-of-life model, diabetes mellitus was an independent risk factor for a low mental component summary score. Anxiety symptoms and the baseline mental component summary score were significant independent predictors of a low mental component summary score. Acute myocardial infarction was found to be a risk factor for a low physical component summary score. Anxiety symptoms, baseline physical component summary score, and systolic blood pressure on admission were significant independent predictive factors. Among patients with acute coronary syndrome, those with acute myocardial infarction, diabetes, or a low baseline quality of life need more care and effective intervention to improve their quality of life.

Abstract:Background: Depression and anxiety have been correlated with elevated risks for quality-of-life (QOL), adverse outcomes, and medical expenditure in patients with acute coronary syndrome (ACS). However, the relevant data are lacking for Chinese ACS populations, especially regarding different effects of major depression, anxiety, and comorbidity. The objective of this study was to evaluate the dynamic changes of depression and/or anxiety over 12 months and examine the effects of depression, anxiety, and comorbidity on QOL, adverse outcomes, and medical expenditure in Chinese patients with ACS. Methods: For this prospective longitudinal study, a total of 647 patients with ACS were recruited from North China between January 2013 and June 2015. Among them, 531 patients (82.1%) completed 12-month follow-ups. Logistic regression model was utilized for analyzing the association of baseline major depression, anxiety, and comorbidity with 12-month all-cause mortality, cardiovascular events, QOL, and health expenditure. Results: During a follow-up period of 12 months, 7.3% experienced non-fatal myocardial infarction (MI) and 35.8% cardiac rehospitalization. Baseline comorbidity, rather than major depression/anxiety, strongly predicted poor 12-month QOL as measured by short-form health survey-12 (odds ratio [OR]: 1.77, 95% confidence interval [CI]: 1.22-2.52, P = 0.003). Regarding 12-month non-fatal MI and cardiac re-hospitalization, baseline anxiety (OR: 2.83, 95% CI: 1.33-5.89, P < 0.01; OR: 4.47, 95% CI: 1.50-13.00, P < 0.01), major depression (OR: 2.58, 95% CI: 1.02-6.15, P < 0.05; OR: 5.22, 95% CI: 1.42-17.57, P < 0.03), and comorbidity (OR: 6.33, 95% CI: 2.96-13.79, P < 0.0001, OR: 14.08, 95% CI: 4.99-41.66, P < 0.0001) were all independent predictors, and comorbidity had the highest predictive value. Number of re-hospitalization stay, admission frequency within 12 months and medical expenditure within 2 months were the highest in patients with ACS with comorbidity. Conclusions: Major depression and anxiety may predict 12-month non-fatal MI and cardiac re-hospitalization. However, comorbidity has the highest predictive value with greater medical expenditure and worse QOL in Chinese patients with ACS. And depression with comorbid anxiety may be a new target of mood status in patients with ACS.
Anxiety and Clinical Outcomes of Patients with Acute Coronary Syndrome: a Meta-Analysis.

Abstract:ObjectivesAnxiety has been suggested to be associated with poor outcomes in patients with acute coronary syndrome (ACS). However, results of previous follow-up studies were inconsistent. The aim of this meta-analysis was to evaluate the association between anxiety and clinical outcomes in patients with ACS, and to investigate the potential role of depression underlying the above association.DesignA meta-analysis of prospective follow-up studies.SettingHospitals.ParticipantsPatients with ACS.InterventionsWe included related prospective follow-up studies up through 20 July 2019 that were identified by searching PubMed and Embase databases. A random-effect model was used for the meta-analysis. Anxiety was evaluated by validated instruments at baseline.Primary and secondary outcome measuresWe determined the association between anxiety and risks of mortality and adverse cardiovascular events (MACEs) in patients with ACS.ResultsOur analysis included 17 studies involving 39 038 patients wqith ACS. Anxiety was independently associated with increased mortality risk (adjusted risk ratio (RR) 1.21, 95% CI 1.07 to 1.37, p=0.002) and MACEs (adjusted RR 1.47, 95% CI 1.24 to 1.74, p<0.001) in patients with ACS. Subgroup analyses showed that depression may at least partly confound the association between anxiety and poor outcomes in patients with ACS. Adjustment of depression significantly attenuated the association between anxiety and MACEs (adjusted RR 1.25, 95% CI 1.04 to 1.52, p=0.02). Moreover, anxiety was not significantly associated with mortality risk after adjusting for depression (adjusted RR 0.88, 95% CI 0.66 to 1.17, p=0.37).ConclusionsAnxiety is associated with increased risk of mortality and MACEs in patients with ACS. However, at least part of the association may be confounded by concurrent depressive symptoms in these patients.
The combined effects of depression or anxiety with high-sensitivity C-reactive protein in predicting the prognosis of coronary heart disease patients

Abstract:Background: Depression, anxiety and high-sensitivity C-reactive protein (hs-CRP) are individually associated with poor prognosis in patients with coronary heart disease (CHD). However, the combined effects of depression with inflammation or anxiety with inflammation on the prognosis have been rarely explored. Methods: This prospective cohort study included 414 patients diagnosed with CHD. The Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7) were used to assess depression and anxiety. A score ≥ 5 points was defined as elevated depression or anxiety. High hs-CRP was defined as ≥ 3 mg/L. Follow-up was performed 2 years after the patients were discharged. The clinical results included noncardiac readmission, cardiac readmission, major cardiovascular events (MACEs), and composite events. The composite events included noncardiac readmission and MACEs. The Cox proportional hazard regression model was used to analyze the prognostic risk. Results: After full adjustment, patients with elevated depression and high hs-CRP had a higher risk in predicting noncardiac readmission (hazard ratio (HR) = 3.87, 95% confidence interval (CI) = 1.10-9.02, p = 0.002) and composite events (HR = 1.93, 95% CI = 1.13-3.30, p = 0.016) than those with high hs-CRP alone. For the anxiety and hs-CRP group, high hs-CRP alone predicted a higher risk of noncardiac readmission (HR = 3.32, 95% CI = 1.57-7.03, p = 0.002) and composite events (HR = 1.75, 95% CI = 1.12-2.76, p = 0.015) than references. Elevated anxiety had no significant effects on all the endpoints. Furthermore, we didn't find interactions between depression and hs-CRP or anxiety and hs-CRP. Conclusion: In patients with CHD, elevated depression with high hs-CRP was found to be significant in predicting the risk of noncardiac readmission and composite events. Early diagnosis and treatment of depression with inflammation are necessary in CHD patients.

Effects of Anxiety and Depression on Arterial Elasticity of Subjects with Suboptimal Physical Health.

Abstract:BackgroundThe authors investigated the effects of suboptimal health status (SHS; high-normal blood pressure, blood glucose, and blood lipids) on arterial elasticity in subjects with or without anxiety or depression.HypothesisSuboptimal physical health status and anxiety or depression increase the risk of cardiovascular diseases.MethodsThis was a cross-sectional, observational, multicenter study. Among 1520 subjects who underwent physical examination between May 2009 and December 2012 in Beijing and Chongqing, China, 955 were included. All subjects completed anxiety and depression questionnaires. Systemic vascular compliance (SVC), systemic vascular resistance, and brachial artery distensibility (BAD) were measured during arterial elasticity evaluation.ResultsOf 955 participants, 633 were classified as having SHS and 322 were classified as healthy. Systemic vascular compliance and BAD were worse in SHS subjects than in healthy subjects (SVC: 1.23 0.22 vs 1.29 +/- 0.25 mL/mm Hg; BAD: 6.26 +/- 1.32 vs 6.61 +/- 1.24%/mm Hg, respectively; both P < 0.05). Of 955 subjects, 37.7% and 43.9% had anxiety and depression, respectively. Systemic vascular compliance and BAD in SHS subjects with concomitant anxiety or depression were significantly lower than in SHS subjects without anxiety or depression (SVC: 1.22 +/- 0.23 vs 1.23 +/- 0.20 mL/mm Hg; BAD: 6.10 +/- 1.36 vs 6.33 +/- 1.20 %/mm Hg, respectively; both P < 0.05) and even lower than in healthy subjects.ConclusionsThough anxiety and depression had less impact on arterial elasticity in a healthy population, they may be involved in pathogenesis of vascular damage in the population with SHS.
Anxiety is associated with coronary microvascular dysfunction: Results from the CAMADA study

Abstract:ObjectiveCoronary microvascular dysfunction (CMD) is an important component of ischemic heart disease. Here, we assessed the associations between anxiety/depression and CMD using coronary microvascular function indicators. MethodsThe study included 81 patients (26 males and 55 females) with nonobstructive coronary artery disease. The symptoms of anxiety and depression were measured with Self-rating Anxiety Scale and Self-rating Depression Scale. Coronary microvascular function was assessed using coronary flow reserve (CFR) measured by transthoracic Doppler echocardiography. ResultsThe anxiety group had significantly lower CFR than that in the no-anxiety group (2.97 +/- 0.63 vs. 3.40 +/- 0.61, p = .029). In bivariate correlation analysis, anxiety was negatively associated with CFR (r = -.333, p = .002). In the multiple linear regression model, after adjusting for age, sex, BMI, hypertension, dyslipidemia, smoking status, and family history of premature coronary heart disease, anxiety was negatively associated with CFR (beta = -.314, p = .008). When both anxiety and depression were included in the multiple linear regression model, anxiety was also negatively associated with CFR (beta = -.345, p = .012). ConclusionAnxiety patients had lower CFR, anxiety was independently associated with CFR. Psychological disorders may play an important role in coronary microvascular dysfunction.
The Influencing Factors and Traditional Chinese Medicine Syndrome Characteristics of Depression/Anxiety in Patients with Coronary Heart Disease: A Cross-sectional Study

Abstract:Abstract Objective: The objective of the study was to investigate the situation of depression/anxiety in patients with coronary heart disease (CHD) at different stages of the disease and to analyze the influencing factors and the evolution characteristics of traditional Chinese medicine (TCM) syndromes. Materials and Methods: From October 2016 to April 2018, a cross-sectional survey was conducted at 48 clinical research centers in 23 provinces, cities, and autonomous regions across China. A total of 11383 cases were collected by outpatient or inpatient cases, including healthy individuals ( n = 1754), low-risk individuals ( n = 2339), metabolic syndrome ( n = 1475), stable CHD ( n = 3366), acute coronary syndrome ( n = 704), perioperative intervention treatment ( n = 753), and heart failure ( n = 992). Survey demographic data, lifestyle habits, disease and health status, TCM symptoms and signs, and other information were collected. Results: The prevalence rates of depression/anxiety in surveyed patients with CHD were 35.7% and 21.0%, respectively, and were higher than those in patients with metabolic syndrome (18.8% and 10.3%, respectively), low-risk individuals (11.7% and 7.5%), and healthy individuals (9.7% and 5.7%, respectively). The significant risk factors for CHD combined with depression analyzed by the generalized linear mixed model included age (odds ratio [OR] = 0.019), gender (OR = 0.632), hypertension (OR = 0.306), course of CHD (OR = 0.022), stent placement (OR = −0.284), heart function level (OR = −4.151/−3.336/−2.118), and phlegm stasis syndrome score (OR = 0.129). The significant risk factors for CHD combined with anxiety included gender (OR = 0.581), heart function level (OR = −1.856), and phlegm stasis syndrome score (OR = 0.094). Factor analysis was conducted on the symptoms and signs of patients with CHD combined with depression/anxiety, and 16 common factors were obtained with cumulative contribution rates of 62.83% and 66.13%, respectively. Disease syndromes included liver and kidney deficiency syndrome, liver stagnation and discomfort syndrome, Qi deficiency syndrome, liver meridian fire heat syndrome, kidney deficiency syndrome, phlegm dampness syndrome, heart and gallbladder Qi deficiency syndrome, blood stasis syndrome, lung Qi inversion syndrome, Yang deficiency syndrome, and three symptoms named diseases, including chest tightness, chest pain, insomnia, and head discomfort. Conclusions: Through cross-sectional design, the data obtained in this study revealed the actual situation of CHD patients with anxiety or depression at different stages. The influencing factors of CHD patients with depression or anxiety were analyzed through the collected cross-sectional information and further revealed the syndromic characteristics of CHD patients with depression or anxiety at different stages from the perspective of TCM syndromes. The data obtained provide a practical basis for further understanding the clinical characteristics of bicardiac diseases and for proposing treatment strategies in stages.

We evaluated the associations of CVH score with risk of incident depression/anxiety among ~0.4 million participants. During follow-up, 17,554 incident events with symptoms of either disorder were recorded. Per 100-point decrease in CVH score was associated with an increased risk of incident either disorder (Hazard ratio [HR] = 1.133, 95% confidence interval [CI]:1.114–1.153), depression (HR = 1.205, 95% CI:1.180–1.231), and anxiety (HR = 1.042, 95% CI:1.017–1.069). Per 100-point decrease in health assessments or health behaviors was associated with an increased risk of incident either disorder (HRhealth assessments = 1.085, 95% CI: 1.058–1.113, HRhealth behaviors = 1.217, 95% CI: 1.186–1.250). Poor CVH is a risk factor for the incident late-life depression/anxiety symptoms of middle-aged and older adults, and healthy behaviors could be targeted for the risk assessment and intervention of depression/anxiety.

Abstract:OBJECTIVE:To detect the risk factors of quality of life (QOL) in patients with acute coronary syndrome (ACS), so as to help doctors to recognize the risk population with impaired QOL.METHODS:403 hospitalized ACS patients, 252 males and 151 females, aged 67 12, 168 with ST-segment elevation myocardial infarction (STEMI), 4 with non-ST-segment elevation myocardial infarction (NSTEMI), and 74 with unstable angina pectoris (UAP), were surveyed with short-form-36 (SF-36) scale (Chinese version), including physical component summary (PCS) and mental component summary (MCS) so as to analyze the QOL, hospital anxiety depression scales (HADS) so as to detect the anxiety status. Multiple linear regression analysis was conducted to identify the variables associated with QOL.RESULTS:Of the 403 patients 84 (20.8%) suffered from anxiety, 43 (10.7%) suffered from depression, and 144 (35.7%) suffered from depression combined with anxiety symptoms . Age (r = -0.237, P < 0.001), being female (r = -0.183, P < 0.001), depression (r = -0.180; P < 0.001), and anxiety (r = -0.211, P <0. 001) were independent risk factors of physical QOL. Age (r = -0.117, P = 0.01), depression (r =-0.169, P = 0.004), and anxiety (r = -0.215, P < 0.001) were independently negatively correlated with mental QOL. Smoking was unexpectedly a protective factor for MCS (r = 0.157, P = 0.001).CONCLUSION:Depression and anxiety are common in patients diagnosed as with ACS, and appear to be related to significant worsening of QOL. The patients being female, with elder age, and suffering from depression and/or anxiety may be with impaired QOL, and need more care and effective intervention.

Abstract:Abstract Background Depression and anxiety are two common mood problems among patients with cardiovascular disease (CVD) and are associated with poor cardiac prognoses. The comorbidity of depression and anxiety is considered to be a more severe psychological status than non-comorbid mood disorders. However, little is known about the relationship between depression or anxiety and noncardiac readmission. We conducted a prospective study on the prognostic impact of depression, anxiety, and the comorbidity of the two among angina pectoris (AP) patients. Method In this prospective study, 443 patients with AP were included in the analysis. Follow-up assessments were performed 1 year, and 2 years after patient discharges. Clinical outcomes of interest included noncardiac readmission, major adverse cardiovascular events (MACEs), and composite events. Depression and anxiety symptom scores derived from the patient health questionnaire-9 (PHQ-9) and generalised anxiety disorder-7 (GAD-7) questionnaire were used to assess mood symptoms at baseline. Participants with symptom scores of ≥10 on both the depression and anxiety questionnaires formed the clinical comorbidity subgroup. We used multivariable Cox proportional hazards models to evaluate the impact of individual mood symptom and comorbidity on clinical outcomes. Results Among all the AP patients, 172 (38. 9%) were determined to have depression symptoms, 127 (28.7%) patients had anxiety symptoms and 71 (16.0%) patients suffered from their comorbidity. After controlling covariates, we found that patients who endured clinical depression (hazard ratio [HR] = 2.38, 95% confidence interval [CI] 1.06–5.33, p = 0.035) and anxiety ([HR] 2.85, 95% [CI] 1.10–7.45, p = 0.032) had a high risk of noncardiac readmission. Compared to participants with no mood symptoms, those with clinical comorbidity of depression and anxiety presented a greater risk of noncardiac readmission ([HR] 2.91, 95% [CI] 1.03–8.18, p = 0.043) MACEs ([HR] 2.38, 95% [CI] 1.11–5.10, p = 0.025) and composite event ([HR] 2.52, 95% [CI] 1.35–4.69, p = 0.004). Conclusion Depression and anxiety were found to have predictive value for noncardiac readmission among patients with AP. Furthermore, prognoses were found to be worse for patients with comorbidity of depression and anxiety than those with single mood symptom. Additional attention needs to be focused on the initial identification and long-term monitoring of mood symptom comorbidity.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Few data have reported the prevalence of depressive and anxiety symptoms in patients with cardiovascular disease (CVD) in China. Our study aimed to estimate the prevalence and related risk factors of these mental health symptoms.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A total of 47841 participants from seven regions of China were enrolled by a two-stage, stratified, community-based, clustering sampling strategy between 2014 and 2016. Data of sociodemographic status and medical history were collected through a standard questionnaire. The Center for Epidemiologic Studies Depression Scale and Zung's anxiety self-rating scale were used to screen depressive and anxiety symptoms.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Among 47588 individuals who completed the self-report questionnaires, the weighted prevalence of depressive symptom was 2.9% and that of anxiety symptom was 1.5%. In females with heart failure (HF) and stroke, prevalence of depressive and anxiety symptoms were 15.1% and 13.8%, respectively; while 9.4% and 8.4% for their male counterparts. Among patients with <span class="math"><math>≥</math></span> any 3 specific CVDs, the prevalence of having either depressive or anxiety symptoms were 13.1% and 6.8% for females and males, respectively. Younger age, female, unmarried, lower income, and disease history of atrial fibrillation, HF and stroke tend to link with higher risks of mental health symptoms.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Limitations</h3><p>Cross-sectional study.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>A high proportion of patients with CVD had depressive and anxiety symptoms. Screening for mental health symptoms is more important in higher-risk populations who are at younger age, being female, unmarried, with low income, and with diagnoses of atrial fibrillation, HF, and stroke.</p>

Abstract:Aims: To investigate the prevalence of anxiety and depression symptoms in intensive care unit (ICU) patients with cardiovascular disease (CVD) and to explore which elements are risk factors for the development of anxiety and depression symptoms. Design: A cross-sectional study. Methods: A total of 1028 ICU patients with CVD were enrolled in this cross-sectional study. Logistic regression was used to assess risk factors and associations between anxiety and depression symptoms, and mediation analysis was used to explore the effect of risk factors on the association between anxiety and depression symptoms. Reporting of the study followed the STROBE checklist. Results: The results showed that among ICU patients with CVD, 38.1% had anxiety symptoms, 28.7% had depression symptoms and 19.3% had both anxiety and depression symptoms, and there was a significant association between anxiety and depression symptoms. We also identified female gender, hypertension, hyperlipidemia and cardiac function class IV as independent risk factors for anxiety and depression symptoms. Importantly, these factors also mediated the association between anxiety and depression symptoms, emphasising their role in the psychological well-being of this patient group. Conclusion: ICU patients with CVD were prone to anxiety and depression symptoms. Female gender, hypertension, hyperlipidemia and cardiac function class IV were identified as independent risk factors that also served as mediators in the relationship between anxiety and depression symptoms. Especially, cardiac function class IV emerged as a critical factor in this association. Relevance to clinical practice: It is imperative for critical care professionals to recognize the elevated risk of depression and anxiety among ICU patients with severe CVD, especially those with cardiac function class IV, hypertension, hyperlipidemia and females. Proactive and supportive measures are essential for this vulnerable group during their ICU stay to safeguard their mental health and prevent negative outcomes. Abstract:Objective To investigate the level of anxiety and depression among patients at the early stage of acute myocardial infarction(AMI), to explore the factors predicting their psychological state, and to provide information for nurses to take preventive psychological measures for AMI patients.Methods A total of 136 AMI patients were evaluated with the Hospital Anxiety and Depression Scale and SF-12 health survey on the 5th, 6th or 7th day after the onset of AMI.Results Among 136 patients, the incidences of anxiety and depression were 52.94% and 16.18%. Patients′ physiological function,exercise habit and mental health in the past influenced the incidence of anxiety; patients′ vitality,social function and smoking in the past influenced the incidence of depression at the early stage of AMI(P<0.05,P<0.01).Conclusion The quality of life and lifestyle before the onset of AMI influence the incidences of anxiety and depression of patients at the early stage of AMI.Nurses should evaluate patients more comprehensively, and provide effective intervention to prevent and control patients′ negative psychological condition.

Abstract:Objective:To present a general data about Chinese patients′ depressive symptoms of acute coronary syndrome(ACS), and evaluate the impact of this kind of symptoms on their quality of life(QOL). Method: A total of 454 Chinese patients with ACS from Shanghai were enrolled. The patients′ baseline data were obtained within 1 week after hospital admission. The Chinese version of depression subscale of the Hospital Anxiety and Depression Scale(HADS-D) and SF-36 were used to assess depressive symptoms and their QOL, respectively. All the participants were divided into two main groups: HADd7 (n=212) and HADa≤7(n=242). Result: There were 46.7% of 454 ACS patients meeting the criteria of minor depression. The baseline characteristics, including the level of inflammatory factor and blood cholesterol, didn′t show a significant difference between two groups (P0.05). The participants from the group without depressive symptoms had a better QOL than depressive group(P0.001). And depressive symptoms were negatively associated with QOL(P0.001). Logistic regression analysis didn′t suggest that inflammatory factor was an important risk factor for depressive symptoms of ACS patients. Conclusion: This study confirmed the western notion that post-ACS depressive symptoms are common psychosocial problems, which also applies to Chinese patients diagnosed with ACS. Depressive ACS patients have rather worse quality of life.
