Abstract:Although obesity and insulin resistance are closely correlated, their temporal sequences in early life and influence on adult hypertension are largely unknown. This study aims to delineate the temporal relationship patterns between body mass index (BMI) and insulin in childhood and their impact on adult hypertension. The longitudinal cohort consisted of 990 adults (630 whites and 360 blacks) who had BMI and fasting insulin measured twice 5.4 years apart in childhood (mean age, 10.5 years at baseline and 15.9 years at follow-up) and blood pressure measured 14.7 years later in adulthood (mean age, 30.5 years). Cross-lagged panel and mediation analysis models were used to examine the temporal relationship between childhood BMI and insulin and its impact on adult hypertension. After adjusting for age, race, sex, and follow-up years, the cross-lagged path coefficient (β=0.33; P <0.001) from baseline BMI to follow-up insulin was significantly greater than the path coefficient (β=−0.02; P >0.05) from baseline insulin to follow-up BMI in childhood with P <0.001 for the difference in βs. Blacks and whites showed similar patterns of the temporal relationship. The path coefficient (β=0.59; P <0.001) from BMI to insulin in the hypertensive group was significantly greater than that (β=0.24; P <0.001) in normotensive group, with P <0.001 for the difference in βs between these 2 groups. The mediation effect of childhood insulin on the childhood BMI–adult hypertension association was estimated at 21.1% ( P <0.001). These findings provide evidence that higher BMI levels precede hyperinsulinemia during childhood, and this 1-directional relation plays a role in the development of hypertension.
Association Between High Birth Weight and Hypertension in Children and Adolescents: a Cross-Sectional Study in China

Abstract:This study aims to examine the relationship between high birth weight (HBW) and blood pressure and assess whether HBW leads to increased hypertension during childhood and adolescence. All the participants aged 6–18 years with HBW were selected from a cross-sectional study from seven provinces in China. Nine thousand nine hundred and sixty-two children were randomly sampled with matched data. Basic information of students was collected with a standardized student and guardian questionnaire. High blood pressure was defined according to sex-, age- and height-specific references. Multi-variance logistic regression was used to estimate the odds ratio (OR) of high blood pressure and HBW after adjustment for confounding factors. The overall mean of systolic BP and diastolic BP in HBW group were significantly higher than normal group in both genders and urban/rural area. Corresponding overall prevalence of high BP, elevated SBP and elevated DBP in HBW group were also higher than normal group in both genders, respectively (boys: 19.41% vs 16.16%; 10.12% vs 8.16%; 14.86% vs 12.71%; girls: 14.95% vs 12.66%; 8.19% vs 6.56%; 11.13% vs 9.86%). In addition, birth weight was positively associated with high BP, elevated SBP and elevated DBP in children and adolescents of both sexes (boys: OR 1.25, 1.27, 1.20; girls: OR 1.21, 1.27, 1.15). However, the positive association was attenuated and even reversed after adjustment for potential confounding variables. In this study, HBW may lead to higher childhood blood pressure, but no association between HBW and childhood hypertension was observed.

Abstract:With childhood hypertension emerging as a global public health concern, understanding its associated factors is crucial. This study investigated the prevalence and associated factors of hypertension among Chinese children. This cross-sectional investigation was conducted in Pinghu, Zhejiang province, involving 2,373 children aged 8–14 years from 12 schools. Anthropometric measurements were taken by trained staff. Blood pressure (BP) was measured in three separate occasions, with an interval of at least two weeks. Childhood hypertension was defined as systolic blood pressure (SBP) and/or diastolic blood pressure (DBP) ≥ age-, sex-, and height-specific 95th percentile, across all three visits. A self-administered questionnaire was utilized to collect demographic, socioeconomic, health behavioral, and parental information at the first visit of BP measurement. Abstract:Background and aims: We prospectively examined the association between three adiposity indices, including body mass index (BMI), waist circumference (WC), and percentage of body fat (PBF), and risk of hypertension in normal-weight Chinese children. Methods and results: The current study included 1526 (713 boys and 813 girls) normal-weight Chinese children (age 6-14 years old), who were free of hypertension at baseline (2014). Heights, body weight, WC, and PBF (estimated by bioelectrical impedance analysis) were measured at the baseline. Blood pressure was repeatedly measured in 2014, 2015 and 2016. Hypertension was defined as either high systolic blood pressure and/or high diastolic blood pressure, according to age-and sex-specific 95th percentile for Chinese children. We used Cox proportional hazards model to calculate the association between exposures and hypertension. We identified 88 incident hypertension cases during two years of follow up. High BMI was associated with high risk of developing hypertension after adjusting for potential confounders. The adjusted hazard ratio for hypertension was 2.88 (95% CI: 1.24, 6.69) comparing two extreme BMI quartiles. Each SD increase of BMI (approximate to 1.85 kg/m(2)) was associated with a 32% higher likelihood to developing hypertension (Hazard ratio = 1.32; 95% CI: 1.003, 1.73). In contrast, we did not find significant associations between WC or PBF and higher hypertension risk (p-trend > 0.2 for both). Conclusion: High BMI, but not WC and PBF, was associated with high risk of hypertension in normal-weight Chinese children. Abstract:BACKGROUND:Low birthweight (BW) and childhood growth have been hypothesized to be associated with an increased risk of hypertension in later life.METHODS:We analysed data among 13,467 women with a recalled BW from the Shanghai Women's Health Study. Cases included those with a self-reported hypertension with ('confirmed cases') or without ('possible cases') antihypertensive medication(s) use. Logistic regression was used to derive adjusted odds ratios (OR) and 95% CI.RESULTS:Birthweight was inversely associated with the odds of early onset (at age 20-40 years) hypertension in a dose response manner (P for trend = 0.01). This association is stronger for 'confirmed' hypertension (only OR for 'confirmed' hypertension are referred to subsequently). Being heavier or taller than average at 15 years of age were both related to elevated odds of early onset hypertension. Women who had a low BW but were heavier than average at age 15 were more than four times (OR = 4.63, 95% CI: 2.40-8.94) more likely to have an early onset hypertension, and those who had a low BW and became taller at 15 years of age had an OR of 1.87 (95% CI: 1.05-3.31). A significant interaction between BW and weight at age 15 was observed (P = 0.04).CONCLUSION:Our study suggests that low BW, particularly if accompanied by accelerated childhood growth, may increase the risk of early onset hypertension in adulthood.

Abstract:In this study, we examined the associations of general and central obesity and hypertension among Chinese children. Data was collected from 1626 children aged 7–16 years and their parents in four mega-cities across China. Mixed effect models examined associations of general and central obesity with hypertension, and between body mass index (BMI), waist circumference (WC), waist-to-height ratio (WHtR), systolic blood pressure (SBP) and diastolic blood pressure (DBP). The prevalence of general obesity, central obesity, and hypertension among the children was 11.1%, 19.7%, and 9.0%, respectively. More boys had general and central obesity than girls (15.2% vs. 6.9%; 27.4% vs. 11.7%, respectively; both P < 0.0001). Sex difference in hypertension rate was not statistically significant (9.3% in boys vs. 8.8% in girls, P = 0.7341). Both SBP and DBP were positively associated with BMI, WC, and WHtR, regardless of sex and region. General obesity (OR = 5.94, 95% confidence interval [CI]: 3.69–9.55) and central obesity (OR = 3.45, 95% CI: 2.27–5.23) were strongly associated with hypertension. The prevalence of general obesity, central obesity, and hypertension was high among Chinese children in the four mega-cities across China. Children’s BMI, WC, and WHtR were positively associated with their SBP and DBP. Obese children were 3–6 times more likely to have hypertension.

Impact of Short-Term Change of Adiposity on Risk of High Blood Pressure in Children: Results from a Follow-Up Study in China

Abstract:This study aimed to examine the impact of short-term adiposity change on risk of high blood pressure (HBP), and to assess the low limit range of body mass index (BMI) and waist-to-height ratio (WHtR) reduction proposed to decrease the HBP risk in children. Children were longitudinally surveyed at baseline and after a short-term follow-up. General obesity (GOB) is categorized by age and gender-specific BMI cut-off points, abdominal obesity (AOB) by WHtR. Logistic regression model was used to estimate relations between adiposity change and HBP risk with adjustment of covariates. A total of 28,288 children (median of baseline age:10 years) were involved with follow-up of 6.88±1.20 months. After the follow-up, 9.4% of the children had persistent general obesity (GOB), 2.8% converted from GOB to non-GOB, 0.9% had newly developed GOB. When compared with children remained non-GOB, children with continuous GOB status, newly developed GOB, converting from GOB to non-GOB had 5.03-fold (95%CI: 4.32~5.86), 3.35-fold (95%CI: 1.99~5.65), 2.72-fold (2.03~3.63) HBP risk, respectively. Similar findings were observed for abdominal obesity (AOB). Reduction of 0.21–0.88 kg/m2of baseline BMI (0.86–3.59%) or 0.009–0.024 of baseline WHtR (1.66–4.42%) in GOB or AOB children, respectively, was associated with significant decrease in HBP risk. Children with persistent obesity, newly developed obesity, or converting from obese to non-obese had significantly higher HBP risk. For children with GOB or AOB, reduction of <3.6% in BMI or <4.5% in WHtR could decrease the HBP risk.

Abstract:A total of 8898 Chinese children (4580 boys and 4318 girls) aged 7-13 years in 6 cities of east China were recruited. Data on height, weight, waist circumference, blood pressure, serum lipid profiles, glucose, and insulin were collected. The overall prevalence of hypertension was 11.1%. Overweight and obese children had a higher risk of developing hypertension than their counterparts (29.1%, 17.4%, and 7.8%, resp.) (P = 0.0001). The means levels of triglycerides, glucose, insulin, and HOMAIR (1.0 mmol/L, 4.5 mmol/ L, 8.4mU/ mL and 1.7, resp.) among hypertensive children were all significantly higher than their normotensive counterparts (0.8 mmol/L, 4.5 mmol/ L, 5.9mU/ mL, and 1.2, resp.) (P = 0.0001). Compared with the healthy children, the risk (odds ratio, OR) of having hypertension among children with high triglycerides, hyperglycemia, and metabolic syndrome was 1.4 (95% confidence interval (CI): 1.0-2.0, P = 0.0334), 1.5 (95% CI: 0.9- 2.5, P = 0.0890), and 2.8 (95% CI: 1.5-5.4, P = 0.0014), respectively, after controlling for age, gender, BMI, income level, parents' education level and puberty. In conclusion, overweight and obese children have higher risk of having hypertension and children with dyslipidemia, hyperglycemia, and metabolic syndrome and higher HOMA-IR have higher risk of developing hypertension.

Abstract:AimTo examine the effects of changes in body size phenotypes between childhood and adulthood on risks of diabetes and left ventricular hypertrophy (LVH) in adulthood.MethodsWe included 3,351 individuals who participated as both children and adults in the Bogalusa Heart Study.ResultsCompared with participants with persistently metabolically healthy normal weight (MHNW) from childhood to adulthood, MHNW children who became metabolically unhealthy in adulthood had increased diabetes burden and LVH risk in adulthood; Metabolically unhealthy normal weight (MUNW) children who became MHNW or metabolically healthy obese (MHO) as adults and individuals with persistent MHO from childhood to adulthood were not at increased risks of diabetes or LVH. The risks were increased if MHO during childhood transitioned to metabolically unhealthy obesity (MUO) by adulthood or MUO stayed from childhood to adulthood. MUO children who became MHO or MHNW as adults had decreased diabetes burden and LVH risk in adulthood.ConclusionsIndividuals maintained MHO from childhood to adulthood and MUNW children who became MHO as adults had a diabetes burden and LVH risk similar to individuals with persistent MHNW. Progression to metabolically unhealthy status and maintenance of metabolically unhealthy status, regardless of childhood BMI status, were associated with increased cardiometabolic outcomes.

Abstract:Objective: Understanding trends in blood pressure (BP) in childhood is crucial to addressing and reducing the burden of adulthood hypertension and associated mortality in the future. In view of growing obesity in Chinese children, we sought to investigate the trends in BP and the influence of body mass index (BMI) on them.Design, setting and participants: We included 1 010 153 children aged 8-17 years, with completed records from a large national successive cross-sectional survey, the Chinese National Survey on Students' Constitution and Health, between 1985 and 2010.Main outcome measurements: BP was measured according to the recommendation of the National High Blood Pressure Education Program Working Group, and the elevated BP was based on sex-, age-and height-specific 95th centile of the recommendation.Results: The adjusted mean systolic BP in boys and girls decreased by 3.9 and 5.6 mm Hg between 1985 and 2005, and increased by 1.3 and 1.0 mm Hg between 2005 and 2010, respectively. Corresponding adjusted prevalence of elevated systolic BP in boys and girls declined from 5.1% and 5.5% to 3.5% and 2.5% between 1985 and 2005, and increased to 4.9% and 3.5% in 2010, respectively. Adjusted mean BMI of boys and girls in 2010 was 2.0 and 1.2 kg/m(2) higher than those in 1985, respectively. The prevalence of obesity rose from 0% to 3.4% in boys and 0.9% in girls. Further adjusting for BMI did not change these trends in systolic BP. A similar pattern was also observed in diastolic BP.Conclusions: After declining for 20 years, BP levels in Chinese children started to climb upwards. These trends in BP cannot be fully explained by BMI. The investigation of other determinants of BP may provide additional opportunity to curb the current upward BP trend in Chinese children.
Prevalence of high blood pressure subtypes and its associations with BMI in Chinese children: a national cross-sectional survey

Abstract:Background Data on prevalence and characteristics of different high blood pressure subtypes are lacking among Chinese children. Regarding the mechanistic differences between isolated systolic high blood pressure and isolated diastolic high blood pressure and their different impact on end organ diseases, it is necessary to examine the prevalence of different high blood pressure subtypes in Chinese children and explore their associations with adiposity. Methods Data were derived from the baseline data of a multi-centered cluster randomized controlled trial involving participants from China. High blood pressure was defined according to age-, gender- and height-specific 95th percentile developed by the National High Blood Pressure Education Program Working Group. Body mass index was used to classify underweight, normal weight, overweight and obesity. Results The prevalence of HBP was 10.2% and 8.9% for boys and girls, respectively. Isolated systolic high blood pressure is the dominant high blood pressure subtype among Chinese boys aged 6–17 years and girls aged 12–17 years, while isolated diastolic high blood pressure was the most common high blood pressure subtype in girls aged 6–11 years. In boys, the status of overweight doubled the risk of isolated systolic high blood pressure (95% CI, 1.73, 2.31; P < 0.001) compared with the normal weight group, and the risk for obese children was 4.32 (95% CI, 3.81, 4.90; P < 0.001). The corresponding odds ratios in girls were 2.04 (95% CI, 1.68, 2.48, P < 0.001) for overweight, and 4.0 (95% CI, 3.36, 4.76, P < 0.001) for obesity. Similar patterns were also observed in the association between combined systolic and diastolic high blood pressure and adiposity. Conclusion The distribution of high blood pressure subtypes in boys differed from those in girls, and boys with adiposity showed a higher risk of high blood pressure than their female counterpart. Difference in strength of association between isolated diastolic high blood pressure and isolated systolic high blood pressure with body mass index was also found. These results may aid current strategies for preventing and controlling pediatric hypertension.
The Association of Gestational Age and Birth Weight with Blood Pressure among Children: a Chinese National Study.

Abstract:Gestational age and birth weight are supposed to associate with childhood blood pressure but remains unclear. This study aimed to investigate the association between gestational age, birth weight, and blood pressure among Chinese children. In all, 49 357 children aged 6–18 years were included from a nationwide survey in China. Gestational age, birth weight, and socioeconomic data were collected by questionnaires. Systolic (SBP) and diastolic (DBP) blood pressure were objectively measured. The associations between birth measures and blood pressure were examined by multivariable linear regression and logistic regression. The prevalence of hypertension was 19.1%, 19.2%, and 21.0% in preterm, term, and post-term subgroups, and 20.1%, 19.1%, and 19.8% in low-, normal-, and high-birth-weight subgroups, respectively. Results showed significantly positive associations of gestational age with SBP, DBP, SBP z-score, and DBP z-score in the overall and term subgroup, but not in the preterm or post-term subgroup. Birth weight was inversely related to SBP, DBP, SBP z-score, and DBP z-score across the entire birth-weight spectrum, and the overall association was stronger in girls than in boys. Similar associations were found in diffident age subgroups. Children with high birth weight had decreased odds of hypertension (OR 0.84, 95% CI 0.77–0.92) after adjustment for covariates. Preterm birth increased the risk of high SBP only in boys. This study suggested that gestational age was positively associated with blood pressure only in term-born children. Birth weight had a negative association with childhood blood pressure across the whole range of birth weight.

Abstract:Background: While both high birth weight and childhood overweight/obesity have been associated with a heightened risk of high blood pressure (BP) during childhood, the association between weight status change from birth to childhood and the risk of high BP has not been fully explored. This study aimed to investigate how changes in weight status from birth to childhood influence the development of high BP in childhood. Methods: The data for this study were obtained from the baseline survey of the Huantai Childhood Cardiovascular Health Cohort Study, which included 1237 children aged 6 to 11. Children with a birth weight under 2500 g or a body mass index (BMI) below the fifth percentile for their age and sex during childhood were excluded. Based on birth weight (high birth weight [>4000 g] vs. healthy [≤4000 g]) and childhood weight status (overweight [including obesity] vs. healthy weight), participants were categorized into four groups: consistently healthy weight, weight decrease, weight increase, and consistently excess weight. Results: Compared to children who maintained a healthy weight from birth to childhood, higher odds of childhood high BP was observed among those with consistently excess weight (odds ratio [OR] = 2.73, 95% confidence interval [CI] = 1.46–5.12) and those with a weight increase (OR = 2.77, 95% CI = 1.91–4.02). In contrast, children with a weight decrease did not exhibit significantly higher odds of childhood high BP (OR = 0.94, 95% CI = 0.36–2.45). Conclusion: Children who become overweight in childhood or who consistently had excess weight from birth were at higher risk of childhood high BP. However, the risk of high BP in childhood may be mitigated or eliminated in individuals with high birth weight who achieve a healthy weight by childhood.

Abstract:AIM:The aim of this study was to quantify the relationships of height, weight and body mass index (BMI) with blood pressure (BP) levels in Chinese children and adolescents. METHODS:Height, weight, BMI and BP measurements were obtained from a nationally representative sample of 231,227 children aged 7-18 years. Body size measurements were converted to age- and sex-specific standard deviation (SD) scores. The relationships between body size and BP measurements were assessed using linear regression analysis. RESULTS:All body size measurements were significantly associated with BP levels. Systolic BP increased 4.14, 3.70 and 2.88 mmHg in boys and 2.98, 2.63 and 1.87 mmHg in girls, corresponding to 1 SD increase in weight, BMI and height, respectively. A similar pattern was also observed for diastolic BP. After adjustment for height, systolic BP increased substantially with increasing weight (3.96 mmHg/SD increase for boys and 2.92 mmHg for girls). With adjustment for weight, systolic BP increased slightly with 1 SD increase in height (0.27 mmHg for boys and 0.10 mmHg for girls). The strength of the association between a body size measurement and BP varied among different ages, peaked at 10-11 years in girls and around 12-14 years in boys. CONCLUSIONS:Weight is the most powerful driving force of BP in children and adolescents, followed by BMI and height. Height has little impact on BP in children with a given weight, while weight has considerable impact on BP in children with a given height.
Comparison of child adiposity indices in prediction of hypertension in early adulthood

Abstract:We aimed to compare child body mass index (BMI) in prediction of hypertension in early adulthood with 4 other adiposity indices (waist circumference [WC], waist circumference-to-height ratio [WHtR], waist-to-hip ratio [WHR], and triceps skinfold [TSF]). The cohort from the China Health and Nutrition Survey 1993-2011 consisted of 1444 adults aged 18-36 years who were examined in childhood and early adulthood. Child adiposity indices and adult blood pressure (BP) were transformed into age-, sex-, and survey year-specific Z-scores. Adult hypertension was defined as BP ≥130/80 mm Hg as per the 2017 American College of Cardiology/American Heart Association guidelines. Adult hypertension prevalence was 32.9% during a mean follow-up of 10.1 years. Childhood BMI showed stronger correlation with adult BP than WHR and TSF (PS for difference <.05). Child BMI showed the better prediction of adult hypertension compared with WHtR, WHR, and TSF using area under the receiver operating characteristic curves (PS for difference <.05). Per SD change in the predictor, child BMI (relative risk [95% confidence interval], 1.11 [1.04-1.18]) and WC (1.12 [1.05-1.20]) were significantly associated with adult hypertension using covariate-adjusted Poisson models with robust standard errors. Child BMI performed equally or better compared with 4 other adiposity indices in predicting adult hypertension.

Abstract:We conducted a retrospective cohort study of adult participants from the China Multicenter Collaborative Study of Cardiovascular Epidemiology to explore the relationship between early adulthood weight status, body weight change from age 25 to middle age and hypertension in the Chinese population. Self-reported weight was used to calculate the body mass index (BMI) at age 25. The association between BMI at age 25, subsequent weight change and hypertension was examined using a logistic regression model. Of the 14 635 study participants sampled in 1998, 3525 were diagnosed with hypertension. After adjusting for age, sex and other potential confounding factors, both the BMI at age 25 and weight gain were positively associated with the risk of hypertension ( P for trend <0.001). Weight loss could more effectively reduce the risk of hypertension for participants with a higher BMI (BMI⩾24 kg m − 2 ) at age 25 than for those who experienced a weight change of −2.5–2.5 kg. The odds ratio (OR) and 95% confidence interval (CI) for a weight change of −7.5 to −2.6 kg were 0.63 (0.46–0.86); for a weight change of <−7.5 kg, these statistics were 0.45 (0.32–0.63). For participants who had a lower BMI (18.5–20.9 kg m − 2 ) at age 25, the OR and 95% CI were 0.86 (0.58–1.26) for a weight change of −7.5 to −2.6 kg and 0.95 (0.44–2.05) for a weight change of <−7.5 kg. Being overweight and obesity in early adulthood and adult weight gain were both independently associated with marked increase in the risk of hypertension in middle-aged men and women. Weight loss may be a protective factor against hypertension for people who were overweight or obese in early adulthood.

Abstract:As childhood obesity rates increase worldwide, the prevalence of obesity-related hypertension is also on the rise. Obesity has been identified as a significant risk factor for hypertension in this age group. National Health Surveys and meta-analyses show increasing trends in obesity and pediatric hypertension in obese children. The diagnosis of hypertension in children involves percentiles relative to age, sex, and height, unlike in adults, where absolute values are considered. Elevated blood pressure (BP) in childhood is consistently associated with cardiovascular disease in adulthood, emphasizing the need for early detection and intervention. The pathogenesis of hypertension in obesity involves multiple factors, including increased sympathetic nervous system activity, activation of the renin-angiotensin-aldosterone system (RAAS), and renal compression due to fat accumulation. Obesity disrupts normal RAAS suppression and contributes to impaired pressure natriuresis and sodium retention, which are critical factors in the development of hypertension. Risk factors for hypertension in obesity include degree, duration, and distribution of obesity, patient age, hormonal changes during puberty, high-sodium diet, sedentary lifestyle, and socioeconomic status. Treatment involves lifestyle changes, with weight loss being crucial to lowering BP. Medications such as angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers may be considered first, and surgical approaches may be an option for severe obesity, requiring tailored antihypertensive medications that consider individual pathophysiology to avoid exacerbating insulin resistance and dyslipidemia.
Systematic Evaluation of Adiposity Indices as Predictors of Hypertension Risk in Children and Adolescents

Abstract:Objective To systemically examine the concomitant impact of adiposity indices on hypertension in children and adolescents. Methods A community-based cohort study by China Health and Nutrition Survey (2009-2011) included 262 children and adolescents aged 7-17. Anthropometric and lipid profiles were analyzed for hypertension risk using logistic and cross-lagged models. Results Of the 227 participants (mean age, 16.5 [2.3] years), 147 (53.9%) were boys. Over a two-year period, 26.4% developed hypertension, 5.3% had elevated systolic blood pressure (SBP), and 25.6% had elevated diastolic blood pressure (DBP). After adjusting for covariates, higher body mass index (BMI), waist circumference (WC), hip circumference (HC), triceps skinfold thickness (TST), and body roundness index (BRI) were associated with increased hypertension risk. Incorporating these measures into a BP-based model improved the AUC for hypertension prediction from 0.551 (95% CI: 0.463, 0.640) to 0.670 (95% CI: 0.592, 0.748). A longitudinal relationship were observed between these adiposity indices and hypertension symptoms. Conclusions The study suggests anthropometry parameters are superior to blood lipid indicators in predicting the occurrence of hypertension in children. Abdominal obesity, as indicated by WC, HC, and BRI, accounts for a significant portion of the risk for hypertension, particularly in children aged 7 to 12 years.

Abstract:Objective In order to find out the relationship between birth weight and systolic and diastolic blood pressure in preschool children.Methods Date for 4 642 healthy preschool children aged 2 to 6 were collected.All these children were born in Shanghai Minhang Maternal and Child Health Hospital.After rest in the sitting position for 5-10 min,the blood pressure(BP) and heart rate(HR) were measured on the right arm at the level of the heart,using appropriate size of cuff,by automated BP recorder(Model 45NEO-E6,Welch Allyn,USA).Elevated blood pressure was defined as systolic blood pressure(SBP) and/or diastolic blood pressure(DBP) equal or greater than the 95th percentile for gender and age,according to the reference based on Japanese preschool children(1997).Low birth weight was defined as < 2 500 g and macrosomia as ≥4 000 g.Multiple regression models were used to analyze the association between birth weight and childhood hypertension.Results In total 4 642 children were analyzed in this study,with 2 458(52.9%) being boys.The proportions of low birth weight and macrosomia were 3.2%(148/4 642) and 5.4%(251/4 642),respectively.The prevalence rate of hypertension was more common in boys than girls(9.4% vs 6.9%,P<0.05).Trend of negative correlation between birth weight and childhood DBP was observed in children with birth weight over 4 000 g.With gender,age,heart rate,body height and body weight adjusted,the relationship between birth weight and childhood hypertension was not significant in either low birth weight or macrosomia group.Rapid heart rate may increase the risk of hypertension at childhood(OR=2.55,95% CI:1.96-3.33).Conclusion Blood pressure levels of preschool children were not significantly associated with birth weight.Rapid heart rate may increase the risk of hypertension.

Abstract:Objectives. To identify body mass index (BMI) trajectories in Chinese children and to compare the risk of incident high blood pressure (HBP) across trajectory groups. Methods. A total of 9286 children were included. The mean age at baseline was 8.9 years; age at endpoint ranged between 16 and 18 years. At least 8 measurements were obtained from each involved child. We used group-based trajectory modeling to identify BMI trajectory groups in each sex. We used blood pressure from each measurement to define HBP. Results. We identified 4 BMI trajectories for each sex. Compared with the low trajectory group, the hazard ratios of HBP in the higher trajectory groups ranged from 1.17 (95% confidence interval [CI] = 1.11, 1.23) to 2.00 (95% CI = 1.78, 2.27) during follow-up, and HBP risk at late adolescence ranged from 1.36 (95% CI = 1.22, 1.52) to 3.63 (95% CI = 3.12, 4.21). All trend P values across trajectories were less than .001. In terms of population level, overweight started 3 years earlier than HBP. Conclusions. Children of higher BMI trajectories had a higher risk of HBP during adolescence. The transition period from overweight to HBP onset could be critical for HBP prevention.
