Abstract:Aim To analysis of patients with non-ST elevation acute coronary syndrome in 2008,and scale the gap between real world pratice and evidence-based guidelines.Methods Clinical data of the dianosis and management of 373 patients with non-ST elevation acute coronary syndrome from January to December in 2006,and scale the gap between real world pratice and evidence-based guidelines.Results Elevated cardiac markers,chest X-ray,echocardiography,dynamic electrocardiogram,exercise electrocardiograph (ECG),coronary angiography were used more extensively,but the usage of cardiac stress imaging and electron-beam computed tomography was low.Significant increases in the use of coronary angiography,aspirin,using 300 mg first dosage of aspirin,β-blocker,angiotensin converting enzyme inhibitor (ACEI),thienopyridine,statins and invasive strategies such as percutaneous coronary intervention (PCI) (51.1%),early invasive intervention occurred.The utilization rate of morphine sulfate,nitrates,unfractionated heparin were low.Conclusions There are great progress in the dianosis and management of patients with non-ST elevation acute coronary syndrome.Still,further improvements are needed for optimal implementation of these guidelines such as the use of β-blocker,adding dosage of β-blocker,platelet glycoprotein Ⅱb/Ⅲa inhibitor,low-molecular-weight heparin.Prompt and effective measures should be taken to enhance the education of evidence-based guidelines.

Abstract:The development of percutaneous coronary intervention (PCI) has been one of the greatest advances in cardiology and has changed clinical practice for patients with coronary artery disease (CAD). Despite continuous improvements in operators' experience, techniques, and the development of new-generation devices, significant challenges remain in improving the efficacy of PCI, including calcification, bifurcation, multivascular disease, stent restenosis, and stent thrombosis, among others. The present review aims to provide an overview of the current status of knowledge of endovascular revascularization in CAD, including relevant trials, therapeutic strategies, and new technologies addressing particular scenarios that can impact the prognosis of this vulnerable population.

Abstract:Cardiology and all its subspecialties continue to push the envelope in developing new treatment strategies for a wide variety of diseases. After screening more than 1300 articles, we highlight a selection of important cardiology articles published in 2023. Starting with prevention, we note articles that look at the effect of semaglutide in patients with obesity as well as a first-in-class drug, bempedoic acid, on cardiovascular outcomes. We have also examined new evidence comparing conservative management with invasive management of frail, older patients with non-ST-segment elevation myocardial infarction (NSTEMI). In patients with cardiac arrest secondary to NSTEMI, another article examines the rationale for expedited transfer to a cardiac arrest center. The STREAM-2 (Strategic Reperfusion in Elderly Patients Early After Myocardial Infarction) trial builds on looking at half-dose thrombolysis in older populations with STEMI. Emphasis is placed on guideline-directed medical therapy before hospital discharge in those with heart failure. In addition, in patients with stable symptomatic coronary artery disease, initial noninvasive testing using coronary computed tomography angiography may be a viable option compared with invasive strategies. More details have emerged on anticoagulation strategies in those with device-detected atrial fibrillation. Finally, transcatheter approaches to treat both mitral and tricuspid regurgitation have also been included.

Abstract:Acute coronary syndromes contribute a substantial part of the global disease burden. To realize a reduction in mortality and morbidity, the management of patients with these conditions involves the integration of several different approaches. Timely delivery of appropriate care is a key factor, as the beneficial effect of reperfusion is greatest when performed as soon as possible. Innovations in antithrombotic therapy have also contributed significantly to improvements in the prevention of ischaemic complications. However, with the use of such treatment, an increase in the risk of bleeding is inevitable. Therefore, the greatest challenge is now to obtain an optimal balance between the prevention of ischaemic complications and the risk of bleeding. In this regard, identification of patients at highest risk of either one is essential.

Abstract:This issue of Acta Cardiologica is devoted to acute and chronic coronary artery disease (CAD). Acute CAD, which includes acute coronary syndrome (ACS) and sudden cardiac death (SCD), manifests with a sudden reduction or cessation of blood flow due to thrombus formation on ruptured plaques [ Citation 1 , Citation 2 ]. Management of acute CAD involves rapid risk assessment, reperfusion therapy, and initiation of antiplatelet medications [ Citation 3–5 ]. Chronic CAD, characterised by stable angina and silent ischaemia, results from progressive atherosclerotic plaque development. Treatment of chronic CAD focuses on symptom relief, risk factor modification, and prevention of disease progression. Lifestyle interventions, pharmacotherapy, and revascularization procedures may be employed to manage chronic CAD effectively [ Citation 6–9 ]. Both acute and chronic CAD require comprehensive management strategies to improve patient outcomes and reduce the risk of adverse cardiovascular events. Timely intervention in acute CAD can minimise myocardial damage and prevent complications such as myocardial infarction [ Citation 10 ]. In contrast, chronic CAD management aims to alleviate symptoms, improve quality of life, and prevent recurrent ischaemic events [ Citation 11 , Citation 12 ]. Overall, a multifaceted approach involving medical therapy, lifestyle modifications, and invasive procedures is essential for optimising outcomes in patients with CAD.

Abstract:Cardiovascular diseases are a major cause of death worldwide, and coronary heart disease (CHD) is a prevalent cardiovascular condition and a significant health burden for the population. In this disease, insufficient blood flow to the heart due to plaque accumulation in the coronary arteries causes chest pain, heart attack, and even death. So, it is vital to identify risk factors, prevention, appropriate treatment, and rehabilitation. Nurses play an indispensable role in managing and caring for patients with CHD. Indeed, they possess a deep understanding of the disease and its complexities, enabling them to provide comprehensive care to patients. Nurses monitor vital signs, administer medications, and perform diagnostic tests, ensuring patients receive timely and appropriate interventions. They also educate patients and their families about CHD, emphasizing lifestyle modifications, medication adherence, and self-care practices. Moreover, nurses offer emotional support, guiding patients through the physical and psychological challenges associated with CHD. Their expertise, compassion, and dedication significantly improve patient outcomes and overall quality of life. Nurses are responsible for assessing, diagnosing, and counseling patients on how to manage their disease, making them the front line of defense in preventing and addressing this serious condition. In the current study, we reviewed the literature to consider the best practices and emerging trends in nursing interventions and care strategies for patients with CHD.

Treating to protect: current cardiovascular treatment approaches and remaining needs

Abstract:Current best practice to reduce cardiovascular disease involves evaluating patients' global cardiovascular risk profiles and devising treatment strategies accordingly. Despite the proven efficacy of this approach, very few physicians are adequately assessing risk, and consequently patients are failing to achieve desired treatment targets. Modifying lifestyle factors, such as diet, exercise, and cessation of smoking, remains one of the simplest and most potent means of reducing risk. Newly emerging evidence suggests that moderate physical activity (such as brisk walking for 30 minutes a day), eg, by raising levels of circulating endothelial progenitor cells, improves endothelial function and enhances vascular repair. However, patients remain remarkably reluctant to lifestyle changes, even in the face of overt, life-threatening disease. Statin treatment reduces cardiovascular morbidity and death in both primary and secondary prevention studies. However, over 90% of adults at high risk for coronary heart disease fail to achieve target low-density lipoprotein cholesterol levels in spite of statin therapy. Similarly, only about 37% of patients with hypertension meet blood pressure targets. Antihypertensive drugs achieve different levels of cardioprotection. Mounting evidence links regimens containing beta-blockers or diuretics with higher incidence of type 2 diabetes. In contrast, angiotensin-converting enzyme inhibitors and angiotensin receptor blockers appear to confer extra protection on target organs on top of blood pressure reduction. The ONTARGET Trial Program is designed to clarify the importance of this effect. Educating patients, raising physicians' awareness, and implementing effective and safe treatment regimens are all necessary steps to bring about the much-needed improvements in cardiac health outcomes.

Heart disease covers a wide range of structural and functional changes that affect the heart, including conditions like coronary artery disease, abnormal heart rhythms, congenital heart defects, valve disease, and heart failure (HF). In many cases, less severe forms of heart disease have the potential to advance or intersect with other conditions, possibly leading to a more significant pathological outcome, including severe HF. Heart failure is a significant public health challenge due to its high prevalence and severe impact on patients and healthcare systems. It affects about 2% of the global population and is expected to increase due to ageing populations and improved survival rates from other cardiac conditions. By 2030, it is projected that nearly 50% of the population over 60 years of age will be affected by HF [1,2]. Cardiac research has increasingly focused on exploring new and innovative approaches to better understand the biological basis of cardiac disease onset and progression, as well as to develop novel therapeutic approaches that target molecular pathways involved in the transition to pathophysiological conditions.This Special Issue of the International Journal of Molecular Science features four research articles and six reviews that cover various aspects of cardiac pathophysiology. These contributions include studies of basic research and therapeutic and functional approaches. They also provide new insights into potential molecular targets for therapeutic strategies and new diagnostic and prognostic markers in heart disease.Two reviews specifically address different aspects of acute myocardial infarction (MI). In particular, Salvadori and colleagues thoroughly analyse and critically discuss the latest research on epigenetic changes following myocardial infarction (MI). They specifically examine the role of miRNAs and their interaction with important signalling pathways in the different early stages of the healing process after an ischemic event, namely the inflammation, resolution, and maturation phases [3]. Additionally, they shed light on the differences in the risk factors and long-term outcomes for MI between males and females.Zheng et al. revisit the role of vitamin C, a well-established antioxidant that reduces oxidative stress [4]. The beneficial role of vitamin C in the cardiovascular system remains controversial, and previous studies on the association between vitamin C levels and MI have shown conflicting results. Abstract:Acute coronary syndrome (ACS) remains a major cause of morbidity and mortality worldwide, requiring ongoing efforts to identify novel therapeutic targets to improve patient outcomes. This manuscript reviews promising therapeutic targets for ACS identified through preclinical research, including novel antiplatelet agents, anti-inflammatory drugs, and agents targeting plaque stabilization. Preclinical studies have expounded these agents' efficacy and safety profiles in mitigating key pathophysiological processes underlying ACS, such as platelet activation, inflammation, and plaque instability. Furthermore, ongoing clinical trials are evaluating the efficacy and safety of these agents in ACS patients, with potential implications for optimizing ACS management. Challenges associated with translating preclinical findings into clinical practice, including patient heterogeneity and trial design considerations, are also discussed. Overall, the exploration of emerging therapeutic targets offers promising avenues for advancing ACS treatment strategies and improving patient outcomes.

Abstract:Ischemic heart disease represents a significant global burden of morbidity and mortality. While revascularization strategies are well defined in acute settings, there are uncertainties regarding chronic coronary artery disease treatment. Recent trials have raised doubts about the necessity of revascularization for "stable", chronic coronary syndromes or disease, leading to a shift towards a more conservative approach. However, the issue remains far from settled. In this narrative review, we offer a summary of the most pertinent evidence regarding revascularization for chronic coronary disease, while reflecting on less-often-discussed details of major clinical trials. The cumulative evidence available indicates that there can be a prognostic benefit from revascularization in chronic coronary syndrome patients, provided there is significant ischemia, as demonstrated by either imaging or coronary physiology. Trials that have effectively met this criterion consistently demonstrate a reduction in rates of spontaneous myocardial infarction, which holds both prognostic and clinical significance. The prognostic benefit of revascularization in patients with heart failure with reduced ejection fraction remains especially problematic, with a single contemporary trial favouring surgical revascularization. The very recent publication of a trial focused on revascularizing non-flow-limiting "vulnerable" plaques adds further complexity to the field. The ongoing debates surrounding revascularization in chronic coronary syndromes emphasize the importance of personalized strategies. Revascularization, added to the foundational pillar of medical therapy, should be considered, taking into account symptoms, patient preferences, coronary anatomy and physiology, ischemia tests and intra-coronary imaging.

Abstract:The best treatment for patients with ischemic heart failure (HF) is still on debate. There is growing evidence that coronary artery bypass graft (CABG) benefits these patients. The current recommendations for revascularization in this context are that CABG is reasonable when it comes to decreasing morbidity and mortality rates for patients with severe left ventricular dysfunction (ejection fraction <35%), and significant coronary artery disease (CAD) and should be considered in patients with operable coronary anatomy, regardless whether or not there is a viable myocardium (class IIb). Percutaneous coronary intervention (PCI) does not have enough data to allow the panels to reach a conclusion. The Korean Acute Heart Failure registry (KorAHF) had its data released recently, showing that patients with acute HF who underwent CABG had lower death rates, more complete revascularization and less adverse outcomes compared with patients treated with PCI. Recent ESC/EACTS guidelines on myocardial revascularization clearly recommended CABG as the first choice of revascularization strategy in patients with multivessel disease and acceptable surgical risk to improve prognosis in this scenario of left ventricular dysfunction. However, a high peri-procedural risk must be compared with the benefit of late mortality, and pros and cons of each strategy (either PCI or CABG) must be weighed in the decision-making process. Spurred on by the publication of the above-mentioned article and the release of new guidelines, we went on to write an overview of the current practice of state-of-the-art coronary revascularization options in patients with HF.

The many roles of urgent catheter interventions: from myocardial infarction to acute stroke and pulmonary embolism

Abstract:Introduction: Cardiovascular diseases (CVDs) are the leading cause of cardiovascular mortality and a major contributor to disability worldwide. The prevalence of CVDs is continuously increasing, and from 1990 to 2019, it has doubled. Global cardiovascular mortality has increased from 12.1 million in 1990 to 18.6 million cases in 2019. The development of therapeutic options for these diseases is at the forefront of interest concerning the extensive socio-economic consequences. Modern endovascular transcatheter therapeutic options contribute to the reduction of cardiovascular morbidity and mortality. Areas covered: The article concentrates on the triad of the most common causes of acute cardiovascular mortality and morbidity - myocardial infarction, ischemic stroke, and pulmonary embolism. Current evidence-based indications, specific interventional techniques, and remaining unsolved issues are reviewed and compared. A personal perspective on the possible implications for the future is provided. Expert opinion: Primary angioplasty for ST-segment elevation myocardial infarction is a well-established therapeutic option with proven mortality benefits. We suppose that catheter-based interventions for acute stroke will spread quickly from centers of excellence to routine clinical practice. We believe that ongoing research will provide a basis for the expansion of interventional treatment of pulmonary embolism soon.

Abstract:Advances in medical and surgical interventions have resulted in a steady increase in the number of patients with congenital heart disease (CHD) reaching adult age. Unfortunately, this ever-growing population faces an added challenge: an increased risk of acquiring coronary artery disease. This review provides insight into the complex interactions between coronary artery disease and CHD in adults. We describe the peculiar features of cardiac anatomy in these patients, the possible role cardiac sequelae may play in an increased risk of myocardial ischemia, and the diagnostic challenges in this patient group. Furthermore, this review outlines the risk factors and potential mechanisms of accelerated atherosclerosis in adults with CHD by pointing out areas where current knowledge is incomplete and highlighting areas for further research. The review concludes by examining potential management strategies for this particular population, emphasizing the necessity for a multidisciplinary approach. Understanding the unique coronary risks that adults with CHD experience can enhance patient care and improve long-term results.
